398 Understanding ASPECTS of Stroke: Interrater Reliability between Emergency Medicine Physician and Radiologist in a Rural Setup

Abstract:
Aims and Objectives: To evaluate the interrater reliability in grading ASPECTS score, between emergency medicine physician at first contact and radiologist among patients with acute ischemic stroke. Materials and Methods: We conducted a retrospective analysis of 86 acute ischemic stroke cases referred to the Department of Radiodiagnosis during November 2014 to January 2016. The imaging (plain CT scan) was performed using GE Bright Speed Elite 16 Slice CT Scanner. ASPECTS score was calculated separately by an emergency medicine physician and radiologist. Interrater reliability for total and dichotomized ASPECTS (≥ 6 and < 6) scores were assessed using statistical analysis (ICC and Cohen ĸ coefficients) on SPSS software (v17.0). Results: Interrater agreement for total and dichotomized ASPECTS was substantial (ICC 0.79 and Cohen ĸ 0.68) between the emergency physician and radiologist. Mean difference in ASPECTS between the two readers was only 0.15 with standard deviation of 1.58. No proportionality bias was detected. Bland Altman plot was constructed to demonstrate the distribution of ASPECT differences between the two readers. Conclusion: Substantial interrater agreement was noted in grading ASPECTS between emergency medicine physician at first contact and radiologist thereby confirming its robustness even in a rural setting.Keywords: ASPECTS, computed tomography, MCA territory, stroke

The findings suggest a significant association between maxillary sinus dimensions and human gender. Abstract:
The 21th century has already witnessed the rapid globalization of catastrophes caused by layered political, social, religious, cultural, and environmental conflicts. The post 9/11 literature that reflects these characteristics retells the experiences of those who are, whether directly or indirectly, involved in the globalized catastrophes of enlarging and endangering their boundaries and consequences. With an Irish-Turkish origin, a Dutch and British educational background, and as an American green-card holder, Joseph O’Neill challenges this changing circumstances of the expanding crisis. In his controversial novel, Netherland (2008), O’Neill embodies the deeply-rooted compromises, the transplanted conflicts, and human internalized crisis in post 9/11 New York City. O’Neill presents to us the transition between Netherland to New York with a post-colonial perspective. This internalized conflicts are revised in The Dog (2014) in which a newly-constructing and expanding global city of gold, Dubai, represents the transitional location from New York City. Abstract:
Sudden unexpected death in epilepsy (SUDEP) is a rarity. Each year, about one in 150 epileptics, whose seizures are not controlled, may die of SUDEP. It is a leading cause of death in young adults with uncontrolled seizures. Understanding the genetic basis for SUDEP, is crucial given that the rate of sudden death in epilepsy patients is 20 fold that of the general population. We encountered one such case of a young male, a known epileptic, who was brought dead after a sudden collapse. Abstract:
This paper presents metallurgical investigation of failed helical gear of diesel engine gear box used in a car. The failure had occurred near the bottomland of the tooth spacing. The failed surface was studied under Scanning Electron Microscope (SEM) and also visually investigated. The images produced through SEM at various magnifications were studied. Detailed metallurgical study indicates that failure was due to foreign material inclusion which is a casting defect. This paper presents the design analysis of saddle support of a horizontal pressure vessel. Since saddle have the vital role to support the pressure vessel and to maintain its stability, it should be designed in such a way that it can afford the vessel load and internal pressure of the vessel due to liquid contained in the vessel. A model of horizontal pressure vessel and saddle support is created in Ansys. Stresses are calculated using mathematical approach and Ansys software. The analysis reveals the zone of high localized stress at the junction part of the pressure vessel and saddle support due to operating conditions. In this study, the potential benefits of playing action video game among congenitally deaf and dumb subjects is reported in terms of EEG ratio indices. The frontal and occipital lobes are associated with development of motor skills, cognition, and visual information processing and color recognition. The sixteen hours of First-Person shooter action video game play resulted in the increase of the ratios β/(α+θ) and β/θ in frontal and occipital lobes. Lead free magnetoelectric (ME) nanoparticulate (1−x) BNT-BKT-BMgT−x CFO (x = 0, 0.1, 0.2, 0.3) composite films were synthesized using chemical solution deposition method. The X-ray diffraction and transmission electron microscope (TEM) reveal that CFO nanoparticles were well distributed in the matrix of BNT-BKT-BMgT. The nanocomposite films exhibit both good magnetic and ferroelectric properties at room temperature (R-T). It is concluded that the modulation in compositions of piezomagnetic/piezoelectric components plays a fundamental role in the magnetoelectric coupling in these nanoparticulate composite films. Abstract:
The Burmese Exodus of 1942, which left more than 4 lakh as refugees and thousands dead, is one of the worst forced migrations in recorded history. Adding to the woes of the refugees is the lack of credible documentation of their lived experiences, trauma, and stories and their erasure from recorded history. Media reports, national records, and mainstream narratives that have registered the exodus provide sanitized versions which have reduced the refugees to a nameless, faceless mass of travelers and obliterated their lived experiences, trauma, and sufferings. This attitudinal problem compels the need to stem the insensitivity that accompanies institutional memory by making a case for a more humanistically evolved policy that puts in place protocols for the way the humanities would voice the concern for the refugee. A definite step in this direction and a far more relevant project in our times is the need to build a comprehensive refugee archive that can be a repository of the refugee experiences and perspectives. The paper draws on Hannah Arendt’s position on the Jewish refugee crisis, Agamben’s work on statelessness and citizenship, Foucault’s notion of governmentality and biopolitics, Edward Said’s concepts on Exile, Fanon’s work on the dispossessed, Derrida’s work on ‘the foreigner and hospitality’ in order to conceptualize the refugee condition which will form the theoretical framework for the paper. It also refers to the existing scholarship in the field of refugee studies such as Roger Zetter’s work on the ‘refugee label’, Philip Marfleet’s work on ‘refugees and history’, Lisa Malkki’s research on the anthropological discourse of the refugee and refugee studies. The paper is also informed by the work that has been done by the international organizations to address the refugee crisis. The emphasis is on building a strong argument for the establishment of the refugee archive that finds but a passing and a none too convincing reference in refugee studies in order to enable a multi-dimensional understanding of the refugee crisis. Some of the old questions cannot be dismissed as outdated as the continuing travails of the refugees in different parts of the world only remind us that they are still, largely, unanswered. The questions are -What is the nature of a Refugee Archive? How is it different from the existing historical and political archives? What are the implications of the refugee archive? What is its contribution to refugee studies? The paper draws on Diana Taylor’s concept of the archive and the repertoire to theorize the refugee archive as a repository that has the documentary function of the ‘archive’ and the ‘agency’ function of the repertoire. It then reads Ayya’s Accounts- a memoir by Anand Pandian -in the light of Hannah Arendt’s concepts of the ‘refugee as vanguard’ and ‘story telling as political action’- to illustrate how the memoir contributes to the refugee archive that provides the refugee a place and agency in history. Abstract:
Creep analysis of a thick-walled composite anisotropic cylinder under internal pressure and considering large strains is presented. Using a threshold creep law for composite materials, expressions for stresses, strains, and strain rates are derived for several anisotropic cases. Numerical results, presented through several graphs and tables, depict the effect of anisotropy on the stress, strain, and strain rate distributions. Since for a specific type of material anisotropy described in the paper, these quantities are found to have the lowest values at the inner radius (the potential location of cylinder failure), it is concluded that by employing such an anisotropic material for the design of a thick-walled cylinder a longer service life for the cylinder may be achieved.Keywords: creep, composites, large strains, thick-walled cylinders, anisotropy

The trench gate MOSFET has shown itself as the most appropriate power device for low to medium voltage power applications due to its lowest possible ON resistance among all power semiconductor devices. In this research work a double-epilayer PSGT structure using a thin layer of N+ polysilicon as gate material. The total ON-state resistance (RON) of UMOSFET can be reduced by optimizing the epilayer thickness. Many times, due to mutilation of body parts, the normal methods of identification using skeletal remains cannot be used in the process of identification. In such cases, the intact components of the skeletal remains or bony parts play an important role in identification. In these situations, digital forensics can come to our rescue. The design load is reached for the spiral casing and stay ring during the emergency closure of the guide apparatus "LC1 Quick Shut Down”. In the present study, the effect of gravity modulation on the onset of convection in viscous fluid layer under the influence of induced magnetic field, salted from above on the boundaries, has been investigated. Linear and nonlinear stability analysis has been performed. A linear stability analysis is performed to show that the gravity modulation can significantly affect the stability limits of the system. A method based on small amplitude of the modulation is used to compute the critical value of Rayleigh number and wave number. Pollution has been a major problem that has haunted India for years. Large amounts of industrial, automobile and domestic waste have resulted in heavy contamination of air, land and water. The Taj Mahal, one of the Seven Wonders of the World, has been and continues to be India’s symbol of a rich history around the globe. Over the years, the beauty of Taj Mahal has also suffered from increasing pollution. Its shiny white exterior has started to turn yellow because of air pollution and acid rain. Illegal factories and uncontrolled construction have played a major role in worsening its condition. Rapid population growth in the city (Agra) meant more water requirement which has led to ground water deterioration under the historical monument making its wooden foundations dry and weak. Despite various measures by the state and central government, there hasn’t been any satisfactory result. Abstract:
This paper addresses the junction stress analysis of orthogonally intersecting thin walled cylindrical shell and thin walled cylindrical nozzle subjected to external loading on nozzle. Junction stresses have been calculated theoretically by welding research council (WRC) bulletins 107 and 297 for different nozzle loads. WRC bulletins 107 and 297 have been used by design engineers for calculating nozzle-vessel junction stresses since their publication. They give simple empirical relations and easy in application. Also 3D FEA in which material is elastic has been done in ANSYS software with 8 node solid element model and results of FEA have been compared with WRC results. Stress intensities obtained by WRC 297 are generally slightly higher than obtained by WRC 107. Membrane stresses obtained by FEA are much higher than WRC and membrane plus bending stresses obtained by FEA are lower than WRC.Keywords: FEA, junction stress, solid element, WRC 107, WRC 297

The objective of the Hydro Turbine Head Cover is to support the guide bearing, guide vane regulating mechanism and even in some design for generator thrust bearing support. Mechanical design of head cover deals with high static as well as fluctuating load acting on the structure. In the present work structural analysis of hydro turbine Head-cover using ANSYS software is carried out. Finite element method is used to calculate stresses on head cover. These calculations were done for the maximum possible loading under operating condition “LCI Quick Shut Down”. Detailed thermal hydraulic investigations are very essential for safe and reliable functioning of liquid metal cooled fast breeder reactors. These investigations are further more important for components with complex profile, since there is no direct correlation available in literature to evaluate the hydraulic characteristics of such components directly. In those cases available correlations for similar profile or geometries may lead to significant uncertainty in the outcome. Hence experimental approach can be adopted to evaluate these hydraulic characteristics more precisely for better prediction in reactor core components. Prototype Fast Breeder Reactor (PFBR), a sodium cooled pool type reactor is under advanced stage of construction at Kalpakkam, India. Several components of this reactor core require hydraulic investigation before its usage in the reactor. Growing business recognizes environmental consciousness as an important concept for survival in the competitive scenario. Environmental consciousness is a critical intersection between manufacturing and product design processes with environmental issues and concerns. This article presents a project in which quality function deployment (QFD) for environment (ECQFD) has been applied to rotary switches for enabling environmentally conscious design in the early stage of product development. ECQFD is capable of handling simultaneously the environmental and traditional product quality requirements. ECQFD consists of four phases. ECQFD phases I and II are concerned with the identification of parts that are important in enhancing environmental consciousness. ECQFD phases III and IV are concerned with the evaluation of effect of design improvement on environmental quality requirements. A method of decreasing the heating load of HVAC systems in a single-dwelling model of a multifamily building, by controlling movable insulation through the optimization of flux, time, surface incident solar radiation, and temperature thresholds. Simulations are completed using a co-simulation between EnergyPlus and MATLAB as an optimization tool to find optimal control thresholds. Abstract:
Selecting an appropriate image representation is the most important factor in implementing an effective Content-Based Image Retrieval (CBIR) system. This paper presents a multi-feature fusion approach for efficient CBIR, based on the distance distribution of features and relative feature weights at the time of query processing. It is a simple yet effective approach, which is free from the effect of features' dimensions, ranges, internal feature normalization and the distance measure. This approach can easily be adopted in any feature combination to improve retrieval quality. The proposed approach is empirically evaluated using two benchmark datasets for image classification (a subset of the Corel dataset and Oliva and Torralba) and compared with existing approaches. The performance of the proposed approach is confirmed with the significantly improved performance in comparison with the independently evaluated baseline of the previously proposed feature fusion approaches.Keywords: feature fusion, image retrieval, membership function, normalization

Located in the Gulf of Guinea, the Kribi-Campo sub-basin belongs to the Aptian salt basins along the West African Margin. In this paper, we investigated the tectono-stratigraphic architecture of the basin, focusing on the role of salt tectonics and strike-slip faults along the Kribi Fracture Zone with implications for reservoir prediction. Using 2D seismic data and well data interpreted through sequence stratigraphy with integrated seismic attributes analysis with Python Programming and unsupervised Machine Learning, at least six second-order sequences, indicating three main stages of tectono-stratigraphic evolution, were determined: pre-salt syn-rift, post-salt rift climax and post-rift stages. The pre-salt syn-rift stage with KTS1 tectonosequence (Barremian-Aptian) reveals a transform rifting along NE-SW transfer faults associated with N-S to NNE-SSW syn-rift longitudinal faults bounding a NW-SE half-graben filled with alluvial to lacustrine-fan delta deposits. The post-salt rift-climax stage (Lower to Upper Cretaceous) includes two second-order tectonosequences (KTS2 and KTS3) associated with the salt tectonics and Campo High uplift. During the rift-climax stage, the growth of salt diapirs developed syncline withdrawal basins filled by early forced regression, mid transgressive and late normal regressive systems tracts. The early rift climax underlines some fine-grained hangingwall fans or delta deposits and coarse-grained fans from the footwall of fault scarps. The post-rift stage (Paleogene to Neogene) contains at least three main tectonosequences KTS4, KTS5 and KTS6-7. The first one developed some turbiditic lobe complexes considered as mass transport complexes and feeder channel-lobe complexes cutting the unstable shelf edge of the Campo High. The last two developed submarine Channel Complexes associated with lobes towards the southern part and braided delta to tidal channels towards the northern part of the Kribi-Campo sub-basin. The reservoir distribution in the Kribi-Campo sub-basin reveals some channels, fan lobes reservoirs and stacked channels reaching up to the polygonal fault systems.Keywords: tectono-stratigraphic architecture, Kribi-Campo sub-basin, machine learning, pre-salt sequences, post-salt sequences

At the moment earth is in grave danger due to threats of global warming. The temperature of the earth has risen by almost 20C. Glaciers in the Arctic have started to melt. It would be foolhardy to think that this is a small effect and in time it would go away. Global warming is caused by a number of factors. However, one sure and simple way to totally eliminate this problem is to put programmable shields in space. Just as an umbrella blocks sunlight, a programmable shield in space will block sun rays from reaching the earth as in a solar eclipse and cause cooling in the penumbral region just as it happens during an eclipse.Keywords: glaciers, green house, global warming space, satellites

Abstract:
The prime importance of tillage is that it prepares the land where the seed easily germinate and later the plant can well establish. Using different types of equipments driven manually or by powered, machines make the soil suitable to place the seeds into the desirable depth. Moreover, tillage loosens the compacted layers. Heavy equipment and tillage implements can cause damage to the soil structure. Effect of various tillage methods on soil compaction was studied in Rabi season of 2013-14 at village Ladwa, Hisar, Haryana (India). The experiments studied the effect of six tillage treatments i.e. no tillage or zero tillage (T1), tillage with rotavator (T2), disc harrow (T3), rotavator + sub soiler (T4), disc harrow + sub soiler (T5) and power harrow (T6) on soil compaction. Soil compaction was measured before tillage and after sowing at 0, 30, 60 and 90 days after sowing. No change in soil resistance was recorded before and after no tillage treatment. Maximum soil resistance was found in zero tillage followed by disc harrow up to 150 mm soil depth. Minimum soil resistance was found in rotavator immediately after the tillage treatment. However, the soil resistance approached the same level as it had been before the tillage after the soil strata where the implement cannot reach.Keywords: tillage, no tillage, rotavator, subsoiler, compaction

Abstract:
At present, research on automobile engineering is in high demand, particularly in the field of fuel combustion. A large number of fossil fuels are being used in combustion, which may get exhausted in the near future and are not economical. To this end, research on the use of magnetic material in combustion engines is in progress to enhance the efficiency of fuel. The present review describes the chemical, physical and mathematical theory behind the magnetic materials along with the working principle of the internal combustion engine. The effect of different magnets like ferrite magnet, Neodymium magnet, and electromagnets was discussed. The effect of magnetic field on the consumption of the fuel, brake thermal efficiency, carbon monoxide, Oxides of Nitrogen, carbon dioxide, and hydrocarbon emission, along with smoke density, have been discussed in detail. Detailed mathematical modelling that shows the effect of magnetic field on fuel combustion is elaborated. Required pictorial representations are included wherever necessary. Attendance rate for hospital outpatient appointments plays a pivotal role in operational efficiency of a hospital. Strategic interventions like ‘reminder systems’ prior to the scheduled appointment has proved to be an effective strategy for outpatient appointment ‘show-ups’. This study is designed with an objective to assess the effectiveness of SMS reminders as an intervention to enhance the effectiveness of hospital outpatient attendance. Method: The survey was conducted at Columbia Asia Hosiptal, Bangalore. We surveyed 60 patients who had a scheduled outpatient appointment in Department of General Medicine, Department of Obstetrics and Gynecology and the Orthopedics department, as these departments had a heavy patient flow and had higher contributions to the top line of the hospital. Results: Majority (64%) of the patients preferred to be sent an SMS reminder on the outpatient appointment schedule. 37 (61%) respondents stated that the ideally, reminders could be effective only if they are sent 24-48 hours prior to the appointment schedule. 41(68%) respondents were of the opinion that a minimum of two reminders would be necessary to ensure patients show up for the appointment. 1% level of significance. It also observed that there is strong association between age and preference on mode of reminder (P=0.002).Keywords: reminder systems, appointment show-ups, SMS reminders, health Information
