1088 Factors Associated with Respiratory Distress Syndrome (RDS) in Preterm Neonates Admitted at FMIC in Kabul City: A Retrospective Cross-Sectional Study
Authors: Omid Ahmad Faizi
Abstract:
INTRODUCTION: Prematurity is one of the primary causes of infant death in Afghanistan, and it complicates a variety of critical issues, such as respiratory distress syndrome (RDS). Although RDS has been associated with serious consequences, there is a lack of scientific information on the associated factors of this problem in Afghanistan; hence, this study was undertaken to fill that gap. OBJECTIVES: The purpose of this study was to find the occurrence rate and associated factors of RDS in premature neonates. METHODS: This retrospective cross-sectional study was conducted at the Neonatal Intensive Care Unit of the French Medical Institute for Mothers and Children Hospital in Kabul City, Afghanistan (01/01/2022-30/12/2022). Statistical analysis was performed by SPSS 24. RESULT: A total of 78 preterm newborns were enrolled in this study, and respiratory distress syndrome developed in 51.3% of them. Based on gestational age, the occurrence rates of RDS within groups of extremely, early, moderate and late preterm neonates were 100%, 55.6%, 44%, and 35.7%, respectively. The occurrence of RDS was found to be 100% in extremely low birth weight, 56.2% in very low birth weight, and 58.8% in low birth weight neonates. The gestational ages of these infants had a positive correlation with birth weight (r = 0.648, p = 0.01, n = 78). The preterm neonates in the RDS group versus the non-RDS group had a mean birth weight of (1610±314.4 g vs 1981±520.3 g), a mean gestational age of (31.65±2.2 w vs 33.18± 2.10 w) and a mean hemoglobin level of (13.85± 3.28 g vs16.09± 3.26 g). There was a significant association between RDS and neonatal anemia (AOR=5.9), neonatal sepsis (AOR=4.2), vaginal delivery (AOR=8.7), delivery at low-resourced settings (AOR=2.7), PROM (AOR=4), and antepartum hemorrhage (6.9). The mortality rate in preterm neonates was found to be 26.8% and was significantly associated with very and extremely low birth weights (AOR=8.2), early and extremely preterm births (AOR=6.3), female gender (AOR=3.8), antepartum hemorrhage (AOR=4.6,) and PROM (AOR=5.7). CONCLUSION: RDS was highly prevalent in preterm newborns, and the highest rates were seen within groups of extremely preterm and extremely low-birth-weight newborn babies. RDS was found to be associated with lower neonatal birth weight, gestational age, and hemoglobin level, as well as neonatal anemia, neonatal sepsis, vaginal delivery, birth in low-resource settings, PROM, and antepartum hemorrhage. The neonatal mortality in preterm neonates was higher than in high-income countries. Proper management of the aforementioned associated factors will reduce the incidence of RDS and neonatal mortality in preterm neonates. Keywords: respiratory distress syndrome (RDS), preterm neonates, prematurity, neonatal mortality

Authors: Zohreh Ramin
Abstract:
One of the most important contributions that Edward Said has had in the realm of critical theory is his insistence on the worldliness of the text and the critic. By this, Said meant that the critic and the text must be considered in their ‘material’ contexts. Foregrounding the substantial role of a critic as embodying what he refers to as ‘critical consciousness’, a true critic, Said maintains, is one who can stand between the ‘dominant culture’ and ‘the totalizing forms of critical systems.’ Considered as one of Iran’s major contemporary intellectuals, Jalal Ale Ahmad is responsible for introducing the idea of ‘Westoxication’ in Iran, constructing a social paradigm of the necessity to return to tradition in contemporary Iran. The present paper intends to study Al-Ahmad’s definition of the orient versus the occident, his criticism of the ‘machination’ of contemporary Iranian society, and his solution to the problem of ‘Westoxication’. The objective of this study is to see whether Ale Ahmad can be considered as embodying the spirit of ‘critical consciousness’ as described by Said as the necessary tool in the hands of an intellectual who is simultaneously attached filitavely to his culture but can detach himself affilitavely through employing critical consciousness. Keywords: Westoxication, filiative, affiliative, machination

Authors: S. A. Faizi, S. Yoshitomi
Abstract:
The optimal design procedure of buckling restrained braces (BRBs) in reinforced concrete (RC) building structures can provide the distribution of horizontal stiffness of BRBs at each story, which minimizes story drift response of the structure under the constraint of specified total stiffness of BRBs. In this paper, a simple rule is proposed to convert continuous horizontal stiffness of BRBs into sectional sizes of BRB which are available from standardized section list assuming realistic structural design stage. Keywords: buckling restrained brace, building engineering, optimal damper placement, structural engineering

Authors: Burkhard Hehenkamp, Tahmina Faizi
Abstract:
In this paper, we analyze the formation of cartels along with the stability of the cartel for the case of three firms that produce differentiated products and differ in their cost of production. Both cost and demand are linear, and firms compete in quantities once a cartel has been formed (or not). It turns out that the degree of product differentiation has a direct effect on the incentive to form a cartel. Firstly, when goods are complements or close substitutes, firms form a grand coalition. Secondly, for weak and medium substitutes, the firm with the lowest cost prefers to remain independent, while both other firms form a coalition. We also find that the producer profit of the stable coalition structure is nonmonotonic in the degree of product differentiation. Keywords: collusion, cartel formation, cartel stability, differentiated market, quantity competition, oligopolies

Authors: Ahmad Esmaillzadeh, Javad Anjom-Shoae, Omid Sadeghi,
Abstract:
Background: Although considerable research has been devoted to the link between consumption of legume and nuts and metabolic abnormalities, few studies have examined legume and nuts consumption in relation to psychological disorders. Objective: The current study aimed to examine the association of legume and nuts consumption with depression, anxiety and psychological distress in Iranian adults. Methods: This cross-sectional study was carried out among 3172 adult participants aged 18-55 years. Assessment of legume and nuts consumption was conducted using a validated dish-based 106-item semi-quantitative food frequency questionnaire. The Iranian validated version of Hospital Anxiety and Depression Scale (HADS) was used to examine psychological health. Scores of 8 or more on either subscale in the questionnaire were considered to indicate the presence of depression or anxiety. Data on psychological distress were collected through the use of General Health Questionnaire (GHQ), in which the score of 4 or more was considered as having psychological distress. Results: Mean age of participants was 36.5±7.9 years. Compared with the lowest quintile, men in the highest quintile of legume and nuts consumption had lower odds of anxiety; such that after adjusting for potential confounding variables, men in the top quintile of legume and nuts consumption were 66% less likely to be anxious than those in the bottom quintile (OR: 0.34; 95% CI: 0.14-0.82). Such relationship was not observed among women. We failed to find any significant association between legume plus nuts consumption and depression or psychological distress after adjustment for potential confounders. Conclusion: We found that consumption of legume and nuts was associated with lower odds of anxiety in men, but not in women. No significant association was seen between consumption of legume and nuts and odds of depression or psychological disorder. Further prospective studies are required to confirm these findings. Keywords: anxiety, depression, legumes, nuts, psychological distress

Abstract:
The objective of this study was to synthesize the smallest possible size of ZnO NPs using a modified wet chemical synthesis method and to prepare core shell using polyethylene glycol (PEG) as shell material. Advanced and sophisticated techniques were used to confirm the synthesis, size, and shape of these NPs. Rounded, clustered NPs of size 5.343 nm were formed. Both the plain and core shell NPs were tested against MDR bacteria (E. cloacae, E. amnigenus, Shigella, S. odorifacae, Citrobacter, and E. coli). Both of the NPs showed excellent antibacterial properties, whereas E. cloacae showed maximum zone of inhibition of 16 mm, 27 mm, and 32 mm for 500 μg/ml, 1000 μg/ml, and 1500 μg/ml, respectively for plain ZnO NPs and 18 mm, 28 mm and 35 mm for 500 μg/ml, 1000 μg/ml and 1500 μg/ml for core shell NPs. These NPs were also biocompatible on human red blood cells showing little hemolysis of only 4% for 70 μg/ml for plain NPs and 1.5% for 70 μg/ml for core shell NPs. Core shell NPs were highly biocompatible because of the PEG. Their therapeutic effect as photosensitizers in photodynamic therapy (PDT) for cancer treatment was also monitored. The cytotoxicity of ZnO and PEG-ZnO was evaluated using MTT assay. Our results demonstrated that these NPs could generate ROS inside tumor cells after irradiation which in turn initiates an apoptotic pathway leading to cell death hence proving to be an effective candidate for PDT.Keywords: ZnO, hemolysis, cytotoxiciy assay, photodynamic therapy, antibacterial

Authors: Ahmad Zahran, Ahmed Herzallah, Ahmad Ahmad, Mahran Quraan
Abstract:
Generation of high DC voltages is necessary for testing the insulation material of high voltage AC transmission lines with long lengths. The harmonic and ripple contents of the output DC voltage supplied by high voltage DC circuits require the use of costly capacitors to smooth the output voltage after rectification. This paper proposes a new modular multiplier high voltage DC generator with embedded Cockcroft-Walton circuits that achieve a negligible harmonic and ripple contents of the output DC voltage without the need for costly filters to produce a nearly constant output voltage. In this new topology, Cockcroft-Walton modules are connected in series to produce a high DC output voltage. The modules are supplied by low input AC voltage sources that have the same magnitude and frequency and shifted from each other by a certain angle to eliminate the harmonics from the output voltage. The small ripple factor is provided by the smoothing column capacitors and the phase shifted input voltages of the cascaded modules. The constituent harmonics within each module are determined using Fourier analysis. The viability of the proposed DC generator for testing purposes and the effectiveness of the cascaded connection are confirmed by numerical simulations using MATLAB/Simulink.Keywords: Cockcroft-Walton circuit, harmonics, ripple factor, HVDC generator

Abstract:
This paper proposes a universal wireless battery charger design for portable electronic devices. As the number of portable electronics devices increases, the demand for more flexible and reliable charging techniques is becoming more urgent. A wireless battery charger differs from a traditional charger in the way the power transferred to the battery. In the latter, the power is transferred through electrical wires that connect the charger terminals to the battery terminals, while in the former; the power is transferred by induction without electrical connections. With a detection algorithm that detects the battery size and chemistry, the proposed charger will be able to accommodate a wide range of applications, and will allow a more flexible and reliable option to most of today’s portable electronics. Keywords: efficiency, magnetically-coupled resonators, resonance frequency, wireless power transfer

Abstract:
The generalized wave equation models various problems in sciences and engineering. In this paper, a new three-time level implicit approach based on cubic trigonometric B-spline for the approximate solution of wave equation is developed. The usual finite difference approach is used to discretize the time derivative while cubic trigonometric B-spline is applied as an interpolating function in the space dimension. Von Neumann stability analysis is used to analyze the proposed method. Two problems are discussed to exhibit the feasibility and capability of the method. The absolute errors and maximum error are computed to assess the performance of the proposed method. The results were found to be in good agreement with known solutions and with existing schemes in literature. Keywords: collocation method, cubic trigonometric B-spline, finite difference, wave equation

Authors: Samina Jam Nazeer Ahmad, Samia Yasin, Ijaz Ahmad, Muhammad Tahir, Jam Nazeer Ahmad
Abstract:
Phytoplasmas are phloem-inhibited plant pathogenic bacteria that are transferred by insect vectors. Among biotic factors, Phytoplasma infection induces abnormality influencing the physiology as well as morphology of plants. In 16Sr-IX group phytoplasma-infected brassica compestris, flower abnormalities have been associated with changes in the expression of floral development genes. To determine whether methylation was involved in down-regulation of flower development, the process of DNA methylation and Demethylation was investigated as a possible mechanism for regulation of floral gene expression in phytoplasma infected Brassica transmitted by Orosious orientalis vector by using RT-PCR, MSRE-PCR, Southern blotting, Bisulfite Sequencing, etc. Transcriptional expression of methylated genes was found to be globally down-regulated in plants infected with phytoplasma, but not severely in those infested by insect vectors and variation in expression was found in genes involved in methylation. These results also showed that genes particularly orthologous to Arabidopsis APETALA3 involved in petal formation and flower development was down-regulated severely in phytoplasma-infected brassica and with the fact that phytoplasma and insect induce variation in developmental gene expression. The DNA methylation status of flower developmental gene in phytoplasma infected plants with 5-azacytidine restored gene expression strongly suggesting that DNA methylation was involved in down-regulation of floral development genes in phytoplasma infected brassica. Keywords: genes expression, phytoplasma, DNA methylation, flower development

Authors: Monia Hejaiej
Abstract:
The fable has lent itself to memorable imitations and reinventions. The writing of fables, in prose and verse, was widely cultivated not only in pre-Islamic Arabia but also in the middle ages, reaching its culmination with the Egyptian poet and man of letters Ahmad Shawqī (1989-1932), who revived the ancient tradition, a relatively minor and unexploited genre in the modern era, and re-wrote rimed fables with an Arab Islamic flavor, articulating a set of modern ethico-political concepts and sensibilities such as a belief in good judgment in governance, individual liberty, democracy, a sense of the brotherhood of man and justice. This essay aims to restore the 20th Century poet to his rightful place in the international pantheon of literary achievement, and offers an examination of the Arabian fabulist tradition as it appears in Arabic literature, and a treatment of this genre re-visiting a few representative samples of Ahmad Shawqī collection of fables and their implications for contemporary politics in the Middle East. Keywords: fable, politcs, governace, democracy, ethics of care

Abstract:
High-speed infrared vertical-cavity surface-emitting laser diodes (VCSELs) with Cu-plated heat sinks were fabricated and tested. VCSELs with 10 mm aperture diameter and 4 mm of electroplated copper demonstrated a -3dB modulation bandwidth (f-3dB) of 14 GHz and a resonance frequency (fR) of 9.5 GHz at a bias current density (Jbias) of only 4.3 kA/cm2, which corresponds to an improved f-3dB2/Jbias ratio of 44 GHz2/kA/cm2. At higher and lower bias current densities, the f-3dB2/ Jbias ratio decreased to about 30 GHz2/kA/cm2 and 18 GHz2/kA/cm2, respectively. Examination of the analogue modulation response demonstrated that the presented VCSELs displayed a steady f-3dB/ fR ratio of 1.41±10% over the whole range of the bias current (1.3Ith to 6.2Ith). The devices also demonstrated a maximum modulation bandwidth (f-3dB max) of more than 16 GHz at a bias current less than the industrial bias current standard for reliability by 25%.Keywords: current density, high-speed VCSELs, modulation bandwidth, small-signal characteristics, thermal impedance, vertical-cavity surface-emitting lasers

Abstract:
In its thousand-year development, architecture has gained valuable patterns. Iran’s desert regions possess developed patterns of traditional architecture and outstanding skeletal features. Unfortunately increasing population and urbanization growth in the past decade as well as the lack of harmony with environment’s texture has destroyed such permanent concepts in the building’s skeleton, causing a lot of energy waste in the modern architecture. The important question is how cooling patterns of Iran’s traditional architecture can be used in a new way in the modern architecture of residential buildings? This research is library-based and documental that looks at sustainable development, analyzes the features of Iranian architecture in hot and dry climate in terms of sustainability as well as historical patterns, and makes a model for real environment. By methodological analysis of past, it intends to suggest a new pattern for residential buildings’ cooling in Iran’s hot and dry climate which is in full accordance to the ecology of the design and at the same time possesses the architectural indices of the past. In the process of cities’ physical development, ecological measures, in proportion to desert’s natural background and climate conditions, has kept the natural fences, preventing buildings from facing climate adversities. Designing and construction of buildings with this viewpoint can reduce the energy needed for maintaining and regulating environmental conditions and with the use of appropriate building technology help minimizing the consumption of fossil fuels while having permanent patterns of desert buildings’ architecture. Keywords: sustainability concepts, sustainable development, energy climate architecture, fossil fuel, hot and dry climate, patterns of traditional sustainability for residential buildings, modern pattern of cooling

Abstract:
Natural diatomite was modified by grinding and acid treatment to increase surface area and to decrease the impurities. Surface area and pore volume of the modified diatomite were 67.45 m² g-1 and 0.105 cm³ g-¹ respectively, and used to immobilize Pb, Zn and Cu in an urban soil. The modified diatomite was added to soil samples at the rates of 2.5, 5, 7.5 and 10% and the samples incubated for 60 days. The addition of modified diatomite increased SSA of the soil. The SSAs of soils with 2.5, 5.0, 7.5 and 10% modified diatomite were 20.82, 22.02, 23.21 and 24.41 m² g-¹ respectively. Increasing the SSAs of the soils by the application of modified diatomite reduced the DTPA extractable concentrations of heavy metals compared with un-amendment control. The concentration of Pb, Zn and Cu were reduced by 91.1%, 82% and 91.1% respectively. Modified diatomite reduced the concentration of Exchangeable and Carbonate bounded species of Pb, Zn and Cu, compared with the control. Also significantly increased the concentration of Fe Mn- OX (Fe-Mn Oxides) and OM (Organic Matter) bound and Res (Residual) fraction. Modified diatomite increased the urease, dehydrogenase and alkaline phosphatase activity by 52%, 57% and 56.6% respectively. Keywords: modified diatomite, chemical specifications, specific surface area, enzyme activity, immobilization, heavy metal, soil remediation

Abstract:
Cantilever beam is a simplified sample of a lot of mechanical components used in a wide range of applications, including many industries such as gas turbine blade. Due to the nature of the operating conditions, beams are subject to variety of damages especially crack propagates. Crack propagation may lead to catastrophic failure during operation. Therefore, online detection of crack presence and its propagation is very important and may reduce possible significant cost of the whole system failure. This paper aims to investigate the effect of cracks presence and crack propagation on one end fixed beam`s vibration. A finite element model will be developed for the blade in which the modal response of the structure with and without crack will be studied. Keywords: blade, crack propagation, health monitoring, modal analysis
