85 The Impact of Diabetes Mellitus on Skin and Soft Tissue Infections

Abstract:
Aim: Diabetes mellitus (DM) is a worldwide pandemic affecting 500 million people. It is known to be associated with increased susceptibility to soft tissue infections (STI). Despite being a major public health burden, the literature relating the effects of DM and the presentation, severity and healing of STIs in general surgical patients remain limited. Methods: We conducted a retrospective review of all patients admitted with STI in a tertiary teaching hospital over a 12-month period. Patient demographics and surgical outcomes were collected and analyzed. Results: During the study period, 1059 patients were admitted for STIs, of which 936 (88%) required surgical intervention. Diabetic patients were presented with a higher body-mass index (BMI) (28 vs 26), larger abscess size (24 vs 14 cm²) and a longer length of stay (LOS)(4.4 days vs 2.9 days). They also underwent a higher proportion of wide debridement as well as application of negative pressure wound therapy (NPWT) (42% vs 35%). More diabetic patients underwent subsequent re-operation within the same sitting (8 vs 4). There were no differences in re-admission rates within 30 days nor subsequent abscess formation in those followed for 6 months. Conclusion: The incidence of STIs among DM patients represents a significant disease burden; surgeons should consider intensive patient counseling and partnering with primary care providers in order to help reduce the incidence of future STI admissions based on lifestyle modification and glucose control.Keywords: general surgery, emergency general surgery, acute care surgery, soft tissue infections, diabetes mellitus

Abstract:
Keyword search in XML documents is based on the notion of lowest common ancestors in the labelled trees model of XML documents and has recently gained a lot of research interest in the database community. In this paper, we propose the Modified Active (MA) algorithm which is an improvement over the active clustering algorithm by taking into consideration the entity aspect of the nodes to find the level of the node pertaining to a particular keyword input by the user. A portion of the bibliography database is used to experimentally evaluate the modified active algorithm and results show that it performs better than the active algorithm. The Laplace-Adomian-Pade method is used for solving fractional Riccati-type initial value differential equations of fractional order. Abstract:
We have immobilized alcohol dehydrogenase on k-carrageenan, which is a natural polysaccharide obtained from seaweeds by entrapment and on copolymer of acrylamide and 2-hydroxy ethylmethaacrylate by covalent coupling. We have optimized all the immobilization parameters and also carried the comparison studies of both. In case of copolymer of acrylamide and 2-hydroxy ethylmethaacrylate, we have activated both the amino and hydroxyl group individually and simultaneously using different activating agents and obtained some interesting results. We have found that covalently bound enzyme was found to be better under all tested conditions. Electrical discharge machining (EDM), a non-conventional machining process, finds wide applications for shaping difficult-to-cut alloys. Process modeling of EDM is required to exploit the process to the fullest. Process modeling of EDM is a challenging task owing to involvement of so many electrical and non-electrical parameters. This work is an attempt to model the EDM process using artificial neural network (ANN). Experiments were carried out on die-sinking EDM taking Inconel 825 as work material. ANN modeling has been performed using experimental data. The prediction ability of trained network has been verified experimentally. The usage of artificially created videos and audio by deep learning is a major problem of the current media landscape, as it pursues the goal of misinformation and distrust. In conclusion, the objective of this work targets generating a reliable deepfake detection model using deep learning that will help detect forged videos accurately. In this work, CelebDF v1, one of the largest deepfake benchmark datasets in the literature, is adopted to train and test the proposed models. Geometry simplification is a key step in performing conjugate heat transfer analysis using CFD. This paper proposes a standard methodology for the geometry simplification of rotating machines, such as electrical generators and electrical motors (both air and liquid-cooled). These machines are extensively deployed throughout the aerospace and automotive industries, where optimization of weight, volume, and performance is paramount -especially given the current global transition to renewable energy sources and vehicle hybridization and electrification. Conjugate heat transfer analysis is an essential step in optimizing their complex design. The use of AI techniques not only reduces the task and work load but also helps in getting better output than manual testing. Although results are same, but the use of Artifical intelligence techniques for GUI testing has proven to provide ideal results. DL-fueled framework helped us to find imperfections of the entire webpage and provides test failure result in a score format between 0 and 1which signifies that are test meets it quality criteria or not. This work deals with two buildings adjacent to each other connected with dampers. The “Imperial Valley Earthquake - El Centro", "May 18, 1940 earthquake time history is used for dynamic analysis of the system in the time domain. The effectiveness of fluid joint dampers is then investigated in terms of the reduction of displacement, acceleration and base shear responses of adjacent buildings. Finally, an extensive parametric study is carried out to find optimum damper properties like stiffness (Kd) and damping coefficient (Cd) for adjacent buildings. Results show that using fluid dampers to connect the adjacent buildings of different fundamental frequencies can effectively reduce earthquake-induced responses of either building if damper optimum properties are selected.Keywords: energy dissipation devices, time history analysis, viscous damper, optimum parameters

Fuzzy fractional diffusion equation is widely useful to depict different physical processes arising in physics, biology, and hydrology. The motive of this article is to deal with the fuzzy fractional diffusion equation. We study a mathematical model of fuzzy space-time fractional diffusion equation in which unknown function, coefficients, and initial-boundary conditions are fuzzy numbers. First, we find out a fuzzy operational matrix of Legendre polynomial of Caputo type fuzzy fractional derivative having a non-singular Mittag-Leffler kernel. The main advantages of this method are that it reduces the fuzzy fractional partial differential equation (FFPDE) to a system of fuzzy algebraic equations from which we can find the solution of the problem. The feasibility of our approach is shown by some numerical examples. Complete labeled data is often difficult to obtain in a practical scenario. Even if one manages to obtain the data, the quality of the data is always in question. In shopping vertical, offers are the input data, which is given by advertiser with or without a good quality of information. In this paper, an author investigated the possibility of using a very simple Semi-supervised learning approach to increase the recall of unhealthy offers (has badly written Offer Title or partial product details) in shopping vertical domain. The author found that the semisupervised learning method had improved the recall in the Smart Phone category by 30% on A=B testing on 10% traffic and increased the YoY (Year over Year) number of impressions per month by 33% at production. Evaluation of dynamic earth pressure on retaining wall is a topic of primary importance. In present paper, dynamic active earth pressure and displacement of flexible cantilever retaining wall has been evaluated analytically using 2-DOF mass-spring-dashpot model by incorporating both wall and backfill properties. The effect of wall flexibility on dynamic active earth pressure and wall displacement are studied and presented in graphical form. The obtained results are then compared with the various conventional methods, experimental analysis and also with PLAXIS analysis. It is observed that the dynamic active earth pressure decreases with increase in the wall flexibility while wall displacement increases linearly with flexibility of the wall. Abstract:
Bearing is an important machinery part in the industry. When bearings fail to meet their expected life the consequences are increased downtime, loss of revenue and missed the delivery. This article describes the failure of a gearbox bearing in rolling contact fatigue. The investigation consists of visual observation, chemical analysis, characterization of microstructures using optical microscopes and hardness test. The present study also considers bearing life as well as the operational condition of bearings. Surface-initiated rolling contact fatigue, leading to a surface failure known as pitting, is a life-limiting failure mode in many modern machine elements, particularly rolling element bearings. Metallography analysis of crack propagation, crack morphology was also described. Indication of fatigue spalling in the ferrography test was also discussed. The analysis suggested the probable reasons for such kind of failure in operation. Abstract:
Gravitation and the expansion of the universe at a large scale are generally regarded as two completely distinct phenomena. Yet, in general, relativity theory, they both manifest as 'curvature' of spacetime. We propose a hypothesis which treats these two 'curvature-producing' phenomena as aspects of an underlying process. This process treats spacetime itself as composed of discrete units (Plancktons) and is 'dynamic' in the sense that these elements of spacetime are continually being both created and annihilated. It is these two complementary processes of Planckton creation and Planckton annihilation which manifest themselves as - 'cosmic expansion' on the one hand and as 'gravitational attraction’ on the other. The Planckton hypothesis treats spacetime as a perfect fluid in the same manner as the co-moving frame of reference of Friedman equations and the Gullstrand-Painleve metric; i.e.Planckton hypothesis replaces 'curvature' of spacetime by the 'flow' of Plancktons (spacetime). Here we discuss how this perspective may allow a unified description of both cosmological and gravitational acceleration as well as providing a mechanism for inducing an irreducible action at every point associated with the creation and annihilation of Plancktons, which could be identified as the zero point energy.Keywords: discrete spacetime, spacetime flow, zero point energy, planktons

Abstract:
This paper presents the findings of successful implementation of Business Process Reengineering (BPR) of cement dispatch activities in a cement manufacturing plant located in India. Simulation model was developed for the purpose of identifying and analyzing the areas for improvement. The company was facing a problem of low throughput rate and subsequent forced stoppages of the plant leading to a high production loss of 15000MT per month. It was found from the study that the present systems and procedures related to the in-plant logistics plant required significant changes. The major recommendations included process improvement at the entry gate, reducing the cycle time at the security gate and installation of an additional weigh bridge. This paper demonstrates how BPR can be implemented for improving the in-plant logistics process. MnS is the most commonly found inclusion in steel, which is desirable for machinability of alloy steels but only up to a certain limit, beyond which it weakens fatigue properties of steel. In present work, the effect of sulfur content and its inclusions on the fatigue behavior of AISI 4140 steel is studied (sulfur content 0.002% and 0.016%). Metallurgical analysis, Mechanical testing and Rotating Bending Fatigue (RBF) test were carried out. With the increase in sulfur content, ductility and toughness of the material decrease significantly and large scatter is observed in UTS and impact energy values. From the results of RBF testing, it can be observed that increase in sulfur content from 0.002% to 0.016% has a negligible effect on the endurance strength of AISI 4140 for similar hardness level. Abstract:
Accurate identification of Sleep Apnea Hypopnea Syndrom Diagnosis is difficult problem for human expert because of variability among persons and unwanted noise. This paper proposes the diagonosis of Sleep Apnea Hypopnea Syndrome (SAHS) using airflow, ECG, Pulse and SaO2 signals. The features of each type of these signals are extracted using statistical methods and ANN learning methods. These extracted features are used to approximate the patient's Apnea Hypopnea Index(AHI) using sample signals in model. Advance signal processing is also applied to snore sound signal to locate snore event and SaO2 signal is used to support whether determined snore event is true or noise. Finally, Apnea Hypopnea Index (AHI) event is calculated as per true snore event detected. Experiment results shows that the sensitivity can reach up to 96% and specificity to 96% as AHI greater than equal to 5.Keywords: neural network, AHI, statistical methods, autoregressive models

Abstract:
TeO2-WO3-Li2O glass doped erbium ions (1mol %) and embedded silver nanoparticles( Ag NPs) has successfully been prepared by melt quenching technique and increasing the heat-treatment duration. The amorphous nature of the glass is determined by X-ray diffraction method, and the presences of silver nanoparticles are confirmed using Transmission Electron Microscopy analysis. TEM image reveals that the Ag NPs are dispersed homogeneously with average size 18 nm. From the UV-Vis absorption spectra, the surface plasmon resonance (SPR) peaks are detected at 550 and 578 nm. Under 980 nm excitation wavelengths, enhancement of red upconversion fluorescence and near-infrared broadband emission around 1550nm of Er3+ ions doped tellurite glasses containing Ag NPs have been observed. The observed enhancement of Er3+ emission is mainly attributed to the local field effects of Ag NPs causes an intensified electromagnetic field around NPs. With the increasing demand for real-time applications in WSN, real-time critical events anticipate an efficient quality-of-service (QoS) based routing for data delivery from the network infrastructure. Hence, maximizing the lifetime of the network through minimizing the energy is an important challenge in WSN; sensors cannot be easily replaced or recharged due to their ad-hoc deployment in a hazardous environment. Considerable research has been focused on developing robust energy efficient QoS based routing protocols. The main focus of this article is primarily on periodical cycling schemes which represent the most compatible technique for energy saving and we also focus on the data-driven approaches that can be used to improve the energy efficiency. Abstract:
The use of nanostructured semiconducting material to catalyze degradation of environmental pollutants still receives much attention to date. One of the desired characteristics for pollutant degradation under ultra-violet visible light is the materials with extended carrier charge separation that allows for electronic transfer between the catalyst and the pollutants. In this work, zinc oxide n-type semiconductor vertically aligned structures were fabricated on silicon (100) substrates using the chemical bath deposition method. The as-synthesized structures were treated with nickel and sulphur. X-ray diffraction, scanning electron microscopy, energy dispersive X-ray spectroscopy were used to characterize the phase purity, structural dimensions and elemental composition of the obtained structures respectively. Photoluminescence emission measurements showed a decrease in both the near band edge emission as well as the defect band emission upon addition of nickel and sulphur with different concentrations. This was attributed to increased charger-carrier-separation due to the presence of Ni-S material on ZnO surface, which is linked to improved charge transfer during photocatalytic reactions.Keywords: Carrier-charge-separation, nickel, photoluminescence, sulphur, zinc oxide

This review provides an overview of the impact that implementation of various Knowledge Management (KM) strategies has had on the growth of a department in a Public Sector Company in India. In a regulated utility controlled by the government, the growth of an organization such as Hindustan Petroleum Corporation Limited (HPCL) had depended largely on the efficiencies of the systems and its people. However, subsequent to the de-regularization & to the entry of the private competition, the need for a ‘systematic templating’ of knowledge was recognized. This necessitated the introduction of Knowledge Management Centre (KMC). Projects & Pipelines Department (P&P) of HPCL introduced KMC that contributed significantly towards KM by adopting various strategies such as standardization, leveraging information system, competency enhancement, and improvements & innovations. These strategies gave both tangible as well as intangible benefits towards KM. Knowledge, technology & people are the three pillars that need to be catered for effective knowledge management in any organization. Abstract:
The evaluation of biodynamic responses of the human body to whole body vibration exposure is necessary to quantify the exposure effects. A force plate model has been designed with the help of CAD software, which was investigated by performing the modal, stress and strain analysis using finite element approach in the software. The results of the modal, stress and strain analysis were under the limits for measurements of biodynamic responses to whole body vibration. The physical model of the force plate was manufactured and fixed to the vibration simulator and further used in the experimentation for the evaluation of apparent mass responses of the ten recruited subjects standing in an erect posture exposed to vertical whole body vibration. The platform was excited with sinusoidal vibration at vibration magnitude: 1.0 and 1.5 m/s2 rms at different frequency of 2, 3, 4, 5, 6, 8, 10, 12.5, 16 and 20 Hz. The results of magnitude of normalised apparent mass have shown the trend observed in the many past studies. The peak in the normalised apparent mass has been observed at 4 & 5 Hz frequency of vertical whole body vibration. The nonlinearity with respect to vibration magnitude has been also observed in the normalised apparent mass responses.Keywords: whole body vibration, apparent mass, modeling, force plate

Abstract:
Malicious users exploit vulnerabilities in computer systems, significantly disrupting their performance and revealing the inadequacies of existing protective solutions. Even machine learning-based approaches, designed to ensure reliability, can be compromised by adversarial attacks that undermine their robustness. This paper addresses two critical aspects of enhancing model reliability. First, we focus on improving model performance and robustness against adversarial threats. To achieve this, we propose a strategy by harnessing deep reinforcement learning. Second, we introduce an approach leveraging generative adversarial networks to counter adversarial attacks effectively. Our results demonstrate substantial improvements over previous works in the literature, with classifiers exhibiting enhanced accuracy in classification tasks, even in the presence of adversarial perturbations. These findings underscore the efficacy of the proposed model in mitigating intrusions and adversarial attacks within the machine learning landscape.Keywords: machine learning, reliability, adversarial attacks, deep-reinforcement learning, robustness

Abstract:
Although the rapid development of forensic speaker recognition technology has been conducted, there are still many problems to be solved. The biggest problem arises when the cases involving disguised voice samples come across for the purpose of examination and identification. Such type of voice samples of anonymous callers is frequently encountered in crimes involving kidnapping, blackmailing, hoax extortion and many more, where the speaker makes a deliberate effort to manipulate their natural voice in order to conceal their identity due to the fear of being caught. Voice disguise causes serious damage to the natural vocal parameters of the speakers and thus complicates the process of identification. The sole objective of this doctoral project is to find out the possibility of rendering definite opinions in cases involving disguised speech by experimentally determining the effects of different disguise forms on personal identification and percentage rate of speaker recognition for various voice disguise techniques such as raised pitch, lower pitch, increased nasality, covering the mouth, constricting tract, obstacle in mouth etc by analyzing and comparing the amount of phonetic and acoustic variation in of artificial (disguised) and natural sample of an individual, by auditory as well as spectrographic analysis.Keywords: forensic, speaker recognition, voice, speech, disguise, identification

Abstract:
The paper discloses a controlled tracked humanoid robot moving platform. A driving and driven wheel are controlled by a control module to drive a robot body to move according to data signals of a monitoring module, in addition, remote transmission can be achieved, and a certain remote control function can be realized. A power management module circuit board looks after in used for providing electric drive for moving of the robot body and distribution of separate power source to be used in internal of robot system. An external port circuit board is arranged, the tracked robot moving platform can be used immediately for any data acquisition. The moving platform is simple and compact in structure, strong in adaptation performance, stable in operation and suitable for being operated in severe environments. Abstract:
Though there exists a plethora of Activation Functions (AFs) used in single and multiple hidden layer Neural Networks (NN), their behavior always raised curiosity, whether used in combination or singly. The popular AFs –Sigmoid, ReLU, and Tanh–have performed prominently well for shallow and deep architectures. Most of the time, AFs are used singly in multi-layered NN, and, to the best of our knowledge, their performance is never studied and analyzed deeply when used in combination. In this manuscript, we experiment with multi-layered NN architecture (both on shallow and deep architectures; Convolutional NN and VGG16) and investigate how well the network responds to using two different AFs (Sigmoid-Tanh, Tanh-ReLU, ReLU-Sigmoid) used alternately against a traditional, single (Sigmoid-Sigmoid, Tanh-Tanh, ReLUReLU) combination. Our results show that using two different AFs, the network achieves better accuracy, substantially lower loss, and faster convergence on 4 computer vision (CV) and 15 Non-CV (NCV) datasets. When using different AFs, not only was the accuracy greater by 6-7%, but we also accomplished convergence twice as fast. We present a case study to investigate the probability of networks suffering vanishing and exploding gradients when using two different AFs. Additionally, we theoretically showed that a composition of two or more AFs satisfies Universal Approximation Theorem (UAT).Keywords: activation function, universal approximation function, neural networks, convergence
