5258 An Efficient Stud Krill Herd Framework for Solving Non-Convex Economic Dispatch Problem

Abstract:
The problem of economic dispatch (ED) is the basic problem of power framework, its main goal is to find the most favorable generation dispatch to generate each unit, reduce the whole power generation cost, and meet all system limitations. A heuristic algorithm, recently developed called Stud Krill Herd (SKH), has been employed in this paper to treat non-convex ED problems. The proposed KH has been modified using Stud selection and crossover (SSC) operator, to enhance the solution quality and avoid local optima. We are demonstrated SKH effects in two case study systems composed of 13-unit and 40-unit test systems to verify its performance and applicability in solving the ED problems. In the above systems, SKH can successfully obtain the best fuel generator and distribute the load requirements for the online generators. The results showed that the use of the proposed SKH method could reduce the total cost of generation and optimize the fulfillment of the load requirements.Keywords: stud krill herd, economic dispatch, crossover, stud selection, valve-point effect

Abstract:
A study has been carried out to see the prospect of wind power potential of southern Sindh namely Karachi, Hawksbay, Norriabad, Hyderabad, Ketibander and Shahbander using local wind speed data. The monthly average wind speed for these area ranges from 4.5m/sec to 8.5m/sec at 30m height from ground. Extractable wind power, wind energy and Weibul parameter for above mentioned areas have been examined. Furthermore, the power output using fast and slow wind machine using different blade diameter along with the 4Kw and 20 Kw aero-generator were examined to see the possible use for deep well pumping and electricity supply to remote villages. The analysis reveals that in this wind corridor of southern Sindh Hawksbay, Ketibander and Shahbander belongs to wind power class-3 Hyderabad and Nooriabad belongs to wind power class-5 and Karachi belongs to wind power class-2. The result shows that the that higher wind speed values occur between June till August. It was found that considering maximum wind speed location, Hawksbay,Noriabad are the best location for setting up wind machines for power generation.Keywords: wind energy generation, Southern Sindh, seasonal change, Weibull parameter, wind machines

Abstract:
Today, financial performance has become one of the important issues in accounting and auditing that companies and their managers have paid attention to this issue and for this reason to the variables that are influential in this field. One of the things that can affect financial performance is corporate governance, which is examined in this research, although some things such as issues related to auditing can also moderate this relationship; Therefore, this research has been conducted with the aim of investigating the relationship between corporate governance and financial performance with regard to the moderating role of feedback and internal control weakness. The research is practical in terms of purpose, and in terms of method, it has been done in a post-event descriptive manner, in which the data has been analyzed using stock market data. Data collection has been done by using stock exchange data which has been extracted from the website of the Iraqi Stock Exchange, the statistical population of this research is all the companies admitted to the Iraqi Stock Exchange. . The statistical sample in this research is considered from 2014 to 2021, which includes 34 companies. Four different models have been considered for the research hypotheses, which are eight hypotheses, in this research, the analysis has been done using EXCEL and STATA15 software. In this article, collinearity test, integration test ,determination of fixed effects and correlation matrix results, have been used. Abstract:
Solar aided power generation (SAPG) technology has been proven as an efficient way to make use of solar energy for power generation purpose. In an SAPG plant, a solar field consisting of parabolic solar collectors is normally used to supply the solar heat in order to displace the high pressure/temperature extraction steam. To understand the performance of such a SAPG plant, a new simulation model was developed by the authors recently, in which the boiler was treated, as a series of heat exchangers unlike other previous models. Through the simulations using the new model, it was found the outlet properties of reheated steam, e.g. temperature, would decrease due to the introduction of the solar heat. The changes make the (lower stage) turbines work under off-design condition. As a result, the whole plant’s performance may not be optimal. In this paper, the second solar filed was proposed to increase the inlet temperature of steam to be reheated, in order to bring the outlet temperature of reheated steam back to the designed condition. A 600MW SAPG plant was simulated as a case study using the new model to understand the impact of the second solar field on the plant performance. It was found in the study, the 2nd solar field would improve the plant’s performance in terms of cycle efficiency and solar-to-electricity efficiency by 1.91% and 6.01%. The solar-generated electricity produced by per aperture area under the design condition was 187.96W/m2, which was 26.14% higher than the previous design.Keywords: solar-aided power generation system, off-design performance, coal-saving performance, boiler modelling, integration schemes

Abstract:
Vegetables harvested early in the morning or late in the afternoon are valued in plant production, and so the time of harvest is important. The biological functions known as circadian clocks have a significant effect on this harvest timing. The purpose of this study was to non-destructively estimate the circadian clock and so construct a method for determining a suitable harvest time. We took eight samples of green busil (Perilla frutescens var. crispa) every 4 hours, six times for 1 day and analyzed all samples at the same time. A hyperspectral camera was used to collect spectrum intensities at 141 different wavelengths (350–1050 nm). Calculation of correlations between spectrum intensity of each wavelength and harvest time suggested the suitability of the hyperspectral camera for non-destructive estimation. However, even the highest correlated wavelength had a weak correlation, so we used machine learning to raise the accuracy of estimation and constructed a machine learning model to estimate the internal time of the circadian clock. Artificial neural networks (ANN) were used for machine learning because this is an effective analysis method for large amounts of data. Using the estimation model resulted in an error between estimated and real times of 3 min. The estimations were made in less than 2 hours. The Republic of Korea has been facing strong storage crisis from nuclear waste generation as At Reactor (AR) temporary storage sites are about to reach saturation. Since the country is densely populated with a rate of 491.78 persons per square kilometer, Construction of High-level waste repository will not be a feasible option. In order to tackle the storage waste generation problem which is increasing at a rate of 350 tHM/Yr. and 380 tHM/Yr. in case of 20 PWRs and 4 PHWRs respectively, the study strongly focuses on the advancement of current nuclear power plants to GEN-IV sustainable and ecological nuclear systems by burning TRUs (Pu, MAs). First, Calculations has made to estimate the generation of SNF including Pu and MA from PWR and PHWR NPPS by using the IAEA code Nuclear Fuel Cycle Simulation System (NFCSS) for the period of 2016, 2030 (including the saturation period of each site from 2024~2028), 2089 and 2109 as the number of NPPS will increase due to high import cost of non-nuclear energy sources. 2ndly, in order to produce environmentally sustainable nuclear energy systems, 4 scenarios to burnout the Plutonium and MAs are analyzed with the concentration on burning of MA only, MA and Pu together by utilizing SFR, LFR and KALIMER-600 burner reactor after recycling the spent oxide fuel from PWR through pyro processing technology developed by Korea Atomic Energy Research Institute (KAERI) which shows promising and sustainable future benefits by minimizing the HLW generation with regard to waste amount, decay heat, and activity. Finally, With the concentration on front and back end fuel cycles for open and closed fuel cycles of PWR and Pyro-SFR respectively, an overall assessment has been made which evaluates the quantitative as well as economical combativeness of SFR metallic fuel against PWR once through nuclear fuel cycle.Keywords: GEN IV nuclear fuel cycle, nuclear waste, waste sustainability, transmutation

Abstract:
Laboratory studies of the stress-strain behavior of rocks specimens were conducted by using acoustic emission and laser-ultrasonic diagnostics. The sensitivity of the techniques allowed changes in the internal structure of the specimens under uniaxial compressive load to be examined at micro- and macro scales. It was shown that microcracks appear in geologic materials when the stress level reaches about 50% of breaking strength. Also, the characteristic stress of the main crack formation was registered in the process of single-stage compression of rocks. On the base of laser-ultrasonic echoscopy, 2D visualization of the internal structure of rocky soil specimens was realized, and the microcracks arising during uniaxial compression were registered.Keywords: acoustic emission, geomaterial, laser ultrasound, uniaxial compression

Abstract:
Wind speed forecasting is an important issue for planning wind power generation facilities. The accuracy in the wind speed prediction allows a good performance of wind turbines for electricity generation. A model based on artificial neural networks is presented in this work. A dataset with atmospheric information about air temperature, atmospheric pressure, wind direction, and wind speed in Pachuca, Hidalgo, México, was used to train the artificial neural network. The data was downloaded from the web page of the National Meteorological Service of the Mexican government. The records were gathered for three months, with time intervals of ten minutes. This dataset was used to develop an iterative algorithm to create 1,110 ANNs, with different configurations, starting from one to three hidden layers and every hidden layer with a number of neurons from 1 to 10. Each ANN was trained with the Levenberg-Marquardt backpropagation algorithm, which is used to learn the relationship between input and output values. The model with the best performance contains three hidden layers and 9, 6, and 5 neurons, respectively; and the coefficient of determination obtained was r²=0.9414, and the Root Mean Squared Error is 1.0559. In summary, the ANN approach is suitable to predict the wind speed in Pachuca City because the r² value denotes a good fitting of gathered records, and the obtained ANN model can be used in the planning of wind power generation grids.Keywords: wind power generation, artificial neural networks, wind speed, coefficient of determination

The Burkh anticline with a length of 50 km and a width of 9 km is located 40 km to the north of Bastak in internal Fars zone in folded-trusted belt of Zagros. In order to assess the active tectonics in the area of study, morphometrical indexes such as V indexes (V), ratio of valley floor to valley width (Vf), the stream length-gradient ratio (Sl), channel sinuosity indexes (S), mountain front faceting indexes (F%) and mountain front sinuosity(Smf) have been studied. These investigations show that the activity is not equal in various sections of the length of Burkh anticline. Abstract:
The aim of this study is to determine key factors and processes for multinationals (MNCs) to develop an effective stakeholder engagement and sustainable development framework. A qualitative multiple-case approach was used. A triangulation method was adopted (interviews, archival documents and observations) to collect data on three global firms (MNCs). 9 senior executives were interviewed for this study (3 from each firm). An initial literature review was conducted to explore possible practices and factors (the deductive approach) to sustainable development. Interview data were analysed using Nvivo to obtain appropriate nodes and themes for the framework. A comparison of findings from interview data and themes, factors developed from the literature review and cross cases comparison were used to develop the final conceptual framework (the inductive approach). The results suggested that stakeholder engagement is a key mediator between ‘stakeholder network’ (internal and external factors) and outcomes (corporate social responsibility, social capital, shared value and sustainable development). Key internal factors such as human capital/talent, technology, culture, leadership and processes such as collaboration, knowledge sharing and co-creation of value with stakeholders were identified. These internal factors and processes must be integrated and aligned with external factors such as social, political, cultural, environment and NGOs to achieve effective stakeholder engagement.Keywords: stakeholder, engagement, sustainable development, shared value, corporate social responsibility

This paper presents a unified framework for assessment of reserve-based reliability levels in electric power systems. The unified approach is based on reserve-based analysis and assessment of the relationship between available generation capacities and required demand levels. The developed approach takes into account the load variations as well as contingencies which occur randomly causing some generation and/or transmission capacities to be lost (become unavailable). The calculated reserve based indices, which are important to assess the reserve capabilities of the power system for various operating scenarios are therefore probabilistic in nature. They reflect the fact that neither the load levels nor the generation or transmission capacities are known with absolute certainty. They are rather subjects to random variations and consequently. The calculated reserve-based reliability indices are all subjects to random variations where only expected values of these indices can be evaluated. This paper presents a unified approach to reserve-based reliability assessment of power systems using various reserve assessment criteria. Abstract:
Migration has a long history in Afghanistan even before, but it has been exacerbated in the last decade. Using actual household data of 1060 in Mazar-i-Sharif, the capital of Balkh province, obtained from a strictly random process, the study examined to evaluate the main causes and benefits of the migration. It is found that the main reasons for internal migration are unemployment and income inequality, in addition to war and poverty as international parameters for migration. Furthermore, the study demonstrated that households receive benefits from their migrants through remittances to increase their income and smooth consumption. Thus, the study suggests that to manage migration in Afghanistan, the government and international organizations should work together for peace and reduction of poverty in Afghanistan otherwise, the crisis of migration will continue in the future as well.Keywords: migration, remittances, socio-demographic, household, Afghanistan

Gas holdup data were obtained from measured pressure drop values in a gas-liquid upflow bubble column in the presence of string of hemispheres promoter internal. The parameters that influenced the gas holdup are gas velocity, liquid velocity, promoter rod diameter, pitch and base diameter of hemisphere. Tap water was used as liquid phase and nitrogen as gas phase. About 26 percent in gas holdup was obtained due to the insertion of promoter in in the present study in comparison with empty conduit. Pitch and rod diameter have not shown any influence on gas holdup whereas gas holdup was strongly influenced by gas velocity, liquid velocity and hemisphere base diameter. Abstract:
We present Dido, a source-to-source auto-generation and optimization framework for multi-dimensional stencil computations. It enables a large programmer community to easily and safely implement stencil codes on distributed-memory parallel architectures with Ordered Read-Write Locks (ORWL) as an execution and communication back-end. ORWL provides inter-task synchronization for data-oriented parallel and distributed computations. It has been proven to guarantee equity, liveness, and efficiency for a wide range of applications, particularly for iterative computations. Dido consists mainly of an implicitly parallel domain-specific language (DSL) implemented as a source-level transformer. It captures domain semantics at a high level of abstraction and generates parallel stencil code that leverages all ORWL features. The generated code is well-structured and lends itself to different possible optimizations. In this paper, we enhance Dido to handle both Jacobi and Gauss-Seidel grid traversals. We integrate temporal blocking to the Dido code generator in order to reduce the communication overhead and minimize data transfers. To increase data locality and improve intra-node data reuse, we coupled the code generation technique with the polyhedral parallelizer Pluto. The accuracy and portability of the generated code are guaranteed thanks to a parametrized solution. The combination of ORWL features, the code generation pattern and the suggested optimizations, make of Dido a powerful code generation framework for stencil computations in general, and for distributed-memory architectures in particular. Background: Parts of researchers assert that external hyperactivity behaviors of ADHD children interfere with their abilities to perform internal cognitive tasks; however, there are still other researchers hold the opposite viewpoint, the external high level of activity may serve as the role of improving internal executive function.Objectives: Thisstudy explored the effects of external motor behavior of ADHD on internal visual attentional performance. Methods: A randomized, two-period crossover design was used in this study, a total of 80 children (aged 6-12) were recruited in this study. 40participants have received ADHD diagnosis, and others are children with typically developing. These children were measured through the visual edition of TOVA (The Test of Variables of Attention) when they wore actigraphy, their testing behavior and movement data werecollected through closely observation and the actigraphies under different research conditions. Result: According to the research result, the author found (1) Higherfrequencyof movement under attentional testing condition was found in children with ADHD, comparing to children with typically developing, and (2) Higher frequency of foot movement showed better attentional performance of the visual attentional test in children with ADHD. However, these results were not showed in children with typically developing. Conclusions: The findings support the functional working memory model, which advocated that a positive relation between gross motor activity and attentional performance within the context of attentive behavior in children with ADHD.Keywords: ADHD, movement, visual attention, children

Abstract:
A bubbly flow in a vertical miniature tube is analyzed theoretically. The liquid and gas phase are co-current flowing upward. The gas phase is injected via a nozzle whose inner diameter is 0.11mm and it is placed on the axis of the tube. A force balance is applied on the bubble at its detachment. The set of governing equations are solved by use of Mathematica software. The bubble diameter and the bubble generation frequency are determined for various inlet phase velocities represented by the inlet mass quality. Abstract:
This study uses a three-dimensional (3D) fully nonlinear model to simulate the wave generation problem caused by the movement of the seabed. The numerical model is first simplified into two dimensions and then compared with the existing two-dimensional (2D) experimental data and the 2D numerical results of other shallow-water wave models. Results show that this model is different from the earlier shallow-water wave models, with the phase being closer to the experimental results of wave propagation. The results of this study are also compared with those of the 3D experimental results of other researchers. Satisfactory results can be obtained in both the waveform and the flow field. This study assesses the application of the model to simulate the wave caused by the circular (radius r0) terrain rising or falling (moving distance bm). The influence of wave-making parameters r0 and bm are discussed. This study determines that small-range (e.g., r0 = 2, normalized by the static water depth), rising, or sinking terrain will produce significant wave groups in the far field. For large-scale moving terrain (e.g., r0 = 10), uplift and deformation will potentially generate the leading solitary-like waves in the far field.Keywords: seismic wave, wave generation, far-field waves, seabed deformation

In this study, a new method for solving sequence alignment problem is proposed, which is named ITS (Improved Tabu Search). This algorithm is based on the classical Tabu Search (TS). ITS is implemented in order to obtain results of multiple sequence alignment. Several ideas concerning neighbourhood generation, move selection mechanisms and intensification/diversification strategies for our proposed ITS is investigated. Material parameters have been considered as power functions. They play important role in the elastoplastic behavior of these materials. To clarify their role, different materials with different parameters have been used under temperature gradient. Finally, their effect and loading effect have been determined in first yield point. Also, the important role of temperature gradient was also shown. At the end the study has been results obtained from changes in the elastic modulus and yield stress. Abstract:
Hydropower and solar photovoltaic (PV) generation are crucial in sustainability and transitioning from fossil fuel to clean energy. Integrating renewable energy sources such as hydropower and solar photovoltaic (PV) into the distributed networks contributes to achieving energy balance, pollution mitigation, and cost reduction. Frequent power outages and a lack of load reliability characterize the current South Sudan electricity distribution system. The country’s electricity demand is 300MW; however, the installed capacity is around 212.4M. Insufficient funds to build new electricity facilities and expand generation are the reasons for the gap in installed capacity. The South Sudan Ministry of Energy and Dams gave a contract to an Egyptian Elsewedy Electric Company that completed the construction of a solar PV plant in 2023. The plant has a 35 MWh battery storage and 20 MW solar PV system capacity. The construction of Juba Solar PV Park started in 2022 to increase the current installed capacity in Juba City to 53 MW. The Financial Service Authority (FSA) was formed as a response to the 1997 monetary crisis and the 2008 financial crisis so that it was more defensive in nature while developments in information and communication technology have required state policies to be more offensive to keep up with times. Reconstruction of Authorities of the FSA's Investigator is intended to keep the agency worthy to be part of an integrated criminal justice system in Indonesia which has implications for expanding its authority in line with efforts to protect and increase the welfare of the people. The results show that internal synergy between sub-sectors in the financial services sector is not optimised, some are even left behind so that the FSA is not truly an authority in the financial services sector. This research method is empirical. Abstract:
We focus on internal stress and displacement of an elastic axisymmetric contact problem for indentation of a layer-substrate body. An elastic layer is assumed to be perfectly bonded to an elastic semi-infinite substrate. The elastic layer is smoothly indented with a flat-ended cylindrical indenter. The analytical and exact solutions were obtained by solving an infinite system of simultaneous equations using the method to express a normal contact stress at the upper surface of the elastic layer as an appropriate series. This paper presented the numerical results of internal stress and displacement distributions for hard-coating system with constant values of Poisson’s ratio and the thickness of elastic layer.Keywords: indentation, contact problem, stress distribution, coating materials, layer-substrate body

Abstract:
There are several solutions to supply electricity to an island in Thailand such as diesel generation, submarine power cable, and renewable energy power generation. However, each alternative has its own limitation like fuel and pollution of diesel generation, submarine power cable length resulting in loss of cable and cost of investment, and potential of renewable energy in the local area. This paper shows microgrid system which is a new alternative for power supply to an island. It integrates local power plant from renewable energy, energy storage system, and microgrid controller. The suitable renewable energy power generation on an island is selected from geographic location and potential evaluation. Thus, photovoltaic system and hydro power plant are taken into account. The capacity of energy storage system is also estimated by transient stability study in order to supply electricity demand sufficiently under normal condition. Microgrid controller plays an important role in conducting, communicating and operating for both sources and loads on an island so that its functions are discussed in this study. The conceptual design of microgrid operation is investigated in order to analyze the reliability and power quality. The result of this study shows that microgrid is able to operate in parallel with the main grid and in case of islanding. It is applicable for electricity supply to an island and a remote area. The advantages of operating microgrid on an island include the technical aspect like improving reliability and quality of power system and social aspects like outage cost saving and CO₂ reduction.Keywords: energy storage, islanding, microgrid, renewable energy

Abstract:
This work presents the theoretical investigation of an enhanced second-harmonic generation of higher-order Gaussian laser beam in plasma having a density ramp. The mechanism responsible for the self-focusing of a laser beam in plasma is considered to be the relativistic mass variation of plasma electrons under the effect of a highly intense laser beam. Using the moment theory approach and considering the Wentzel-Kramers-Brillouin approximation for the non-linear Schrodinger wave equation, the differential equation is derived, which governs the spot size of the higher-order Gaussian laser beam in plasma. The nonlinearity induced by the laser beam creates the density gradient in the background plasma electrons, which is responsible for the excitation of the electron plasma wave. The large amplitude electron plasma wave interacts with the fundamental beam, which further produces the coherent radiations with double the frequency of the incident beam. The analysis shows the important role of the different modes of higher-order Gaussian laser beam and density ramp on the efficiency of generated harmonics.Keywords: density rippled plasma, higher order Gaussian laser beam, moment theory approach, second harmonic generation.

Abstract:
Diazepam (known as valium) is a medication for calming effect. Many reports on committed suicide cases shown that diazepam is frequently used for this purpose. This research aims to study effect of diazepam on the development of forensically important blowflies, Chrysomya megacephala (Diptera: Calliphoridae) using micro-computed tomography (micro CT). In this study, four rabbits were treated with three different lethal doses of diazepam and one control (LD₀, LD₅₀, LD₁₀₀ and LC). The rabbit’s livers were removed for rearing the blowflies. Pupae were sampled for two series (ages; S1: 24h and S2: 120h) of development. After preparing the specimens, all samples were performed Micro CT using Skyscan 1172. The results shown the effect of diazepam on internal organs and tissues such as brain, cavity of the body, gas bubble, meconium and especially fat body. In the control group, in series 1 (LCS1), fat body was equally dispersed in the head, thorax, and abdomen, development of internal organs were not completed, however, brain, thoracic muscle, wings, legs and rectum were able to observe at 24h after developing into the pupal stage. Development of each organ in the control group in the series two was completed. In the treatment groups, LD₀, LD₅₀, LD₁₀₀ (Series 1 and Series 2), tissues are different, such as gas bubble in LD₀S1, was observed due to rapidity morphological changes during the metamorphosis of blowfly’s pupa in this treatment. Meconium was observed in LD₅₀S2 group because excretion of metabolic waste was not completed. All of the samples in the treatment groups had differentiation of fat bodies because metabolic activities were not completed and these changes affected on functions of every internal system. Discovering of differentiated fat bodies are important results because fat bodies of insect functions as liver in human, therefore it is shown that toxin eliminates from blowfly’s body and homeostatic maintenance of the hemolymph proteins, lipid and carbohydrates in each treatment group are abnormal.Keywords: forensic toxicology, forensic entomology, diptera, diazepam

In this paper, design details, theoretical analysis and thermal performance analysis of a solar energy concentrator suited to combined heat and thermoelectric power generation are presented. The thermoelectric device is attached to the absorber plate to convert concentrated solar energy directly into electric energy at the focus of the concentrator. Abstract:
Baixo Alentejo is a region of Portugal characterized by an aging population, geographic isolation, social deprivation and a lack of medical staff. It is one of the most problematic regions in regards to mental health, particularly due to the factors mentioned. The aim of this study is a presentation of liaison psychiatry in Hospital José Joaquim Fernandes; a sample of the work done, the current situation and future perspectives. The aim is to present a retrospective study of internal psychiatric emergencies from January 1st, 2016 to August 31st, 2016. Liaison psychiatry of Department of Psychiatry and Mental Health (Psychiatry Service) of ULSBA includes the following activities: internal psychiatry emergencies, HIV consultation (comprised in the general consultation) and liaison psychology (oncology and pain), consisting of a total of 111 internal psychiatry emergencies during the identified period. Gender distribution was uniform. The most prevalent age group was 71-80 years, and 66,6% of patients were 60 years old and over. The majority of the emergency observations was requested by hospital services of medicine (56,8%) and surgery (24,3%). The most frequent reasons for admission were: respiratory disease (18,0%); tumors (15.3%); other surgical and orthopedic pathology (14,5%) and stroke (11,7%). The most frequent psychiatric diagnoses were: neurotic and organic depression (24,3%); delirium (26,1%) and adjustment reaction (14,5%). Major psychiatric pathology (schizophrenia and affective disorders) was found in 10,8%. Antidepressive medication was prescribed in 37,8% patients; antipsychotics in 34,2%. In 9.9% of the cases, no psychotropic drug was prescribed, and 5,4% of patients received psychologic support. Regarding hospital discharge, 42,4% of patients were referred to the general practitioner or to the medical specialist; 22,5% to outpatient gerontopsychiatry; 17,1% to psychiatric outpatient and 14,4% deceased. A future perspective is to start liaison in areas of HIV and psycho oncology in multidisciplinary approach and to improve collaboration with colleagues of other specialties for refining psychiatric referrals.Keywords: psychiatry, liaison, internal emergency, psychiatric referral

Abstract:
Background: Coronary bifurcation is one of the most complex lesion in patients with coronary ar-tery disease. Provisional T-stenting is currently one of the recommended techniques. The aim was to assess optimal methods of treatment in the era of drug-eluting stents (DES). Methods: The regis-try consisted of data from 1916 patients treated with coronary percutaneous interventions (PCI) using either first- or second-generation DES. Patients with bifurcation lesion entered the analysis. Major adverse cardiac and cardiovascular events (MACCE) were assessed at one year of follow-up and comprised of death, acute myocardial infarction (AMI), repeated PCI (re-PCI) of target ves-sel and stroke. Results: Of 1916 registry patients, 204 patients (11%) were diagnosed with bifurcation lesion >50% and entered the analysis. The most commonly used technique was provi-sional T-stenting (141 patients, 69%). Optimization with kissing-balloons technique was performed in 45 patients (22%). In 59 patients (29%) second-generation DES was implanted, while in 112 pa-tients (55%), first-generation DES was used. In 33 patients (16%) both types of DES were used. The procedure success rate (TIMI 3 flow) was achieved in 98% of patients. In one-year follow-up, there were 39 MACCE (19%) (9 deaths, 17 AMI, 16 re-PCI and 5 strokes). Provisional T-stenting resulted in similar rate of MACCE to other techniques (16% vs. 5%, p=0.27) and similar occurrence of re-PCI (6% vs. 2%, p=0.78). The results of post-PCI kissing-balloon technique gave equal out-comes with 3% vs. 16% of MACCE in patients in whom no optimization technique was used (p=0.39). The type of implanted DES (second- vs. first-generation) had no influence on MACCE (4% vs 14%, respectively, p=0.12) and re-PCI (1.7% vs. 51% patients, respectively, p=0.28). Con-clusions: The treatment of bifurcation lesions with PCI represent high-risk procedures with high rate of MACCE. Stenting technique, optimization of PCI and the generation of implanted stent should be personalized for each case to balance risk of the procedure. In this setting, the operator experience might be the factor of better outcome, which should be further investigated.Keywords: coronary bifurcation, drug eluting stents, long-term follow-up, percutaneous coronary interventions
