Authors: Aisha Maroof
Abstract:
Introduction: Coronavirus Disease 2019 (COVID-19) is spreading rapidly around the world with devastating consequences on patients, health care workers and health systems. Severe 2019 novel coronavirus infectious disease (COVID-19) with pneumonia is associated with high rates of admission to the intensive care unit (ICU) and they are at high risk to obtain the hospital acquire bloodstream infection (HAIs) such as central line associated bloodstream infection (CLABSI), catheter associated urinary tract infections (CAUTI) and laboratory confirm bloodstream infection (LCBSI). The chances of infection transmission increase when healthcare worker’s (HCWs) practice is inappropriate. Risk related to hand hygiene (HH) and personal protective equipment (PPE) as regards multidrug-resistant organism transmission: use of multiple gloving instead of HH and incorrect use of PPE can lead to a significant increase of device-related infections. As it reaches low- and middle-income countries, its effects could be even more, because it will be difficult for them to react aggressively to the pandemic. HAIs are one of the biggest medical concerns, resulting in increased mortality rates. Objective: To assess the effect of intervention on compliance of hand hygiene and PPE among HCWs reduce the rate of HAI in COVID-19 patients. Method: An interventional study was done between July to December, 2020. CLABSI, CAUTI and LCBSI data were collected from the medical record and direct observation. There were total of 50 Nurses, 18 doctors and all patients with laboratory-confirmed severe COVID-19 admitted to the hospital were included in this research study. Respiratory tract specimens were obtained after the first 48 h of ICU admission. Practices were observed after and before intervention. Education was provided based on WHO guidelines. Results: During the six months of study July to December, the rate of CLABSI, CAUTI and LCBSI pre and post intervention was reported. CLABSI rate decreasedd from 22.7 to 0, CAUTI rate was decreased from 1.6 to 0, LCBSI declined from 3.3 to 0 after implementation of intervention. Conclusion: HAIs are an important cause of morbidity and mortality. Most of the device related infections occurs due to lack of correct use of PPE and hand hygiene compliance. Hand hygiene and PPE is the most important measure to protect patients, through education it can be improved the correct use of PPE and hand hygiene compliance and can reduce the bacterial infection in COVID-19 patients. Keywords: hospital acquire infection, healthcare workers, hand hygiene, personal protective equipment

Authors: Man Wai Leung, Margaret O’Donoghue, Lorna K. P. Suen
Abstract:
Personal and household hygiene measures are important to prevent upper respiratory tract infections (URTIs) and other infectious diseases, including coronavirus disease 2019 (COVID-19). An online survey recruited 414 eligible parents in Hong Kong to study their hygiene knowledge, attitudes, and practices (KAP) in the prevention of URTIs among their children. The average knowledge score was high (10.2/12.0), but some misconceptions were identified. The majority of participants agreed that good personal hygiene (93.5%) and good environmental hygiene (92.8%) can prevent URTIs. The average score for hand hygiene practices was high (3.78/4.00), but only 56.8% of parents always perform hand hygiene before touching their mouth, nose, or eyes. For environmental hygiene, only some household items were disinfected with disinfectants (69.8%: door handles, 60.4%: toilet seats, 42.8%: floor, 24.2%: dining chairs, 20.5%: dining tables). Higher knowledge score was associated with parents having a tertiary educational level or above, working as healthcare professionals, living at private residential flat or staff quarter, and having a household income of $70,000 or above. Hand hygiene practices varied significantly with parents’ age and income. During the 5th wave of the COVID-19 epidemic, misconceptions about hygiene knowledge were found among parents. Health promotion programs should target parents, especially those who are in old age, obtain lower educational levels, live in public housing, or have a lower income. Hand hygiene moments and proper use of disinfectants could be one of the targeted educational topics. Keywords: hygiene, upper respiratory tract infection, parents, children, COVID-19

Authors: Awad Hassoup, Sayed Abdallah, Mohamed Dahy
Abstract:
Northern Egypt is known to be seismically active from the past several thousand years, based on the historical records and documents of eyewitnesses on one- hand and instrumental records on the other hand. Instrumental, historical and pre- historical seismicity data indicate that large destructive earthquakes have occurred quite frequently in the investigated area. The interaction of the African, Arabian, Eurasian plates and Sinai sub-plate is the main factor behind the seismicity of northern part of Egypt. All earthquakes occur at shallow depth and are concentrated at four seismic zones, these zones including the Gulfs of Suez and Aqaba, around the entrance of the Gulf of Suez and the fourth one is located at the south- west of great Cairo (Dahshour area). The seismicity map of the previous zones shows that the activity is coincide with the major tectonic trends of the Suez rift, Aqaba rift with their connection with the great rift system of the Red Sea and Gulf of Suez- Cairo- Alexandria trend. On the other hand, the focal mechanisms of some earthquakes occurred inside the studied area and having small to moderate size show a variety of patterns. The most predominant type is normal faulting. Keywords: Northern Egypt, seismic active zone, seismicity, focal mechanism

Authors: Benny Alexander, Ikhlashia N. Fadhilah, Muhammad R. Pasha, Michelle Julia, Anne Z. Syahrial
Abstract:
Brake shoe is a component that serves to reduce speed or stop the train's speed by utilizing the friction force. Generally, the material used as a brake shoe is cast iron, where cast iron itself is a heavy, expensive, and easily worn material. Aluminum matrix composites are one of candidates for the cast iron replacement material as the basic material for brake shoe. The matrix in the composite used is Aluminum AC4B. Reinforcement used in aluminum matrix composites is nano-alumina, where the use of nano-alumina of 0.25%, 0.3%, 0.35%, 0.4%, and 0.5% volume fraction will be tested. The sample is made using the stir casting method; then, it will be tested mechanically. The use of nano-alumina as a reinforcement will increase the strength of the matrix. SEM (scanning electron microscopy) testing is used to test the distribution of reinforcing particles due to stirring. Therefore, the addition of nano-alumina will improve AC4B aluminum matrix composites. Keywords: aluminium matrix composites, brake shoe application, stir casting, nano-alumina

Abstract:
The renewable resource character of NTFPs is an opportunity to its sustainability, this study analyzed the role of social capital in the adoption of sustainable management practices of NTFPs by households in the community forest (CF) Morikouali-ye. The analysis shows that 67% of households surveyed perceive the level of degradation of NTFPs in their CF as time passes and are close to 74% for adoption of sustainable management practices of NTFPs that are domestication, sustainable management of the CF, the logging ban trees and uprooting plants, etc. 26% refused to adopt these practices estimate that, at 39% it is better to promote logging in the CF. The estimated probit model shows that social capital through trust, solidarity and social inclusion significantly influences the probability of households to adopt sustainable NTFP management practices. In addition, age, education level and income from the sale of NTFPs have a significant impact on the probability of adoption. The probability of adoption increases with the level of education and confidence among households. So should they be animated by a spirit of solidarity and trust and not let a game of competition for sustainable management of NTFPs in their CF.Keywords: community forest, social capital, NTFP, trust, solidarity, social inclusion, sustainable management
Procedia PDF Downloads 3703694 Evaluation of Cooperative Hand Movement Capacity in Stroke Patients Using the Cooperative Activity Stroke Assessment
Authors: F. A. Thomas, M. Schrafl-Altermatt, R. Treier, S. Kaufmann
Abstract:
Stroke is the main cause of adult disability. Especially upper limb function is affected in most patients. Recently, cooperative hand movements have been shown to be a promising type of upper limb training in stroke rehabilitation. In these movements, which are frequently found in activities of daily living (e.g. opening a bottle, winding up a blind), the force of one upper limb has to be equally counteracted by the other limb to successfully accomplish a task. The use of standardized and reliable clinical assessments is essential to evaluate the efficacy of therapy and the functional outcome of a patient. Many assessments for upper limb function or impairment are available. However, the evaluation of cooperative hand movement tasks are rarely included in those. Thus, the aim of this study was (i) to develop a novel clinical assessment (CASA - Cooperative Activity Stroke Assessment) for the evaluation of patients’ capacity to perform cooperative hand movements and (ii) to test its inter- and interrater reliability. Furthermore, CASA scores were compared to current gold standard assessments for upper extremity in stroke patients (i.e. Fugl-Meyer Assessment, Box & Blocks Test). The CASA consists of five cooperative activities of daily living including (1) opening a jar, (2) opening a bottle, (3) open and closing of a zip, (4) unscrew a nut and (5) opening a clipbox. Here, the goal is to accomplish the tasks as fast as possible. In addition to the quantitative rating (i.e. time) which is converted to a 7-point scale, also the quality of the movement is rated in a 4-point scale. To test the reliability of CASA, fifteen stroke subjects were tested within a week twice by the same two raters. Intra-and interrater reliability was calculated using the intraclass correlation coefficient (ICC) for total CASA score and single items. Furthermore, Pearson-correlation was used to compare the CASA scores to the scores of Fugl-Meyer upper limb assessment and the box and blocks test, which were assessed in every patient additionally to the CASA. ICC scores of the total CASA score indicated an excellent- and single items established a good to excellent inter- and interrater reliability. Furthermore, the CASA score was significantly correlated to the Fugl-Meyer and Box & Blocks score. The CASA provides a reliable assessment for cooperative hand movements which are crucial for many activities of daily living. Due to its non-costly setup, easy and fast implementation, we suggest it to be well suitable for clinical application. In conclusion, the CASA is a useful tool in assessing the functional status and therapy related recovery in cooperative hand movement capacity in stroke patients. Keywords: activitites of daily living, clinical assessment, cooperative hand movements, reliability, stroke
Procedia PDF Downloads 3193693 Influence of Resin Finishes on Properties of Khadi Fabric
Authors: Shivi Rastogi, Suman Pant
Abstract:
Khadi is an Indian fabric and also known by another name “Khaddar”. During pre-independence era, the movement of khadi manufacturing gained momentum. Over the years, khadi fabrics that were generally considered as the “second skin” of the Swadesh revolutionists changed its uniqueness. It underwent a metamorphosis from that of a patriot’s fabric, and a farmer’s apparel, to become a “fashion fabric”. Drape of garment is governed by draping quality of fabric used. Drape is an essential parameter to decide both appearance and handle of fabric. It is also a secondary determinant of fabric mechanical properties as influenced by the low stress properties, like bending length, formability, tensile and shear properties and compressibility of the fabric. In finishing, fabric is treated to add something to coat the fabric or fiber and thereby temporarily or permanently fix. Film forming agents such as thermoplastic and thermosetting resins and other surface deposits alter hand. In this study, resins were used to modify fabric hand. Three types of resins have been applied on the khadi fabric at three concentration. The effect of these finishes on drapeability, crease recovery, stiffness, tearing strength and smoothness of khadi fabrics were assessed. Silicone gave good results in imparting properties specially drape, smoothness and softness and hand of cotton and khadi fabric. KES result also showed that silicone treated samples enhanced THV rating amongst all treated samples when compared to the control fabric. Keywords: crease recovery, drapeability, KES, silicone, THV

Procedia PDF Downloads 4123688 Case Study on the Effects of Early Mobilization in the Post-Surgical Recovery of Athletes with Open Triangular Fibrocartilage Complex Repair
Authors: Blair Arthur Agero Jr., Lucia Garcia Heras
Abstract:
The triangular fibrocartilage complex (TFCC) is one of the crucial stabilizing ligaments of the wrist. The TFCC is also subject to excessive stress amongst performance athletes and enthusiasts. The excessive loading of the TFCC may lead to a partial or complete rupture that requires surgery. The recovery from an open TFCC surgical repair may take several months. Immobilization of the repaired wrist for a given period is part of all the current protocols in the post-surgical treatment. The immobilization to prevent the rotation of the forearm can last from six weeks to eight weeks with the wrist held in a neutral position. In all protocols reviewed, the pronosupination is only initiated between the 6th week and 8th week or even later after the cast is removed. The prolonged immobilization can cause stiffness of the wrist and hand. Furthermore, the entire period of post-surgical hand therapy has its economic impact, especially for performing athletes. However, delayed mobilization, specifically rotation of the wrist, is necessary to allow ligament healing. This study aims to report the effects of early mobilization of the wrist in athletes who had an open surgical repair of the TFCC. The surgery was done by the co-author, and the hand therapy was implemented by the main author. The cases documented spans from 2014 to 2019 and were all performed in Dubai, United Arab Emirates. All selected participants in this case study were provided with a follow-up questionnaire to ascertain their current condition since their surgery. The respondents reported high satisfaction in the results of their treatment and have verified zero re-rupture of their TFCC despite mobilizing and rotating the wrist at the third-week post-surgery during their hand therapy. There is also a negligible number of respondents who reported a limitation in their ranges of pronosupination. This case study suggests that early mobilization of the wrist after an open TFCC surgical repair can be more beneficial to the patient as opposed to the traditional treatment of prolonged immobilization. However, it should be considered that the patients selected in this case study are professional performance athletes and advanced fitness enthusiasts. Athletes are known to withstand vigorous physical stress in their training that may correlate to their ability to better cope with the progressive stress that was implemented during their hand therapy. Nevertheless, this approach has its merits, and application of it may be adjusted for patients with a similar injury and surgical procedure. Keywords: hand therapy, performance athlete, TFCC repair, wrist ligament

Authors: Qian Liu, Steve Furber
Abstract:
To explore how the brain may recognize objects in its general, accurate and energy-efficient manner, this paper proposes the use of a neuromorphic hardware system formed from a Dynamic Video Sensor~(DVS) silicon retina in concert with the SpiNNaker real-time Spiking Neural Network~(SNN) simulator. As a first step in the exploration on this platform a recognition system for dynamic hand postures is developed, enabling the study of the methods used in the visual pathways of the brain. Inspired by the behaviours of the primary visual cortex, Convolutional Neural Networks (CNNs) are modeled using both linear perceptrons and spiking Leaky Integrate-and-Fire (LIF) neurons. In this study's largest configuration using these approaches, a network of 74,210 neurons and 15,216,512 synapses is created and operated in real-time using 290 SpiNNaker processor cores in parallel and with 93.0% accuracy. A smaller network using only 1/10th of the resources is also created, again operating in real-time, and it is able to recognize the postures with an accuracy of around 86.4% -only 6.6% lower than the much larger system. The recognition rate of the smaller network developed on this neuromorphic system is sufficient for a successful hand posture recognition system, and demonstrates a much-improved cost to performance trade-off in its approach. Keywords: spiking neural network (SNN), convolutional neural network (CNN), posture recognition, neuromorphic system

Authors: Rose-Juliet Anyanwu
Abstract:
The principal objective of this paper is to investigate whether some sort of phonological processes, such as elision, aspiration, glottalisation, and hardening can be used to account for the behaviour of the glottal fricative (or approximant, as the case may be) ‘h’ in both French and Yoruba. French and Yoruba speakers generally tend to say, for instance ‘ockey’ and ‘amburger’, instead of ‘hockey’ and ‘hamburger’, respectively. Whereas the Yoruba conversely say, for instance ‘hadd’ for ‘add’, ‘heat’ for ‘eat’ on the one hand and ‘ard’ for ‘hard’, ‘eat’ for ‘heat’ on the other hand, on a similar note, it is not quite clear whether the French, however, if not at least in rare instances, would tend to force themselves to pronounce (in any form whatsoever) the h-sound. Recorded sentences containing h-initial as well as vowel-initial words will be used for the investigation. The present paper is meant to contribute to work on aspiration, compensation, elision, and glottalisation, as well as hardening. Keywords: aspiration, compensation, glottalisation, hardening

Procedia PDF Downloads 3493684 Real-Time Finger Tracking: Evaluating YOLOv8 and MediaPipe for Enhanced HCI
Authors: Zahra Alipour, Amirreza Moheb Afzali
Abstract:
In the field of human-computer interaction (HCI), hand gestures play a crucial role in facilitating communication by expressing emotions and intentions. The precise tracking of the index finger and the estimation of joint positions are essential for developing effective gesture recognition systems. However, various challenges, such as anatomical variations, occlusions, and environmental influences, hinder optimal functionality. This study investigates the performance of the YOLOv8m model for hand detection using the EgoHands dataset, which comprises diverse hand gesture images captured in various environments. Over three training processes, the model demonstrated significant improvements in precision (from 88.8% to 96.1%) and recall (from 83.5% to 93.5%), achieving a mean average precision (mAP) of 97.3% at an IoU threshold of 0.7. We also compared YOLOv8m with MediaPipe and an integrated YOLOv8 + MediaPipe approach. The combined method outperformed the individual models, achieving an accuracy of 99% and a recall of 99%. These findings underscore the benefits of model integration in enhancing gesture recognition accuracy and localization for real-time applications. The results suggest promising avenues for future research in HCI, particularly in augmented reality and assistive technologies, where improved gesture recognition can significantly enhance user experience. Keywords: YOLOv8, mediapipe, finger tracking, joint estimation, human-computer interaction (HCI)

Authors: Önder M. Özdem
Abstract:
Producing and circulating films of professional standards have become technically easier with the development and widespread use of digital recording and distribution technologies. Additionally, film festivals on common platforms have rapidly increased in numbers and diversity. On the one hand, no-charge applications result in excessive submissions; thus, it complicates the evaluation and selection process. On the other hand, festival’s high submission fees may make the distribution of films with a limited budget very difficult. Inspired by the author’s engagement with the film industry as both a pre-jury member of an international film festival and an applicant to many festivals, this study discusses the causes and consequences of the increasing commercialization of film festivals. The author’s double identity, both as a jury and an applicant, provides a comparative perspective through which one can unfold the different dimensions and dynamics in the film production and distribution processes. Keywords: commercialization, film distribution, film festivals, film production

Authors: Mylena McCoggle, Shyra Wilson, Andrea Rivera, Rocio Alba-Flores
Abstract:
This work introduces the use of EMGs (electromyography) from muscle sensors to develop an Artificial Neural Network (ANN) for pattern recognition to control a small unmanned aerial vehicle. The objective of this endeavor exhibits interfacing drone applications beyond manual control directly. MyoWare Muscle sensor contains three EMG electrodes (dual and single type) used to collect signals from the posterior (extensor) and anterior (flexor) forearm and the bicep. Collection of raw voltages from each sensor were connected to an Arduino Uno and a data processing algorithm was developed with the purpose of interpreting the voltage signals given when performing flexing, resting, and motion of the arm. Each sensor collected eight values over a two-second period for the duration of one minute, per assessment. During each two-second interval, the movements were alternating between a resting reference class and an active motion class, resulting in controlling the motion of the drone with left and right movements. This paper further investigated adding up to three sensors to differentiate between hand gestures to control the principal motions of the drone (left, right, up, and land). The hand gestures chosen to execute these movements were: a resting position, a thumbs up, a hand swipe right motion, and a flexing position. The MATLAB software was utilized to collect, process, and analyze the signals from the sensors. The protocol (machine learning tool) was used to classify the hand gestures. To generate the input vector to the ANN, the mean, root means squared, and standard deviation was processed for every two-second interval of the hand gestures. The neuromuscular information was then trained using an artificial neural network with one hidden layer of 10 neurons to categorize the four targets, one for each hand gesture. Once the machine learning training was completed, the resulting network interpreted the processed inputs and returned the probabilities of each class. Based on the resultant probability of the application process, once an output was greater or equal to 80% of matching a specific target class, the drone would perform the motion expected. Afterward, each movement was sent from the computer to the drone through a Wi-Fi network connection. These procedures have been successfully tested and integrated into trial flights, where the drone has responded successfully in real-time to predefined command inputs with the machine learning algorithm through the MyoWare sensor interface. Authors: Lakhoua Najeh
Abstract:
Aims of the work: The aim of this paper is to improve the reliability and the availability of a Packer production line of cigarettes based on two methods: The SADT method (Structured Analysis Design Technique) and the FMECA approach (Failure Mode Effects and Critically Analysis). The first method enables us to describe the functionality of the Packer production line of cigarettes and the second method enables us to establish an FMECA analysis. Methods: The methodology adopted in order to contribute to the improvement of the reliability and the availability of a Packer production line of cigarettes has been proposed in this paper, and it is based on the use of Structured Analysis Design Technique (SADT) and Failure mode, effects, and criticality analysis (FMECA) methods. This methodology consists of using a diagnosis of the existing of all of the equipment of a production line of a factory in order to determine the most critical machine. In fact, we use, on the one hand, a functional analysis based on the SADT method of the production line and on the other hand, a diagnosis and classification of mechanical and electrical failures of the line production by their criticality analysis based on the FMECA approach. Results: Based on the methodology adopted in this paper, the results are the creation and the launch of a preventive maintenance plan. They contain the different elements of a Packer production line of cigarettes; the list of the intervention preventive activities and their period of realization. Conclusion: The diagnosis of the existing state helped us to found that the machine of cigarettes used in the Packer production line of cigarettes is the most critical machine in the factory. Then this enables us in the one hand, to describe the functionality of the production line of cigarettes by SADT method and on the other hand, to study the FMECA machine in order to improve the availability and the performance of this machine. Keywords: production system, diagnosis, SADT method, FMECA method
Procedia PDF Downloads 1423677 Experimental Study on Use of Crumb Rubber to Mitigate Expansive Soil Pressures on Basement Walls
Authors: Kwestan Salimi, Jenna Jacoby, Michelle Basham, Amy Cerato
Abstract:
The extreme annual weather patterns of the central United States have increased the need for underground shelters for protection from destructive tornadic activity. However, very few residential homes have basements due to the added construction expense and the prevalence of expansive soils covering the central portion of the United States. These expansive soils shrink and swell, increasing earth pressure on basement walls. To mitigate the effect of expansive soils on basement walls, this study performed bench-scale tests using a common natural expansive soil mitigated with a backfill layer of crumb rubber. The results revealed that at 80% soil compaction, a 1:6 backfill height to total height ratio produced a 66% reduction in swell pressure. However, this percent reduction decreased to 27% for 90% soil compaction. It was also found that there is a strong linear correlation between compaction percentage and reduction in swell pressure when using the same backfill height to total height ratio. Using this correlation and extrapolating to 95% compaction, the percent reduction in swell pressure was approximately 12%.Keywords: expansive soils, swell/shrink, swell pressure, stabilization, crumb rubber

Authors: Ehsan Atash Zaban, Mehdi Beyk
Abstract:
In recent years, concerns over the depletion of non-renewable fuels and environmental pollution have led countries around the world to look for alternative energy sources for these fuels. An energy source that can have the necessary reliability, be a suitable alternative to fossil fuels, be technologically achievable, comply with environmental standards to the maximum, and at the same time cause countries to meet domestic consumption for electricity production. Iran is one of the richest countries in the world in terms of various energy sources because, on the one hand, it has extensive sources of fossil and non-renewable fuels such as oil and gas, and on the other hand, it has great potential for renewable energy. In this paper, the potential of renewable energy in Iran, which includes solar, wind, geothermal, hydrogen technology, and biomass, has been reviewed and analyzed. Keywords: renewable energy, solar stations, wind, biomass, hydropower
Procedia PDF Downloads 903675 Gaze Patterns of Skilled and Unskilled Sight Readers Focusing on the Cognitive Processes Involved in Reading Key and Time Signatures
Authors: J. F. Viljoen, Catherine Foxcroft
Abstract:
Expert sight readers rely on their ability to recognize patterns in scores, their inner hearing and prediction skills in order to perform complex sight reading exercises. They also have the ability to observe deviations from expected patterns in musical scores. This increases the “Eye-hand span” (reading ahead of the point of playing) in order to process the elements in the score. The study aims to investigate the gaze patterns of expert and non-expert sight readers focusing on key and time signatures. 20 musicians were tasked with playing 12 sight reading examples composed for one hand and five examples composed for two hands to be performed on a piano keyboard. These examples were composed in different keys and time signatures and included accidentals and changes of time signature to test this theory. Results showed that the experts fixate more and for longer on key and time signatures as well as deviations in examples for two hands than the non-expert group. The inverse was true for the examples for one hand, where expert sight readers showed fewer and shorter fixations on key and time signatures as well as deviations. This seems to suggest that experts focus more on the key and time signatures as well as deviations in complex scores to facilitate sight reading. The examples written for one appeared to be too easy for the expert sight readers, compromising gaze patterns. Keywords: cognition, eye tracking, musical notation, sight reading

Authors: Shun-Qi Zhang, Shu-Yang Zhang, Min Chen
Abstract:
Applying strong electric field on piezoelectric actuators, on one hand very significant electroelastic material nonlinear effects will occur, on the other hand piezo plates and shells may undergo large displacements and rotations. In order to give a precise prediction of piezolaminated smart structures under large electric field, this paper develops a finite element (FE) model accounting for both electroelastic material nonlinearity and geometric nonlinearity with large rotations based on the first order shear deformation (FSOD) hypothesis. The proposed FE model is applied to analyze a piezolaminated semicircular shell structure. Keywords: smart structures, piezolamintes, material nonlinearity, strong electric field
Procedia PDF Downloads 4263673 Patient Perspectives on Telehealth During the Pandemic in the United States
Authors: Manal Sultan Alhussein, Xiang Michelle Liu
Abstract:
Telehealth is an advanced technology using digital information and telecommunication facilities that provide access to health services from a distance. It slows the transmission factor of COVID-19, especially for elderly patients and patients with chronic diseases during the pandemic. Therefore, understanding patient perspectives on telehealth services and the factors impacting their option of telehealth service will shed light on the measures that healthcare providers can take to improve the quality of telehealth services. This study aimed to evaluate perceptions of telehealth services among different patient groups and explore various aspects of telehealth utilization in the United States during the COVID-19 pandemic. An online survey distributed via social media platforms was used to collect research data. In addition to the descriptive statistics, both correlation and regression analyses were conducted to test research hypotheses. The empirical results highlighted that the factors such as accessibility to telehealth services and the type of specialty clinics that the patients required play important roles in the effectiveness of telehealth services they received. However, the results found that patients’ waiting time to receive telehealth services and their annual income did not significantly influence their desire to select receiving healthcare services via telehealth. The limitations of the study and future research directions are discussed. Keywords: telehealth, patient satisfaction, pandemic, healthcare, survey

Authors: M. Ridwanul Hassan, Samuel George
Abstract:
Aims: Steroid injections are commonly used as a diagnostic tool or an alternative to surgical management of carpal tunnel syndrome (CTS) and are generally safe. Ischaemia is a rare complication with very few cases reported in the literature. Methods: We report a case of a 50-year-old female that presented with a necrotic wound to her left palm one month after a steroid injection into the carpal tunnel. She had a 2-year history of CTS in her left hand that was treated with six previous steroid injections in primary care during this period. The wound evolved from a blister to a necrotic ulcer which led to a painful, hollow defect in the centre of her palm. She did not report any history of trauma, nor did she have any co-morbidities. Clinical photographs were taken. Results: On examination, she had a 0.5 cmx1 cm defect in the palm of her left hand down to aponeurosis. There was purulent discharge in the wound with surrounding erythema but no spreading cellulitis. She had full function of her fingers but was very tender on movements and at rest. She was admitted for intravenous antibiotics and underwent a debridement, washout, and carpal tunnel release the next day. The defect was packed to heal by secondary intention and has now fully healed one month following her operation. Conclusions: This is an extremely rare complication of steroid injections to the carpal tunnel and may have been avoided by earlier referral for surgery rather than treatment using multiple steroid injections. Keywords: hand surgery, complication, rare, carpal tunnel syndrome
