3120 Circle of Learning Using High-Fidelity Simulators Promoting a Better Understanding of Resident Physicians on Point-of-Care Ultrasound in Emergency Medicine

Abstract:
Introduction: Ultrasound in emergency room has advantages of safer, faster, repeatable and noninvasive. Especially focused Point-Of-Care Ultrasound (POCUS) is used daily for prompt and accurate diagnoses, for quickly identifying critical and life-threatening conditions. That is why ultrasound has demonstrated its usefulness in emergency medicine. The true value of ultrasound has been once again recognized in recent years. It is thought that all resident physicians working at emergency room should perform an ultrasound scan to interpret signs and symptoms of deteriorating patients in the emergency room. However, a practical education on ultrasound is still in development. To resolve this issue, we established a new educational program using high-fidelity simulators and evaluated the efficacy of this course. Methods: Educational program includes didactic lectures and skill stations in half-day course. Instructor gives a lecture on POCUS such as Rapid Ultrasound in Shock (RUSH) and/or Focused Assessment Transthoracic Echo (FATE) protocol at the beginning of the course. Then, attendees are provided for training of scanning with cooperation of normal simulated patients. In the end, attendees learn how to apply focused POCUS skills at clinical situation using high-fidelity simulators such as SonoSim® (SonoSim, Inc) and SimMan® 3G (Laerdal Medical). Evaluation was conducted through surveillance questionnaires to 19 attendees after two pilot courses. The questionnaires were focused on understanding course concept and satisfaction. Results: All attendees answered the questionnaires. With respect to the degree of understanding, 12 attendees (number of valid responses: 13) scored four or more points out of five points. High-fidelity simulators, especially SonoSim® was highly appreciated to enhance learning how to handle ultrasound at an actual practice site by 11 attendees (number of valid responses: 12). All attendees encouraged colleagues to take this course because the high level of satisfaction was achieved. Discussion: Newly introduced educational course using high-fidelity simulators realizes the circle of learning to deepen the understanding on focused POCUS by gradual stages. SonoSim® can faithfully reproduce scan images with pathologic findings of ultrasound and provide experimental learning for a growth number of beginners such as resident physicians. In addition, valuable education can be provided if it is used combined with SimMan® 3G. Conclusions: Newly introduced educational course using high-fidelity simulators is supposed to be effective and helps in providing better education compared with conventional courses for emergency physicians.Keywords: point-of-care ultrasound, high-fidelity simulators, education, circle of learning

Abstract:
Zone Plates (ZPs) are used in many areas of physics where planar fabrication is advantageous in comparison with conventional curved lenses. There are several types of ZPs, such as the well-known Fresnel ZPs or the more recent Fractal ZPs and Fibonacci ZPs. The material selection of the lens plays a very important role in the beam modulation control. This work presents a comparison between two Fresnel ZP made from different materials in the ultrasound domain: Polylactic Acid (PLA) and brass. PLA is the most common material used in commercial 3D-printers due to its high design flexibility and low cost. Numerical simulations based on Finite Element Method (FEM) and experimental results are shown, and they prove that the focusing capabilities of brass ZPs and PLA ZPs are similar. For this reason, PLA is proposed as a Magnetic Resonance Imaging (MRI) compatible material with great potential for therapeutic ultrasound focusing applications.Keywords: FZP, PLA, focus, ultrasound, MRI

Abstract:
Data security needed in data transmission, storage, and communication to ensure the security. The single step parallel contour extraction (SSPCE) method is used to create the edge map as a key image from the different Gray level/Binary image. Performing the X-OR operation between the key image and each bit plane of the original image for image pixel values change purpose. The Arnold transform used to changes the locations of image pixels as image scrambling process. Experiments have demonstrated that proposed algorithm can fully encrypt 2D Gary level image and completely reconstructed without any distortion. Also shown that the analyzed algorithm have extremely large security against some attacks like salt & pepper and JPEG compression. Its proof that the Gray level image can be protected with a higher security level. Abstract:
Thyroid nodules can often be diagnosed with ultrasound imaging, although differentiating between benign and malignant nodules can be challenging for medical professionals. This work suggests a novel approach to increase the precision of thyroid nodule identification by combining machine learning and deep learning. The new approach first extracts information from the ultrasound pictures using a deep learning method known as a convolutional autoencoder. A support vector machine, a type of machine learning model, is then trained using these features. With an accuracy of 92.52%, the support vector machine can differentiate between benign and malignant nodules. Abstract:
Introduction: Carpal Tunnel Syndrome has numerous nonsurgical treatments including splint, physical therapy and corticosteroid injections. Aim: The purpose of this study was to evaluate the effectiveness of an ultrasound guided treatment procedure, for individuals with severe carpal tunnel syndrome. Materials and Method: 20 patients with an electrodiagnostic evidence of severe carpal tunnel syndrome were treated by an office-based ultrasound guided procedure (combination of percutaneous needle release of carpal tunnel and corticosteroid injection). Electrodiagnostic (nerve conduction study), clinical (Boston Carpal Tunnel Questionnaire, grip strength) and ultrasonic (median nerve and carpal tunnel cross-sectional area) measurements were recorded at baseline and one month after intervention. Results: Our preliminary data analysis showed that in one month follow up, patients had a significantly smaller cross-sectional area of the median nerve compared to pretreatment values (mean difference 0.06; 95%CI: 0.02-0.1; p < 0.001). In addition, patients had significantly less functional impairment (mean difference 35; 95% CI:28.7-43.4 ; p < 0.001), and an improved hand grip strength in one month follow up (mean difference 5.4; 95%CI: 3.1-7.8; p < 0.001;). There were no significant complications. Conclusion: Patients with severe carpal tunnel syndrome, who are candidates for surgical intervention, can consider office-based ultrasound guided needle release of carpal tunnel as an alternative safe treatment.Keywords: Carpal Tunnel Syndrome, needle release, pain, ultrasound

An approach to super-resolve the low-resolution (LR) image is presented in this paper which is very useful in multimedia communication, medical image enhancement and satellite image enhancement to have a clear view of the information in the image. The proposed Advanced B-Spline method generates a high-resolution (HR) image from single LR image and tries to retain the higher frequency components such as edges in the image. This method uses B-Spline technique and Crispening. This work is evaluated qualitatively and quantitatively using Mean Square Error (MSE) and Peak Signal to Noise Ratio (PSNR). The method is also suitable for real-time applications. Document Image Analysis recognizes text and graphics in documents acquired as images. An approach without Optical Character Recognition (OCR) for degraded document image analysis has been adopted in this paper. The technique involves document imaging methods such as Image Fusing and Speeded Up Robust Features (SURF) Detection to identify and extract the degraded regions from a set of document images to obtain an original document with complete information. In case, degraded document image captured is skewed, it has to be straightened (deskew) to perform further process. A special format of image storing known as YCbCr is used as a tool to convert the Grayscale image to RGB image format. The presented algorithm is tested on various types of degraded documents such as printed documents, handwritten documents, old script documents and handwritten image sketches in documents. In order to protect data privacy, image with sensitive or private information needs to be encrypted before being outsourced to the cloud. However, this causes difficulties in image retrieval and data management. A secure image retrieval method based on orthogonal decomposition is proposed in the paper. The image is divided into two different components, for which encryption and feature extraction are executed separately. As a result, cloud server can extract features from an encrypted image directly and compare them with the features of the queried images, so that the user can thus obtain the image. Different from other methods, the proposed method has no special requirements to encryption algorithms. Thermographic inspection is a relatively new technique for Non-Destructive Testing (NDT) which has been gathering increasing interest due to its relatively low cost hardware and extremely fast data acquisition properties. This technique is especially promising in the area of rapid automated damage detection and quantification. In collaboration with a major industry partner from the aerospace sector advanced thermography-based NDT software for impact damaged composites is introduced. The software is based on correlation analysis of time-temperature profiles in combination with an image enhancement process. The prototype software is aiming to a) better visualise the damages in a relatively easy-to-use way and b) automatically and quantitatively measure the properties of the degradation. Text and image are two important elements in the field of Iranian art, the text component and the image component have always been manifested together. The image narrates the text and the text is the factor in the formation of the image and they are closely related to each other. The connection between text and image is an interactive and two-way connection in the tradition of Iranian manuscript arrangement. The interaction between the narrative description and the image scene is the result of a direct and close connection between the text and the image, which in addition to the decorative aspect, also has a descriptive aspect. In this article the connection between the text element and the image element and its adaptation to the theory of Roland Barthes, the structuralism theorist, in this regard will be discussed. This study tends to investigate the question of how the connection between text and image in illustrated manuscripts of the Jalayrid period is defined according to Barthes’ theory. And what kind of proportion has the artist created in the composition between text and image. Based on the results of reviewing the data of this study, it can be inferred that in the Jalayrid period, the image has a reference connection and although it is of major importance on the page, it also maintains a close connection with the text and is placed in a special proportion. It is not necessarily balanced and symmetrical and sometimes uses imbalance for composition. Abstract:
Background: Obesity is an increasingly common problem in childhood. Childhood obesity is considered the main risk factor for the development of metabolic syndrome (MetS) (diabetes type 2, dyslipidemia, and hypertension). Recent studies estimated that among children with obesity 30-60% will develop MetS. Visceral fat thickness is a valuable predictor of the development of MetS. Computed tomography and dual-energy X-ray absorptiometry are the main techniques to assess visceral fat. However, they carry the risk of radiation exposure and are expensive procedures. Consequently, they are seldom used in the assessment of visceral fat in children. Some studies explored the potential of ultrasound as a substitute to assess visceral fat in the elderly and found promising results. Given the vulnerability of children to radiation exposure, we sought to evaluate ultrasound as a safer and more cost-efficient alternative for measuring visceral fat in obese children. Additionally, we assessed the correlation between visceral fat and obesity indicators such as insulin resistance. Methods: A cross-sectional study was conducted on 46 children with obesity (aged 6–16 years). Their visceral fat was evaluated by ultrasound. Subcutaneous fat thickness (SFT), i.e., the measurement from the skin-fat interface to the linea alba, and visceral fat thickness (VFT), i.e., the thickness from the linea alba to the aorta, were measured and correlated with anthropometric measures, fasting lipid profile, homeostatic model assessment for insulin resistance (HOMA-IR) and liver enzymes (ALT). Results: VFT assessed via ultrasound was found to strongly correlate with the BMI, HOMA-IR with AUC for VFT as a predictor of insulin resistance of 0.858 and cut off point of >2.98. VFT also correlates positively with serum triglycerides and serum ALT. VFT correlates negatively with HDL. Conclusions: Ultrasound, a safe and cost-efficient technique, could be a useful tool for measuring the abdominal fat thickness in children with obesity. Ultrasound-measured VFT could be an appropriate prognostic factor for insulin resistance, hypertriglyceridemia, and elevated liver enzymes in obese children.Keywords: metabolic syndrome, pediatric obesity, sonography, visceral fat

Abstract:
Mitral regurgitation (MR) is a heart disorder which the mitral valve does not close properly when the heart pumps out blood. MR is the most common form of valvular heart disease in the adult population. The diagnostic echocardiographic finding of MR is straightforward due to the well-known clinical evidence. In the determination of MR severity, quantification of sonographic findings would be useful for clinical decision making. Clinically, the vena contracta is a standard for MR evaluation. Vena contracta is the point in a blood stream where the diameter of the stream is the least, and the velocity is the maximum. The quantification of vena contracta, i.e. the vena contracta width (VCW) at mitral valve, can be a numeric measurement for severity assessment. However, manually delineating the VCW may not accurate enough. The result highly depends on the operator experience. Therefore, this study proposed an automatic method to quantify VCW to evaluate MR severity. Based on color Doppler ultrasound, VCW can be observed from the blood flows to the probe as the appearance of red or yellow area. The corresponding brightness represents the value of the flow rate. In the experiment, colors were firstly transformed into HSV (hue, saturation and value) to be closely align with the way human vision perceives red and yellow. Using ellipse to fit the high flow rate area in left atrium, the angle between the mitral valve and the ultrasound probe was calculated to get the vertical shortest diameter as the VCW. Taking the manual measurement as the standard, the method achieved only 0.02 (0.38 vs. 0.36) to 0.03 (0.42 vs. 0.45) cm differences. The result showed that the proposed automatic VCW extraction can be efficient and accurate for clinical use. This paper presents a steganography to hide the transmitted information without excite suspicious and also illustrates the level of secrecy that can be increased by using cryptography techniques. The proposed system has been implemented firstly by encrypted image file one time pad key and secondly encrypted message that hidden to perform encryption followed by image embedding. Then the new image file will be created from the original image by using four triangles operation, the new image is processed by one of two image processing techniques. The proposed two processing techniques are thresholding and differential predictive coding (DPC). Afterwards, encryption or decryption keys are generated by functional key generator. The generator key is used one time only. Abstract:
Big strums of thyroid glandule observed by a simple viewing can be witnessed in everyday life. Medical cabinets evidence patients withpalpablenodes of thyroid glandule, mainly nodes of the size of 10 millimeters. Further, more cases which have resulted in negative under palpation have resulted in positive at ultrasound examination. Therefore, the use of ultrasound for diagnosing has increased the number of patients with nodes of thyroid glandule in the last couple of decades in all countries, Albania included. Thus, there has been evidence of an increased number of patients affected by this pathology, where female patients dominate. Demographically, the capital shows high numbers due to the high population, but of interest is the high incidence of those areas distanced from the sea. While regarding related pathologies, no significant link was evidenced, an element of ancestry was evident in the nodes of the thyroid glandule. When we talk of nodes of the thyroid glandule, we should consider hyperplasia, neoplasia, and inflammatory diseases that cause nodes of the thyroid glandule. This increase parallels the world’s increase of the incidence of thyroid glandule, with malign cases, which are at about 5% and are not depended on size. Given the numbers, with most thyroid glandule nodes being benign, the main objective of the examination of the nodes was the determination of benign and malign cases to avoid undue surgery. Subject of this study were 212 patients that underwent fine-needle aspiration (FNA) under ultrasound guidance at the Medical University Center of Tirana. All the patients came to the Mother Teresa University Hospital from public and private hospitals and other polyclinics. These patients had an ultrasound examination before visiting the Center of Nuclear Medicine for a scintigraph of thyroid glandule in the period September 2016 and September 2017. To correlate, all patients had been examined via ultrasound of the thyroid glandule prior to the scintigraph. The ultrasound included evaluation of the number of nodes, their size, their solid, cystic, or solid-cystic structure, echogenicity according to the gray scale, the presence of calcification, the presence of lymph nodes, the presence of adenopathy, and the correlation of the cytology results from the Laboratory of Pathological Anatomy of Medical University Center of Tirana.Keywords: thyroid nodes, fine needle aspiration, ultrasound, scintigraphy

The aim of this research is to present a multi spectral image enhancement methods used to achieve highly real digital image populates only a small portion of the available range of digital values. Also, a quantitative measure of image enhancement is presented. This measure is related with concepts of the Webers Low of the human visual system. For decades, several image enhancement techniques have been proposed. Although most techniques require profuse amount of advance and critical steps, the result for the perceive image are not as satisfied. This study involves changing the original values so that more of the available range is used; then increases the contrast between features and their backgrounds. It consists of reading the binary image on the basis of pixels taking them byte-wise and displaying it, calculating the statistics of an image, automatically enhancing the color of the image based on statistics calculation using algorithms and working with RGB color bands. Finally, the enhanced image is displayed along with image histogram. A number of experimental results illustrated the performance of these algorithms. Higher computational complexity is one of the major component of existing techniques to detect such tampering. Moreover, copy move forgery is usually performed in three steps. The aim of this experimental and numerical study is to analyze the effects of acoustic streaming generated by 40 kHz ultrasonic waves on heat transfer in forced convection, with and without 40 PPI aluminum metal foam. Preliminary dynamic and thermal studies were done with COMSOL Multiphase, to see heat transfer enhancement degree by inserting a 40PPI metal foam (10 × 2 × 3 cm) on a heat sink, after having determined experimentally its permeability and Forchheimer's coefficient. The results obtained numerically are in accordance with those obtained experimentally, with an enhancement factor of 205% for a velocity of 0.4 m/s compared to an empty channel. The influence of 40 kHz ultrasound on heat transfer was also tested with and without metallic foam. Results show a remarkable increase in Nusselt number in an empty channel with an enhancement factor of 37,5%, while no influence of ultrasound on heat transfer in metal foam presence.Keywords: acoustic streaming, enhancing heat transfer, laminar flow, metal foam, ultrasound

The evolution and advancement of deep learning and convolutional neural networks (CNN) have greatly improved the capabilities of object detection and image-based classification. Target detection is important to research in the field of computer vision, especially in video surveillance systems. To solve this problem, we will be applying a convolutional neural network of multiple scales at multiple locations in the image in one sliding window. Most translation networks move away from the bounding box around the area of interest. In contrast to this architecture, we consider the problem to be a classification problem where each pixel of the image is a separate section. Image classification is the method of predicting an individual category or specifying by a shoal of data points. Image classification is a part of the classification problem, including any labels throughout the image. The image can be classified as a day or night shot. Or, likewise, images of cars and motorbikes will be automatically placed in their collection. Edge is variation of brightness in an image. Edge detection is useful in many application areas such as finding forests, rivers from a satellite image, detecting broken bone in a medical image etc. The paper discusses about finding edge of multiple aerial images in parallel .The proposed work tested on 38 images 37 colored and one monochrome image. The time taken to process N images in parallel is equivalent to time taken to process 1 image in sequential. Abstract:
Mechanical index (MI) is used for quantifying acoustic cavitation and the relationship between acoustic pressure and the frequency. In this study, modeling of the MI was applied to provide treatment protocol and to understand the effective physical processes on reproducibility of stem cells. The acoustic pressure and MI equations are modeled and solved to estimate optimal MI for 28, 40, 150 kHz and 1 MHz frequencies. Radial and axial acoustic pressure distribution was extracted. To validate the results of the modeling, the acoustic pressure in the water and near field depth was measured by a piston hydrophone. Results of modeling and experiments show that the model is consistent well to experimental results with 0.91 and 0.90 correlation of coefficient (p<0.05) for 1 MHz and 40 kHz. Low intensity ultrasound with 0.40 MI is more effective on the proliferation rate of the spermatogonial stem cells during the seven days of culture, in contrast, high MI has a harmful effect on the spermatogonial stem cells. This model provides proper treatment planning in vitro and in vivo by estimating the cavitation phenomenon.Keywords: ultrasound, mechanical index, modeling, stem cell

Abstract:
Pectin is a complex colloidal polysaccharide, found on the cell walls of all young plants such as fruit and vegetables. It acts as a thickening, stabilizing and gelling agent in foods. Pectin was extracted from mandarin and lemon peels using ultrasound and microwave assisted extraction methods to compare with these two different sources and methods of pectin production. In this work, the effect of microwave power (360, 600 W) and irradiation time (1, 2, 3 min) on the yield of extracted pectin from mandarin and lemon peels for microwave assisted extraction (MAE) were investigated. For ultrasound assisted extraction (UAE), parameters were determined as temperature (60, 75 °C) and sonication time (15, 30, 45 min) and hydrochloric acid (HCl) was used as an extracting agent for both extraction methods. The highest yields of extracted pectin from lemon peels were found to be 8.16 % (w/w) for 75 °C, 45 min by UAE and 8.58 % (w/w) for 360 W, 1 min by MAE. Additionally, the highest yields of extracted pectin from mandarin peels were found to be 11.29 % (w/w) for 75 °C, 45 min by UAE and 16.44 % (w/w) for 600 W, 1 min by MAE. The results showed that the use of microwave assisted extraction promoted a better yield when compared to the two extraction methods. On the other hand, according to the results of experiments, mandarin peels contain more pectin than lemon peels when the compared to the pectin product values of two sources. Therefore, these results suggested that MAE could be used as an efficient and rapid method for extraction of pectin and mandarin peels should be preferred as sources of pectin production compared to lemon peels.Keywords: mandarin peel, lemon peel, pectin, ultrasound, microwave, extraction

Abstract:
In this work, fractal compression (FIC) technique is introduced based on using moment features to block indexing the zero-mean range-domain blocks. The moment features have been used to speed up the IFS-matching stage. Its moments ratio descriptor is used to filter the domain blocks and keep only the blocks that are suitable to be IFS matched with tested range block. The results of tests conducted on Lena picture and Cat picture (256 pixels, resolution 24 bits/pixel) image showed a minimum encoding time (0.89 sec for Lena image and 0.78 of Cat image) with appropriate PSNR (30.01dB for Lena image and 29.8 of Cat image). Abstract:
In the context of global waste management and sustainable resource utilization, millets emerge as a vital yet underutilized cereal resource. Despite their exceptional nutritional profile and resilience to harsh environmental conditions, their potential remains largely untapped. This study aims to contribute to the valorization of seven Algerian pearl millet landraces (Pennisetum glaucum (L.) R. Br) from the southern region by focusing on the characterization of their starches. Utilizing both conventional wet milling, incorporating sodium azide as a microbial growth inhibitor, and a novel green technology—Ultrasound-assisted isolation, we explore avenues for enhancing the functional properties of these starches. Analysis of key functional properties such as swelling power and water solubility index reveals significant enhancements, particularly during heat treatment near the gelatinization temperature [70 - 80 °C]. Furthermore, our investigation into the influence of pre-treatment methods on isolated starches highlights the potential of Ultrasound-assisted isolation in reducing absorbency and water solubility compared to conventional methods. Through rigorous data analysis using SPSS software (Version 23), we ascertain the efficiency of Ultrasound-assisted isolation, underscoring its promising role in the valorization of pearl millet waste. This research not only sheds light on the functional properties of pearl millet starch but also underscores the imperative of sustainable waste management in harnessing the full potential of underutilized cereal resources.Keywords: isolation, solubility, starch, swelling, ultrasound

Abstract:
The aim of this paper is image encryption using Genetic Algorithm (GA). The proposed encryption method consists of two phases. In modification phase, pixels locations are altered to reduce correlation among adjacent pixels. Then, pixels values are changed in the diffusion phase to encrypt the input image. Both phases are performed by GA with binary chromosomes. For modification phase, these binary patterns are generated by Local Binary Pattern (LBP) operator while for diffusion phase binary chromosomes are obtained by Bit Plane Slicing (BPS). Initial population in GA includes rows and columns of the input image. Instead of subjective selection of parents from this initial population, a random generator with predefined key is utilized. It is necessary to decrypt the coded image and reconstruct the initial input image. Fitness function is defined as average of transition from 0 to 1 in LBP image and histogram uniformity in modification and diffusion phases, respectively. Randomness of the encrypted image is measured by entropy, correlation coefficients and histogram analysis. Experimental results show that the proposed method is fast enough and can be used effectively for image encryption.Keywords: correlation coefficients, genetic algorithm, image encryption, image entropy

This paper presents an approach for data hiding methods which provides a secret communication between sender and receiver. The data is hidden in gray-scale images and the boundary of gray-scale image is used to store the mapping information. In this an approach data is in ASCII format and the mapping is in between ASCII value of hidden message and pixel value of cover image, since pixel value of an image as well as ASCII value is in range of 0 to 255 and this mapping information is occupying only 1 bit per character of hidden message as compared to 8 bit per character thus maintaining good quality of stego image.Keywords: ASCII value, cover image, PSNR, pixel value, stego image, secret message

Abstract:
Steganography has been known for centuries as an efficient approach for covert communication. Due to its popularity and ease of access, image steganography has attracted researchers to find secure techniques for hiding information within an innocent looking cover image. In this research, we propose a novel deep-learning approach to digital image steganography. The proposed method, DeepWaveletFusion, uses convolutional neural networks (CNN) to hide a secret image into a cover image of the same size. Two CNNs are trained back-to-back to merge the Discrete Wavelet Transform (DWT) of both colored images and eventually be able to blindly extract the hidden image. Based on two different image similarity metrics, a weighted gain function is used to guide the learning process and maximize the quality of the retrieved secret image and yet maintaining acceptable imperceptibility. Experimental results verified the high recoverability of DeepWaveletFusion which outperformed similar deep-learning-based methods.Keywords: deep learning, steganography, image, discrete wavelet transform, fusion
