Procedia PDF Downloads 25955 Performance Parameters of an Abbreviated Breast MRI Protocol
Authors: Andy Ho
Abstract:
Breast cancer is a common cancer in Australia. Early diagnosis is crucial for improving patient outcomes, as later-stage detection correlates with poorer prognoses. While multiparametric MRI offers superior sensitivity in detecting invasive and high-grade breast cancers compared to conventional mammography, its extended scan duration and high costs limit widespread application. As a result, full protocol MRI screening is typically reserved for patients at elevated risk. Recent advancements in imaging technology have facilitated the development of Abbreviated MRI protocols, which dramatically reduce scan times (<10 minutes compared to >30 minutes for full protocol). The potential for Abbreviated MRI to offer a more time- and cost-efficient alternative has implications for improving patient accessibility, reducing appointment durations, and enhancing compliance—especially relevant for individuals requiring regular annual screening over several decades. The purpose of this study is to assess the diagnostic efficacy of Abbreviated MRI for breast cancer screening among high-risk patients at the Royal Prince Alfred Hospital (RPA). This study aims to determine the sensitivity, specificity, and inter-reader variability of Abbreviated MRI protocols when interpreted by subspecialty-trained Breast Radiologists. A systematic review of the RPA’s electronic Picture Archive and Communication System identified high-risk patients, defined by Australian ‘Medicare Benefits Schedule’ criteria, who underwent Breast MRI from 2021 to 2022. Eligible participants included asymptomatic patients under 50 years old referred by the High-Risk Clinic due to a high-risk genetic profile or relevant familial history. The MRIs were anonymized, randomized, and interpreted by four Breast Radiologists, each independently completing standardized proforma evaluations. Radiological findings were compared against histopathology as the gold standard or follow-up imaging if biopsies were unavailable. Statistical metrics, including sensitivity, specificity, and inter-reader variability, were assessed. The Fleiss-Kappa analysis demonstrated a fair inter-reader agreement (kappa = 0.25; 95% CI: 0.19–0.32; p < 0.0001). The sensitivity for detecting malignancies was 0.75, with a specificity of 0.84. These findings underline the potential of Abbreviated MRI as a reliable screening tool for malignancies with significant specificity, though reduced sensitivity highlights the importance of robust radiologist training and consistent evaluation standards. Abbreviated MRI protocols exhibit promise as a viable screening option for high-risk patients, combining reduced scan times and acceptable diagnostic accuracy. Further work to refine interpretation practices and optimize training is essential to maximize the protocol’s utility in routine clinical screening and facilitate broader accessibility. Keywords: abbreviated, breast, cancer, MRI

Procedia PDF Downloads 44551 Food Insecurity and Its Implication for Poverty Alleviation in Nigeria
Authors: Peter Okpamen
Abstract:
Food security concentrates on the collective efforts of all nations to produce enough food to feed their people. Recently, though the emphasis shifted from food availability to accessibility constraints, which entails the difficulties undernourished people face in gaining access to food even when it is available. Broadly speaking, access to food depends on an individual’s access to resources, markets and food transfers. The opportunities to obtain food through these channels are entitlements, which when denied constitute food insecurity. Evidence shows that a significant percentage of Nigerians are undernourished with adverse implications for the fight against poverty. The greatest danger or consequence of food insecurity is malnutrition. Food insecurity as both an agent and consequence of poverty also increases the economic, political and social tensions in the country. The undernourished in Nigeria are marginalised in several ways to the extent that they are often ill; and because of illness, their work capacity is reduced with attendant reduction in their income. Without adequate income, they cannot save nor invest enough resources to take care of their basic needs. In this paper therefore, we used the political economy approach and statistical analysis to demonstrate that poverty alleviation in Nigeria would be a mirage if food security problems are not adequately resolved. Keywords: alleviation, demographic, food insecurity, undernourished

Procedia PDF Downloads 32747 S/Pace: Discontinuing the Otherness of the Other in Travel
Authors: Okikiola Olusanu
Abstract:
Power dynamics, one of the lasting effects of the legacies of coloniality, left an indelible scar on our identity and space. This structure ensures a system that deliberately slows down the pace of the colonizer, either within her space or while traveling the world of the colonizer. The politics of oppression through the intersection of race, gender, class, and sex empowers the ideology of sameness and difference. This ideology regulates and sustains the borderlines of the colonizers’ space. Sociologists, anti-colonialists, and feminist theorists have argued that the restrictions that the colonized experience while traveling to the space of the colonizers can be interpreted as an inter-body war that reinforces gender, class, and race inequalities. To foster belonging and accessibility, this study examines the effect of colonial legacy restrictions on traveling. This paper aims to deconstruct the coloniality of knowledge, space, and body for a transnational and decolonial identity transcending borders. it argue that borders are intentionally constructed to keep the other at a significant distance, like identity. We deduce that travel restriction is antibody because the pace is slow, and access is limited. For the analysis of this study, this study examines how the perception of the body of the colonizer influence her travail when traveling to and through the space of the ex-colonizer and the measures necessary for the decolonization of knowledge, space and body. Our interest is not in moving physical borders from space, it is rather in decolonizing the mind that create systematic, social, and political borders. Keywords: space, body, travel, identity

Procedia PDF Downloads 32844 An Equitable Strategy to Amend Zero-Emission Vehicles Incentives for Travelers: A Policy Review
Authors: Marie Louis
Abstract:
Even though many stakeholders are doing their very best to promote public transportation around the world, many areas are still public transportation non-accessible. With travelers purchasing and driving their private vehicles can be considered as a threat to all three aspects of the sustainability (e.g., economical, social, environmental). However, most studies that considered simultaneously all three aspects of the sustainability concept when planning and designing public transportation for a corridor have found tradeoffs among the said three aspects. One of the tradeoffs was identified by looking at tipping points of the travel demands to question whether transit agencies/and or transportation policymakers should either operate smaller buses or provide incentives to purchase Leadership in Energy and Environmental Design (LEED)-Qualified low-emission vehicles or greener vehicles (e.g., hybrid). However, how and when do the department of environmental protection (DEP) and the department of revenue (DOR) figure out how much incentives to give to each traveler who lives in a zoning that is considered as public transportation inaccessible or accessible? To answer this policy question, this study aims to compare the greenhouse gases (GHGs) emissions when hybrid and conventional cars are used to access public transportation stops/stations. Additionally, this study also intends to review previous states that have already adopted low-emissions vehicle (LEVs) or Zero-Emissions Vehicles (ZEVs) to diminish the daily GHGs pollutants. Keywords: LEED-qualified vehicles, public transit accessibility, hybrid vehicles incentives, sustainability trade-offs
Procedia PDF Downloads 19543 Mobile Assembly of Electric Vehicles: Decentralized, Low-Invest and Flexible
Authors: Achim Kampker, Kai Kreiskoether, Johannes Wagner, Sarah Fluchs
Abstract:
The growing speed of innovation in related industries requires the automotive industry to adapt and increase release frequencies of new vehicle derivatives which implies a significant reduction of investments per vehicle and ramp-up times. Emerging markets in various parts of the world augment the currently dominating established main automotive markets. Local content requirements such as import tariffs on final products impede the accessibility of these micro markets, which is why in the future market exploitation will not be driven by pure sales activities anymore but rather by setting up local assembly units. The aim of this paper is to provide an overview of the concept of decentralized assembly and to discuss and critically assess some currently researched and crucial approaches in production technology. In order to determine the scope in which complementary mobile assembly can be profitable for manufacturers, a general cost model is set up and each cost driver is assessed with respect to varying levels of decentralization. One main result of the paper is that the presented approaches offer huge cost-saving potentials and are thus critical for future production strategies. Nevertheless, they still need to be further exploited in order for decentralized assembly to be profitable for companies. The optimal level of decentralization must, however, be specifically determined in each case and cannot be defined in general. Keywords: automotive assembly, e-mobility, production technology, release capability, small series assembly
Procedia PDF Downloads 20142 Subway Stray Current Effects on Gas Pipelines in the City of Tehran
Authors: Mohammad Derakhshani, Saeed Reza Allahkarama, Michael Isakhani-Zakaria, Masoud Samadian, Hojjat Sharifi Rasaey
Abstract:
In order to investigate the effects of stray current from DC traction systems (subway) on cathodically protected gas pipelines, the subway and the gas network maps in the city of Tehran were superimposed and a comprehensive map was prepared. 213 intersections and about 100150 meters of parallel sections of gas pipelines were found with respect to the railway right of way which was specified for field measurements. The potential measurements data were logged for one hour in each test point. 24-hour potential monitoring was carried out in selected test points as well. Results showed that dynamic stray current from subway on pipeline potential appears as fluctuations in its static potential that is visible in the diagrams during night periods. These fluctuations can cause the pipeline potential to exit the safe zone and lead to corrosion or overprotection. In this study, a maximum potential shift of 100 mv in the pipe-to-soil potential was considered as a criterion for dynamic stray current effective presence. Results showed that a potential fluctuation range between 100 mV to 3 V exists in measured points on pipelines which exceeds the proposed criterion and needs to be investigated. Corrosion rates influenced by stray currents were calculated using coupons. Results showed that coupon linked to the pipeline in one of the locations at region 1 of the city of Tehran has a corrosion rate of 4.2 mpy (with cathodic protection and under influence of stray currents) which is about 1.5 times more than free corrosion rate of 2.6 mpy. Keywords: stray current, DC traction, subway, buried Pipelines, cathodic protection list

Authors: Beatrice Eyram Afi Ziorklui, Norihisa Shima
Abstract:
Community Driven Development (CDD) is now a widely recommended and accepted development strategy for informal communities across the continent. Centered on the utilization of social capital through community structures, different informal settlements have different structures and different levels of social capital, which affect the implementation and ability to overcome CDD challenges. Although known to be very successful, there are few perspectives on the implementation of CDD initiatives in different informal settlements. This study assesses the implementation of CDD initiatives in migrant and indigenous informal settlements and their ability to navigate challenges. The case study research design was adopted in this research, and respondents were chosen through simple random sampling. Using the Statistical Package for social scientists (SPSS) for data analysis, the study found that migrant informal settlements implement CDD projects through the network of hierarchical structures based on government systems, whereas indigenous informal settlements implement through the hierarchical social structure based on traditions and culture. The study also found that, with the exception of the challenge of land accessibility in migrant informal settlements, all other challenges, such as participation, resource mobilization, and maintenance, have a significant relationship with social capital, although indigenous informal settlements have higher levels of social capital than migrant informal settlements. The study recommends a framework that incorporates community characteristics and the underlying social capital to facilitate upgrading strategies in informal in Ghana. Keywords: community driven development, informal settlements, social capital, upgrading

Abstract:
As internet accessibility and smartphones continue to increase in Nigeria, Africa’s most populous country, social media platforms have become ubiquitous, causing students of 18-25 age brackets to spend more time on social media. The research investigated the impact of prolonged social media use on the physical health of students, with a specific focus on computer vision syndrome, hand pain, headaches and mental status. The study adopted a mixed-methods approach combining quantitative surveys to gather statistical data on usage patterns and symptoms, along with qualitative interviews into the experiences and perceptions of medical practitioners concerning cases under study within the geopolitical region. The result was analyzed using Regression analysis. It was observed that there is a significant correlation between social media usage by the students in the study age bracket concerning computer vision syndrome, hand pain, headache and general mental status. The research concluded by providing valuable insights into potential interventions and strategies to mitigate the adverse effects of excessive social media use on student well-being and recommends, among others, that educational institutions, parents, and students themselves collaborate to implement strategies aimed at promoting responsible and balanced use of social media. Keywords: social media, student health, computer vision syndrome, hand pain, headaches, mental staus
