481 Proportion and Factors Associated with Presumptive Tuberculosis among Suspected Pediatric TB Patients

Abstract:
The study addresses the increasing challenge of pediatric presumptive tuberculosis, emphasizing the need to understand the factors associated with it. The research aims to determine the proportion of presumptive TB and factors associated with it among suspected pediatric tuberculosis patients. A cross-sectional study was conducted at ICDDR-Bangladesh, collecting specimens from suspected pediatric patients and using logistic regression for data analysis. The study found a high proportion of presumptive TB (85.7%) but no statistically significant differences between presumptive and non-presumptive TB. Theoretical importance of the study highlights the importance of identifying factors associated with presumptive TB for better control and management strategies. Specimens were collected from 84 suspected pediatric patients diagnosed with TB based on clinical symptoms/radiological findings. Microbiological tests like smear-microscopy, culture, and GeneXpert were used to isolate presumptive TB and confirmed TB. The proportion of presumptive TB was 85.7% among suspected pediatric TB patients. Among various factors that were not found to be associated with the presumptive TB. The study concludes that despite a high proportion of presumptive TB, no significant differences were found between presumptive and non-presumptive TB cases.

Abstract:
This project aims to study and evaluate the motorcycle crash bar, which is used to reduce injuries caused by side impacts to the motorcycle, and then develop an improved design using the engineering design process theory based on the current benchmark crash bar in order to lower the severity of motorcycle crash injuries. For this purpose, simulations for the crash bar are set up so that it travels at an angle towards a fixed concrete wall and collides at certain velocities. 3D CAD models are first designed in SolidWorks and dynamic crash simulations are then carried out using ANSYS to determine the lowest maximum Von-Mises stress over time and deformations by adjusting the parameters used in manufacturing the crash bar, including the velocity of the crash, material used, geometries with various radius fillets, and different thicknesses for the bar. The results of the simulation are used to determine the optimum parameters for a safer crash bar to withstand higher stress and deformation. Specifically, the von-Mises stress was reduced by at least 75% compared with the benchmark design by choosing aluminum alloy and a true unibar design.

Abstract:
This study found that most corporate personnel are using social media to communicate with colleagues to make the process of working more efficient. Complete satisfaction occurred on the use of security within the University’s computer network. The social network usage for communication, collaboration, entertainment and demonstrating concerns accounted for fifty percent of variance to predict interpersonal relationships of corporate personnel. This evaluation on the effectiveness of social networking involved 213 corporate personnel’s. The data was collected by questionnaires. This data was analyzed by using percentage, mean, and standard deviation. The results from the analysis and the effectiveness of using online social networks were derived from the attitude of private users and safety data within the security system. The results showed that the effectiveness on the use of an online social network for corporate personnel of Suan Sunandha Rajabhat University was specifically at a good level, and the overall effects of each aspect was (Ẋ=3.11).Keywords: Behaviors, Social Media, Social Network.

Abstract:
Over the past few years, a lot of research has been conducted to bring Automatic Speech Recognition (ASR) into various areas of Air Traffic Control (ATC), such as air traffic control simulation and training, monitoring live operators for with the aim of safety improvements, air traffic controller workload measurement and conducting analysis on large quantities controller-pilot speech. Due to the high accuracy requirements of the ATC context and its unique challenges, automatic speech recognition has not been widely adopted in this field. With the aim of providing a good starting point for researchers who are interested bringing automatic speech recognition into ATC, this paper gives an overview of possibilities and challenges of applying automatic speech recognition in air traffic control. To provide this overview, we present an updated literature review of speech recognition technologies in general, as well as specific approaches relevant to the ATC context. Based on this literature review, criteria for selecting speech recognition approaches for the ATC domain are presented, and remaining challenges and possible solutions are discussed.Keywords: Automatic Speech Recognition, ASR, Air Traffic Control, ATC.

Abstract:
Long-term differential settlement between the bridge structure and the bridge embankment typically results in an abrupt grade change, causing driver discomfort, impairing driver safety, and exerting a potentially excessive impact traffic loading on the abutment. This paper has analysed a case of study showing the effect of an approaching slab realized in a bridge constructed at Tirane-Elbasan Motorway. The layer thickness under the slab is modeled as homogenous, the slab is a reinforced concrete structure and over that the asphaltic layers take place. Analysis indicates that reinforced concrete approaching slab distributes the stresses quite uniformly into the road fill layers and settlements varies in a range less than 2.50 cm in the total slab length of 6.00 m with a maximum slope of 1/240. Results taken from analytical analysis are compared with topographic measurements done on field and they carry great similarities.Keywords: Approach slab, bridge, road pavement, differential settlement.

Lean, which was initially developed by Toyota, is widely implemented in other companies to improve competitiveness. This research is an attempt to identify the adoption of lean in the production system of Malaysian car manufacturer, Proton using case study approach. To gain the in-depth information regarding lean implementation, an activity on the assembly line called Set Parts Supply (SPS) was studied. The result indicates that by using lean principles, tools and techniques in the implementation of SPS enabled to achieve the goals on safety, quality, cost, delivery and morale. The implementation increased the size of the workspace, improved the quality of assembly and the delivery of parts supply, reduced the manpower, achieved cost savings on electricity and also increased the motivation of manpower in respect of attendance at work. Abstract:
This paper presents a new strategy of identification and classification of pathological voices using the hybrid method based on wavelet transform and neural networks. After speech acquisition from a patient, the speech signal is analysed in order to extract the acoustic parameters such as the pitch, the formants, Jitter, and shimmer. Obtained results will be compared to those normal and standard values thanks to a programmable database. Sounds are collected from normal people and patients, and then classified into two different categories. Speech data base is consists of several pathological and normal voices collected from the national hospital “Rabta-Tunis". Speech processing algorithm is conducted in a supervised mode for discrimination of normal and pathology voices and then for classification between neural and vocal pathologies (Parkinson, Alzheimer, laryngeal, dyslexia...). Abstract:
Bipartite medial cuneiforms are relatively rare but may play a significant role in biomechanical and gait abnormalities. It is believed that a bipartite medial cuneiform may alter the available range of motion due to its larger morphological variant, thus limiting the metatarsal plantarflexion needed to achieve adequate hallux dorsiflexion for normal gait. Radiographic and clinical assessment were performed on two patients who reported with foot pain along the first ray. Both patients had visible bipartite medial cuneiforms on MRI. Using gait plate and Metascan ™ analysis, both were noted to have four measurements far beyond the expected range. Medial and lateral heel peak pressure, hallux peak pressure, and 1st metatarsal peak pressure were all noted to be increased. These measurements are believed to be increased due to the hindrance placed on the available ROM of the first ray by the increased size of the medial cuneiform. A larger patient population would be needed to fully understand this developmental anomaly.

This research work proposes to develop a novel control system to enhance the efficiency of the vehicles under constraints of various conditions, and contributes in stability and controllability issues, considering relevant safety and operational requirements with command control communication and various sensors to avoid accidents. The approach of speed scheduling, management and control in local and distributed environment is given to fulfill the dire needs of modern trend and enhance the vehicles control systems in automation. Abstract:
The knowledge about rock layers thickness,especially above drilled mining pavements is crucial for workers safety. The measuring systems used nowadays are generally imperfect and there is a strong demand for improvement. The application of a new type of a measurement system called Self-excited Acoustical System is presentedin the paper. The system was applied until now to monitor stress changes in metal and concrete constructions. The change in measurement methodology resulted in possibility of measuring the thickness of the rocks above the tunnels as well as thickness of a singular rocklayer. The idea is to find two resonance frequencies of the self-exited system,which consists of a vibration exciter and vibration receiver placed at a distance, which are coupled with a proper power amplifier, and which operate in a closed loop with a positive feedback. The resonance with the higher amplitude determines thickness of the whole rock, whereas the lower amplitude resonance indicates thickness of a singular layer. The results of the laboratory tests conducted on a group of different rock materials are also presented.

By GEO5 FEM program with four rockfill slope modeling and stability analysis was performed for S1, S2, S3 and S4 slopes where landslides of the shalefills were limited. Effective angle of internal friction (φ'°) 17°-22.5°, the effective cohesion (c') from 0.5 to 1.8 kPa, saturated unit weight 1.78-2.43 g/cm3, natural unit weight 1.9-2.35 g/cm3, dry unit weight 1.97-2.40 g/cm3, the permeability coefficient of 1x10-4 - 6.5x10-4 cm/s. In cross-sections of the slope, GEO 5 FEM program possible critical surface tension was examined. Rockfill dump design was made to prevent sliding slopes. Bulk material designated geotechnical properties using also GEO5 programs FEM and stability program via a safety factor determined and calculated according to the values S3 and S4 No. slopes are stable S1 and S2 No. slopes were close to stable state that has been found to be risk. Healthcare delivery systems around the world are in crisis. The need to improve health outcomes while decreasing healthcare costs have led to an imminent call to action to transform the healthcare delivery system. While Bioinformatics and Biomedical Engineering have primarily focused on biological level data and biomedical technology, there is clear evidence of the importance of the delivery of care on patient outcomes. Classic singular decomposition approaches from reductionist science are not capable of explaining complex systems. Approaches and methods from systems science and systems engineering are utilized to structure healthcare delivery system data. Specifically, systems architecture is used to develop a multi-scale and multi-dimensional characterization of the healthcare delivery system, defined here as the Healthcare Delivery System Knowledge Base. The evolution and integration of automated vehicles have become more and more tangible in recent years. State-of-the-art technological advances in the field of camera-based Artificial Intelligence (AI) and computer vision greatly favor the performance and reliability of Advanced Driver Assistance System (ADAS), leading to a greater knowledge of vehicular operation and resembling the human behaviour. However, the exclusive use of this technology still seems insufficient to control the vehicular operation at 100%. To reveal the degree of accuracy of the current camera-based automated driving AI modules, this paper studies the structure and behavior of one of the main solutions in a controlled testing environment. The results obtained clearly outline the lack of reliability when using exclusively the AI model in the perception stage, thereby entailing using additional complementary sensors to improve its safety and performance.

Abstract:
This paper details the utilization of artificial intelligence (AI) in the field of slope stability whereby quick and convenient solutions can be obtained using the developed tool. The AI tool used in this study is the artificial neural network (ANN), while the slope stability analysis methods are the finite element limit analysis methods. The developed tool allows for the prompt prediction of the safety factors of fill slopes and their corresponding probability of failure (depending on the degree of variation of the soil parameters), which can give the practicing engineer a reasonable basis in their decision making. In fact, the successful use of the Extreme Learning Machine (ELM) algorithm shows that slope stability analysis is no longer confined to the conventional methods of modeling, which at times may be tedious and repetitive during the preliminary design stage where the focus is more on cost saving options rather than detailed design. Therefore, similar ANN-based tools can be further developed to assist engineers in this aspect.

Abstract:
This paper mainly studies the analyses of parameters in the intersection collision avoidance (ICA) system based on the radar sensors. The parameters include the positioning errors, the repeat period of the radar sensor, the conditions of potential collisions of two cross-path vehicles, etc. The analyses of the parameters can provide the requirements, limitations, or specifications of this ICA system. In these analyses, the positioning errors will be increased as the measured vehicle approach the intersection. In addition, it is not necessary to implement the radar sensor in higher position since the positioning sensitivities become serious as the height of the radar sensor increases. A concept of the safety buffer distances for front and rear of the measured vehicle is also proposed. Medical images require special safety and confidentiality because critical judgment is done on the information provided by medical images. Transmission of medical image via internet or mobile phones demands strong security and copyright protection in telemedicine applications. Here, highly secured and robust watermarking technique is proposed for transmission of image data via internet and mobile phones. The Region of Interest (ROI) and Non Region of Interest (RONI) of medical image are separated. Only RONI is used for watermark embedding. This technique results in exact recovery of watermark with standard medical database images of size 512x512, giving 'correlation factor' equals to 1. The correlation factor for different attacks like noise addition, filtering, rotation and compression ranges from 0.90 to 0.95. The PSNR with weighting factor 0.02 is up to 48.53 dBs. The presented scheme is non blind and embeds hospital logo of 64x64 size.

Abstract:
We present a new method for the fully automatic 3D reconstruction of the coronary artery centerlines, using two X-ray angiogram projection images from a single rotating monoplane acquisition system. During the first stage, the input images are smoothed using curve evolution techniques. Next, a simple yet efficient multiscale method, based on the information of the Hessian matrix, for the enhancement of the vascular structure is introduced. Hysteresis thresholding using different image quantiles, is used to threshold the arteries. This stage is followed by a thinning procedure to extract the centerlines. The resulting skeleton image is then pruned using morphological and pattern recognition techniques to remove non-vessel like structures. Finally, edge-based stereo correspondence is solved using a parallel evolutionary optimization method based on f symbiosis. The detected 2D centerlines combined with disparity map information allow the reconstruction of the 3D vessel centerlines. A reliable estimate of the average bond stress within the anchorage of steel reinforcing bars in tension is critically important for the design of reinforced concrete member. This paper describes part of a recently completed experimental research program in the Centre for Infrastructure Engineering and Safety (CIES) at the University of New South Wales, Sydney, Australia aimed at assessing the effects of different factors on the anchorage requirements of modern high strength steel reinforcing bars. The study found that an increase in the anchorage length and bar diameter generally leads to a reduction of the average ultimate bond stress. By the extension of a well established analytical model of bond and anchorage, it is shown here that the differences in the average ultimate bond stress for different anchorage lengths is associated with the variable degree of plastic deformation in the tensile zone of the concrete surrounding the bar.Keywords: Anchorage, Bond stress, Development length, Reinforced concrete.

Transmission shafts are affected by various forces, for example, during acceleration or sudden breaks, bending during transportation, vertical forces that lead to cuts. One of the main failures in combines is breaking shaft which repairmen refer it. Structural resistance of canal against torque is very important in the beginning of the movement. For analyzing stress, a typical sample from a type of combine was selected, called JD955 combine. Long shaft in this combine was analyzed with finite element method by Ansys13 generic package under static load. Conducted analysis showed that there is a maximum stress in contact surfaces of indentations and also in place of changing diameter. Safety factor value is low in parts of the shaft and this increases the probability of failure at these points. This paper presents the electrical effect of two neighboring distribution substation during the construction phase. The size of auxiliary grounding grid have an effect on entire grounding system. The bigger the size of auxiliary grounding grid, the lower the GPR and maximum touch voltage, with the exception that when the two grids are unconnected, i.e. the bigger the size of auxiliary grounding grid, the higher the maximum step voltage. The results in this paper could be served as design guideline of grounding system, and perhaps remedy of some troublesome grounding grids in power distribution’s system. Modeling and simulation is carried out on the Current Distribution Electromagnetic interference Grounding and Soil structure (CDEGS) program. The simulation results exhibit the design and analysis of power system grounding and perhaps could be set as a standard in grounding system design and modification in distribution substations.

Understanding patient factors related to physical activity behavior is important in the management of Type 2 Diabetes. This study applied the Theory of Planned Behavior model to understand physical activity behavior among sampled Type 2 diabetics in Kenya. The study was conducted within the diabetic clinic at Kisii Level 5 Hospital and adopted sequential mixed methods design beginning with qualitative phase and ending with quantitative phase. Qualitative data was analyzed using grounded theory analysis method. Structural equation modeling using maximum likelihood was used to analyze quantitative data. The common fit indices revealed that the theory of planned behavior fitted the data acceptably well among the Type 2 diabetes and within physical activity behavior {¤ç2 = 213, df = 84, n=230, p = .061, ¤ç2/df = 2.53; TLI = .97; CFI =.96; RMSEA (90CI) = .073(.029, .08)}. This theory proved to be useful in understanding physical activity behavior among Type 2 diabetics.

Abstract:
The paper presents results of the project conducted in Poland devoted to study on bird strikes at military airfields. The main aim of this project was to develop methods of aircraft protection against threats from birds. The studies were carried out using two methods. One by transect and the other one by selected sector scanning. During the research, it was recorded, that 104 species of birds in the number about of 36000 were observed. The most frequent ones were starling Sturnus vulgaris (31.0%), jackdaw Corvus monedula (18.3%), rook Corvus frugilegus (15.9 %), lapwing Vanellus vanellus (6.2%). Moreover, it was found, that starlings constituted the most serious threat. It resulted from their relatively high attendance at the runway (about 300 individuals). Possible repellent techniques concerning of the Deblin military airfield were discussed. The analysis of the birds’ concentration depending on the altitude, part of the day, year, part of the airfield constituted a base to work out critical flight phase and appropriate procedures to prevent bird strikes.

Abstract:
Private data are more often breached by clever social engineering rather than exploiting technical vulnerabilities in the systems. Complete information security requires good data safety practices to go along with technical solutions. Hackers often begin their operation by simply sending spoofed emails or fraudulent URLs to their targets and trick them into providing sensitive information such as passwords or bank account details. This technique is called phishing. Phishing attacks can be launched on email addresses, open ports and unsecured web browsers. This study uses quantitative method of research to execute phishing experiments on the participants to test their response to the phishing emails. These experiments were run on Kali Linux distribution which came bundled with multiple open-source intelligence (OSINT) tools that were used in the study. The aim of this research is to see how successful phishing attacks can be launched using OSINT and to test the response of people to spoofed emails.

Abstract:
This paper presents an analytical model to estimate the cost of an optimized design of reinforced concrete isolated footing base on structural safety. Flexural and optimized formulas for square and rectangular footingare derived base on ACI building code of design, material cost and optimization. The optimization constraints consist of upper and lower limits of depth and area of steel. Footing depth and area of reinforcing steel are to be minimized to yield the optimal footing dimensions. Optimized footing materials cost of concrete, reinforcing steel and formwork of the designed sections are computed. Total cost factor TCF and other cost factors are developed to generalize and simplify the calculations of footing material cost. Abstract:
With the increasing of human space activities, the number of space debris has increased dramatically, and the possibility that spacecrafts on orbit are impacted by space debris is growing. A method is of the vital significance to real-time detect and assess spacecraft damage, determine of gas leak accurately, guarantee the life safety of the astronaut effectively. In this paper, acoustic sensor array is used to detect the acoustic signal which emits from the damage of the spacecraft on orbit. Then, we apply the time difference of arrival and beam forming algorithm to locate the damage and leakage. Finally, the extent of the spacecraft damage is evaluated according to the nonlinear ultrasonic method. The result shows that this method can detect the debris impact and the structural damage, locate the damage position, and identify the damage degree effectively. This method can meet the needs of structural damage detection for the spacecraft in-orbit.

Abstract:
Along with the advances in medicine, providing medical information to individual patient is becoming more important. In Japan such information via Braille is hardly provided to blind and partially sighted people. Thus we are researching and developing a Web-based automatic translation program “eBraille" to translate Japanese text into Japanese Braille. First we analyzed the Japanese transcription rules to implement them on our program. We then added medical words to the dictionary of the program to improve its translation accuracy for medical text. Finally we examined the efficacy of statistical learning models (SLMs) for further increase of word segmentation accuracy in braille translation. As a result, eBraille had the highest translation accuracy in the comparison with other translation programs, improved the accuracy for medical text and is utilized to make hospital brochures in braille for outpatients and inpatients.

Due to the heavy burden and pressure of caring for infants, an integrated automatic baby watching system based on IoT smart sensing and deep learning machine vision techniques is proposed in this paper. By monitoring infant body conditions such as heartbeat, breathing, body temperature, sleeping posture, as well as the surrounding conditions such as dangerous/sharp objects, light, noise, humidity and temperature, the proposed system can analyze and predict the obvious/potential dangerous conditions according to observed data and then adopt suitable actions in real time to protect the infant from harm. Thus, reducing the burden of the caregiver and improving safety efficiency of the caring work. The experimental results show that the proposed system works successfully for the infant care work and thus can be implemented in various life fields practically.Keywords: Baby care system, internet of things, deep learning, machine vision.

Abstract:
The paper presents a novel screening method to indicate congenital heart diseases (CHD), which otherwise could remain undetected because of their low level. Therefore, not belonging to the high-risk population, the pregnancies are not subject to the regular fetal monitoring with ultrasound echocardiography. Based on the fact that CHD is a morphological defect of the heart causing turbulent blood flow, the turbulence appears as a murmur, which can be detected by fetal phonocardiography (fPCG). The proposed method applies measurements on the maternal abdomen and from the recorded sound signal a sophisticated processing determines the fetal heart murmur. The paper describes the problems and the additional advantages of the fPCG method including the possibility of measurements at home and its combination with the prescribed regular cardiotocographic (CTG) monitoring. The proposed screening process implemented on a telemedicine system provides an enhanced safety against hidden cardiac diseases.Keywords: Cardiac murmurs, fetal phonocardiography, screening of CHDs, telemedicine system.

This paper will define the system that minimize the risk of the ship accidents because of high or dangerous waves namely early warning system. Since Indonesia is located in a strategic position, many internasional vessels pass by the Indonesian Sea Lanes. Therefore many issues often occur in Indonesian waters, one of the issues is the shipwreck because of dangerous waves. In order to do the preventive action for the vessels that indicated exposed the dangerous waves, Indonesian Maritime Security Coordinating Board or Bakorkamla, has built up and implemented an early warning system through integrated system, called Bakorkamla Integrated Information System (BIIS). By implementing BIIS means that Bakorkamla has already done one of the Five Principles of Sea and Coast Guard Agency, which is safety and security, and Bakorkamla also has already saved the lives of many people on the ship that will have an accident due to high waves.

Abstract:
Female breast cancer is the second in frequency after cervical cancer. Surgery is the most common treatment for breast cancer, followed by chemotherapy as a treatment of choice. Although effective, it causes serious side effects. Controlled-release drug delivery is an alternative method to improve the efficacy and safety of the treatment. It can release the dosage of drug between the minimum effect concentration (MEC) and minimum toxic concentration (MTC) within tumor tissue and reduce the damage of normal tissue and the side effect. Because an in vivo experiment of this system can be time-consuming and labor-intensive, a mathematical model is desired to study the effects of important parameters before the experiments are performed. Here, we describe a 3D mathematical model to predict the release of doxorubicin from pluronic gel to treat human breast cancer. This model can, ultimately, be used to effectively design the in vivo experiments.
