1506 Case Study Approach Using Scenario Analysis to Analyze Unabsorbed Head Office Overheads

Abstract:
Head office overhead (HOOH) is an indirect cost and is recovered through individual project billings by the contractor. Delay in a project impacts the absorption of HOOH cost allocated to that particular project and thus diminishes the expected profit of the contractor. This unabsorbed HOOH cost is later claimed by contractors as damages. The subjective nature of the available formulae to compute unabsorbed HOOH is the difficulty that contractors and owners face and thus dispute it. The paper attempts to bring together the rationale of various HOOH formulae by gathering contractor’s HOOH cost data on all of its project, using case study approach and comparing variations in values of HOOH using scenario analysis. The case study approach uses project data collected from four construction projects of a contractor in India to calculate unabsorbed HOOH costs from various available formulae. Scenario analysis provides further variations in HOOH values after considering two independent situations mainly scope changes and new projects during the delay period. Interestingly, one of the findings in this study reveals that, in spite of HOOH getting absorbed by additional works available during the period of delay, a few formulae depict an increase in the value of unabsorbed HOOH, neglecting any absorption by the increase in scope. This indicates that these formulae are inappropriate for use in case of a change to the scope of work. Results of this study can help both parties in deciding on an appropriate formula more objectively, considering the events on a project causing the delay and contractor's position in respect of obtaining new projects.Keywords: absorbed and unabsorbed overheads, head office overheads, scenario analysis, scope variation

Abstract:
The study determined the Administrative traits and capabilities of Mindanao State University Heads of office as perceived by their respondents. Specifically, this study attempted to find out: To get the primary data, a self- constructed survey questionnaire which was validated by a panel of experts, including the adviser. Most of the MSU head of office were aware of their duties and responsibilities as a manager. Considering their vast knowledge and expertise on the technical or task aspects of the job, it is not surprising that respondents perceived them to a high degree as work or task oriented. MSU head of office were knowledgeable and capable in performing field-specific, specialized tasks and enabling them to coordinate work, solve problems, communicate effectively, and also understand the big picture in light of the front-line work that must be performed. The significance of coaching or mentoring in this instance may be explained by the less number of Master’s or Doctorate degree holder among employees resulting to close supervision and mentorship of head of office towards the latter; Without comparison, interpersonal or human relation capabilities is a very effective way in dealing with people as it gives them the opportunity to influence their employees. In the case of MSU head of office, the best way of dealing with problematic employees is by establishing trust and allowing them to partake in the decision making even on setting organizational goals as it would make them feel part of the organization; Thus, it is recommended that the success of an organization depends largely with the effectiveness of the head of unit. In this case, being development oriented would mean encouraging both head officers & employees to know not only the technical know hoe of the organisation but also the visions, missions, goals & the latter’s aspirations to establish cooperation & harmonious working environment; hence, orientation & reorientation time to time would enable them to be more development oriented; With respect to human relations, effective interpersonal relationship between head of unit & employee is of paramount importance. Abstract:
Barclays Bank has identified the need for a head office building in Lusaka, Zambia, and construction of a 7200 m2 three-storey reinforced concrete office building with a structural steel roof is currently underway. A unique characteristic of the development is that the building footprint is positioned on dolomitic land. Dolomite rock has the tendency to react with and breakdown in the presence of slightly acidic water, including rainwater. This leads to a potential for subsidence and sinkhole formation. Subsidence and the formation of sinkholes beneath a building can be detrimental during both the construction and operational phases. This paper outlines engineering principles which were considered during the structural design of the raft foundation for the Barclays head office building. In addition, this paper includes multidisciplinary considerations and the impact of these on the structural engineering design of the raft foundation. By ensuring that the design of raft foundations on dolomitic land incorporates the requirements of all disciplines and relevant design codes during the design process, the risk associated with subsidence and sinkhole formation can be effectively mitigated during the operational phase of the building.Keywords: dolomite, dolomitic land, raft foundation, structural engineering design

Abstract:
Estimating costs is a crucial step in construction management and should be completed at the beginning of every project to establish the project's budget. The precision of the cost estimate plays a significant role in the success of construction projects as it allows project managers to effectively manage the project's costs. Site overhead costs constitute a significant portion of construction project budgets, necessitating accurate prediction and management. These costs are influenced by a multitude of factors, requiring a thorough examination and analysis to understand their relative importance and impact. Thus, the main aim of this research is to enhance the contractor’s ability to predict and manage site overheads by identifying and analyzing the main factors influencing the site overheads costs in the Egyptian construction industry. Through a comprehensive literature review, key factors were first identified and subsequently validated using a thorough comparative analysis of data from 55 real-life construction projects. Through this comparative analysis, the relationship between each factor and site overheads percentage as well as each site overheads subcategory and each project construction phase was identified and examined. Furthermore, correlation analysis was done to check for multicollinearity and identify factors with the highest impact. The findings of this research offer valuable insights into the key drivers of site overhead costs in the Egyptian construction industry. Abstract:
The purpose of this study was to investigate the attitudes of the staff in the Faculty of Science and Technology towards the e-office documentary system of the Faculty of Science and Technology, Suan Sunandha Rajabhat University. The subjects of this study included 98 staffs of the Faculty of Science and Technology, Suan Sunandha Rajabhat University. Questionnaires were used to ask the attitude of the staffs towards the implementation of e-office system. The results showed that most of the users of e-office are female with the age between 31-40 years old, master degree of education and in the academic positions. They have working experiences between 1-5 years and reported the time of using e-office between 8:30-12:30 during the weekday with the frequency of 3-5 times/day. Most of them reported their opinions on the e-office at high level (x=3.84) and problems and obstacles in using e-office at high level, as well (x=3.63).Keywords: attitudes, e-office, staff, documentary system

Abstract:
All changes in the office environment were and are still driven by advances in technology. The impact of computers on office work has resulted in numerous changes in office activities, procedures and the expectations from office managers and secretaries. This study investigated the level of knowledge and use of computer office application packages by secretaries and office managers in higher educational institutions in Ogun State and the implications of these on their performance enhancement. The study is an ex post facto research and adopted the survey design for the collection of data. Two hypotheses were formulated, and a questionnaire was developed and tested at 0.05 level of significance. All office managers and secretaries in the service of higher educational institutions in Ogun State, Nigeria formed the population of the study. The study was limited to federal institutions and a total of 120 office managers/secretaries were selected to form the sample such that 40 office managers/secretaries were randomly selected from each of the three Federal higher institutions in the State, that is Federal University of Agriculture, Abeokuta, Federal Polytechnic, Ilaro and Federal College of Education, Osiele, Abeokuta, Ogun State. Analysis of data and hypotheses tests were carried out with frequency counts, percentage and T-Test. The result indicated varying levels of awareness on office application tools with limited knowledge and use of computer application packages by office managers/secretaries. The results also showed that good knowledge and high use of office application tools enhance performance of office managers/secretaries. The study recommended that there should be maximum institutional resources and support and personal development on the part of the office managers to ensure update knowledge and maximal use of office application tools by office managers/secretaries.Keywords: application packages, computer, office managers, performance enhancement

The issue of energy crisis for big cities in Indonesia are issues raised in line with the development of the city is rapidly increasing. Various attempts were made by the government in overcoming problems of energy needs in Indonesia. In addition to the efforts of the government required the efforts made by the public to solve this problem. The concept of green building in the design of the building with efforts to use energy efficiently can be one of the efforts that can be applied to solve this problem. Jakarta is capital and the one of the major cities in Indonesia with high economic growth. This leads to increased demand for office space for the people. So that the construction of office buildings in big cities like Jakarta very numerous. Office building is one of the buildings that require large energy consumption. As a building that could potentially require huge amounts of energy, the design should consider the use of energy to help provide solutions to problems of energy crisis in Indonesia. The concept of energy efficient is one of the concepts addressed in an effort to use energy in buildings to save energy needs of the building operations. Therefore, it is necessary to have a study that explores the application of the concept of energy efficiency and conservation in office buildings in Jakarta. In this study using two (2) buildings case study that Sequis Center Building and Sampoerna Strategic Square. Both are office buildings in Jakarta have earned the Green Building Certificate of Green Building Council Indonesia (GBCI). The study used literature review methods to address issues raised earlier. Whether it's related to a literature review on the study of office buildings and green building. With this paper is expected to be obtained on the application of the concept of energy efficiency and conservation in office buildings that have earned recognition as a green building by GBCI. The result could be a reference to the architect in designing the next office buildings, especially related to the concept of energy use in buildings. From this study, it can be concluded that the concept of energy efficiency and conservation in the design of office buildings can be applied to its orientation, the openings, the use shade in buildings, vegetation and building material selection and efficient use of water. So that it can reduce energy requirements needed to meet the needs of the building user activity. So the concept of energy efficiency and conservation in office buildings can be one of the efforts to realize the Green Office Building. Recommendations from this study is that the design of office buildings should be able to apply the concept of energy utilization in the design office. Under severe accident cases, the nuclear reactor core may meltdown inside the lower head of the reactor pressure vessel (RPV). Retaining the melt pool inside the RPV is an important strategy of severe accident management. During this process, the inner wall of the lower head will be heated to high temperature of a thousand centigrade, and the outer wall is immersed in a large amount of cooling water. The material of the lower head will have serious creep damage under the high temperature and the temperature difference, and this produces a great threat to the integrity of the RPV. In this paper, the ANSYS program is employed to build the finite element method (FEM) model of the lower head, the creep phenomena is simulated under the severe accident case, the time dependent strain and stress distribution is obtained, the creep damage of the lower head is investigated, the integrity of the RPV is evaluated and the theoretical basis is provided for the optimized design and safety assessment of the RPV.Keywords: severe accident, lower head of RPV, creep, FEM

As per current scenario, commercial aircrafts have been very well functioning with higher efficiency, but there is something that affects it. Every aircraft runs with the combustion produced by mixture of fuel and air. For example: A flight to travel from Mumbai to Kolkata it takes 2h: 30 min and from Kolkata to Mumbai it takes 2h: 45 min. It happens due to head and tail wind. Due to head wind air craft travels faster than its usual velocity and it takes 2h: 30 min to reach to Kolkata, while it takes 2h;45min vis versa. This lag in time is caused due to head wind that increases the drag and reduces the relative velocity of the plane. So in order to reduce this wastage of fuel there is an optimal flight altitude at which the head and tail wind action is reduced compared to the present scenario.Keywords: drag, head wind, tail wind, aircraft

Abstract:
Every year, children develop hydrocephalus during the first year of life. If it is not treated, hydrocephalus can lead to brain damage, a loss in mental and physical abilities, and even death. To be treated, first, we have to do a proper diagnosis using some examinations especially to detect hydrocephalus earlier. One of the examination that could be done is using a head circumference measurement. Increased head circumference is a first and main sign of hydrocephalus, especially in infant (0-1 year age). Head circumference is a measurement of a child's head largest area. In this measurement, we want to get the distance from above the eyebrows and ears and around the back of the head using a measurement tape. If the head circumference of an infant is larger than normal, this infant might potentially suffer hydrocephalus. If early diagnosis and timely treatment of hydrocephalus could be done most children can recover successfully. There are some problems with early detection of hydrocephalus using regular tape for head circumference measurement. One of the problem is the infant’s comfort. We need to make the infant feel comfort along the head circumference measurement to get a proper result of the examination. For that, we can use a helpful stuff, like a hat. This paper is aimed to describe the possibility of using a head circumference measuring instrument for early detection of hydrocephalus in an infant with a mike hat, coloured-tape-in-hat. In the first life, infants’ head size is about 35 centimeters. First three months after that infants will gain 2 centimeters each month. The second three months, infant’s head circumference will increase 1 cm each month. And for the six months later, the rate is 0.5 cm per month, and end up with an average of 47 centimeters. This formula is compared to the WHO’s head circumference growth chart. The shape of this tape-in-hat is alike an upper arm measurement. This tape-in-hat diameter is about 47 centimeters. It contains twelve different colours range by age. If it is out of the normal colour, the infant potentially suffers hydrocephalus. This examination should be done monthly. If in two times of measurement there still in the same range abnormal of head circumference, or a rapid growth of the head circumference size, the infant should be referred to a pediatrician. There are the pink hat for girls and blue hat for boys. The objective of the Hydro Turbine Head Cover is to support the guide bearing, guide vane regulating mechanism and even in some design for generator thrust bearing support. Mechanical design of head cover deals with high static as well as fluctuating load acting on the structure. In the present work structural analysis of hydro turbine Head-cover using ANSYS software is carried out. Finite element method is used to calculate stresses on head cover. These calculations were done for the maximum possible loading under operating condition “LCI Quick Shut Down”. About, Documentation of verbal and written head injury advice given to all adults presenting following a head injury. Our aim was to assess verbal & written head injury advice for an adult patient attending ED in Wythenshawe hospital during the period from January 2022 to May 2022, with a view to evaluating the NICE head injury guidelines concerning discharge advice and also to review the clinical notes to ensure that all adult patients presenting with a head injury are documented to have received both verbal & written head injury advice as per the NICE guidelines. Here we collected data from a random sample over a 1 month period. This data was furtherly filtered to include the adult patient >16 years and resulted in 54 patients with head injuries attending ED during this time period; then patient’s age, sex and hospital number were used to identify the discharge advice for the purpose of chart review and to assess the documentation of head injuries compliance with recommendation for NICE assessment. Data were checked between January 2022 up to May 2022 to allow more intervals for better assessment. Our finding indicates that documentation of verbal advice, 26% of patients were not recorded to have received this in January compared to only 3% in May & Written advice was not recorded in 44% of patients studied in January compared to 1% in May.Keywords: head, injuries, advice, leaflets

Abstract:
Breadth-First Search(BFS) is a core graph algorithm that is widely used for graph analysis. As it is frequently used in many graph applications, improve the BFS performance is essential. In this paper, we present a graph ordering method that could reorder the graph nodes to achieve better data locality, thus, improving the BFS performance. Our method is based on an observation that the sibling relationships will dominate the cache access pattern during the BFS traversal. Therefore, we propose a frequency-based model to construct the graph order. First, we optimize the graph order according to the nodes’ visit frequency. Nodes with high visit frequency will be processed in priority. Second, we try to maximize the child nodes overlap layer by layer. As it is proved to be NP-hard, we propose a heuristic method that could greatly reduce the preprocessing overheads. We conduct extensive experiments on 16 real-world datasets. The result shows that our method could achieve comparable performance with the state-of-the-art methods while the graph ordering overheads are only about 1/15.Keywords: breadth-first search, BFS, graph ordering, graph algorithm

The study aimed at recognizing the obstacles of applying of Electronic Administration (e-administration), which refers to any of a number of mechanisms which convert what in a traditional office are paper processes into electronic processes, with the goal being to create a paperless office and improve productivity and performance at the University of Tabuk from its Academic Leaders' Perspectives. The sample of the study consisted of (98) members from deans, vice deans and head of departments from different specialization, gender and position. For achieving the aim of the study, a questionnaire was developed including (45) items distributed into three domains (administrative, human and technical obstacles) . By using appropriate statistical methods to analyze the information, the results indicated that the administrative obstacles domain came in the first rank with a high degree, and the human and technical obstacles came at the second rank with a moderate degree. Nowadays, green office principles are very broadly initiated in many offices, organizations, as well as in universities. The concepts of green office are composed of seven prominent issues. One of them, physical implementation, is to develop a pleasant atmosphere for staff in the faculty with selected optimum plant species for the office. 50 species from NASA research and other documents were studied for the selection criteria of plants which were appropriate for specific locations in order to reduce indoor air pollutants such as formaldehyde, benzene, and trichloroethylene. For the copy and examination preparation room in which particulate matter and volatile organic compounds can be found, some plants such as peace lily, gerbera daisy, and bamboo palm should be set, which are very effective in treating trichloroethylene. This research aims to study the level of e-office system competence of Suan Sunandha Rajabhat University graded by age, education background, position and work experience. Sample of this research is 291 staff at Suan Sunandha Rajabhat University. Data were collected by questionnaire. Statistics used in the research are percentage, mean and standard deviation. The result shows that the overall competence of E-office System of the university staff is at average level. When considered in each aspect, it was found that competency level for creating-forwarding-signing documents is high, while competency level for booking meeting rooms, requesting for transportation service, blackboard system, public relations and making appointment and meeting are average.Keywords: competence, e-office, education background, work experience

This research aims to study the democratic political socialization of the 5th and 6th Graders under the Authority of Dusit District Office, Bangkok by using stratified sampling for probability sampling and using purposive sampling for non-probability sampling to collect data toward the distribution of questionnaires to 300 respondents. This covers all of the schools under the authority of Dusit District Office. The researcher analyzed the data by using descriptive statistics which include arithmetic mean and standard deviation. The result shows that 5th and 6th graders under the authority of Dusit District Office, Bangkok, have displayed some characteristics following democratic political socialization both inside and outside classroom as well as outside school. Abstract:
One of the factors that can play an important role in increasing productivity is the optimal use of information technology, which in this area today has a significant role to play in computer systems of office automation in organizations and companies. Therefore, this research has been conducted with the aim of investigating the effect of the relationship between computerized systems of office automation and the productivity of employees in the municipality of Ilam city. The statistical population of this study was 110 people. Using Cochran formula, the minimum sample size is 78 people. The present research is a descriptive-looking research in terms of the type of objective view. A questionnaire was used to collect data. To assess the reliability of variables, Cornbrash’s alpha coefficient was used, which was equal to 0.85; SPSS19 and Pearson test were used to analyze the data and test the hypothesis of the research. In this research, three hypotheses of the relationship between office automation with efficiency, performance, and effectiveness were investigated. The results showed a direct and positive relationship between the office automation system and the increase in the efficiency, effectiveness, and efficiency of employees, and there was no reason to reject these hypotheses.Keywords: efficiency, performance, effectiveness, automation

Abstract:
This work investigates the effect of head and bucket splitter angle on the power output of a pelton turbine (water turbine), so as to boost the efficiency of Hydro-electric power generation systems. A simulation program was developed using MatLab to depict the force generated by the bucket as the water jet strikes the existing splitter angle (100 to 150) and predicted (10 to 250) splitter angles. Result shows that in addition to the existing splitter angle, six more angles have been investigated for the two operating conditions to give maximum power. The angles are 250, 60 and 190 for high head and low flow with increased pressure while low head and high flow with decreased pressure are 230, 210 and 30 in order of the maximum generating power. The Turbine power output for simulation was more than that of the experiment. Abstract:
The purpose of the study was to identify the factors associated with head and neck cancer-related lymphoedema (HNCRL)-related symptoms, body image, and HNCRL-related functional outcomes among patients with resected advanced head and neck cancer. A cross-sectional correlational design was conducted to examine the predictors of HNCRL-related functional outcomes in patients with resected advanced head and neck cancer. Eligible patients were recruited from a single medical center in northern Taiwan. Consecutive patients were approached and recruited from the Radiation Head and Neck Outpatient Department of this medical center. Eligible subjects were assessed for the Symptom Distress Scale–Modified for Head and Neck Cancer (SDS-mhnc), Brief International Classification of Functioning, Disability and Health (ICF) Core Set for Head and Neck Cancer (BCSQ-H&N), Body Image Scale–Modified (BIS-m), The MD Anderson Head and Neck Lymphedema Rating Scale (MDAHNLRS), The Foldi’s Stages of Lymphedema (Foldi’s Scale), Patterson’s Scale, UCLA Shoulder Rating Scale (UCLA SRS), and Karnofsky’s Performance Status Index (KPS). The results showed that the worst problems with body HNCRL functional outcomes. Patients’ HNCRL symptom distress and performance status are robust predictors across over for overall HNCRL functional outcomes, problems with body HNCRL functional outcomes, and activity and social functioning HNCRL functional outcomes. Based on the results of this period research program, we will develop a Cancer Rehabilitation and Lymphedema Care Program (CRLCP) to use in the care of patients with resected advanced head and neck cancer.Keywords: head and neck cancer, resected, lymphedema, symptom, body image, functional outcome

Abstract:
Energy consumption and users’ satisfaction were compared in three LEED certified office buildings in turkey and an office building in Egypt. The field studies were conducted in summer 2012. The measured environmental parameters in the four buildings were indoor air temperature, relative humidity, CO2 percentage and light intensity. The traditional building is located in Smart Village in Abu Rawash, Cairo, Egypt. The building was studied for 7 days resulting in 84 responds. The three rated buildings are in Istanbul; Turkey. A Platinum LEED certified office building is owned by BASF and gained a platinum certificate for new construction and major renovation. The building was studied for 3 days resulting in 13 responds. A Gold LEED certified office building is owned by BASF and gained a gold certificate for new construction and major renovation. The building was studied for 2 days resulting in 10 responds. A silver LEED certified office building is owned by Unilever and gained a silver certificate for commercial interiors. The building was studied for 7 days resulting in 84 responds. The results showed that all buildings had no significant difference regarding occupants’ satisfaction with the amount of lighting, noise level, odor and access to the outdoor view. There was significant difference between occupants’ satisfaction in LEED certified buildings and the traditional building regarding the thermal environment and the perception of the general environment (colors, carpet and decoration. The findings suggest that careful design could lead to a certified building that enhances the thermal environment and the perception of the indoor environment leading to energy consumption without scarifying occupants’ satisfaction.Keywords: energy consumption, occupants’ satisfaction, rating systems, office buildings

This research paper consisted of three objectives: 1) to investigate actual perception of Suan Sunandha Rajabhat University’s members towards roles and performances of the Coordinating Council Office under the University Council; 2) to investigate expectation of Suan Sunandha Rajabhat University’s members towards roles and performances of the Coordinating Council Office under the University Council; and 3) to compare actual perception and expectation of Suan Sunandha Rajabhat University’s members towards roles and performances of the Coordinating Council Office under the University Council. A total of 316 samples from the population of the members of Suan Sunandha Rajabhat University were selected by use of the simple random sampling technique. Descriptive statistics and Dependent T- Test for paired samples were used, where the Dependent T- Test was for an analysis of a comparison of actual perception and expectation of Suan Sunandha Rajabhat University’s members towards roles and performances of the Coordinating Council Office under the University Council. The findings unveiled significantly high levels for the following roles: [i] appropriately circulating agendas and meeting files before time; [ii] preparing appropriate amount and quality of audio- visual equipment for meetings; [iii] compiling and keeping up-to-date documents; [iv] coordinating and working on linking all useful information to serve for the university uses for strategic policing; and [v] preparing appropriate meeting venues.Keywords: coordinating council office of the university council, expected role, perceived role, performances of duties

Avascular necrosis of femoral head is a pathologic condition that the main cause is decreased blood supply of femoral head. Among predisposing risk factors, chronic use of corticosteroids, alcoholism, smocking and hip traumas have more important role. Also we can mention OCP consumption as a risk factor among less common predisposing factors that lead to AVNF, in this study we introduce another cause of AVNF with a period of treatment with moderate dose of corticosteroids accompanied by OCP as a probable facilitating factor that leads to AVNF.Keywords: AVN, corticosteroids consumption, femoral head osteonecrosis, OCP

Abstract:
Complement ordering principles of natural language phrases (XPs) stipulate that Head terms be consistently placed phrase initially or phrase-finally, yielding two basic theoretical orders – Head – Complement order or Complement – Head order. This paper examines the principles which determine complement ordering in English V- and N-bar structures. The aim is to determine the extent to which complement linearisations in the two phrase types are consistent with the two theoretical orders outlined above given the flexible and varied nature of natural language structures. The objective is to see whether there are variation(s) in the complement linearisations of the XPs studied and the implications which such variations hold for the inter-language syntax of English and Ibibio. A corpus-based approach was employed in obtaining the English data. V- and -N – bar structures containing complement structures were isolated for analysis. Data were examined from the perspective of the X-bar and Government – theories of Chomsky’s (1981) Government-Binding format. Findings from the analysis show that in V – bar structures in English, heads are consistently placed phrase – initially yielding a Head – Complement order; however, complement linearisation in the N – bar structures studied exhibited parametric variations. Thus, in some N – bar structures in English the nominal head is ordered to the left whereas in others, the head term occurs to the right. It may therefore be concluded that the principles which determine complement ordering are both Language – Particular and Phrase – specific following insights provided within Phrasal Syntax.Keywords: complement order, complement–head order, head–complement order, language–particular principles

Abstract:
Quantitative analysis of mouse whisker movement can be used to study functional recovery and regeneration of facial nerve after an injury. However, it is challenging to accurately track mouse whisker movements, and most whisker tracking methods require manual intervention, e.g. fixing the head of the mouse during a study. Here we describe a semi-automated image processing method that is applied to high-speed video recordings of free-moving mice to track whisker movements. We first track the head movement of a mouse by delineating the lower head contour frame-by-frame to locate and determine the orientation of its head. Then, a region of interest is identified for each frame, with subsequent application of the Hough transform to track individual whisker movements on each side of the head. Our approach is used to examine the functional recovery of damaged facial nerves in mice over a course of 21 days.Keywords: mystacial macrovibrissae, whisker tracking, head tracking, facial nerve recovery
Procedia PDF Downloads 5901477 A Retrospective Analysis of the Impact of the Choosing Wisely Canada Campaign on Emergency Department Imaging Utilization for Head Injuries

Abstract:
Head injuries are a commonly encountered presentation in emergency departments (ED) and the Choosing Wisely Canada (CWC) campaign was released in June 2015 in an attempt to decrease imaging utilization for patients with minor head injuries. The impact of the CWC campaign on imaging utilization for head injuries has not been explored in the ED setting. In our study, we describe the characteristics of patients with head injuries presenting to a tertiary care academic ED and the impact of the CWC campaign on CT head utilization. This retrospective cohort study used linked databases from the province of Ontario, Canada to assess emergency department visits with a primary diagnosis of head injury made between June 1, 2014 and Aug 31, 2016 at the University Health Network in Toronto, Canada. We examined the number of visits during the study period, the proportion of patients that had a CT head performed before and after the release of the CWC campaign, as well as mode of arrival, and disposition. There were 4,322 qualifying visits at our site during the study period. The median presenting age was 44.12 years (IQR 27.83,67.45), the median GCS was 15 (IQR 15,15) and the majority of patients presenting had intermediate acuity (CTAS 3). Overall, 43.17% of patients arrived via ambulance, 49.24 % of patients received a CT head and 10.46% of patients were admitted. Compared to patients presenting before the CWC campaign release, there was no significant difference in the rate of CT heads after the CWC (50.41% vs 47.68%, P = 0.07). There were also no significant differences between the two groups in mode of arrival (ambulance vs ambulatory) (42.94% vs 43.48%, P = 0.72) or admission rates (9.85% vs 11.26%, P = 0.15). However, more patients belonged to the high acuity groups (CTAS 1 or 2) in the post CWC campaign release group (12.98% vs 8.11% P <0.001). Visits for head injuries make up a significant proportion of total ED visits and approximately half of these patients receive CT imaging in the ED. The CWC campaign did not seem to impact imaging utilization for head injuries in the 14 months following its launch. Further efforts, including local quality improvement initiatives, are likely needed to increase adherence to its recommendation and reduce imaging utilization for head injuries.Keywords: choosing wisely, emergency department, head injury, quality improvement
