747 A Quality Improvement Project on Eye Care in the Intensive Care Unit

Abstract:
Background Sedated and paralyzed patients have an impaired blink reflex leading to ophthalmic complications such as conjunctivitis, epithelial defects, bacterial keratitis, and more. These are entirely preventable complications through regular eye care. Methods Patients at level 3 or above (intubated/paralyzed) care in the Intensive Care Unit (ICU) were reviewed between February and April. Data was pulled from Metavision and adherence was compared to Royal College of Ophthalmology (RCOphth) recommendations[4]. Using a multi-pronged approach through posters, individual teaching sessions and faculty teaching, we aimed to educate staff about eye care in the ICU. Patients were reaudited in the period July to August. Results Out of 40 patients, only 23% were assessed for eye care needs on admission compared to 77% after teaching; eye care was only delivered 59% of the time it was due, compared to 61%; 2.5% of patients had eyedrops prescribed compared to 41%. This shows an overall increase in meeting RCOphth standards. Key messages Eye care is an overlooked aspect of patient care in the ICU, associated with avoidable ocular complications. Healthcare staff need further rigorous education on the provision and importance of eye care to reduce avoidable complications.Keywords: ICU, eye care, risk, QIP

Abstract:
Background: The opioid epidemic is the most pressing healthcare crisis of our time. There is increasing recognition that opioids have limited long-term efficacy and are associated with hyperalgesia, addiction, and increased morbidity and mortality. Therefore, alternative strategies to combat chronic pain are paramount. We initiated a multicenter retrospective case series to review the efficacy of DRG stimulation in facilitating opioid tapering, opioid discontinuation and as a viable alternative to chronic opioid therapy. Purpose: The dorsal root ganglion (DRG) plays a key role in the development and maintenance of pain. Recent innovations in neuromodulation, specifically, dorsal root ganglion stimulation, offers an effective alternative to opioids in the treatment of chronic pain. This retrospective case series demonstrates preliminary evidence that DRG stimulation facilitates opioid tapering, opioid discontinuation and presents a viable alternative to chronic opioid therapy. Procedure: This small multicenter retrospective case series provides preliminary evidence that DRG stimulation facilitates opioid weaning, opioid tapering and is a viable option to opioid therapy in the treatment of chronic pain. A retrospective analysis was completed. Visual analog scale pain scores and pain medication usage were collected at the baseline visit and after four weeks, 3 months and 6 months of treatment. Ten consecutive patients across two study centers were included. The pain was rated 7.38 at baseline and decreased to 1.50 at the 4-week follow-up, a reduction of 79.5%. All patients significantly decreased their opioid pain medication use with an average > 30% reduction in morphine equivalents and four were able to discontinue their medications entirely. Conclusion: This Retrospective case series demonstrates preliminary evidence that DRG stimulation facilitates opioid tapering, opioid discontinuation and presents a viable alternative to chronic opioid therapy.Keywords: dorsal root ganglion, neuromodulation, opioid sparing, stimulation

Abstract:
The changes observed in the last years in the field of manufacturing and production engineering, popularly known as "Fourth Industry Revolution", utilizes the achievements in the different areas of computer sciences, introducing new solutions at almost every stage of the production process, just to mention such concepts as mass customization, cloud computing, knowledge-based engineering, virtual reality, rapid prototyping, or virtual models of measuring systems. To effectively speed up the production process and make it more flexible, it is necessary to tighten the bonds connecting individual stages of the production process and to raise the awareness and knowledge of employees of individual sectors about the nature and specificity of work in other stages. It is important to discover and develop a suitable education method adapted to the specificities of each stage of the production process, becoming an extremely crucial issue to exploit the potential of the fourth industrial revolution properly. Because of it, the project “Train4Dim” (T4D) intends to develop complex training material for digital manufacturing, including content for design, manufacturing, and quality control, with a focus on coordinate metrology and portable measuring systems. In this paper, the authors present an approach to using an active learning methodology for digital manufacturing. T4D main objective is to develop a multi-degree (apprenticeship up to master’s degree studies) and educational approach that can be adapted to different teaching levels. It’s also described the process of creating the underneath methodology. The paper will share the steps to achieve the aims of the project (training model for digital manufacturing): 1) surveying the stakeholders, 2) Defining the learning aims, 3) producing all contents and curriculum, 4) training for tutors, and 5) Pilot courses test and improvements.Keywords: learning, Industry 4.0, active learning, digital manufacturing

Abstract:
To face the water crisis, understanding the role of human activities on nutrient concentrations in aquatic ecosystems needs more investigations to compare to extensively studies which have been carried out to understand these impacts on the water quality of different aquatic ecosystems. We hypothesized human activates on the catchments of Tigris river may change nutrient concentrations in water along the river. The results showed that phosphate concentration differed significantly among the studied sites due to distributed human activities, while nitrate concentration did not. Phosphate and nitrate concentrations were not affected by water temperature. We concluded that human activities on the surrounding landscapes could be more essential sources for nutrients of aquatic ecosystems than role of ongoing climate warming. Despite the role of warming in driving nutrients availability in aquatic ecosystems, our findings suggest to take the different activities on the surrounding catchments into account in the studies caring about the trophic status classification of aquatic ecosystems.Keywords: nitrate, phosphate, anthropogenic, warming

The paper gives a clear picture of high‐throughput satellites (HTS) and the associated technologies with performances as they align and differ with the traditional geostationary orbit or Geosynchronous Equatorial Orbit (GEO) Communication Satellites. The paper also highlights critical subsystems and processes in project conceptualization and execution. The paper discusses the configuration of the payload. Abstract:
Radiological properties of gel dosimeters and phantom materials are often evaluated in terms of effective atomic number, electron density, photon mass attenuation coefficient, photon mass energy absorption coefficient and total stopping power of electrons. To evaluate the water equivalence of such materials for fast neutron attenuation 19 different types of gel dosimeters and phantom materials were considered. Macroscopic removal cross-sections for fast neutrons (ΣR cm-1) have been calculated for a range of ferrous-sulphate and polymeric gel dosimeters using Nxcom Program. The study showed that the value of ΣR/ρ (cm2.g-1) for all polymer gels were in close agreement (1.5- 2.8%) with that of water. As such, the slight differences in ΣR/ρ between water and gels are small and may be considered negligible. Also, the removal cross-section of the studied phantom materials were very close (~ ±1.5%) to that of water except bone (cortical) which had about 38% variation. Convective vortices are normal highlights of air that ingest lower-entropy-energy at higher temperatures than they dismiss higher-entropy-energy to space. By means of the thermodynamic proficiency, it has been anticipated that the force of convective vortices relies upon the profundity of the convective layer. The atmospheric vortex engine is proposed as a gadget for delivering mechanical energy by methods for artificially produced vortex. The task of the engine is in view of the certainties that the environment is warmed from the base and cooled from the top. By generation of the artificial vortex, it is planned to take out the physical solar updraft tower and decrease the capital of the solar chimney power plants. The study shows the essentials of the atmospheric vortex engine, furthermore, audits the cutting edge in subject. Moreover, the study talks about a thought on using the solar energy as heat source to work the framework. Background: It has been estimated that up to 50% of any patient population is at least partially non-adherent to their prescribed treatment. Identifying barriers to adherence is required to develop effective interventions for psychiatric patients. Objective: To explore the prevalence and factors of non-adherence among psychiatric patients present at four psychiatric clinics. Method: A cross-sectional questionnaire-based study. A sample of psychiatric patients attending outpatient psychiatric clinics was enrolled between March and April 2011. Results: A total of 243 psychiatric patients took part in this study with the majority of patients (92.5%) being prescribed more than one psychiatric disorder. The majority (64.2%) of the patients was classified as non-adherent according to the Morisky adherence questionnaire and forgetfulness was the most prevalent reason for that. Conclusions: Non-adherence is a common and important issue among psychiatric patients. Polypharmacy, safety concerns and lack of insight towards the prescribed treatment were reported as the main reasons of non-adherence.Keywords: medication adherence, psychiatric disorders, clinical pharmacy, polypharmacy

Abstract:
Atmospheric carbon dioxide emissions are considered as the greatest environmental challenge the world is facing today. The challenges to control the emissions include the recovery of CO2 from flue gas. This concern has been improved due to recent advances in materials process engineering resulting in the development of inorganic gas separation membranes with excellent thermal and mechanical stability required for most gas separations. This paper therefore evaluates the performance of a highly selective inorganic membrane for CO2 recovery applications. Analysis of results obtained is in agreement with experimental literature data. Further results show the prediction performance of the membranes for gas separation and the future direction of research. The materials selection and the membrane preparation techniques are discussed. Abstract:
Many algorithms have been proposed to provide privacy preserving in data mining. These protocols are based on two main approaches named as: the perturbation approach and the Cryptographic approach. The first one is based on perturbation of the valuable information while the second one uses cryptographic techniques. The perturbation approach is much more efficient with reduced accuracy while the cryptographic approach can provide solutions with perfect accuracy. However, the cryptographic approach is a much slower method and requires considerable computation and communication overhead. In this paper, a new scalable protocol is proposed which combines the advantages of the perturbation and distortion along with cryptographic approach to perform privacy preserving in distributed frequent itemset mining on horizontally distributed data. Abstract:
Amine absorption process is an approach for mitigation of CO2 from flue gas that produces from power plant. This process is the most common system used in chemical and oil industries for gas purification to remove acid gases. On the challenges of this process is high energy requirement for solvent regeneration to release CO2. In the past few years, mixed alkanolamines have received increasing attention. In most cases, the mixtures contain N-methyldiethanolamine (MDEA) as the base amine with the addition of one or two more reactive amines such as PZ. The reason for the application of such blend amine is to take advantage of high reaction rate of CO2 with the activator combined with the advantages of the low heat of regeneration of MDEA. Several experimental and simulation studies have been undertaken to understand this process using blend MDEA/PZ solvent. Despite those studies, the mechanism of CO2 absorption into the aqueous MDEA is not well understood and available knowledge within the open literature is limited. The aim of this study is to investigate the intermolecular interaction of the blend MDEA/PZ using Molecular Dynamics (MD) simulation. MD simulation was run under condition 313K and 1 atm using NVE ensemble at 200ps and NVT ensemble at 1ns. The results were interpreted in term of Radial Distribution Function (RDF) analysis through two system of interest i.e binary and tertiary. The binary system will explain the interaction between amine and water molecule while tertiary system used to determine the interaction between the amine and CO2 molecule. For the binary system, it was observed that the –OH group of MDEA is more attracted to water molecule compared to –NH group of MDEA. The –OH group of MDEA can form the hydrogen bond with water that will assist the solubility of MDEA in water. The intermolecular interaction probability of –OH and –NH group of MDEA with CO2 in blended MDEA/PZ is higher than using single MDEA. This findings show that PZ molecule act as an activator to promote the intermolecular interaction between MDEA and CO2.Thus, blend of MDEA with PZ is expecting to increase the absorption rate of CO2 and reduce the heat regeneration requirement.Keywords: amine absorption process, blend MDEA/PZ, CO2 capture, molecular dynamic simulation, radial distribution function

Abstract:
Autism Spectrum Disorder (ASD) is a condition related to issues with brain development that affects how a person recognises and communicates with others which results in difficulties with interaction and communication socially and it is constantly growing. Early recognition of ASD allows children to lead safe and healthy lives and helps doctors with accurate diagnoses and management of conditions. Therefore, it is crucial to develop a method that will achieve good results and with high accuracy for the measurement of ASD in children. In this paper, ASD datasets of toddlers and children have been analyzed. We employed the following machine learning techniques to attempt to explore ASD and they are Random Forest (RF), Decision Tree (DT), Na¨ıve Bayes (NB) and Support Vector Machine (SVM). Then Feature selection was used to provide fewer attributes from ASD datasets while preserving model performance. As a result, we found that the best result has been provided by the Support Vector Machine (SVM), achieving 0.98% in the toddler dataset and 0.99% in the children dataset.Keywords: autism spectrum disorder, machine learning, feature selection, support vector machine

The demand for parking spaces in urban areas, particularly in developing countries, has led to a significant issue in the absence of sufficient parking spaces in crowded areas, which results in daily traffic congestion as drivers search for parking. This not only affects the appearance of the city but also has indirect impacts on the economy, society, and environment. In response to these challenges, researchers from various countries have sought technical and intelligent solutions to mitigate the problem through the development of smart parking systems. This paper aims to analyze and design three models of parking lots, with a focus on parking time and security. The study used computer software and Uppaal tools to simulate the models and determine the best among them. The results and suggestions provided in the paper aim to reduce the parking problems and improve the overall efficiency and safety of the parking process. Sample B in contrast was comprised of 10 female students who their major is English from college of Education. Results show that each genre in literature has different influence on the students' motivation in learning the language which proves that literacy texts are powerful. Generally, Omani English teachers tend to avoid teaching literature because they think that it is a difficult method to use in teaching field. However, the advantages and the influences of teaching poetries, short stories, and plays are discussed. Abstract:
Object detection is a computer technology that deals with searching through digital images and videos for occurrences of semantic elements of a particular class. It is associated with image processing and computer vision. On top of object detection, we detect camouflage objects within an image using Deep Learning techniques. Deep learning may be a subset of machine learning that's essentially a three-layer neural network Over 6500 images that possess camouflage properties are gathered from various internet sources and divided into 4 categories to compare the result. Those images are labeled and then trained and tested using vgg16 architecture on the jupyter notebook using the TensorFlow platform. The architecture is further customized using Transfer Learning. Methods for transferring information from one or more of these source tasks to increase learning in a related target task are created through transfer learning. Abstract:
Zinc Oxide (ZnO) nanostructures have been attracting growing interest in recent years; their optical and electrical properties make them useful as attractive and promising materials for optoelectronic applications. In this study, pure and Hafnium doped ZnO nanostructures were synthesized using a green processing method. The structural, optical and electrical properties of samples were investigated structural and optical spectroscopies and electrical measurements. The synthesis and chemical composition of pure and Hafnium doped ZnO were confirmed by SEM observation. The XRD studies of Hafnium doped ZnO demonstrate the formation of wurtzite structure with preferred c-axis orientation. Moreover, the optical and electrical properties of doped material have improved after the doping process. The rapid growth in the number of mobile phone subscribers has resulted in an increased number of mobile base stations all over the world. Generally, mobile base stations are existing in huge numbers in populated areas than in non-populated ones to serve the largest number of users. The total number of mobile subscriptions in the Kingdom of Saudi Arabia reached around 50 million at the end of 2014, with a penetration rate of 165.1% according to the quarterly electronic newsletter issued by the Communications and Information Technology Commission. The current investigation was conducted primarily to measure the level of electromagnetic fields emitted from 400 mobile base stations for the purpose of environmental safety and radiation protection in light of national guidelines for public exposure as well as the International Commission on Non-Ionizing Radiation Protection (ICNIRP). Abstract:
A clay soil which classified under A-7-6 soil according to AASHTO soil classification system and CH according to the unified soil classification system was stabilized using A-3 soil (AASHTO soil classification system). The clay soil was replaced with 0%, 10%, 20% to 100% A-3 soil, compacted at both the BSL and BSH compaction energy level and using unconfined compressive strength as evaluation criteria. The MDD of the compactions at both the BSL and BSH compaction energy levels showed increase in MDD from 0% A-3 soil replacement to 40% A-3 soil replacement after which the values reduced to 100% A-3 soil replacement. The trend of the OMC with varied A-3 soil replacement is similar to that of MDD but in a reversed order. The OMC reduced from 0% A-3 soil replacement to 40% A-3 soil replacement after which the values increased to 100% A-3 soil replacement. This trend was attributed to the observed reduction in the void ratio from 0% A-3 soil replacement to 40% A-3 soil replacement after which the void ratio increased to 100% A-3 soil replacement. The maximum UCS for clay at varied A-3 soil replacement increased from 272 and 770kN/m2 for BSL and BSH compaction energy level at 0% A-3 soil replacement to 295 and 795kN/m2 for BSL and BSH compaction energy level respectively at 10% A-3 soil replacement after which the values reduced to 22 and 60kN/m2 for BSL and BSH compaction energy level respectively at 70% A-3 soil replacement. The present study experimentally investigated the impact of incorporating unprocessed waste fly ash (UWFA) on the residual mechanical properties of self-compacting concrete (SCC) after exposure to elevated temperature. Three mixtures of SCC have been produced by replacing the cement mass by 0%, 15% and 30% of UWFA. Generally, the fire resistance of SCC has been enhanced by replacing the cement up to 15% of UWFA, especially in case of residual modulus of elasticity which considers more sensitive than other mechanical properties at elevated temperature. However, a strong linear relationship has been observed between the residual flexural strength and modulus of elasticity, where both of them affected significantly by the cracks appearance and propagation as a result of elevated temperature. Sustainable products could be produced by incorporating unprocessed waste powder materials in the production of concrete, where the waste materials, CO2 emissions, and the energy needed for processing are reduced.Keywords: self-compacting high-performance concrete, unprocessed waste fly ash, fire resistance, residual modulus of elasticity

Abstract:
To face the water crisis, understanding the role of human activities on nutrient concentrations in aquatic ecosystems needs more investigations compare to extensively studies which have been carried out to understand these impacts on water quality of different aquatic ecosystems. We hypothesized human activates on the catchments of Tigris river may change nutrient concentrations in water along the river. The results showed that phosphate concentration differed significantly among the studied sites due to distributed human activities, while nitrate concentration did not. Phosphate and nitrate concentrations were not affected by water temperature. We concluded that human activities on the surrounding landscapes could be more essential sources for nutrients of aquatic ecosystems than role of ongoing climate warming. Despite the role of warming in driving nutrients availability in aquatic ecosystems, our findings suggest to take the different activities on the surrounding catchments into account in the studies caring about trophic status classification of aquatic ecosystems.Keywords: phosphate, nitrate, anthropogenic, warming

Abstract:
Wear as an unavoidable phenomenon in stainless steel contact sliding parts is investigated In this work. Two grades of austenitic AISI 304, and S31254, as well as duplexes of S32205, and AISI 2507, were chosen to compare their wear behavior in temperatures ranging from room temperature to 550°C. The experimental results show that AISI 304 austenitic and AISI 2205 duplex stainless steel had lower wear resistance compared with S31254 and AISI 2507 in various temperatures. When the temperature rose to 140°C, and the wear rate of all grades increased, AISI 304 had the highest at 7.028x10-4 mm3/Nm, and AISI 2507 had the lowest at 4.9033 x 10-4 mm3/Nm. At 300°C, the oxides began to form on the worn surfaces, causing the wear rate to slow. As a result, when temperatures exceeded 300°C, the specific wear rate decreased significantly in all specimens. According to the XRD patterns, the main types of oxides formed on worn surfaces were magnetite, hematite, and chromite.Keywords: wear, stainless steel, temperature, groove, oxide

This study aimed at investigating major issues in translating the Arabic Hadith terminologies into English, focusing on choosing the most appropriate translation for each, reviewing major Hadith works in English. This study is confined to twenty terminologies with regard to classification of Hadith based on authority, strength, number of transmitters and connections in Isnad. Almost all available translations are collected and analyzed to find the most proper translation based on linguistic and translational values. To the researcher, many translations lack precise understanding of either Hadith terminologies or English language and varieties of methodologies have influence on varieties of translations. This study provides a classification of translational and conceptual issues. Translational issues are related to translatability of these terminologies and their equivalence. Conceptual issues provide a list of misunderstandings due to wrong translations of terminologies. Abstract:
Underground tunnels are one of the most popular public facilities for various applications such as transportation, water transfer, network utilities and etc. Experience from the past earthquake reveals that the underground tunnels also become vulnerable components and may damage at certain percentage depending on the level of ground shaking and induced phenomena. In this paper a numerical analysis is conducted in evaluating the sensitivity of two types of circular shallow tunnel lining models to wide ranging changes in the geotechnical design parameter. Critical analysis has been presented about the current methods of analysis, structural typology, ground motion characteristics, effect of soil conditions and associated uncertainties on the tunnel integrity. The response of the tunnel is evaluated through 2D non-linear finite element analysis, which critically assesses the impact of increasing levels of seismic loads. In any team sport, the most important variables that should be used to measure performance are physical condition, and technical and tactical performance. In a complex game like football, it is extremely difficult to measure the relative importance of each of these variables. However, physical fitness itself has been shown to consist of several components, like endurance, strength, flexibility, agility, coordination and speed. Each of these components has been shown to consist of several subcomponents. This paper attempts to describe a test battery to assess and evaluate physical performance in football players. This battery comprises a functional, structured training session of about 2.5hrs. it consists of quality rating of the warm-up procedure, tests of flexibility, football skills, power, speed, and endurance. Acceptable values for performance in each of the tests are also presented under each test. It is hoped that this battery of tests will be helpful to the coach in determining the effect of a specific training program. It would also be helpful to train physician and trainer, to monitor progress during rehabilitation after sustaining any injury.Keywords: assessment, evaluation, performance, programs

This paper investigates the shear strength of the joints produced by friction stir spot welding process (FSSW). FSSW parameters such as tool rotational speed, plunge depth, shoulder diameter of the welding tool and dwell time play the major role in determining the shear strength of the joints. The effect of these four parameters on FSSW process as well as the shear strength of the welded joints was studied via five levels of each parameter. Taguchi method was used to minimize the number of experiments required to determine the fracture load of the friction stir spot-welded joints by incorporating independently controllable FSSW parameters. Abstract:
The enthusiasm for gluten avoidance in a growing market is met by improvements in sensitive detection methods for analysing gluten content. Paradoxically, manufacturers employ no such systems in the production process but continue to market their product as gluten free, a significant risk posed to an undetermined coeliac population. The paper resonates with an immunological response that causes gastrointestinal scarring and villous atrophy with the conventional description of personal injury. The current developing regime in the UK however, it is discussed, has avoided creating specific rules to provide an adequate level of protection for this type of vulnerable ‘characteristic’. Due to the struggle involved with identifying an appropriate cause of action, this paper analyses whether a claim brought in misrepresentation, negligence and/or under the Consumer Protect Act 1987 could be sustained. A necessary comparison is then made with the approach adopted by the Americans with Disability Act 1990 which recognises this chronic disease as a disability.
