341 Mechanical Ventilation: Relationship between Body Mass Index and Selected Patients' Outcomes at a University Hospital in Cairo

Abstract:
Background: The mechanically ventilated patients need a special nursing care with continuous closed observation. The patients’ body mass index may affect their prognosis or outcomes. Aim of the study: to investigate the relationship between BMI and selected outcomes of critically ill mechanically ventilated patients. Research Design: A descriptive correlational research design was utilized Research questions: a) what is the BMI profile of mechanically ventilated patients admitted to critical care units over a period of six months? b) What is the relationship between body mass index and frequency of organ dysfunction, length of ICU stay, weaning from mechanical ventilation, and the mortality rate among adult critically ill mechanically ventilated patients? Setting: different intensive care units of Cairo University Hospitals. Sample: A convenience sample of 30 mechanically ventilated patients for at least 72 hours. Tools of data collection: Three tools were utilized to collect data pertinent to the current study: tool 1: patients’ sociodemographic and medical data sheet, tool 2: BURNS Wean Assessment Program (BWAP) checklist, tool 3: Sequential organ failure assessment (SOFA score) sheet. Results: The majority of the studied sample (77%) was males, and (26.7 %) of the studied sample were in the age group of 18-28 years old, and (26.7 %) were in the age group of 40-50 years old. Moreover, two thirds (66.7%) of the studied sample were within normal BMI. No significant statistical relationship between BMI category and ICU length of stay or the mortality rate among the studied sample, (X² = 11.31, P value = 0.79), (X² = 0.15, P value = 0.928) respectively. No significant statistical relationship between BMI category and the weaning trials from mechanical ventilation among the studied sample, (X² = 0.15, P value = 0.928). No significant statistical relationship was found between BMI category and the occurrence of organ dysfunction among the studied sample, (X² = 2.54, P value = 0.637). Conclusion: No relationship between the BMI categories and the selected patients’ outcomes (weaning from MV, length of ICU stay, occurrence of organ dysfunction, mortality rate). Recommendations: Replication of this study on a larger sample from different geographical locations in Arab Republic of Egypt, conducting farther studies to assess the effect of the quality of nursing care on the mechanically ventilated patients’ outcomes.Keywords: mechanical ventilation, body mass index, outcomes of mechanically ventilated patient, organ failure

Abstract:
During the Ba'ath Party's rule in Iraq, propaganda was utilized to justify and to promote Saddam Hussein's image in the collective memory as the greatest Arab leader. Consequently, urban walls were routinely covered with images of Saddam. Relying on these images, the regime aimed to provide a basis for evoking meanings in the public opinion, which would supposedly strengthen Saddam’s power and reconstruct facts to legitimize his political ideology. Nonetheless, Saddam was not always portrayed with common and explicit elements but in certain periods of his rule, the paintings depicted him in an unusual context, where various historical and contemporary elements were combined in a narrative background. Therefore, an understanding of the implied socio-political references of these elements is required to fully elucidate the impact of these images on forming the memory and collective unconscious of the Iraqi people. To obtain such understanding, one needs to address the following questions: a) How Saddam Hussein is portrayed in mural during his rule? b) What of elements and mythical-historical narratives are found in the paintings? c) Which Saddam's political views were subject to the collective memory through mural? Employing visual semiotics, this study reveals that during Saddam Hussein's regime, the paintings were initially simple portraits but gradually transformed into narrative images, characterized by a complex network of historical, mythical and religious elements. These elements demonstrate the transformation of a secular-nationalist politician into a Muslim ruler who tried to instill three major policies in domestic and international relations i.e. the arabization of Iraq, as well as the propagation of pan-arabism ideology (first period), the implementation of anti-Israel policy (second period) and the implementation of anti-American-British policy (last period).Keywords: Ba'ath Party, Saddam Hussein, mural, Iraq, propaganda, collective memory

Abstract:
Unmanned Aircraft Systems (UAS) are playing increasingly prominent roles in defense programs and defense strategies around the world. Technology advancements have enabled the development of it to do many excellent jobs as reconnaissance, surveillance, battle fighters, and communications relays. Simulating a small unmanned aerial vehicle (SUAV) dynamics and analyzing its behavior at the preflight stage is too important and more efficient. The first step in the UAV design is the mathematical modeling of the nonlinear equations of motion. In this paper, a survey with a standard method to obtain the full non-linear equations of motion is utilized,and then the linearization of the equations according to a steady state flight condition (trimming) is derived. This modeling technique is applied to an Ultrastick-25e fixed wing UAV to obtain the valued linear longitudinal and lateral models. At the end, the model is checked by matching between the behavior of the states of the non-linear UAV and the resulted linear model with doublet at the control surfaces.Keywords: UAV, equations of motion, modeling, linearization

Abstract:
In this paper, the finite element analysis is applied to study the performance of the bonded composite reinforcement or repair for reducing stress concentration at a semi-circular lateral notch and repairing cracks emanating from this kind of notch. The effects of the adhesive properties on the variation of the stress intensity factor at the crack tip were highlighted. The obtained results show that the stress concentration factor at the notch tip is reduced about 30% and the maximal reduction of the stress intensity factor is about 80%. The adhesive properties must be optimized in order to increase the performance of the patch repair or reinforcement.Keywords: bonded repair, notch, crack, adhesive, composite

Abstract:
In this study the Finite element method is used to analyse the effect of the thermal residual stresses resulting from adhesive curing on the performances of the bonded composite repair in aircraft structures. The stress intensity factor at the crack tip is chosen as fracture criterion in order to estimate the repair performances. The obtained results show that the presence of the thermal residual stresses reduces considerably the repair performances and consequently decreases the fatigue life of cracked structures. The effects of the curing temperature, the adhesive properties and the adhesive thickness on the Stress Intensity Factor (SIF) variation with thermal stresses are also analysed.Keywords: bonded composite repair, residual stress, adhesion, stress transfer, finite element analysis

Abstract:
The word “climate change” has become the most popular term when anyone observes any uncertain climate variation in their respective region. Asian countries are more prone to the impact of this phenomenon, and Pakistan is the leading affected country. Last few years, governments all over the world have been trying to cater to this issue for the best entrust of their population, especially agriculture. Now the farmers in Pakistan are fully aware of the term “climate change” and are more concerned about its solutions. On the other hand, developed countries like the USA are setting a benchmark for developing countries in every sphere of life. Based on cultural and other variations, the research was carried out to identify the behavior of farmers regarding the same issue. Cross-sectional survey research was designed for an in-depth study of relevant research questions. Face-to-face interviews were conducted in Pakistan, while virtual and face-to-face interviews were conducted in the Indiana State of the USA. The results of the present study and the responses of farmers were very interesting. The common climate change mitigation strategies suggested by farmers of both countries were less use of motor vehicles (replacement with bicycles in the circle of 10 Km), less dependency on chemical fertilizers (increased use of Manure, Bio-fertilizer, Compost), and plantation of the tree. The difference of opinion was in less government interest, lack of farmers’ education, political instability (views of Pakistani farmers), awareness of local communities, self-satisfaction, and economic disparities (views of USA farmers). Based on the given evidence, it was recommended that there is a dire need to address the climate change issue all over the world without discrimination of race, color, region, or religion. Abstract:
Helical piles are widely used as axially and laterally loaded deep foundations. Once they are required to resist bearing combined loads (BCLs), as axial compression and lateral thrust, different behaviour is expected, necessitating further investigation. The objective of the present article is to clarify the behaviour of a single helical pile of different shaft rigidity embedded in cohesion-less soil and subjected to simultaneous or successive loading patterns of BCLs. The study was first developed analytically and extended numerically. The numerical analysis was further verified through a laboratory experimental program on a set of helical pile models. The results indicate highly interactive effects of the studied parameters, but it is obviously confirmed that the pile performance increases with both the increase of shaft rigidity and the change of BCLs loading pattern from simultaneous to successive. However, it is noted that the increase of vertical load does not always enhance the lateral capacity but may cause a decrement in lateral capacity, as observed with helical piles of flexible shafts. Patients suffering from gait disturbances are referred by having muscle group dysfunctions. There is a need for more studies investigating the contribution of muscle moments of the lower limb to the vertical ground reaction force using 3D gait analysis system. The purpose of this study was to investigate how the hip, knee and ankle moments in the sagittal plane contribute to the vertical ground reaction force in healthy subjects during normal speed of walking. Forty healthy male individuals volunteered to participate in this study. They were filmed using six high speed (120 Hz) Pro-Reflex Infrared cameras (Qualisys) while walking on an AMTI force platform. The data collected were the percentage contribution of the moments of the hip, knee and ankle joints in the sagittal plane at the instant of occurrence of the first peak, second peak, and the trough of the vertical ground reaction force. The results revealed that at the first peak of the ground reaction force (loading response), the highest contribution was generated from the knee extension moment, followed by the hip extension moment. Knee flexion and ankle plantar flexion moments produced high contribution to the trough of the ground reaction force (midstance) with approximately equal values. The second peak of the ground reaction force was mainly produced by the ankle plantar flexion moment. Conclusion: Hip and knee flexion and extension moments and ankle plantar flexion moment play important roles in the supporting phase of normal walking.Keywords: gait analysis, ground reaction force, moment contribution, normal walking

Data consisting of 2757 records from tow Holstein herds made between 2000 and 2010 were used to examine environmental factors affecting age at first calving (AFC) and calving intervals (CI) and consequently estimate genetic and phenotypic parameters and trends. The overall means and standard errors for AFC and CI were 39.4 ± 7.2 months and 487.5 ± 151.6 days respectively. The respective heritability estimates were 0.091 ± 0.05 and 0.044 ± 0.032, while the repeatability estimate for CI was 0.096 ± 0.001. The genetic trends for CI and AFC were -0.6 d/yr and -0.01 mo/yr respectively and were both significant (P < 0.001), indicating a decrease in mean breeding value over the study period. Phenotypic trends were -0.31 mo/yr and -0.35 d/yr for AFC and CI respectively though non-significant (P > 0.05). The low heritability for CI and AFC indicated that temporary environmental influences were much greater than genetic influences or permanent environmental influences on these traits.Keywords: Holstein, reproductive, genetic parameters, heritability

It is important to formulate suitable learning environments ca-pable to be customized according to value perceptions of the university. In this paper, light is shed on the concepts of integration between knowledge management (KM), and e-learning (EL) in the higher education sector of the economy in Abu Dhabi Emirate, United Arab Emirates (UAE). A discussion on and how KM and EL can be integrated and leveraged for effective education and training is presented. The results are derived from the literature and interviews with 16 of the academics in eight universities in the Emirate. The conclusion is that KM and EL have much to offer each other, but this is not yet reflected at the implementation level, and their boundaries are not always clear. So this study tries to understand the effect of question type on responses in Social Q & A sites. The study found that among the answered queries, majority of them were answered within 24 hours of posting the questions and surprisingly most replies were received within one hour of posting. In Ultra high-field MRI scanners (3T and higher), parallel RF transmission techniques using multiple RF chains with multiple transmit elements are a promising approach to overcome the high-field MRI challenges in terms of inhomogeneity in the RF magnetic field and SAR. However, mutual coupling between the transmit array elements disturbs the desirable independent control of the RF waveforms for each element. Vision, the basis for performing navigational tasks, is absent or greatly reduced in visually impaired people due to which they face many hurdles. For increasing the navigational capabilities of visually impaired people a desktop application ODAPTA is presented in this paper. The application uses camera to capture video from surroundings, apply various image processing algorithms to get information about path and obstacles, tracks them and delivers that information to user through voice commands. In this paper, a new computationally-efficient rectangular flat shell finite element named 'ACM_RSBEC' is presented. The formulated element is obtained by superposition of a new rectangular membrane element 'RSBEC' based on the strain approach and the well known plate bending element 'ACM'. This element can be used for the analysis of thin shell structures, no matter how the geometrical shape might be. Tests on standard problems have been examined. The convergence of the new formulated element is also compared to other types of quadrilateral shell elements. Polymer nano composites of polyvinylpyrrolidone and poly-ethylene glycol with different concentrations of aluminum oxide (Al2O3) nano particles have been prepared by solution cast method. The optical characterizations have been done by analyzing the absorption (A) spectra in the 300–800 nm spectral region. It was found that the optical energy gap decreases with the increasing of Al2O3 nano particles content. In this paper, we consider singular perturbation reaction-diffusion boundary value problems, which contain a small uncertain perturbation parameter. To solve these problems, we propose a numerical method which is based on an exponential spline and Shishkin mesh discretization. While interval analysis principle is used to deal with the uncertain parameter, sensitivity analysis has been conducted using different methods. An experiment al investigation is made to determine the effects of the nozzle dimensions and the inlet pressure on the heating and cooling performance of the counter flow Ranque–Hilsch vortex tube when air used as a working fluid. The all results were taking under inlet pressures were adjusted from 200 kPa to 600 kPa with 100 kPa increments. The conventional tangential generator with number of nuzzle of 6 was used and inner diameter of 7.5 mm. During the experiments, a vortex tube is used with an L/D ratio varied from 10 to 30. Fifty-five core marine sediments from three locations at South China Sea and one location each at Sulu Sea and Sulawesi Sea of coastal East Malaysia was analyzed for heavy metals using Instrumental Neutron Activation Analysis and Inductively Coupled Plasma Mass Spectroscopy. The enrichment factor of As, Cd, Cr, Cu, Ni, Pb, and Zn varied from 0.42 to 4.26, 0.50 to 2.34, 0.31 to 0.82, 0.20 to 0.61, 0.91 to 1.92, 0.23 to 1.52, and 0.90 to 1.28 respectively, with the modified degree of contamination values below 0.6. The paper focuses on the prevalence of informational efficiency and integration of GCC Shariah Equity market for the period of 01st January 2010 to 31st June 2015 with daily equity returns of Kuwait, Oman, Qatar, Bahrain, Saudi Arabia and United Arab Emirates. The study employs traditional as well as the modern approach of tracing out the efficiency and integration in the return series. From the results of efficiency it was observed that the market lacked efficiency in terms of its past information. The results of integration test clearly indicates that there was a long memory in the returns of GCC Shariah during the study period. Abstract:
A compact ultra-wide band micro-strip MIMO antenna is introduced. The antenna consists of two elements each of size 24X24 mm2 (square millimetre) while the total MIMO size is 58X24 mm2 after the spacing between MIMO elements and adding a decouple circuit. The first one covers from 3.29 to 6.9 GHZ using digital ground and the second antenna covers from 8.76 to 13.27 GHZ using defective ground. This type of antenna is used for 3G and 4G applications. Abstract:
China launched satellite GF-2 in 2014. This study deals with comparing nearest neighbor object-based classification and neural network classification methods for classification of the fused GF-2 image. Firstly, rectification of GF-2 image was performed. Secondly, a comparison between nearest neighbor object-based classification and neural network classification for classification of fused GF-2 was performed. Thirdly, the overall accuracy of classification and kappa index were calculated. Abstract:
Many formulas are set to estimate the pile load capacity based on the in-situ pile load tests. However, discrepancy is obvious between the results of these applications. Some formulas are more adequate than others with respect to soil formation and its characteristics. In this research, attempts were undertaken to evaluate the adequacy of the most well-known formulas based on a series of pile load tests carried out in Port Said city in the northeast of Egypt for major residential projects. Comparisons were undertaken between the different formulas supported by the results of in-situ Cone Penetration Tests (CPT). Based on this study, a guide for engineers for using the proper formula can be adopted with consideration of soil type and characteristics. The Egyptian Code which relies on the results of some formulas is involved in the study as a guiding aspect in the pile design.Keywords: pile load formula, load test of piles, CPT, Egyptian code

In this era of globalization, adoption of technology is quite difficult for people with physical disabilities compared to people with normal abilities. The advancement in mobile based accessible applications has opened up several different avenues for the visually challenged across the globe. Smartphones applications are not very common for blind people, but they access and use these applications in their daily lives to some extent. Several smartphone applications have a number of usability issues for the visually impaired. In this paper, we evaluate the usability of various android and iPhone applications for blind people through analysis and surveys. This paper aspires to provide guidance in order to increase smartphone application accessibility for the visually impaired. Abstract:
In this paper full-scale brick masonry room along with the veranda of a typical school building was subjected to eight successive blast tests with increasing charge weights ranging from 0.5kg to 16.02kg at 3.66m fixed stand-off distance. Pressure-time histories were obtained by data acquisition system from pressure sensors, installed on different points of room as well as veranda columns. The resulting damage pattern of different locations was observed during each test. Weak zones of masonry room were identified. Scaled distances for different damage levels in masonry room were experimentally obtained. The results provided a basis for determining the response of masonry room building against blast loading in a specific threat scenario.Keywords: peak pressure, composition-B, TNT, pressure sensor, scaled distance, masonry
