9060 Prevalence of Down Syndrome: A Single-Center Study in Bandung, Indonesia

Abstract:
Down syndrome (DS) is a chromosomal abnormality characterised by complete 21 chromosome trisomy (classical or non-disjunction), or partial 21 chromosome trisomy (mosaicism), or chromosome rearrangement involving chromosome 21 (translocation). This study was carried out to describe the frequency of DS patients in a research institution in the city of Bandung, Indonesia. This descriptive study also provides a picture of the residential location and surrounding area of their dwellings. This study involved people with DS in various age whose chromosome were evaluated by conventional karyotyping method and FISH. Data were collected from 60 patients with DS from a total 150 patients during the period of September 2015 to August 2016 who were referred to Cell Culture and Cytogenetics Laboratory, Faculty of Medicine Universitas Padjadjaran, Indonesia. Results showed that the most common type of DS was non-disjunction (93%), followed by mosaicism (5%), no patient with translocation DS (0%), and a very rare type of tetrasomy 21 (2%). There were 39 males (65%) and 21 females (35%) of DS patient. Most of them live in suburban area beyond Bandung city (55%) while the rest live inside urban area of Bandung city (45%). They live mostly in dense area of greater Bandung area (65%) and only a few live in mid-density area (25%) and the least live in sparse populated area (10%). Their houses are mostly located in residential estate area (55%), nearby industrial area (37%), and around agricultural area (8%). Based on the study, it could be concluded that non-disjunction DS is the most common type. DS patients referred to the laboratory mostly came from dense residential zone in suburban area outside Bandung city. The low number of DS patients referred to the laboratory for chromosome analysis was the highlight to improve health service for people with genetic disorder. This study offered several information regarding area of DS patients’ residence and the condition of neighbourhood in Bandung city where they live as well.Keywords: chromosome, descriptive, Down syndrome, prevalence

Abstract:
Blötberget mining area in central Sweden, part of the Bergslagen mineral district, is well known for its various type of mineralization particularly iron-oxide deposits since the 1600. To shed lights on the knowledge of the host rock structures, depth extent and tonnage of the mineral deposits and support deep mineral exploration potential in the study area, new ground gravity and existing aeromagnetic data (from the Geological Survey of Sweden) were used for interpretations and modelling. A major boundary separating a gravity low from a gravity high in the southern part of the study area is noticeable and likely representing a fault boundary separating two different lithological units. Gravity data and modeling offers a possible new target area in the southeast of the known mineralization while suggesting an excess high-density region down to 800 m depth.Keywords: gravity, magnetics, ore deposit, geophysics

Abstract:
Heterogeneity of soil hydraulic properties directly affects hydrological processes at different scales. Understanding heterogeneity of soil hydraulic properties such as soil moisture is therefore essential for modeling watershed ecohydrological processes, particularly in hard to access, topographically complex mountainous watersheds. This study maps spatial variations of soil moisture by in situ observation network that consists of sampling points, zones, and tributaries, and monitors corresponding hydrological variables of air and soil temperatures, evapotranspiration, infiltration, and runoff in the Upper Reach of the Heihe River Watershed, a second largest inland river (terminal lake) with a drainage area of over 128,000 km² in Northwest China. Subsequently, the study uses a hydrological model, SWAT (Soil and Water Assessment Tool) to simulate the effects of heterogeneity of soil moisture on watershed hydrological processes. The spatial clustering method, Full-Order-CLK was employed to derive five soil heterogeneous zones (Configuration 97, 80, 65, 40, and 20) for soil input to SWAT. Results show the simulations by the SWAT model with the spatially clustered soil hydraulic information from the field sampling data had much better representation of the soil heterogeneity and more accurate performance than the model using the average soil property values for each soil type derived from the coarse soil datasets. Thus, incorporating detailed field sampling soil heterogeneity data greatly improves performance in hydrologic modeling.Keywords: heterogeneity, soil moisture, SWAT, up-scaling

Abstract:
The AOI (AND – OR- INVERTER) based design of XOR gate is proposed in this paper with less number of gates. This new XOR gate required four basic gates and basic gate include only AND, OR, Inverter (AOI). Conventional XOR gate required five basic gates. Ripple Carry Adder (RCA) used in parallel addition but propagation delay time is large. RCA replaced with Carry Select Adder (CSLA) to reduce propagation delay time. CSLA design with dual RCA considering carry = ‘0’ and carry = ‘1’, so it is not an area efficient adder. To make area efficient, modified CSLA is designed with single RCA considering carry = ‘0’ and another RCA considering carry = ‘1’ replaced with Binary to Excess 1 Converter (BEC). Now replacement of conventional XOR gate by new design of XOR gate in modified CSLA reduces much area compared to regular CSLA and modified CSLA.Keywords: CSLA, BEC, XOR gate, area efficient

Abstract:
Energy is one of the important issues with high priority property in the world. Energy demand is rapidly increasing depending on the growing population and industry. The useable energy sources in the world will be insufficient to meet the need for energy. Therefore, the efficient and economical usage of energy sources is getting more importance. In a survey conducted among electric consuming machines, the electrical machines are consuming about 40% of the total electrical energy consumed by electrical devices and 96% of this consumption belongs to induction motors. Induction motors are the workhorses of industry and have very large application areas in industry and urban systems like water pumping and distribution systems, steel and paper industries and etc. Monitoring and the control of the motors have an important effect on the operating performance of the motor, driver selection and replacement strategy management of electrical machines. The sensorless monitoring system for monitoring and calculating efficiency of induction motors are studied in this study. The equivalent circuit of IEEE is used in the design of this study. The terminal current and voltage of induction motor are used in this motor to measure the efficiency of induction motor. The motor nameplate information and the measured current and voltage are used in this system to calculate accurately the losses of induction motor to calculate its input and output power. The efficiency of the induction motor is monitored online in the proposed method without disconnecting the motor from the driver and without adding any additional connection at the motor terminal box. The proposed monitoring system measure accurately the efficiency by including all losses without using torque meter and speed sensor. The monitoring system uses embedded architecture and does not need to connect to a computer to measure and log measured data. The conclusion regarding the efficiency, the accuracy and technical and economical benefits of the proposed method are presented. The experimental verification has been obtained on a 3 phase 1.1 kW, 2-pole induction motor. The proposed method can be used for optimal control of induction motors, efficiency monitoring and motor replacement strategy.Keywords: induction motor, efficiency, power losses, monitoring, embedded design

Abstract:
The purposes of this study are 1) to study the over 20-year attempt of Mahakan fort community to negotiate with Bangkok Metropolitan Administration (BMA) to remain in their residential area belonging to the state, and 2) to apply the new social and cultural dimension between the state and the community as an alternative for local participation in keeping their residential area. This is a qualitative research, and the findings reveal that the community claimed their ancestors’ right as owners of this piece of land for over 200 years. The community, therefore, requested to take part in the preservation of land, culture and local intellect and the area management in terms of being a learning resource on the cultural road in Rattanakosin Island. However, BMA imposed the law concerning the community area relocation in Rattanakosin Island. The result of law enforcement led to the failure of the area relocation, and the hard hit on physical structure of the area including the overall deterioration of the cultural road renovated in the year 1982, the 200 years’ celebration of Bangkok. The enforcement of law by the state required the move of the community, and the landscape improvement based on the capital city plan. However, this enforcement resulted in the unending conflicts between the community and the state, and the solution of this problem was unclear. At the same time the community has spent a long time opposing the state’s action, and preparing themselves by administrating the community behind Mahakan fortress with community administrative committee under the suggestion of external organization by registering all community members, providing funds for community administration. At the meantime the state lacked the continuation of the enforcement due to political problem and BMA’s administration problem. It is, therefore, suggested that an alternative solution to this problem lie at the negotiation between the state and the community with the purpose of the collaboration between the two to develop the area under the protective law of each side.Keywords: Pom-Mahakan community, reduction of conflicts, social area disclosure, residential area

Abstract:
The cultural park of the Ahaggar, occupying a large area of Algeria, is characterized by a rich wetlands area to be preserved and managed both in time and space. The management of a large area, by its complexity, needs large amounts of data, which for the most part, are spatially localized (DEM, satellite images and socio-economic information...), where the use of conventional and traditional methods is quite difficult. The remote sensing, by its efficiency in environmental applications, became an indispensable solution for this kind of studies. Remote sensing imaging data have been very useful in the last decade in very interesting applications. They can aid in several domains such as the detection and identification of diverse wetland surface targets, topographical details, and geological features... In this work, we try to extract automatically wetlands area using multispectral remotely sensed data on-board the Earth Observing 1 (EO-1) and Landsat satellite. Both are high-resolution multispectral imager with a 30 m resolution. The instrument images an interesting surface area. We have used images acquired over the several area of interesting in the National Park of Ahaggar in the south of Algeria. An Extraction Algorithm is applied on the several spectral index obtained from combination of different spectral bands to extract wetlands fraction occupation of land use. The obtained results show an accuracy to distinguish wetlands area from the other lad use themes using a fine exploitation on spectral index.Keywords: multispectral data, EO1, landsat, wetlands, Ahaggar, Algeria

Abstract:
The availability of free global and historical satellite imagery provides a valuable opportunity for mapping and monitoring the year by year for the built-up area, constantly and effectively. Land distribution guidelines and identification of changes are important in preparing and reviewing changes in the ground overview data. This study utilizes Landsat images for thirty years of information to acquire significant, and land spread data that are extremely valuable for urban arranging. This paper is mainly introducing to focus the basic of extracting built-up area for the city development area from the satellite images of LANDSAT 5,7,8 and Sentinel 2A from USGS in every five years. The purpose analyses the changing of the urban built-up area according to the year by year and to get the accuracy of mapping built-up and bare land areas in studying the trend of urban built-up changes the periods from 1990 to 2020. The GIS tools such as raster calculator and built-up area modelling are using in this study and then calculating the indices, which include enhanced built-up and bareness index (EBBI), Normalized difference Built-up index (NDBI), Urban index (UI), Built-up index (BUI) and Normalized difference bareness index (NDBAI) are used to get the high accuracy urban built-up area. Therefore, this study will point out a variable approach to automatically mapping typical enhanced built-up and bare land changes (EBBI) with simple indices and according to the outputs of indexes. Therefore, the percentage of the outputs of enhanced built-up and bareness index (EBBI) of the sentinel-2A can be realized with 48.4% of accuracy than the other index of Landsat images which are 15.6% in 1990 where there is increasing urban expansion area from 43.6% in 1990 to 92.5% in 2020 on the study area for last thirty years.Keywords: built-up area, EBBI, NDBI, NDBAI, urban index

Abstract:
In the Tokyo metropolitan area, traffic congestion and long commute times are caused by overconcentration in the central area. To resolve these problems, a core business city development policy was adopted in 1988. The core business cities, which include Yokohama, Chiba, Saitama, Tachikawa, and others, have designated business facilities accumulation districts where assistance measures are applied. Focusing on Yokohama city, this study investigates the trends in the number of offices, employees, and commuters at 2001 and 2012 Economic Census, as well as the average commute time in the Tokyo metropolitan area from 2005 to 2015 Metropolitan Transportation Census. Surveys were administered in 2001 and 2012 Economic Census to participants who worked in Yokohama, according to their distribution in the city's 1,757 subregions. Four main findings emerged: (1) The number of offices increased in Yokohama when the number of offices decreased in the Tokyo metropolitan area overall. Additionally, the number of employees at Yokohama increased. (2) The number of commuters to Tokyo's central area increased from Saitama prefecture, Tokyo Tama area, and Tokyo central area. However, it decreased from other areas. (3) The average commute time in the Tokyo metropolitan area was 67.7 minutes in 2015, a slight decrease from 2005 and 2010. Abstract:
Introduction: Lercanidipine hydrochloride (LD), an effective calcium channel blocker, widely used for the treatment of chronic stable angina and hypertension seems to be potential transdermal therapeutic system candidate, mainly due to its low oral bio availability, short half life and high first-pass metabolism. Objective: To develop transdermal therapeutic systems for LD and to evaluate its in vivo performance in rabbits. Methodology: Transdermal patches of LD were formulated using the polymer blend of eudragit RL100 (ERL) and polyvinyl pyrolidone (PVP) by casting method Propylene glycol (PG) and tween 80 were used as plasticizer and permeation enhancer respectively. The pharmaco kinetic parameters of LD after the administration of transdermal patches was compared with that of oral administration. The study was carried out in a two way crossover design in male New Zealand albino rabbits. Results: The formulation with ERL: PVP ratio 1:4 with 15% w/w PG as plasticizer and 4% w/w tween 80 as permeation enhancer showed the best drug release results. The pharmacokinetic parameters such as Cmax, tmax, mean residence time (MRT) and area under the curve (AUC 0-∞) were significantly different following transdermal administration compared to oral administration. The terminal half life of transdermally administered LD was found to similar that of oral administration. A sustained drug release over a period of 24 hrs was observed after transdermal administration. Conclusion: The fabricated transdermal delivery system have the potential to provide controlled and extended drug release, better bio availability and thus, this may improve the patient compliance.Keywords: transdermal therapeutic system, lercanidipine hydrochloride, eudragit, skinpermeation

Abstract:
Background: Ensuring patients with ST-elevation myocardial infarction (STEMI) access to hospitals that could perform percutaneous coronary intervention (PCI) in time is an important concern of healthcare managers. One commonly used the method to assess the coverage of population access to PCI hospital is the use GIS-estimated linear distance (crow's fly distance) between the district centroid and the nearest PCI hospital. If the distance is within a given distance (such as 20 km), the entire population of that district is considered to have appropriate access to PCI. The premise of using district centroid to estimate the coverage of population resident in that district is that the people live in the district are evenly distributed. In reality, the population density is not evenly distributed within the administrative district, especially in rural districts. Fortunately, the Taiwan government released basic statistical area (on average 450 population within the area) recently, which provide us an opportunity to estimate the coverage of population access to PCI services more accurate. Objectives: We aimed in this study to compare the population covered by a give PCI hospital according to traditional administrative district versus basic statistical area. We further examined if the differences between two geographic units used would be larger in a rural area than in urban area. Method: We selected two hospitals in Tainan City for this analysis. Hospital A is in urban area, hospital B is in rural area. The population in each traditional administrative district and basic statistical area are obtained from Taiwan National Geographic Information System, Ministry of Internal Affairs. Results: Estimated population live within 20 km of hospital A and B was 1,515,846 and 323,472 according to traditional administrative district and was 1,506,325 and 428,556 according to basic statistical area. Conclusion: In urban area, the estimated access population to PCI services was similar between two geographic units. This paper can provide a series that is no less interesting with Fourier series, so that will add new material about the series can be calculated with sigma notation. Finally, this paper provides analytical and geometric proof of the vast area in between the lines and curves that give the area is formed by y=ka2 dan kurva y=kx2, x-axis, line x=√a and x=-√a make a series of numbers for k=1 and a ∈ original numbers. ∑_(i=0)^n=(4n√n)/3=0+4/3+(8√2)/3+4√3+⋯+(4n√n)/3. The present work describes relative contributions of operating parameters on required heat transfer area of three heat exchangers viz. evaporator, condenser and cascade condenser of two stage R404A-R508B cascade refrigeration system using Taguchi method. The operating parameters considered in present study includes (1) condensing temperature of high temperature cycle and low temperature cycle (2) evaporating temperature of low temperature cycle (3) degree of superheating in low temperature cycle (4) refrigerating effect. Heat transfer areas of three heat exchangers are studied with variation of above operating parameters and also optimum working levels of each operating parameter has been obtained for minimum heat transfer area of each heat exchanger using Taguchi method. The analysis using Taguchi method reveals that evaporating temperature of low temperature cycle and refrigerating effect contribute relatively largely on the area of evaporator. Condenser area is mainly influenced by both condensing temperature of high temperature cycle and refrigerating effect. Abstract:
In the present study, wet digestion with HCl and HNO3 mixture was used to extract the heavy metals (copper (Cu), chromium (Cr), zinc (Zn), lead (Pb) and cadmium (Cd)) from the leaves, the stems and the roots of Malva sylvestris L., which were subsequently analyzed by AAS. The samples (soil and parts of species) were collected from different sites: the industrial area (IA) (Rouiba), the rubbish dump area (RDA) (Boudouaou), the residential area (RA) with large open fields and construction activities (Blida), the Montaigne area (MA) (Chrea) and the high plateau area (HPA) (Berouaguia). The study showed differences in metal concentrations according to the analysed parts and the different sampling locations. In the contaminated site of the industrial area (IA), high content of the toxic heavy metals (Cd: 3.18 µg/g DW and Pb: 34.48 µg/g DW) were found in the leaves of Malva sylvestris L. This finding suggests that the consumers of this species could be exposed to a risk associated with this higher level of these toxic metals. It was found that Malva sylvestris L. is rich by Zn and Cu in some sites, which are considered to be the essential elements for the human health. The obtained results with the control site (Montaigne area) suggest that this species can be applicable in both the health and food, feasible alternatives as medicinal plant without any risk.Keywords: Malva sylvestris L., toxic heavy metal, medicinal plant, impact on human health

Abstract:
Considering that coal mining is one of the important industrial activities, it may cause damages to environment. According to the author’s best knowledge, the effect of traditional coal mining activities on plant biodiversity has not been investigated in the Hyrcanian forests. Therefore, in this study, the effect of coal mining activities on vegetation and tree diversity was investigated in Hyrcanian forest, North Iran. After filed visiting and determining the mine, 16 plots (20×20 m2) were established by systematic-randomly (60×60 m2) in an area of 4 ha (200×200 m2-mine entrance placed at center). An area adjacent to the mine was not affected by the mining activity, and it is considered as the control area. In each plot, the data about trees such as number and type of species were recorded. The biodiversity of vegetation cover was considered 5 square sub-plots (1 m2) in each plot. PAST software and Ecological Methodology were used to calculate Biodiversity indices. The value of Shannon Wiener and Simpson diversity indices for tree cover in control area (1.04±0.34 and 0.62±0.20) was significantly higher than mining area (0.78±0.27 and 0.45±0.14). The value of evenness indices for tree cover in the mining area was significantly lower than that of the control area. The value of Shannon Wiener and Simpson diversity indices for vegetation cover in the control area (1.37±0.06 and 0.69±0.02) was significantly higher than the mining area (1.02±0.13 and 0.50±0.07). The value of evenness index in the control area was significantly higher than the mining area. Plant communities are a good indicator of the changes in the site. Study about changes in vegetation biodiversity and plant dynamics in the degraded land can provide necessary information for forest management and reforestation of these areas.Keywords: vegetation biodiversity, species composition, traditional coal mining, Caspian forest

Many heritage sites abound in the shores of Nigeria with enormous tourism potentials. Heritage sites do not only depict the cultural and historical transmutation of people but also functions in the image design and promotion of a locality. This reveals the unique role of heritage sites to structural development of an area. Heritage sites have of recent been a victim of degradation and social abuse arising from seasonal ignorance; hence minimizing its potentials to the socio-economic development of an area. This paper is emphasizing on the adoption of community development approaches in heritage preservation in Igbo area. Its modalities, applications, challenges and prospect were discussed. Abstract:
Adequate and reliable estimates of aquifer parameters are of utmost importance for proper management of vital groundwater resources. At present scenario the ground water is polluted because of industrial waste disposed over the land and the contaminants are transported in the aquifer from one area to another area which is depending on the characteristics of the aquifer and contaminants. To know the contaminant transport, the accurate estimation of aquifer properties is highly needed. Conventionally, these properties are estimated through pumping tests carried out on water wells. The occurrence and movement of ground water in the aquifer are characteristically defined by the aquifer parameters. The pumping (aquifer) test is the standard technique for estimating various hydraulic properties of aquifer systems, viz, transmissivity (T), hydraulic conductivity (K), storage coefficient (S) etc., for which the graphical method is widely used. The study area for conducting pumping test is Pydibheemavaram Industrial area near the coastal belt of Srikulam, AP, India. Abstract:
High Conservation Value (HCV) is an area with conservation function within oil palm plantation. Despite the important role of HCV area in biodiversity conservation and various studies on HCV, there was a lack of research studying the factors determining its success. A preliminary study was conducted to identify the determinant factor of HCV that affected the diversity. Line transect method was used to calculate the species diversity of butterfly, birds, mammals, and herpetofauna species as well as their richness. Specifically for mammals, camera traps were also used. The research sites comprised of 12 HCV areas in 3 provinces of Indonesia (Central Kalimantan, Riau, and Palembang). The relationship between the HCV biophysical factor with the species number and species diversity for each wildlife class was identified using Chi-Square analysis with Cross tab (contingency table). Results of the study revealed that species diversity varied by research locations. Four factors determining the success of HCV area in relations to the number and diversity of wildlife species are land cover types for mammals, the width of area and distance to rivers for birds, and distance to settlements for butterflies.Keywords: wildlife diversity, oil palm plantation, high conservation value area, ecological factors

Abstract:
Epilepsy is a common neurological disorder affecting all age groups. It is one of the world's most prevalent non-communicable diseases. Increased evidence suggesting that long term usage of anti-epileptic drugs can have adverse effects on bone mineralization and bone molding .Aiming to study these effects and to give guide lines to support bone health through early intervention. From Neurology Out-Patient Clinic kaser Elaini University Hospital, 60 Patients were enrolled, 40 patients on antiepileptic drugs for at least two years and 20 controls matched with age and sex, epileptic but before starting treatment both chosen under specific criteria. Patients were divided into four groups, three groups with monotherapy treated with either Phynetoin, Valporic acid or Carbamazipine and fourth group treated with both Valporic acid and Carbamazipine. Estimation of serum Carboxy-Terminal Telopeptide of Type I- Collagen(ICTP) bone resorption marker, serum 25(OH )vit D3, calcium ,magnesium and phosphorus were done .Results showed that all patients on AED had significant low levels of 25(OH) vit D3 (p<0.001) ,with significant elevation of ICTP (P<0.05) versus controls. In group treated with Phynotoin highly significant elevation of (ICTP) marker and decrease of both serum 25(OH) vit D3 (P<0, 0001) and serum calcium(P<0.05)versus control. Double drug group showed significant decrease of serum 25(OH) vit D3 (P<0.0001) and decrease in Phosphorus (P<0.05) versus controls. Serum magnesium showed no significant differences between studied groups. We concluded that Anti- epileptic drugs appears to be an aggravating factor on bone mineralization ,so therapeutically it can be worth wile to supplement calcium and vitamin D even before initiation of antiepileptic therapy. This research aimed to know the condition of the gap in the Malinau Regency border and its influence on the development of the border region as well as knowing the problems related to the economy development of society in the area of the border district of Malinau. Methods of analysis are used in namely descriptive analysis that represent analysis of land use and analysis of movement activities of the population, level analysis facility and infrastructure, economy analysis that represent top commodity determination analysis (LQ and Growth Share) and accessibility. The results of the study showed that the condition of the Malinau Regency border come within the gap as seen from the contributions of infrastructure repair facilities and accessibility advocates, communities, scattered seed commodities come within the borders and human resources with the condition of the most Upstream Bahau town in the backwaters of the town more. There are a few problems that cause the condition area of the border experiencing inequality, lack of human resources, poor infrastructure, lack of accessibility and low levels of security so that it brings development recommendations was the development of the flagship commodities and infrastructure as well as supporting community economic infrastructure, as well as human resources.Keywords: border, economy, development, Malinau

Abstract:
Objective: To analyze the employee compensation status of secondary public hospitals of Pudong New Area in Shanghai in order to provide information for compensation reform of public hospitals in Shanghai and as well as in China. Methods: We surveyed all 15 secondary public hospitals of Pudong New Area in Shanghai to collect hospital annual compensation data for their employees and to investigate their suggestions for compensation reform in public hospitals in China. We also collected related annual compensation data of employees in Shanghai and of physicians in the USA from Shanghai statistical Yearbook 2013 and from Bureau of Labor Statistics, U.S. Department of Labor. Results: The average annual compensation for the employees in secondary public hospitals of Pudong New Area in Shanghai in 2012 was 2.65 times of that for overall employees in Shanghai. The physician’s compensation in these public hospitals was relatively lower than that in the USA. Conclusion: The physicians’ compensation in the secondary public hospitals of Pudong New Area in Shanghai should be increased rationally and new compensation reform in public hospitals in Shanghai should be carefully designed.Keywords: human resource, compensation, public hospital, Shanghai

Abstract:
Background: Heart failure has been a rising concern in Tanzania. New drugs have been introduced, including the group of drugs called Angiotensin receptor Neprilysin Inhibitor (ARNI), but due to their high cost, angiotensin-converting enzymes inhibitors (ACEIs) and Angiotensin receptor blockers (ARBs) have been mostly used in Tanzania. However, according to our knowledge, the efficacy comparison of the two groups is yet to be studied in Tanzania. The aim of this study was to compare the efficacy of ACEIs and ARBs among patients with heart failure. Methodology: This was a hospital-based prospective cohort study done at Jakaya Kikwete Cardiac Institution (JKCI), Tanzania, from June to December 2020. Consecutive enrollment was done until fulfilling the inclusion criteria. Clinical details were measured at baseline. We assessed the relationship between ARBs and ACEIs users with N-terminal pro-brain natriuretic peptide (NT pro-BNP) levels at admission and at 1-month follow-up using a chi-square test. A Kaplan-Meier curve was used to estimate the survival time of the two groups. Results: 155 HF patients were enrolled, with a mean age of 48 years, whereby 52.3% were male, and their mean left ventricular ejection fraction (LVEF) was 37.3%. 52 (33.5%) heart failure patients were on ACEIs, 57 (36.8%) on ARBs, and 46 (29.7%) were neither using ACEIs nor ARBs. At least half of the patients did not receive a guideline-directed medical therapy (GDMT), with only 82 (52.9%) receiving a GDMT. A drop in NT pro-BNP levels was observed during admission and at 1-month follow-up on both groups, from 6389.2 pg/ml to 4000.1 pg/ml for ARB users and 5877.7 pg/ml to 1328.2 pg/ml for the ACEIs users. There was no statistical difference between the two groups when estimated by the Kaplan-Meier curve, though more deaths were observed in those who were neither on ACEIs nor ARBs, with a calculated P value of 0.01. Conclusion: This study demonstrates that ACEIs have more efficacy and overall better clinical outcome than ARBs, but this should be taken under the patient-based case, considering the side effects of ACEIs and patients’ adherence.Keywords: angiotensin converting enzymes inhibitors, angiotensin receptor blockers, guideline direct medical therapy, N-terminal pro-brain natriuretic peptide

Abstract:
Marine oil spill response operation requires extensive vessel maneuvering and navigation skills. At-sea oil containment and recovery include both single vessel and multi-vessel operations. Towing long oil containment booms that are several hundreds of meters in length, is a challenge in itself. Boom deployment and towing in multi-vessel configurations is an added challenge that requires precise coordination and control of the vessels. Efficient communication, as a prerequisite for shared situational awareness, is needed in order to execute the response task effectively. To gain and maintain adequate maritime skills, practical training is needed. Field exercises are the most effective way of learning, but especially the related vessel operations are resource-intensive and costly. Field exercises may also be affected by environmental limitations such as high sea-state or other adverse weather conditions. In Finland, the seasonal ice-coverage also limits the training period to summer seasons only. In addition, environmental sensitiveness of the sea area restricts the use of real oil or other target substances. This paper examines, whether maritime simulator training can offer a complementary method to overcome the training challenges related to field exercises. The objective is to assess the efficiency and the learning impact of simulator training, and the specific skills that can be trained most effectively in simulators. This paper provides an overview of learning results from two oil spill response pilot courses, in which maritime navigational bridge simulators were used to train the oil spill response authorities. The simulators were equipped with an oil spill functionality module. The courses were targeted at coastal Fire and Rescue Services responsible for near shore oil spill response in Finland. The competence levels of the participants were surveyed before and after the course in order to measure potential shifts in competencies due to the simulator training. In addition to the quantitative analysis, the efficiency of the simulator training is evaluated qualitatively through feedback from the participants. The results indicate that simulator training is a valid and effective method for developing marine oil spill response competencies that complement traditional field exercises. Simulator training provides a safe environment for assessing various oil containment and recovery tactics. One of the main benefits of the simulator training was found to be the immediate feedback the spill modelling software provides on the oil spill behaviour as a reaction to response measures.Keywords: maritime training, oil spill response, simulation, vessel manoeuvring

Abstract:
This paper presented the changes in temperature and air ventilation of an inner urban area at different development stages during 2002 to 2012 and the high-rise buildings to be built in 2018. 3D simulation models ENVI-met and Autodesk Falcon were used. The results indicated that replacement of old residence buildings or open space with high-rise buildings will increase the air temperature of inner urban area; the air temperature at the pedestrian level will increase more than that at the upper levels. The temperature of the inner street in future will get higher than that in 2002, 2008 and 2012. It is attributed that heat is trapped in the street canyons as the air permeability at the pedestrian levels is lower. High-rise buildings with massive podium will further reduce the air ventilation in that area. In addition, sufficient separations among buildings is essential in design. High-rise buildings aligned along the waterfront will obstruct the wind flowing into the inner urban area and accelerate the temperature increase both in daytime and night time.Keywords: micro-climate change, urban design, ENVI-met, construction engineering

Abstract:
Egypt is known as cradle of civilization due to its ancient history and archeological sites, but Egypt possess also a cradle of Geo-sites, which qualify it to be listed as one of the most important Geo-heritage sites all over the country. Geology and landscape in Abu-Roash area is considered as one of the most important geological places (geo-sites) inside Cairo which help us to know and understand geology and geologic processes, so the area is used mainly for geological education purposes, also the area contain an archeological sites; pyramid complex, tombs, and Coptic monastery which give the area unique importance. Abu-Roash area is located inside Cairo 9 km north of the Giza Pyramids, which make the accessibility to the area easy and safe, the geology of Abu-Roash constitutes a complex Cretaceous sedimentary succession mass with showing outstanding tectonic features (Syrian Arc system event), these features are considered as a Geo-heritage, which will be the main designation of ‘Geo-parks’ establishing. The research is dealing with the numerous geo-sites found in the area, and its geologic and archeological importance, the relation between geo-sites and archeology, also the research proposed a detailed maps for these sites depicting Geo-routes and the hazardous places surrounding Abu-Roash area. The research is proposing a new proposal not applied in Egypt before, establishing a Geo-park, to promote this unique geo-heritage from hazardous factors and anthropogenic effects, also it will offer geo-educational opportunities to the general public and to the scientific community, enhancement of Geo-tourism which will be linked easily with the Ancient Egyptian tourism, it will also provide a significant economic benefit to Abu-Roash residential area. We studied the selection of winter habitat by Persian Gazelle (Gazella subguttrosa) in Bydoyeh protected area. Habitat variables such as plant species number, vegetation percent, distance to the nearest water sources and plant patch of present sites were compared with randomly selected non- used sites. The results showed that the most important factors influencing habitat selection were number and vegetation percent of Artemisia sieberi. In order to achieve high data rate, we propose a cooperative scheme that employs hierarchical modulation. Bekasi Industrial Area in Kab. Bekasi and SEZ (Special Economic Zone) Sei Mangkei in Kab. Simalungun are two areas whose have the same main economic activity that are manufacturing industrial. Manufacturing industry in Bekasi Industrial Area contributes more than 70% of Kab. Bekasi’s GDP, while manufacturing industry in SEZ Sei Mangkei contributes less than 20% of Kab. Simalungun’s GDP. The dependent variable in the research is labor productivity, while the independent variable is the amount of labor, the level of labor education, the length of work and salary. This research used linear regression method to find the model for represent actual condition of productivity in two industrial area, then the equalization using dummy variable on labor education level variable. The initial hypothesis (Ho) in this research is that labor productivity in Bekasi Industrial Area will be higher than the productivity of labor in SEZ Sei Mangkei.
