Authors: Mareen Brosterhaus, Antje Hammer, Steffen Kalina, Stefan Grau, Anjali A. Roeth, Hany Ashmawy, Thomas Gross, Marcel Binnebosel, Wolfram T. Knoefel, Tanja Manser
Abstract:
Background: The identification of critical incidents in hospitals is an essential component of improving patient safety. To date, various methods have been used to measure and characterize such critical incidents. These methods are often viewed by physicians and nurses as external quality assurance, and this creates obstacles to the reporting events and the implementation of recommendations in practice. One way to overcome this problem is to use tools that directly involve staff in measuring indicators of quality and safety of care in the department. One such instrument is the global trigger tool (GTT), which helps physicians and nurses identify adverse events by systematically reviewing randomly selected patient records. Based on so-called ‘triggers’ (warning signals), indications of adverse events can be given. While the tool is already used internationally, its implementation in German hospitals has been very limited. Objectives: This study aimed to assess the feasibility and potential of the global trigger tool for identifying adverse events in German hospitals. Methods: A total of 120 patient records were randomly selected from two surgical, and one neurosurgery, departments of three university hospitals in Germany over a period of two months per department between January and July, 2017. The records were reviewed using an adaptation of the German version of the Institute for Healthcare Improvement Global Trigger Tool to identify triggers and adverse event rates per 1000 patient days and per 100 admissions. The severity of adverse events was classified using the National Coordinating Council for Medication Error Reporting and Prevention. Results: A total of 53 adverse events were detected in the three departments. This corresponded to adverse event rates of 25.5-72.1 per 1000 patient-days and from 25.0 to 60.0 per 100 admissions across the three departments. 98.1% of identified adverse events were associated with non-permanent harm without (Category E–71.7%) or with (Category F–26.4%) the need for prolonged hospitalization. One adverse event (1.9%) was associated with potentially permanent harm to the patient. We also identified practical challenges in the implementation of the tool, such as the need for adaptation of the global trigger tool to the respective department. Conclusions: The global trigger tool is feasible and an effective instrument for quality measurement when adapted to the departmental specifics. Based on our experience, we recommend a continuous use of the tool thereby directly involving clinicians in quality improvement. Keywords: adverse events, global trigger tool, patient safety, record review

Authors: Elif Tugce Aksun Tumerkan
Abstract:
Although seafood is an important part of human diet and categorized highly traded food industry internationally, it is remain overlooked generally in the global food security aspect. Food product authentication is the main interest in the aim of both avoids commercial fraud and to consider the risks that might be harmful to human health safety. In recent years, with increasing consumer demand for regarding food content and it's transparency, there are some instrumental analyses emerging for determining food fraud depend on some analytical methodologies such as proteomic and metabolomics. While, fish and seafood consumed as fresh previously, within advanced technology, processed or packaged seafood consumption have increased. After processing or packaging seafood, morphological identification is impossible when some of the external features have been removed. The main fish and seafood quality-related issues are the authentications of seafood contents such as mislabelling products which may be contaminated and replacement partly or completely, by lower quality or cheaper ones. For all mentioned reasons, truthful consistent and easily applicable analytical methods are needed for assurance the correct labelling and verifying of seafood products. DNA-barcoding methods become popular robust that used in taxonomic research for endangered or cryptic species in recent years; they are used for determining food traceability also. In this review, when comparing the other proteomic and metabolic analysis, DNA-based methods are allowing a chance to identification all type of food even as raw, spiced and processed products. This privilege caused by DNA is a comparatively stable molecule than protein and other molecules. Furthermore showing variations in sequence based on different species and founding in all organisms, make DNA-based analysis more preferable. This review was performed to clarify the main advantages of using DNA-barcoding for determining seafood fraud among other techniques. Keywords: DNA-barcoding, genetic analysis, food fraud, mislabelling, packaged seafood

Abstract:
Quality healthcare delivery is a crucial indicator in assessing the overall developmental status of a country. There are many limitations in the Ghanaian healthcare supply chain due to the lack of studies about the correlation between quality health service and the healthcare supply chain. Patients who visit various healthcare providers face unpleasant experiences such as delays in the availability of their medications. In this study, an assessment of the quality of services provided to Ghanaian outpatients who visit public healthcare providers was investigated to establish its effect on the healthcare supply chain using a conceptual model. The Donabedian’s structure, process, and outcome theory for service quality evaluation were used to analyse 20 Ghanaian hospitals. The data obtained was tested using the structural equation model (SEM). The findings from this research will help us to improve the overall quality of the Ghanaian healthcare supply chain. Authors: Norul Husna A. Kasim, Siva K. Balasundram
Abstract:
The gap between ‘actual yield’ and ‘potential yield’ has remained a problem in the Malaysian oil palm industry. Ineffective maturity assessment and untimely harvesting have compounded this problem. Typically, the traditional method of palm oil quality and yield assessment is destructive, costly and laborious. Fluorescence-sensing offers a new means of assessing palm oil quality and yield non-destructively. This work describes the estimation of palm oil quality and yield using a multi-parametric fluorescence sensor (Multiplex®) to quantify the concentration of secondary metabolites, such as anthocyanin and flavonoid, in fresh fruit bunches across three different palm ages (6, 9, and 12 years-old). Results show that fluorescence sensing is an effective means of assessing FFB maturity, in terms of palm oil quality and yield quantifications. Keywords: anthocyanin, flavonoid fluorescence sensor, palm oil yield and quality
Procedia PDF Downloads 80911352 Effect of Modulation Factors on Tomotherapy Plans and Their Quality Analysis
Authors: Asawari Alok Pawaskar
Abstract:
This study was aimed at investigating quality assurance (QA) done with IBA matrix, the discrepancies observed for helical tomotherapy plans. A selection of tomotherapy plans that initially failed the with Matrix process was chosen for this investigation. These plans failed the fluence analysis as assessed using gamma criteria (3%, 3 mm). Each of these plans was modified (keeping the planning constraints the same), beamlets rebatched and reoptimized. By increasing and decreasing the modulation factor, the fluence in a circumferential plane as measured with a diode array was assessed. A subset of these plans was investigated using varied pitch values. Factors for each plan that were examined were point doses, fluences, leaf opening times, planned leaf sinograms, and uniformity indices. In order to ensure that the treatment constraints remained the same, the dose-volume histograms (DVHs) of all the modulated plans were compared to the original plan. It was observed that a large increase in the modulation factor did not significantly improve DVH uniformity, but reduced the gamma analysis pass rate. This also increased the treatment delivery time by slowing down the gantry rotation speed which then increases the maximum to mean non-zero leaf open time ratio. Increasing and decreasing the pitch value did not substantially change treatment time, but the delivery accuracy was adversely affected. This may be due to many other factors, such as the complexity of the treatment plan and site. Patient sites included in this study were head and neck, breast, abdomen. The impact of leaf timing inaccuracies on plans was greater with higher modulation factors. Point-dose measurements were seen to be less susceptible to changes in pitch and modulation factors. The initial modulation factor used by the optimizer, such that the TPS generated ‘actual’ modulation factor within the range of 1.4 to 2.5, resulted in an improved deliverable plan. Keywords: dose volume histogram, modulation factor, IBA matrix, tomotherapy

Authors: Su Sin Chong, Abdul Aziz Abdul Raman, Sulaiman Wadi Harun, Hamzah Arof
Abstract:
Chemical Oxygen Demand (COD) plays a vital role determination of an appropriate strategy for wastewater treatment including the control of the quality of an effluent. In this study, a new sensing method was introduced for the first time and developed to investigate chemical oxygen demand (COD) using a Mach-Zehner Interferometer (MZI)-based dye sensor. The sensor is constructed by bridging two single mode fibres (SMF1 and SMF2) with a short section (~20 mm) of multimode fibre (MMF) and was formed by tapering the MMF to generate evanescent field which is sensitive to perturbation of sensing medium. When the COD concentration increase takes effect will induce changes in output intensity and effective refractive index between the microfiber and the sensing medium. The adequacy of decisions based on COD values relies on the quality of the measurements. Therefore, the dual output response can be applied to the analytical procedure enhance measurement quality. This work presents a detailed assessment of the determination of COD values in synthetic wastewaters. Detailed models of the measurement performance, including sensitivity, reversibility, stability, and uncertainty were successfully validated by proficiency tests where supported on sound and objective criteria. Comparison of the standard method with the new proposed method was also conducted. This proposed sensor is compact, reliable and feasible to investigate the COD value. Keywords: chemical oxygen demand, environmental sensing, Mach-Zehnder interferometer sensor, online monitoring

Abstract:
Background: Hearing loss (HL) affect people of all ages and stages in life. To author's best knowledge, if patients with an HL have reduced Generic Quality of life (QoL), has yet not been answered. Aim: The aim of the present study was to investigate the relationship between HL and generic and disease-specific Health Related Quality of Life (HRQoL) in adult patients (aged 18–78 years) with an HL, seeking Hearing Aid (HA). Material and Methods: 158 adult (aged 18-78 years) patients with HL, referred for HA fitting at Haukeland University Hospital in western Norway, participated in the study. Both first-time users, as well as patients referred for HA renewals, were included. First-time users had been pre-examined by an Ear Nose and Throat specialist. The questionnaires were answered before the actual HA fitting procedure. The pure-tone average (PTA; frequencies 0.5, 1, 2 and 4 kHz) was determined for each ear. The generic European Organization for Research and Treatment of Cancer (EORTC) Quality of Life Questionnaire general part and a shortened version of the Abbreviated Profile of Hearing Aid Benefit (APHAB) were answered. In addition, EORTC HRQoL answers from a general population and patients with former head and neck cancer served as comparison. Results: In general, no lowered HRQoL scores were determined among HL patients compared to the general population. Patients with unilateral HL to some extent showed lower HRQoL than those with bilateral HL (social function and fatigue). The APHAB scores correlated significantly with the EORTC HRQoL scores. By stepwise linear regression analysis, the APHAB scores were scored secondary to PTA (best ear), cognitive and physical function. Conclusion: HRQoL scores in HL patients, in general, seems to be at the population level, but the unilateral HL patients scored to some extent lower than the bilateral HI patients. APHAB and generic QoL scores levels are associated. Both HRQoL and APHAB scores are generated more complexly than anticipated. Keywords: quality of life, hearing loss, hearing impairment, distress, depression, anxiety, hearing aid

Authors: H. Ashrafi, S. Ebrahimi, H. Kamalzadeh
Abstract:
With the development of business competition, it is important for healthcare providers to improve their service qualities. In order to improve service quality of a clinic, four important dimensions are defined: tangibles, responsiveness, empathy, and reliability. Moreover, there are several service stages in hospitals such as financial screening and examination. One of the most challenging limitations for improving service quality is budget which impressively affects the service quality. In this paper, we present an approach to address budget uncertainty and provide guidelines for service resource allocation. In this paper, a service quality improvement approach is proposed which can be adopted to multistage service processes to improve service quality, while controlling the costs. A multi-objective function based on the importance of each area and dimension is defined to link operational variables to service quality dimensions. The results demonstrate that our approach is not ultra-conservative and it shows the actual condition very well. Moreover, it is shown that different strategies can affect the number of employees in different stages. Keywords: allocation, budget uncertainty, healthcare resource, service quality assessment, robust optimization

Abstract:
In the process of manufacturing, a machine breakdown usually forces a modified flow shop out of the prescribed state, this strategy reschedules part of the initial schedule to match up with the pre-schedule at some point with the objective to create a schedule that is reliable with the other production planning decisions like material flow, production and suppliers by utilizing a critical decision-making concept. We propose a rescheduling strategy and a match-up point that will have a determination procedure through an advanced feedback control mechanism to increase both the schedule quality and stability. Authors: Sakda Loetpiparwanich, Preecha Vichitthamaros
Abstract:
Nowadays, poor data quality is considered one of the majority costs for a data project. The data project with data quality awareness almost as much time to data quality processes while data project without data quality awareness negatively impacts financial resources, efficiency, productivity, and credibility. One of the processes that take a long time is defining the expectations and measurements of data quality because the expectation is different up to the purpose of each data project. Especially, big data project that maybe involves with many datasets and stakeholders, that take a long time to discuss and define quality expectations and measurements. Therefore, this study aimed at developing meaningful indicators to describe overall data quality for each dataset to quick comparison and priority. The objectives of this study were to: (1) Develop a practical data quality indicators and measurements, (2) Develop data quality dimensions based on statistical characteristics and (3) Develop Composite Indicator that can describe overall data quality for each dataset. The sample consisted of more than 500 datasets from public sources obtained by random sampling. After datasets were collected, there are five steps to develop the Dataset Quality Index (SDQI). First, we define standard data quality expectations. Second, we find any indicators that can measure directly to data within datasets. Thirdly, each indicator aggregates to dimension using factor analysis. Next, the indicators and dimensions were weighted by an effort for data preparing process and usability. Finally, the dimensions aggregate to Composite Indicator. The results of these analyses showed that: (1) The developed useful indicators and measurements contained ten indicators. (2) the developed data quality dimension based on statistical characteristics, we found that ten indicators can be reduced to 4 dimensions. (3) The developed Composite Indicator, we found that the SDQI can describe overall datasets quality of each dataset and can separate into 3 Level as Good Quality, Acceptable Quality, and Poor Quality. The conclusion, the SDQI provide an overall description of data quality within datasets and meaningful composition. We can use SQDI to assess for all data in the data project, effort estimation, and priority. The SDQI also work well with Agile Method by using SDQI to assessment in the first sprint. After passing the initial evaluation, we can add more specific data quality indicators into the next sprint. Keywords: data quality, dataset quality, data quality management, composite indicator, factor analysis, principal component analysis

Authors: Ratna Tantra
Abstract:
From a risk assessment point of view, solubility is a property that has been identified as being important. If nanomaterial is completely soluble, then its disposal can be treated much in the same way as ‘ordinary’ chemicals, which subsequently will simplify testing and characterization regimes. The measurement of solubility has been highlighted as important in a pan-European project, Framework Programme (FP) 7 NANoREG. Some of the project outputs surrounding this topic will be presented here, in which there are two parts. First, a review on existing methods capable of measuring nanomaterial solubility will be discussed. Second, a case study will be presented based on using colorimetry methods to quantify dissolve zinc from ZnO nanomaterial upon exposure to digestive juices. The main findings are as follows: a) there is no universal method for nanomaterial solubility testing. The method chosen will be dependent on sample type and nano-specific application/scenario. b) The colorimetry results show a positive correlation between particle concentration and amount of [Zn2+] released; this was expected c) results indicate complete dissolution of the ZnO nanomaterial, as a result of the digestion protocol but only a fraction existing as free ions. Finally, what differentiates the F7 NANoREG project over other projects is the need for participating research laboratories to follow a set of defined protocols, necessary to establish quality control and assurance. The methods and results associated with mandatory testing that carried out by all partners in NANoREG will be discussed. Keywords: nanomaterials, nanotoxicology, solubility, zinc oxide

The pressure from globalization has made manufacturing organizations to move towards three major competitive arenas: quality, cost, and responsiveness. Quality is a universal value and has become a global issue. In order to survive and be able to provide customers with good products, manufacturing organizations’ supporting systems, tools, and structures it uses must grow or evolve. The majority of quality management concepts and strategies that are practiced recently are aimed at detecting and correcting problems which already exist and serve to limit losses. In agile manufacturing environment there is no room for defect and error so it needs a quality management which is proactively directed at problem prevention. This proactive quality management avoids losses by focusing on failure prevention, virtual elimination of the possibility of premature failure, mistake-proofing, and assuring consistently high quality in the definition and design of creation processes. To achieve this, a decision support tool for quality control and management is suggested. Current decision support tools/methods used by most manufacturing companies in Botswana for quality management and control are not integrated, for example they are not consistent since some tests results data is recorded manually only whilst others are recorded electronically. It is only a set of procedures not a tool. These procedures cannot offer interactive decision support. This point brings to light the aim of this research which is to develop a framework which will help manufacturing companies in Botswana build a decision support tool for quality control and management. Keywords: decision support tool, manufacturing, quality control, quality management

Abstract:
Being diagnosed with metastatic breast cancer, the patients as well as their caretakers are affected physically and mentally. Although the medical systems in Thailand have been attempting to improve the quality and effectiveness of the treatment of the disease in terms of physical illness, the success of the treatment also depends on the quality of mental health. Metastatic breast cancer patients have found that social support is a key factor that helps them through this difficult time. It is recognized that social support in different dimensions, including emotional support, social network support, informational support, instrumental support and appraisal support, are contributing factors that positively affect the quality of life of patients in general, and it is undeniable that social support in various forms is important in promoting the quality of life of metastatic breast patients. However, previous studies have not been dedicated to investigating their quality of life concerning affective, cognitive, and behavioral outcomes. Therefore, this study aims to develop integrated social support through social networks to improve the quality of life of metastatic breast cancer patients in Thailand. Keywords: social support, metastatic breath cancer, quality of life, social network

Authors: K. Ebrahimi, S. Shahid, H. Dehban
Abstract:
Climate variability can affect surface water quality. The objective of present study is to assess the impacts of climate variability on water quality of Gorganroud River, Iran, over the time period 1971 to 2011. To achieve this aim, climate variability and water quality variations were studied involving a newly developed drought index (MRDI) and hysteresis curves, respectively. The results show that climate variability significantly affected surface water quality over the time. The existence of yearly internal variation and hysteresis phenomenon for pH and EC parameters was observed. It was found that though drought affected pH considerably, it could not affect EC significantly. Keywords: climate variability, hysteresis curves, multi drought index, water quality

Authors: Kailas Bhandarkar, Talib Dar, Laura Karia, Manasvi Upadhyaya
Abstract:
Introduction: Frenotomy for tongue tie is commonly performed in breastfed infants who experience difficulty in latching after failed conservative management for tongue tie. However, there is no consensus for the routine use of massage following frenotomy. Our aim was to assess the efficacy of massage in preventing recurrence following frenotomy. Methods: The tongue tie service in our tertiary referral hospital consists of 5 consultants and a breastfeeding (BF) midwife. 3 consultants routinely advice massage post procedure. Babies are assessed by the midwife after the procedure and a follow-up consultation after a week. After due ethical approval, data were collected by two staff members who were independent of TT service on a standardized questionnaire to avoid bias. Fischer exact test was employed (p < 0.05 considered significant). Results: Six hundred and thirty-two babies attended the clinic from January 2018 to December 2018. Thirty-three of these were excluded as the procedure was not needed. Parents were contacted at a median of six months post-procedure (range 2-10 months). 282/599 were advised massage. 92/282 could be contacted. 40/ 92 adhered to massage regimen. None of these had a recurrence. 52/92 (54%), although advised, did not perform massage. Reasons cited for lack of adherence to massage included difficulty in performing massaging and conflicting advice given by other health care professionals involved in patient care like paediatricians and group practice and lack of information on the internet). Overall, 4/599 (0.66%) had recurrences, and this difference was not statistically significant. Conclusion: In our experience, the rate of recurrence after frenotomy is low enough for us to conclude that there is no significant benefit of massage after frenotomy for tongue tie. We could also conclude that among parents who were advised massage more than half failed to adhere to the advice. Keywords: tongue tie, frenotomy, massage, recurrence
