Procedia PDF Downloads 22072 Successful Rehabilitation of Recalcitrant Knee Pain Due to Anterior Cruciate Ligament Injury Masked by Extensive Skin Graft: A Case Report
Authors: Geum Yeon Sim, Tyler Pigott, Julio Vasquez
Abstract:
A 38-year-old obese female with no apparent past medical history presented with left knee pain. Six months ago, she sustained a left knee dislocation in a motor vehicle accident that was managed with a skin graft over the left lower extremity without any reconstructive surgery. She developed persistent pain and stiffness in her left knee that worsened with walking and stair climbing. Examination revealed healed extensive skin graft over the left lower extremity, including the left knee. Palpation showed moderate tenderness along the superior border of the patella, exquisite tenderness over MCL, and mild tenderness on the tibial tuberosity. There was normal sensation, reflexes, and strength in her lower extremities. There was limited active and passive range of motion of her left knee during flexion. There was instability noted upon the valgus stress test of the left knee. Left knee magnetic resonance imaging showed high-grade (grade 2-3) injury of the proximal superficial fibers of the MCL and diffuse thickening and signal abnormality of the cruciate ligaments, as well as edema-like subchondral marrow signal change in the anterolateral aspect of the lateral femoral condyle weight-bearing surface. There was also notable extensive scarring and edema of the skin, subcutaneous soft tissues, and musculature surrounding the knee. The patient was managed with left knee immobilization for five months, which was complicated by limited knee flexion. Physical therapy consisting of quadriceps, hamstrings, gastrocnemius stretching and strengthening, range of motion exercises, scar/soft tissue mobilization, and gait training was given with marked improvement in pain and range of motion. The patient experienced a further reduction in pain as well as an improvement in function with home exercises consisting of continued strengthening and stretching. Keywords: ligamentous injury, trauma, rehabilitation, knee pain
Procedia PDF Downloads 10871 Study on a Family of Optimal Fourth-Order Multiple-Root Solver
Authors: Young Hee Geum
Abstract:
In this paper,we develop the complex dynamics of a family of optimal fourth-order multiple-root solvers and plot their basins of attraction. Mobius conjugacy maps and extraneous fixed points applied to a prototype quadratic polynomial raised to the power of the known integer multiplicity m are investigated. A 300 x 300 uniform grid centered at the origin covering 3 x 3 square region is chosen to visualize the initial values on each basin of attraction in accordance with a coloring scheme based on their dynamical behavior. The illustrative basins of attractions applied to various test polynomials and the corresponding statistical data for convergence are shown to confirm the theoretical convergence. Keywords: basin of attraction, conjugacy, fourth-order, multiple-root finder
Procedia PDF Downloads 29370 A Case of Myelofibrosis-Related Arthropathy: A Rare and Underrecognized Entity
Authors: Geum Yeon Sim, Jasal Patel, Anand Kumthekar, Stanley Wainapel
Abstract:
A 65-year-old right-hand dominant African-American man, formerly employed as a security guard, was referred to Rehabilitation Medicine with bilateral hand stiffness and weakness. His past medical history was only significant for myelofibrosis, diagnosed 4 years earlier, for which he was receiving scheduled blood transfusions. Approximately 2 years ago, he began to notice stiffness and swelling in his non-dominant hand that progressed to pain and decreased strength, limiting his hand function. Similar but milder symptoms developed in his right hand several months later. There was no history of prior injury or exposure to cold. Physical examination showed enlargement of metacarpophalangeal (MCP) and proximal interphalangeal (PIP) joints with finger flexion contractures, Swan-neck and Boutonniere deformities, and associated joint tenderness. Changes were more prominent in the left hand. X-rays showed mild osteoarthritis of several bilateral PIP joints. Anti-nuclear antibodies, rheumatoid factor, and cyclic citrullinated peptide antibodies were negative. MRI of the hand showed no erosions or synovitis. A rheumatology consultation was obtained, and the cause of his symptoms was attributed to myelofibrosis-related arthropathy with secondary osteoarthritis. The patient was tried on diclofenac cream and received a few courses of Occupational Therapy with limited functional improvement. Primary myelofibrosis (PMF) is a rare myeloproliferative neoplasm characterized by clonal proliferation of myeloid cells with variable morphologic maturity and hematopoietic efficiency. Rheumatic manifestations of malignancies include direct invasion, paraneoplastic presentations, secondary gout, or hypertrophic osteoarthropathy. PMF causes gradual bone marrow fibrosis with extramedullary metaplastic hematopoiesis in the liver, spleen, or lymph nodes. Musculoskeletal symptoms are not common and are not well described in the literature. The first reported case of myelofibrosis related arthritis was seronegative arthritis due to synovial invasion of myeloproliferative elements. Myelofibrosis has been associated with autoimmune diseases such as systemic lupus erythematosus, progressive systemic sclerosis, and rheumatoid arthritis. Gout has been reported in patients with myelofibrosis, and the underlying mechanism is thought to be related to the high turnover of nucleic acids that is greatly augmented in this disease. X-ray findings in these patients usually include erosive arthritis with synovitis. Treatment of underlying PMF is the treatment of choice, along with anti-inflammatory medications. Physicians should be cognizant of recognizing this rare entity in patients with PMF while maintaining clinical suspicion for more common causes of joint deformities, such as rheumatic diseases. Keywords: myelofibrosis, arthritis, arthralgia, malignancy
Procedia PDF Downloads 9869 Cascaded Multi-Level Single-Phase Switched Boost Inverter
Authors: Van-Thuan Tran, Minh-Khai Nguyen, Geum-Bae Cho
Abstract:
Recently, multilevel inverters have become more attractive for researchers due to low total harmonic distortion (THD) in the output voltage and low electromagnetic interference (EMI). This paper proposes a single-phase cascaded H-bridge quasi switched boost inverter (CHB-qSBI) for renewable energy sources applications. The proposed inverter has the advantage over the cascaded H-bridge quasi-Z-source inverter (CHB-qZSI) in reducing two capacitors and two inductors. As a result, cost, weight, and size are reduced. Furthermore, the dc-link voltage of each module is controlled by individual shoot-through duty cycle to get the same values. Therefore, the proposed inverter solves the imbalance problem of dc-link voltage in traditional CHB inverter. This paper shows the operating principles and analysis of the single-phase cascaded H-bridge quasi switched boost inverter. Also, a control strategy for the proposed inverter is shown. Experimental and simulation results are shown to verify the operating principle of the proposed inverter. Keywords: renewable energy sources, cascaded h-bridge inverter, quasi switched boost inverter, quasi z-source inverter, multilevel inverter

Procedia PDF Downloads 54364 Fire Resistance Capacity of Reinforced Concrete Member Strengthened by Fiber Reinforced Polymer
Authors: Soo-Yeon Seo, Jong-Wook Lim, Se-Ki Song
Abstract:
Currently, FRP (Fiber Reinforced Polymer) materials have been widely used for reinforcement of building structural members. However, since the FRP and the epoxy material for attaching it have very low resistance to heat, there is a problem in application where high temperature is an issue. In this paper, the resistance performance of FRP member made of carbon fiber at high temperature was investigated through experiment under temperature change. As a result, epoxy encapsulating FRP is damaged at not high temperatures, and the fibers are degraded. Therefore, when reinforcing a structure using FRP, a separate refractory heat treatment is necessary. The use of a 30 mm thick calcium silicate board as a fireproofing method can protect FRP up to 600ᵒC outside temperature. Keywords: FRP (Fiber Reinforced Polymer), high temperature, experiment under temperature change, calcium silicate board

Procedia PDF Downloads 73857 Preparation of Low-Molecular-Weight 6-Amino-6-Deoxychitosan (LM6A6DC) for Immobilization of Growth Factor
Authors: Koo-Yeon Kim, Eun-Hye Kim, Tae-Il Son
Abstract:
Epidermal Growth Factor (EGF, Mw=6,045) has been reported to have high efficiency of wound repair and anti-wrinkle effect. However, the half-life of EGF in the body is too short to exert the biological activity effectively when applied in free form. Growth Factors can be stabilized by immobilization with carbohydrates from thermal and proteolytic degradation. Low molecular weight chitosan (LMCS) and its derivate prepared by hydrogen peroxide has high solubility. LM6A6DC was successfully prepared as a reactive carbohydrate for the stabilization of EGF by the reactions of LMCS with alkalization, tosylation, azidation and reduction. The structure of LM6A6DC was confirmed by FT-IR, 1H NMR and elementary analysis. For enhancing the stability of free EGF, EGF was attached with LM6A6DC by using water-soluble carbodiimide. EGF-LM6A6DC conjugates did not show any cytotoxicity on the Normal Human Dermal Fibroblast(NHDF) 3T3 proliferation at least under 100 ㎍/㎖. In the result, it was considered that LM6A6DC is suitable to immobilize of growth factor. Keywords: epidermal growth factor (EGF), low-molecular-weight chitosan, immobilization

Procedia PDF Downloads 29054 A Study on the Relationship between Shear Strength and Surface Roughness of Lined Pipes by Cold Drawing
Authors: Mok-Tan Ahn, Joon-Hong Park, Yeon-Jong Jeong
Abstract:
Diffusion bonding has been continuously studied. Temperature and pressure are the most important factors to increase the strength between diffusion bonded interfaces. Diffusion bonding is an important factor affecting the bonding strength of the lined pipe. The increase of the diffusion bonding force results in a high formability clad pipe. However, in the case of drawing, it is difficult to obtain a high pressure between materials due to a relatively small reduction in cross-section, and it is difficult to prevent elongation or to tear of material in heat drawing even if the reduction in section is increased. In this paper, to increase the diffusion bonding force, we derive optimal temperature and pressure to suppress material stretching and realize precise thickness precision. Keywords: drawing speed, FEM (Finite Element Method), diffusion bonding, temperature, heat drawing, lined pipe

Authors: Yeon-Jong Jeong, Mok-Tan Ahn, Seok-Hyeon Park, Seong-Hun Ha, Joon-Hong Park, Jong-Bae Park
Abstract:
Multi-stage drawing process is an important technique for forming a shape that cannot be molded in a single process. multi-stage drawing process in number of passes and the shape of the die are an important factors influencing the productivity and formability of the product. The number and shape of the multi-path in the mold of the drawing process is very influencing the productivity and formability of the product. Half angle of the die and mandrel affects the drawing force and it also affects the completion of the final shape. Thus reducing the number of pass and the die shape optimization are necessary to improve the formability of the billet. Analyzing the load on the die through the FEM analysis and in consideration of the formability of the material presents a die model. Keywords: multi-pass shape drawing, equivalent stress, FEM, finite element method, optimum shape

Abstract:
The Ising model, consisting magnetic spins, is the simplest system showing phase transitions and critical phenomena at finite temperatures. The Ising model has played a central role in our understanding of phase transitions and critical phenomena. Also, the Ising model explains the gas-liquid phase transitions accurately. Authors: Sang-Hyeon Bae, Sung-Yeon Kim, Min-Gyo Jeong, Ji-Hong Kim, Wang-Sang Lee
Abstract:
Traditional heating methods such as electric ovens or steam heating are slow and not very efficient. For continuously heating the objects, a microwave conveyor-belt drier is widely used in the industrial microwave heating systems. However, there is a problem in which electromagnetic wave leaks toward outside of the heating cavity through the insertion opening. To achieve the prevention of the leakage of microwaves and improved heating characteristics, the corrugated rectangular waveguide at the entrance and exit openings of a microwave conveyor-belt drier is proposed and its electromagnetic interference (EMI) shielding effectiveness is analyzed and verified. The corrugated waveguides in the proposed microwave heating system achieve at least 20 dB shielding effectiveness while ensuring a sufficient height of the openings. Keywords: corrugated, electromagnetic wave, microwave conveyor-belt drier, rectangular waveguide, shielding effectiveness

Abstract:
Device-to-device (D2D) communication is considered a promising technique to provide wireless peer-to-peer communication services. Due to increasing demand on mobile services, available spectrum for radio frequency (RF) based communications becomes scarce. Recently, visible light communications (VLC) has evolved as a high speed wireless data transmission technology for indoor environments with abundant available bandwidth. In this paper, a novel VLC based D2D communication that provides wireless peer-to-peer communication is proposed. Potential low operating power devices for an efficient D2D communication over increasing distance of separation between devices is analyzed. Optical repeaters (OR) are also proposed to enhance the performance in an environment where direct D2D communications yield degraded performance. Simulation results show that VLC plays an important role in providing efficient D2D communication up to a distance of 1 m between devices. It is also found that the OR significantly improves the coverage distance up to 3.5 m.Keywords: visible light communication, light emitting diode, device-to-device, optical repeater

Authors: Seo Soo-Yeon, Kim Sang-Ku, Noh Sang-Hyun, Lee Ji-Eun, Kim Seol-Ki, Lim Jong-Wook
Abstract:
This paper presents a test result to find the structural capacity of Hollow-Precast Concrete (HPC) column with Head-Splice Sleeve (HSS) for the connection of bars under horizontal cyclic load. Two Half-scaled HPC column specimens were made with the consideration of construction process in site. The difference between the HPC specimens is the location of HSS for bar connection. The location of the first one is on the bottom slab or foundation while the other is above the bottom slab or foundation. Reinforced concrete (RC) column was also made for the comparison. In order to evaluate the hysteretic behavior of the specimens, horizontal cyclic load was applied to the top of specimen under constant axial load. From the test, it is confirmed that the HPC columns with HSS have enough structural capacity that can be emulated to RC column. This means that the HPC column with HSS can be used in the moment resisting frame system. Keywords: structural capacity, hollow-precast concrete column, head-splice sleeve, horizontal cyclic load
