Authors: Ali Hussian Ali AlTimemy, Fawzi M. Al Naima
Abstract:
This paper presents the prediction of kidney dysfunction using different neural network (NN) approaches. Self organization Maps (SOM), Probabilistic Neural Network (PNN) and Multi Layer Perceptron Neural Network (MLPNN) trained with Back Propagation Algorithm (BPA) are used in this study. Six hundred and sixty three sets of analytical laboratory tests have been collected from one of the private clinical laboratories in Baghdad. For each subject, Serum urea and Serum creatinin levels have been analyzed and tested by using clinical laboratory measurements. The collected urea and cretinine levels are then used as inputs to the three NN models in which the training process is done by different neural approaches. SOM which is a class of unsupervised network whereas PNN and BPNN are considered as class of supervised networks. These networks are used as a classifier to predict whether kidney is normal or it will have a dysfunction. The accuracy of prediction, sensitivity and specificity were found for each type of the proposed networks .We conclude that PNN gives faster and more accurate prediction of kidney dysfunction and it works as promising tool for predicting of routine kidney dysfunction from the clinical laboratory data. Keywords: Kidney Dysfunction, Prediction, SOM, PNN, BPNN, Urea and Creatinine levels.

Authors: Rujira Kongnuy, Puntani Pongsumpun, I-Ming Tang
Abstract:
Mathematical models can be used to describe the dynamics of the spread of infectious disease between susceptibles and infectious populations. Dengue fever is a re-emerging disease in the tropical and subtropical regions of the world. Its incidence has increased fourfold since 1970 and outbreaks are now reported quite frequently from many parts of the world. In dengue endemic regions, more cases of dengue infection in pregnancy and infancy are being found due to the increasing incidence. It has been reported that dengue infection was vertically transmitted to the infants. Primary dengue infection is associated with mild to high fever, headache, muscle pain and skin rash. Immune response includes IgM antibodies produced by the 5th day of symptoms and persist for 30-60 days. IgG antibodies appear on the 14th day and persist for life. Secondary infections often result in high fever and in many cases with hemorrhagic events and circulatory failure. In the present paper, a mathematical model is proposed to simulate the succession of dengue disease transmission in pregnancy and infancy. Stability analysis of the equilibrium points is carried out and a simulation is given for the different sets of parameter. Moreover, the bifurcation diagrams of our model are discussed. The controlling of this disease in infant cases is introduced in the term of the threshold condition. Keywords: Dengue infection, equilibrium states, maternalantibodies, pregnancy and infancy.

Abstract:
This paper presents a customized deformable model for the segmentation of abdominal and thoracic aortic aneurysms in CTA datasets. An important challenge in reliably detecting aortic aneurysm is the need to overcome problems associated with intensity inhomogeneities and image noise. Level sets are part of an important class of methods that utilize partial differential equations (PDEs) and have been extensively applied in image segmentation. A Gaussian kernel function in the level set formulation, which extracts the local intensity information, aids the suppression of noise in the extracted regions of interest and then guides the motion of the evolving contour for the detection of weak boundaries. The speed of curve evolution has been significantly improved with a resulting decrease in segmentation time compared with previous implementations of level sets. The results indicate the method is more effective than other approaches in coping with intensity inhomogeneities. Keywords: Abdominal and thoracic aortic aneurysms, intensityinhomogeneity, level sets, local fitting binary.

Abstract:
The main objective of this paper is to provide an efficient tool for delineating brain tumors in three-dimensional magnetic resonance images and set up compression-transmit schemes to distribute result to the remote doctor. To achieve this goal, we use basically a level-sets approach to delineating brain tumors in threedimensional. Then introduce a new compression and transmission plan of 3D brain structures based for the meshes simplification, adapted for time to the specific needs of the telemedicine and to the capacities restricted by wireless network communication. Abstract:
Alpfa-fetoprotein and its fragments may be an important vehicle for targeted delivery of radionuclides to the tumor. We investigated the effect of conditions on the labeling of biologically active synthetic peptide based on the (F-afp) with technetium-99m. The influence of the nature of the buffer solution, pH, concentration of reductant, concentration of the peptide and the reaction temperature on the yield of labeling was examined. As a result, the following optimal conditions for labeling of (F-afp) are found: pH 8.5 (phosphate and bicarbonate buffers) and pH from 1.7 to 7.0 (citrate buffer). The reaction proceeds with sufficient yield at room temperature for 30 min at the concentration of SnCl2 and (Fafp) (F-afp) is to be less than 10 mkg/ml and 25 mkg/ml, respectively. Investigations of the test drug accumulation in the tumor cells of human breast cancer were carried out. Results can be assumed that the in vivo study of the (F-afp) in experimental tumor lesions will show concentrations sufficient for imaging these lesions by SPECT.

Abstract:
Synchronization between 0.1 Hz oscillations in heart rate and blood pressure is studied and its change during vertical tilt is evaluated in 37 myocardial infarction patients. Two groups of patients are identified with decreased and increased, respectively, synchronization of the studied oscillations as a response to a tilt test. It is shown that assessment of synchronization of 0.1 Hz oscillations as a response to vertical tilt can be used as a guideline for selecting optimal dose of beta-blocker treatment in post-myocardial infarction patients.

Abstract:
A method and apparatus for noninvasive measurement of blood glucose concentration based on transilluminated laser beam via the Index Finger has been reported in this paper. This method depends on atomic gas (He-Ne) laser operating at 632.8nm wavelength. During measurement, the index finger is inserted into the glucose sensing unit, the transilluminated optical signal is converted into an electrical signal, compared with the reference electrical signal, and the obtained difference signal is processed by signal processing unit which presents the results in the form of blood glucose concentration. This method would enable the monitoring blood glucose level of the diabetic patient continuously, safely and noninvasively. Keywords: Anisotropy factor, Blood glucose, Diabetes Mellitus, Noninvasive method, Photo detectors.

Abstract:
The linear methods of heart rate variability analysis such as non-parametric (e.g. fast Fourier transform analysis) and parametric methods (e.g. autoregressive modeling) has become an established non-invasive tool for marking the cardiac health, but their sensitivity and specificity were found to be lower than expected with positive predictive value <30%. This may be due to considering the RR-interval series as stationary and re-sampling them prior to their use for analysis, whereas actually it is not. This paper reviews the non-linear methods of HRV analysis such as correlation dimension, largest Lyupnov exponent, power law slope, fractal analysis, detrended fluctuation analysis, complexity measure etc. which are currently becoming popular as these uses the actual RR-interval series. These methods are expected to highly accurate cardiac health prognosis. Keywords: chaos, nonlinear dynamics, sample entropy, approximate entropy, detrended fluctuation analysis.

Abstract:
This study examined the toxicological effects and safety of polypeptide k isolated from the seeds of Momordica charantia in laboratory rats. 30 male Sprague Dawley rats (12 weeks old, bodyweight 180-200 g) were randomly divided into 3 groups (1000 mg/kg, 500 mg and 0 mg/kg). Rats were acclimatized to laboratory conditions for 7 days and at day 8 rats were dosed orally with polypeptide k (in 2% DMSO/normal saline) and the controls received the dosed vehicle only. Rats were then observed for 72 hours before sacrificed. Rats were anaesthetized by pentobarbital (50 mg/kg ip) and 2-3.0 mL of blood was taken by cardiac puncture and rats were scarified by anaesthetic overdose. Immediately, organs (heart, lungs, liver, kidneys) were weigh and taken for histology. Organ sections were then evaluated by a histopathologist. Serum samples were assayed for liver functions (ALT and γ-GT) and kidney functions (BUN and creatinine). All rats showed normal behavior after the dosing and no statistical changes were observed in all blood parameters and organ weight. Histological examinations revealed normal organ structures. In conclusion, dosing of rats up to 1000 mg/kg did not have any effects on the rat behavior, liver or kidney functions nor histology of the selected organs. Keywords: Polypeptide k, safety, histology, toxicology, Momordica charantia

Abstract:
One of the approaches enabling people with amputated limbs to establish some sort of interface with the real world includes the utilization of the myoelectric signal (MES) from the remaining muscles of those limbs. The MES can be used as a control input to a multifunction prosthetic device. In this control scheme, known as the myoelectric control, a pattern recognition approach is usually utilized to discriminate between the MES signals that belong to different classes of the forearm movements. Since the MES is recorded using multiple channels, the feature vector size can become very large. In order to reduce the computational cost and enhance the generalization capability of the classifier, a dimensionality reduction method is needed to identify an informative yet moderate size feature set. This paper proposes a new fuzzy version of the well known Fisher-s Linear Discriminant Analysis (LDA) feature projection technique. Furthermore, based on the fact that certain muscles might contribute more to the discrimination process, a novel feature weighting scheme is also presented by employing Particle Swarm Optimization (PSO) for estimating the weight of each feature. The new method, called PSOFLDA, is tested on real MES datasets and compared with other techniques to prove its superiority. Keywords: Discriminant Analysis, Pattern Recognition, SignalProcessing.

Abstract:
This study determines the effect of naked and heparinbased super-paramagnetic iron oxide nanoparticles on the human cancer cell lines of A2780. Doxorubicin was used as the anticancer drug, entrapped in the SPIO-NPs. This study aimed to decorate nanoparticles with heparin, a molecular ligand for 'active' targeting of cancerous cells and the application of modified-nanoparticles in cancer treatment. The nanoparticles containing the anticancer drug DOX were prepared by a solvent evaporation and emulsification cross-linking method. The physicochemical properties of the nanoparticles were characterized by various techniques, and uniform nanoparticles with an average particle size of 110±15 nm with high encapsulation efficiencies (EE) were obtained. Additionally, a sustained release of DOX from the SPIO-NPs was successful. Cytotoxicity tests showed that the SPIO-DOX-HP had higher cell toxicity than the individual HP and confocal microscopy analysis confirmed excellent cellular uptake efficiency. These results indicate that HP based SPIO-NPs have potential uses as anticancer drug carriers and also have an enhanced anticancer effect. Keywords: Heparin, A2780 cells, ovarian cancer, nanoparticles, doxorubicin.

Abstract:
The p53 tumor suppressor gene plays two important roles in genomic stability: blocking cell proliferation after DNA damage until it has been repaired, and starting apoptosis if the damage is too critical. Codon 72 exon4 polymorphism (Arg72Pro) of the P53 gene has been implicated in cancer risk. Various studies have been done to investigate the status of p53 at codon 72 for arginine (Arg) and proline (Pro) alleles in different populations and also the association of this codon 72 polymorphism with various tumors. Our objective was to investigate the possible association between P53 Arg72Pro polymorphism and susceptibility to colorectal cancer among Isfahan and Chaharmahal Va Bakhtiari (a part of south west of Iran) population. We investigated the status of p53 at codon 72 for Arg/Arg, Arg/Pro and Pro/Pro allele polymorphisms in blood samples from 145 colorectal cancer patients and 140 controls by Nested-PCR of p53 exon 4 and digestion with BstUI restriction enzyme and the DNA fragments were then resolved by electrophoresis in 2% agarose gel. The Pro allele was 279 bp, while the Arg allele was restricted into two fragments of 160 and 119 bp. Among the 145 colorectal cancer cases 49 cases (33.79%) were homozygous for the Arg72 allele (Arg/Arg), 18 cases (12.41%) were homozygous for the Pro72 allele (Pro/Pro) and 78 cases (53.8%) found in heterozygous (Arg/Pro). In conclusion, it can be said that p53Arg/Arg genotype may be correlated with possible increased risk of this kind of cancers in south west of Iran. Keywords: TP53, Polymorphism, Colorectal Cancer, Iran

Abstract:
Feature-based registration is an effective technique for clinical use, because it can greatly reduce computational costs. However, this technique, which estimates the transformation by using feature points extracted from two images, may cause misalignments. To handle with this limitation, we propose to extract the salient edges and extracted control points (CP) of medical images by using efficiency of multiresolution representation of data nonsubsampled contourlet transform (NSCT) that finds the best feature points. The MR images were first decomposed using the NSCT, and then Edge and CP were extracted from bandpass directional subband of NSCT coefficients and some proposed rules. After edge and CP extraction, mutual information was adopted for the registration of feature points and translation parameters are calculated by using particle swarm optimization (PSO). The experimental results showed that the proposed method produces totally accurate performance for registration medical CT-MR images.

Abstract:
Raman spectroscopy are used to characterize the chemical changes in normoxic polyhydroxyethylacrylate gel dosimeter (PHEA) induced by radiation. Irradiations in the low dose region are performed and the polymerizations of PHEA gels are monitored by the observing the changes of Raman shift intensity of the carbon covalent bond of PHEA originated from both monomer and the cross-linker. The variation in peak intensities with absorbed dose was observed. As the dose increase, the peak intensities of covalent bond of carbon in the polymer gels decrease. This point out that the amount of absorbed dose affect the polymerization of polymer gels. As the absorbed dose increase, the polymerizations also increase. Results verify that PHEA gel dosimeters are sensitive even in lower dose region. Keywords: normoxic polymer gel, ascorbic acid, Ramanspectroscopy, radiation dosimetry.

Abstract:
The purpose of this research is to compare the original intra-oral digital dental radiograph images with images that are enhanced using a combination of image processing algorithms. Intraoral digital dental radiograph images are often noisy, blur edges and low in contrast. A combination of sharpening and enhancement method are used to overcome these problems. Three types of proposed compound algorithms used are Sharp Adaptive Histogram Equalization (SAHE), Sharp Median Adaptive Histogram Equalization (SMAHE) and Sharp Contrast adaptive histogram equalization (SCLAHE). This paper presents an initial study of the perception of six dentists on the details of abnormal pathologies and improvement of image quality in ten intra-oral radiographs. The research focus on the detection of only three types of pathology which is periapical radiolucency, widen periodontal ligament space and loss of lamina dura. The overall result shows that SCLAHE-s slightly improve the appearance of dental abnormalities- over the original image and also outperform the other two proposed compound algorithms. Keywords: intra-oral dental radiograph, histogram equalization, sharpening, CLAHE.

The purpose of this study was to examine the effect of stiffness and damping coefficient in a 3-dimensional (3D) model of a motor pump and spring. In the present paper, experimental and mathematical tests for the moments of inertia of the 3D model and the material properties were investigated by an INSTRON machine. The response surfaces could be generated by using 3D multi-body analysis and the design of experiment method. It showed that differences in contours of the response surface were clearly found for the particular area. Displacement of the center of the motor pump was decreased at K≈2000 N/M, C≈12.5 N-sec/M. However, the frequency was increased at K≈2000 N/M, C≈15 N-sec/M. In this study, this study suggested experimental technique for vibration reduction for a motor pump in medical device. Abstract:
In this work a surgical simulator is produced which enables a training otologist to conduct a virtual, real-time prosthetic insertion. The simulator provides the Ear, Nose and Throat surgeon with real-time visual and haptic responses during virtual cochlear implantation into a 3D model of the human Scala Tympani (ST). The parametric model is derived from measured data as published in the literature and accounts for human morphological variance, such as differences in cochlear shape, enabling patient-specific pre- operative assessment. Haptic modeling techniques use real physical data and insertion force measurements, to develop a force model which mimics the physical behavior of an implant as it collides with the ST walls during an insertion. Output force profiles are acquired from the insertion studies conducted in the work, to validate the haptic model. The simulator provides the user with real-time, quantitative insertion force information and associated electrode position as user inserts the virtual implant into the ST model. The information provided by this study may also be of use to implant manufacturers for design enhancements as well as for training specialists in optimal force administration, using the simulator. The paper reports on the methods for anatomical modeling and haptic algorithm development, with focus on simulator design, development, optimization and validation. Abstract:
Paced Auditory Serial Addition Test (PASAT) has been used as a common research tool for different neurological disorders like Multiple Sclerosis. Recently, technology let researchers to introduce a new versions of the visual test, the paced visual serial addition test (PVSAT). In this paper, the computerized version of these two tests is introduced. Beside the number of true responses are interpreted, the reaction time of subjects are calculated by the software. We hypothesize that paying attention to the reaction time may be valuable. For this purpose, sixty eight female normal subjects and fifty eight male normal subjects are enrolled in the study. We investigate the similarity between the PASAT3 and PVSAT3 in number of true responses and the new criterion (the average reaction time of each subject). The similarity between two tests were rejected (p-value = 0.000) which means that these two test differ. The effect of sex in the tests were not approved since the pvalues of different between PASAT3 and PVSAT3 in both sex is the same (p-value = 0.000) which means that male and female subjects performed the tests at no different level of performance. The new criterion shows a negative correlation with the age which offers aged normal subjects may have the same number of true responses as the young subjects but they have latent responses. This will give prove for the importance of reaction time. Keywords: Paced Auditory Serial Addition Test, Pace Visual Serial Addition Test, reaction time.

Abstract:
In this paper we used data mining techniques to identify outlier patients who are using large amount of drugs over a long period of time. Any healthcare or health insurance system should deal with the quantities of drugs utilized by chronic diseases patients. In Kingdom of Bahrain, about 20% of health budget is spent on medications. For the managers of healthcare systems, there is no enough information about the ways of drug utilization by chronic diseases patients, is there any misuse or is there outliers patients. In this work, which has been done in cooperation with information department in the Bahrain Defence Force hospital; we select the data for Cardiac patients in the period starting from 1/1/2008 to December 31/12/2008 to be the data for the model in this paper. We used three techniques for finding the drug utilization for cardiac patients. First we applied a clustering technique, followed by measuring of clustering validity, and finally we applied a decision tree as classification algorithm. The clustering results is divided into three clusters according to the drug utilization, for 1603 patients, who received 15,806 prescriptions during this period can be partitioned into three groups, where 23 patients (2.59%) who received 1316 prescriptions (8.32%) are classified to be outliers. The classification algorithm shows that the use of average drug utilization and the age, and the gender of the patient can be considered to be the main predictive factors in the induced model. Keywords: Data Mining, Clustering, Classification, Drug Utilization..

Authors: Thaweesak Yingthawornsuk, Chusak Thanawattano
Abstract:
Acoustical properties of speech have been shown to be related to mental states of speaker with symptoms: depression and remission. This paper describes way to address the issue of distinguishing depressed patients from remitted subjects based on measureable acoustics change of their spoken sound. The vocal-tract related frequency characteristics of speech samples from female remitted and depressed patients were analyzed via speech processing techniques and consequently, evaluated statistically by cross-validation with Support Vector Machine. Our results comparatively show the classifier's performance with effectively correct separation of 93% determined from testing with the subjectbased feature model and 88% from the frame-based model based on the same speech samples collected from hospital visiting interview sessions between patients and psychiatrists. Keywords: Depression, SVM, Vocal Extract, Vocal Tract

Authors: Dalanya Asaad Mohammed, Dara Abdul Razaq
Abstract:
A total of twenty tensile biopsies were collected from children undergoing tonsillectomy from teaching hospital ENT department and Kurdistan private hospital in sulaimani city. All biopsies were homogenized and cultured; the obtained bacterial isolates were purified and identified by biochemical tests and VITEK 2 compact system. Among the twenty studied samples, only one Pseudomonas putida with probability of 99% was isolated. Antimicrobial susceptibility was carried out by disk diffusion method, Pseudomonas putida showed resistance to all antibiotics used except vancomycin. The isolate further subjected to PCR and DNA sequence analysis of blaVIM gene using different set of primers for different regions of VIM gene. The results were found to be PCR positive for the blaVIM gene. To determine the sequence of blaVIM gene, DNA sequencing performed. Sequence alignment of blaVIM gene with previously recorded blaVIM gene in NCBI- database showed that P. putida isolate have different blaVIM gene. Keywords: Clinical isolates, Putida, Sulaimani, Vim gene.

Authors: Hirofumi Taki, Takuya Sakamoto, Makoto Yamakawa, Tsuyoshi Shiina, Toru Sato
Abstract:
For the improvement of the ability in detecting small calcifications using Ultrasonography (US) we propose a novel indicator of calcifications in an ultrasound B-mode image without decrease in frame rate. Since the waveform of an ultrasound pulse changes at a calcification position, the decorrelation of adjacent scan lines occurs behind a calcification. Therefore, we employ the decorrelation of adjacent scan lines as an indicator of a calcification. The proposed indicator depicted wires 0.05 mm in diameter at 2 cm depth with a sensitivity of 86.7% and a specificity of 100%, which were hardly detected in ultrasound B-mode images. This study shows the potential of the proposed indicator to approximate the detectable calcification size using an US device to that of an X-ray imager, implying the possibility that an US device will become a convenient, safe, and principal clinical tool for the screening of breast cancer. Keywords: Ultrasonography, Calcification, Decorrelation, Forward scattered wave

Abstract:
Purpose: To explore the use of Curvelet transform to extract texture features of pulmonary nodules in CT image and support vector machine to establish prediction model of small solitary pulmonary nodules in order to promote the ratio of detection and diagnosis of early-stage lung cancer. Methods: 2461 benign or malignant small solitary pulmonary nodules in CT image from 129 patients were collected. Fourteen Curvelet transform textural features were as parameters to establish support vector machine prediction model. Results: Compared with other methods, using 252 texture features as parameters to establish prediction model is more proper. And the classification consistency, sensitivity and specificity for the model are 81.5%, 93.8% and 38.0% respectively. Conclusion: Based on texture features extracted from Curvelet transform, support vector machine prediction model is sensitive to lung cancer, which can promote the rate of diagnosis for early-stage lung cancer to some extent. Keywords: CT image, Curvelet transform, Small pulmonary nodules, Support vector machines, Texture extraction.
