306 Adverse Drug Reactions Monitoring in the Northern Region of Zambia

Authors: Ponshano Kaselekela, Simooya O. Oscar, Lunshano Boyd

Abstract:

The Copperbelt University Health Services (CBUHS) was designated by the Zambia Medicines Regulatory Authority (ZAMRA), formally the Pharmaceutical Regulatory Authority (PRA) as a regional pharmacovigilance centre to carryout activities of drug safety monitoring in four provinces in Zambia. CBUHS’s mandate included stimulating the reporting of adverse drug reactions (ADRs), as well as collecting and collating ADR reports from health institutions in the four provinces. This report covers the researchers’ experiences from May 2008 to September, 2016. The main objectives are 1) to monitor ADRs in the Zambian population, 2) to disseminate information to all health professionals in the region advising that the CBU health was a centre for reporting ADRs in the region, 3) to monitor polypharmacy as well as the benefit-risk profile of medicines, 4) to generate independent, evidence based recommendations on the safety of medicines, 5) to support ZAMRA in formulating safety related regulatory decisions for medicines, and 6) to communicate findings with all key stakeholders. The methodology involved monthly visits, beginning in early May 2008 to September, 2016, by the CBUHS to health institutions in the programme areas. Activities included holding discussions with health workers, distribution of ADR forms and collection of ADRs reports. These reports, once collected, were documented and assessed at the CBUHS. A report was then prepared for ZAMRA on quarterly basis. At ZAMRA, serious ADRs were noted and recommendations made to the Ministry of Health of the Republic of Zambia. The results show that 2,600 ADRs reports were received at the pharmacovigilance regional centre. Most of the ADRs reports that received were due to antiretroviral drugs, as well as a few from anti-malarial drugs like Artemether/Lumefantrine – Coartem®. Three hundred and twelve ADRs were entered in the Uppsala Monitoring Centre WHO Vigiflow for further analysis. It was concluded that in general, 2008-16 were exciting years for the pharmacovigilance group at CBUHS. From a very tentative beginning, a lot of strides were made and contacts established with healthcare facilities in the region. The researchers were encouraged by the support received from the Copperbelt University management, the motivation provided by ZAMRA and most importantly the enthusiasm of health workers in all the health care facilities visited. As a centre for drug safety in Zambia, the results show it achieves its objectives for monitoring ADRs, Pharmacovigilance (drug safety monitoring), and activities of monitoring ADRs as well as preventing them. However, the centre faces critical challenges caused by erratic funding that prevents the smooth running of the programme.

304 Advanced Information Extraction with n-gram based LSI

Authors: Ahmet Güven, Ö. Özgür Bozkurt, Oya Kalıpsız

Abstract:

Number of documents being created increases at an increasing pace while most of them being in already known topics and little of them introducing new concepts. This fact has started a new era in information retrieval discipline where the requirements have their own specialties. That is digging into topics and concepts and finding out subtopics or relations between topics. Up to now IR researches were interested in retrieving documents about a general topic or clustering documents under generic subjects. However these conventional approaches can-t go deep into content of documents which makes it difficult for people to reach to right documents they were searching. So we need new ways of mining document sets where the critic point is to know much about the contents of the documents. As a solution we are proposing to enhance LSI, one of the proven IR techniques by supporting its vector space with n-gram forms of words. Positive results we have obtained are shown in two different application area of IR domain; querying a document database, clustering documents in the document database.

303 Fastest Growing Crime with Invisible Chains: A Review of Escaping Sex Trafficking Frameworks in Canada

Authors: Alisha Fisher

Abstract:

Survivors of sex trafficking often report extensive harm not just from the violence itself, but multiple levels such as internalized shame, societal misunderstandings, and the process of reporting, exiting, and healing. The aim of this article is to examine the multi-layered approach to supporting survivors who are exiting sex trafficking through immediate, short-term, and long-term care approaches. We present a systematic review of the current barriers structurally, psychosocially, and psychologically through a Canadian perspective, and apply them to the interventions within the service continuum, basic needs, and further needs and supports to consider. This article suggests that ongoing and additional funding to survivor’s support services, specialized police and heath care training, and increased prevention and public education on the realities of sex trafficking in Canada is a necessity for survivor healing, and the prevention of further harm.

Authors: Muhammad I. Ibrahimy, Mamun B.I. Reaz, Khandaker Asaduzzaman, Sazzad Hussain

Abstract:

An approach to develop the FPGA of a flexible key RSA encryption engine that can be used as a standard device in the secured communication system is presented. The VHDL modeling of this RSA encryption engine has the unique characteristics of supporting multiple key sizes, thus can easily be fit into the systems that require different levels of security. A simple nested loop addition and subtraction have been used in order to implement the RSA operation. This has made the processing time faster and used comparatively smaller amount of space in the FPGA. The hardware design is targeted on Altera STRATIX II device and determined that the flexible key RSA encryption engine can be best suited in the device named EP2S30F484C3. The RSA encryption implementation has made use of 13,779 units of logic elements and achieved a clock frequency of 17.77MHz. It has been verified that this RSA encryption engine can perform 32-bit, 256-bit and 1024-bit encryption operation in less than 41.585us, 531.515us and 790.61us respectively.

298 Geometry Design Supported by Minimizing and Visualizing Collision in Dynamic Packing

Authors: Johan Segeborn, Johan S. Carlson, Robert Bohlin, Rikard Söderberg

Abstract:

This paper presents a method to support dynamic packing in cases when no collision-free path can be found. The method, which is primarily based on path planning and shrinking of geometries, suggests a minimal geometry design change that results in a collision-free assembly path. A supplementing approach to optimize geometry design change with respect to redesign cost is described. Supporting this dynamic packing method, a new method to shrink geometry based on vertex translation, interweaved with retriangulation, is suggested. The shrinking method requires neither tetrahedralization nor calculation of medial axis and it preserves the topology of the geometry, i.e. holes are neither lost nor introduced. The proposed methods are successfully applied on industrial geometries.

297 Numerical Investigation on the Effects of Deep Excavation on Adjacent Pile Groups Subjected to Inclined Loading

Authors: Ashkan Shafee, Ahmad Fahimifar

Abstract:

There is a growing demand for construction of high-rise buildings and infrastructures in large cities, which sometimes require deep excavations in the vicinity of pile foundations. In this study, a two-dimensional finite element analysis is used to gain insight into the response of pile groups adjacent to deep excavations in sand. The numerical code was verified by available experimental works, and a parametric study was performed on different working load combinations, excavation depth and supporting system. The results show that the simple two-dimensional plane strain model can accurately simulate the excavation induced changes on adjacent pile groups. It was found that further excavation than pile toe level and also inclined loading on adjacent pile group can severely affect the serviceability of the foundation.

292 Seismic Behavior of a Jumbo Container Crane in the Low Seismicity Zone Using Time-History Analyses

Authors: Huy Q. Tran, Bac V. Nguyen, Choonghyun Kang, Jungwon Huh

Abstract:

Jumbo container crane is an important part of port structures that needs to be designed properly, even when the port locates in low seismicity zone such as in Korea. In this paper, 30 artificial ground motions derived from the elastic response spectra of Korean Building Code (2005) are used for time history analysis. It is found that the uplift might not occur in this analysis when the crane locates in the low seismic zone. Therefore, a selection of a pinned or a gap element for base supporting has not much effect on the determination of the total base shear. The relationships between the total base shear and peak ground acceleration (PGA) and the relationships between the portal drift and the PGA are proposed in this study.

Authors: Minsoo Lee, Yoon-kyung Lee, Hyejung Yoon, Soo-kyung Song, Sujeong Cheong

Abstract:

Data Warehousing tools have become very popular and currently many of them have moved to Web-based user interfaces to make it easier to access and use the tools. The next step is to enable these tools to be used within a portal framework. The portal framework consists of pages having several small windows that contain individual data warehouse query results. There are several issues that need to be considered when designing the architecture for a portal enabled data warehouse query tool. Some issues need special techniques that can overcome the limitations that are imposed by the nature of data warehouse queries. Issues such as single sign-on, query result caching and sharing, customization, scheduling and authorization need to be considered. This paper discusses such issues and suggests an architecture to support data warehouse queries within Web portal frameworks.

288 Supporting Technology Transfer with Communities and Social Software Solutions

Authors: G. Schuh, S. Aghassi

Abstract:

In order to bridge the gap between research and industry, promoting technology and knowledge transfer becomes increasingly important. Especially small- and medium-sized enterprises, having only little R&D resources themselves, depend on external technology development activities for remaining innovative. Academia research on the other hand needs potential industrial partners, who are capable and willing to commercialize their technologies as most public funding programs require some sort of technology transfer or dissemination activities. Modern web technologies offer more and more “social” functionalities and open up new ways of user interaction. In the past years several technology transfer platforms were developed, making use of modern web technologies in order to enable and support technology transfer. In this paper we report on the results of a state-of-the art analyses of existing technology transfer platforms, point out their advantages and deficits and give a perspective to the development of an improved technology transfer platform.

286 Hypergraph Models of Metabolism

Authors: Nicole Pearcy, Jonathan J. Crofts, Nadia Chuzhanova

Abstract:

In this paper, we employ a directed hypergraph model to investigate the extent to which environmental variability influences the set of available biochemical reactions within a living cell. Such an approach avoids the limitations of the usual complex network formalism by allowing for the multilateral relationships (i.e. connections involving more than two nodes) that naturally occur within many biological processes. More specifically, we extend the concept of network reciprocity to complex hyper-networks, thus enabling us to characterise a network in terms of the existence of mutual hyper-connections, which may be considered a proxy for metabolic network complexity. To demonstrate these ideas, we study 115 metabolic hyper-networks of bacteria, each of which can be classified into one of 6 increasingly varied habitats. In particular, we found that reciprocity increases significantly with increased environmental variability, supporting the view that organism adaptability leads to increased complexities in the resultant biochemical networks.

Abstract:

In order to apply partially filled steel grid composite deck as the horizontal supporting structure of various kinds of infrastructures, the variation of its flexural strength according to design parameters such as cross and longitudinal bars constituting the steel grid and the type of shear connection is evaluated and compared experimentally. The result shows that the design sensitivity of the deck to the spacing of the cross bars is insignificant in the case of structure with low risk of punching failure or without load distribution problem. By means of shear connection composed by transverse rebar and longitudinal bar without additional shear stud bolts, the complete interaction between steel grid and concrete slab is able to be achieved and the composite deck can develop its bending resistance capacity.

278 A Retrospective Cohort Study on an Outbreak of Gastroenteritis Linked to a Buffet Lunch Served during a Conference in Accra

Authors: Benjamin Osei Tutu, Sharon Annison

Abstract:

On 21st November, 2016, an outbreak of foodborne illness occurred after a buffet lunch served during a stakeholders’ consultation meeting held in Accra. An investigation was conducted to characterise the affected people, determine the etiologic food, the source of contamination and the etiologic agent and to implement appropriate public health measures to prevent future occurrences. A retrospective cohort study was conducted via telephone interviews, using a structured questionnaire developed from the buffet menu. A case was defined as any person suffering from symptoms of foodborne illness e.g. diarrhoea and/or abdominal cramps after eating food served during the stakeholder consultation meeting in Accra on 21st November, 2016. The exposure status of all the members of the cohort was assessed by taking the food history of each respondent during the telephone interview. The data obtained was analysed using Epi Info 7. An environmental risk assessment was conducted to ascertain the source of the food contamination. Risks of foodborne infection from the foods eaten were determined using attack rates and odds ratios. Data was obtained from 54 people who consumed food served during the stakeholders’ meeting. Out of this population, 44 people reported with symptoms of food poisoning representing 81.45% (overall attack rate). The peak incubation period was seven hours with a minimum and maximum incubation periods of four and 17 hours, respectively. The commonly reported symptoms were diarrhoea (97.73%, 43/44), vomiting (84.09%, 37/44) and abdominal cramps (75.00%, 33/44). From the incubation period, duration of illness and the symptoms, toxin-mediated food poisoning was suspected. The environmental risk assessment of the implicated catering facility indicated a lack of time/temperature control, inadequate knowledge on food safety among workers and sanitation issues. Limited number of food samples was received for microbiological analysis. Multivariate analysis indicated that illness was significantly associated with the consumption of the snacks served (OR 14.78, P < 0.001). No stool and blood or samples of etiologic food were available for organism isolation; however, the suspected etiologic agent was Staphylococcus aureus or Clostridium perfringens. The outbreak could probably be due to the consumption of unwholesome snack (tuna sandwich or chicken. The contamination and/or growth of the etiologic agent in the snack may be due to the breakdown in cleanliness, time/temperature control and good food handling practices. 277 Unified Fusion Approach with Application to SLAM

Authors: Xinde Li, Xinhan Huang, Min Wang

Abstract:

In this paper, we propose the pre-processor based on the Evidence Supporting Measure of Similarity (ESMS) filter and also propose the unified fusion approach (UFA) based on the general fusion machine coupled with ESMS filter, which improve the correctness and precision of information fusion in any fields of application. Here we mainly apply the new approach to Simultaneous Localization And Mapping (SLAM) of Pioneer II mobile robots. A simulation experiment was performed, where an autonomous virtual mobile robot with sonar sensors evolves in a virtual world map with obstacles. By comparing the result of building map according to the general fusion machine (here DSmT-based fusing machine and PCR5-based conflict redistributor considereded) coupling with ESMS filter and without ESMS filter, it shows the benefit of the selection of the sources as a prerequisite for improvement of the information fusion, and also testifies the superiority of the UFA in dealing with SLAM.
