1372 The Current Home Hemodialysis Practices and Patients’ Safety Related Factors: A Case Study from Germany

Abstract:
The increasing costs of healthcare on one hand, and the rise in aging population and associated chronic disease, on the other hand, are putting increasing burden on the current health care system in many Western countries. For instance, chronic kidney disease (CKD) is a common disease and in Europe, the cost of renal replacement therapy (RRT) is very significant to the total health care cost. However, the recent advancement in healthcare technology, provide the opportunity to treat patients at home in their own comfort. It is evident that home healthcare offers numerous advantages apparently, low costs and high patients’ quality of life. Despite these advantages, the intake of home hemodialysis (HHD) therapy is still low in particular in Germany. Many factors are accounted for the low number of HHD intake. However, this paper is focusing on patients’ safety-related factors of current HHD practices in Germany. The aim of this paper is to analyze the current HHD practices in Germany and to identify risks related factors if any exist. A case study has been conducted in a dialysis center which consists of four dialysis centers in the south of Germany. In total, these dialysis centers have 350 chronic dialysis patients, of which, four patients are on HHD. The centers have 126 staff which includes six nephrologists and 120 other staff i.e. nurses and administration. The results of the study revealed several risk-related factors. Most importantly, these centers do not offer allied health services at the pre-dialysis stage, the HHD training did not have an established curriculum; however, they have just recently developed the first version. Only a soft copy of the machine manual is offered to patients. Surprisingly, the management was not aware of any standard available for home assessment and installation. The home assessment is done by a third party (i.e. the machines and equipment provider) and they may not consider the hygienic quality of the patient’s home. The type of machine provided to patients at home is similar to the one in the center. The model may not be suitable at home because of its size and complexity. Even though portable hemodialysis machines, which are specially designed for home use, are available in the market such as the NxStage series. Besides the type of machine, no assistance is offered for space management at home in particular for placing the machine. Moreover, the centers do not offer remote assistance to patients and their carer at home. However, telephonic assistance is available. Furthermore, no alternative is offered if a carer is not available. In addition, the centers are lacking medical staff including nephrologists and renal nurses.

Abstract:
Based on three dimensional potential flow theory and hinged rigid body motion equations, structure RAOs of Pelamis wave energy converter is analyzed. Analysis of numerical simulation is carried out on Pelamis in the irregular wave conditions, and the motion response of structures and total generated power is obtained. The paper analyzes influencing factors on the average power including diameter of floating body, section form of floating body, draft, hinged stiffness and damping. The optimum parameters are achieved in Zhejiang Province. Compared with the results of the pelamis experiment made by Glasgow University, the method applied in this paper is feasible.

Abstract:
Environment both endowed and built are essential for tourism. However tourism and environment maintains a complex relationship, where in most cases environment is at the receiving end. Many tourism development activities have adverse environmental effects, mainly emanating from construction of general infrastructure and tourism facilities. These negative impacts of tourism can lead to the destruction of precious natural resources on which it depends. These effects vary between locations; and its effect on a hill destination is highly critical. This study aims at developing a Sustainable Tourism Planning Model for an environmentally sensitive tourism destination in Kerala, India. Being part of the Nilgiri mountain ranges, Munnar falls in the Western Ghats, one of the biological hotspots in the world. Endowed with a unique high altitude environment Munnar inherits highly significant ecological wealth. Giving prime importance to the protection of this ecological heritage, the study proposes a tourism planning model with resource conservation and sustainability as the paramount focus. Conceiving a novel approach towards sustainable tourism planning, the study proposes to assess tourism attractions using Ecological Sensitivity Index (ESI) and Tourism Attractiveness Index (TAI). Integration of these two indices will form the Ecology – Tourism Matrix (ETM), outlining the base for tourism planning in an environmentally sensitive destination. The ETM Matrix leads to a classification of tourism nodes according to its Conservation Significance and Tourism Significance. The spatial integration of such nodes based on the Hub & Spoke Principle constitutes sub – regions within the STZ. Ensuing analyses lead to specific guidelines for the STZ as a whole, specific tourism nodes, hubs and sub-regions. The study results in a multi – dimensional output, viz., (1) Classification system for tourism nodes in an environmentally sensitive region/ destination (2) Conservation / Tourism Development Strategies and Guidelines for the micro and macro regions and (3) A Sustainable Tourism Planning Tool particularly for Ecologically Sensitive Destinations, which can be adapted for other destinations as well.Keywords: Tourism, Environment, Spatial Planning, Model

Abstract:
This study investigated how the concrete confinement strength and axial load carrying capacity of reinforced concrete columns are affected by corrosion damage to the stirrups. A total of small-scale 12 test specimens were cast for evaluating the effect of stirrup corrosion on confinement strength of concrete. The results of this study show that the stirrup corrosion alone dramatically decreases the axial load carrying capacity of corroded reinforced concrete columns. Recommendations were presented for improved inspection practices which will allow estimating concrete confinement strength of corrosion-damaged reinforced concrete bridge columns.

Abstract:
Climate change projections based on the emission of greenhouse effect gases suggest an increase in the concentration of atmospheric carbon dioxide, in up to 750 ppm. In this scenario, we have significant changes in plant development, and consequently, in agricultural systems. This study aims to evaluate the interaction between culture (Glycine max) and weed (Amaranthus viridis and Euphorbia heterophylla) in two conditions of CO2, 400 and 800 ppm. The results showed that the coexistence of culture with both weed species resulted in a mutual loss, with decrease in dry mass productivity of culture + weeds, in both conditions of CO2. However, when the culture is grown in association with E. heterophylla, total dry mass of culture + weed was smaller at 800 ppm. Soybean was more aggressive in comparison to the A. viridis in both the concentrations of CO2, but not in relation to the E. heterophylla.

Abstract:
Biological treatment of secondary effluent wastewater by two combined denitrification/oxic filtration systems packed with Lock type(denitrification filter) and ceramic ball (oxic filter) has been studied for 5months. Two phases of operating conditions were carried out with an influent nitrate and ammonia concentrations varied from 5.8 to 11.7mg/L and 5.4 to 12.4mg/L,respectively. Denitrification/oxic filter treatment system were operated under an EBCT (Empty Bed Contact Time) of 4h at system recirculation ratio in the range from 0 to 300% (Linear Velocity increased 19.5m/d to 78m/d). The system efficiency of denitrification , nitrification over 95% respectively. Total nitrogen and COD removal range from 54.6%(recirculation 0%) to 92.3%(recirculation 300%) and 10% to 62.5%, respectively.Keywords: Advanced treatment , Biofilter, Nitrogen removal, Two combined denitrification/oxic filter

Small tanks, the ancient man-made rain water storage systems, support the pheasant life and agriculture of the dry zone of Sri Lanka. Many small tanks were abandoned with time due to various reasons. Such tanks, rehabilitated in the recent past, were found to be less sustainable and most of these rehabilitation approaches have failed. The objective of this research is to assess the impact of the rehabilitation approaches in the management of small tanks in the Kurunegala District of Sri Lanka with respect to eight small tanks. A Sustainability index was developed using seven indicators representing the ability and commitment of the villagers to maintain these tanks. The sustainability index of the eight tanks varied between 79.2 and 47.2 out of a total score of 100. The conclusion is that, the approaches used for tank rehabilitation have a significant effect on the sustainability of the management of these small tanks.Keywords: Minor irrigation schemes, Participatory, Small Tanks, Sustainable, Water resource management.

Abstract:
Many optimization techniques available in the literature have been developed in order to solve the problem of voltage stability enhancement in power systems. However, there are a number of drawbacks in the use of previous techniques aimed at determining the optimal location and size of reactive compensators in a network. In this paper, an Improved Cuckoo Search algorithm is applied as an appropriate optimization algorithm to determine the optimum location and size of a Static Var Compensator (SVC) in a transmission network. The main objectives are voltage stability improvement and total cost minimization. The results of the presented technique are then compared with other available optimization techniques.

Abstract:
This paper presents a transmission planning methodology that considers the conversion of HVAC transmission lines to HVDC as an alternative of expansion of power systems, as a consequence of restrictions for the construction of new lines. The transmission expansion planning problem formulates an optimization problem that minimizes the total cost that includes the investment cost to convert lines from HVAC to HVDC and possible required reinforcements of the power system prior to the conversion. The costs analysis assesses the impact of the conversion on the reliability because transmission lines are out of service during the conversion work. The accelerated growth in aircraft industries desire effectual schemes, programs, innovative designs of advanced systems and facilities to accomplish the augmenting need for home-free air transportation. In this paper, a contemporary conceptual design of a cambered airfoil has been proposed in order to providing augmented effective lift force relative to the airplane, and to eliminating drawbacks and limitations of an airfoil in a commercial airplane by using a kind of smart materials. This invention of an unsymmetrical airfoil structure utilizes the amplified air momentum around the airfoil and increased camber length to providing improved aircraft performance and assist to enhancing the reliability of the aircraft components. Moreover, this conjectured design helps to reducing airplane weight and total drag.

In this paper, the problem of reducing switching activity in on-chip buses at the stage of high-level synthesis is considered, and a high-level low power bus binding based on dynamic bit reordering is proposed. Whereas conventional methods use a fixed bit ordering between variables within a bus, the proposed method switches a bit ordering dynamically to obtain a switching activity reduction. As a result, the proposed method finds a binding solution with a smaller value of total switching activity (TSA). Experimental result shows that the proposed method obtains a binding solution having 12.0-34.9% smaller TSA compared with the conventional methods.Keywords: bit reordering, bus binding, low power, switching activity matrix

In recent decades, the lean methodology, and the development of its principles and concepts have widely been applied in supply chain management. One of the most important strategies of being lean is having efficient inventory within the chain. On the other hand, managing inventory efficiently requires appropriate management of safety stock in order to protect against increasing stretch in the breaking points of the supply chain, which in turn can result in possible reduction of inventory. This paper applies a safety stock cost minimization model in a manufacturing company. Bone hydatidosis is an infection in worldwide distribution. Although there is no evidence in literature on Bone Hydatid disease in Libya, we tried to present the first Epidemiological study of this disease in Eastern Libya through retrospective study from 1995 to 2013. Our data were collected from 3 hospitals in Eastern Libya particularly the sheep-raising areas with total number of musculoskeletal infection cases of two thousand one hundred ninety four (2,194). There were five (5) five cases of bone infection, four (4) of it have been diagnosed after more than three (3) months. Our study is comparable to other international study but this type of bone infection need further studies for effective control strategies for all dogs to avoid serious complications that might happened from the delay in diagnosing this type of disease.

Optimization of extraction of phenolic compounds from Avicennia marina using response surface methodology was carried out during the present study. Five levels, three factors rotatable design (CCRD) was utilized to examine the optimum combination of extraction variables based on the TPC of Avicennia marina leaves. The best combination of response function was 78.41 °C, drying temperature; 26.18°C; extraction temperature and 36.53 minutes of extraction time. Joint ventures are one of the most frequently used contractual forms in strategic alliances. There are various motivations for cooperation between two or more firms e.g., accessing to technical know-how, accessing to financial resources and managing risks. The firms must know about these motivations to encourage for establishing joint venture. So, it is important for managers to understand about these motives. On the other hand, the cooperation section is one of the most effective parts in each country. In this way, our study identifies goals of joint venture between cooperative manufacturing firms, and prioritizes those using TOPSIS1. A 7-step method (with 25 sub-steps) to assess risk of air pollutants is introduced. These steps are: pre-considerations, sampling, statistical analysis, exposure matrix and likelihood, doseresponse matrix and likelihood, total risk evaluation, and discussion of findings. All mentioned words and expressions are wellunderstood; however, almost all steps have been modified, improved, and coupled in such a way that a comprehensive method has been prepared. Accordingly, the SADRA (Statistical Analysis-Driven Risk Assessment) emphasizes extensive and ongoing application of analytical statistics in traditional risk assessment models. Abstract:
This research proposes Bee Algorithm (BA) to optimize Ready Mixed Concrete (RMC) truck scheduling problem from single batch plant to multiple construction sites. This problem is considered as an NP-hard constrained combinatorial optimization problem. This paper provides the details of the RMC dispatching process and its related constraints. BA was then developed to minimize total waiting time of RMC trucks while satisfying all constraints. The performance of BA is then evaluated on two benchmark problems (3 and 5construction sites) according to previous researchers. The simulation results of BA are compared in term of efficiency and accuracy with Genetic Algorithm (GA) and all problems show that BA approach outperforms GA in term of efficiency and accuracy to obtain optimal solution. Hence, BA approach could be practically implemented to obtain the best schedule.

Abstract:
In this investigation, rubberized HMA technology was examined to address the most critical forms of pavement distresses in the State of Kuwait, namely, high temperature rutting, and moisture induced raveling. PelletPAVE™ additive was selected as the preferred technology, since it offered a convenient method of directly modifying the exiting local HMA recipe without having to polymer modify the bitumen. Experimental work, using various Pelletpave contents was carried out at Kuwait Institute for Scientific Research (KISR) to design an optimum rubberized HMA formulation prior to conducting a pilot-scale road trial. With the aid of a gyratory compactor, the compaction and volumetric properties of HMAs containing 2.5% and 3.0% Pelletpave additive were investigated at a range of bitumen contents, all by mass of total mix.

Timing driven physical design, synthesis, and optimization tools need efficient closed-form delay models for estimating the delay associated with each net in an integrated circuit (IC) design. The total number of nets in a modern IC design has increased dramatically and exceeded millions. Therefore efficient modeling of interconnection is needed for high speed IC-s. This paper presents closed–form expressions for RC and RLC interconnection trees in current mode signaling, which can be implemented in VLSI design tool. These analytical model expressions can be used for accurate calculation of delay after the design clock tree has been laid out and the design is fully routed. Abstract:
In this paper we address a multi-objective scheduling problem for unrelated parallel machines. In unrelated parallel systems, the processing cost/time of a given job on different machines may vary. The objective of scheduling is to simultaneously determine the job-machine assignment and job sequencing on each machine. In such a way the total cost of the schedule is minimized. The cost function consists of three components, namely; machining cost, earliness/tardiness penalties and makespan related cost. Such scheduling problem is combinatorial in nature. Therefore, a Simulated Annealing approach is employed to provide good solutions within reasonable computational times. Computational results show that the proposed approach can efficiently solve such complicated problems.

This paper examines the students’ self-concept among 16- and 17- year- old adolescents in Malaysian secondary schools. Previous studies have shown that positive self-concept played an important role in student adjustment and academic performance during schooling. This study attempts to investigate the factors influencing students’ perceptions toward their own self-concept. A total of 1168 students participated in the survey. This study utilized the CoPs (UM) instrument to measure self-concept. Principal Component Analysis (PCA) revealed three factors: academic selfconcept, physical self-concept and social self-concept. This study confirmed that students perceived certain internal context factors, and revealed that external context factor also have an impact on their self-concept.

It is necessary to be aware of the fact that firm's success in the contemporary business environment depends not only on the technical equipment, financial resources, availability of raw materials and information and effective management, but one of the crucial factors of firm's success is its human potential. The article emphasizes that the well working remuneration system has a very important position in the organization in the broadest sense. Polylactic acid (PLA) is the most commercially available bio-based and biodegradable plastic at present. PLA has been used in plastic related industries including single-used containers, disposable and environmentally friendly packaging owing to its renewability, compostability, biodegradability, and safety. Although PLA demonstrates reasonably good optical, physical, mechanical and barrier properties comparable to the existing petroleum-based plastics, its brittleness and mold shrinkage as well as its price are the points to be concerned for the production of rigid and semi-rigid packaging. Blending PLA with other bio-based polymers including thermoplastic starch (TPS) is an alternative not only to achieve a complete bio-based plastic, but also to reduce the brittleness, shrinkage during molding and production cost of the PLA-based products. TPS is a material produced mainly from starch which is cheap, renewable, biodegradable, compostable, and nontoxic. It is commonly prepared by a plasticization of starch under applying heat and shear force. Although glycerol has been reported as one of the most plasticizers used for preparing TPS, its migration caused the surface stickiness of the TPS products. In some cases, mixed plasticizers or natural fibers have been applied to impede the retrogradation of starch or reduce the migration of glycerol. The introduction of fibers into TPS-based materials could reinforce the polymer matrix as well. Therefore, the objective of the present research is to study the effect of starch type (i.e. native starch and phosphate starch), plasticizer type (i.e. glycerol and xylitol with a weight ratio of glycerol to xylitol of 100:0, 75:25, 50:50, 25:75 and 0:100) and fiber content (i.e. in the range of 1-25 %wt) on properties of PLA/TPS blend and composite. PLA/TPS blends and composites were prepared using a twin-screw extruder and then converted into dumbbell-shaped specimens using an injection molding machine. The PLA/TPS blends prepared by using phosphate starch showed higher tensile strength and stiffness than the blends prepared by using native one. In contrast, the blends from native starch exhibited higher extensibility and heat distortion temperature (HDT) than those from the modified starch. Increasing xylitol content resulted in enhanced tensile strength, stiffness and water resistance, but decreased extensibility and HDT of the PLA/TPS blend. Tensile properties and hydrophobicity of the blend could be improved by incorporating silane treated-jute fibers.Keywords: Polylactic acid, Thermoplastic starch, Jute fiber, Composite, Blend.

Abstract:
This work is carried out to evaluate the possibility of using to-be-wasted steel as reinforcement after welding together pieces of reinforcing steel bars, left over during construction activities. Tests were performed on a total of nine samples. These were made by welding pieces of reinforcing steel bars purchased from the local scrap steel market. The samples were tested in uniaxial tension using a universal testing machine (UTM). It was found that the failure of the welded bars is governed by the thickness of the weld. It is concluded that suitable design of the weld is essential for achieving the desired level of ductility/elongation of these bars, if they are to be used as conventional reinforcement in reinforced concrete members.

Abstract:
The main objective of this study was to investigate particulate matter concentration on main and secondary roadsides in urban area and study the concentration of some heavy metals including lead (Pb), zinc (Zn), copper (Cu) and cadmium (Cd) in particulate matter in the Bangkok area.
The averaged particle concentration for main roadsides is higher than secondary roadsides. The particulate matter less than 10 micron concentration contribute the majority of the Total Suspended Particulate matter for main roads and zinc concentrations were higher than copper and lead for both sites.

Abstract:
Urban planning is associated with land transformation from natural areas to modified and developed ones which leads to modification of natural environment. The basic knowledge of relationship between both should be ascertained before proceeding for the development of natural areas. Changes on land surface due to build up pavements, roads and similar land cover, affect surface water flow. There is a gap between urban planning and basic knowledge of hydrological processes which should be known to the planners. The paper aims to identify these variations in surface flow due to urbanization for a temporal scale of 40 years using Storm Water Management Mode (SWMM) and again correlating these findings with the urban planning guidelines in study area along with geological background to find out the suitable combinations of land cover, soil and guidelines. For the purpose of identifying the changes in surface flows, 19 catchments were identified with different geology and growth in 40 years facing different ground water levels fluctuations. The increasing built up, varying surface runoff are studied using Arc GIS and SWMM modeling, regression analysis for runoff. Resulting runoff for various land covers and soil groups with varying built up conditions were observed. The modeling procedures also included observations for varying precipitation and constant built up in all catchments. All these observations were combined for individual catchment and single regression curve was obtained for runoff. Thus, it was observed that alluvial with suitable land cover was better for infiltration and least generation of runoff but excess built up could not be sustained on alluvial soil. Similarly, basalt had least recharge and most runoff demanding maximum vegetation over it. Sandstone resulted in good recharging if planned with more open spaces and natural soils with intermittent vegetation. Hence, these observations made a keystone base for planners while planning various land uses on different soils. This paper contributes and provides a solution to basic knowledge gap, which urban planners face during development of natural surfaces.

Abstract:
The study investigated the 2011 TPGA Ever Rich Championship – North Bay Open spectators- on-the-site spectating motivations, experiences, and satisfactions. The research was conducted on a convenience sample of the on-the-spot spectators at the North Bay Golf and Country Club. A total of 200 questionnaires were distributed, of which 185 valid questionnaires were collected, approaching a 92.5% response rate. The data obtained was analyzed with statistical techniques. First, the data showed significant differences in motivations, experiences, and satisfactions relative to demographic variables among the on-the-spot spectators. Second, spectating motivation, experience, and satisfaction were significantly related to one another.Keywords: Spectating motivation, spectating experience, spectating satisfaction.

Abstract:
A new algorithm called Character-Comparison to Character-Access (CCCA) is developed to test the effect of both: 1) converting character-comparison and number-comparison into character-access and 2) the starting point of checking on the performance of the checking operation in string searching. An experiment is performed using both English text and DNA text with different sizes. The results are compared with five algorithms, namely, Naive, BM, Inf_Suf_Pref, Raita, and Cycle. With the CCCA algorithm, the results suggest that the evaluation criteria of the average number of total comparisons are improved up to 35%. Furthermore, the results suggest that the clock time required by the other algorithms is improved in range from 22.13% to 42.33% by the new CCCA algorithm.

Abstract:
Heat transfer of leaves is a crucial factor in optimal operation of metabolic functions in plants. In order to quantify this phenomenon in different leaves and investigate the influence of leaf shape on heat transfer, natural convection for pine, orange and olive leaves was simulated as representatives of different groups of leaf shapes. CFD techniques were used in this simulation with the purpose to calculate heat transfer of leaves in similar environmental conditions. The problem was simulated for steady state and threedimensional conditions. From obtained results, it was concluded that heat fluxes of all three different leaves are almost identical, however, total rate of heat transfer have highest and lowest values for orange leaves, and pine leaves, respectively.Keywords: Computational fluid dynamic, heat flux, heat transfer, natural convection.

This paper is concerned about optimizing the selection of the manufacturing process along with the material used in the product under performance indices and availability constraints. In this paper, the material selection problem is formulated using binary programming and solved by genetic algorithm.
