Abstract:
Variety of different factors could influence on the measure urban perception. Both physical and non-physical factors, at least, make the quality of perception through the urban spaces. The value of lighting is one of the important factors which could make the better quality of environmental perception for the user. The perception of urban space in most of the Iranian cities is offer by different factors during the night time which caused to the death of nightlife and social activities. Therefore, this research is an attempt to study on the different of user perception during day and night in the Koohsangi Street. As the case study area in Iran in order to bring out the main influential factors during perception process. To deal with this good we used chi-square test on a sample size made up of on hundred participants. The result shows that for improving the night quality of urban spaces the legibility, navigation, and role stimulation were in important perception factors. Therefore, by focusing on these factors it would be possible to find out more functional solution for improving the activity of night perception.Keywords: perception, urban space, legibility, imageability, nightscape

Abstract:
The paper deals with the problem of optimizing management options for urban parks within different scenarios of nature protection and tourism importance. The procedure is demonstrated on a case study example of urban parks in Novi Sad (Serbia). Six management strategies for the selected area have been processed by the decision support method PROMETHEE. Two criteria used for the evaluation were nature protection and tourism and each of them has been divided into a set of indicators: for nature protection those were biodiversity and preservation of original landscape, while for tourism those were recreation potential, aesthetic values, accessibility and culture features. It was pre-assumed that each indicator in a set is equally important to a corresponding criterion. This way, the research was focused on a sensitivity analysis of criteria weights. In other words, weights of indicators were fixed and weights of criteria altered along the entire scale (from the value of 0 to the value of 1), and the assessment has been performed in two-dimensional surrounding. As a result, one could conclude which management strategy would be the most appropriate along with changing of criteria importance. The final ranking of management alternatives was followed up by investigating the mean PROMETHEE Φ values for all options considered and when altering the importance of nature protection/tourism. This type of analysis enabled detecting an alternative with a solid performance along the entire scale, i.e., regardlessly of criteria importance. That management strategy can be seen as a compromise solution when the weight of criteria is not defined. As a conclusion, it can be said that, in some cases, instead of having criteria importance fixed it is important to test the outputs depending on the different schemes of criteria weighting. The research demonstrates the state of the final decision when the decision maker can estimate criteria importance, but also in cases when the importance of criteria is not established or known.Keywords: criteria weights, PROMETHEE, sensitivity analysis, urban parks

Abstract:
In content-based 3D shape retrieval system, achieving high search performance has become an important research problem. A challenging aspect of this problem is to find an effective shape descriptor which can discriminate similar shapes adequately. To address this problem, we propose a new shape descriptor for 3D shape models by combining multi-resolution with multi-block center-symmetric local binary pattern operator. Given an arbitrary 3D shape, we first apply pose normalization, and generate a set of multi-viewed 2D rendered images. Second, we apply Gaussian multi-resolution filter to generate several levels of images from each of 2D rendered image. Then, overlapped sub-images are computed for each image level of a multi-resolution image. Our unique multi-block CS-LBP comes next. It allows the center to be composed of m-by-n rectangular pixels, instead of a single pixel. This process is repeated for all the 2D rendered images, derived from both ‘depth-buffer’ and ‘silhouette’ rendering. Finally, we concatenate all the features vectors into one dimensional histogram as our proposed 3D shape descriptor. Through several experiments, we demonstrate that our proposed 3D shape descriptor outperform the previous methods by using a benchmark dataset.Keywords: 3D shape retrieval, 3D shape descriptor, CS-LBP, overlapped sub-images

Historic paths in today's cities are facing the pressure of the urban development due to the rapid urban growth. Every new development is tearing the old urban fabric and the socio-economic character of the historic paths. Furthermore, in some cases historic paths suffer from negligence and decay. Kom El Dekka historic path was one of those deteriorated paths in the city of Alexandria, Egypt, in spite of its high heritage and socio-economic value. Therefore, there was a need to develop urban regeneration strategies as a part of a wider sustainable development vision, to handle the situation and revitalize the path as a livable space in the heart of the city. This study aims to develop a comprehensive assessment methodology to evaluate the different values of the path and to create community-oriented and economic-based analysis methodology for its socio-economic values. Abstract:
Cross ventilation is an important and practical mean to achieve thermal comfort and conserve energy. This is especially true in the breezy waterfront settings. However, due to a number of factors, cross ventilation in buildings is usually studied by using oversimplified scenarios. It is then reasonable to study the impact of complex set of factors on the accuracy of predicting air flow rate because of wind driven cross ventilation. The objective of this paper is to provide architects with the tools necessary to achieve natural ventilation for cooling purposes in a waterfront urban canyon context. Also, urban canyons have not received much attention in terms of their impact on cross ventilation, and while we know how the wind flows between buildings in different urban canyon settings, the effect of the parallel-to-the-wind urban canyon on cross ventilation in buildings remains unclear. For this, we use detailed weather data, boundary layer correction factor, and CFD simulations to study the pressure patterns that form on the canyons surfaces in the case study of Alexandria. We found that the simplified numerical methods of calculating the cross ventilation in buildings can lead to inaccurate design decisions.Keywords: cross ventilation, Alexandria, CFD, urban canyon

Abstract:
The city as a complex system exhibiting chaotic behaviour is in a state of constant change, in response to prevailing social, economic, environmental and technological factors. Without adequate investigation and control mechanisms to tame the sporadic nature of growth in most urban areas of cities in developing regions, organic sprawling visibly manifests with its attendant problems, most especially at peri-urban areas. The Lagos Megacity region in southwest Nigeria, as one of the largest megacities in the world contends with the challenges of sprawling at the peri-urban areas especially along emerging transit corridors. Due to the seemingly unpredictable nature of this growth, this paper attempts a morphological investigation into the growth of peri-urban settlements along the Mowe-Ibafo transit corridor of the Megacity region over a temporal space of three decades (1984-2014). This study adopts the application of the Fractal Analysis and Regression Analysis methods through the correlation of population density and fractal dimension values to establish the pattern and nature of growth, due to the inadequacies of conventional methods of urban analysis which cannot deal with the unpredictability of such complex urban forms as the peri-urban areas. It was deduced that the dynamic urban expansion in the last three decades resulted in about 74.2% urban change rate between 1984 and 2000 and 63.4% urban change rate between 2000 and 2014. With the R2 value between the fractal dimension and population density been 1, the regression model indicates a positive correlation between Fractal Dimension (D) and Population Density (pop/km2), where the increase in the population density from 5740 pop/km2 to 8060 pop/km2 and later decrease to 7580 pop/km2 leads to an increase in the fractal dimension of urban growth from 1.451 in 1984 to 1.853 in 2014. This, therefore, justifies the ability to predict and determine the nature and direction of growth of complex entities and is sufficient to substantially suggest the need for adequate policy framework towards sustainable urban planning and infrastructural provision in the Peri-urban areas.Keywords: fractal analysis, Lagos Megacity, peri-urban, sprawling, urban morphology

We believe that in the majority of the “recent production of public space", the overvaluation of the "image", of the "ephemeral" and of the "objectual", has come to determine the configuration of banal and (more or less) arbitrary "public spaces", mostly linked to a problem of “outdoor decoration”, reflecting a clear sign of uncertainty and arbitrariness about the meaning, the role and shape of public space and public art.This "inconsistency" which is essentially linked to the loss of urban, but also social, cultural and political, vocation of the disciplines that “shape” the urban space (but is also linked to the lack of urban and technical culture of techinicians and policy makers) converted a significant set of the recently built "public space" and “urban art” into diffuse and multi-referenced pieces, which generally shares the inability of confering to the urban space, civic, aesthetic, social and symbolic meanings. In this sense we consider it is essential to undertake a theoretical reflection on the values, the meaning(s) and the shape(s) that open space, and urban art may (or must) take in the current urban and cultural context, in order to redeem for public space its status of significant physical reference, able to embody a spatial and urban identity, and simultaneously enable the collective accession and appropriation of public space. Taking as reference public space interventions built in the last decade on the European context, we will seek to explore and defend the need of considering public space as a true place of exception, an exceptional support where the emphasis is placed on the quality of the experience, especially by the relations public space/urban art can established with the city, with nature and geography in a broad sense, referring us back to a close and inseparable and timeless relationship between nature and culture.Keywords: art, city, nature, public space

Wetlands contribute significantly to the national economy. Nevertheless, urban wetlands in Tanzania have been taken for granted; many have been converted into waste disposal areas and settlements despite their substantial role in climate-change flood attenuation and livelihood. This is due to the lacking informing assessments from a socio-economic perspective. This study assesses the contribution of urban wetlands to the livelihood of marginalised communities in Dar es Salaam City, Tanzania. Specifically, the study assesses the an extent and nature of change in wetlands in Dar es Salaam City for the past 30 years using the land-use land-cover change approach and the contribution of wetlands to livelihood using questionnaires. The results show that the loss of wetlands in Dar es Salaam is high to extent that will likely jeopardise their future contributions to livelihood. Intelligent Transportation System is an integrated application of communication, control and monitoring and display process technologies for developing a user–friendly transportation system for urban areas in developing countries. In fact, the development of a country and the progress of its transportation system are complementary to each other. Urban traffic has been growing vigorously due to population growth as well as escalation of vehicle ownership causing congestion, delays, pollution, accidents, high-energy consumption and low productivity of resources. The development and management of urban transport in developing countries like India however, is at tryout stage with very few accumulations. Under the umbrella of ITS, urban corridor management strategy have proven to be one of the most successful system in accomplishing these objectives. The present study interprets and figures out the performance of the 27.4 km long Urban Bus route having six intersections, five flyovers and 29 bus stops that covers significant area of the city by causality analysis. Worldwide rapid urbanisation has accelerated city expansion in both developed and developing nations. This unprecedented urbanisation trend due to the increasing population and economic growth has caused challenges for the decision-makers in city planning and urban management. Metropolitan cities, class-I towns, and major urban centres undergo a continuous process of evolution due to interaction between socio-cultural and economic attributes. This constant evolution leads to urban expansion in all directions. Understanding the patterns and dynamics of urban built-up growth is crucial for policymakers, urban planners, and researchers, as it aids in resource management, decision-making, and the development of sustainable strategies to address the complexities associated with rapid urbanisation. Identifying spatio-temporal patterns of urban growth has emerged as a crucial challenge in monitoring and assessing present and future trends in urban development. Analysing urban growth patterns and tracking changes in land use is an important aspect of urban studies. This study analyses spatio-temporal urban transformations and land-use and land cover changes using remote sensing and GIS techniques. Built-up growth analysis has been done for the city of Surat as a case example, using the GIS tools of NDBI and GIS models of the Built-up Urban Density Index and Shannon Entropy Index to identify trends and the geographical direction of transformation from 2005 to 2020. Surat is one of the fastest-growing urban centres in both the state and the nation, ranking as the 4th fastest-growing city globally. This study analyses the dynamics of urban built-up area transformations both zone-wise and geographical direction-wise, in which their trend, rate, and magnitude were calculated for the period of 15 years. Urban neighborhoods, as important urban forming cells, play a key role in creating urban texture and integrated form. Nowadays, most of neighborhood divisions are based on urban management systems but without considering social issues and the other aspects of urban life. This can cause problems such as providing inappropriate services for city dwellers, the loss of local identity and etc. In this regard for regenerating of such neighborhoods, it is essential to locate neighborhood centers with appropriate access and services for all residents. The main objective of this article is reaching to the location of neighborhood centers in a way that, most of issues relating to the physical features (such as the form of access network and texture permeability and etc.) and other qualities such as land uses, densities and social and economic features can be done simultaneously. This paper attempts to use methods of spatial analysis in order to surveying spatial structure and space syntax of urban textures and Urban Network Analysis Systems. This can be done by one of GIS toolbars which is named UNA (Urban Network Analysis) with the use of its five functions (include: Reach, Betweenness, Gravity, Closeness, Straightness).These functions were written according to space syntax theory and offer its relating output. This paper tries to locate and evaluate the optimal location of neighborhood centers in order to create local centers. The last two decades have witnessed a significant number of regeneration practices in historical Chinese cities with the context of rapid urbanisation and economic development. As a response to the possible loss of place identity in the redevelopment process, city makers recognise the potentials of places with cultural significance for maintaining the original landscape. In Chongqing, the prosperities of human settlement history together with the anti-war and post-industrial culture bring this riverside city with its unique urban landscape. Great amounts of historical sites are identified and subjected to conservation planning approaches for urban revitalisation while also maintaining the historic urban landscape. This paper reviews three practices of cultural-led regeneration projects (Hongyadong, Ciqikou, Danzishi) in Chongqing, detailing the urban design and planning principals for the case sites. The paper also presents the conflicting opinions from groups with different interests. By carrying a systematical sustainability evaluation assessment on those projects, the paper critically analyzes the influence of these projects on the broad socio-economic aspects. Although these regeneration cases are thought to achieve the general success in abstaining economic benefits, they are criticised for the over-tourism issues and damages on the authenticity, which further fails to achieve sustainable development.Keywords: Chongqing, historic urban landscape, sustainable development, urban regeneration

Abstract:
Urban climate change has short- and long-term implications for decision-makers in urban development. The problem for this important metropolitan regional of population and economic value is that there is very little usable information on climate change. Research about urban climate change has been carried out in Bali Indonesia by using Conformal Cubic Atmospheric Model (CCAM) that runs with Representative Concentration Pathway (RCP)4.5. The history data means average data from 1975 to 2005, climate projections with RCP4.5 scenario means average data from 2006 to 2099, and anomaly (urban climate change) is RCP4.5 minus history. The results are the history of temperature between 22.5-27.5 OC, and RCP4.5 between 25.5-29.5 OC. The temperature anomalies can be seen in most of northern Bali that increased by about 1.6 to 2.9 OC. There is a reduced humidity tendency (drier) in most parts of Bali, especially the northern part of Bali, while a small portion in the south increase moisture (wetter). The comfort index of Bali region in history is still relatively comfortable (20-26 OC), but on the condition RCP4.5 there is no comfortable area with index more than 26 OC (hot and dry). We have proposed a novel scheme— iterative word-extension (IWE) to enhance the cliff effect of Reed-Solomon codes regarding the error performance at a specific Eb/N0. The scheme can be readily extended to block codes and the important properties of IWE are further investigated here. In order to select proper block codes specifying the desired cliff Eb/N0, the associated features of IWE are explored. These properties and features grant IWE ability to enhance security regarding the received Eb/N0 in physical layer so that IWE scheme can cooperate with the traditional presentation layer approach — cryptography, to meet the secure requirements in diverse applications. Abstract:
Leadership is commonly classified as formal leadership and informal leadership, which ignores and neglects the effects of 3rd type leadership. The emergence of 3rd type of leadership is closely related to special relations. To figure out the mechanism and effects of 3rd type leadership as well as the impacts of formal leadership and informal leadership on employee performance, this study collects data from 350 participants through a survey and proposes three hypotheses respectively from the perspective of expectation theory. The analytical results provide strong evidence for two of the three hypotheses, which demonstrate the positive correlation between formal leadership and individual performance and the negative relationship between 3rd type leadership and individual performance. This study contributes to leadership literature by putting forward the concept of the 3rd type of leadership. Urban design aims to provide a stage for public life that, when once brought to life, is right away subject to subtle but continuous transformation. This paper explores such transformations and searches for ways how public life can be reinforced in the case of a housing settlement for the displaced in Nicosia, Cyprus. First, a sound basis of theoretical knowledge is established through literature review, notably the theory of the Production of Space by Henri Lefebvre, exploring its potential and defining key criteria for the following empirical analysis. The analysis is pinpointing the differences between spatial practice, representation of space and spaces of representation as well as their interaction, alliance, or even conflict. In doing so uncertainties, chances and challenges are unraveled that will be consequently linked to practice and action and lead to the formulation of a design strategy. Public and private spaces are major components of the morphology of the city. This research aims to study the interactions between public and private domains in terms of urban space in Casablanca. The research focuses on a general vision of a socio-spatial issue. It plans to identify the public, private, and transition (semi-public, semi-private) spaces as the constituent of the urban space. Moreover, the study investigates the link between public and private spaces with the social dimensions. Additionally, the research argues that the public space is a place of social interaction; as a reflection, this interaction is the intersection between urban space and social connectedness. Besides, social interaction can be the key to distinguishing between the public and private spheres. The methodological approach of the research is based on the literature review and field study. The article is targeting a case study on the old Medina of Casablanca, from daily use of the public and private spaces, the urban tissue, and the urban space types. In conclusion, the research exhibits that a public space could influence the privacy of the residents of a local urban area; thus, this privacy is inverted on the social interaction. This social interaction is the link between the urban space and social connectedness. So in this paper, a detailed work on urban land use pattern, with a special reference to build up land in Bangalore city is analyzed in view of the various master plans from 1975to 2011. An attempt has been made to study the status of urban land use of Bangalore city during this period to detect the changes on land utilization rate that has taken place in each master plan period, particularly in the built-up land. Rack type warehouses are different from general buildings in the kinds, amount, and arrangement of stored goods, so the fire risk of rack type warehouses is different from those buildings. The fire pattern of rack type warehouses is different in combustion characteristic and storing condition of stored goods. The initial fire burning rate is different in the surface condition of materials, but the running time of fire is closely related with the kinds of stored materials and stored conditions. The stored goods of the warehouse are consisted of diverse combustibles, combustible liquid, and so on. Fire detection time may be delayed because the residents are less than office and commercial buildings. If fire detectors installed in rack type warehouses are inadaptable, the fire of the warehouse may be the great fire because of delaying of fire detection. In this paper, we studied what kinds of fire detectors are optimized in early detecting of rack type warehouse fire by real-scale fire tests. The fire detectors used in the tests are rate of rise type, fixed type, photo electric type, and aspirating type detectors. Abstract:
Urban development is often dependent on adequate land for expansion, except that sometimes these areas have vulnerability. This is the case of karst regions characterized by carbonate geological formations marked by the presence of cavities and cracks. The impact of climate variability in Cheria area marked by a growing shortage of rainfall, the impact resulted in the development of the vulnerability of these structures. This vulnerability has led to the appearance of collapse phenomena as well in both agricultural and urban areas. Two phenomena have emerged to explain the collapses, the first is assigned a filling process in the cavities, and the second is due to a weakening of the resistance that collapses limestone slab shear phenomenon. In urban areas, the weight of the buildings has increased the load on the limestone slab and accelerated the collapse. The analysis of the environmental process is in the context of our modest work, after which we indicate the appropriate methods for management policy of urban expansion. Abstract:
This paper presents a multi-objective framework for sustainable urban development. The proposed framework aims to address different aspects of sustainability in urban development planning. These aspects include, but are not limited to education, health, job opportunities, architecture, culture, environment, mobility, energy, water, waste, and so on. Then, the proposed framework is applied to the Brackenridge Tract (an area in downtown Austin, Texas), to redevelop that area in a sustainable way. The detail of the implementation process is presented in this paper. The ultimate goal of this paper is to develop a sustainable area in downtown Austin with ensuring that it locally meets the needs of present and future generations with respect to economic, social, environmental, health as well as cultural aspects. Individuals with prediabetes increase the risk of developing type 2 diabetes. Exercise is a potent stimulator of skeletal muscle glucose uptake and thus good for maintaining glucose homeostasis. That could be a conducive method to improve blood glucose regulation and prevent type 2 diabetes without medication intake. The aim of this study was to summarize mechanisms of insulin resistance and investigate the beneficial effects of acute and chronic aerobic exercise on glycemic control in prediabetes and type 2 diabetes. Aerobic exercise regulates glucose homeostasis and reduces blood glucose, insulin concentrations. Space does not play role just in mentioning the place or locations. It also takes part in people attendance and social structures. Urban space is of substantial aspects of city which is a public sphere for free and unlimited appearance of citizens. Along with such appearances and regarding physical, environmental and functional conditions, different personal and social behaviors can be seen and analyzed toward people. The main principle of an urban space is including social relations and communications. In this survey, urban space has been referred to one in which physical, environmental and functional attractions cause pause and staying of people. Surveys have shown that urban designers have discussed about place more than architects or planners. With attention to mutual relations between urban space, society and civilization, proper policy making and planning are essential due to achieving an ideal urban space. The survey has been decided to analyze the effect of functional and physical structure of urban spaces on citizens' social behaviors. Hence, Valiasr crossroads, Tehran identified public space, has been selected in which analytic-descriptive method utilized. To test the accuracy of assumptions, statistical test has been accomplished by SPSS. Abstract:
The process of urbanization in China has entered a critical period of transformation from urban expansion and construction to delicate urban design, thus forming a new direction in the field of urban design. So far, catalyst theory has become a prominent guiding strategy in urban planning and design. In this paper, under the background of urban renewal, catalyst theory is taken as the guiding ideology to explore the method of urban design in shouxian county. Firstly, this study briefly introduces and analyzes the catalyst theory. Through field investigation, it is found that the city has a large number of idle Spaces, such as abandoned factories and schools. In the design, the idle Spaces in the county town are utilized and interlinked in space, and functional interaction is carried out from the pattern of the county town. On the one hand, the results showed that the catalyst theory can enhance the vitality of the linear street space with a small amount of monomer construction. On the other hand, the city can also increase the cultural and economic sites of the city without damaging the historical relics and the sense of alterations of the ancient city, to improve the quality of life and quality of life of citizens. Abstract:
Nowadays, the traffic congestion becomes increasingly a chronic problem. Sometimes, the cause is attributed to the recurrent road works that create barriers to the efficient movement. But congestion, which usually occurs in cities, can take diverse forms and magnitudes. The case study of Biskra city in Algeria and the diagnosis of its road network show that throughout all the micro regional system, the road network seems at first quite dense. However, this density although it is important, does not cover all areas. A major flow is concentrated in the axis Sidi Okba – Biskra – Tolga. The largest movement of people in the Wilaya (prefecture) revolves around these three centers and their areas of influence. Centers farthest from the trio are very poorly served. This fact leads us to ask questions about the extent of congestion in Biskra city and its relationship to the imbalance of the urban framework. The lack of water supply results increases the chance of unhygienic situations in the cities. In this study, the urban sanitation issues are identified with respect to water supply and utility management. The study compared based on their best practices and initiatives. From this, best practices and initiatives identify suitable sustainable measures to address water supply issues in the city level. Abstract:
Pneumococcal Vaccine -10 (PCV 10) was included in the Expanded Program of immunization (EPI) in Sindh, Pakistan in February 2013. This study was carried out immediately before the introduction of PCV 10 to establish baseline pneumococcal carriage and prevalent serotypes in naso-pharynx of children 3-11 months of age in an urban and rural community in Sindh, Pakistan. An additional sample of children aged 12 to 59 months was drawn from the urban community. Nasopharyngeal specimens were collected from a random sample of children. Samples were processed in a central laboratory in Karachi. Pneumococci were cultured on 5% Sheep Blood Agar and serotyping was performed using CDC standardized sequential multiplex PCR assay on bacterial colonies. Serotypes were then categorized into vaccine (PCV-10 and PCV-13) type and non-vaccine types. A total of 670 children were enrolled. Carriage rate for pneumococcus based on culture positivity was 74% and 79.5 % in the infant group in Karachi and Matiari respectively. Carriage rate was 78.2% for children aged 12 to 59 months in Karachi. Proportion of PCV 10 serotypes in infants was 38.8% and 33.5% in Karachi and Matiari respectively. In the older age group in Karachi, the proportion was 30.6%. Most common serotypes were 6A, 6B, 23F, 19A and 18C. This survey establishes vaccine and non-vaccine serotype carriage rate in a vaccine-naïve pediatric population among rural and urban communities in Sindh province. Annually planned surveys in the same communities will inform change in carriage rate after the introduction and uptake of PCV 10 in these communities.Keywords: Naso-Pharyngeal carriage, Pakistan, PCV10, Pneumococcus

Currently, planners try to have more green travel options to decrease economic, social and environmental problems. Therefore, this study tries to find significant urban travel factors to be used to increase the usage of alternative urban travel modes. This paper attempts to identify the relationship between prominent urban mobility indicators and daily trips by public transport in 30 cities from various parts of the world. Different travel modes, infrastructures and cost indicators were evaluated in this research as mobility indicators. Abstract:
After the 1990s, the concept of urban sustainable development has been increasing attention in urban planning and urban design. High carbon city, not a sustainable city construction model, has become an important problem which restricts the sustainable development of the city. Therefore, low-carbon city construction is the urgent need to solve the problem, and China is one of the core areas of low-carbon city construction in the world. The research work of low-carbon cities were participated by the Chinese government and academic institutes on theory and practice since 2007, and nowadays it comes to a practice stage with six low-carbon pilot provinces and 36 low-carbon pilot cities identified. To achieve the low-carbon target, developing low-carbon energy, adopting non-pollution technique, constructing green buildings and adopting ecolife-style are suggest by the government. Meanwhile, besides a new standard system and a new eco-environmental status evaluation method, the government also established the Chinese urban development institute including the Low-Carbon City Group. Abstract:
Introduction: Thyroidectomies are extensive surgeries involving a significant degree of tissue handling and dissection and are associated with considerable postoperative pain. Regional anaesthesia techniques have immerged as possible inexpensive and safe alternatives to opioids in the management of pain after thyroidectomy. The front of the neck is innervated by branches from the cervical plexus, and hence, several approaches for superficial and deep cervical plexus block (CPB) have been described to provide postoperative analgesia after neck surgery. However, very few studies have explored the analgesic efficacy of intermediate CPB for thyroid surgery. In this study, we have evaluated the effects of ultrasound-guided bilateral intermediate CPB on perioperative opioid consumption in patients undergoing thyroidectomy under general anesthesia. Methods: In this prospective randomized controlled study, fifty ASA grade I-II adult patients undergoing thyroidectomy were randomly divided into two groups: the study group received ultrasound-guided bilateral intermediate CPB with 10 ml 0.5% ropivacaine on each side, while the control group received the same block with 10 ml normal saline on each side just after induction of anesthesia. Anesthesia was induced with propofol, fentanyl, and vecuronium and maintained with propofol infusion titrated to maintain the BIS between 40 and 60. During the postoperative period, rescue analgesia was provided with PCA fentanyl, and the pain scores, total fentanyl consumption, and incidence of nausea and vomiting during 24 hours were recorded, and overall patient satisfaction was assessed. Results: The groups were well-matched with respect to age, gender, BMI, and duration of surgery. The difference in intraoperative propofol and fentanyl consumption was not statistically significant between groups. However, the intraoperative haemodynamic parameters were better maintained in the study group than in the control group. The postoperative pain scores, as measured by VAS at rest and during movement, were lower, and the total fentanyl consumption during 24 hours was significantly less in the study group as compared to the control group. Patients in the study group reported better satisfaction scores than those in the control group. No adverse effects of ultrasound-guided intermediate CPB block were reported. Conclusion: We concluded that ultrasound-guided intermediate cervical plexus block is a safe and effective method for providing perioperative analgesia during thyroid surgery.Keywords: thyroidectomy, cervical plexus block, pain relief, opioid consumption
