83 A CD40 Variant is Associated with Systemic Bone Loss Among Patients with Rheumatoid Arthritis

Abstract:
Objectives: Little is known about genes predisposing to systemic bone loss (SBL) in rheumatoid arthritis (RA). Therefore, we examined the association between SBL and a variant of CD40 gene, which is known to play a critical role in both immune response and bone homeostasis among patients with RA. Methods: CD40 rs48104850 was genotyped in 176 adult RA patients. Bone mineral density (BMD) was measured using dual-energy X-ray absorptiometry (DXA). Results: Low BMD was observed in 116 (65.9%) patients. Among them, 60 (34.1%) had low femoral neck (FN) Z score, 72 (40.9%) had low total femur (TF) Z score, and 105 (59.6%) had low lumbar spine (LS) Z score. CD40 rs4810485 was found to be associated with reduced TF Z score with the CD40 rs4810485 T allele protecting against reduced TF Z score (OR = 0.40, 95% CI = 0.23-0.68, p = 0.0005). This association was confirmed in the multivariate logistic regression analysis (OR=0.31, 95% CI= 0.16-0.59, p=3.84 x 10₋₄). Moreover, median FN BMD was reduced among RA patients with CD40 rs4810485 GG genotype compared to RA patients harbouring CD40 rs4810485 TT and GT genotypes (0.788± 0.136 versus 0.826± 0.146g/cm², p=0.001). Abstract:
The present study was aimed to investigate the pharmacokinetics behavior and optimal dosage regimen of danofloxacin in 8 adult healthy buffaloes of local breed (Nili Ravi) following single intravenous administration at the dose of 2.5 mg/kg body weight. Plasma drug concentrations at various time intervals were measured by HPLC method. In vitro plasma protein binding was determined employing the ultrafiltration technique. The distribution and elimination of danofloxacin was rapid, as indicated by the values (Mean±SD) of distribution half-life (t1/2α = 0.25±0.09 hours) and elimination half life (t1/2β = 3.26±0.43 hours), respectively. Volume of distribution at steady state (Vss) was 1.14±0.12 L/kg, displaying its extensive distribution into various body fluids and tissues. The high value of AUC (9.80±2.14 µg/ml.hr) reflected the vast area of the body covered by drug concentration. The mean residence time was noted to be 4.78±0.52 hours. On the basis of pharmacokinetic parameters, a suitable intravenous regimen for danofloxacin in adult buffaloes would be 6.5 mg/kg to be repeated after 12 hours intervals. The present study is the foremost pharmacokinetic study of danofloxacin in the local species which would provide the valueable contribution in the local manufacturing of danofloxacin in Pakistan in future.Keywords: danofloxacin, pharmacokinetics, plasma protein binding, buffaloes, dosage regimen

The purpose of this study was to reduce patient waiting times, improve system throughput and improve resources utilization in radiology department. A discrete event simulation model was developed using Arena simulation software to investigate different alternatives to improve the overall system delivery based on adding resource scenarios due to the linkage between patient waiting times and resource availability. Abstract:
The levels of metal pollution (Pb, Cd, Cu, Zn) were investigated in the leaves of Dalbergia sisso in urban areas of the Sahiwal and Faisalabad City. For this purpose, three habitats were selected for sampling (roads, residential areas and parks). High concentration of metal was found in roadside samples as compared to residential areas and parks. In Sahiwal city the mean concentration of Copper (7.68µgg-¹) Zinc (43.55µgg-¹) and lead (4.79µgg-¹) were detected. Similarly, concentration of Cu, Zn, Pb and Cd in leaves of Faisalabad city ranged from 14.4-11.3µgg-¹, 49.7-49.5µgg-¹,138.7-47.1µgg-¹. Highest concentration of heavy metals was detected in Faisalabad as compared to Sahiwal city and level of heavy metals was below the threshold limits, therefore, the concentration of heavy metals was not high in Dalbergia sissoo.Keywords: cadmium, copper, lead, zinc

Abstract:
C. tubulosa is a rare parasitic plant. It is found to be endangered and it acquires nutrition by penetrating roots deep in host roots. It has momentous potential to fulfill local and national health needs. This specie became endangered due to its parasitic mode of life and lack of awareness. Investigation of distribution and habitat conditions of C. tubulosa from District Pakpattan is the objective of this study. To explore its habitat conditions and community ecology phytosociological survey of C. tubulosa in different habitats i.e roadsides and graveyards was carried out. It was found that C. tubulosa occurs successfully in different habitats like graveyards and roadsides with specific neighboring species. Soil analysis was carried out by taking soil samples from seven sites. Soil was analyzed for pH, EC, soil texture, OM, N %age, Ca, Mg, P and K, which shows that soil of C. tubulosa is rich in all these nutrients.Keywords: organic matter, potassium, phosphorus, magnesium

Abstract:
Background and aims: Given the importance of health promotion in elderly and attention to health factors including physical activity and self-image reinforcing, this study aimed to investigate the relationship between exercise attitude and performance with self-image concept in elderly men. Methods: In this descriptive–correlational study, 50 different daily exercise activities of the elderly men living in Iran (mean age: 60.94 years) were selected through simple sampling method. Participants completed a questionnaire regarding exercise attitude and performance and Beck self-image concept. Pearson correlation test was used for analysis of the data. Results: The results showed the significant correlation between optimism and exercise performance (p = 0.012) and exercise attitude (p = 0.005). Conclusion: Findings show that exercise performance and attitude are associated positively with optimism in elderly women. So, increasing exercise or improving attitude toward exercise can lead to improving optimism.Keywords: elderly, exercise performance and attitude, self-image, descriptive–correlational study

Abstract:
Distortion produced during welding of thin metal plates is a problem in many industries. The purpose of this research was to study distortion produced during welding in 2mm Mild Steel plate by simulating the welding process using Finite Element Analysis. Simulation of welding process requires a couple field transient analyses. At first a transient thermal analysis is performed and the temperature obtained from thermal analysis is used as input in structural analysis to find distortion. An actual weld sample is prepared and the weld distortion produced is measured. The simulated and actual results were in quite agreement with each other and it has been found that there is profound deflection at center of plate. Temperature dependent material properties play significant role in prediction of weld distortion. The results of this research can be used for prediction and control of weld distortion in large steel structures by changing different weld parameters.Keywords: welding simulation, FEA, welding distortion, temperature dependent mechanical properties

Abstract:
The research work focuses on exploring better technologies for solar energy storage. The research has the objective of substituting fossil fuels with renewable solar energy technology. This was the reason that motivated the research team to search for alternatives to develop an eco-friendly desalination process, which fully depends on the solar energy source. The Authors also investigated the potential of using different salt mixtures for better solar energy storage and better pure water productivity. Experiments were conducted to understand the impacts of solar energy collection and storage techniques on heat accumulation, heat storage capacity of various compositions of salt mixtures. Based on the experiments conducted, the economic and technical advantages of the integrated water desalination was assessed. Experiments also showed that the best salts with a higher storage efficiency of heat energy are NaCl, KNO3, and MgCl26H2O and polymers such as Poly Propylene and Poly Ethylene Terephthalate.Keywords: molten salts, desalination, solar energy storage and utilization, polymers

With the introduction of online social Q&A sites, people are able to reach each other efficiently for information seeking and simultaneously creating social bonds. There prevails an issue of low or no response for some questions posed by an information seeker on these sites. So this study tries to understand the effect of question type on responses in Social Q & A sites. The study found that among the answered queries, majority of them were answered within 24 hours of posting the questions and surprisingly most replies were received within one hour of posting. One of the parameters that affect the performance of microwave absorbers is the shape of the absorbers. This paper shows the performance (reflection loss) of truncated pyramidal and truncated wedge microwave absorbers in the range frequency between 8.2 to 12.4 GHz (X-Band) in simulation. The material used is sugarcane bagasse (SCB) which is one of the new materials that used to fabricate the microwave absorber. The complex permittivity was measured using Agilent dielectric probe technique. The designs were simulated using CST Microwave Studio Software. The aim of this study is to improve the quality of machined parts by introducing different cutting tools during finishing operations. As the cutting is performed in different directions, the surfaces presented on part can be classified into several categories. Therefore, suitable cutting tools are assigned to machine particular surfaces and to improve the quality. Experimental studies have been carried out by fabricating several parts based on the suggested approach. This paper presents the influence of cutting parameters (cutting speed, feed and depth of cut) on flank wear (VB) in turning of 52100 hard alloy steel using multilayer coated carbide insert under dry condition. Nine experiments were performed based on Taguchi’s L9 orthogonal array. Analysis of variance (ANOVA) was used to determine the effects of the cutting parameters on flank wear. The results of the study revealed that the cutting speed (A) and feed rate (B) are the dominant factors affecting flank wear, while the depth of cut (C) has not a significant effect. The optimal combination of the cutting parameters for flank wear is found to be A1B1C1. The mathematical model for flank wear is found to be statistically significant. In the current experiment, in vitro anticoccidial potential of Vitis venifera (grape seed) extract was evaluated. For this purpose, an in vitro sporulation inhibition assay was used. Collected oocysts of different Eimeria species of chicken were exposed to six different concentrations (w/v) of Vitis venifera extract (TAE) in 10% dimethylsulphoxide solution (DMSO). Dimethylsulphoxide (DMSO) and potassium dichromate solution (K₂Cr₂O₇) served as control groups. Results of the study revealed that Vitis venifera extract (TAE) showed an inhibitory effect on sporulation (%) and damage (%) of Eimeria oocysts in a dose-dependent manner as compared to both control groups. Vitis venifera extract also damaged the morphology of oocysts in terms of shape, size, and number of sporocysts.Keywords: Vitis venifera, in vitro, Eimeria, oocysts

Abstract:
The heat transfer coefficient can be improved passively by using a dimpled surface on the tube. The contact area where heat transfer takes place can be enlarged and turbulence will be purposefully produced inside the duct; as a consequence, higher heat transfer quality will be achieved by employing an extended inner or outer surface (dimpled surface). In order to compare the rate and quality of heat transfer between a regular-shaped pipe and a dimpled pipe, a dimpled tube with a fixed dimple radius was created. Numerical analysis of the plain and dimpled pipes was performed using ANSYS. A 23% increase in Nusselt number was seen for dimpled tubes compared to plain tubes. In comparison to plain tubes, dimpled tubes' increase in thermal performance index was found to be between 8% and 10%. This study has estimated the trade between USA and individual members of European Union using Gravity Model of Trade as The USA has a complex trade relationship with the European countries consist of a large number of consumers, which make USA dependent on EU for major of its total world trade. However, among the member of EU, the trade potential of USA with individual members of EU is not known. Panel data techniques e.g. Random Effect, Fixed Effect and Pooled Panel have been applied to secondary quantitative data to analyze the Trade between USA and EU. Trade Potential of USA with individual members of EU has been obtained using the ratio of Actual trade of USA with EU members and the trade as predicted by Gravity Model. The Study concluded that the USA has greater trade potential with 16 members of EU, including Croatia, Portugal and United Kingdom on top. An up-flow submerged biofilm reactor packed with sponge was investigated for sewage treatment. The reactor was operated two cycles as single aerobic (1-1 at 3.5 L/L.d HLR and 1-2 at 3.8 L/L.day HLR) and four cycles as single anaerobic/aerobic reactor; 2-1 and 2-2 at low HLR (3.7 and 3.5 L/L.day) and 2-3 and 2-4 at high HLR (5.1 and 5.4 L/L.day). During the aerobic cycles, 50% effluent recycling significantly reduces the system performance except for phosphorous. In case of the anaerobic/aerobic reactor, the effluent recycling, significantly improves system performance at low HLR while at high HLR only phosphorous removal was improved. Excess sludge production was limited to 0.133 g TSS/g COD with better sludge volume index (SVI) in case of anaerobic/aerobic cycles; (54.7 versus 58.5 ml/g).Keywords: aerobic, anaerobic/aerobic, up-flow, submerged biofilm, sponge

A simple, rapid and non-invasive electromagnetic sensor (C-FAST device) was- patented; for diagnosis of HCV RNA. Aim: To test the validity of the device compared to standard HCV PCR. Subjects and Methods: The first phase was done as pilot in Egypt on 79 participants; the second phase was done in five centers: one center from Egypt, two centers from Pakistan and two centers from India (800, 92 and 113 subjects respectively). The third phase was done nationally as multicenter study on (1600) participants for ensuring its representativeness. Results: When compared to PCR technique, C-FAST device revealed sensitivity 95% to 100%, specificity 95.5% to 100%, PPV 89.5% to 100%, NPV 95% to 100% and positive likelihood ratios 21.8% to 38.5%. Conclusion: It is practical evidence that HCV nucleotides emit electromagnetic signals that can be used for its identification. Abstract:
A 58 year old gentleman presented to accident and emergency at the district general hospital with worsening shortness of breath and a non-productive cough over a period of five days. He was initially admitted under the medical team for suspicion of SARS-CoV-2 (COVID-19) pneumonitis. Subsequently, upon deterioration of observations and a positive COVID-19 PCR, he was taken to intensive care for invasive mechanical ventilation. He required frequent proning, inotropic support and was intubated for thirty-three days. After successful extubation, he developed myopathy with a limited range of motion to his right knee and right shoulder. Plain film imaging of these limbs demonstrated an unusual formation of heterotopic ossification without any precipitating trauma or surgery. Current literature demonstrates limited case series portraying heterotopic ossification post-COVID-19. There has been negligible evidence of heterotopic ossification in the ipsilateral knee and shoulder post-prolonged immobility secondary to a critical illness. Physiotherapy and rehabilitation are post-intensive care can be prolonged due to the formation of heterotopic ossification around joints. Prolonged hospital stays may lead to a higher risk of developing infections of the chest, urine and pressure sores. This raises the question of whether a severe systemic inflammatory immune response from the SARS-CoV-2 virus results in histopathological processes leading to the formation of heterotopic ossification not previously seen, requiring prolonged physiotherapy.Keywords: orthopaedics, rehabilitation, physiotherapy, heterotopic ossification, COVID-19

Biometallic materials are the most important materials for use in biomedical applications especially in manufacturing a variety of biological artificial replacements in a modern worlds, e.g. hip, knee or shoulder joints, due to their advanced characteristics. Titanium (Ti) and its alloys are used extensively in biomedical applications based on their high specific strength and excellent corrosion resistance. Beta-Ti alloys containing completely biocompatible elements are exceptionally prospective materials for manufacturing of bioimplants. They have superior mechanical, chemical and electrochemical properties for use as biomaterials. These biomaterials have the ability to introduce the most important property of biochemical compatibility which is low elastic modulus. This review examines current information on the recent developments in alloying elements leading to improvements of beta Ti alloys for use as biomaterials. This paper focuses on the study of DC-to-DC converters, which are suitable for low-voltage high-power applications. The output voltages generated by renewable energy sources such as photovoltaic arrays and fuel cell stacks are generally low and required to be increased to high voltage levels. Development of DC-to-DC converters, which provide high step-up voltage conversion ratios with high efficiencies and low voltage stresses is one of the main issues in the development of renewable energy systems. A procedure for three converters-conventional DC-to-DC converter, interleaved boost converter, and isolated flyback based converter, is illustrated for a given set of specifications. Abstract:
Major trauma is a dynamic public health epidemic that is continuously evolving. Major trauma care services rely on multi-disciplinary team input involving highly trained pre and in-hospital critical care teams. Pre-hospital critical care teams (PHCCTs), major trauma centres (MTCs), trauma units, and rehabilitation facilities all form an efficient and organised trauma system. England comprises 27 MTCs funded by the National Health Service (NHS). Major trauma care entails enhanced resuscitation protocols coupled with the expertise of dedicated trauma teams and rapid radiological imaging to improve trauma outcomes. Literature reports a change in the demographic of major trauma as elderly patients (silver trauma) with injuries sustained from a fall of 2 metres or less commonly present to services. Evidence of an increasing population age with multiple comorbidities necessitates treatment within the first hour of injury (golden hour) to improve trauma survival outcomes. Staffing and funding pressures within the NHS have subsequently led to a shortfall of available physician-led PHCCTs. Thus, there is a strong emphasis on targeted research and funding to appropriately deploy resources to deprived areas. This review article will discuss the current English trauma system whilst critically appraising present challenges, identifying insufficiencies, and recommending aims for an improved future trauma system in England.Keywords: trauma, orthopaedics, major trauma, trauma system, trauma network

This study was carried out immediately before the introduction of PCV 10 to establish baseline pneumococcal carriage and prevalent serotypes in naso-pharynx of children 3-11 months of age in an urban and rural community in Sindh, Pakistan. An additional sample of children aged 12 to 59 months was drawn from the urban community. Nasopharyngeal specimens were collected from a random sample of children. Samples were processed in a central laboratory in Karachi. Pneumococci were cultured on 5% Sheep Blood Agar and serotyping was performed using CDC standardized sequential multiplex PCR assay on bacterial colonies. Serotypes were then categorized into vaccine (PCV-10 and PCV-13) type and non-vaccine types. A total of 670 children were enrolled. Carriage rate for pneumococcus based on culture positivity was 74% and 79.5 % in the infant group in Karachi and Matiari respectively. Carriage rate was 78.2% for children aged 12 to 59 months in Karachi. Proportion of PCV 10 serotypes in infants was 38.8% and 33.5% in Karachi and Matiari respectively. In the older age group in Karachi, the proportion was 30.6%. Most common serotypes were 6A, 6B, 23F, 19A and 18C. This survey establishes vaccine and non-vaccine serotype carriage rate in a vaccine-naïve pediatric population among rural and urban communities in Sindh province. Annually planned surveys in the same communities will inform change in carriage rate after the introduction and uptake of PCV 10 in these communities.Keywords: Naso-Pharyngeal carriage, Pakistan, PCV10, Pneumococcus

Abstract:
The wide use of titanium (Ti) materials in medicine gives impetus to a search for development new techniques with elevated properties such as strength, corrosion resistance and Young's modulus close to that of bone tissue. This article presents the most recent state of the art on the use of equal channel angular pressing (ECAP) technique in evolving mechanical characteristics of the ultrafine-grained bio-grade Ti materials. Over past few decades, research activities in this area have grown enormously and have produced interesting results, including achieving the combination of conflicting properties that are desirable for biomedical applications by severe plastic deformation (SPD) processing. A comprehensive review of the most recent work in this area is systematically presented. The challenges in processing ultrafine-grained Ti materials are identified and discussed. An overview of the biomedical Ti alloys processed with ECAP technique is given in this review, along with a summary of their effect on the important mechanical properties that can be achieved by SPD processing. Abstract:
Copper (Cu) is an essential micronutrient for normal plant growth and development, but in excess, it is also toxic to plants. The present study investigated the influence of ethylenediaminetetraacetic acid (EDTA) in enhancing Cu uptake and tolerance as well as the morphological and physiological responses of Brassica napus L. seedlings under Cu stress. Four-week-old seedlings were transferred to hydroponics containing Hoagland’s nutrient solution. After 2 weeks of transplanting, three levels (0, 50, and 100 μM) of Cu were applied with or without application of 2.5 mM EDTA and plants were further grown for 8 weeks in culture media. Results showed that Cu alone significantly decreased plant growth, biomass, photosynthetic pigments, and gas exchange characteristics. Cu stress also reduced the activities of antioxidants, such as superoxide dismutase (SOD), peroxidase (POD), ascorbate peroxidase (APX), and catalase (CAT) along with protein contents. Cu toxicity increased the concentration of reactive oxygen species (ROS) as indicated by the increased production of malondialdehyde (MDA) and hydrogen peroxide (H2O2) in both leaves and roots. The application of EDTA significantly alleviated Cu-induced toxic effects in B. napus, showing remarkable improvement in all these parameters. EDTA amendment increased the activity of antioxidant enzymes by decreasing the concentrations of MDA and H2O2 both in leaves and roots of B. napus. Although, EDTA amendment with Cu significantly increased Cu uptake in roots, stems, and leaves in decreasing order of concentration but increased the growth, photosynthetic parameters, and antioxidant enzymes. These results showed that the application of EDTA can be a useful strategy for phytoextraction of Cu by B. napus from contaminated soils.Keywords: antioxidants, biomass, copper, EDTA, phytoextraction, tolerance

Purpose of study: The main theme of this study is to scrutinize and evaluate the available research on “role of libraries and librarians during COVID-19”. Design/Methodology/Approach: The quantitative research tool Bibliometric was used for the analysis of collected data. All the retrieved articles have been analyzed through MS office, MS Excel. Findings: It was depicted that 48 papers were published in 2021. The researchers from international countries have been published 45 and Pakistani researchers published only3 research papers on the subject cited above. Delimitations of the study: This study is delimited to the research articles published on the role of libraries and librarians during COVID-19. Practical implications: The findings of this study will be guideline for policy makers, higher ups and authorities of various facets of the libraries to revisit their priorities and work for the betterment of effective library culture in the country. Contribution to the knowledge: To the best of author's knowledge, no significant study has been done on this important topic of Library and Information Science research in Pakistan. Abstract:
This paper illustrates the study of three isolated zero voltage switched (ZVS) PWM full bridge (FB) converters to charge the high voltage battery in the charger of electric vehicle (EV). EV battery chargers have several challenges such as high efficiency, high reliability, low cost, isolation, and high power density. The cost of magnetic and filter components in the battery charger is reduced when switching frequency is increased. The increase in the switching frequency increases switching losses. ZVS is used to reduce switching losses and to operate the converter in the battery charger at high frequency. The performance of each of the three converters is evaluated on the basis of ZVS range, dead times of the switches, conduction losses of switches, circulating current stress, circulating energy, duty cycle loss, and efficiency. The limitations and merits of each PWM FB converter are reviewed. Abstract:
The objective of the study is to forecast the US Dollar and Pak Rupee exchange rate by using time series models. For this purpose, daily exchange rates of US and Pakistan for the period of January 01, 2007 - June 2, 2017, are employed. The data set is divided into in sample and out of sample data set where in-sample data are used to estimate as well as forecast the models, whereas out-of-sample data set is exercised to forecast the exchange rate. The ADF test and PP test are used to make the time series stationary. To forecast the exchange rate ARIMA model and GARCH model are applied. Among the different Autoregressive Integrated Moving Average (ARIMA) models best model is selected on the basis of selection criteria. Due to the volatility clustering and ARCH effect the GARCH (1, 1) is also applied. Results of analysis showed that ARIMA (0, 1, 1 ) and GARCH (1, 1) are the most suitable models to forecast the future exchange rate. Further the GARCH (1,1) model provided the volatility with non-constant conditional variance in the exchange rate with good forecasting performance. This study is very useful for researchers, policymakers, and businesses for making decisions through accurate and timely forecasting of the exchange rate and helps them in devising their policies.Keywords: exchange rate, ARIMA, GARCH, PAK/USD

Abstract:
The present study aims to investigate the effect of reflective teaching practices on prospective teachers’ performance. Reflective teaching practice helps teachers to plan, implement and improve their performance by rethinking about their strengths and weaknesses. An action research was conducted by the researcher. All prospective teachers of sixth semester in a women university’s teacher education program were the population of the study. From 40 students, 20 students were taken as experimental group, and the rest of 20 students were taken as control group. During the action research a cyclic process of producing a module, training teachers for the reflective practices and then observing them during their class for reflective practice was done by the researchers. The research used a set of rubrics and checklists for assessing prospective teachers’ performance during their class. Finally, the module was modified with the help of findings. It was found that the training has improved the performance of teachers as they revised and modified their teaching strategies through reflective practice. However, they were not able to train their students for reflective practice as per expectation. The study has implications for teacher training programs to include reflective practice modules as part of their course work for making them better teachers.Keywords: reflective practices, prospective teacher, effect, performance
