78 Link Between Intensity-trajectories Of Acute Postoperative Pain And Risk Of Chronicization After Breast And Thoracopulmonary Surgery

Abstract:
Introduction: The risk factors for the chronicization of postoperative pain are numerous and often intricately intertwined. Among these, the severity of acute postoperative pain is currently recognized as one of the most determining factors. Mastectomy and thoracotomy are described as among the most painful surgeries and the most likely to lead to chronic post-surgical pain (CPSP). Objective: To examine the aspects of acute postoperative pain potentially involved in the development of chronic pain following breast and thoracic surgery. Patients and Methods: A prospective study involving 164 patients was conducted over a six-month period. Postoperative pain (during mobilization) was assessed using a Visual Analog Scale (VAS) at various time points after surgery: Day 0, 1st, 2nd, 5th days, 1st and 6th months. Moderate to severe pain was defined as a VAS score ≥ 4. A comparative analysis (univariate analysis) of postoperative pain intensities at different evaluation phases was performed on patients with and without CPSP to identify potential associations with the risk of chronicization six months after surgery. Results: At the 6th month post-surgery, the incidence of CPSP was 43.0%. Moderate to severe acute postoperative pain (in the first five days) was observed in 64% of patients. The highest pain scores were reported among thoracic surgery patients. Comparative measures revealed a highly significant association between the presence of moderate to severe acute pain, especially lasting for ≥ 48 hours, and the occurrence of CPSP (p-value <0.0001). Likewise, the persistence of subacute pain (up to 4 to 6 weeks after surgery), especially of moderate to severe intensity, was significantly associated with the risk of chronicization at six months (p-value <0.0001). Conclusion: CPSP after breast and thoracic surgery remains a fairly common morbidity that profoundly affects the quality of life. Severe acute postoperative pain, especially if it is prolonged and/or with a slow decline in intensity, can be an important predictive factor for the risk of chronicization. Therefore, more effective and intensive management of acute postoperative pain, as well as longitudinal monitoring of its trajectory over time, should be an essential component of strategies for preventing chronic pain after surgery.Keywords: chronic post-surgical pain, acute postoperative pain, breast and thoracic surgery, subacute postoperative pain, pain trajectory, predictive factor

Introduction: Post-surgical chronic pain (PSCP) is a pathological condition with a rather complex etiopathogenesis that extensively involves sensitization processes and neuronal damage. The neuropathic component of these pains is almost always present, with variable expression depending on the type of surgery. Objective: To assess the presumed beneficial effect of Regional Anesthesia-Analgesia Techniques (RAAT) on the development of post-surgical chronic neuropathic pain (PSCNP) in various surgical procedures. Patients and Methods: A comparative study involving 510 patients distributed across five surgical models (mastectomy, thoracotomy, hernioplasty, cholecystectomy, and major abdominal-pelvic surgery) and randomized into two groups: Group A (240) receiving conventional postoperative analgesia and Group B (270) receiving balanced analgesia, including the implementation of a Regional Anesthesia-Analgesia Technique (RAAT). These patients were longitudinally followed over a 6-month period, with postsurgical chronic neuropathic pain (PSCNP) defined by a Neuropathic Pain Score DN2≥ 3. Comparative measurements through univariate and multivariable analyses were performed to identify associations between the development of PSCNP and certain predictive factors, including the presumed preventive impact (protective effect) of RAAT. Results: At the 6th month post-surgery, 419 patients were analyzed (Group A= 196 and Group B= 223). The incidence of PSCNP was 32.2% (n=135). Among these patients with chronic pain, the prevalence of neuropathic pain was 37.8% (95% CI: [29.6; 46.5]), with n=51/135. It was significantly lower in Group B compared to Group A, with respective percentages of 31.4% vs. 48.8% (p-value = 0.035). The most significant differences were observed in breast and thoracopulmonary surgeries. In a multiple regression analysis, two predictors of PSCNP were identified: the presence of preoperative pain at the surgical site as a risk factor (OR: 3.198; 95% CI [1.326; 7.714]) and RAAT as a protective factor (OR: 0.408; 95% CI [0.173; 0.961]). Conclusion: The neuropathic component of PSCNP can be observed in different types of surgeries. This study employs a multidisciplinary approach to investigate the intricate processes governing biofilm-surface interactions. Results indicate that surface properties significantly influence initial microbial attachment, with materials characterized by increased roughness and hydrophobicity promoting enhanced biofilm adhesion. Moreover, the chemical composition of materials plays a crucial role in impacting the development of biofilms. Environmental factors, such as temperature fluctuations and nutrient availability, were identified as key determinants affecting biofilm formation dynamics. Advanced imaging techniques revealed complex three-dimensional biofilm structures, emphasizing microbial communication and cooperation within these networks. There is an increasing interest in phytochemicals as new source of natural antioxidant and antimicrobial agents. Plants essential oils have come more into the focus of phytomedicine. Many researchers have reported various biological and/or pharmacological properties of Artemisia herba alba Asso essential oil. The present study describes antimicrobial and antioxidant properties of Artemisia herba alba Asso essential oil. Artemisia herba alba Asso essential oil obtained by hydrodistillation (using Clevenger type apparatus) growing in Algeria (M’sila) was analyzed by GC-MS. The essential oil yield of the study was 0.7%. The major components were found to be camphor, chrysanthenone et 1,8-cineole. The antimicrobial activity of the essential oil was tested against four bacteria (Gram-negative and Gram-positive) and three fungi using the diffusion method and by determining the inhibition zone. The oil was found to have significant antibacterial activity. In addition, antioxidant activity was determined by 1, 1-diphenyl-1-picrylhydrazyl (DPPH) assay, ferric reducing (FRAP) assay and β-carotene bleaching test, and high activity was found for Artemisia herba-alba oil.Keywords: Artemisia herba-alba, essential oil, antibacterial activity, antioxidant activity

The market of Wireless Sensor Network WSN has a great potential and development opportunities. Researchers are focusing on optimization in many fields like efficient deployment and routing protocols. In this article, we will concentrate on energy efficiency for WSN because WSN nodes are habitually deployed in severe No Man’s Land with batteries are not rechargeable, so reducing energy consumption represents an important challenge to extend the life of the network. We will present the design of new WSN mote based on ultra low power STM32L microcontrollers and the ZIGBEE transceiver CC2520. In the last few years, due to the misuse of antibiotics and an increasing incidence of immunodeficiency-related diseases, the development of microbial drug resistance has become more and more of a pressing problem. Recently, natural products from medicinal plants represent a fertile ground for the development of novel antibacterial agents. Plants essential oils have come more into the focus of phytomedicine. The present study describes antimicrobial activity of Rosmarinus officinalis L. essential oil known medicinally for its powerful antibacterial properties. The essential oil of rosemary obtained by hydrodistillation (using Clevenger type apparatus) growing in Algeria (Djelfa city of south Algeria) was investigated by GC-MS. The essential oil yield of the study was 1.4 %. The major components were found to be camphor, camphene, 1,8-cineole. The essential oil has been tested for antimicrobial activity against eight bacteria (Gram-negative and Gram-positive), and three fungi including Candida albicans. Inhibition of growth was tested by the agar diffusion method based on the determination of the diameter of inhibition. The oil was found to have significant antibacterial activity and therefore can be used as a natural antimicrobial agent for the treatment of several infectious diseases caused by those germs, which have developed resistance to antibiotics.Keywords: antimicrobial activity, Rosmarinus officinalis L., essential oils, GC/MS, camphor

This paper presents a method for finding the optimal location and capacity of dispatchable DGs connected to the distribution feeders for optimal planning for a specified power loss without violating the system practical constraints. The distributed generation units in the proposed algorithm is modeled as voltage controlled node with the flexibility to be converted to constant power node in case of reactive power limit violation. The proposed algorithm is implemented in MATLAB and tested on the IEEE 37-nodes feeder. For a long time proportional hazards Cox regression (ph. Cox) was and still the most well-known statistical method to predict survival outcome. But due to the revolution of data sciences; new predication models were employed and proved to be more flexible and provided higher accuracy in that type of studies. Artificial neural network is one of those models that is suitable to handle time to event predication. Rhamnus alaternus L. is a shrub that belongs to the family of Rhamnaceae. It is a medicinal plant that is largely used in traditional medicine in Algeria. Five flavonoic extracts obtained of Rhamnus alaternus L. leaves. The flavonoids were evaluated by a method that uses aluminum chloride AlCl3 of each extract; the content is estimated at 19.33 (Hexanic. Extract), 18.42 (Chlroformic.extract), 16.75 (Acetate. Extract), 3.9 (Brute. Extract), and 3.02 (Aqueous. Extract) mg Equivalent quercetine/gram of extract (mg QE/ g extract). The construction of a new airport or the extension of an existing one requires massive investments and many times public private partnerships were considered in order to make feasible such projects. One characteristic of these projects is uncertainty with respect to financial and environmental impacts on the medium to long term. Another one is the multistage nature of these types of projects. While many airport development projects have been a success, some others have turned into a nightmare for their promoters. This communication puts forward a new approach for airport investment risk assessment. The approach takes explicitly into account the degree of uncertainty in activity levels prediction and proposes milestones for the different stages of the project for minimizing risk. Uncertainty is represented through fuzzy dual theory and risk management is performed using dynamic programming. In Jordan having deficit atmospheric precipitation, an increase in water demand during summer months . Jordan can be regarded with a relatively high potential for waste water recycling and reuse. The main purpose of this paper was to investigate the removal of Total suspended solids (TSS) and chemical oxygen demand (COD) for olive mill waste water (OMW) by the electrocoagulation (EC) process. In the combination of electrocoagulation by using coupled iron–aluminum electrodes the optimum working pH was found to be in range 6. The efficiency of the electrocoagulation process allowed removal of TSS and COD about 82.5% and 47.5% respectively at 45 mA/cm2 after 70 minutes by using coupled iron–aluminum electrodes. Abstract:
Advances in concrete technology and implementation of new materials made it possible to produce special types of concrete for different structural applications. In this research, granite and galena were incorporated in different concrete mixes to obtain high performance concrete for shielding against gamma radiations in nuclear facilities. Chemically prepared industrial galena was used to replace different volume fractions of the fine aggregate. The test specimens were exposed to different conditions of heating cycles and irradiation. The exposed specimens and counterpart unexposed specimens were tested to evaluate the density, the compressive strength and the attenuation coefficient. The proposed mixes incorporating galena showed better performance in terms of compressive strength and gamma attenuation capacity, especially after the exposure to different heating cycles.Keywords: concrete, galena, shielding, attenuation, radiation

Abstract:
Figs are so much consumed in the Mediterranean region; they present a high nutritional value and also multiple therapeutic virtues. Our work contributes to the study of the antidiabetic activity of the common fig of the region of El Amra (AinDefla) Algeria. To do this, 20 Wistar rats female, divided into 4 lots, were used: Lot 1: 5 normal controls; Lot 2: 5 normal controls treated with dry fig juice at 20%; Lot 3: 5 diabetic controls; Lot 4: 5 diabetic controls treated with dry fig juice at 20%. The rats are rendered diabetic by intra-peritoneal injection of a streptozotocin solution. The blood glucose is measured after 1 hour, 2 hours, 3 hours and after 4 hours of the administration of the fig juice; it’s measured also on the 5th day, 8th day and 9th day of the beginning of the experiment. The determination of cholesterol and triglycerides blood is carried out at the beginning and the end of the study. On the 9th day, we recorded a very significant decrease of the blood sugar level of diabetic rats treated with dry fig juice. This blood glucose level normalized for 3 rats/5rats, we also recorded a decrease, but not significant, of cholesterol and triglycerides blood levels. In the short term (for 4 hours), an increase of blood sugar level, one hour after administration, for normal and diabetic rats. This increase is probably due to the high level of sugar content in the preparation. The blood glucose level is then corrected, four hours later. This may be the result of anti hyperglycemic effect of the active ingredients contained in the figs.Keywords: antidiabetic, figs, hypoglycemia, streptozotocin

Many researches have concentrated on improving the aerodynamic performance of wind turbine blade through testing and theoretical studies. A small wind turbine blade is designed, fabricated and tested. The power performance of small horizontal axis wind turbines is simulated in details using Computational Fluid Dynamic (CFD). The three-dimensional CFD models are presented using ANSYS-CFX v13 software for predicting the performance of a small horizontal axis wind turbine. The simulation results are compared with the experimental data measured from a small wind turbine model, which designed according to a vehicle-based test system. The analysis of wake effect and aerodynamic of the blade can be carried out when the rotational effect was simulated. Finally, comparison between experimental, numerical and analytical performance has been done. Data mining has, over recent years, seen big advances because of the spread of internet, which generates everyday a tremendous volume of data, and also the immense advances in technologies which facilitate the analysis of these data. In particular, classification techniques are a subdomain of Data Mining which determines in which group each data instance is related within a given dataset. It is used to classify data into different classes according to desired criteria. Generally, a classification technique is either statistical or machine learning. Each type of these techniques has its own limits. Nowadays, current data are becoming increasingly heterogeneous; consequently, current classification techniques are encountering many difficulties. This paper defines new measure functions to quantify the resemblance between instances and then combines them in a new approach which is different from actual algorithms by its reliability computations. Designing a controller for stochastic decentralized interconnected large scale systems usually involves a high degree of complexity and computation ability. Noise, observability, and controllability of all system states, connectivity, and channel bandwidth are other constraints to design procedures for distributed large scale systems. The quasi-steady state model investigated in this paper is a reduced order model of the original system using singular perturbation techniques. This paper results in an optimal control synthesis to design an observer based feedback controller by standard stochastic control theory techniques using Linear Quadratic Gaussian (LQG) approach and Kalman filter design with less complexity and computation requirements. Abstract:
This paper investigates the dynamic relationship between gold and stock markets using data for BRICS counties. For this purpose, we estimate three multivariate GARCH models (namely CCC, DCC and BEKK) for weekly stock and gold data. Our main objective is to examine time variations in conditional correlations between the two assets and to check the effectiveness use of gold as a hedge for equity markets. Empirical results reveal that dynamic conditional correlations switch between positive and negative values over the period under study. This correlation is negative during the major financial crises suggesting that gold can act as a safe haven during the major stress period of stock markets. We also evaluate the implications for portfolio diversification and hedging effectiveness for the pair gold/stock. Our findings suggest that adding gold in the stock portfolio enhance its risk-adjusted return.Keywords: gold, financial markets, hedge, multivariate GARCH

Abstract:
Elasticity is the essential property of cloud computing. As the name suggests, it constitutes the ability of a cloud system to adjust resource provisioning in relation to fluctuating workload. There are two types of elasticity operations, vertical and horizontal. In this work, we are interested in horizontal scaling, which is ensured by two mechanisms; scaling in and scaling out. Following the sizing of the system, we can adopt scaling in in the event of over-supply and scaling out in the event of under-supply. In this paper, we propose a formal model, based on colored and temporized Petri nets, for the modeling of the duplication and the removal of a virtual machine from a server. This model is based on formal Petri Nets modeling language. This paper examines the dependence structure of Islamic stock markets on Bitcoin’s realized volatility components in bear, normal, and bull market periods. A quantile regression approach is employed, after adjusting raw returns with respect to a broad set of relevant global factors and accounting for structural breaks in the data. The results reveal that upside volatility tends to exert negative influences on Islamic developed-market returns more in bear than in bull market conditions, while downside volatility positively affects returns during bear and bull conditions. For emerging markets, we find that the upside (downside) component exerts lagged negative (positive) effects on returns in bear (all) market regimes. By and large, the dependence structures turn out to be asymmetric. This research presents a study to handle the recruitment services system. It aims to enhance a business intelligence system by embedding data mining in its core engine and to facilitate the link between job searchers and recruiters companies. The purpose of this study is to present an intelligent management system for supporting recruitment services based on data mining methods. It consists to apply segmentation on the extracted job postings offered by the different recruiters. The details of the job postings are associated to a set of relevant features that are extracted from the web and which are based on critical criterion in order to define consistent clusters. Thereafter, we assign the job searchers to the best cluster while providing a ranking according to the job postings of the selected cluster. In the frame of this work, we present an optical multicasting approach based on optical code-words. Our approach associates, in the edge node, an optical code-word to a group multicast address. In the core node, a set of tunable decoders are used to send a traffic data to multiple destinations based on the received code-word. The use of code-words, which correspond to the combination of an input port and a set of output ports, allows the implementation of an optical switching matrix. At the reception of a burst, it will be delayed in an optical memory. And, the received optical code-word is split to a set of tunable optical decoders. Fiber reinforced polymer (FRP) installation is a very effective way to repair and strengthen structures that have become structurally weak over their life span. This technique attracted the concerning of researchers during the last two decades. This paper presents a simple uniaxial nonlinear finite element model (UNFEM) able to accurately estimate the load-carrying capacity, different failure modes and the interfacial stresses of reinforced concrete (RC) continuous beams flexurally strengthened with externally bonded FRP plates on the upper and lower fibers. Results of the proposed finite element (FE) model are verified by comparing them with experimental measurements available in the literature. The agreement between numerical and experimental results is very good. Considering fracture energy of adhesive is necessary to get a realistic load carrying capacity of continuous RC beams strengthened with FRP. This research delves into the intricate dynamics of biofilm adhesion on surfaces, particularly focusing on the widely employed X52 surface in oil and gas industry pipelines. Biofilms, characterized by microorganisms within a self-produced matrix, pose significant challenges due to their detrimental impact on surfaces. Our study integrates advanced molecular techniques and cutting-edge microscopy, such as scanning electron microscopy (SEM), to identify microbial communities and visually assess biofilm adhesion. Simultaneously, we concentrate on the X52 surface, utilizing impedance spectroscopy and potentiodynamic polarization to gather electrochemical responses under various conditions. In conjunction with the broader investigation, we propose a novel approach to mitigate biofilm-induced corrosion challenges. This involves environmentally friendly inhibitors derived from plants, offering a sustainable alternative to conventional chemical treatments. Our inquiry screens and selects inhibitors based on their efficacy in hindering biofilm formation and reducing corrosion rates on the X52 surface. This study contributes valuable insights into the interplay between electrochemical processes and biofilm attachment on the X52 surface. Furthermore, the outcomes of this research have broader implications for the oil and gas industry, where biofilm-related corrosion is a persistent concern. The exploration of eco-friendly inhibitors not only holds promise for corrosion control but also aligns with environmental considerations and sustainability goals. As global demand for natural gas intensifies, Algeria is increasing its production to meet this rising need, placing significant strain on the nation's extensive pipeline infrastructure. Sonatrach, Algeria's national oil and gas company, faces persistent challenges from metal corrosion, particularly microbiologically influenced corrosion (MIC), leading to substantial economic losses. This study investigates the corrosion-inhibiting properties of Calotropis procera extracts, known as karanka, as a sustainable alternative to conventional inhibitors, which often pose environmental risks. The Calotropis procera extracts were evaluated for their efficacy on carbon steel API 5L X52 through electrochemical techniques, including potentiodynamic polarization and electrochemical impedance spectroscopy (EIS), under simulated operational conditions at varying concentrations, particularly at 10%, and elevated temperatures up to 60°C. The results demonstrated remarkable inhibition efficiency, achieving 96.73% at 60°C, attributed to the formation of a stable protective film on the metal surface that suppressed anodic and cathodic corrosion reactions. Scanning electron microscopy (SEM) confirmed the stability and adherence of these protective films, while EIS analysis indicated a significant increase in charge transfer resistance, highlighting the extract's effectiveness in enhancing corrosion resistance. The abundant availability of Calotropis procera in Algeria and its low-cost extraction processes present a promising opportunity for sustainable biocorrosion management strategies in the oil and gas industry, reinforcing the potential of plant-based extracts as viable alternatives to synthetic inhibitors for environmentally friendly corrosion control.Keywords: corrosion inhibition, calotropis procera, microbiologically influenced corrosion, eco-friendly inhibitor

The development of sensor technology (such as microelectromechanical systems (MEMS), wireless communications, embedded systems, distributed processing and wireless sensor applications) has contributed to a broad range of WSN applications which are capable of collecting a large amount of spatiotemporal data in real time. These systems require real-time data processing to manage storage in real time and query the data they process. In order to cover these needs, we propose in this paper a Snapshot spatiotemporal data model based on object-oriented concepts. This model allows saving storing and reducing data redundancy which makes it easier to execute spatiotemporal queries and save analyzes time. Further, to ensure the robustness of the system as well as the elimination of congestion from the main access memory we propose a spatiotemporal indexing technique in RAM called Captree *.
