Authors: Novutry Siregar, Afdal, Emilzon Taslim
Abstract:
Hyaline Membrane Disease (HMD) is the main cause of respiratory failure in preterm neonates caused by surfactant deficiency. Nasal Continuous Positive Airway Pressure (NCPAP) is the therapy for HMD. The success of therapy is determined by gestational age, birth weight, HMD grade, time of NCAP administration, and time of breathing frequency recovery. The aim of this research is to identify the predictive factor of NCPAP therapy success in preterm neonates with HMD. This study used a cross-sectional design by using medical records of patients who were treated in the Perinatology of the Pediatric Department of Dr. M. Djamil Padang Central Hospital from January 2015 to December 2017. The samples were eighty-two neonates that were selected by using the total sampling technique. Data analysis was done by using the Chi-Square Test and the Multiple Logistic Regression Prediction Model. The results showed the success rate of NCPAP therapy reached 53.7%. Birth weight (p = 0.048, OR = 3.34 95% CI 1.01-11.07), HMD grade I (p = 0.018, OR = 4.95 CI 95% 1.31-18.68), HMD grade II (p = 0.044, OR = 5.52 95% CI 1.04-29.15), and time of breathing frequency recovery (p = 0,000, OR = 13.50 95% CI 3.58-50, 83) are the predictive factors of NCPAP therapy success in preterm neonates with HMD. The most significant predictive factor is the time of breathing frequency recovery. Keywords: predictive factors, the success of therapy, NCPAP, preterm neonates, HMD

Authors: Sania Sana, Afnan Nasim, Usman Malik, Maroof Tahir
Abstract:
The aim of this research is to scrutinize the interdependent relationship between terrorism and behavioral changes in the tourism activities in Pakistan with the moderating impact of a unique variable titled 'Financial Risk Tolerance'. The article looks at the inter-reliant relationship with the alleged political and economic aspects and behavioral changes in the tourists and the consumers by these variables over time. The researchers used many underlying theories like the catastrophe theory by (Svyantek, Deshon and Siler 1991), information integration theory (Anderson 1981, 1982) and prospect theory (Kahneman and Tversky 1979) to shape the study’s framework as per tourist decision making model. A sample of around 110 locals was used for this purpose and the data was gathered by convenience sampling. The responses were analyzed using regression analysis. The results exhibited how terrorism along with the influence of financial risk tolerance had inclined a behavioral shift in the travelling patterns and vacation destination choice of the local tourists. Lastly, the paper proposes a number of suggestive measures for the tourism industry and the legislative bodies to ensure the safety of travelers and to boost the tourist activities in the vacation industry of Pakistan. Keywords: terrorism, tourism, financial risk tolerance, tourist decision-making, destination choice
Procedia PDF Downloads 2361721 Catastrophic Health Expenditures: Evaluating the Effectiveness of Nepal's National Health Insurance Program Using Propensity Score Matching and Doubly Robust Methodology
Authors: Simrin Kafle, Ulrika Enemark
Abstract:
Catastrophic health expenditure (CHE) is a critical issue in low- and middle-income countries like Nepal, exacerbating financial hardship among vulnerable households. This study assesses the effectiveness of Nepal’s National Health Insurance Program (NHIP), launched in 2015, to reduce out-of-pocket (OOP) healthcare costs and mitigate CHE. Conducted in Pokhara Metropolitan City, the study used an analytical cross-sectional design, sampling 1276 households through a two-stage random sampling method. Data was collected via face-to-face interviews between May and October 2023. The analysis was conducted using SPSS version 29, incorporating propensity score matching to minimize biases and create comparable groups of enrolled and non-enrolled households in the NHIP. PSM helped reduce confounding effects by matching households with similar baseline characteristics. Additionally, a doubly robust methodology was employed, combining propensity score adjustment with regression modeling to enhance the reliability of the results. This comprehensive approach ensured a more accurate estimation of the impact of NHIP enrollment on CHE. Among the 1276 samples, 534 households (41.8%) were enrolled in NHIP. Of them, 84.3% of households renewed their insurance card, though some cited long waiting times, lack of medications, and complex procedures as barriers to renewal. Approximately 57.3% of households reported known diseases before enrollment, with 49.8% attending routine health check-ups in the past year. The primary motivation for enrollment was encouragement from insurance employees (50.2%). The data indicates that 12.5% of enrolled households experienced CHE versus 7.5% among non-enrolled. Enrollment into NHIP does not contribute to lower CHE (AOR: 1.98, 95% CI: 1.21-3.24). Key factors associated with increased CHE risk were presence of non-communicable diseases (NCDs) (AOR: 3.94, 95% CI: 2.10-7.39), acute illnesses/injuries (AOR: 6.70, 95% CI: 3.97-11.30), larger household size (AOR: 3.09, 95% CI: 1.81-5.28), and households below the poverty line (AOR: 5.82, 95% CI: 3.05-11.09). Other factors such as gender, education level, caste/ethnicity, presence of elderly members, and under-five children also showed varying associations with CHE, though not all were statistically significant. The study concludes that enrollment in the NHIP does not significantly reduce the risk of CHE. The reason for this could be inadequate coverage, where high-cost medicines, treatments, and transportation costs are not fully included in the insurance package, leading to significant out-of-pocket expenses. We also considered the long waiting time, lack of medicines, and complex procedures for the utilization of NHIP benefits, which might result in the underuse of covered services. Finally, gaps in enrollment and retention might leave certain households vulnerable to CHE despite the existence of NHIP. Key factors contributing to increased CHE include NCDs, acute illnesses, larger household sizes, and poverty. To improve the program’s effectiveness, it is recommended that NHIP benefits and coverage be expanded to better protect against high healthcare costs. Additionally, simplifying the renewal process, addressing long waiting times, and enhancing the availability of services could improve member satisfaction and retention. Targeted financial protection measures should be implemented for high-risk groups, and efforts should be made to increase awareness and encourage routine health check-ups to prevent severe health issues that contribute to CHE.Keywords: catastrophic health expenditure, effectiveness, national health insurance program, Nepal

Authors: Souhila Terfi, Fatma Hassaine-Sadi
Abstract:
In the present study, wet digestion with HCl and HNO3 mixture was used to extract the heavy metals (copper (Cu), chromium (Cr), zinc (Zn), lead (Pb) and cadmium (Cd)) from the leaves, the stems and the roots of Malva sylvestris L., which were subsequently analyzed by AAS. The samples (soil and parts of species) were collected from different sites: the industrial area (IA) (Rouiba), the rubbish dump area (RDA) (Boudouaou), the residential area (RA) with large open fields and construction activities (Blida), the Montaigne area (MA) (Chrea) and the high plateau area (HPA) (Berouaguia). The study showed differences in metal concentrations according to the analysed parts and the different sampling locations. In the contaminated site of the industrial area (IA), high content of the toxic heavy metals (Cd: 3.18 µg/g DW and Pb: 34.48 µg/g DW) were found in the leaves of Malva sylvestris L. This finding suggests that the consumers of this species could be exposed to a risk associated with this higher level of these toxic metals. It was found that Malva sylvestris L. is rich by Zn and Cu in some sites, which are considered to be the essential elements for the human health. The obtained results with the control site (Montaigne area) suggest that this species can be applicable in both the health and food, feasible alternatives as medicinal plant without any risk. Keywords: Malva sylvestris L., toxic heavy metal, medicinal plant, impact on human health

Procedia PDF Downloads 491711 Teaching English to Learners with Special Education Needs: An Interpretive Phenomenology
Authors: Merry Pearl Rupinta, Rivika Alda
Abstract:
Inclusive education provides the same opportunity for all students regardless of the difficulties they encounter. Teaching English to learners with special education needs (LSENs) is a significant factor for learners to cope with the academic demands in the regular class. However, these special education (SPED) teachers lack specialized training to teach specific skills in English. Using purposive sampling, this study explored the lived experience of nine SPED teachers handling English to learners with intellectual disabilities from the four divisions of Region 10. An interview schedule, validated by experts, was used in conducting the investigation, where a semi-structured one-on-one interview was employed. The data were analyzed using Smith's (2009) five-step data analysis. The study highlighted themes on challenges in teaching English, coping strategies, adjusted classroom management, and inclusivity. The results suggest that SPED teachers' experience teaching English is a continuous and indefinite loop that continues until the primary goal of mainstreaming LSENs into the regular class is achieved. This study recommends that policymakers and curriculum developers focus their efforts on professional development through a series of training on various strategies and approaches in teaching English that can help SPED teachers provide a more effective and efficient teaching-learning experience. Keywords: English language teaching, inclusive education, intellectual disability, special education

Authors: Olanike O. Akinwale, Isiaka S. Adedoyin
Abstract:
Revenue is a fund realized by the government to meet both current and capital expenditures. The study found out the various ways through which local governments in Nigeria generate revenue or obtain funds and determined whether the people of Boripe local government are paying tax as at when due and also evaluated how the revenue generated is being used by the local government. During the course of this study, research questionnaires were drafted and distributed to respondents in the local government secretariat who supplied the information needed to carry out the research work. Data were collected by using simple random sampling technique where members of the population have been given equal chance of being picked as a member of the sample. Data were analysed using chart table; the chart analyzed the figure of the past two years revenue and expenditure of the local government. It was deduced from the result that revenue generated but this was not up to what one expected for this local government to finance the projected expenditure when the size was considered, its location as well as its natural endowment of this local government. This was due to lack of cooperation of the people and staffs within the local government in the local government jurisdiction as well as fraudulent activities the revenue collectors engaged in. Revenue generation is a fuel for development in any organization whether public or private. The ability of revenue drive of Boripe was not strong enough since the targeted revenue from taxation was not enough to meet the projected expenditure for a particular year as in 2016, the difference was carried forward to the next year. Keywords: appraisal, expenditure, local government, questionnaire, revenue

Authors: Yongmin You
Abstract:
Electric machines for office automation device such as printer and scanner have been required the low noise and vibration performance. Many researches about the low noise and vibration of polygon mirror scanner motor have been also progressed. The noise and vibration of polygon mirror scanner motor can be classified by aerodynamic, structural and electromagnetic. Electromagnetic noise and vibration can be occurred by high cogging torque and nonsinusoidal back EMF. To improve the cogging torque and back EMF characteristic, we apply unequal air-gap. To analyze characteristic of a polygon mirror scanner motor, two dimensional finite element method is used. To minimize the cogging torque of a polygon mirror motor, Kriging based on latin hypercube sampling (LHS) is utilized. Compared to the initial model, the torque ripple of the optimized unequal air-gap model was reduced by 23.4 % while maintaining the back EMF and average torque. To verify the optimal design results, the experiment was performed. We measured the vibration in motors at 23,600 rpm which is the rated velocity. The radial and axial gravitational acceleration of the optimal model were declined more than seven times and three times, respectively. From these results, a shape optimized unequal polygon mirror scanner motor has shown the usefulness of an improvement in the torque ripple and electromagnetic vibration characteristic. Keywords: polygon mirror scanner motor, optimal design, finite element method, vibration

Authors: H. K. Bonsu-Owu
Abstract:
Media portrayal of women in traditional stereotypical roles such as mothers, or seductress has been the norm for years. However, the changing socioeconomic and political environment and advancement of women in today’s society have given rise to questions on the appropriate portrayal of women in the media today. The purpose of the study is to analyze the portrayal of women in Ghanaian newspapers and find women’s perception on the issue. The study uses a three step approach in gathering data for analysis. Using the stratified sampling method, it analyzes front page images of women from 210 issues of the selected newspapers. Further, it administers questionnaires to 100 female students to find out how they relate to the images of women in the selected newspapers. Finally, editors of the newspapers are interviewed to find their rational for portraying women as seen on their front pages. The findings suggest that the newspapers portray women for varied reasons such as promoting sales and influencing the public agenda. Further, the female students claim that in spite of women’s vast contribution to the growth of society, the media continue to marginalize them. They add that such portrayals promote and reinforce social construct, however, refuse to see themselves through the male gaze concept. The study concludes that the stereotyped portrayal of women is likely to continue if the government, regulatory bodies, the media and society do not make a conscious effort to address this problem. Keywords: women, newspaper, portrayal, social construct

Authors: Brian H. Marsh
Abstract:
Nitrogen fertility is an important component for optimum potato yield and quality. Best management practices are necessary in regards to N applications to achieve these goals without applying excess N with may contribute to ground water contamination. Eight potato fields in the Southern San Joaquin Valley were sampled for nitrogen inputs and uptake, tuber and vine dry matter and residual soil nitrate-N. The fields had substantial soil nitrate-N prior to the potato crop. Nitrogen fertilizer was applied prior to planting and in irrigation water as needed based on in-season petiole sampling in accordance with published recommendations. Average total nitrogen uptake was 237 kg ha-1 on 63.5 Mg ha-1 tuber yield and nitrogen use efficiency was very good at 81 percent. Sixty-nine percent of the plant nitrogen was removed in tubers. Soil nitrate-N increased 14 percent from pre-plant to post-harvest averaged across all fields and was generally situated in the upper soil profile. Irrigation timing and amount applied did not move water into the lower profile except for a single location where nitrate also moved into the lower soil profile. Pre-plant soil analysis is important information to be used. Rotation crops having deeper rooting growth would be able to utilize nitrogen that remained in the soil profile. Keywords: potato, nitrogen fertilization, irrigation management, leaching potential

The aim of the present study was to assess the effect of social support and self-regulation, as external and internal factors, on changes in exercise behavior among infertile women. Methods: For a cross-sectional study conducted in Sanandaj (Iran) in 2020, we recruited infertile women (n=483) from 35 comprehensive healthcare centers by means of convenience sampling. Standardized face-to-face interviews were conducted using established and reliable instruments for the assessment of EB, social support, and self-regulation. Logistic regression models were applied to assess the association between EB, social support and self-regulation. Results: The majority of the participants (56.7%) had secondary infertility, while 70.8% of them did not perform any exercise. Self-regulation and social support were significantly higher in women with secondary infertility than in those with primary infertility (p < 0.01). Self-regulation was significantly lower in women whose height was below 160 centimeters (cm) (p<0.05). Social support was significantly higher among participants aged ≥ 35 years and weighing ≥ 60 kilograms (kg) (p < 0.01). The odds of EB adoption increased with self-regulation and social support (OR=1.05, 95% CI=1.02-1.09, p <0.01), (OR=1.06, 95% CI=1.02-1.11, p <0.01). Conclusion: Social support and self-regulation almost equally influenced EB in infertile women. Designing support and consultation programs can be considered in encouraging infertile women to do exercise in future research. Keywords: social support, regulation, infertility, women, exercise

Authors: Benard Omogo
Abstract:
The Kenyan Constitution 2010 introduced various commissions to devolve the powers that were previously centralized through the imperial Presidency. One of the commissions that directly determine the levels of remuneration and terms of service of Kenyan workers is the Salary and Remuneration Commission (SRC). Article 230 of the Kenyan Constitution 2010 mandates this commission to regularly review the remuneration and benefits of all the state officers and to advise the national and county governments on the remuneration and benefits of all other public officers. At the same time, article 54 of the Kenyan Labor Relations Act 2007 provides for the recognition of trade unions and collective bargaining agreements. The emerging challenges, therefore, originate from the conflicts of the mandate of the Salary and Remuneration Commission, whose advice is almost adopted as the order and this undermines the outcome of the Collective Bargaining Agreements. This has seen so many trade unions in Kenya being rendered irrelevant. This research paper is therefore going to sample the various trade unions of Kenya to assess the challenges that result from the position of the Salary and Remuneration Commission. We will also extend it by purposively sampling several trade unions in Africa to determine how they handle such challenges. The results from this paper will be useful to the Kenyan Lawmakers and Africa at large and may inform them to consider reviewing the laws and acts that relate to the trade unions for prosperity. Keywords: salary, remuneration, collective, bargaining, labor laws

Authors: Mehrdad Kamali-Sanzighi, Maziar Kamali-Sanzighi
Abstract:
The aim of this investigation is to study the phytoplankton and saprobic index at warm water fish ponds in the East of Golestan province, Gonbade Kavous city. Phytoplankton and ciliate sampling were done monthly during one season of culture. Finally, 39 genera from 7 classes of phytoplankton and 4 genera from the ciliate group were identified. Although, among different classes, Chlorophyceae, Cyanophyceae, Bacillariophyceae, Charophyceae, Chrysophyceae, Dinophyceae, and Euglenophyceae had the highest and lowest frequency percent of phytoplankton community with 23, 21, 20, 14, 11, 6 and 5 percent respectively. The results show that there are no significant differences between the saprobic index of different ponds (P > 0.05). But there are significant differences between the saprobic index value of different months and seasons during season culture (P < 0.05). Also, in current research, the saprobic index indicated the ß-mesosaprob water quality level. There was a general tends of decrease in the saprobic index value from the beginning to the end of the culture season. Parameters such as biomass increase of grower fishes, an increase of introduced chemical fertilizer and manure sedimentation, uneaten fish feed, fish fecal, and no regular exchangeable water resources are some of these changes' reasons. Keywords: fish pond, Golestan Province, saprobi index, phytoplankton, water quality

Authors: D. L. David, J. A. Wahedi, Q. T. Zaku
Abstract:
A study was conducted at River Mayo Ranewo and River Lau, Taraba State Nigeria. The two rivers empty into the Upper Benue Basin. A visual encounter survey was conducted within the two wetlands from June to August, 2014. The fish record was based entirely on landings of fishermen, number of canoes that land fish was counted, types of nets and baits used on each sampling day. Fish were sorted into taxonomic groups, identified to family/species level, counted and weighed in groups. The relative species abundance was determined by dividing the number of species from a site by the total number of species from all tributaries/sites. Fish was preserved in 2% formaldehyde solution and taken to the laboratory, where they were identified. Shannon-Weiner index of species diversity indicated that the diversity was highest at River Mayo Ranewo than River Lau. In the result showed at River Mayo Ranewo, the family Mochokidae recorded the highest (23.15%), followed by Mormyridae (2.64%) and the least was the family Lepidosirenidae (0.04%). While at River Lau the family Mochokidae recorded the highest occurrence of (24.1%), followed by Bagridae (20.20%), and then Mormyridae, which also was the second highest in River Lau, with 18.46% occurrence. There was no occurrence of Malapteruridae and Osteoglossidae (0%) in River Lau, but the least occurrence was the family Gymnarchidae (0.04%). These results indicated that the fish composition were not significantly (p ≤ 0.05) different based on t-test.Keywords: conservation, diversity index, Lau, Mayo Ranewo, wetlands
