Procedia PDF Downloads 379214 Xen45 Gel Implant in Open Angle Glaucoma: Efficacy, Safety and Predictors of Outcome
Authors: Fossarello Maurizio, Mattana Giorgio, Tatti Filippo.
Abstract:
The most widely performed surgical procedure in Open-Angle Glaucoma (OAG) is trabeculectomy. Although this filtering procedure is extremely effective, surgical failure and postoperative complications are reported. Due to the its invasive nature and possible complications, trabeculectomy is usually reserved, in practice, for patients who are refractory to medical and laser therapy. Recently, a number of micro-invasive surgical techniques (MIGS: Micro-Invasive Glaucoma Surgery), have been introduced in clinical practice. They meet the criteria of micro-incisional approach, minimal tissue damage, short surgical time, reliable IOP reduction, extremely high safety profile and rapid post-operative recovery. Xen45 Gel Implant (Allergan, Dublin, Ireland) is one of the MIGS alternatives, and consists in a porcine gelatin tube designed to create an aqueous flow from the anterior chamber to the subconjunctival space, bypassing the resistance of the trabecular meshwork. In this study we report the results of this technique as a favorable option in the treatment of OAG for its benefits in term of efficacy and safety, either alone or in combination with cataract surgery. This is a retrospective, single-center study conducted in consecutive OAG patients, who underwent Xen45 Gel Stent implantation alone or in combination with phacoemulsification, from October 2018 to June 2019. The primary endpoint of the study was to evaluate the reduction of both IOP and number of antiglaucoma medications at 12 months. The secondary endpoint was to correlate filtering bleb morphology evaluated by means of anterior segment OCT with efficacy in IOP lowering and eventual further procedures requirement. Data were recorded on Microsoft Excel and study analysis was performed using Microsoft Excel and SPSS (IBM). Mean values with standard deviations were calculated for IOPs and number of antiglaucoma medications at all points. Kolmogorov-Smirnov test showed that IOP followed a normal distribution at all time, therefore the paired Student’s T test was used to compare baseline and postoperative mean IOP. Correlation between postoperative Day 1 IOP and Month 12 IOP was evaluated using Pearson coefficient. Thirty-six eyes of 36 patients were evaluated. As compared to baseline, mean IOP and the mean number of antiglaucoma medications significantly decreased from 27,33 ± 7,67 mmHg to 16,3 ± 2,89 mmHg (38,8% reduction) and from 2,64 ± 1,39 to 0,42 ± 0,8 (84% reduction), respectively, at 12 months after surgery (both p < 0,001). According to bleb morphology, eyes were divided in uniform group (n=8, 22,2%), subconjunctival separation group (n=5, 13,9%), microcystic multiform group (n=9, 25%) and multiple internal layer group (n=14, 38,9%). Comparing to baseline, there was no significative difference in IOP between the 4 groups at month 12 follow-up visit. Adverse events included bleb function decrease (n=14, 38,9%), hypotony (n=8, 22,2%) and choroidal detachment (n=2, 5,6%). All eyes presenting bleb flattening underwent needling and MMC injection. The higher percentage of patients that required secondary needling was in the uniform group (75%), with a significant difference between the groups (p=0,03). Xen45 gel stent, either alone or in combination with phacoemulsification, provided a significant lowering in both IOP and medical antiglaucoma treatment and an elevated safety profile. Keywords: anterior segment OCT, bleb morphology, micro-invasive glaucoma surgery, open angle glaucoma, Xen45 gel implant

Authors: T. Wanyama, B. Far
Abstract:
Automatic irrigation systems usually conveniently protect landscape investment. While conventional irrigation systems are known to be inefficient, automated ones have the potential to optimize water usage. In fact, there is a new generation of irrigation systems that are smart in the sense that they monitor the weather, soil conditions, evaporation and plant water use, and automatically adjust the irrigation schedule. In this paper, we present an agent based smart irrigation system. The agents are built using a mix of commercial off the shelf software, including MATLAB, Microsoft Excel and KEPServer Ex5 OPC server, and custom written code. The Irrigation Scheduler Agent uses fuzzy logic to integrate the information that affect the irrigation schedule. In addition, the Multi-Agent system uses Open Platform Connectivity (OPC) technology to share data. OPC technology enables the Irrigation Scheduler Agent to communicate over the Internet, making the system scalable to a municipal or regional agent based water monitoring, management, and optimization system. Finally, this paper presents simulation and pilot installation test result that show the operational effectiveness of our system. Keywords: community water usage, fuzzy logic, irrigation, multi-agent system

Authors: Christine Odinga, Fred Otieno, Josiah Adeyemo
Abstract:
A study was done to establish the effectiveness of wetland plants: Echinocloa pyramidalis (L) and Cyperus papyrus (L) in purifying wastewater from sugar factory stabilization pond effluent. A pilot-scale Free Water Surface Wetland (FWSCW) system was constructed in Chemelil sugar factory, Kenya for the study. The wetland was divided into 8 sections (cells) and planted with C. papyrus and E. pyramidalis in alternating sequence. Water samples and plant specimen were taken fortnightly at inlets and outlets of the cells and analysed for total phosphates and total nitrates. The data was analysed by use of Microsoft excel and SPSS computer packages. Water analysis recorded a reduction in the nutrient levels between the inlet pond nine and the final outlet channel to River Nyando. The plants grown in the wetland experienced varied increases and reductions in the level of total foliar nitrogen and phosphorous, indicating that though the nutrients were being removed from the wetland, the same were not those assimilated by the plants either. The control plants had higher folia phosphorous and nitrogen, an indication that the system of the constructed wetland was able to eliminate the nutrients effectively from the plants. Keywords: wetlands, constructed, plants, nutrients, wastewater, industrial
Procedia PDF Downloads 300210 Network Based Molecular Profiling of Intracranial Ependymoma over Spinal Ependymoma
Authors: Hyeon Su Kim, Sungjin Park, Hae Ryung Chang, Hae Rim Jung, Young Zoo Ahn, Yon Hui Kim, Seungyoon Nam
Abstract:
Ependymoma, one of the most common parenchymal spinal cord tumor, represents 3-6% of all CNS tumor. Especially intracranial ependymomas, which are more frequent in childhood, have a more poor prognosis and more malignant than spinal ependymomas. Although there are growing needs to understand pathogenesis, detailed molecular understanding of pathogenesis remains to be explored. A cancer cell is composed of complex signaling pathway networks, and identifying interaction between genes and/or proteins are crucial for understanding these pathways. Therefore, we explored each ependymoma in terms of differential expressed genes and signaling networks. We used Microsoft Excel™ to manipulate microarray data gathered from NCBI’s GEO Database. To analyze and visualize signaling network, we used web-based PATHOME algorithm and Cytoscape. We show HOX family and NEFL are down-regulated but SCL family is up-regulated in cerebrum and posterior fossa cancers over a spinal cancer, and JAK/STAT signaling pathway and Chemokine signaling pathway are significantly different in the both intracranial ependymoma comparing to spinal ependymoma. We are considering there may be an age-dependent mechanism under different histological pathogenesis. We annotated mutation data of each gene subsequently in order to find potential target genes. Keywords: systems biology, ependymoma, deg, network analysis
Procedia PDF Downloads 298209 Spatial Orientation of Land Use Activities along Buffalo River Estuary: A Study in Buffalo City Metropolitan Municipality, Eastern Cape South Africa
Authors: A. Ngunga, M. K. Soviti, S. Nakin
Abstract:
South Africa is one of the developing countries rich in estuary ecosystem. Previous studies have identified many impacts of land use activities on the pollution status of the estuaries. These land use activity and related practices are often blamed for the many pollution problems affecting the estuaries. For example, the estuarine ecosystems on a global scale are experiencing vast transformations from anthropogenic influences; Buffalo River Estuary is one of the influenced estuaries whereby the sources of pollution are unknown. These problems consequently lead to the degradation of the estuaries. The aim of the research was to establish the factors that have the potential to impact pollution status of Buffalo river estuary. Study focuses on Identifying and mapping land use activities along Buffalo River Estuary. Questionnaire survey, structured interviews, direct observation, GPS survey and ArcGIS mapping were the methods used for data collection in the area, and results were analyzed and presented by ANOVA and Microsoft Excel statistical methods. The results showed that harbour is the main source of pollution, in Buffalo River Estuary, through Ballast water discharge. Therefore that requires more concern for protecting and cleaning the estuary. Keywords: estuary, land-use activities, pollution, mapping, water pollution

Procedia PDF Downloads 396207 Nilsson Model Performance in Estimating Bed Load Sediment, Case Study: Tale Zang Station
Authors: Nader Parsazadeh
Abstract:
The variety of bed sediment load relationships, insufficient information and data, and the influence of river conditions make the selection of an optimum relationship for a given river extremely difficult. Hence, in order to select the best formulae, the bed load equations should be evaluated. The affecting factors need to be scrutinized, and equations should be verified. Also, re-evaluation may be needed. In this research, sediment bed load of Dez Dam at Tal-e Zang Station has been studied. After reviewing the available references, the most common formulae were selected that included Meir-Peter and Muller, using MS Excel to compute and evaluate data. Then, 52 series of already measured data at the station were re-measured, and the sediment bed load was determined. 1. The calculated bed load obtained by different equations showed a great difference with that of measured data. 2. r difference ratio from 0.5 to 2.00 was 0% for all equations except for Nilsson and Shields equations while it was 61.5 and 59.6% for Nilsson and Shields equations, respectively. 3. By reviewing results and discarding probably erroneous measured data measurements (by human or machine), one may use Nilsson Equation due to its r value higher than 1 as an effective equation for estimating bed load at Tal-e Zang Station in order to predict activities that depend upon bed sediment load estimate to be determined. Also, since only few studies have been conducted so far, these results may be of assistance to the operators and consulting companies. Keywords: bed load, empirical relation ship, sediment, Tale Zang Station

Procedia PDF Downloads 74205 Converting Scheduling Time into Calendar Date Considering Non-Interruptible Construction Tasks
Authors: Salman Ali Nisar, Suzuki Koji
Abstract:
In this paper we developed a new algorithm to convert the project scheduling time into calendar date in order to handle non-interruptible activities not to be split by non-working days (such as weekend and holidays). In a construction project some activities might require not to be interrupted even on non-working days, or to be finished on the end day of business days. For example, concrete placing work might be required to be completed by the end day of weekdays i.e. Friday, and curing in the weekend. This research provides an algorithm that imposes time constraint for start and finish times of non-interruptible activities. The algorithm converts working days, which is obtained by Critical Path Method (CPM), to calendar date with consideration of the start date of a project. After determining the interruption by non-working days, the start time of a certain activity should be postponed, if there is enough total float value. Otherwise, the duration is shortened by hiring additional resources capacity or/and using overtime work execution. Then, time constraints are imposed to start time and finish time of the activity. The algorithm is developed in Excel Spreadsheet for microcomputer and therefore we can easily get a feasible, calendared construction schedule for such a construction project with some non-interruptible activities. Keywords: project management, scheduling, critical path method, time constraint, non-interruptible tasks

Authors: Ezeh Chukwunonso Peter Excel, Akruti Vg
Abstract:
Aim/Background: To show that even after undergoing 1-5 courses of chemotherapy for Gestational Trophoblastic Disease (GTD) reproductive health of women is intact and they conceive successfully after it. Method: Retrospective cohort analysis using data from the Lugansk regional maternity hospital database of years 1993-2013, which shows n=18 females had GTD and underwent 1-5 courses of chemotherapy. Results and Discussion: Frequency of GTD was rare. All 18 patients (pts) belong to age group of 17-39 years, covering wide range of reproductive age. Out of 18 pts, 15 had hydatidiform mole (HM) while other 3 had choriocarcinoma (CC). In anamnesis, among CC pts, 1 had early pre-eclampsia at 24 weeks and 1 had 4th week of late postpartum (PP) bleeding, while all HM pts had genital inflammatory diseases, 1 pt of HM during follow-up had High hCG and 3 times curettage in 5 months. 18 women became pregnant for 25 times after chemotherapy. Chemotherapy was given under indication of either high level of HCG, luteal cyst >6cm or path-morphological results of curettage. CC 3 pts had (2 spontaneous abortions (SA), 2 term cesarean section (CS), 1 preterm CS). HM 15 pts had (3 artificial abortion, 2 SA, 7CS (5 term and 2 preterm), 8 vaginal deliveries (7 term and 1 preterm)). Conclusion: During our research we got 22.2% preterm deliveries and 55.6% CS which is higher than the normal cases, but still all the 18 women were able to have kids successfully after chemotherapy. Authors: Ahmed Hamzi, Bryan Norman
Abstract:
Having scribes help document patient records in electronic health record systems can improve hospitalists’ productivity. But hospitals need to determine the optimum number of scribes to hire to maximize scribe cost effectiveness. Scribe attendance uncertainty due to planned and unplanned absences is a primary challenge. This paper presents simulation and analytical models to determine the optimum number of scribes for a hospital to hire. Scribe staffing practices vary from one context to another; different staffing scenarios are considered where having extra attending scribes provides or does not provide additional value and utilizing on-call scribes to fill in for potentially absent scribes. These staffing scenarios are assessed for different scribe revenue ratios (ratio of the value of the scribe relative to scribe costs) ranging from 100% to 300%. The optimum solution depends on the absenteeism rate, revenue ratio, and desired service level. The analytical model obtains solutions easier and faster than the simulation model, but the simulation model is more accurate. Therefore, the analytical model’s solutions are compared with the simulation model’s solutions regarding both the number of scribes hired and cost-effectiveness. Additionally, an Excel tool has been developed to facilitate decision-makers in easily obtaining solutions using the analytical model. Keywords: hospitalists, workload, optimization cost, economic analysis

Authors: Hamed Zolfaghari, Mojtaba Kord
Abstract:
Project planners and controllers are frequently faced with the challenge of inadequate software for the preparation of automatic project progress reports based on actual project information updates. They usually make dashboards in Microsoft Excel, which is local and not applicable online. Another shortcoming is that it is not linked to planning software such as Microsoft Project, which lacks the database required for data storage. This study aimed to propose a model for the preparation of reports on automatic online project progress based on actual project information updates by the integration of Primavera P6, SQL database, and Power BI for a construction project. The designed model could be applicable to project planners and controller agents by enabling them to prepare project reports automatically and immediately after updating the project schedule using actual information. To develop the model, the data were entered into P6, and the information was stored on the SQL database. The proposed model could prepare a wide range of reports, such as earned value management, HR reports, and financial, physical, and risk reports automatically on the Power BI application. Furthermore, the reports could be published and shared online. Keywords: primavera P6, SQL, Power BI, EVM, integration management

Authors: Masoud Abbasalipour
Abstract:
In this study, the impact of family status on individuals, involving factors such as parents' literacy level, family size, individuals' blood group, and susceptibility to addiction, was investigated. Statistical tests were employed to scrutinize the relationships among these specified factors. The statistical population of the study consisted of 338 samples divided into two groups: individuals with addiction and those without addiction in the city of Amol. The addicted group was selected from individuals visiting the substance abuse treatment center in Amol, and the non-addicted group was randomly selected from individuals in urban and rural areas. The Chi-square test was used to examine the presence or absence of relationships among the variables, and Kramer's V test was employed to determine the strength of the relationship between them. Excel software facilitated the initial entry of data, and SPSS software was utilized for the desired statistical tests. The research results indicated a significant relationship between the variable of parents' education level and individuals' addiction. The analysis showed that the education level of their parents was significantly lower compared to non-addicted individuals. However, the variables of the number of family members and blood group did not significantly impact individuals' susceptibility to addiction. Keywords: addiction, blood group, parents' literacy level, family status

Authors: Awadhesh Kishore, Tushar Sharma
Abstract:
The goal of this study was to compare the free radical-scavenging (FRS) characteristics of four Indian moringa (Moringa oleifera) plant components: flowers, tender and mature leaves, and seeds that were collected from three Indian districts: Jaipur, Dehra Dun, and Gwalior; in every month of 2021–2022. The samples were collected from three randomly selected agroforest locations from each district. The samples were extracted, and antioxidant properties were determined following the DPPH method with minor modifications. The FRS properties were calculated as the non-absorbance values of the sample in percentage. The factorial ANOVA statistical analysis technique was implemented for comparing FRS properties, and an MS Office Excel 2016 analysis pack was used to compare data. The flowers from Dehra Dun had superior FRS properties (27.06±1.03%), while the seeds from the same location were inferior (8.64±0.17%). The FRS properties of flowers (26.27±0.61%) were not statistically different (P > 0.05) compared to those of tender (27.30±0.63%) and mature leaves (28.37±0.59%), but significantly higher (P < 0.05) than those of seeds (9.31±0.16%). However, the FRS properties in Indian moringa were significantly higher during the winter (Jan 28.67±1.48%) compared to that in the summer (Jun 14.03±0.79%) season, but collected from three locations, viz. Gwalior (22.35±0.70%), Jaipur (23.06±0.73%), and Dehra Dun (23.10±0.76%), were not significantly different (P > 0.05). Based on this study, it can be concluded that the FRS value of flowers during the winter season is superior. Keywords: flowers, free radical-scavenging, leaves, moringa oleifera, seeds
