94 Impact of Clinical Pharmacist Intervention in Improving Drug Related Problems in Patients with Chronic Kidney Disease

Abstract:
Drug related problems (DRPs) are common in chronic kidney disease (CKD) patients and end stage patients undergoing hemodialysis. To treat the co-morbid conditions of the patients, more complex therapeutic regimen is required, and it leads to development of DRPs. So, this calls for frequent monitoring of the patients. Due to the busy work schedules, physicians are unable to deliver optimal care to these patients. Addition of a clinical pharmacist in the team will improve the standard of care offered to CKD patients by minimizing DRPs. In India, the role of clinical pharmacists in the improving the health outcomes in CKD patients is poorly recognized. Therefore, this study is conducted to put an insight on the role of clinical pharmacist in improving Drug Related Problems in patients with chronic kidney disease, thereby helping them to achieve desired therapeutic outcomes in the patients. A prospective interventional study was conducted for a year in a 620 bedded tertiary care hospital in India. Data was collected using an unstructured questionnaire, medication charts, etc. DRPs were categorized using Hepler and Strand classification. Relationships between the age, weight, GFR, average no of medication taken, average no of comorbidities, and average length of hospital days with the DRPs were identified using Mann Whitney U test. The study population primarily constituted of patients above the age of 50 years with a mean age of 59.91±13.59. Our study showed that 25% of the population presented with DRPs. On an average, CKD patients are prescribed at least 8 medications for the treatment in our study. This explains the high incidence of drug interactions in patients suffering from CKD (45.65%). The least common DRPs in our study were found to be sub therapeutic dose (2%) and adverse drug reactions (2%). Out of this, 60 % of the DRPs were addressed successfully. In our study, there is an association between the DRPs with the average number of medications prescribed, the average number of comorbidities, and the length of the hospital days with p value of 0.022, 0.004, and 0.000, respectively. In the current study, 86% of the proposed interventions were accepted, and 41 % were implemented by the physician, and only 14% were rejected. Hence, it is evident that clinical pharmacist interventions will contribute significantly to diminish the DRPs in CKD patients, thereby decreasing the economic burden of healthcare costs and improving patient’s quality of life.Keywords: chronic kidney disease, clinical pharmacist, drug related problem, intervention

Abstract:
The utilization of industrial waste materials and by-products in the cement industry helps in the conservation of natural resources besides avoiding the problems arising due to waste dumping. The use of non-carbonated materials as raw mix components in clinker manufacturing is identified as one of the key areas to reduce Green House Gas (GHG) emissions. Chrome sludge is a waste material generated from the manufacturing process of sodium dichromate. This paper aims to present studies on the use of chrome sludge in clinker manufacturing, its impact on the development of clinker mineral phases and on the cement properties. Chrome sludge was found to contain substantial amounts of CaO, Fe2O3 and Al2O3 and therefore was used to replace some conventional sources of alumina and iron in the raw mix. Different mixes were prepared by varying the chrome sludge content from 0 to 5 % and the mixes were evaluated for burnability. Laboratory prepared clinker samples were evaluated for qualitative and quantitative mineralogy using X-ray Diffraction Studies (XRD). Optical microscopy was employed to study the distribution of clinker phases, their granulometry and mineralogy. Since chrome sludge also contains considerable amounts of chromium, studies were conducted on the leachability of heavy elements in the chrome sludge as well as in the resultant cement samples. Estimation of heavy elements, including chromium was carried out using ICP-OES. Further, the state of chromium valence, Cr (III) & Cr (VI), was studied using conventional chemical analysis methods coupled with UV-VIS spectroscopy. Assimilation of chromium in the clinker phases was investigated using SEM-EDXA studies. Bulk cement was prepared from the clinker to study the effect of chromium sludge on the cement properties such as setting time, soundness, strength development against the control cement. Studies indicated that chrome sludge can be successfully utilized and its content needs to be optimized based on raw material characteristics.Keywords: chrome sludge, leaching, mineralogy, non-carbonate materials

Abstract:
Enhancing the fly ash utilization in the manufacture of cement is identified as one of the key areas to mitigate the Green House Gas emissions from the cement industry. Though increasing the fly ash content in cement has economic and environmental benefits, it results in a decrease in the compressive strength values, particularly at early ages. Quality of clinker and fly ash were identified as predominant factors that govern the extent of absorption of fly ash in the manufacturing of cement. This paper presents systematic investigations on the effect of clinker and fly ash quality on the properties of resultant cement. Since mechanical activation alters the physicochemical properties such as particle size distribution, surface area, phase morphology, understanding the variation of these properties with activation is required for its applications. The effect of mechanical activation on fly ash surface area, specific gravity, flow properties, lime reactivity, comparative compressive strength (CCS), reactive silica and mineralogical properties were also studied. The fineness of fly ash was determined by Blaine’s method, specific gravity, lime reactivity, CCS were determined as per the method IS 1727-1967. The phase composition of fly ash was studied using the X-ray Diffraction technique. The changes in the microstructure and morphology with activation were examined using the scanning electron microscope. The studies presented in this paper also include evaluation of Portland Pozzolana Cement (PPC), prepared using high volume fly ash. Studies are being carried out using clinker from cement plants located in different regions/clusters in India. Blends of PPC containing higher contents of activated fly ash have been prepared and investigated for their chemical and physical properties, as per Indian Standard procedures. Changes in the microstructure of fly ash with activation and mechanical properties of resultant cement containing high volumes of fly ash indicated the significance of optimization of the quality of clinker and fly ash fineness for better techno-economical benefits.Keywords: flow properties, fly ash enhancement, lime reactivity, microstructure, mineralogy

Using two broad constructs of board characteristics, namely, board capital and board control, in the Malaysian context, this research attempts to infer their conjoint relevance to accounting values. Abstract:
Plant regeneration was achieved from shoot tip and nodal segment of Gonystylus bancanus (Miq) Kurz. cultured in Murashige and Skoog’s medium supplemented with various concentrations of 6-benzylaminopurine (BAP). The most optimum concentration of BAP for shoot initiation is 10.0 mgl⁻¹ with approximately 10% of shoot tip and 15% of nodal segment produced single shoot after 28 and 15 days of culture incubation respectively. Rooting was achieved when shoots were transferred into MS medium supplemented with 5.0 mgl⁻¹ Naphthalene acetic acid (NAA). Steel concrete composite construction means the concrete slab is connected to the steel beam with the help of shear connectors so that they act as a single unit. In the present work, steel concrete composite with RCC options are considered for comparative study of G+9 story commercial building which is situated in earthquake zone-III and for earthquake loading, the provisions of IS: 1893(Part1)-2002 is considered. A three dimensional modeling and analysis of the structure are carried out with the help of SAP 2000 software. Equivalent Static Method of Analysis and Response spectrum analysis method are used for the analysis of both Composite & R.C.C. structures. Assessment of behavior of reinforced concrete structures subjected to fire load, and its behavior for the multi-floor fire have been presented in this paper. This research is the part of the study to evaluate the performance of ten storied RC structure when it is subjected to fire loads at multiple floors and to evaluate the post-fire effects on structure such as deflection and stresses occurring due to combined effect of static and thermal loading. Thermal loading has been assigned to different floor levels to estimate the critical floors that initiate the collapse of the structure. The structure has been modeled and analyzed in Solid Works and commercially available Finite Element Software ABAQUS. This paper proposed the Multi-Carrier Pulse Width Modulation for the minimization of Total Harmonic Distortion in Cascaded H-Bridge Multi-Level Inverter. Multicarrier Pulse Width Modulation method uses Alternate Position of Disposition scheme to determine the appropriate switching angle to Multi-Level Inverter. Introduction: Patients with oropharyngeal masses pose a challenge for an anaesthetist in terms of ventilation and tracheal intubation. Thus, preoperative assessment and preparation become an integral part of managing such anticipated difficult airway cases. Case report: A 45- year old female presented with growth in the oropharynx causing dysphagia and hoarseness of voice. Clinical examination and investigations predicted a difficult airway. It was managed with fibreoptic nasotracheal intubation with a successful perioperative outcome. Tracheostomy was kept as plan B in case of the CVCI situation. Conclusion: Careful preoperative examination and assessment is required to prepare oneself for difficult airway. Fibreoptic bronchoscope-guided nasotracheal intubation in a spontaneously breathing patient is a safe and successful airway management technique in difficult airway cases.Keywords: airway, difficult, mass, oropharyngeal

The objective of this study is removal of Methylene blue dye or reactive orange-16 from industrial waste water or from soil using bioremediation technique. As huge amount of dyes are releasing from textile industry in water and soil environment during dyeing process. In this study, we focused on removal of Methylene blue dye and Reactive orange dye from industrial soil at different initial concentration of dye. An experiment study was carried out at methylene blue dye or Reactive orange-16 dye at varying concentration of both the dye as 50 ppm, 100ppm, 200 ppm, 300 ppm and 400 ppm. Maximum removal is obtained at 16-20 hours Experiments are carried out for pH, Temperature and MSM composition. The final concentration has been observed by UV-VIS. Texture features of mammogram images are useful for finding masses or cancer cases in mammography, which have been used by radiologists. Textures are greatly succeeded for segmented images rather than normal images. It is necessary to perform segmentation for exclusive specification of cancer and non-cancer regions separately. Region of interest (ROI) is most commonly used technique for mammogram segmentation. Limitation of this method is that it is unable to explore segmentation for large collection of mammogram images. Therefore, this paper is proposed multi-ROI segmentation for addressing the above limitation. It supports greatly in finding the best texture features of mammogram images. PTFE/HNTs nanocomposites are fabricated with 4%, 6%, and 8% by weight fraction, and the optimization study of wear parameters are performed using response surface methodology (RSM). The experiments are carried out on a pin on disc (POD) wear tester under different operating parameters planned according to Taguchi L27 orthogonal array. The input factors considered are wt% HNTs addition, sliding velocity, load, and distance with three levels for each factor. From ANOVA: The factors load, speed and distance and their interactions have a significant effect on COF. Also for SWR, composition factor and interaction of load and speed are observed to be significant ( < 0.05) Optimum input parameters corresponding to desirability 1 are found to be: COF (0.11) and SWR (17.5)×10⁻⁶ (mm3/N-m) at 6.34 wt% of composition, 5N of load, 2 km of distance and 1 m/sec of velocity.Keywords: PTFE/HNT, nanocomposites, response surface methodology (RSM), specific wear rate

Colon cancer causes the deaths of about half a million people every year. The common method of its detection is histopathological tissue analysis, it leads to tiredness and workload to the pathologist. A novel method is proposed that combines both structural and statistical pattern recognition used for the detection of colon cancer. This paper presents a comparison among the different classifiers such as Multilayer Perception (MLP), Sequential Minimal Optimization (SMO), Bayesian Logistic Regression (BLR) and k-star by using classification accuracy and error rate based on the percentage split method. The result shows that the best algorithm in WEKA is MLP classifier with an accuracy of 83.333% and kappa statistics is 0.625. Carcinoma of cervix and carcinoma of vulva have been associated with common predisposing factors like human papillomavirus and smoking. Skip metastases and metachronous appearance of both these tumours have been reported. There is no case report showing synchronous appearance of these tumours in English literature. We herewith report a case report of a middle aged female patient who presented with per vaginal bleeding, and on examination, a cervical mass was palpable. Also, a proliferative growth was seen over her left vulva. Biopsy of both lesions came out to be squamous cell carcinoma and carcinoma in situ, respectively. A radical hysterectomy and bilateral pelvic and paraaortic lymph nodal dissection was performed along with left simple vulvectomy. This thereby underscores that any lesion over vulva appearing during or after treatment of cervical carcinoma should be biopsied to rule out vulvar carcinoma.Keywords: carcinoma of cervix, carcinoma of vulva, synchronous tumours, gynecological oncology

The Correlation Structure associated with a portfolio is subjected to vary across time. Studying the structural breaks in the time-dependent Correlation matrix associated with a collection had been a subject of interest for a better understanding of the market movements, portfolio selection, etc. The current paper proposes a methodology for testing the change in the time-dependent correlation structure of a portfolio in the high dimensional data using the techniques of generalized inverse, singular valued decomposition and multivariate distribution theory which has not been addressed so far. The asymptotic properties of the proposed test are derived. Also, the performance and the validity of the method is tested on a real data set. Potential applications of wireless sensor networks are in healthcare, natural disaster prediction, data security, environmental monitoring, home appliances, entertainment etc. The design, development and deployment of WSN based on application requirements. The WSN design performance is optimized to improve network lifetime. The sensor node resources constrain such as energy and bandwidth imposes the limitation on efficient resource utilization and sensor node management. Abstract:
Diosmin is a flavonoid, most abundantly found in many citrus fruits. As a flavonoid, it possesses a multitude of biological activities including anti-hyperglycemic, anti-lipid peroxidative, anti-inflammatory, antioxidant, and anti-mutagenic properties. At this point, we established the anti-proliferative and apoptosis-inducing activities of diosmin in Hep-2 and KB cells. Diosmin has cytotoxic effects through inhibiting cellular proliferation of Hep-2 and KB cells, which leads to the induction of apoptosis, as apparent by an increase in the fraction of cells in the sub-G1phase of the cell cycle. Results exposed that inhibition of cell proliferation is associated with regulation of the Interleukins/STAT pathway. In addition, Diosmin treatment with Hep-2 and KB cells actively stimulated reactive oxygen species (ROS) and mitochondrial membrane depolarization. And also an imbalance in the Bax/Bcl-2 ratio triggered the caspase cascade and shifting the balance in favor of apoptosis. These observations conclude that Diosmin induce apoptosis via Interleukins /STAT-mediated pathway.Keywords: diosmin, apoptosis, antioxidant, STAT pathway

MnS is the most commonly found inclusion in steel, which is desirable for machinability of alloy steels but only up to a certain limit, beyond which it weakens fatigue properties of steel. In present work, the effect of sulfur content and its inclusions on the fatigue behavior of AISI 4140 steel is studied (sulfur content 0.002% and 0.016%). Metallurgical analysis, Mechanical testing and Rotating Bending Fatigue (RBF) test were carried out. With the increase in sulfur content, ductility and toughness of the material decrease significantly and large scatter is observed in UTS and impact energy values. From the results of RBF testing, it can be observed that increase in sulfur content from 0.002% to 0.016% has a negligible effect on the endurance strength of AISI 4140 for similar hardness level. Machining parameters are very important in determining the surface quality of any material. In the past decade, some new engineering materials were developed for the manufacturing industry which created a need to conduct an investigation on the impact of the said parameters on their surface roughness. The polyurethane (PU) block is widely used in the automotive industry to manufacture parts such as checking fixtures that are used to verify the dimensional accuracy of automotive parts. In this paper, the design of experiment (DOE) was used to investigate the effect of the milling parameters on the PU block. Furthermore, an analysis of the machined surface chemical composition was done using scanning electron microscope (SEM). It was found that the surface roughness of the PU block is severely affected when PU undergoes a flood machining process instead of a dry condition. Heavy machinery especially compactors has more vibrations induced from the compactor mechanism than the engines. Since the operator’s comfort is most important in any of the machines, this paper shows interest in studying the vibrations on the steering wheel for a double drum compactor. As there are no standard procedures available for testing vibrations on the steering wheel of double drum compactors, this paper tries to evaluate the vibrations on the steering wheel by considering most of the possibilities. In addition to the feasibility for the operator to adjust the steering wheel tilt as in the case of automotive, there is an option for the operator to change the orientation of the operator platform for the complete view of the road’s edge on both the ends of the front and rear drums. When the orientation is either +/-180°, the operator will be closer to the compactor mechanism; also there is a possibility for the shuffle in the modes with respect to the operator. Hence it is mandatory to evaluate the vibrations levels in both cases. This paper attempts to evaluate the vibrations on the steering wheel by considering the two operator platform positions and three steering wheel tilting angles.Keywords: FEA, CAE, steering column, steering column orientation position

The objective of this study is to investigate the significance of coal mine waste and improved clay soil when used as a fill and for the construction of embankment. To determine the bearing capacities of coal mine waste and improved clay soil apart from laboratory tests, PLAXIS 2D software is used to make the analysis simpler. Depending upon the bearing capacities obtained for different cases, the conclusion can be obtained. Load carrying capacities are determined for coal mine waste, clay and by altering their height ratio when clay (H2) is at the bottom and coal mine waste (H1) is on the top with three different cases (H = 0.25H1 + 0.75H2, 0.5H1 + 0.5H2, 0.75H1 + 0.25H2) in addition to this bearing capacity of improved clay soil (by replacing clay with 10% CMW, 30% CMW and 50% CMW, in addition, Polycom) is also determined. The safe height of the embankment that can be constructed with the improved clay for different slopes, i.e., for 1:1, 1.5: 1, 2: 1, is also determined by using PLAXIS 2D software by limiting the Factor of safety to 1.5.Keywords: cohesion, angle of shearing resistance, elastic modulus, coefficient of consolidation, coal mine waste

This paper proposed the harmonic elimination of Cascaded H-Bridge Multi-Level Inverter by using Selective Harmonic Elimination-Pulse Width Modulation method programmed with Particle Swarm Optimization algorithm. PSO method determine proficiently the required switching angles to eliminate low order harmonics up to the 11th order from the inverter output voltage waveform while keeping the magnitude of the fundamental harmonics at the desired value. Results demonstrate that the proposed method does efficiently eliminate a great number of specific harmonics and the output voltage is resulted in minimum Total Harmonic Distortion. Abstract:
Polymer nanocomposites represent a new class of materials in which nanomaterials act as the reinforcing material in composites, wherein small additions of nanomaterials lead to large enhancements in thermal, optical, and mechanical properties. A boost in these properties is due to the large interfacial area per unit volume or weight of the nanoparticles and the interactions between the particle and the polymer. Micro-sized particles used as reinforcing agents scatter light, thus, reducing light transmittance and optical clarity. Efficient nanoparticle dispersion combined with good polymer–particle interfacial adhesion eliminates scattering and allows the exciting possibility of developing strong yet transparent films, coatings and membranes. This paper aims at synthesizing zinc oxide nanoparticles which are reinforced in poly vinyl alcohol (PVA) polymer. Abstract:
The objective of this study is to investigate the preparation of gramine nano suspension and protective effect of Gramine on the apoptosis of laryngeal cancer cells cell line (HEp-2 and KB). The growth inhibition rate of Hep-2 and KB cells in vitro were measured by MTT assay and apoptosis by, levels of reactive oxygen species, mitochondrial membrane potential, morphological changes and flowcytometry. Based on the results, we determined the effective doses of gramine as 127.23µm/ml for 24 hr and 119.81 µm/ml for 48hr in hep-2 cell line and 147.58 µm ml for 24 hr and 123.74µm µm/ml for 48hr in KB cell line. cytotoxicity effects of gramine were confirmed by treatment of HEp-2 cell and KB cell with IC50 concentration of gramine resulted in sequences of events marked by the enhance the apoptosis accompanied by loss of cell viability, modulation of reactive oxygen species and cell cycle arrest through the induction of G0/G1 phase arrest on HEp-2 cells. Our study suggests that the nanosuspension of gramine possesses the more cytotoxic effect of cancer cells and a novel candidate for cancer chemoprevention.Keywords: apoptosis, HEp-2 cell line, KB cell line mitochondria, gramine, nanosuspension

Abstract:
This paper deals with the effect of Global Solar Radiation (GSR) on the performance of double flow solar air heater having corrugated plate as an absorber. An analytical model of a double flow solar air heater has been presented, and a computer program in C++ language has been developed to calculate the outlet air temperature, heat gain, pressure drop for estimating the thermal and thermohydraulic efficiencies. The performance of double flow corrugated absorber is compared with double flow flat plate and conventional solar air heaters. It is found that the double flow arrangement effectively increases the air temperature rise and efficiencies in comparison to a conventional collector. However, corrugated absorber is more superior to that of flat plate double flow solar air heater. The results indicate that increasing the solar radiation leads to achieve higher air temperature rise and efficiencies.Keywords: corrugated absorber, double flow, flat plate, solar air heater

Abstract:
The paper provides an updated version of volatility spillover among the equity markets of South Asian countries, including Pakistan, India, Srilanka, and Bangladesh. The analysis uses both symmetric and asymmetric Generalized Autoregressive Conditional Heteroscedasticity models to investigate volatility persistence and leverage effect. The bivariate EGARCH model is used to test for volatility transmission between two equity markets. Weekly data for the period February 2013 to August 2019 is used for empirical analysis. The findings indicate that the leverage effect exists in the equity markets of all the countries except Bangladesh. The volatility spillover from the equity market of Bangladesh to all other countries is negative and significant whereas the volatility of the equity market of Sri-Lanka does influence the volatility of any other country’s equity market. Indian equity market influence only the volatility of the Sri-Lankan equity market; and there is bidirectional volatility spillover between the equity markets of Pakistan and Bangladesh. Abstract:
Increasing concentration of green house gases (GHG's), such as CO2 is of major concern and start showing its impact nowadays. The recent studies are focused on developing the continuous system using photoautotrophs for CO2 mitigation and simultaneous production of primary and secondary metabolites as a value addition. The advent of carbon concentrating mechanism had blurred the distinction between autotrophs and heterotrophs and now the paradigm has shifted towards the carbon capture and utilization (CCU) rather than carbon capture and sequestration (CCS). In the present work, a bioreactor was developed utilizing the chemolithotrophic bacterial species using CO2 mitigation and simultaneous value addition. The kinetic modeling was done and the biokinetic parameters are obtained for developing the bioreactor. The bioreactor was developed and studied for its operation and performance in terms of volumetric loading rate, mass loading rate, elimination capacity and removal efficiency. The characterization of effluent from the bioreactor was carried out for the products obtained using the analyzing techniques such as FTIR, GC-MS, and NMR. The formability of metals depends on a number of variables such as strain, strain rate, and temperature. Though most of the metals are formable at room temperature, few are not. To evaluate the workability of such metals at elevated temperatures, thermomechanical experiments should be carried out to find out the forming temperatures and strain rates. Though a number of constitutive relations are available to correlate the material parameters and the corresponding formability at elevated temperatures, the constitutive rule proposed by Arrhenius has been used in this work. Thus, in the present work, the material constants such as A (constant), α (stress multiplier), β (constant), and n (stress exponent) of AA 2017 has been found by conducting a series of hot compression tests at different temperatures such as 400°C, 450°C, 500°C, and 550°C and at different strain rates such as 0.16, 0.18, and 0.2. True stress (σt), true strains (εt) deformation activation energy (Q), and the Zener-Hollomon parameter (Z value) were also calculated. The results indicate that the value of ln (Z) decreases as the temperature increases and it increases as the strain rate increases.Keywords: hot compression test, aluminium alloy, flow stress, activation energy

Determining the content of free lime is especially important to judge reactivity of the raw materials and clinker quality. The free lime limit isn’t the same for all cements; it depends on several factors, especially the temperature reached during the cooking and the grain size distribution in cement after grinding. Estimation of free lime by conventional method is influenced by the presence of portlandite and misleads the actual free lime content in the clinker for quality check up conditions. To ensure the product quality according to the standard specifications in terms of within the quality limits or not, a reliable, precise, and very reproducible method to quantify the relative phase abundances in the Portland Cement clinker and Portland Cements is to use X-ray diffraction (XRD) in combination with the Rietveld method. In the present study, a methodology was proposed using XRD to validate the obtained results of free lime by conventional method. The XRD and TG/DTA results confirm the presence of portlandite in the clinker to take the decision on the obtained free lime results through conventional method.Keywords: free lime, quantitative phase analysis, conventional method, x ray diffraction
