Abstract:
There is no epidemiologic data on this gene polymorphism in several countries. Therefore, this study aimed to assess the genotype and allele frequencies of the gene variant in three countries. This study involved healthy individuals from Colombia, Mozambique, and Portugal. Genomic DNA was isolated from blood samples using the Qiamp DNA Extraction Kit (Qiagen). The isolated DNA was genotyped using Polymerase Chain Reaction (PCR) - Restriction Fragment Length Polymorphism. Microstat and GraphPad quick cal software were used for the Chi-square test and evaluation of Hardy-Weinberg equilibrium, respectively. A total of 181 individuals’ blood sample was analyzed. Overall, TT (74.0%) genotype was the highest, and CC (7.8%) was the lowest. Country wise genotypic frequencies were Colombia 47(70.2%) TT, 12(17.9%) TC and 8(11.9%) CC; Mozambique 47(88.7%) TT, 5(9.4%) TC, and 1(1.9%) CC; and Portugal 40(65.6%) TT, 16(26.2%) TC, and 5(8.2%) CC. The reference (T) allele was highest among Mozambicans (93.4%) compared to Colombians (79.1%) and Portuguese (78.7%). Mozambicans showed statistically significant genotypic and allelic frequency differences compared to Colombians (p<0.01) and Portuguese (p <0.01). Overall and country-wise, the CC genotype was less frequent and relatively high for Colombians and Portuguese populations. This finding may imply statins risk-benefit variability associated with CC genotype among these populations that needs further understanding. Keywords: c.521T>C, polymorphism, SLCO1B1, SNP, statins

Abstract:
The work aims to develop a robot in the form of autonomous vehicle to detect, inspection and mapping of underground pipelines through the ATmega328 Arduino platform. Hardware prototyping very similar to C / C ++ language that facilitates its use in robotics open source, resembles PLC used in large industrial processes. The robot will traverse the surface independently of direct human action, in order to automate the process of detecting buried pipes, guided by electromagnetic induction. The induction comes from coils that sends the signal to the Arduino microcontroller contained in that will make the difference in intensity and the treatment of the information, then this determines actions to electrical components such as relays and motors, allowing the prototype to move on the surface and getting the necessary information. The robot was developed by electrical and electronic assemblies that allowed test your application. The assembly is made up of metal detector coils, circuit boards and microprocessor, which interconnected circuits previously developed can determine, process control and mechanical actions for a robot (autonomous car) that will make the detection and mapping of buried pipelines plates. Keywords: robotic, metal detector, embedded system, pipeline inspection

Authors: Hazem Hasan Hushayish
Abstract:
Motivation and teaching strategies are commonly believed to improve students’ communicative competence in English as a foreign language; still, there is not much empirical evidence to support this claim. The present study is intended to focus on the effects of motivational factors and teaching strategies on the communicative competence among the Palestinian undergraduates. In the first phase, one hundred and eighty participants, who are studying English language in three Palestinian universities, answered a questionnaire. The questionnaire included items derived from Gardner’s 2001, 2004, 2006, 2007 Attitude/Motivation Test Battery AMTB and items from Dörnyei 2007 and Guilloteaux and Dörnyei 2008 teaching strategies framework for foreign language classrooms. In the second phase, 6 participants, from the same universities, were interviewed. The quantitative results indicated that participants’ communicative competence is significantly affected by motivation and teaching strategies. Also, the qualitative results indicated that teaching strategies do not directly affect students’ communicative competence, but rather affect their motivation. Consequently, the current study will add substantively to the literature concerning the effects of motivation and teaching strategies in communicative competencies among EFL learners in the Palestinian context, and some suggested procedures and suggestions that help improve learners’ communicative competences. Keywords: communicative competence, motivation, teaching strategies, Palestinian undergraduates
Procedia PDF Downloads 19712829 The Differences of Vascular Endothelial Growth Factor Levels in Serum to Determine Follicular Adenoma and Follicular Carcinoma of Thyroid
Authors: Tery Nehemia Nugraha Joseph, J. D. P. Wisnubroto
Abstract:
Thyroid cancer is a healthcare problem with high morbidity and mortality. Follicular adenoma and follicular carcinoma are thyroid tumors from the thyroid follicular cells differentiation with a microfollicular pattern that consists of follicular cuboidal cells. vascular endothelial growth factor (VEGF) is a potent and powerful mitogen for endothelial cells and increases vascular permeability. Therefore, due to an increase in thyroid-stimulating hormone (TSH), VEGF production is activated in the thyroid that leads to the end of mitogenic TSH stimulation and initiation of angiogenesis. The differences in VEGF levels in the follicular carcinoma of thyroid tissue with follicular adenoma thyroid can be used as a basis in differentiating the two types of neoplasms. This study aims to analyze VEGF in the serum so that it can be used to differentiate the types of thyroid carcinoma before surgery. This study uses a cross-sectional research design. Samples were carried out by taking serum samples, and the VEGF levels were calculated. Data were analyzed using the Mann-Whitney test. The results found a significant difference between VEGF levels in the follicular carcinoma thyroid group and VEGF levels in the follicular adenoma thyroid group with a value of p = 0.007 (p < 0.05). The results obtained are 560,427 ± 160,506 ng/mL in the type of follicular carcinoma thyroid and 320.943 ± 134.573 ng/mL in the type of follicular adenoma thyroid. VEGF levels between follicular adenoma and follicular carcinoma are different. VEGF levels are higher in follicular carcinoma thyroid than follicular adenoma thyroid. Keywords: follicular adenoma thyroid, follicular carcinoma thyroid, thyroid, VEGF

Authors: Hanbey Hazar, Hakan Gul, Ugur Ozturk
Abstract:
In order to decrease the hazardous emissions of the internal combustion engines and to improve the combustion and thermal efficiency, thermal barrier coatings are applied. In this experimental study, cylinder, piston, exhaust, and inlet valves which are combustion chamber components have been coated with a ceramic material, and this earned the engine LHR feature. Cylinder, exhaust and inlet valves of the diesel engine used in the tests were coated with ekabor-2 commercial powder, which is a ceramic material, to a thickness of 50 µm, by using the boriding method. The piston of a diesel engine was coated in 300 µm thickness with bor-based powder by using plasma coating method. Pumpkin seeds oil methyl ester (PSME) was produced by the transesterification method. In addition, dimethoxymethane additive materials were used to improve the properties of diesel fuel, pumpkin seeds oil methyl ester (PSME) and its mixture. Dimethoxymethane was blended with test fuels, which was used as a pilot fuel, at the volumetric ratios of 4% and 8%. Due to thermal barrier coating, the diesel engine's CO, HC, and smoke density values decreased; but, NOx and exhaust gas temperature (EGT) increased. Keywords: boriding, diesel engine, exhaust emission, thermal barrier coating

Authors: Kanika Sharma, Shaila Bahl, Birendra Singh, Pratik Kumar, S. P. Lochab, A. Pandey
Abstract:
In the present investigation, initially nano-particles of CaF2 were prepared by the chemical co-precipitation method and later the prepared salt was activated by thulium (0.1 mol%) using the combustion technique. The final product was characterized and confirmed by X-Ray diffraction (XRD) and transmission electron microscopy (TEM). Further, the thermoluminescence (TL) properties of the nanophosphor were studied by irradiating it with 1.25 MeV of gamma radiation and 65 MeV of carbon (C6+) ion beam. For gamma rays, two prominent TL peaks were observed with a low temperature peak at around 1070C and a high temperature peak at around 1570C. Furthermore, the nanophosphor maintained a linear TL response for the entire range of studied doses i.e. 10 Gy to 2000 Gy for both the temperature peaks. Moreover, when the nanophosphor was irradiated with 65 MeV of C6+ ion beam the shape and structure of the glow curves remained spectacularly similar and the nanophosphor displayed a linear TL response for the full range of studied fluences i.e. 5*1010 ions/cm2 to 1 *1012 ions/ cm2. Finally, various tests like reproducibility test and batch homogeneity were also carried out to define the final product. Thus, co-precipitation method followed by combustion technique was successful in effectively producing dosimetric grade CaF2:Tm for dosimetry of gamma as well as carbon (C6+) beam. Keywords: gamma radiation, ion beam, nanocrystalline, radiation dosimetry

Authors: Amir Ahmad Dehghani, Morteza Nabizadeh
Abstract:
This paper presents the application of an Adaptive Neuro-Fuzzy Inference System (ANFIS) to flood hydrograph modeling of Shahid Rajaee reservoir dam located in Iran. This was carried out using 11 flood hydrographs recorded in Tajan river gauging station. From this dataset, 9 flood hydrographs were chosen to train the model and 2 flood hydrographs to test the model. The different architectures of neuro-fuzzy model according to the membership function and learning algorithm were designed and trained with different epochs. The results were evaluated in comparison with the observed hydrographs and the best structure of model was chosen according the least RMSE in each performance. To evaluate the efficiency of neuro-fuzzy model, various statistical indices such as Nash-Sutcliff and flood peak discharge error criteria were calculated. In this simulation, the coordinates of a flood hydrograph including peak discharge were estimated using the discharge values occurred in the earlier time steps as input values to the neuro-fuzzy model. These results indicate the satisfactory efficiency of neuro-fuzzy model for flood simulating. This performance of the model demonstrates the suitability of the implemented approach to flood management projects. Keywords: adaptive neuro-fuzzy inference system, flood hydrograph, hybrid learning algorithm, Shahid Rajaee reservoir dam

Authors: Binyamien Ibrahim Rasoul
Abstract:
Controlled burning of rice husk can produce amorphous rice husk ash (RHA) with high silica content which can significantly enhance the properties of concrete. This study has been undertaken to investigate the relationship between the incineration temperatures and time to produce RHA with ultimate reactivity. The rice husk samples were incinerated in an electrical muffle furnace at 350°C, 400°C, 425°C 450°C, 475°C, and 500°C for 60 and 90 minutes, respectively. The silica structure in the Rice Husk Ash (RHA) was determined using X-Ray diffraction analysis, while chemical properties obtained using X-Ray Fluorescence. The results show that RHA appeared to be the totally amorphous when the husk incineration up to 425°C for 60 and even at 90 minutes. However, with increased temperature to 450°C, 475°C and 500°C, traces of crystalline silica (quartz) were detected. However, cannot be taken into account as it does not affect on the ash structure. In conclusion, the result gives an idea of the temperature and the time required to produce ash from rice husk with totally amorphous form. Keywords: rice husk ash, silica, compressive strength, tensile strength, X-Ray diffraction, X-R florescence, pozzolanic activity

Abstract:
Our study is based on chemical and phytochemical characterization of Artemisia absinthium L and in vitro tests to demonstrate the biological activities of essential oil and natural extract. A qualitative and quantitative comparison of the essential oil extracted by two extraction procedures was performed by analysis of CG/SM and the yield calculation. The method of hydrodistillation has a chemical composition and provides oil content than the best training water vapor. These oils are composed mainly of thujone followed chamazulene and ρ-cymene. The antimicrobial activity of wormwood oil was tested in vitro by two methods (agar diffusion and microdilution) on four plant pathogenic fungi (Aspergillus sp, Botrytis cinerea, Fusarium culmorum and Helminthosporium sp). The study of the antifungal effect showed that this oil has an inhibitory effect counterpart the microorganisms tested in particular the strain Botrytis cinerea. Otherwise, this activity depends on the nature of the oil and the germ itself. The antioxidant activity in vitro was studied with the DPPH method. The activity test shows that the oil and extract of Artemisia absinthium have a very low antioxidant capacity compared to the antioxidants used as a reference. The extract has a potentially high antiradical power not from its oil. The quantitative determinations of phenolic compounds by the Folin-Ciocalteu revealed that absinthe is low in total polyphenols and tannins. Keywords: artemisia absinthium, biological activities, essential oil, extraction processes
Procedia PDF Downloads 34312823 Enhanced Method of Conceptual Sizing of Aircraft Electro-Thermal De-Icing System
Authors: Ahmed Shinkafi, Craig Lawson
Abstract:
There is a great advancement towards the All-Electric Aircraft (AEA) technology. The AEA concept assumes that all aircraft systems will be integrated into one electrical power source in the future. The principle of the electro-thermal system is to transfer the energy required for anti/de-icing to the protected areas in electrical form. However, powering a large aircraft anti-icing system electrically could be quite excessive in cost and system weight. Hence, maximising the anti/de-icing efficiency of the electro-thermal system in order to minimise its power demand has become crucial to electro-thermal de-icing system sizing. In this work, an enhanced methodology has been developed for conceptual sizing of aircraft electro-thermal de-icing System. The work factored those critical terms overlooked in previous studies which were critical to de-icing energy consumption. A case study of a typical large aircraft wing de-icing was used to test and validate the model. The model was used to optimise the system performance by a trade-off between the de-icing peak power and system energy consumption. The optimum melting surface temperatures and energy flux predicted enabled the reduction in the power required for de-icing. The weight penalty associated with electro-thermal anti-icing/de-icing method could be eliminated using this method without under estimating the de-icing power requirement. Keywords: aircraft, de-icing system, electro-thermal, in-flight icing

Authors: Alan (Avi) Kirschenbaum, Carmit Rapaport
Abstract:
The behavior of bystanders in emergencies and disasters have been examined for over 50 years. Such acts have been cited as contributing to saving lives in terms of providing first responder help until official emergency units can arrive. Several reasons have been suggested for this type of behavior but most focused on a broad segment of individual psychological decision-making processes. Recent theoretical evidence suggests that the external factors for such bystander decisions, mainly disaster community based social contexts factors, are also important. We aim to test these competing arguments. Specifically, we examine alternative explanatory perspectives by focusing on self-efficacy as a proxy for the accepted individual psychological case and contrast it with potential bystander characteristics of the individual as well factors as embedded in the social context of the disaster community. To do so, we will utilize a random sampling of the population from a field study of an urban community in Israel that experienced five years of continuous terror attacks. The results strongly suggest that self-efficacy, as well as external factors: preparedness and having skills for intervention during emergencies along with gender best, predict potential helping behaviors. These results broaden our view of bystander behavior and open a window for enhancing this phenomenon as another element in disaster and crisis management. Keywords: bystander behavior, disasters emergencies, psychological motivation to help, social context for helping

Authors: A. P. I. Popoola, O. S. I. Fayomi, V. S. Aigbodion
Abstract:
Mild steel has found many engineering applications due to its great formability, availability, low cost and good mechanical properties among others. However its functionality and durability is subject of concern due to corrosion deterioration. Based on these Yttrium is selected as reinforcing particles using electroplating process in this work to enhance the corrosion resistance. Bath formulation of zinc-yttrium was prepared at moderated temperature and pH, to coat mild steel sample. Corrosion and wear behaviour were analyzed using electrochemical potentiostat and abrasive test rig. The composition and microstructure of coated films were investigated standard method. The microstructure of the deposited plate obtained from optimum (10%Yttrium) bath revealed fine-grained deposit of the alloy in the presence of condensation product and hence modified the morphology of zinc–yttrium alloy deposit. It is demonstrated that by adding yttria particles, mild steel can be strengthened with improved polarization behaviour and higher resistance to corrosive in sodium chloride solutions. Microhardness of the coating compared to plain mild steel have increased before and after heat treatment, and an increased wear resistance was also obtained from the modified coating of zinc-yttrium.Keywords: microhardness, zinc-yttrium, coating, mild steel, microstructure, wear, corrosion

Authors: L. Totladze
Abstract:
The role of financial sector in supporting economic growth and development is well acknowledged. The term spread (the difference between the yields on long-term and short-term Treasury securities) has been found useful for predicting economic variables as output growth, inflation, industrial production, consumption. The temp spread is one of the leading economic indicators according to NBER methodology. Leading economic indicators are widely used in forecasting of economic activity. Many empirical studies find that the term spread predicts future economic activity. The article shortly explains how the term spread might predict future economic activity. This paper analyses the dynamics of the spread between short and long-term interest rates in countries with transition economies. The research paper analyses term spread dynamics in Georgia and compare it with post-communist countries and transition economies spread dynamics. Abstract:
This paper studies the physical and rheological behaviours of water/in/diesel fuel nanoemulsions prepared by modified low energy method. Twenty of water/in/diesel fuel nanoemulsions were prepared using mixed nonionic surfactants of sorbitan monooleate and polyoxyethylene sorbitan trioleate (MTS) at Hydrophilic-Lipophilic Balance (HLB) value of 10 and a working temperature of 20°C. The influence of the prepared nanoemulsions on the physical properties such as kinematic viscosity, density, and calorific value was studied. Also, nanoemulsion systems were subjected to rheological evaluation. The effect of water loading percentage (5, 6, 7, 8, 9 and 10 wt.%) on rheology was assessed at temperatures range from 20 to 60°C with temperature interval of 10 for time lapse 0, 1, 2 and 3 months, respectively. Results show that all of the sets nanoemulsions exhibited a Newtonian flow character of low-shear viscosity in the range of 132 up to 191 1/s, and followed by a shear-thinning region with yield value (Non-Newtonian behaviour) at high shear rate for all water ratios (5 to 10 wt.%) and at all test temperatures (20 to 60°C) for time ageing up to 3 months. Also, the viscosity/temperature relationship of all nanoemulsions fitted well Arrhenius equation with high correlation coefficients that ascertain their Newtonian behavior. Keywords: alternative fuel, nanoemulsion, surfactant, diesel fuel

Abstract:
The research aims to study the association between job satisfaction, motivation and the five factors of organizational citizenship behavior (i.e. Altruism, Conscientiousness, Sportsmanship, Courtesy and Civic virtue) among Public Sector Employees in Pakistan. In this research Structure Equation Modeling with confirmatory factor analysis was used to test the relationship between two independent and five dependent variables. Data was collected through questionnaire survey from 152 Public Servants Working in Gujrat District-Pakistan in different capacities. Stratified Random Sampling Technique was used to conduct this survey. The results of the study indicate that five factors of OCB have positive significant relation with both motivation and job satisfaction except the relationship of Civic Virtue with Motivation. The research findings implicate that factors other than motivation and job satisfaction may also affect OCB. Likewise, all the five factors of OCB may not be present in all populations. Thus, Managers must concentrate on increasing motivation and job satisfaction to increase OCB. Furthermore, the present research gives a direction to future researchers to use more independent variables (e.g. Culture, leadership, workplace environment, various job attitudes, types of motivation, etc.) on different types of populations with larger sample size in order to find the reasons behind insignificant relationship of civic virtue with Motivation in the research in hand and to generalize the tested model. Keywords: five factors of organizational citizenship behavior (OCB), motivation, job satisfaction, public sector employees in Pakistan

Procedia PDF Downloads 26912812 Assessing the Impact of Decentralization on Governance and Development in Malawi
Authors: Vincent Chumbu
Abstract:
This study examines the impact of decentralization on development and government in Malawi. Decentralization has been a key element in Malawi's attempts to alter its political system since the early 1990s. This study uses both qualitative and quantitative methods to look into how well devolution promotes local development, improves service delivery, and supports effective governance. The findings suggest that while devolution has resulted in particular improvements in local government or service provision, significant challenges persist. Limited financial decentralization, inadequate local competency, and governmental meddling in local decision-making processes are some of these difficulties. The paper concludes with recommendations for strengthening Malawi's decentralization initiatives to better promote good governance and sustainable development. Keywords: governance, development, malawi, local government

The main difficulty encountered in the formulation of high-performance concrete (HPC) consists in choosing the most efficient cement-superplasticizer pair allowing to obtain maximum water reduction, good workability of the concrete in the fresh state, and very good mechanical resistance in the hardened state. The aim of this work is to test the efficiency of three polycarboxylate ether-based superplasticizers (PCE) marketed in Algeria with CEMI 52.5 R cement and to study the effect of chemical structure of PCE on zeta potential, rheological and mechanical properties of cement pastes. The property of the polymers in cement was tested by a Malvern Zetasizer 2000 apparatus and VT 550 viscometer. Results showed that the zeta potential and its rheological properties are related to the molecular weight and the density carboxylic of PCE. The PCE with a moderate molecular weight and the highest carboxylic groups had the best dispersion (high value of zeta potential) and lowest viscosity. The effect of the chemical structure of PCEs on mechanical properties is evaluated by the formulation of cement mortar with these PCEs. The result shows that there is a correlation between the zeta potential of polymer and the compressive strength of cement paste. Keywords: molecular weight, polycarboxylate-ether superplasticizer, rheology, zeta potential

Abstract:
In this paper, we present a semi-automatic tool for calculating ejection fraction from an echocardiographic video signal which is derived from a database in DICOM format, of Clinica de la Costa - Barranquilla. Described in this paper are each of the steps and methods used to find the respective calculation that includes acquisition and formation of the test samples, processing and finally the calculation of the parameters to obtain the ejection fraction. Two imaging segmentation methods were compared following a methodological framework that is similar only in the initial stages of processing (process of filtering and image enhancement) and differ in the end when algorithms are implemented (Active Contour and Region Growing Algorithms). The results were compared with the measurements obtained by two different medical specialists in cardiology who calculated the ejection fraction of the study samples using the traditional method, which consists of drawing the region of interest directly from the computer using echocardiography equipment and a simple equation to calculate the desired value. The results showed that if the quality of video samples are good (i.e., after the pre-processing there is evidence of an improvement in the contrast), the values provided by the tool are substantially close to those reported by physicians; also the correlation between physicians does not vary significantly. Keywords: echocardiography, DICOM, processing, segmentation, EDV, ESV, ejection fraction

Abstract:
Pakistan being the developing country is facing double burden of communicable and non-communicable disease. The aspect of secondary prevention of ischemic heart disease in developing countries is the dire need for public health specialists, clinicians and policy makers. There is some evidence that psychotherapeutic measures, including psychotherapy, recreation, exercise and stress management training have positive impact on secondary prevention of cardiovascular diseases but there are some contradictory findings as well. Cardiac rehabilitation program (CRP) has not yet fully implemented in Pakistan. Psychological, physical and specific health-related quality of life (HRQoL) outcomes needs assessment with respect to its practicality, effectiveness, and success. Objectives: To determine the effect of cardiac rehabilitation program (CRP) on the health-related quality of life (HRQoL) measures of post MI patients compared to the usual care. Hypothesis: Post MI patients who receive the interventions (CRP) will have better HRQoL as compared to those who receive the usual cares. Methods: The randomized control trial was conducted at a Cardiac Rehabilitation Unit of Lady Reading Hospital (LRH), Peshawar. LRH is the biggest hospital of the Province Khyber Pakhtunkhwa (KP). A total 206 participants who had recent first myocardial infarction were inducted in the study. Participants were randomly allocated into two group i.e. usual care group (UCG) and cardiac rehabilitation group (CRG) by permuted-block randomization (PBR) method. CRP was conducted in CRG in two phases. Three HRQoL outcomes i.e. general health questionnaire (GHQ), self-rated health (SRH) and MacNew quality of life after myocardial infarction (MacNew QLMI) were assessed at baseline and follow-up visits among both groups. Data were entered and analyzed by appropriate statistical test in STATA version 12. Results: A total of 195 participants were assessed at the follow-up period due to lost-to-follow-up. The mean age of the participants was 53.66 + 8.3 years. Males were dominant in both groups i.e. 150 (76.92%). Regarding educational status, majority of the participants were illiterate in both groups i.e. 128 (65.64%). Surprisingly, there were 139 (71.28%) who were non-smoker on the whole. The comorbid status was positive in 120 (61.54%) among all the patients. The SRH at follow-up among UCG and CRG was 4.06 (95% CI: 3.93, 4.19) and 2.36 (95% CI: 2.2, 2.52) respectively (p<0.001). GHQ at the follow-up of UCG and CRG was 20.91 (95% CI: 18.83, 21.97) and 7.43 (95% CI: 6.59, 8.27) respectively (p<0.001). The MacNew QLMI at follow-up of UCG and CRG was 3.82 (95% CI: 3.7, 3.94) and 5.62 (95% CI: 5.5, 5.74) respectively (p<0.001). All the HRQoL measures showed strongly significant improvement in the CRG at follow-up period. Conclusion: HRQOL improved in post MI patients after comprehensive CRP. Education of the patients and their supervision is needed when they are involved in their rehabilitation activities. It is concluded that establishing CRP in cardiac units, recruiting post-discharged MI patients and offering them CRP does not impose high costs and can result in significant improvement in HRQoL measures. Authors: Dmitri Terzi
Abstract:
The article presents the results of an algorithmic study of a special optimization problem of the transport type (traveling salesman problem): 1) To solve the problem, a new natural algorithm has been developed based on the decomposition of the initial data into convex hulls, which has a number of advantages; it is applicable for a fairly large dimension, does not require a large amount of memory, and has fairly good performance. The relevance of the algorithm lies in the fact that, in practice, programs for problems with the number of traversal points of no more than twenty are widely used. For large-scale problems, the availability of algorithms and programs of this kind is difficult. The proposed algorithm is natural because the optimal solution found by the exact algorithm is not always feasible due to the presence of many other factors that may require some additional restrictions. 2) Another inverse problem solved here is to describe a class of traveling salesman problems that have a predetermined optimal solution. The constructed algorithm 2 allows us to characterize the structure of traveling salesman problems, as well as construct test problems to evaluate the effectiveness of algorithms and other purposes. 3) The appendix presents a software implementation of Algorithm 1 (in MATLAB), which can be used to solve practical problems, as well as in the educational process on operations research and optimization methods. Keywords: traveling salesman problem, solution construction algorithm, convex hulls, optimality verification

Authors: Magdy I. A. Alshourbagi
Abstract:
Background: The National Institute for Deafness and Communication Disorders defines idiopathic sudden sensorineural hearing loss as the idiopathic loss of hearing of at least 30 dB across 3 contiguous frequencies occurring within 3 days. The most common clinical presentation involves an individual experiencing a sudden unilateral hearing loss, tinnitus, a sensation of aural fullness and vertigo. The etiologies and pathologies of ISSNHL remain unclear. Several pathophysiological mechanisms have been described including: vascular occlusion, viral infections, labyrinthine membrane breaks, immune associated disease, abnormal cochlear stress response, trauma, abnormal tissue growth, toxins, ototoxic drugs and cochlear membrane damage. The rationale for the use of hyperbaric oxygen to treat ISSHL is supported by an understanding of the high metabolism and paucity of vascularity to the cochlea. The cochlea and the structures within it require a high oxygen supply. The direct vascular supply, particularly to the organ of Corti, is minimal. Tissue oxygenation to the structures within the cochlea occurs via oxygen diffusion from cochlear capillary networks into the perilymph and the cortilymph. . The perilymph is the primary oxygen source for these intracochlear structures. Unfortunately, perilymph oxygen tension is decreased significantly in patients with ISSHL. To achieve a consistent rise of perilymph oxygen content, the arterial-perilymphatic oxygen concentration difference must be extremely high. This can be restored with hyperbaric oxygen therapy. Subject and Methods: A 37 year old man was presented at the clinic with a five days history of muffled hearing and tinnitus of the right ear. Symptoms were sudden onset, with no associated pain, dizziness or otorrhea and no past history of hearing problems or medical illness. Family history was negative. Physical examination was normal. Otologic examination revealed normal tympanic membranes bilaterally, with no evidence of cerumen or middle ear effusion. Tuning fork examination showed positive Rinne test bilaterally but with lateralization of Weber test to the left side, indicating right ear sensorineural hearing loss. Audiometric analysis confirmed sensorineural hearing loss across all frequencies of about 70- dB in the right ear. Routine lab work were all within normal limits. Clinical diagnosis of idiopathic sudden sensorineural hearing loss of the right ear was made and the patient began a medical treatment (corticosteroid, vasodilator and HBO therapy). The recommended treatment profile consists of 100% O2 at 2.5 atmospheres absolute for 60 minutes daily (six days per week) for 40 treatments .The optimal number of HBOT treatments will vary, depending on the severity and duration of symptomatology and the response to treatment. Results: As HBOT is not yet a standard for idiopathic sudden sensorineural hearing loss, it was introduced to this patient as an adjuvant therapy. The HBOT program was scheduled for 40 sessions, we used a 12-seat multi place chamber for the HBOT, which was started at day seven after the hearing loss onset. After the tenth session of HBOT, improvement of both hearing (by audiogram) and tinnitus was obtained in the affected ear (right). Conclusions: In conclusion, HBOT may be used for idiopathic sudden sensorineural hearing loss as an adjuvant therapy. It may promote oxygenation to the inner ear apparatus and revive hearing ability. Patients who fail to respond to oral and intratympanic steroids may benefit from this treatment. Further investigation is warranted, including animal studies to understand the molecular and histopathological aspects of HBOT and randomized control clinical studies. Keywords: idiopathic sudden sensorineural hearing loss (issnhl), hyperbaric oxygen therapy (hbot), the decibel (db), oxygen (o2)
