2147 Study on Quality of Life among Patients Undergoing Hemodialysis in National Kidney Centre, Banasthali, Kathmandu

Abstract:
Health and well being of people is a crucial for accomplishing sustainable development goals of any country. The present study focuses on quality of life of patients undergoing hemodialysis. Hemodialysis is a life sustaining treatment for patients with end stage renal disease (ESRD). Hemodialysis can bring about significant impairment in health related quality of life (HRQOL). The purpose of this study was to assess the quality of life of hemodialysis patients undergoing hemodialysis. A descriptive cross-sectional research design was utilized in total 100 samples using random sampling technique. The findings revealed that the total quality of life of the patients was 30.41±3.99 out of 100. The total physical component score was statistically significant with education status of the patients where p value for t test was 0.03 (p=0.03) and occupation of the patients where p value for the ANOVA test was 0.007 (p=0.007). The study recommended that it would be better if awareness programs regarding chronic kidney disease and life style modification in hemodialysis patients is given to the patients so that it would help patients to maintain the HRQOL.Keywords: health and well bing, hemodialysis, patients quality of life

The field of advanced diagnostics is a very rapidly changing one. A new technology that has not been fully used yet are resistotrons. A resistotron is a physical device thatis used to detect the presence of low energy alpha particles. It has been used for many years in nuclear physics as an alpha particle detector. Since they are used in nuclear physics, they have to be accurate. They have to be able to differentiate between alpha particles and other types of radiation. The resistotrons are primarily used for safety. They are used in areas where people or animals can get exposed to radiation. A typical example is in the treatment of nuclear waste. As it is with any nuclear physics instrument, a resistotron has to be very accurate and reliable. In the past, the instrument was very expensive because they were made out of copper. This paper addresses a system for monitoring patients using biomedical sensors and displaying it in a remote place. The main challenges in present health monitoring devices are lack of remote monitoring and logging for future evaluation. Typical instruments used for health parameter measurement provide basic information regarding health status. This paper identifies a set of design principles to address these challenges. This system includes continuous measurement of health parameters such as Heart rate, electrocardiogram, SpO2 level and Body temperature. Abstract:
PDAM (local water company) determines customer charges by considering the customer's building or house. Charges determination significantly affects PDAM income and customer costs because the PDAM applies a subsidy policy for customers classified as small households. Periodic updates are needed so that pricing is in line with the target. A thorough customer survey in Surabaya is needed to update customer building data. However, the survey that has been carried out so far has been by deploying officers to conduct one-by-one surveys for each PDAM customer. Surveys with this method require a lot of effort and cost. For this reason, this research offers a technology called moblie mapping, a mapping method that is more efficient in terms of time and cost. The use of this tool is also quite simple, where the device will be installed in the car so that it can record the surrounding buildings while the car is running. Mobile mapping technology generally uses lidar sensors equipped with GNSS, but this technology requires high costs. In overcoming this problem, this research develops low-cost mobile mapping technology using a webcam camera sensor added to the GNSS and IMU sensors. The camera used has specifications of 3MP with a resolution of 720 and a diagonal field of view of 78⁰. The principle of this invention is to integrate four camera sensors, a GNSS webcam, and GPS to acquire photo data, which is equipped with location data (latitude, longitude) and IMU (roll, pitch, yaw). This device is also equipped with a tripod and a vacuum cleaner to attach to the car's roof so it doesn't fall off while running. The output data from this technology will be analyzed with artificial intelligence to reduce similar data (Cosine Similarity) and then classify building types. Data reduction is used to eliminate similar data and maintain the image that displays the complete house so that it can be processed for later classification of buildings. The AI method used is transfer learning by utilizing a trained model named VGG-16. From the analysis of similarity data, it was found that the data reduction reached 50%. Then georeferencing is done using the Google Maps API to get address information according to the coordinates in the data. After that, geographic join is done to link survey data with customer data already owned by PDAM Surya Sembada Surabaya.Keywords: mobile mapping, GNSS, IMU, similarity, classification

The purpose of this research is to examine the relationships among business collaboration effectiveness, modern management excellence, proactive operational management, and firm performance to bring competitive advantage to the firm. Furthermore, the population and sample selected are exporters on textile businesses in Thailand in total of 566 companies. The data were collected by questionnaire survey and sent direct to the directors or managerial managers of each company which is appropriate as the key informant of this research. Moreover, the statistic to test hypothesis uses the hierarchical multiple regression analysis and provides those five hypotheses to testing. Abstract:
Over the past two decades, migration has gained central importance in the global landscape. Europe hosts the largest number of migrants, totaling 92.9 million people, approximately 37.4 million of whom are regular residents within the European Union's borders. Reception services and different modes of management have received increasing attention precisely because of the complexity of the phenomenon, which necessarily impacts the wider community. Indeed, opening a reception center in an area entails major challenges for that context, for the community that inhabits it, and for the people who use that service. Questioning the strategies needed to offer a functional reception service means listening to the different actors involved who daily face the difficulties involved in working in the field. Recognizing the importance of the professional figures who work closely with migrant people, each with their own specific experiences has led researchers to study and analyze the different types of reception centers and their management. This has led to the development of intervention models and best practices in various countries. However, research from this perspective is still limited, especially in Italy. From this theoretical framework, this study aims to bring out an innovative qualitative tool, such as the world café, the work experiences of 29 social workers working in shelters in the Italian context. Most of the participants were female and lived in the Northwest regions of Italy. Through this tool, the aim was to bring out and share reflections on the critical issues encountered in working in reception centers, with a view to identifying possible solutions for better management of services. The World café represents a tool used in participatory action research that promotes dialogue among participants through the sharing of reflections and ideas. In fact, from critical reflections, participants are invited to identify and share possible solutions to provide a more functional service with benefits to the entire community. Therefore, this research, through the innovative technique of the World café, aims to promote critical thinking processes that can help participants find solutions that can be introduced into their work contexts or proposed to decision-makers. Specifically, the findings shed light on several issues, including complex bureaucratic procedures, insufficient project planning, and inefficiencies in the services provided to migrants. These concerns collectively contribute to what participants perceive as a disorganized and uncoordinated system. In addition, the study explores potential solutions that promote more efficient networking practices, coordinated project management, and a more positive approach to cultural diversity. This paper provides a literature review of the sources of women’s rights under the Saudi legal framework, taking account of the constitutional primacy of Sharia under the Saudi legal system as well as the state’s obligations under international law. Building on one of the central aims of the paper, it conducts an exploration of how Saudi Arabia already has or might be further able to more clearly delineate its position and reservations in the adoptions of international human rights agreements while preserving its core religious beliefs and societal practices in regard to women’s rights at the domestic level. In this regard, the paper will consider the apparent tension between certain jurisprudential and customary aspects on gender equality and contemporary discourses of women’s rights from within and outside the Muslim world. The vapour pressures of the three binary mixtures: 1, 2- dichloroethane + 1,3-dioxolane, + 1,4-dioxane or + tetrahydropyrane, are carried out at ten temperatures ranging from 273 to 353.15 K. An accurate static device was employed for these measurements. The VLE data were reduced using the Redlich-Kister equation by taking into consideration the vapour pressure non-ideality in terms of the second molar virial coefficient. Innovation and strategy discussions do highlight open innovation as a new paradigm in business. Yet, a number of stumbling blocks in the form of closed innovation principles weaved into the fabric of a traditional business model stand in the way of the new paradigm’s momentum to increase value in various business contexts. The paper argues that businesses considering an engagement with the open innovation paradigm would need to take steps to improve their multiplicative, absorptive and relational capabilities, respectively. The needed improvements would amount to a business model evolutionary transformation and eventually bring about a paradigm overhaul in business. The transformation is worth staging over time to ensure that open innovation is developed across interconnected and partnered areas of strategic importance. With the advancement of the economy, technology promotes online teaching, and learning has become one of the common features in the educational field. Meanwhile, the dramatic expansion of the online environment provides opportunities for more learners, including second language learners. A greater command of vocabulary improves students’ learning capacity, and word acquisition and development play a critical role in learning. Furthermore, the Word Wall is an effective tool to improve students’ knowledge of words, which works for a wide range of age groups. Therefore, this study is going to use the Word Wall as an intervention to examine whether it can bring some memorization changes in an online English language class for a second language learner based on the word morphology method. The participant will take ten courses in the experiment as it plans. The findings show that the Word Wall activity plays a slight role in improving word memorizing, but it does affect instant memorization. Abstract:
There are many difficulties in measuring a small flow rate of an order of milli grams per minute (LPM) or less using a conventional flowmeter. Therefore, a flow meter with minimal loss and based on a new concept was designed as part of this paper. A chamber was manufactured with a level transmitter and an on-off control valve. When the level of the collected condensed water reaches the top of the chamber, the valve opens to allow the collected water to drain back into the tank. To allow the water to continue to drain when the signal is lost, the valve is held open for a few seconds by a time delay switch and then closed. After an examination, the condensing flow rate was successfully measured with the uncertainty of ±5.7% of the full scale for the chamber.Keywords: chamber, condensation, flow meter, milli-grams

Abstract:
Traditional elastic metamaterials have difficulties in achieving independent tunable working frequency in two orthogonal directions. In this work, we proposed a pragmatic active elastic metamaterial to obtain extreme anisotropic stiffness with a tunable working frequency range. Piezoelectric patches shunted with variable conductance are properly proposed in the microstructure unit cell to manipulate the effective elastic stiffness along two principal directions at the subwavelength scale. Simulation of manipulation of wave propagation in such metamaterials is performed. An experimental study is also conducted to validate the design, and the results are in good agreement with mathematic analysis and numerical predictions. The proposed active elastic metamaterial will bring forth significant guidelines for ultrasonic imaging technique, and the results are expected to offer novel and general design methodology for elastic metamaterials.Keywords: microstructure, active elastic metamaterials, piezoelectric patches, experimental study

Abstract:
This study considers the possible effects of geomagnetic activity (GMA) on humans situated on Earth by performing experiments concerning specific sensitive reactions in humans in both: natural conditions during different GMA and by the simulation of different GMA in the lab. The measurements of autonomic nervous system (ANS) responses to different GMA via measuring the heart rate variability (HRV) indices and stress index (SI) and their comparison with the K-index of GMA have been presented and discussed. The results of experiments indicate an intensification of the sympathetic part of the ANS as a stress reaction of the human organism when it is exposed to high level of GMA as natural as well as in simulated conditions. Aim: We tested the hypothesis whether the GMF when disturbed can have effects on human ANS causing specific sensitive stress-reactions depending on the initial type of ANS. Methods: The study focuses on the effects of different GMA on ANS by comparing of HRV indices and stress index (SI) of n= 78, 18-24 years old healthy male volunteers. Experiments were performed as natural conditions on days of low (K= 1-3) and high (K= 5-7) GMA as well as in the lab by the simulation of different GMA using the device of geomagnetic storm (GMS) compensation and simulation. Results: In comparison with days of low GMA (K=1-3) the initial values of HRV shifted towards the intensification of the sympathetic part (SP) of the ANS during days of GMSs (K=5-7) with statistical significance p-values: HR (heart rate, p= 0.001), SDNN (Standard deviation of all Normal to Normal intervals, p= 0.0001), RMSSD (The square root of the arithmetical mean of the sum of the squares of differences between adjacent NN intervals, p= 0.0001). In comparison with conditions during GMSs compensation mode (K= 0, B= 0-5nT), the ANS balance was observed to shift during exposure to simulated GMSs with intensities in the range of natural GMSs (K= 7, B= 200nT). However, the initial values of the ANS resulted in different dynamics in its variation depending of GMA level. In the case of initial balanced regulation type (HR > 80) significant intensification of SP was observed with p-values: HR (p= 0.0001), SDNN (p= 0.047), RMSSD (p= 0.28), LF/HF (p=0.03), SI (p= 0.02); while in the case of initial parasympathetic regulation type (HR < 80), an insignificant shift to the intensification of the parasympathetic part (PP) was observed. Conclusions: The results indicate an intensification of SP as a stress reaction of the human organism when it is exposed to high level of GMA in both natural and simulated conditions.Keywords: autonomic nervous system, device of magneto compensation/simulation, geomagnetic storms, heart rate variability

Recently, Graphene Nanoribbon Field Effect Transistors (GNR FETs) attract a great deal of attention due to their better performance in comparison with conventional devices. In this paper, channel length Modulation (CLM) effect on the electrical characteristics of GNR FETs is analytically studied and modeled. To this end, the special distribution of the electric potential along the channel and current-voltage characteristic of the device is modeled. The obtained results of analytical model are compared to the experimental data of published works. In the present study, the dynamics of nanobeam resting on fractional order softening nonlinear viscoelastic pasternack foundations is studied. The Hamilton principle is used to derive the nonlinear equation of the motion. Approximate analytical solution is obtained by applying the standard averaging method. The Melnikov method is used to investigate the chaotic behaviors of device, the critical curve separating the chaotic and non-chaotic regions are found. Abstract:
The need for sustainable and reliable electricity supply in rural communities of Nigeria remains a pressing issue, given the country's vast energy deficit and the significant number of inhabitants lacking access to electricity. This research focuses on the development of a portable hybrid renewable energy system designed to provide a sustainable and efficient electricity supply to these underserved regions. The proposed system integrates multiple renewable energy sources, specifically solar and wind, to harness the abundant natural resources available in Nigeria. The design and development process involves the selection and optimization of components such as photovoltaic panels, wind turbines, energy storage units (batteries), and power management systems. These components are chosen based on their suitability for rural environments, cost-effectiveness, and ease of maintenance. The hybrid system is designed to be portable, allowing for easy transportation and deployment in remote locations with limited infrastructure. Key to the system's effectiveness is its hybrid nature, which ensures continuous power supply by compensating for the intermittent nature of individual renewable sources. Solar energy is harnessed during the day, while wind energy is captured whenever wind conditions are favourable, thus ensuring a more stable and reliable energy output. Energy storage units are critical in this setup, storing excess energy generated during peak production times and supplying power during periods of low renewable generation. These studies include assessing the solar irradiance, wind speed patterns, and energy consumption needs of rural communities. The simulation results inform the optimization of the system's design to maximize energy efficiency and reliability. This paper presents the development and evaluation of a 4 kW standalone hybrid system combining wind and solar power. The portable device measures approximately 8 feet 5 inches in width, 8 inches 4 inches in depth, and around 38 feet in height. It includes four solar panels with a capacity of 120 watts each, a 1.5 kW wind turbine, a solar charge controller, remote power storage, batteries, and battery control mechanisms. Designed to operate independently of the grid, this hybrid device offers versatility for use in highways and various other applications. It also presents a summary and characterization of the device, along with photovoltaic data collected in Nigeria during the month of April. The construction plan for the hybrid energy tower is outlined, which involves combining a vertical-axis wind turbine with solar panels to harness both wind and solar energy. Positioned between the roadway divider and automobiles, the tower takes advantage of the air velocity generated by passing vehicles. The solar panels are strategically mounted to deflect air toward the turbine while generating energy. Generators and gear systems attached to the turbine shaft enable power generation, offering a portable solution to energy challenges in Nigerian communities. The study also addresses the economic feasibility of the system, considering the initial investment costs, maintenance, and potential savings from reduced fossil fuel use. The production of logic devices faces the occurrence of faults during manufacturing. This work analyses the complexity of testing a special type of logic device on inverse, adhesion, and constant input faults. The focus of this work is on devices that implement cryptographic functions. The complexity values for the general case faults and for some frequently occurring subsets were determined and proved in this work. For a special case, when the length of the text block is equal to the length of the key block, the complexity of testing is proven to be asymptotically half the complexity of testing all logic devices on the same types of input faults.Keywords: complexity, cryptographic devices, input faults, testing

New design suggestions have been made on the basis of available guidelines and two case studies in Taiwan. Interior design can provide positive and sensory stimulation through memory stimulation, and by creating a friendly and comfortable environment for demented older people, which can reduce patient anxiety and reduce stress on caregivers. The paints are used extensively today in the industry to protect the metallic structures of the aggressive environments. This work is devoted to the study of corrosion resistance and aging behavior of a paint coating providing external protection for oil tankers. To avoid problems related to corrosion of these vessels, two protection modes are provided: An electro chemical active protection (cathodic protection of the hull). A passive protection by external painting. Investigations are conducted using stationary and non-stationary electro chemical tools such as electro chemical impedance spectroscopy has allowed us to characterize the protective qualities of these films. The application of the EIS on our damaged in-situ painting shows the existence of several capacitive loops which is an indicator of the failure of our tested paint. Abstract:
Soft switching has attracted the interest of various researchers constantly. Many techniques are in vogue to achieve soft switching (ZVS and/or ZCS) in Boost converters. These techniques utilize an auxiliary switch to incorporate the ZCS/ZVS. Such schemes require additional control circuit and induce complexity in design. This paper proposes an elegant fly back approach which guarantees zero current switching of the main Switch without the need for any additional active device. A simple flyback transformer scheme is implemented which absorbs the initial turn ON energy (or the Reverse recovery energy of Boost diode) and delivers to the output.Keywords: boost converter, complete energy transfer, flyback, zero current switching

Abstract:
Pilot stages are one of the most common positioners and regulators in industry. In this paper, we present two novel concepts for pilot stages with low power consumption to regulate a pneumatic device. Pilot 1, first concept, is designed based on a conventional frame core electro-magnetic actuator and a leaf spring to control the air flow and pilot 2 has an axisymmetric actuator and spring made of non-oriented electrical steel. Concepts are simulated in a system modeling tool to study their dynamic behavior. Both concepts are prototyped and tested. Experimental results are comprehensively analyzed and compared. Abstract:
Global oxygen transfer coefficient (kLa) was characterized in a mechanically agitated airlift bio reactor. The experiments were carried out in an airlift bio reactor (3.2 L) with internal re circulation (a concentric draft-tube airlift vessel device); the agitation is carried out through a turbine Rushton impeller located along with the gas sparger in the region comprised in the riser. The experiments were conducted using xanthan gum (0.6%) at 250 C and a constant rotation velocity of 0 and 800 rpm, as well as in the absence of agitation (airlift mode); the superficial gas velocity varied from 0.0157 to 0.0262 ms-1. The volumetric oxygen transfer coefficient dependence of the rotational speed revealed that the presence of agitation increased up to two times the kLa value.Keywords: aeration, mass transfer, non-Newtonian fluids, stirred airlift bioreactor

Abstract:
In this paper, we formulate a general equilibrium theory that explains the existence and stability of democratically elected governments that support certain groups of individuals in society (insiders) to the detriment of everybody else (outsiders), even if the latter constitute a majority. The vehicle is a dynamic general equilibrium model, where insiders get monopoly rents and outsiders get less than what they would have gotten under a common good regime. We construct such political economy equilibria, and we identify the conditions under which such political regimes (coalitions of insiders): (a) can safeguard against opportunistic behavior (i.e., do not fall from within) and (b) may come to power in the first place (i.e., manage to get elected). This article reviews social science understanding to explore the challenges megaprojects face before and after implementation. It also sheds light on the problems directly and indirectly caused by mega development projects in the project implemented areas. By Using a qualitative approach such as thematic analysis, the article uses recent literature such as published articles, government reports, and books to cite examples of different mega projects worldwide. The study report that mega development projects are a necessary element of the modern-day infrastructural development process as they represent the perfect example of urban socioeconomic development. They are introduced and implemented by multinational companies with the support of state authorities to produce the common good. However, they are not devoid of their critical challenges and bring implicit and explicit problems to the targeted localities. The article takes insights from social science research for suggestions on how to reduce the challenges faced by project implementers and problems received by local people due to the fault lines of such projects.Keywords: development, mega-projects, challenges, problems

Abstract:
This experimental study is aimed to examine the radiation characteristics of different plasma structures of a surface wave-driven plasma antenna by an automated measuring system. In this study, a 30 cm long plasma column of argon gas with a diameter of 3 cm is excited by surface wave discharge mechanism operating at 13.56 MHz with RF power level up to 100 Watts and gas pressure between 0.01 to 0.05 mb. The study reveals that a single structured plasma monopole can be modified into an array of plasma antenna elements by forming multiple striations or plasma blobs inside the discharge tube by altering the values of plasma properties such as working pressure, operating frequency, input RF power, discharge tube dimensions, i.e., length, radius, and thickness. It is also reported that plasma length, electron density, and conductivity are functions of operating plasma parameters and controlled by changing working pressure and input power. To investigate the antenna radiation efficiency for the far-field region, an automation-based radiation measuring system has been fabricated and presented in detail. This developed automated system involves a combined setup of controller, dc servo motors, vector network analyzer, and computing device to evaluate the radiation intensity, directivity, gain and efficiency of plasma antenna. In this system, the controller is connected to multiple motors for moving aluminum shafts in both elevation and azimuthal plane whereas radiation from plasma monopole antenna is measured by a Vector Network Analyser (VNA) which is further wired up with the computing device to display radiations in polar plot forms. Here, the radiation characteristics of both continuous and array plasma monopole antenna have been studied for various working plasma parameters. The experimental results clearly indicate that the plasma antenna is as efficient as a metallic antenna. The radiation from plasma monopole antenna is significantly influenced by plasma properties which provides a wider range in radiation pattern where desired radiation parameters like beam-width, the direction of radiation, radiation intensity, antenna efficiency, etc. can be achieved in a single monopole. Due to its wide range of selectivity in radiation pattern; this can meet the demands of wider bandwidth to get high data speed in communication systems. The imaging phantom is utilized for the verification, evaluation and tuning of the medical imaging device and system. Although it could be costly, 3D printing is an ideal technique for a rapid, customized, multi-modal phantom making. In this article, we propose the multi-modal phantom using 3D printing. First of all, the Dicom images for were measured by CT (Computed Tomography) and 3D X-ray systems (PET/CT and Angio X-ray system of Siemens) and then were analyzed. Finally, the 3D modeling was processed using Dicom images. Abstract:
The present study was conducted to investigate the relationship between career anchors and job satisfaction with emphasis on the mediating role of person-job fit. 502 managers and supervisors of ten operational areas of a large energy Company were selected as a cluster sample appropriate to the volume. The instruments used in this study were Career Anchor Questionnaire, Job Satisfaction Questionnaire and Person-job fit Questionnaire. Pearson correlation coefficient was used to analyze the data and AMOS software was used to determine the effect of career anchor variables and person-job fit on job satisfaction. Anchors of service and dedication, pure challenge and security and stability increase the person-job fit among managers and also the person-job fit plays a mediating role in relation to the effect it has on job satisfaction through these anchors. In contrast, the anchors of independence and autonomy reduce the person-job fit. Considering the importance of positive organizational attitudes and in order to have an optimal fit between job and worker, it is better that in human resources processes such as hiring and employing, the career anchors of the person should be considered so that the person can have more job satisfaction; and thus bring higher productivity for themselves and the organization.Keywords: career anchor, job satisfaction, person-job fit, energy company, managers

We investigated the dynamic properties of graphene-nanoribbon (GNR) memory encapsulating graphene-nanoflake (GNF) shuttle in the potential to be applicable as a non-volatile random access memory via molecular dynamics simulations. This work explicitly demonstrates that the GNR encapsulating the GNF shuttle can be applied to nonvolatile memory. The potential well was originated by the increase of the attractive vdW energy between the GNRs when the GNF approached the edges of the GNRs. So the bistable positions were located near the edges of the GNRs. Such a nanoelectromechanical non-volatile memory based on graphene is also applicable to the development of switches, sensors, and quantum computing.Keywords: graphene nanoribbon, graphene nanoflake, shuttle memory, molecular dynamics
