1876 Exploring Empathy Through Patients’ Eyes: A Thematic Narrative Analysis of Patient Narratives in the UK
Authors: Qudsiya Baig
Abstract:
Empathy yields an unparalleled therapeutic value within patient physician interactions. Medical research is inundated with evidence to support that a physician’s ability to empathise with patients leads to a greater willingness to report symptoms, an improvement in diagnostic accuracy and safety, and a better adherence and satisfaction with treatment plans. Furthermore, the Institute of Medicine states that empathy leads to a more patient-centred care, which is one of the six main goals of a 21st century health system. However, there is a paradox between the theoretical significance of empathy and its presence, or lack thereof, in clinical practice. Recent studies have reported that empathy declines amongst students and physicians over time. The three most impactful contributors to this decline are: (1) disagreements over the definitions of empathy making it difficult to implement it into practice (2) poor consideration or regulation of empathy leading to burnout and thus, abandonment altogether, and (3) the lack of diversity in the curriculum and the influence of medical culture, which prioritises science over patient experience, limiting some physicians from using ‘too much’ empathy in the fear of losing clinical objectivity. These issues were investigated by conducting a fully inductive thematic narrative analysis of patient narratives in the UK to evaluate the behaviours and attitudes that patients associate with empathy. The principal enquiries underpinning this study included uncovering the factors that affected experience of empathy within provider-patient interactions and to analyse their effects on patient care. This research contributes uniquely to this discourse by examining the phenomenon of empathy directly from patients’ experiences, which were systematically extracted from a repository of online patient narratives of care titled ‘CareOpinion UK’. Narrative analysis was specifically chosen as the methodology to examine narratives from a phenomenological lens to focus on the particularity and context of each story. By enquiring beyond the superficial who-whatwhere, the study of narratives prescribed meaning to illness by highlighting the everyday reality of patients who face the exigent life circumstances created by suffering, disability, and the threat of life. The following six themes were found to be the most impactful in influencing the experience of empathy: dismissive behaviours, judgmental attitudes, undermining patients’ pain or concerns, holistic care and failures and successes of communication or language. For each theme there were overarching themes relating to either a failure to understand the patient’s perspective or a success in taking a person-centred approach. An in-depth analysis revealed that a lack of empathy was greatly associated with an emotive-cognitive imbalance, which disengaged physicians with their patients’ emotions. This study hereby concludes that competent providers require a combination of knowledge, skills, and more importantly empathic attitudes to help create a context for effective care. The crucial elements of that context involve (a) identifying empathy clues within interactions to engage with patients’ situations, (b) attributing a perspective to the patient through perspective-taking and (c) adapting behaviour and communication according to patient’s individual needs. Empathy underpins that context, as does an appreciation of narrative, and the two are interrelated. Keywords: empathy, narratives, person-centred, perspective, perspective-taking

Authors: Chirag Kothawade, Gourie Jagtap, PreetKaur Relusinghani, Vedang Chavan, Smitha S. Bhosale
Abstract:
Psychological research has proven that music relieves stress, elevates mood, and is responsible for the release of “feel-good” chemicals like oxytocin, serotonin, and dopamine. It comes as no surprise that music has been a popular tool in rehabilitation centers and therapy for various disorders, thus with the interminably rising numbers of people facing mental health-related issues across the globe, addressing mental health concerns is more crucial than ever. Despite the existing music recommendation systems, there is a dearth of holistically curated algorithms that take care of the needs of users. Given that, an undeniable majority of people turn to music on a regular basis and that music has been proven to increase cognition, memory, and sleep quality while reducing anxiety, pain, and blood pressure, it is the need of the hour to fashion a product that extracts all the benefits of music in the most extensive and deployable method possible. Our project aims to ameliorate our users’ mental state by building a comprehensive mood-based music recommendation system called “Viby”.Keywords: language, communication, speech recognition, interaction
Procedia PDF Downloads 631874 A Framework for Automated Nuclear Waste Classification
Authors: Seonaid Hume, Gordon Dobie, Graeme West
Abstract:
Detecting and localizing radioactive sources is a necessity for safe and secure decommissioning of nuclear facilities. An important aspect for the management of the sort-and-segregation process is establishing the spatial distributions and quantities of the waste radionuclides, their type, corresponding activity, and ultimately classification for disposal. The data received from surveys directly informs decommissioning plans, on-site incident management strategies, the approach needed for a new cell, as well as protecting the workforce and the public. Manual classification of nuclear waste from a nuclear cell is time-consuming, expensive, and requires significant expertise to make the classification judgment call. Also, in-cell decommissioning is still in its relative infancy, and few techniques are well-developed. As with any repetitive and routine tasks, there is the opportunity to improve the task of classifying nuclear waste using autonomous systems. Hence, this paper proposes a new framework for the automatic classification of nuclear waste. This framework consists of five main stages; 3D spatial mapping and object detection, object classification, radiological mapping, source localisation based on gathered evidence and finally, waste classification. The first stage of the framework, 3D visual mapping, involves object detection from point cloud data. A review of related applications in other industries is provided, and recommendations for approaches for waste classification are made. Object detection focusses initially on cylindrical objects since pipework is significant in nuclear cells and indeed any industrial site. The approach can be extended to other commonly occurring primitives such as spheres and cubes. This is in preparation of stage two, characterizing the point cloud data and estimating the dimensions, material, degradation, and mass of the objects detected in order to feature match them to an inventory of possible items found in that nuclear cell. Many items in nuclear cells are one-offs, have limited or poor drawings available, or have been modified since installation, and have complex interiors, which often and inadvertently pose difficulties when accessing certain zones and identifying waste remotely. Hence, this may require expert input to feature match objects. The third stage, radiological mapping, is similar in order to facilitate the characterization of the nuclear cell in terms of radiation fields, including the type of radiation, activity, and location within the nuclear cell. The fourth stage of the framework takes the visual map for stage 1, the object characterization from stage 2, and radiation map from stage 3 and fuses them together, providing a more detailed scene of the nuclear cell by identifying the location of radioactive materials in three dimensions. The last stage involves combining the evidence from the fused data sets to reveal the classification of the waste in Bq/kg, thus enabling better decision making and monitoring for in-cell decommissioning. The presentation of the framework is supported by representative case study data drawn from an application in decommissioning from a UK nuclear facility. This framework utilises recent advancements of the detection and mapping capabilities of complex radiation fields in three dimensions to make the process of classifying nuclear waste faster, more reliable, cost-effective and safer. Keywords: nuclear decommissioning, radiation detection, object detection, waste classification

Authors: Kelsi Matwick, Keri Matwick
Abstract:
This presentation focuses on national campaigns as a government tactic of social behavior and order. Focusing on one of Singapore’s first national campaigns, Keep Singapore Clean (1968), particularly its iterations of public hygiene in recent years: Keep the Toilets Clean (2012-2019) and UnLittering with Mary Chongo (2019), the study examines how humor and the use of multimodality reflect contemporary practices in political practice. A critical social semiotic analysis involving the textual (linguistic and visual design) and material (print cartoons and videos) is undertaken to show how these messages are communicated. Incongruity and parody are humorous mechanisms used to project the government as likeable, effectively capture the public attention, and instill individual responsibility for the greater community. In focusing on public hygiene national campaigns, the study further illustrates how humor offers a polite way to address crude behavior while providing models of exemplary behavior. Keywords: communication strategies, critical social semiotics, humor, national campaigns

Authors: Arafat A. A. Shabaneh
Abstract:
Harsh environments demand a developed detection of an optical communication system to ensure a high level of security and safety. Fiber Bragg gratings (FBG) are emerging sensing instruments that respond to variations in strain and temperature via varying wavelengths. In this paper, cascaded uniform FBG as a strain sensor for 6 km length at 1550 nm wavelength with 30 oC is designed with analyzing of dynamic strain and wavelength shifts. FBG is placed in a small segment of optical fiber, which reflects light of a specific wavelength and passes the remaining wavelengths. This makes a periodic alteration in the refractive index within the fiber core. The alteration in the modal index of fiber produced due to strain consequences in a Bragg wavelength. When the developed sensor exposure to a strain of cascaded uniform FBG by 0.01, the wavelength is shifted to 0.0000144383 μm. The sensing accuracy of the developed sensor is 0.0012. Simulation results show reliable and effective strain monitoring sensors for remote sensing applications. Keywords: Cascaded fiber Bragg gratings, Strain sensor, Remote sensing, Wavelength shift

Authors: Jungho Moon, Myunggon Yoon
Abstract:
This paper presents an implementation of a configurable UART-to-Ethernet converter using an ARM-based 32-bit microcontroller as well as a dedicated configuration program running on a PC for configuring the operating parameters of the converter. The program was written in Python. Various parameters pertaining to the operation of the converter can be modified by the configuration program through the Ethernet interface of the converter. The converter supports 3 representative asynchronous serial communication protocols, RS-232, RS-422, and RS-485 and supports 3 network modes, TCP/IP server, TCP/IP client, and UDP client. The TCP/IP and UDP protocols were implemented on the microcontroller using an open source TCP/IP protocol stack called lwIP (A lightweight TCP/IP) and FreeRTOS, a free real-time operating system for embedded systems. Due to the use of a real-time operating system, the firmware of the converter was implemented as a multi-thread application and as a result becomes more modular and easier to develop. The converter can provide a seamless bridge between a serial port and an Ethernet port, thereby allowing existing legacy apparatuses with no Ethernet connectivity to communicate using the Ethernet protocol. Keywords: converter, embedded systems, ethernet, lwIP, UART

Procedia PDF Downloads 1301865 Probabilistic Gathering of Agents with Simple Sensors: Distributed Algorithm for Aggregation of Robots Equipped with Binary On-Board Detectors
Authors: Ariel Barel, Rotem Manor, Alfred M. Bruckstein
Abstract:
We present a probabilistic gathering algorithm for agents that can only detect the presence of other agents in front of or behind them. The agents act in the plane and are identical and indistinguishable, oblivious, and lack any means of direct communication. They do not have a common frame of reference in the plane and choose their orientation (direction of possible motion) at random. The analysis of the gathering process assumes that the agents act synchronously in selecting random orientations that remain fixed during each unit time-interval. Two algorithms are discussed. The first one assumes discrete jumps based on the sensing results given the randomly selected motion direction, and in this case, extensive experimental results exhibit probabilistic clustering into a circular region with radius equal to the step-size in time proportional to the number of agents. The second algorithm assumes agents with continuous sensing and motion, and in this case, we can prove gathering into a very small circular region in finite expected time. Keywords: control, decentralized, gathering, multi-agent, simple sensors

Procedia PDF Downloads 3681862 Global Winners versus Local Losers: Globalization Identity and Tradition in Spanish Club Football
Authors: Jim O'brien
Abstract:
Contemporary global representation and consumption of La Liga across a plethora of media platform outlets has resulted in significant implications for the historical, political and cultural developments which shaped the development of Spanish club football. This has established and reinforced a hierarchy of a small number of teams belonging to or aspiring to belong to a cluster of global elite clubs seeking to imitate the blueprint of the English Premier League in respect of corporate branding and marketing in order to secure a global fan base through success and exposure in La Liga itself and through the Champions League. The synthesis between globalization, global sport and the status of high profile clubs has created radical change within the folkloric iconography of Spanish football. The main focus of this paper is to critically evaluate the consequences of globalization on the rich tapestry at the core of the game’s distinctive history in Spain. The seminal debate underpinning the study considers whether the divergent aspects of globalization have acted as a malevolent force, eroding tradition, causing financial meltdown and reducing much of the fabric of club football to the status of by standers, or have promoted a renaissance of these traditions, securing their legacies through new fans and audiences. The study draws on extensive sources on the history, politics and culture of Spanish football, in both English and Spanish. It also uses primary and archive material derived from interviews and fieldwork undertaken with scholars, media professionals and club representatives in Spain. The paper has four main themes. Firstly, it contextualizes the key historical, political and cultural forces which shaped the landscape of Spanish football from the late nineteenth century. The seminal notions of region, locality and cultural divergence are pivotal to this discourse. The study then considers the relationship between football, ethnicity and identity as a barometer of continuity and change, suggesting that tradition is being reinvented and re-framed to reflect the shifting demographic and societal patterns within the Spanish state. Following on from this, consideration is given to the paradoxical function of ‘El Clasico’ and the dominant duopoly of the FC Barcelona – Real Madrid axis in both eroding tradition in the global nexus of football’s commodification and in protecting historic political rivalries. To most global consumers of La Liga, the mega- spectacle and hyperbole of ‘El Clasico’ is the essence of Spanish football, with cultural misrepresentation and distortion catapulting the event to the global media audience. Finally, the paper examines La Liga as a sporting phenomenon in which elite clubs, cult managers and galacticos serve as commodities on the altar of mass consumption in football’s global entertainment matrix. These processes accentuate a homogenous mosaic of cultural conformity which obscures local, regional and national identities and paradoxically fuses the global with the local to maintain the distinctive hue of La Liga, as witnessed by the extraordinary successes of Athletico Madrid and FC Eibar in recent seasons. Keywords: Spanish football, globalization, cultural identity, tradition, folklore

Authors: Shahid Imran
Abstract:
'Mediatization' has influenced the politics by shaping and transforming the attitudes and practices of political actors. It is a serious challenge to democracy in today’s era. This study aims to analyze the dynamics of media politics interplay in Pakistan and the contextual factors which govern this interplay. It will also address the perceived influence of media on the practices of politicians from the perspectives of the actors. The objectives have been achieved qualitatively through Interpretive Phenomenological Analysis (IPA). The phenomenological data have been collected using semi-structured interviews of journalists and politicians of Pakistan. The findings depict that politics in Pakistan is more driven by media logic than political or democratic logic. Media and politics have a ‘Tom and Jerry’ relationship. Political ecology is highly media-induced: politicians strategically adopt and adapt the media logic to be in the ‘media spotlight’; journalists, on the other hands, do not practice ‘fair journalism rather a more politically parallelized. The mediatized political communication behaviours of the actors are the undermining the public service logic and affecting the spirit of democracy in Pakistan. The study offers some valued implications for media, politicians and policy makers. Keywords: medialization, media logic, politics, political logic

Authors: V. Vicente E. Mujica, Gustavo Gonzalez
Abstract:
The global coverage of broadband multimedia and internet-based services in terrestrial-satellite networks demand particular interests for satellite providers in order to enhance services with low latencies and high signal quality to diverse users. In particular, the delay of on-board processing is an inherent source of latency in a satellite communication that sometimes is discarded for the end-to-end delay of the satellite link. The frame work for this paper includes modelling of an on-orbit satellite payload using an agent model that can reproduce the properties of processing delays. In essence, a comparison of different spatial interpolation methods is carried out to evaluate physical data obtained by an GEO satellite in order to define a discretization function for determining that delay. Furthermore, the performance of the proposed agent and the development of a delay discretization function are together validated by simulating an hybrid satellite and terrestrial network. Simulation results show high accuracy according to the characteristics of initial data points of processing delay for Ku bands. Keywords: terrestrial-satellite networks, latency, on-orbit satellite payload, simulation

Authors: Mohamed Khiatine, Ramdane Bahar
Abstract:
The research performed during the last years has revealed that the seismic response of the soilis significantly non linear and hysteresis to the deformationsitundergoes during earthquakes and notably during violent shaking. This nonlinear behavior of soils can be characterized by curves showing the evolution of shearmodulus and damping versus distortion. Also, in this context, geotechnical seismic engineering problems often require the characterization of dynamic soil properties over a wide range of deformation. This determination of dynamic soil properties is key to predict the seismic response of soils for important civil engineering structures. This communication discusses a numerical analysis method for evaluating the nonlinear dynamic properties of soils in Algeriausing the FLAC2D software and the database resulting from geophysical and geotechnical studies when laboratory dynamic tests are not available. The nonlinear model proposed by Ramberg-Osgood and limited by the Mohr-coulomb criterion is used. Keywords: degradation, shear modulus, damping, ramberg-osgood, numerical analysis.

Authors: Hanh Thi My Nguyen, Phuong Thu Dinh
Abstract:
People with intellectual disability are at high risk for sexual abuse. The reasons may originate from their communication skills deficits, lack of skills and knowledge to protect themselves from sexual abuse, or limited access to sexual abuse prevention programs. This article aims to present a systematic review about strategies for preventing sexual abuse for young people with intellectual disability. A range of articles in 10 years from 2009 to 2018 are searched by using online database. 5 papers are included for the final review. The results of this comprehensive literature review showed that there are two main strategies used: programs designed for people with intellectual, including evaluation on sex education programs; and sexual education program for parents of children with intellectual disability. However, none of the papers were conducted in low-and middle-income countries. Therefore, cautions should be taken when it comes to interpret these findings. The findings of studies showed that participants increased their awareness and skills for protecting themselves from sexual abuse after participating in the programs. It is also recommended that more effective evidence-based programs should be developed. Keywords: intellectual disability, prevention, sexual abuse, sexual education program

Authors: Bilal Ashraf, Qianxiao Zhang
Abstract:
This study empirically investigated the linkage amid financial development and its important determinants such as information and communication technology, natural resource rents, economic growth, current account balance, and gross savings in 107 economies. This paper preferred to employ the second-generation unit root tests to handle the issues of slope heterogeneity and “cross-sectional dependence” in panel data. The “Kao, Pedroni, and Westerlund tests” confirm the long-lasting connections among the variables under study, while the significant endings of “cross-sectionally augmented autoregressive distributed lag (CS-ARDL)” exposed that NRR, CAB, and S negatively affected the financial development while ICT and EG stimulates the procedure of FD. Further, the robustness analysis's application of FGLS supports the appropriateness and applicability of CS-ARDL. Finally, the findings of “DH causality analysis” endorse the bidirectional causality linkages amongst research factors. Based on the study's outcomes, we suggest some policy suggestions that empower the process of financial development, globally. Keywords: determinants of financial developments, CS-ARDL, financial development, global sample, causality analysis

Authors: L. Wilkening, G. Ackermann, T. T. Do
Abstract:
This article presents an approach for stand-alone and grid-connected mode of a German low-voltage grid with high share of photovoltaic. For this purpose, suitable dynamic system models have been developed. This allows the simulation of dynamic events in very small time ranges and the operation management over longer periods of time. Using these simulations, suitable control parameters could be identified, and their effects on the grid can be analyzed. In order to validate the simulation results, a LV-grid test bench has been implemented at the University of Technology Hamburg. The developed control strategies are to be validated using real inverters, generators and different realistic loads. It is shown that a battery hybrid system installed next to a voltage transformer makes it possible to operate the LV-grid in stand-alone mode without using additional information and communication technology and without intervention in the existing grid units. By simulating critical days of the year, suitable control parameters for stable stand-alone operations are determined and set point specifications for different control strategies are defined. Keywords: battery, e-mobility, photovoltaic, smart grid

Authors: Mohamad Abdullah
Abstract:
Recent Advancements in technology have stimulated deep change in the language learning classroom. Flipped classroom as a new pedagogical method is at the center of this change. It turns the classroom into a student-centered environment and promotes interactive and autonomous learning. The present study is an attempt to examine the effectiveness of the Flipped Classroom Model (FCM) on students’ motivation level in English speaking performance. This study was carried out with 27 undergraduate female English majors who enrolled in the course of Advanced Communication Skills (ENGL 154) at Buraimi University College (BUC). Data was collected through Motivation in English Speaking Performance Questionnaire (MESPQ) which has been distributed among the participants of this study pre and post the implementation of FCM. SPSS was used for analyzing data. The Paired T-Test which was carried out on the pre-post of (MESPQ) showed a significant difference between them (p < .009) that revealed participants’ tendency to increase their motivation level in English speaking performance after the application of FCM. In addition, respondents of the current study reported positive views about the implementation of FCM.Keywords: english speaking performance, motivation, flipped classroom model, learner-contentedness
