7865 A Case of Prosthetic Vascular-Graft Infection Due to Mycobacterium fortuitum
Authors: Takaaki Nemoto
Abstract:
Case presentation: A 69-year-old Japanese man presented with a low-grade fever and fatigue that had persisted for one month. The patient had an aortic dissection on the aortic arch 13 years prior, an abdominal aortic aneurysm seven years prior, and an aortic dissection on the distal aortic arch one year prior, which were all treated with artificial blood-vessel replacement surgery. Laboratory tests revealed an inflammatory response (CRP 7.61 mg/dl), high serum creatinine (Cr 1.4 mg/dL), and elevated transaminase (AST 47 IU/L, ALT 45 IU/L). The patient was admitted to our hospital on suspicion of prosthetic vascular graft infection. Following further workups on the inflammatory response, an enhanced chest computed tomography (CT) and a non-enhanced chest DWI (MRI) were performed. The patient was diagnosed with a pulmonary fistula and a prosthetic vascular graft infection on the distal aortic arch. After admission, the patient was administered Ceftriaxion and Vancomycine for 10 days, but his fever and inflammatory response did not improve. On day 13 of hospitalization, a lung fistula repair surgery and an omental filling operation were performed, and Meropenem and Vancomycine were administered. The fever and inflammatory response continued, and therefore we took repeated blood cultures. M. fortuitum was detected in a blood culture on day 16 of hospitalization. As a result, we changed the treatment regimen to Amikacin (400 mg/day), Meropenem (2 g/day), and Cefmetazole (4 g/day), and the fever and inflammatory response began to decrease gradually. We performed a test of sensitivity for Mycobacterium fortuitum, and found that the MIC was low for fluoroquinolone antibacterial agent. The clinical course was good, and the patient was discharged after a total of 8 weeks of intravenous drug administration. At discharge, we changed the treatment regimen to Levofloxacin (500 mg/day) and Clarithromycin (800 mg/day), and prescribed these two drugs as a long life suppressive therapy. Discussion: There are few cases of prosthetic vascular graft infection caused by mycobacteria, and a standard therapy remains to be established. For prosthetic vascular graft infections, it is ideal to provide surgical and medical treatment in parallel, but in this case, surgical treatment was difficult and, therefore, a conservative treatment was chosen. We attempted to increase the treatment success rate of this refractory disease by conducting a susceptibility test for mycobacteria and treating with different combinations of antimicrobial agents, which was ultimately effective. With our treatment approach, a good clinical course was obtained and continues at the present stage. Conclusion: Although prosthetic vascular graft infection resulting from mycobacteria is a refractory infectious disease, it may be curative to administer appropriate antibiotics based on the susceptibility test in addition to surgical treatment. Keywords: prosthetic vascular graft infection, lung fistula, Mycobacterium fortuitum, conservative treatment

Authors: Elena E. Lyakso, Olga V. Frolova, Yuri N. Matveev, Aleksey S. Grigorev, Alexander S. Nikolaev, Viktor A. Gorodnyi
Abstract:
The study of the children’s emotional sphere depending on age and psychoneurological state is of great importance for the design of educational programs for children and their social adaptation. Atypical development may be accompanied by violations or specificities of the emotional sphere. To study characteristics of the emotional state reflection in the voice and speech features of children, the perceptual study with the participation of adults and the automatic recognition of speech were conducted. Speech of children with typical development (TD), with Down syndrome (DS), and with autism spectrum disorders (ASD) aged 6-12 years was recorded. To obtain emotional speech in children, model situations were created, including a dialogue between the child and the experimenter containing questions that can cause various emotional states in the child and playing with a standard set of toys. The questions and toys were selected, taking into account the child’s age, developmental characteristics, and speech skills. For the perceptual experiment by adults, test sequences containing speech material of 30 children: TD, DS, and ASD were created. The listeners were 100 adults (age 19.3 ± 2.3 years). The listeners were tasked with determining the children’s emotional state as “comfort – neutral – discomfort” while listening to the test material. Spectrographic analysis of speech signals was conducted. For automatic recognition of the emotional state, 6594 speech files containing speech material of children were prepared. Automatic recognition of three states, “comfort – neutral – discomfort,” was performed using automatically extracted from the set of acoustic features - the Geneva Minimalistic Acoustic Parameter Set (GeMAPS) and the extended Geneva Minimalistic Acoustic Parameter Set (eGeMAPS). The results showed that the emotional state is worse determined by the speech of TD children (comfort – 58% of correct answers, discomfort – 56%). Listeners better recognized discomfort in children with ASD and DS (78% of answers) than comfort (70% and 67%, respectively, for children with DS and ASD). The neutral state is better recognized by the speech of children with ASD (67%) than by the speech of children with DS (52%) and TD children (54%). According to the automatic recognition data using the acoustic feature set GeMAPSv01b, the accuracy of automatic recognition of emotional states for children with ASD is 0.687; children with DS – 0.725; TD children – 0.641. When using the acoustic feature set eGeMAPSv01b, the accuracy of automatic recognition of emotional states for children with ASD is 0.671; children with DS – 0.717; TD children – 0.631. The use of different models showed similar results, with better recognition of emotional states by the speech of children with DS than by the speech of children with ASD. The state of comfort is automatically determined better by the speech of TD children (precision – 0.546) and children with ASD (0.523), discomfort – children with DS (0.504). The data on the specificities of recognition by adults of the children’s emotional state by their speech may be used in recruitment for working with children with atypical development. Automatic recognition data can be used to create alternative communication systems and automatic human-computer interfaces for social-emotional learning. Authors: Maryam Azish
Abstract:
Russia, as an emerging global player in recent years, has projected a special place in the Middle East. Despite all the challenges it has faced over the years, it has always considered its presence in various fields with a strategy that has defined its maneuvering power as a level of competition and even confrontation with the United States. Therefore, its current approach is considered important as an influential actor in the Middle East. After the collapse of the Soviet Union, when the Russians withdrew completely from the Middle East, the American scene remained almost unrivaled by the Americans. With the start of the US-led war in Iraq and Afghanistan and the subsequent developments that led to the US military and political defeat, a new chapter in regional security was created in which ISIL and Taliban terrorism went along with the Arab Spring to destabilize the Middle East. Because of this, the Americans took every opportunity to strengthen their military presence. Iraq, Syria and Afghanistan have always been the three areas where terrorism was shaped, and the countries of the region have each reacted to this evil phenomenon accordingly. The West dealt with this phenomenon on a case-by-case basis in the general circumstances that created the fluid situation in the Arab countries and the region. Russian President Vladimir Putin accused the US of falling asleep in the face of ISIS and terrorism in Syria. In fact, this was an opportunity for the Russians to revive their presence in Syria. This article suggests that utilizing the recognition policy along with the constructivism theory will offer a better knowledge of Russia’s endeavors to endorse its international position. Accordingly, Russia’s distinctiveness and its ambitions for a situation of great power have played a vital role in shaping national interests and, subsequently, in foreign policy, in Putin's era in particular. The focal claim of the paper is that scrutinize Russia’s foreign policy with realistic methods cannot be attained. Consequently, with an aim to fill the prevailing vacuum, this study exploits the politics of acknowledgment in the context of constructivism to examine Russia’s foreign policy in the Middle East. The results of this paper show that the key aim of Russian foreign policy discourse, accompanied by increasing power and wealth, is to recognize and reinstate the position of great power in the universal system. The Syrian crisis has created an opportunity for Russia to unite its position in the developing global and regional order after ages of dynamic and prevalent existence in the Middle East as well as contradicting US unilateralism. In the meantime, the writer thinks that the question of identifying Russia’s position in the global system by the West has played a foremost role in serving its national interests. Keywords: constructivism, foreign Policy, middle East, Russia, regionalism

Procedia PDF Downloads 4167855 An Observation Approach of Reading Order for Single Column and Two Column Layout Template
Authors: In-Tsang Lin, Chiching Wei
Abstract:
Reading order is an important task in many digitization scenarios involving the preservation of the logical structure of a document. From the paper survey, it finds that the state-of-the-art algorithm could not fulfill to get the accurate reading order in the portable document format (PDF) files with rich formats, diverse layout arrangement. In recent years, most of the studies on the analysis of reading order have targeted the specific problem of associating layout components with logical labels, while less attention has been paid to the problem of extracting relationships the problem of detecting the reading order relationship between logical components, such as cross-references. Over 3 years of development, the company Foxit has demonstrated the layout recognition (LR) engine in revision 20601 to eager for the accuracy of the reading order. The bounding box of each paragraph can be obtained correctly by the Foxit LR engine, but the result of reading-order is not always correct for single-column, and two-column layout format due to the table issue, formula issue, and multiple mini separated bounding box and footer issue. Thus, the algorithm is developed to improve the accuracy of the reading order based on the Foxit LR structure. In this paper, a creative observation method (Here called the MESH method) is provided here to open a new chance in the research of the reading-order field. Here two important parameters are introduced, one parameter is the number of the bounding box on the right side of the present bounding box (NRight), and another parameter is the number of the bounding box under the present bounding box (Nunder). And the normalized x-value (x/the whole width), the normalized y-value (y/the whole height) of each bounding box, the x-, and y- position of each bounding box were also put into consideration. Initial experimental results of single column layout format demonstrate a 19.33% absolute improvement in accuracy of the reading-order over 7 PDF files (total 150 pages) using our proposed method based on the LR structure over the baseline method using the LR structure in 20601 revision, which its accuracy of the reading-order is 72%. And for two-column layout format, the preliminary results demonstrate a 44.44% absolute improvement in accuracy of the reading-order over 2 PDF files (total 18 pages) using our proposed method based on the LR structure over the baseline method using the LR structure in 20601 revision, which its accuracy of the reading-order is 0%. Until now, the footer issue and a part of multiple mini separated bounding box issue can be solved by using the MESH method. However, there are still three issues that cannot be solved, such as the table issue, formula issue, and the random multiple mini separated bounding boxes. But the detection of the table position and the recognition of the table structure are out of the scope in this paper, and there is needed another research. In the future, the tasks are chosen- how to detect the table position in the page and to extract the content of the table. Keywords: document processing, reading order, observation method, layout recognition

Procedia PDF Downloads 1977850 Characterization of Number of Subgroups of Finite Groups
Authors: Khyati Sharma, A. Satyanarayana Reddy
Abstract:
The topic of how many subgroups exist within a certain finite group naturally arises in the study of finite groups. Over the years, different researchers have investigated this issue from a variety of angles. The significant contributions of the key mathematicians over the time have been summarized in this article. To this end, we classify finite groups into three categories viz. (a) Groups for which the number of subgroups is less than |G|, (b) equals to |G|, and finally, (c) greater than |G|. Because every element of a finite group generates a cyclic subgroup, counting cyclic subgroups is the most important task in this endeavor. A brief survey on the number of cyclic subgroups of finite groups is also conducted by us. Furthermore, we also covered certain arithmetic relations between the order of a finite group |G| and the number of its distinct cyclic subgroups |C(G)|. In order to provide pertinent context and possibly reveal new novel areas of potential research within the field of research on finite groups, we finally pose and solicit a few open questions. Keywords: abstract algebra, cyclic subgroup, finite group, subgroup

Procedia PDF Downloads 3707848 Mathematical Model That Using Scrambling and Message Integrity Methods in Audio Steganography
Authors: Mohammed Salem Atoum
Abstract:
The success of audio steganography is to ensure imperceptibility of the embedded message in stego file and withstand any form of intentional or un-intentional degradation of message (robustness). Audio steganographic that utilized LSB of audio stream to embed message gain a lot of popularity over the years in meeting the perceptual transparency, robustness and capacity. This research proposes an XLSB technique in order to circumvent the weakness observed in LSB technique. Scrambling technique is introduce in two steps; partitioning the message into blocks followed by permutation each blocks in order to confuse the contents of the message. The message is embedded in the MP3 audio sample. After extracting the message, the permutation codebook is used to re-order it into its original form. Md5sum and SHA-256 are used to verify whether the message is altered or not during transmission. Experimental result shows that the XLSB performs better than LSB.Keywords: XLSB, scrambling, audio steganography, security
Procedia PDF Downloads 3637847 Analysis of Tandem Detonator Algorithm Optimized by Quantum Algorithm
Authors: Tomasz Robert Kuczerski
Abstract:
The high complexity of the algorithm of the autonomous tandem detonator system creates an optimization problem due to the parallel operation of several machine states of the system. Many years of experience and classic analyses have led to a partially optimized model. Limitations on the energy resources of this class of autonomous systems make it necessary to search for more effective methods of optimisation. The use of the Quantum Approximate Optimization Algorithm (QAOA) in these studies shows the most promising results. With the help of multiple evaluations of several qubit quantum circuits, proper results of variable parameter optimization were obtained. In addition, it was observed that the increase in the number of assessments does not result in further efficient growth due to the increasing complexity of optimising variables. The tests confirmed the effectiveness of the QAOA optimization method. Keywords: algorithm analysis, autonomous system, quantum optimization, tandem detonator

Authors: María Del Mar Ramos-Lorente, Rafael Martínez-Martín
Abstract:
More than 40 years have passed since the Spanish Constitution in force today was approved in 1978. The period prior to that Constitution, which marked the transition to democracy, was marked by National Catholicism, which actively limited the existence of religions other than Catholicism in the national territory. The approval of this norm allowed the opening in many aspects, including the religious one. This work will profusely describe the evolution of the appearance of religious minorities in Spain from the moment of the transition, in which the space for religious freedom appears up to the present. The methodology is twofold. On the one hand, qualitative analysis of the legislation has allowed the religious opening. On the other, the quantitative analysis of the NMRs implemented in Spain. The entire analysis establishes the increase in religious organizations as a result, with notable variations across the territory. Keywords: new religious movements, religious minorities, sociological analysis, Spain

Authors: Aiden Wang, Joseph Marino
Abstract:
In this study, we investigated the impact of different tempo variations on 1-repetition maximum (1RM) growth, focusing on the eccentric, isometric, and concentric phases of the lift. Through a 6-week longitudinal study involving 20 individuals with 1-5 years of barbell training experience, we compared the effects of various tempo schemes on bench press performance. Our results revealed that subjects who performed a tempo bench press with a 3-second eccentric phase, 3-second isometric phase, and explosive concentric phase on a weekly basis experienced the most significant increases in 1RM. Notably, this tempo also led to improved technique and stability during the exercise. Our findings provide valuable insights for strength trainers and coaches seeking to optimize bench press performance and overcome strength plateaus effectively. Keywords: exercise science, powerlifting, barbell, interventionist, longitudinal study

Authors: A. N. Moulai Khatir
Abstract:
SOI-Multifin-FETs are placed to be the workhorse of the industry for the coming few generations, and thus, in a few years because their excellent transistor characteristics, ideal sub-threshold swing, low drain induced barrier lowering (DIBL) without pocket implantation, and negligible body bias dependency. The corner effect may also exist in the two lower corners; this effect is called the BOX effect, which can also occur in the direction X-Z. The electric field lines from the source and drain cross the bottom oxide and arrive in the silicon. This effect is also called DIVSB (Drain Induced Virtual Substrate Basing). The potential in the silicon film in particular near the drain is increased by the drain bias. It is similar to DIBL and result in a decrease of the threshold voltage. This work provides an understanding of the limitation of this effect by reducing the fin width for components with increased fin number. Keywords: SOI, finFET, corner effect, dual-gate, tri-gate, BOX, multi-finFET

Procedia PDF Downloads 2517839 Road Systems as Environmental Barriers: An Overview of Roadways in Their Function as Fences for Wildlife Movement
Authors: Rachael Bentley, Callahan Gergen, Brodie Thiede
Abstract:
Roadways have a significant impact on the environment in so far as they function as barriers to wildlife movement, both through road mortality and through resultant road avoidance. Roads have an im-mense presence worldwide, and it is predicted to increase substantially in the next thirty years. As roadways become even more common, it is important to consider their environmental impact, and to mitigate the negative effects which they have on wildlife and wildlife mobility. In a thorough analysis of several related studies, a common conclusion was that roads cause habitat fragmentation, which can lead split populations to evolve differently, for better or for worse. Though some populations adapted positively to roadways, becoming more resistant to road mortality, and more tolerant to noise and chemical contamination, many others experienced maladaptation, either due to chemical contamination in and around their environment, or because of genetic mutations from inbreeding when their population was fragmented too substantially to support a large enough group for healthy genetic exchange. Large mammals were especially susceptible to maladaptation from inbreed-ing, as they require larger areas to roam and therefore require even more space to sustain a healthy population. Regardless of whether a species evolved positively or negatively as a result of their proximity to a road, animals tended to avoid roads, making the genetic diversity from habitat fragmentation an exceedingly prevalent issue in the larger discussion of road ecology. Additionally, the consideration of solu-tions, such as overpasses and underpasses, is crucial to ensuring the long term survival of many wildlife populations. In studies addressing the effectiveness of overpasses and underpasses, it seemed as though animals adjusted well to these sorts of solutions, but strategic place-ment, as well as proper sizing, proper height, shelter from road noise, and other considerations were important in construction. When an underpass or overpass was well-built and well-shielded from human activity, animals’ usage of the structure increased significantly throughout its first five years, thus reconnecting previously divided populations. Still, these structures are costly and they are often unable to fully address certain issues such as light, noise, and contaminants from vehicles. Therefore, the need for further discussion of new, crea-tive solutions remains paramount. Roads are one of the most consistent and prominent features of today’s landscape, but their environmental impacts are largely overlooked. While roads are useful for connecting people, they divide landscapes and animal habitats. Therefore, further research and investment in possible solutions is necessary to mitigate the negative effects which roads have on wildlife mobility and to pre-vent issues from resultant habitat fragmentation. Keywords: fences, habitat fragmentation, roadways, wildlife mobility

Authors: A. Meydanlioglu, F. Arikan
Abstract:
In recent years, thanks to the development of information and communication technologies, the computer and internet have been used widely in higher education. Internet-based education is impacting traditional higher education as online components increasingly become integrated into face-to-face (FTF) courses. The goal of combined internet-based and traditional education is to take full advantage of the benefits of each platform in order to provide an educational opportunity that can promote student learning better than can either platform alone. Research results show that the use of hybrid learning is more effective than online or FTF models in higher education. Due to the potential benefits, an increasing number of institutions are interested in developing hybrid courses, programs, and degrees. Future research should evaluate the effectiveness of hybrid learning. This paper is designed to determine the impact of hybrid learning on higher education. Keywords: e-learning, higher education, hybrid learning, online education
Procedia PDF Downloads 9097837 Realistic Study Discover Some Posture Deformities According to Some Biomechanical Variables for Schoolchildren
Authors: Basman Abdul Jabbar
Abstract:
The researchers aimed to improve the importance of the good posture without any divisions & deformities. The importance of research lied in the discovery posture deformities early so easily treated before its transformation into advanced abnormalities difficult to treat and may need surgical intervention. Research problem was noting that some previous studies were based on the discovery of posture deformities, which was dependent on the (self-evaluation) which this type did not have accuracy to discover deformities. The Samples were (500) schoolchildren aged (9-11 years, males) at Baghdad al Karak. They were students at primary schools. The measure included all posture deformities. The researcher used video camera to analyze the posture deformities according to biomechanical variables by Kinovea software for motion analysis. The researcher recommended the need to use accurate scientific methods for early detection of posture deformities in children which contribute to the prevention and reduction of distortions. Keywords: biomechanics, children, deformities, posture

Authors: Nalin Simasathiansophon
Abstract:
This research aims to study English communication of Thai travel agencies and the impact of the ASEAN Economic Community (AEC) on Thai travel industry. A questionnaire was used in this research. The multi-stage sampling method was also utilized with 474 respondents from 79 Thai travel agencies. Descriptive statistics included percentage, average, and standard deviation. The findings revealed that English communication for most travel agencies was between the poor and intermediate level and therefore improvement is needed, especially the listening and speaking skills. In other words, the majority of respondents needed more training in terms of communicating in English. Since the age average of travel agencies was around 30-39 years, the training technique should integrate communicating skills together, such as stimulating technique or cooperating technique that could encourage travel agencies to use English in communicating with foreigners. Keywords: travel agencies, English communication, AEC, Thai
