15282 Enhancing Patient Outcomes Through Quality Improvement: Reducing Contamination Rates in Karyotyping Samples via Effective Audits and Staff Engagement
Authors: Rofaida Ashour
Abstract:
This study discusses the implementation of quality improvement initiatives aimed at reducing contamination rates in cultured karyotyping samples. The primary objective was to enhance patient outcomes through systematic audits and targeted staff engagement. Recognizing the critical impact of sample integrity on diagnostic accuracy, a thorough analysis was conducted to identify the root causes of contamination. The project involved two audit cycles, which facilitated a comprehensive assessment of adherence to local protocols. Key issues identified included lapses in the use of personal protective equipment (PPE) and inadequate awareness of proper sample handling procedures among staff. To address these challenges, a multi-faceted approach was adopted. Firstly, a presentation was delivered to the laboratory team emphasizing the significance of strict adherence to PPE guidelines during the collection and handling of samples. This session aimed to raise awareness and foster a culture of safety within the unit. Additionally, informative posters illustrating the correct procedures were strategically placed around the laboratory to serve as ongoing visual reminders for staff. Recognizing the heightened risk associated with patients exhibiting fever or signs of infection, special measures were introduced to manage their sample collection. These proactive strategies were designed to minimize the likelihood of introducing contaminated samples into the culture process. The results of the audits demonstrated a significant reduction in contamination rates, underscoring the effectiveness of the interventions. This experience reinforced the importance of continuous quality improvement in healthcare settings, particularly in ensuring the delivery of high-quality, safe, and efficient services. Conducting regular audits not only provided valuable insights into operational practices but also highlighted the critical role of active team engagement and a data-driven approach in decision-making. Effective communication and collaboration among team members emerged as essential components for the success of quality improvement initiatives. Keywords: quality improvement, contamination rates, karyotyping samples, healthcare protocols, staff engagement

Abstract:
During the period 2009–2015, in Joniškėlis Experimental Station of the Lithuanian Research Centre for Agriculture and Forestry, the effect of ecologic fertilizers Ekoplant, bio-activators Biokal 01 and Terra Sorb Foliar and their combinations on the formation of the productivity elements, grain yield and quality of winter wheat, spelt (Triticum spelta L.), and common wheat (Triticum aestivum L.) was analysed in ecological agro-system. The soil under FAO classification – Endocalcari-Endo-hypogleyic-Cambisol. In a clay loam soil, ecological fertilizer produced from sunflower hull ash and this fertilizer in combination with plant extracts and bio-humus exerted an influence on the grain yield of spelt and common wheat and their mixture (increased the grain yield by 10.0%, compared with the unfertilized crops). Spelt grain yield was by on average 16.9% lower than that of common wheat and by 11.7% lower than that of the mixture, but the role of spelt in organic production systems is important because with no mineral fertilization it produced grains with a higher (by 4%) gluten content and exhibited a greater ability to suppress weeds (by on average 61.9% lower weed weight) compared with the grain yield and weed suppressive ability of common wheat and mixture. Spelt cultivation in a mixture with common wheat significantly improved quality indicators of the mixture (its grain contained by 2.0% higher protein content and by 4.0% higher gluten content than common wheat grain), reduced disease incidence (by 2-8%), and weed infestation level (by 34-81%).Keywords: common and spelt-wheat, ecological fertilizers, bio-activators, productivity elements, yield, quality

Procedia PDF Downloads 15215276 Enhancing Postharvest Quality and Shelf-Life of Leaf Lettuce (Lactuca sativa L.) by Altering Growing Conditions
Authors: Jung-Soo Lee, Ujjal Kumar Nath, IllSup Nou, Dulal Chandra
Abstract:
Leaf lettuce is one of the most important leafy vegetables that is used as raw for salad and part of everyday dishes in many parts of the world including Asian countries. Since it is used as fresh, its quality maintenance is crucial which depends on several pre- and postharvest factors. In order to investigate the effects of pre-fix factors on the postharvest quality, the interaction of pre-fix factors such as growing conditions and fixed factor like cultivars were evaluated. Four Korean leaf lettuce cultivars ‘Cheongchima’, ‘Cheongchuckmyeon’, ‘Geockchima’ and ‘Geockchuckmyeon’ were grown under natural condition (as control) and altered growing condition (green house) with excess soil water and 50% shading to monitor their postharvest qualities. Several growth parameters like plant height, number of leaves, leaf thickness, fresh biomass yield as well as postharvest qualities like fresh weight loss, respiration rate, changes in color and shelf-life were measured in lettuce during storage up to 36 days at 5°C. Plant height and the number of leaves were affected by both pre-fix growing conditions as well as the cultivars. However, fresh biomass yield was affected by only growing condition, whereas leaf thickness was affected by cultivars. Additionally, the degrees of fresh weight loss and respiration rate of leaf lettuce at postharvest stages were influenced by pre-fix growing conditions and cultivars. However, changes in color of leaves during storage were less remarkable in samples harvested from of ‘Cheongchima’ and ‘Cheongchuckmyeon’ cultivars grown in excess watering with 50% shade than that grown in control condition. Consequently, these two cultivars also showed longer shelf-life when they were grown in excess watering with 50% shade than other cultivars or samples were grown in control condition. Based on the measured parameters, it can be concluded that postharvest quality of leaf lettuce might be accelerated by growing lettuce under excess soil water with 50% shading. Keywords: cultivar, growing condition, leaf lettuce, postharvest quality, shelf-life

Authors: Hamitri-Guerfi Fatiha, Madani Khodir, Hadjal Samir, Kati Djamel, Youyou Ahcene
Abstract:
Date palm (Phoenix dactylifera) is a principal fruit that is grown in many regions of the world, resulting in a surplus production of dates. Algeria is considered to be one of the date producing countries. Date seeds (pits) have been a problem to the date industry as a waste stream. However, finding a way to make a profit on the pits would benefit date farmers substantially. This work concentrated on the valorization of date seed oils. A preliminary study was carried out on three varieties (soft, half soft, and dry) and we selected the dry variety. This work concerns the valorization of the date seed oil of the dry variety: ‘Mech Degla’ by its incorporation in a food formulation: margarine of table. Lipid extraction was carried out by hot extraction with the soxhlet; the extracts obtained are rich in fat contents, the results gave outputs of 13.21±0.21 %. The antioxidant activity of extracted oils was studied by the test of DPPH, the content polyphenols as well as the anti-radicalaire activity. The analysis of fatty acids was made by CPG. Thus, it comes out from our results that the recovered fat contents are interesting and considerable. A formulation of the margarine ‘BIO’ was elaborated on the scale industrialist by the addition of the extracts of date seeds ‘Mech-Degla’ oil in order to substitute a synthetic additive. The physicochemical characteristics of the elaborate margarines prove to be in conformity with the standards set by the Algerian companies. The texture of the elaborate margarine has an acceptable color, an aspect brilliant and homogeneous, it is plastic and easy to paste having an index of required SFC and the margarine melts easily in the mouth. Moreover, the evaluation of oxidative stability is carried out by the test of Rancimat. The result obtained reported that the margarine enriched with date seed oil, proved more resistant to oxidation, than the margarine without extract, which is improved much during incorporation of the extracts simultaneously. By conclusion, considering the content of polyphénols noted in the two extracts (aqueous and oily), we can exhort the scientific community to become aware of the treasures of our country especially the wonders of the south which are the dates and theirs under products (pits). Keywords: antioxydant activity, date seed oil, quality characteristics, margarine

Authors: Matjaz Prek
Abstract:
Impact of three different combinations of cooling and ventilation systems on the indoor environmental quality (IEQ) has been studied. Comparison of chilled ceiling cooling in combination with displacement ventilation, cooling with fan coil unit and cooling with flat wall displacement outlets was performed. All three combinations were evaluated from the standpoint of whole-body and local thermal comfort criteria as well as from the standpoint of ventilation effectiveness. The comparison was made on the basis of numerical simulation with DesignBuilder and Fluent. Numerical simulations were carried out in two steps. Firstly the DesignBuilder software environment was used to model the buildings thermal performance and evaluation of the interaction between the environment and the building. Heat gains of the building and of the individual space, as well as the heat loss on the boundary surfaces in the room, were calculated. In the second step Fluent software environment was used to simulate the response of the indoor environment, evaluating the interaction between building and human, using the simulation results obtained in the first step. Among the systems presented, the ceiling cooling system in combination with displacement ventilation was found to be the most suitable as it offers a high level of thermal comfort with adequate ventilation efficiency. Fan coil cooling has proved inadequate from the standpoint of thermal comfort whereas flat wall displacement outlets were inadequate from the standpoint of ventilation effectiveness. The study showed the need in evaluating indoor environment not solely from the energy use point of view, but from the point of view of indoor environmental quality as well. Keywords: cooling, ventilation, thermal comfort, ventilation effectiveness, indoor environmental quality, IEQ, computational fluid dynamics

Authors: Yeo Kyeong Lee, Ji Yeon Kang, Eun Mi Ryu, Hee Sun Kim, Yeong Soo Shin
Abstract:
The objective of this paper is an investigation of the effects of the thickness and axial loading during a fire test on the load-bearing capacity of a fire-damaged normal-strength concrete wall. Two factors are attributed to the temperature distributions in the concrete members and are mainly obtained through numerous experiments. Toward this goal, three wall specimens of different thicknesses are heated for 2 h according to the ISO-standard heating curve, and the temperature distributions through the thicknesses are measured using thermocouples. In addition, two wall specimens are heated for 2 h while simultaneously being subjected to a constant axial loading at their top sections. The test results show that the temperature distribution during the fire test depends on wall thickness and axial load during the fire test. After the fire tests, the specimens are cured for one month, followed by the loading testing. The heated specimens are compared with three unheated specimens to investigate the residual load-bearing capacities. The fire-damaged walls show a minor difference of the load-bearing capacity regarding the axial loading, whereas a significant difference became evident regarding the wall thickness. To validate the experiment results, finite element models are generated for which the material properties that are obtained for the experiment are subject to elevated temperatures, and the analytical results show sound agreements with the experiment results. The analytical method based on validated thought experimental results is applied to generate the fire-damaged walls with 2,800 mm high considering the buckling effect: typical story height of residual buildings in Korea. The models for structural analyses generated to deformation shape after thermal analysis. The load-bearing capacity of the fire-damaged walls with pin supports at both ends does not significantly depend on the wall thickness, the reason for it is restraint of pinned ends. The difference of the load-bearing capacity of fire-damaged walls as axial load during the fire is within approximately 5 %.Keywords: normal-strength concrete wall, wall thickness, axial-load ratio, slenderness ratio, fire test, residual strength, finite element analysis

Authors: Muhammad Akmal Putera, Noriyuki Yasufuku, Adel Alowaisy, Ahmad Rifai
Abstract:
Indonesia’s government has planned a project for a high-speed railway connecting the capital cities, Jakarta and Surabaya, about 700 km. Based on that location, it has been planning construction above the lowland soil region. The lowland soil region comprises cohesive soil with high water content and high compressibility index, which in fact, led to a settlement problem. Among the variety of railway track structures, the adoption of the ballastless track was used effectively to reduce the settlement; it provided a lightweight structure and minimized workspace. Contradictorily, deploying this thin layer structure above the lowland area was compensated with several problems, such as lack of bearing capacity and deflection behavior during traffic loading. It is necessary to combine with ground improvement to assure a settlement behavior on the clayey soil. Reflecting on the assurance of strength increment and working period, those were convinced by adopting methods such as cement-treated soil as the substructure of railway track. Particularly, evaluating mechanical properties in the field has been well known by using the plate load test and cone penetration test. However, observing an increment of mechanical properties has uncertainty, especially for evaluating cement-treated soil on the substructure. The current quality control of cement-treated soils was established by laboratory tests. Moreover, using small strain devices measurement in the laboratory can predict more reliable results that are identical to field measurement tests. Aims of this research are to show an intercorrelation of confining pressure with the initial condition of the Young modulus (E_o), Poisson ratio (υ_o) and Shear modulus (G_o) within small strain ranges. Furthermore, discrepancies between those parameters were also investigated. Based on the experimental result confirmed the intercorrelation between cement content and confining pressure with a power function. In addition, higher cement ratios have discrepancies, conversely with low mixing ratios. Keywords: amount of cement, elastic zone, high-speed railway, lightweight structure

Authors: Haruna Modibbo Usman, Mustapha Muktar, Nasiru Inuwa
Abstract:
This paper examined the long run relationship between health outcomes and economic growth in Nigeria from 1961 to 2012. Using annual time series data, Augmented Dickey-Fuller (ADF) test is conducted to check the stochastic properties of the variables. Also, the long run relationship among the variables is confirmed based on Johansen Multivariate Cointegration approach whereas the long run and short run dynamics are observed using Vector Error Correction Mechanism (VECM). In addition, VEC Granger causality test is employed to examine the direction of causality among the variables. On the whole, the results obtained revealed the existence of a long run relationship between health outcomes and economic growth in Nigeria and that both life expectancy and crude death rate as measures of health are found to have a long run negative and statistically significant impact on the economic growth over the study period. This is further buttressed by the results of Granger causality test which indicated the existence of unidirectional causality running from life expectancy and crude death rate to economic growth. The study therefore, calls for governments at various levels to create preconditions for health improvements in Nigeria in order to boost the level of health outcomes. Keywords: cointegration, economic growth, Granger causality, health outcomes, VECM

Abstract:
The problem of economic dispatch (ED) is the basic problem of power framework, its main goal is to find the most favorable generation dispatch to generate each unit, reduce the whole power generation cost, and meet all system limitations. A heuristic algorithm, recently developed called Stud Krill Herd (SKH), has been employed in this paper to treat non-convex ED problems. The proposed KH has been modified using Stud selection and crossover (SSC) operator, to enhance the solution quality and avoid local optima. We are demonstrated SKH effects in two case study systems composed of 13-unit and 40-unit test systems to verify its performance and applicability in solving the ED problems. In the above systems, SKH can successfully obtain the best fuel generator and distribute the load requirements for the online generators. The results showed that the use of the proposed SKH method could reduce the total cost of generation and optimize the fulfillment of the load requirements. Keywords: stud krill herd, economic dispatch, crossover, stud selection, valve-point effect

Authors: K. Zehouani, I. Oldal
Abstract:
Purpose: In the human knee joint, degenerative joint disease may happen with time. The standard treatment of this disease is the total knee replacement through prosthesis implanting. The reason lies in the fact that this phenomenon causes different material abrasion as compare to pure sliding or rolling alone. This study focuses on developing a knee prosthesis geometry, which fulfills the mechanical and kinematical requirements. Method: The MSC ADAMS program is used to describe the rotation of the human knee joint as a function of flexion, and to investigate how the flexion and rotation movement changes between the condyles of a multi-body model of the knee prosthesis as a function of flexion angle (in the functional arc of the knee (20-120º)). Moreover, the multi-body model with identical boundary conditions is constituted, and the numerical simulations are carried out using the MSC ADAMS program system. Results: It is concluded that the use of the multi-body model reduces time and cost since it does not need to manufacture the tibia and the femur as it requires for the knee prosthesis of the test machine. Moreover, without measuring or by dispensing with a test machine for the knee prosthesis geometry, approximation of the results of our model to a human knee is carried out directly. Conclusion: The pattern obtained by the multi-body model provides an insight for future experimental tests related to the rotation and flexion of the knee joint concerning the actual average and friction load. Keywords: biomechanics, knee joint, rotation, flexion, kinematics, MSC ADAMS

Authors: Ubaka Chukwuemeka Michael, Ukwe Chinwe Victoria
Abstract:
Background: The use of antipsychotic monotherapy remains the standard for schizophrenic disorders so also a prescription switch from older typical to newer atypical classes of antipsychotics on the basis of better efficacy and tolerability. However, surveys on the quality of antipsychotic drug use and substitution in developing countries are very scarce. This study was intended to evaluate quality and factors that drive the prescription and substitution of antipsychotic drugs among schizophrenic patients visiting a regional psychiatric hospital. Methods: Case files of patients visiting a federal government funded Neuropsychiatric Hospital between July 2012 and July 2014 were systematically retrieved. Patient demographic characteristics, clinical details and drug management data were collected and subjected to descriptive and inferential data analysis to determine quality and predictors of utilization. Results: Of the 600 case files used, there were more male patients (55.3%) with an overall mean age of 33.7±14.4 years. Typical antipsychotic agents accounted for over 85% of prescriptions, with majority of the patients receiving more than 2 drugs in at least a visit (80.9%). Fluphenazine (25.2%) and Haloperidol (18.8%) were mostly given as antipsychotics for treatment initiation while Olazenpine (23.0%) and Benzhexol (18.3%) were the most currently prescribed antipsychotics. Nearly half (42%, 252/600) of these patients were switched from one class to another, with 34.5% (207/600) of them switched from typical to atypical drug classes. No demographic or clinical factors influenced drug substitutions but a younger age and being married influenced being prescribed a polypharmacy regimen (more than 2 drugs) and an injectable antipsychotic agent. Conclusion: The prevalence of antipsychotic polypharmacy and use of typical agents among these patients was high. However, only age and marital status affected the quality of antipsychotic prescriptions among these patients. Keywords: antipsychotics, drug substitution, pharmacoepidemiology, polypharmacy

Abstract:
The fish meat plays an important role in the human health as it contains high quality protein. The tilapia fish considered as the third largest group of farmed fish. The oxidative deterioration of fish meat may occur during the cooking process. The proper cooking process and using natural antioxidant to prevent oxidation and enhance the quality of the tilapia fish fillet is necessary. Hence, this research was carried out to evaluate the potential of clove essential oil to prevent lipid peroxidation and enhance the antioxidant activity of tilapia fish fillet cooked using microwave and griller. The results showed that cooking using microwave significantly (p < 0.05) increased the lipid peroxidation and decreased the DPPH and ferric reducing activity power of the fish fillet as compared to grilling. The fortification of fish fillet using clove essential oil prevented from lipid peroxidation and enhanced the antioxidant activity of the fish fillet significantly (p < 0.05). Consequently, fortification of tilapia fish fillet using clove essential oil followed by cooking using griller to have high quality cooked fish meat is recommended. Keywords: antioxidant activity, fillet, fish, fortification, lipid peroxidation

Abstract:
Combination therapy may allow interaction on two drugs or more that can give adverse effects on patients. Simvastatin is a drug of antihyperlipidemia it can interact with drugs which work on cytochrome P450 CYP3A4 because it can interfere the performance of simvastatin. Flavonoid found in plants can inhibit the cytochrome P450 CYP3A4 if taken with simvastatin and can increase simvastatin levels in the body and increases the potential side effects of simvastatin such as myopati and rhabdomyolysis. Green tea leaves and mint are herbal medicine which has the effect of antihiperlipidemia. This study aims to determine the potential interaction of simvastatin with herbal drinks (green tea leaves and mint). This research method are experimental post-test only control design. Test subjects were divided into 5 groups: normal group, negative control group, simvastatin group, a combination of green tea group and the combination group mint leaves. The study was conducted over 32 days and total cholesterol levels were analyzed by enzymatic colorimetric test method. Results of this study is the obtainment of average value of total cholesterol in each group, the normal group (65.92 mg/dL), the negative control group the average total cholesterol test in the normal group was (69.86 mg/dL), simvastatin group (58.96 mg/dL), the combination of green tea group (58.96 mg/dL), and the combination of mint leaves (63.68 mg/dL). The conclusion is between simvastatin combination therapy with herbal drinks have the potential for pharmacodynamic interactions with a synergistic effect, antagonist, and a powerful additive, so the combination therapy are no more effective than a single administration of simvastatin therapy. Keywords: hyperlipidemia, simvastatin, herbal drinks, green tea leaves, mint leaves, drug interactions

Authors: Maris Klavins, Valery Rodinov
Abstract:
The aim of this study was to analyze long term changes of surface water quality in Latvia, spatial variability of water chemical composition, possible impacts of different pollution sources as well as to analyze the measures to protect national water resources - river basin management. Within this study, the concentrations of major water ingredients and microelements in major rivers and lakes of Latvia have been determined. Metal concentrations in river and lake waters were compared with water chemical composition. The mean concentrations of trace metals in inland waters of Latvia are appreciably lower than the estimated world averages for river waters and close to or lower than background values, unless regional impacts determined by local geochemistry. This may be explained by a comparatively lower level of anthropogenic load. In the same time in several places, direct anthropogenic impacts are evident, regarding influences of point sources both transboundary transport impacts. Also, different processes related to pollution of surface waters in Latvia have been analyzed. At first the analysis of changes and composition of pollutant emissions in Latvia has been realized, and the obtained results were compared with actual composition of atmospheric precipitation and their changes in time. Keywords: water quality, trend analysis, pollution, human impact
