3468 Evaluation of the Microscopic-Observation Drug-Susceptibility Assay Drugs Concentration for Detection of Multidrug-Resistant Tuberculosis

Abstract:
New diagnostic tools are urgently needed to interrupt the transmission of tuberculosis and multidrug-resistant tuberculosis. The microscopic-observation drug-susceptibility (MODS) assay is a rapid, accurate and simple liquid culture method to detect multidrug-resistant tuberculosis (MDR-TB). MODS were evaluated to determine a lower and same concentration of isoniazid and rifampin for detection of MDR-TB. Direct drug-susceptibility testing was performed with the use of the MODS assay. Drug-sensitive control strains were tested daily. The drug concentrations that used for both isoniazid and rifampin were at the same concentration: 0.16, 0.08 and 0.04μg per milliliter. We tested 56 M. tuberculosis clinical isolates and the control strains M. tuberculosis H37RV. All concentration showed same result. Of 53 M. tuberculosis clinical isolates, 14 were MDR-TB, 38 were susceptible with isoniazid and rifampin, 1 was resistant with isoniazid only. Drug-susceptibility testing was performed with the use of the proportion method using Mycobacteria Growth Indicator Tube (MGIT) system as reference. The result of MODS assay using lower concentration was significance (P<0.001) compare with the reference methods. A lower and same concentration of isoniazid and rifampin can be used to detect MDR-TB. Operational cost and application can be more efficient and easier in resource-limited environments. However, additional studies evaluating the MODS using lower and same concentration of isoniazid and rifampin must be conducted with a larger number of clinical isolates.Keywords: isoniazid, MODS assay, MDR-TB, rifampin

Abstract:
Background: In-hospital management of Out-of-Hospital Cardiac Arrest (OHCA) is complex as the aetiologies are varied. Acute coronary angiography has been shown to improve outcomes for patients with coronary occlusion as the cause; however, these patients are difficult to identify. ECG results may help identify these patients, but the accuracy of this diagnostic test is under debate, and requires further investigation. Methods: Arrest and hospital management information was collated retrospectively for OHCA patients who presented to a single clinical site between 2009 and 2013. Angiography results were then collected and checked for significance with survival to discharge. The presence of a severe lesion (>70%) was then compared to categorised ECG findings, and the accuracy of the test was calculated. Results: 104 patients were included in this study, 44 survived to discharge, 52 died and 8 were transferred to other clinical sites. Angiography appears to significantly correlate with survival to discharge. ECG showed 54.8% sensitivity for detecting the presence of a severe lesion within the group that received angiography. A combined criterion including any ECG pathology showed 100% sensitivity and negative predictive value, however, a low specificity and positive predictive value. Conclusion: In the cohort investigated, ST elevation on ECG is not a sensitive enough screening test to be used to determine whether OHCA patients have coronary stenosis as the likely cause of their arrest, and more investigation into whether screening with a combined ECG criterion, or whether all patients should receive angiography routinely following OHCA is needed.Keywords: out of hospital cardiac arrest, coronary angiography, resuscitation, emergency medicine

Abstract:
In the previous airfield construction industry, pavements made of reinforced concrete have been used very rarely; however, the necessity to use this type of pavements in an emergency situations justifies the need reference to this issue. The paper concerns the problem of airfield pavement dimensioning made of reinforced concrete and the evaluation of selected dimensioning methods of reinforced concrete slabs intended for airfield pavements. Analysis of slabs dimensioning, according to classical method of limit states has been performed and it has been compared to results obtained in case of methods complying with Eurocode 2 guidelines. Basis of an analysis was a concrete slab of class C35/45 with reinforcement, located in tension zone. Steel bars of 16.0 mm have been used as slab reinforcement. According to comparative analysis of obtained results, conclusions were reached regarding application legitimacy of the discussed methods and their design advantages.Keywords: rainforced concrete, cement concrete, airport pavements, dimensioning

Abstract:
Objectives: To determine the risk factors for high resistance of ciprofloxacin in complicated urinary tract infections. Materials and Methods: It is an analytical study that was conducted in the department of Urology (Team ‘C’) at Institute of Kidney Diseases Hayatabad Peshawar from 1st June 2012 till 31st December 2012. Total numbers of 100 patients with complicated UTI was selected in the study. Multivariate analysis and linear regression were performed for the detection of risk factors. All the data was recorded on structured Proforma and was analyzed on SPSS version 17. Results: The mean age of the patient was 55.6 years (Range 3-82 years). 62 patients were male while 38 patients were female. 66 isolates of E-Coli were found sensitive to ciprofloxacin while 34 isolates were found Resistant for ciprofloxacin. Using multivariate analysis and linear regression, an increasing age above 50 (p=0.002) History of urinary catheterization especially for bladder outflow obstruction (p=0.001) and previous multiple use of ciprofloxacin (p=0.001) and poor brand of ciprofloxacin were found to be independent risk factors for high resistance of ciprofloxacin. Conclusion: UTI is common illness across the globe with increasing trend of antimicrobial resistance for ciprofloxacin against E Coli in complicated UTI. The risk factors for emerging resistance are increasing age, urinary catheterization and multiple use and poor brand of ciprofloxacin.Keywords: urinary tract infection, ciprofloxacin, urethral catheterization, antimicrobial resistance

Information integration is a well known procedure to provide consolidated view on sets of heterogeneous information sources. It not only provides better statistical analysis of information but also facilitates users to query without any knowledge on the underlying heterogeneous information sources The problem of providing a consolidated view of information can be handled using Semantic data (information stored in such a way that is understandable by machines and integrate-able without manual human intervention). However, integrating information using semantic web technology without any access management enforced, will results in increase of privacy and confidentiality concerns. In this research we have designed and developed a framework that would allow information from heterogeneous formats to be consolidated, thus resolving the issue of interoperability. We have also devised an access control system for defining explicit privacy constraints. We designed and applied our framework on both semantic and non-semantic data from heterogeneous resources. Abstract:
The Single Cell Gel Electrophoresis Assay (SCGE, known as comet assay) is a potential, uncomplicated, sensitive and state-of-the-art technique for quantitating DNA damage at individual cell level and repair from in vivo and in vitro samples of eukaryotic cells and some prokaryotic cells, being popular in its widespread use in various areas including human biomonitoring, genotoxicology, ecological monitoring and as a tool for research into DNA damage or repair in different cell types in response to a range of DNA damaging agents, cancer risk and therapy. The method involves the encapsulation of cells in a low-melting-point agarose suspension, lysis of the cells in neutral or alkaline (pH > 13) conditions, and electrophoresis of the suspended lysed cells, resulting in structures resembling comets as observed by fluorescence microscopy; the intensity of the comet tail relative to the head reflects the number of DNA breaks. The likely basis for this is that loops containing a break lose their supercoiling and become free to extend towards the anode. This is followed by visual analysis with staining of DNA and calculating fluorescence to determine the extent of DNA damage. This can be performed by manual scoring or automatically by imaging software. The assay can, therefore, predict an individual’s tumor sensitivity to radiation and various chemotherapeutic drugs and further assess the oxidative stress within tumors and to detect the extent of DNA damage in various cancerous and precancerous lesions of oral cavity.Keywords: comet assay, single cell gel electrophoresis, DNA damage, early detection test

The number of people suffering from hemiplegia is growing each year. This lower limb disability affects all the aspects of their lives by taking away their autonomy. This implicates their close relatives, as well as the health system to provide the necessary care they need. The integration of exoskeletons in the medical field became a promising solution to resolve this issue. This paper presents an exoskeleton designed to help hemiplegic people get back the sensation and ability of normal walking. For this purpose, three step models have been created. The first step allows a simple forward movement of the leg. The second method is designed to overcome some obstacles in the patient path, and finally the third step model gives the patient total control over the device. There is an inevitable demand of web mining due to rapid increase of huge information on the Internet, but the striking variety of web structures has made required content retrieval a difficult task. To counter this issue, Web Content Mining (WCM) emerges as a potential candidate which extracts and integrates suitable resources of data to users. In past few years, research has been done on several extraction techniques for WCM i.e. agent-based, template-based, assumption-based, statistic-based, wrapper-based and machine learning. However, it is still unclear that either these approaches are efficiently tackling the significant challenges of WCM or not. To answer this question, this paper identifies these challenges such as language independency, structure flexibility, performance, automation, dynamicity, redundancy handling, intelligence, relevant content retrieval, and privacy. Abstract:
Sediment refers to the accumulation of varying amounts of sediment material in natural waters and the formation of bottom sludge. Sediments are the most important sources of pollutants as well as important future sources and carriers of pollutants. The accumulation of pollutants in sediments can cause serious environmental problems for the surrounding areas. Heavy metals (such as Cr, Cd, Al, Pb, Cu, Al, Zn) disrupt the water quality, affect the useful use of sediment, affect the ecosystem and have a toxic effect on the life of the sediment layer. This effect, which accumulates in the aquatic organisms, can enter the human body with the food chain and affect health seriously. Potential metal toxicity can be determined by comparing acid volatile sulfides (AVS) – simultaneously extracted metal (SEM) ratio in anoxic sediments to determine the effect of metals. Determination of the concentration of SEM and AVS is useful in screening sediments for potential toxicity due to the high metal concentration. In the case of SEM/AVS < 0 (anoxic sediment); in terms of AVS biomass production, its toxicity can be controlled. No toxic effects may be observed when SEM / AVS < 0. SEM / AVS > 0 (in the case of oxic sediment); metals with sensitive fraction such as Cu, As, Ag, Zn are stored. In this study, AVS and SEM measurements of sediment samples collected from five different points in the district of Tekkeköy in Samsun province were performed. The SEM - AVS ratio was greater than 0 in all samples. This research has been to investigate the cause of the stress corrosion cracking on dryer equipment (2DR-1745 and DR-1402) in Shahid Tondguyan Petrochemical Company (STPC). These dryers are as a drying powder Terphetalic acid in CTA2 and CTA1 unit. After passing through RVF equipment, wet cake moisture content of about 14% and temperature of 90C changed into a dry cake with a moisture content of less than 0.1% and the final temperature of about 140C and sent out Final Silo (FS-1820). After the declaration of the operation department concerning the observation of acid leakage under the primary insulation was decided that at the first opportunity, this issue must be investigated. This article addressed the issue of using physical exam in nursing. Nurses hold different attitudes toward using physical exam in the clinical settings. These attitudes determine to embrace physical examination in practice. So, the aim of the study was to examine registered nurses’ attitudes and practices, identify perceived barriers, and to identify the factors which influence the performance and the attitudes towards physical examinations. Results showed that even though nurses reported performing physical exam often, they had negative attitudes toward it. Stress and performing physical examinations on someone of the opposite gender (n=236; 87.4%) were the main barriers. Nurse's level of education influenced the attitude (t=-4.3; p < .01). These results indicated that RNs recognize the necessity of physical examinations, but they face many barriers and challenges which hinder the performance of the examination. Abstract:
This paper focuses on the application of general healing design theories to modulate them into case-specific and contextual design considerations. Existing literature focuses on the relationship between architecture and mental health. Primary case studies are selected in India to focus on the effect of a specific location on design considerations. They are qualitatively analysed to further contextualise the inferences from the literature study. An academic project is cited as an example to apply the learnings from the study and understand the influence of various parameters on the design process for further conclusion. Literature studies, case studies and hypothetical design applications helped in finding the different ways of achieving the similar goal of a sensitive approach toward mental health. Along with salutogenic parameters, category of establishment, age group, location of the site and user preference plays a crucial role in the design process. Design of mental health establishments, especially in India, has to involve transparency between stakeholders and users. Owing to different climatic zones and diverse sociocultural traditions, the approach toward healing should adapt accordingly. It should be an effort towards striking a balance between contradictory elements of healing design and resolving the dilemmas with sensitivity and consensus. Clean and abundant renewable energy sources (RES) such as solar energy is seen as the best solution to replace conventional energy source. Unpredictable power generation is a major issue in the penetration of solar energy, as power generated is governed by the irradiance received. Controlling the power generated from solar PV (SPV) panels to battery and load is a challenging task. In this paper, power flow control from SPV to load and energy storage device (ESD) is controlled by a fuzzy decision support system (FDSS) on the availability of solar irradiation. The issue of human capital mobility inside Europe is still an open one. Even though there were created some tools in order to better move from one country to another to work and study the number of the people doing this is very low because of various factors presented in this paper. The "rethinking educational" agenda of the European Commission has open the floor for new projects which can create steps towards a European language for skills and competences, qualifications. One of these projects is the Partnership for Exchange of experience in Student on-the-job Training. As part of this project, we are interested to see the situation of the human capital inside EU and the elements that were created until now to support this mobility. Also, the main objective of the project is to make a comparison between the four countries involved in PEST project (Romania, Hungary, Finland, and Estonia), at the education and internship level. Abstract:
Management of offtake of any fluvial river is very sensitive in terms of long-term sustainability where the variation of water flow and sediment transport range are wide enough throughout a hydrological year. The Gorai River is a major distributary of the Ganges River in Bangladesh and is termed as a primary source of fresh water for the South-West part of the country. Every year, significant siltation of the Gorai offtake disconnects it from the Ganges during the dry season. As a result, the socio-economic and environmental condition of the downstream areas has been deteriorating for a few decades. To improve the overall situation of the Gorai offtake and its dependent areas, a study has been conducted by the Institute of Water Modelling, Bangladesh, in 2022. Using the mathematical morphological modeling tool MIKE 21C of DHI Water & Environment, Denmark, simulated results revealed the need for dredging/river training structures for offtake management at the Gorai offtake to ensure significant dry season flow towards the downstream. The dry season flow is found to increase significantly with the proposed river interventions, which also improves the environmental conditions in terms of salinity of the South-West zone of the country. The presented paper contributes to the experimental current of research on privacy. The question of privacy is being discussed at length at present, primarily among lawyers and politicians. However, the matter of privacy has been of interest for economists for some time as well. The valuation of privacy by people is of great importance now. This article is about how people valuate their privacy. An experimental method has been utilised in the conducted research – the survey was carried out among customers of an online store, and the studied issue was whether their readiness to sell their data (WTA) was different from the willingness to buy data back (WTP). The basic aim of this article is to analyse whether people shopping on the Internet differentiate their privacy depending on whether they protect or sell it. The achieved results indicate the presence of major differences in this respect, which do not always come up with the original expectations. The obtained results have supported the hypothesis that people are more willing to sell their data than to repurchase them. However, the hypothesis that the value of proposed remuneration affects the willingness to sell/buy back personal data (one’s privacy) has not been supported.Keywords: privacy, experimental economics, behavioural economics, internet

Abstract:
Based on a new method of the nonparametric estimator of the drift function, we propose a consistent test for the parametric specification of the drift function in the short rate diffusion process using observations from a panel of yields. The test statistic is shown to follow an asymptotic normal distribution under the null hypothesis that the parametric drift function is correctly specified, and converges to infinity under the alternative. Taking the daily 7-day European rates as a proxy of the short rate, we use our test to examine whether the drift of the short rate diffusion process is linear or nonlinear, which is an unresolved important issue in the short rate modeling literature. The testing results indicate that none of the drift functions in this literature adequately captures the dynamics of the drift, but nonlinear specification performs better than the linear specification.Keywords: diffusion process, nonparametric estimation, derivative security price, drift function and volatility function

There are inefficient global scheduling parallelism and local scheduling parallelism prone to processor starvation in current scheduling algorithms. Regarding this issue, this paper proposed a load-aware queue partitioning scheduling strategy by first allocating the queues according to the number of processor cores, calculating the load factor to specify the load queue capacity, and it assigned the awaiting nodes to the appropriate perceptual queues through the precursor nodes and the communication computation overhead. At the same time, real-time computation of the load factor could effectively prevent the processor from being starved for a long time. Experimental comparison with two classical algorithms shows that there is a certain improvement in both performance metrics of scheduling length and task speedup ratio.Keywords: load-aware, scheduling algorithm, perceptual queue, heterogeneous multi-core

Although in the last few decades, English as a foreign language has been a compulsory subject in almost all colleges and universities in Serbia, students who enter the first year come with different levels of knowledge, which is immense task and a burden on teachers not only which literature and how to conduct classes in heterogeneous groups but also how to evaluate and assess the progress.This paper aims to discuss the issue of determining the target level of knowledge of English as a foreign language in higher education in Serbia due to the great need for these levels to equalize. The research was conducted at several colleges and universities where first-year students took a placement test, and we also carried out a review and comparison of the literature used in teaching English in those schools. Abstract:
The understanding of vibration propagation in complex structures such as automotive body in white remains a challenging issue in car design regarding NVH performances. The current analysis is limited to the low frequency range where modal concepts are dominant. Higher frequencies, between 200 and 1000 Hz, will become critical With the rise of electrification. EVs annoying sounds are mostly whines created by either Gears or e-motors between 300 Hz and 2 kHz. Structural intensity analysis was Experienced a few years ago on finite element models. The application was promising but limited by the fact that the propagating 3D intensity vector field is masked by a rotational Intensity field. This rotational field should be filtered using a differential operator. The expression of this operator in the framework of finite element modeling is not yet known. Abstract:
Today, supply chain managers face extensive disruptions in raw material pricing, transportation block, and quality issue due to product complexity. While digitalization might help managers to mitigate the disruption risk and increase supply chain resilience by sharing information between sellers and buyers through the supply chain, entities are reluctant to build such a system. The main reason is it is not clear what information should be shared and who has access to the stored information. In this research, we propose a smart contract built by blockchain technology. This contract helps both buyer and seller to identify the type of information, the access to the information, and how to trace the information. This contract helps managers control their orders through the supply chain and address any disruption they see fit. Furthermore, with the same smart contract, the supplier can track the production process of an order and increase production efficiency by eliminating waste.Keywords: blockchain, production process, smart contract, supply chain resilience

It is generally recognized that the main task of modern society is human development. The quality of human capital has been identified as a key driver of economic development in the region. In this article, considered the quality of human capital as one of the main types of social and economic potential for the region’s development. The phenomenon of human capital represents both material and intellectual components of human activity. It is show that the necessary population characterized by certain quantitative and qualitative indicators (qualification and professional structure, education or social general condition and others) and is an necessary resource for the development of the regional economy. The connection of the regional goals with the quality of human capital is discussed in the article and a number of recommendations on its improvement were given. Solving the tasks stated in the article, the authors used analytical and statistical methods of research, scientific publications of domestic and foreign scientists on this issue. The main goals the proses was to attract finance and activities to the historical city centers through the citizens and municipalities participation while cities expanded their boundaries toward suburban areas. Todays the main problems which facing to the most historical city centers, is loss of their centrality through effect of urbanization on any point of the cities which is the most important issue on neglect and abandonment of the historical central area by decentralizing living, commerce and public areas. This article evaluate the ways in which city center revitalization can be effect on vitality and viability of the central area in case of Naghshe Jahan square which situated at the center of Isfahan city, Iran. Abstract:
Since the outbreak of mad cow disease and bird flu, consumers have become more concerned with meat quality and safety. As a result, meat traceability is adopted as one approach to handle consumers’ concern in this issue. Nevertheless, in Thailand, meat traceability is rarely used as a marketing tool to persuade consumers. As a consequence, the present study attempts to understand consumer trust in the meat traceability system by conducting a study in this country to examine the impact of three types of consumer knowledge on this trust. The study results reveal that out of three types of consumer knowledge, marketplace knowledge was the sole predictor of consumer trust in meat traceability and it has a positive influence. General product class knowledge and knowledge in meat products with traceability, however, did not significantly influence consumer trust. The fifth-generation (5G) wireless systems is featured by extreme densities of cell stations to overcome the higher future demand. Hence, interference management is a crucial challenge in 5G ultra-dense cellular networks. In contrast to the classical inter-cell interference coordination approach, which is no longer fit for the high density of cell-tiers, this paper proposes a novel virtual coordination based on the dynamic common cognitive monitor channel protocol to deal with the inter-cell interference issue. A tractable and flexible model for the coverage probability of a typical user is developed through the use of the stochastic geometry model. Abstract:
In modern societies, stress has become a widespread phenomenon and therefore an issue of major concern to employees, organizations, and the state. As senior management of an organization, executives are not immune to this problem because they carry out lots of activities while on duty. This paper is centered on the benefits of Tourism, Leisure and Recreation to the management of executive stress. Executives work has always been considered to be stressful. The key objective of the research is to gain a better understanding of the causes of stress among executives and to find out how tourism, leisure, and recreational activities could be used as a means to managing stress. Interview and observation data were analyzed using SPSS. The major finding revealed that that human system has specific limitations and nature cannot be cheated. It is recommended that executives should take regular and mandatory vacation of least forty days in a year. The only answer then is rest. The research recommends that a break tends to improves and relaxes, refreshes the mind and enhances performance.Keywords: executive, recreation, stress, tourism

Abstract:
To evaluate the vigor of wheat seeds and stress of premature aging effects on germination percentage, root length and shoot length of five wheat cultivars that include Vynak, Karkheh, Chamran, Star and Kavir which underwent a period of zero, two, three, four days in terms of premature aging with 41 °C temperature and 100% relative humidity. Seed germination percentage, root length and shoot length in these conditions were measured. This experiment was conducted as a factorial completely randomized design with four replications in laboratory conditions. The results showed that each of aging treatments used in this experiment can be used to detect differences in vigor of wheat varieties. Wheat cultivars illustrated significant differences in germination percentage, root length and shoot length in terms of premature aging. The wheat cultivars; Astar and Vynak had maximum germination percentage and Karkheh, respectively Kavir and Chamran had lowest percentage of seed germination. Reactions of root and shoot length of wheat cultivars was also different. The results showed that the seeds with a stronger vigor affected less in premature aging condition and the difference between the percentage of seed germination under normal conditions and stress was significant and the seeds with the weaker vigor were more sensitive to the premature aging stress and the premature aging had more severe negative impact on seed vigor.Keywords: wheat cultivars, seed vigor, premature aging effects, germination
