2000 A Randomized Comparative Evaluation of Efficacy of Ultrasound Guided Costoclavicular and Supraclavicular Approaches of Brachial Plexus Block for Upper Limb Surgeries

Abstract:
Introduction: The costoclavicular approach, a modification to the infraclavicular approach, has been described for anesthesia for upper limb surgeries. Material And Methods: In this randomized and single-blind study, fourty patients undergoing emergency/elective upper limb surgery were allocated to two groups. Group C and S received ultrasound-guided Costoclavicular block and Supraclavicular block, respectively, with 20 ml 0.5 % ropivacaine with 8 mg dexamethasone under strict asepsis. The primary outcome assessed was the total duration of sensory and motor block in the postoperative period. Secondary outcomes were to compare the time taken to perform the procedure, block characteristics in terms of onset of motor and sensory blockade, the efficacy of analgesia with respect to the time of administration of the first rescue analgesic dose with both the blocks and note the side effects pertaining to either of the blocks. Results: The mean total duration of sensory and motor blockade was longer in group C vs. group S (p=0.002 and 0.024, respectively). The mean duration to perform a block in group S was more than in group C (p=0.012). The mean onset of sensory and motor Blockade Time in group S was more than in group C (p<0.001 and <0.001, respectively). The mean duration to perform a block in group S was more than in group C (p=0.012). Conclusion: The costoclavicular approach is better than supraclavicular in terms of rapid execution, faster onset of sensory-motor blockade, prolonged postoperative analgesia and similar PONV and safety profile.Keywords: costoclavicular, supraclavicular, ropivacaine, dexamethasone

Abstract:
The optimal conditions for green synthesis of zero-valent (G-NZVI) synthesis are investigated in this study using a Box Behnken design. The factors that were used in the study consisted of 3 factors as follows: the iron solution to mango peel extract ratio (1:1-1:3), feeding rate of mango peel extracts (1-5 mL/min), and agitation speed (300-30 rpm). The results showed that the optimization of conditions using the regression model was appropriate. The optimal conditions of the synthesis of G-NZVI for arsenate removal are the iron solution to mango peel extract ratio of 1:1, the feeding rate of mango peel extract at 5 mL/min, and the agitation speed rate of 300 rpm, which was able to arsenate removal of 100%.Keywords: Box Behnken design, arsenate removal, green nano zero valent iron, arsenic

Abstract:
The Chainat 1 variety (CN1) of rice, which generally has high amylose starch, is distributed in the lower part of Northern Thailand. CN1 rice starch can be used in both food and non-food products. In this research, the CN1 rice starch from the wet-milling process was prepared by Pre-Gelatinization (Heat-Moisture Treatments, HMT) under different conditions: percentage of moisture contents (20% and 30%) and duration time in minutes (0, 30, 60, and 90) at a specific temperature 110°C. The physicochemical characteristics of CN1 rice starch modification, such as amylose content, viscosity, swelling, and solubility property, were evaluated and compared with native CN1 rice starch. The results showed that modification CN1 rice starch tends to have some characteristics better than native starch. The appearance color and starch granule of modified CN1 by HMT have more effective characteristics than native starch when increased duration time. The duration time and moisture content are significant factors to the CN1 starch characteristic by HMT. Abstract:
This study was conducted during 2021.05.01 – 2021.08.31 at the Lithuanian University of health sciences and one Lithuanian dairy farm with 500 dairy cows (55.911381565736, 21.881321760608195). Average calving – 50 cows per month. Cows (n=20) in the treatment group (TG) were fed with feed supplement Optipartum C+ 200 (Enzymes: Alfa- Amylase 57 Units; Beta-Glucanase 107 Units) from 21 days before calving till 30 days after calving with feeding rate 200g/cow/day. Cows in the control group (CG) were fed a feed ration without feed supplement. Measurements started from 6 days before calving and continued till 21 days after calving. The following indicators were registered: with the RumiWatch System: Rumination time; Eating time; Drinking time; Rumination chews; Eating chews; Drinking gulps; Bolus; Chews per minute; Chews per bolus. With SmaXtec system - the temperature, pH of the contents of cows' reticulorumens and cows' activity. According to our results, we found that feeding of cows, from 21 days before calving to 30 days after calving, with a feed supplement with alfa- amylase and beta-glucanase (Optipartum C+ 200) (with dose 200g/cow/day) can produce an increase in: 9% rumination time and eating time, 19% drinking time, 11% rumination chews, 16% eating chews,13% number of boluses per rumination, 5% chews per minute and 16% chews per bolus. We found 1.28 % lower reiticulorumen pH and 0.64% lower reticulorumen temperature in cows fed with the supplement compared with control group cows. Background: Central venous catheter associated infections (CVC-AI) are among the serious hospital-acquired infections. The aims of this study are to determine the incidence of CVC-AI, and their risk factors among patients followed in a Tunisian medical intensive care unit (ICU). Materials / Methods: A prospective cohort study conducted between September 15th, 2015 and November 15th, 2016 in an 8-bed medical ICU including all patients admitted for more than 48h. CVC-AI were defined according to CDC of ATLANTA criteria. The enrollment was based on clinical and laboratory diagnosis of CVC-AI. For all subjects, age, sex, underlying diseases, SAPS II score, ICU length of stay, exposure to CVC (number of CVC placed, site of insertion and duration catheterization) were recorded. Risk factors were analyzed by conditional stepwise logistic regression. The p-value of < 0.05 was considered significant. Results: Among 192 eligible patients, 144 patients (75%) had a central venous catheter. Twenty-eight patients (19.4%) had developed CVC-AI with density rate incidence 20.02/1000 CVC-days. Among these infections, 60.7% (n=17) were systemic CVC-AI (with negative blood culture), and 35.7% (n=10) were bloodstream CVC-AI. The mean SAPS II of patients with CVC-AI was 32.76 14.48; their mean Charlson index was 1.77 1.55, their mean duration of catheterization was 15.46 10.81 days and the mean duration of one central line was 5.8+/-3.72 days. Gram-negative bacteria was determined in 53.5 % of CVC-AI (n= 15) dominated by multi-drug resistant Acinetobacter baumani (n=7). Staphylococci were isolated in 3 CVC-AI. Fourteen (50%) patients with CVC-AI died. Univariate analysis identified men (p=0.034), the referral from another hospital department (p=0.03), tobacco (p=0.006), duration of sedation (p=0.003) and the duration of catheterization (p=0), as possible risk factors of CVC-AI. Multivariate analysis showed that independent factors of CVC-AI were, male sex; OR= 5.73, IC 95% [2; 16.46], p=0.001, Ramsay score; OR= 1.57, IC 95% [1.036; 2.38], p=0.033, and duration of catheterization; OR=1.093, IC 95% [1.035; 1.15], p=0.001. Conclusion: In a monocenter cohort, CVC-AI had a high density and is associated with poor outcome. Abstract:
At the present time in Kazakhstan, the area for saxaul growing is strongly depopulateddue to anthropogenic and other factors. To prevent further reduction of natural haloxylon forest area their artificial crops are offered. Seed germination and survival of young plants in such haloxylon crops are very low. Insects, as one of the most important nutrient factors have appreciable effect on seed germination and saxaul productivity at the all stages of its formation. Insects, feeding on leaves, flowers, seeds and developing inside the trunk, branches, twigs, roots have a change in its formation and influence on the lifespan of saxaul. Representatives of Lepidoptera troop (Lepidopteraare the most harmful pests forsaxaul. As a result of our research we have identified 15 species of Lepidoptera living on haloxylon which display very different cycles and different types of food relations. It allows them to inhabit a variety of habitats, and feeding on various parts of saxaul. Some of them cause significant and sometimes very heavy damage for saxaul. There are 17identified species of Lepidoptera from the Coleophoridaefamily - 1, Gelechidae - 5, Pyralidae - 4, Noctuidae - 4, Lymantridae- 1, Cossidae - 2 species. At the same time we found 8 species for the first time, which have not been mentioned in the literature before. According to food specialization they are divided into monophages (2 types), oligophages (6 species) and polyphages (3 species). By affinity to plant parts, leaves and seeds are fed by 8 species, shoots by 1 specie, scions by 5 species, flowers, scions, seeds by 1, and 2species damage the roots and trunks. In whole installed seasonal groups of Lepidoptera - saxaul pests in the desert area, confined to the certain parts of the year, as well as certain parts of the plant for feeding. Abstract:
Neuropsychological problems are more common in hemodialysis (HD) patients than in healthy individuals. The aim of this study was to investigate the effect of long term HD on memory types of HD patients. To assess the different type of memory, we used memory parts of the Persian Papers and Pencil Cognitive assessment package (PCAP) and Addenbrooke's Cognitive Examination (ACE-R). Our study included 80 HD patients of whom 39 had less than six months of HD and 41 patients and another group which had a history of HD more than six months. The population had a mean age of 51.60 years old and 27.5% of them were female. The scores of patients who have been hemodialyzed for a long time (median time of HD was up to 4 years) had lower score in anterograde, explicit, visual, recall and recognition memory (5.44±1.07, 9.49±3.472, 22.805±6.6913, 5.59±10.435, 11.02±3.190 score) than the HD patients who underwent HD for a shorter term, where the median time was 3 to 5 months (P<0.01). The regression result shows that, by increasing the HD duration, all memory types are reduced (R2=0.600, P<0.01). The present study demonstrated that HD patients who were under HD for a long time had significantly lower scores in the different types of memory. Abstract:
The objective of this research was to study the appropriate thermal level and time for dabbing customers with the massage compressed pack reported from their perception. The investigation was conducted by comparing different angles of tilted heads done by the customers together with their perception before and after the dabbing. The variables included different temperature of the compressed packs and different dabbing duration. Samples in this study included volunteers who got massage therapy and dabbing with hot compressed packs by traditional Thai medical students. The experiment was conducted during January to June 2013. The research tool consisted of angle meters, stop watches, thermometers, and massage compressed packs. The customers were interviewed for their perceptions before and after the dabbing. The results showed that: 1. There was a difference of the average angles of tilted heads before and after the dabbing. 2. There was no difference of the average angles at different temperatures but constant duration. 3. There was no difference of the average angles at different durations. 4. The customers reported relaxation no matter what the various temperatures and various dabbing durations were. Abstract:
Hoverflies are commonly known as flower flies, sun flies or garden flies. Hoverflies are very important group of insects because their ecosystem services are diverse. They are an attractive group of insects with their striped abdomens. They are day-flying insects from small to large size, have worldwide distribution, but mostly prefer to live in relatively cold weather areas. In the world, about 6,000 species of 200 genera of two sub-families have been described. Their larvae exhibit a variety of feeding modes i.e. aphidophagous, saprophagous, zoophagous and Phytophagus, where adults are floral visitors of hundreds of different plants species. These floral resources enhance the longevity and fecundity of adult dipterous flies. Many syrphid species also have been documented as efficient crop pollinators. Aphids are commonly called plant louse, greenflies and blackflies. In the past 10 years, computers and cellphones have become one of the most important factors in our lives, and one which has a tremendously negative impact on our muscles. Muscle tension may be one of the causes of sleep disturbance. Tension in the shoulders and neck can affect blood circulation to the muscles. This research uses a dry needling treatment to reduce muscle tension in order to determine if the strain in the head and shoulders can influence sleep duration. All 38 patients taking part in the testing suffered from tinnitus and have been experiencing disturbed sleep for at least one to five years. Even after undergoing drug therapy treatments and traditional acupuncture therapies, their sleep disturbances have not shown any improvement. After five to 10 dry needling treatments, 24 of the patients reported an improvement in their sleep duration. Five patients considered themselves to be completely recovered, while 12 patients experienced no improvement. This study investigated these pathogenic and therapeutic problems. The standard treatment for sleep disturbances is drug-based therapy; the results of most standard treatments are unfortunately negative. The result of this clinical research has demonstrated that: The possible cause of sleep disturbance for a lot of patients is the result of tensions in the neck and shoulder muscles. Blood circulation to those muscles is also influenced by the duration of sleep. Hypertonic neck and shoulder muscles are considered to impact sleeping patterns and lead to disturbed sleep. Poor posture, often adopted while speaking on the phone, is one of the main causes of hypertonic neck and shoulder muscle problems. Abstract:
Aim: This retrospective study was created to analyze the psychometric, anthropometric and body composition values in patients at the beginning and the discharge of their of hospitalization in the residential care clinic for eating and feeding disorders (EFD’s). Method: The sample was composed by (N=59) patients with mean age N= 33,50, divided in subgroups: Anorexia Nervosa (AN) (N=28), Bulimia Nervosa (BN) (N=13) and Binge Eating Disorders (BED) (N=14) recruited from a residential care clinic for eating and feeding disorders. The psychometrics level was measured with self-report questionnaires: Eating Disorders Inventory-3 (EDI-3) The Body Uneasiness Test (BUT), Minnesota Multiphasic Personality Inventory (MMPI – 2). The anthropometric and nutritional values was collected by Body Impedance Assessment (B.I.A), Body mass index (B.M.I.). Measurements were made at the beginning and at the end of hospitalization, with an average time of recovery of about 8,6 months. B.M.I mean value in AN group is (T0 14,83 – T1 18,41) BN group (T0 20 – T1 21,33) BED group (T0 42,32 – T1 34,97) Phase Angle results: AN group (T0 4,78 – T1 5,64) BN (T0 6 – T1 6,53) BED group (T0 6 – T1 6,72). Discussion and conclusion: The evident presence that on the whole sample, we have an altered serious psychiatric and clinic conditions at the beginning of recovery. The interesting conclusions that we can draw from this analysis are that a multidisciplinary approach that includes the entire care of the subject: from the pharmacological treatment, analytical psychotherapy, Psychomotricity, nutritional rehabilitation, and rehabilitative, educational activities. Abstract:
The relationship between antecedent soil moisture content and soil erosion is a complicated phenomenon. Some studies confirm the effect of antecedent soil moisture content on soil erosion, but some deny it. Therefore, the objective of this study is to clarify such contradictions through field experiments. This study conducted two-year field observations of soil losses from natural rainfall events on runoff plots with a length of 10 meters, width of 3 meters, and uniform slope of 9%. Volumetric soil moisture sensors were used to log the soil moisture changes for each rainfall event. A total of 49 effective events were monitored. Results of this study show that antecedent soil moisture content promotes the generation of surface runoff, especially for rainfall events with short duration or lower magnitudes. A positive correlation was found between antecedent soil moisture content and soil loss per unit Rainfall-Runoff Erosivity Index, which indicated that soil with high moisture content is more susceptible to detachment. Once the rainfall duration exceeds 10 hours, the impact from the rainfall duration to soil erosion overwrites, and the effect of antecedent soil moisture is almost negligible.Keywords: antecedent soil moisture content, soil loss, runoff coefficient, rainfall-runoff erosivity

Radar reflectivity data from Phimai weather radar station of DRRAA (Department of Royal Rainmaking and Agricultural Aviation) were used to analyzed the rainstorm characteristics via Thunderstorm Identification Tracking Analysis and Nowcasting (TITAN) algorithm. The Phimai weather radar station was situated at Nakhon Ratchasima province, northeastern Thailand. The data from 277 days of rainstorm events occurring from May 2016 to May 2017 were used to investigate temporal distribution characteristics of convective individual rainclouds. The important storm properties, structures, and their behaviors were analyzed by 9 variables as storm number, storm duration, storm volume, storm area, storm top, storm base, storm speed, storm orientation, and maximum storm reflectivity. The rainstorm characteristics were also examined by separating the data into two periods as wet and dry season followed by an announcement of TMD (Thai Meteorological Department), under the influence of southwest monsoon (SWM) and northeast monsoon (NEM). According to the characteristics of rainstorm results, it can be seen that rainstorms during the SWM influence were found to be the most potential rainstorms over northeastern region of Thailand. The SWM rainstorms are larger number of the storm (404, 140 no./day), storm area (34.09, 26.79 km²) and storm volume (95.43, 66.97 km³) than NEM rainstorms, respectively. For the storm duration, the average individual storm duration during the SWM and NEM was found a minor difference in both periods (47.6, 48.38 min) and almost all storm duration in both periods were less than 3 hours. The storm velocity was not exceeding 15 km/hr (13.34 km/hr for SWM and 10.67 km/hr for NEM). For the rainstorm reflectivity, it was found a little difference between wet and dry season (43.08 dBz for SWM and 43.72 dBz for NEM). Abstract:
India is a tropical country, and it is estimated that biological and abiological agents are the major factors in the destruction and deterioration of archival materials like herbarium, paper, cellulose, bookbinding, etc. Silverfish, German Cockroaches, Termites, Booklice, Tobacco beetle and Carpet beetles are the common insect's pests in the museum, which causes deterioration to collections of museum specimens. Among them, silverfish is one of the most notorious pests and primarily responsible for the deterioration of Archival materials. So far, the investigation has been carried to overcome this existing problem as different management strategies such as chemical insecticides, fungicides, herbicides, nematicides, etc., have been applied. Moreover, Synthetic molecules lead to affect the ecological balance, have a detrimental effects on human health, reduce the beneficial microbial flora and fauna, etc. With a view, numbers of chemicals have been banned and advised not to be used due to their long-lasting persistency in soil ecosystem, water and carcinogenic. That’s why the authors used natural products with biocidal activity, cost-effective and eco-friendly approaches. In this study, various concentrations (30, 60 and 90 ml/L) of clove and lemongrass essential oil at different treatment duration (30, 60, 90 and 120-minutes) were investigated to test its properties as a silverfish repellent and insecticidal effect. The result of two ways ANOVA revealed that the mortality was significantly influenced by oil concentration, treatment duration and interaction between two independent factors was also found significant. The mortality rate increased with increasing the oil concentration in clove oil, and 100 % mortality was recorded in 0.9 ml at 120-minute. It was also observed that the treatment duration has the highest effect on the mortality rate of silverfish. The clove oil had the greatest effect on the silverfish in comparison to lemongrass. While in the case of percentage, repellency of adult silverfish was oil concentration and treatment duration-dependent, i.e., increase in concentration and treatment duration resulted in higher repellency percentage. The clove oil was found more effective, showing maximum repellency of 80.00% at 0.9ml/cm2 (highest) concentration, and in lemongrass highest repellency was observed at 33.4% at 0.9 ml/cm2 concentration in the treated area.Keywords: adult silverfish, oils, oil concentration, treatment duration, mortality (%) and repellency

Abstract:
Studies have shown that timing of protein intake, leucine content, and speed of digestion significantly affect postprandial protein utilization. Our aim was to determine if one can spare lean body mass during energy restriction by varying the quality and the timing of protein intake. Obese volunteers followed a 6-wk restricted energy diet. Four groups were compared: casein pulse, casein spread, milk-soluble protein (MSP, = whey) pulse, and MSP spread (n = 10-11 per group). In casein groups, caseins were the only protein source; it was MSP in MSP groups. Proteins were distributed in four meals per day in the proportion 8:80:4:8% in the pulse groups; it was 25:25:25:25% in the spread groups. We measured weight, body composition, nitrogen balance, 3-methylhistidine excretion, perception of hunger, plasma parameters, adipose tissue metabolism, and whole body protein metabolism. Volunteers lost 7.5 ± 0.4 kg of weight, 5.1 ± 0.2 kg of fat, and 2.2 ± 0.2 kg of lean mass, with no difference between groups. In adipose tissue, cell size and mRNA expression of various genes were reduced with no difference between groups. Hunger perception was also never different between groups. In the last week, due to a higher inhibition of protein degradation and despite a lower stimulation of protein synthesis, postprandial balance between whole body protein synthesis and degradation was better with caseins than with MSP. It seems likely that the positive effect of caseins on protein balance occurred only at the end of the experiment.Keywords: lean body mass, fat mass, casein, whey, protein metabolism

Abstract:
Considering response of uterus to ecbolic effect of oxytocin near the time of parturition, this study was done for investigating the effect of prophylactic administration of this hormone on duration of fetal membrane retention, time interval to first detectable estrus, time interval to first service, and conception rate at first service in cases of both normal parturition and dystocia. For this reason cows with (n=18) and without (n=18) dystocia assigned randomly to treatment (n=12) or control (n=6) groups and received intramuscular injection of 100 IU of oxytocin or 10 mL of normal saline respectively. Further observations and investigations indicate that duration of fetal retention is significantly shorter in treatment group cows compared to control groups, regardless of having dystocia (P=0.002) or normal spontaneous calving (P=0.001). The same trend exists for conception rate at first service in which cows in treatment groups had significantly higher conception rate (CR) in comparison to cows in control groups with (P=0.0003) or without dystocia (P=0.017). Abstract:
Biometrics and dietary habit of Citharinus citharus in the lower Niger River area of kogi state were studied between October and December, 2010. A total of 120 fish sampled were used for the study. The total length, standard length and weight were taken for each fish sample for the estimations of length-weight relationship using the formula W = aLb and transformed to Log W = Log a + b Log L. Stomach contents were analyzed by frequency of occurrence method. The standard length of males, females and combined sexes ranged between 6.8 - 16.5, 7.3 – 14.3 cm, 6.8 – 74.2 (cm) respectively, with b – values of 3.0963, 3.174 and 3.1382. The condition factor ranged from 2.04 – 2.80, 1.88 – 2.86 and 1.88 – 2.86 respectively. The food and feeding habits shows that the fish feeds mainly sand grain (25.83%), mud (24.16%), plant parts (12.50%), insect part (2.50%), algae (12.50%) and unidentified items (5.00%). C. citharus in the lower Niger area of kogi state could be termed to an omnivore. Abstract:
The delta inverter is considered as the reduced three-phase dc/ac converter topology. It contains only three two-quadrant power switches compared to six in the conventional one. This reduced power conversion topology is widely considered in many industrial applications, such as electric traction and large photovoltaic systems. This paper is focused on a new sinusoidal pulse width modulation algorithm (SPWM) developed for the delta inverter. As an unconventional inverter’s structure, irregular modulating functions waveforms of the SPWM switching technique are generated. The performances of the proposed SPWM technique was proven through computer simulations carried out on a delta inverter feeding a three-phase RL load. Digital Signal Processor (DSP) implementation of the novel SPWM algorithm have been realized on a laboratory prototype of the delta inverter feeding an RL load and a squirrel cage induction motor. Abstract:
Objectives: Malnutrition in hospitalized patients is related to increased morbidity and mortality. The purpose of our study was to compare various nutritional scores in order to detect the most suitable one for assessing the nutritional status of elderly, hospitalized patients and correlate them with mortality and extension of admission duration, due to patients’ critical condition. Methods: Sample population included 150 patients (78 men, 72 women, mean age 80±8.2). Nutritional status was assessed by Mini Nutritional Assessment (MNA full, short-form), Malnutrition Universal Screening Tool (MUST) and short Nutritional Appetite Questionnaire (sNAQ). Sensitivity, specificity, positive and negative predictive values and ROC curves were assessed after adjustment for the cause of current admission, a known prognostic factor according to previously applied multivariate models. Primary endpoints were mortality (from admission until 6 months afterwards) and duration of hospitalization, compared to national guidelines for closed consolidated medical expenses. Results: Concerning mortality, MNA (short-form and full) and SNAQ had similar, low sensitivity (25.8%, 25.8% and 35.5% respectively) while MUST had higher sensitivity (48.4%). In contrast, all the questionnaires had high specificity (94%-97.5%). Short-form MNA and sNAQ had the best positive predictive value (72.7% and 78.6% respectively) whereas all the questionnaires had similar negative predictive value (83.2%-87.5%). MUST had the highest ROC curve (0.83) in contrast to the rest questionnaires (0.73-0.77). With regard to extension of admission duration, all four scores had relatively low sensitivity (48.7%-56.7%), specificity (68.4%-77.6%), positive predictive value (63.1%-69.6%), negative predictive value (61%-63%) and ROC curve (0.67-0.69). Conclusion: MUST questionnaire is more advantageous in predicting mortality due to its higher sensitivity and ROC curve. Heat stress (HS) negatively affects the physiology of pigs. In this study, 6 pigs will be subjected to temperatures of 35 ± 2℃ for 12 hrs/day for a duration of 21 days. The changes in the gastrointestinal tract (GIT) microbiota will be observed by analyzing the freshly collected faeces on days 1, 3, 7, 14 and 21. The changes will be compared to faeces from a set of 6 control pigs kept simultaneously at temperatures of 26 ± 2℃ for the same duration of 21 days. Different types of stresses such a weaning have a detrimental effect on GIT microflora. Similarly, HS is expected to have a harmful effect on the microbial diversity of the GIT. How these changes affect the immune system of the pigs will be studied and therapeutics to reduce the negative effects of HS will be developed.Keywords: GIT microbiota, heat stress, immune system, therapeutics

Hence, there is vagueness about the time duration for activities in network planning. Fuzzy sets can handle such vague or imprecise concepts and has an application to such network. The vague activity times can be represented by triangular fuzzy numbers. In this paper, a general network with fuzzy activity times is considered and conditions for the critical path are obtained also we compute total float time of each activity. This study was designed to investigate the effect of breed and feeding level before slaughter on the skin\leather quality of the three main breeds of Sudan goats. Thirty (30) pieces of fresh skins from the three goat breeds (an average age 1-1.5 years) were chosen for the study purpose. For whole variations between the three breeds in two levels of feeding (poor and rich pastures) Complete Randomized Design (CRD) was used for data analysis. The results revealed that, leather weight (kg), elongation%, tensile strength (kg/cm2), cracking load (kg), thickness (mm), tear load (kg/cm) and chrome% findings were significantly affected (P≥0.05) by breed variation. Flexibility, moisture%, Ash% and fat % were not significantly affected (P ≥ 0.05) by breed. On the other hand, skin weight (kg), Cracking load (kg), Tear load (kg/cm) and Ash% were significantly affected (P≥0.05) by pasture quality. While Leather Elongation%, Tensile strength (kg/cm2), Thickness (mm), Flexibility, Moisture%, Fat % and Chrome% were not statistically (P ≥ 0.05) affected by pastures quality.Keywords: skin\leather quality, goats leather, natural pasture, Sudan

Abstract:
Blind oro-/naso-pharyngeal suction and feeding tube placement are very common practices in neonatal intensive care unit. Though esophageal perforation is a rare complication of these instrumentations, its prevalence is highest in extremely premature neonates. Due to its association with significant morbidity (including respiratory deterioration, pneumothorax, and sepsis) and even mortality, it is an important issue to prevent this iatrogenic complication in the field of premature care. We demonstrate an esophageal perforation in an extreme-low-birth-weight neonate after oro-gastric tube placement. This female baby weighing 680 grams was delivered by caesarean section at 25 weeks of gestational age. She initially received oro-tracheal intubation with mechanical ventilation which was smoothly weaned to non-invasive positive-pressure ventilation at 7-day-old. However, after insertion of a 5-French oro-gastric tube, the baby’s condition suddenly worsened with apnea requiring mechanical ventilation. Her chest radiogram showed the oro-gastric tube in right pleural space, and thus another oro-gastric tube was replaced, and its position was radiographically confirmed. The malpositioned tube was then removed. The baby received 2-week course of intravenous antibiotics for her esophageal perforation. Feeding was then reintroduced and increased to full feeds in a smooth course. She was discharged at 107-day-old. Esophageal perforation in newborn is very rare. Sudden respiratory deterioration in a neonate after naso-/oro-gastric tube placement should alarm us to consider esophageal perforation, and further radiological investigation is required for the diagnosis. Tube materials, patient condition, and age are major risk factors of esophageal perforation. The use of softer tube material, such as silicone, in extreme premature baby might prevent this fetal complication.Keywords: esophageal perforation, preterm, newborn, feeding tube

Abstract:
In the development of a text-to-speech synthesizer, automatic derivation of correct pronunciation from the grapheme form of a text is a central problem. Particularly deriving phonological features which are not shown in orthography is challenging. In the Amharic language, geminates and epenthetic vowels are very crucial for proper pronunciation but neither is shown in orthography. In this paper, we proposed and integrated a morphological analyzer into an Amharic Text-to-Speech system, mainly to predict geminates and epenthetic vowel positions, and prepared a duration modeling method. Amharic Text-to-Speech system (AmhTTS) is a parametric and rule-based system that adopts a cepstral method and uses a source filter model for speech production and a Log Magnitude Approximation (LMA) filter as the vocal tract filter. The naturalness of the system after employing the duration modeling was evaluated by sentence listening test and we achieved an average Mean Opinion Score (MOS) 3.4 (68%) which is moderate. By modeling the duration of geminates and controlling the locations of epenthetic vowel, we are able to synthesize good quality speech. Our system is mainly suitable to be customized for other Ethiopian languages with limited resources.Keywords: Amharic, gemination, speech synthesis, morphology, epenthesis

Abstract:
Dementia is a progressive terminal disease. Despite this recognition, research shows that most family caregivers do not know it, and it is unclear how this knowledge affects the quality of patient care. The aim of this qualitative study of 20 family caregivers for patients with advanced dementia is to examine how the caregiver's knowledge about dementia affects the quality of patient care in the context of healthcare decision-making, advanced care planning, and access to adequate support systems. Knowledge about dementia implies family caregivers' understanding of dementia trajectories, common symptoms/complications, and alternative treatment options (e.g., comfort feeding versus tube feeding). Data were collected in semi-structured interviews with 20 family caregivers. The interviews were conducted in person by the author and designed to elicit rich descriptions of family caregivers' experiences with healthcare decision-making and the management of common symptoms/complications of end-stage dementia as patient healthcare proxies. The study findings suggest that caregivers who recognize that dementia is a terminal disease are less likely to opt for life-extending treatments during the advanced stages. They are also more likely to seek palliative/hospice care, and consequently, they are better able to avoid unnecessary hospitalizations or medical procedures. For example, those who know that dementia is a terminal disease tend to opt for "comfort feeding" rather than "tube feeding" in managing the swallowing difficulties that accompany advanced dementia. In the context of advance care planning, family caregivers who know that dementia is a terminal disease tend to have more meaningful advance directives (e.g., Power of Attorney and Do Not Resuscitate orders). They are better prepared to anticipate common problems and pursue treatments that foster the best quality of patient life and care. Greater knowledge about advanced dementia helps them make more informed decisions that focus on enhancing the quality of patient life rather than just survival. In addition, those who know that dementia is a terminal disease are more likely to establish adequate support systems to help them cope with the complex demands of caregiving. For example, they are more likely to seek dementia-oriented primary care programs that offer house visits or respite services. Based on the study findings, knowledge about dementia as a terminal disease is critical in the optimal management of patient care needs and the establishment of adequate support systems. More research is needed to better understand what caregivers need to know to better prepare them for the complex demands of dementia caregiving.Keywords: dementia education, family caregiver, management of dementia, quality of care

The mite Varroa destructor Anderson & Trueman (Mesostigmata: Varroidae), is an exclusive hematophagous parasite of the Apis honey bees (Apidae: Hymenoptera). The early phoretic female mites have multiple small inactivated oocytes. Consequently, for the initial growth and vitellogenesis of the oocytes, the mother mite must feed on the hemolymph of the host, as a unique food source, by taking intermittently variable number of blood meals: 1. During the phoretic phase, to initiate vitellogenesis of the terminal oocyte, 2. From a freshly capped bee cell with a bee larva, up to the apolysis stage, to complete vitellogenesis and embryogenesis of the terminal oocyte, and 3. From all pupal stages, up to the imago stage, to induce oogenesis and vitellogenesis of the would-be nonembryonic female eggs. Additionally, oogenesis and vitellogenesis expressions in Varroa destructor and other Varroidae varies according to environmental conditions, e.g., chemical attractions produced by the adult bee and larvae, race of bees, sex of the larva, developmental period of the bee larva, food quality and quantity, and superparasitism (several cofoundressess). Also, the feeding stimuli obtained from the host hemolymph, indirectly regulate the reproductive physiology of the mite, by inducing different vitellogenin expressions, the production of a male egg first in the sequence followed by vitellogenesis of the would-be female eggs during the pupal stages. Furthermore, the different uptakes of hemolymph from the host, also indirectly induce the production of the male egg first in the sequence, local mate competition (LMC) and variable adaptive female sex ratios in the broods, especially when superparasitism occurs. Consequently, reproduction in Varroa destructor, and probably in other Varroidae, depends exclusively on feeding in the hemolymph of the bee host, even during the phoretic phase, the prepupal stages and during the pupal stages; and that, the feeding factors are common syndromes in other Varroidae.Keywords: oogenesis, sex determination, varroa destructor, vitellogenesis

Abstract:
Introduction: Feeding and Eating Disorders (FED) represent the psychiatric pathology with the highest mortality rate and one of the major disorders with the highest psychiatric and clinical comorbidity. The vagus nerve represents one of the main components of the sympathetic and parasympathetic nervous system and is involved in important neurophysiological functions. In FED, there is a spectrum of symptoms which with TaVNS (Transcutaneous Auricular Vagus Nerve Stimulation) therapy, is possible to have a therapeutic efficacy. Materials and Methods: Sample subjects are composed of 15 female subjects aged > 18 ± 51. Admitted to a psychiatry community having been diagnosed according to DSM-5: anorexia nervosa (AN) (N= 9), bulimia nervosa (BN) (N= 5), binge eating disorder (BED) (N= 1). The protocol included 9 weeks of Ta-VNS stimulation at a frequency of 1.5-3.5 mA for 4 hours per day. The variables detected are the following: Heart Rate Variability (HRV), Hamilton Depression Rating Scale (HAMD-HDRS-17), Body Mass Index (BMI), Beck Anxiety Index (BAI). Results: Data analysis showed statistically significant differences between recording times (p > 0.05) in HAM-D (t0 = 18.28 ± 5.31; t4 = 9.14 ± 7.15), in BAI (t0 = 24.7 ± 10.99; t4 = 13.8 ± 7.0). The reported values show how during (T0-T4) the treatment there is a decay of the degree in the depressive state, in the state of anxiety, and an improvement in the value of BMI. In particular, the BMI in the AN-BN sub-sample had a minimum gain of 5% and a maximum of 11%. The analysis of HRV did not show a clear change among subjects, thus confirming the discordance of the activity of the sympathetic and parasympathetic nervous system in FED. Conclusions: Although the sample does not possess a relevant value to determine long-term efficacy of Ta-VNS or on a larger population, this study reports how the application of neuro-stimulation in FED may become a further approach therapeutic. The total of obese mothers is growing throughout Asia. Breastfeeding provides the perfect nutrition for infants, by promoting a higher IQ and protecting against childhood and adult diseases. A prospective cohort study was carried out of mothers attending eight antenatal clinics run by the Ministry of Health in Selangor, Malaysia to document the prevalence of obesity and its relationship with breastfeeding outcomes. Mothers were enrolled during the antenatal period and followed up until 6 months postpartum to document breastfeeding outcomes. A total of 652 Malay mothers were recruited for the study a response rate of 93.1 %. The pre-pregnancy body mass index (BMI) of the mothers showed that 36.5% of the mothers were overweight or obese. There were a total of 78 obese mothers in the sample and 41 (52.6%) of these mothers were able to initiate breastfeeding within one hour of birth compared to 238/337 (70.6 %, χ² 9.35, p<0.001) of those with a normal BMI. At 6 months, 23.1 % of obese mothers were exclusively breastfeeding their infants, compared to 56.0 % of the normal BMI mothers. On the other hand, the rate of infant formula feeding was higher in the obese mothers by 53.8 % compared to 19.0 % among normal weight mothers, χ² 37.6, p<0.001). Further analysis suggested these factors were found to be positively associated with discontinued exclusive breastfeeding at 6 months among obese mothers; mothers whom delayed breastfeeding initiation, had health problems during pregnancy, caesarean delivery, reported had insufficient colostrum/milk and babies had sucking problems at or before 4 weeks. Besides that, mothers who perceived their biological mothers had preference towards formula feeding or were ambivalent about the feeding method and had biological mothers without experience in breastfeeding for more than 1 month also were more likely to discontinue exclusive breastfeeding at 6 months. These findings suggested that the greater the pre-pregnant BMI, the earlier the cessation of exclusive breastfeeding and they were also less likely to initiate breastfeeding and have less adequate milk supply. Future investigations of the effects of maternal obesity on breastfeeding outcomes should be conducted along with effective interventions to advance the care of obese women at reproductive age and their children.Keywords: exclusive breastfeeding, body mass index (BMI), breastfeeding discontinuation, maternal obesity

In this study, towards a green approach, we have investigated the effect of operating conditions of solvent assessed twin-screw extruder (TSE) for the production of 4, 4-bipyridine (1-dimensional coordinated polymer (1D)) based coordinated polymer using cobalt nitrate as a metal precursor with molar ratio 1:1. Different operating parameters such as solvent percentage, screw speed and feeding rate are considered. The resultant product is characterized using offline characterization methods, namely Powder X-ray diffraction (PXRD), Raman spectroscopy and scanning electron microscope (SEM) in order to investigate the product purity and surface morphology. A lower feeding rate increased the product’s quality as more resident time was provided for the reaction to take place. The most important influencing factor was the amount of liquid added. Background: Factors associated with overweight and obesity among antiretroviral therapy (ART) recipients have not been sufficiently studied in Botswana. We aimed to study (i) the prevalence and trends in overweight/obesity by duration of exposure to ART among recipients, (ii) changes in body mass index (BMI) categories among recipients before ART initiation (BMI-1) and after ART initiation (BMI-2), (iii) associations between ART and overweight/obesity and (iv) factors associated with BMI changes among ART recipients. Methods: A 12 years retrospective record-based review was conducted. Factors potentially associated with BMI change among patients after at least three years of ART exposure were examined using multiple regression model. Adjusted odds ratios (AOR) and their 95% confidence intervals (CIs) were computed. ART regimens, duration of exposure to ART, and recipients’ demographic and biomedical characteristics including the presence or absence of diabetes mellitus related comorbidities (DRC) were investigated as potential factors associated with overweight/obesity. Results: Twenty-nine percent of recipients were overweight, 16.6% had obesity of whom 2.4% were morbidly-obese at the last clinic visit. Overweight/obesity recipients were more likely to be female, to have DRC and less likely to have nadir CD4 count or CD4 count between 201 – 249 cells/mm³. Neither the first-line nor the second-, third-line ART regimens predicted overweight/obesity more than the other and neither did the duration of exposure to ART. No significant linear trends were observed in the prevalence of overweight/obesity by the duration of exposure to ART. Conclusions: These results indicate that overweight/obesity seen among ART recipients is not directly induced by ART. ART used CD4 and/or DRC pathway to induce overweight/obesity seen among recipients; suggesting that, weight gain documented herein is likely a reflection of improved health status that mirrors trends in the general population or a DRC related effect.
