Authors: B. Paygozar, S.A. Dizaji
Abstract:
Adhesive bonding is a highly valued type of fastening mechanical parts in complex structures, where joining some simple components is always needed. This method is of several merits, such as uniform stress distribution, appropriate bonding strength, and fatigue performance, and lightness, thereby outweighing other sorts of bonding methods. This study is to investigate the failure load of adhesive single-strap joints, including adherends of different sizes and materials. This kind of adhesive joint is very practical in different industries, especially when repairing the existing joints or attaching substrates of dissimilar materials. In this research, experimentally validated numerical analyses carried out in a commercial finite element package, ABAQUS, are utilized to extract the failure loads of the joints, based on the cohesive zone model. In addition, the stress analyses of the substrates are performed in order to acquire the effects of lowering the thickness of the substrates on the stress distribution inside them to avoid designs suffering from the necking or failure of the adherends. It was found out that this method of bonding is really feasible in joining dissimilar materials which can be utilized in a variety of applications. Moreover, the stress analyses indicated the minimum thickness for the adherends so as to avoid the failure of them. Keywords: cohesive zone model, dissimilar materials, failure load, single strap joint

Authors: Yingkun Huang, Li Guo, Zekang Lan, Kai Tian
Abstract:
Hard disk drives (HDD) failure of the exascale supercomputing system may lead to service interruption and invalidate previous calculations, and it will cause permanent data loss. Therefore, initiating corrective actions before hard drive failures materialize is critical to the continued operation of jobs. In this paper, a highly accurate analysis model based on CNN-LSTM and oversampling technique was proposed, which can correctly predict the necessity of a disk replacement even ten days in advance. Generally, the learning-based method performs poorly on a training dataset with long-tail distribution, especially fault prediction is a very classic situation as the scarcity of failure data. To overcome the puzzle, a new oversampling was employed to augment the data, and then, an improved CNN-LSTM with the shortcut was built to learn more effective features. The shortcut transmits the results of the previous layer of CNN and is used as the input of the LSTM model after weighted fusion with the output of the next layer. Finally, a detailed, empirical comparison of 6 prediction methods is presented and discussed on a public dataset for evaluation. The experiments indicate that the proposed method predicts disk failure with 0.91 Precision, 0.91 Recall, 0.91 F-measure, and 0.90 MCC for 10 days prediction horizon. Thus, the proposed algorithm is an efficient algorithm for predicting HDD failure in supercomputing. Keywords: HDD replacement, failure, CNN-LSTM, oversampling, prediction
Procedia PDF Downloads 792333 Failure Mechanisms of Isolated vs. in Aggregate Historical Buildings: A Case Study for Timisoara, Romania
Authors: I. Apostol, M. Mosoarca
Abstract:
Romania is a seismic country, with two major seismic zones, Vrancea and Banat. One of the most important cities from Banat seismic area is Timisoara, where a lot of valuable historical buildings were built before any design codes, but still they kept their stability during past earthquakes. This article presents the influence of the adjacent buildings during an earthquake and the way that the specific failure mechanism is changed when the building is part of an aggregate. The investigation was made using nonlinear analysis based on Tremuri software, first analyzing the buildings as isolated and second, considering the entire aggregate of buildings. There were noticed significant differences through the two situations regarding the specific failure mechanism activated for each building, showing the fact that in some situations, the presence of the adjacent buildings has positive or negative contribution for the seismic behavior of the analyzed one. The difference between the failure mechanism of the same buildings considered isolated and in aggregate aims to provide explications for the good structural state of the existing historical areas of Timisoara, as part of a larger multidisciplinary study, which will help local authorities to prioritize the consolidation works for the historical buildings in order to assure that the history of the city will be kept alive for the next generations. Keywords: failure mechanism, analysis, aggregate, masonry, earthquake
Procedia PDF Downloads 1452332 A Study of Small Business Failure: Impact of Leadership and the Leadership Process
Authors: Theresa Robinson Harris
Abstract:
Small businesses are important to the United States economy, yet the majority struggle to remain relevant and close before their fifth year. This qualitative study explored small business failure by comparing the experiences of small-business owners to understand their involvement with leadership during the early stages of the business, and the impact of this on the firms’ ability to survive. Participants’ experiences from two groups were compared to glean an understanding of the leadership process, how leadership differs between the groups, and to see what themes or constructs emerged that could help to explain the high failure rate. Leadership was perceived to be important when envisioning a path for the future and when providing a platform for employees to succeed. Those who embraced leadership as a skillset were more likely to get through the challenges of the early developmental years while those ignoring the importance of leadership were more likely to close prematurely. These findings suggest a disconnect with regards to the understanding, role, and benefits of leadership in small organizations, particularly young organizations in the early stages of development. Keywords: leadership, small business, entrepreneurship, success, failure

Authors: Lormaine Anne A. Branzuela, Elysa V. Largo, Monet Concepcion M. Detras, Neil C. Concibido
Abstract:
The electricity demand is still increasing, and currently, the Philippine government is investigating the feasibility of operating the Bataan Nuclear Power Plant (BNPP) to address the country’s energy problem. However, the lack of process safety studies on BNPP focused on the effects of hazardous substances on the integrity of the structure, equipment, and other components, have made the plant operationalization questionable to the public. The three major nuclear power plant incidents – TMI-2, Chernobyl, and Fukushima – have made many people hesitant to include nuclear energy in the energy matrix. This study focused on the safety evaluation of possible operations of a nuclear power plant installed with a Pressurized Water Reactor (PWR), which is similar to BNPP. Failure Mode and Effects Analysis (FMEA) is one of the Process Hazard Analysis (PHA) techniques used for the identification of equipment failure modes and minimizing its consequences. Using the FMEA technique, this study was able to recognize 116 different failure modes in total. Upon computation and ranking of the risk priority number (RPN) and criticality rating (CR), it showed that failure of the reactor coolant pump due to earthquakes is the most critical failure mode. This hazard scenario could lead to a nuclear meltdown and radioactive release, as identified by the FMEA team. Safeguards and recommended risk reduction strategies to lower the RPN and CR were identified such that the effects are minimized, the likelihood of occurrence is reduced, and failure detection is improved. Keywords: PHA, FMEA, nuclear power plant, bataan nuclear power plant

Abstract:
Korea baseball experts reckoned Samsung Lions as the best baseball team. It has the unparalleled records of winning first place in the pennant race for five straight years from 2011 to 2015 and winning the Korean series for four years in a row from 2011 to 2014. However, the team made an unbelievably miserable record of ninth place in the pennant race in 2016 and 2017. How come the strong competitive superiority has gone and what kind of slump made the team how it is now. This study investigates this organizational failure case of Samsung Lions, the professional baseball team in Korea. What factors have brought the organizational failure to Samsung Lions? Based on an in-depth examination on how a league-fore-runner drastically lost its competitive superiority, this verifies the necessity of risk management to which common corporations as well as sport teams can be subject at any time in these days. Keywords: Samsung Lions, organizational failure, baseball, slump
Procedia PDF Downloads 3152327 Wind Fragility for Honeycomb Roof Cladding Panels Using Screw Pull-Out Capacity
Authors: Viriyavudh Sim, Woo Young Jung
Abstract:
The failure of roof cladding mostly occurs due to the failing of the connection between claddings and purlins, which is the pull-out of the screw connecting the two parts when the pull-out load, i.e. typhoon, is higher than the resistance of the connection screw. As typhoon disasters in Korea are constantly on the rise, probability risk assessment (PRA) has become a vital tool to evaluate the performance of civil structures. In this study, we attempted to determine the fragility of roof cladding with the screw connection. Experimental study was performed to evaluate the pull-out resistance of screw joints between honeycomb panels and back frames. Subsequently, by means of Monte Carlo Simulation method, probability of failure for these types of roof cladding was determined. The results that the failure of roof cladding was depends on their location on the roof, for example, the edge most panel has the highest probability of failure. Keywords: Monte Carlo Simulation, roof cladding, screw pull-out strength, wind fragility
Procedia PDF Downloads 2532326 A Finite Element Model to Study the Behaviour of Corroded Reinforced Concrete Beams Repaired with near Surface Mounted Technique
Authors: B. Almassri, F. Almahmoud, R. Francois
Abstract:
Near surface mounted reinforcement (NSM) technique is one of the promising techniques used nowadays to strengthen reinforced concrete (RC) structures. In the NSM technique, the Carbon Fibre Reinforced Polymer (CFRP) rods are placed inside pre-cut grooves and are bonded to the concrete with epoxy adhesive. This paper studies the non-classical mode of failure ‘the separation of concrete cover’ according to experimental and numerical FE modelling results. Experimental results and numerical modelling results of a 3D finite element (FE) model using the commercial software Abaqus and 2D FE model FEMIX were obtained on two beams, one corroded (25 years of corrosion procedure) and one control (A1CL3-R and A1T-R) were each repaired in bending using NSM CFRP rod and were then tested up to failure. The results showed that the NSM technique increased the overall capacity of control and corroded beams despite a non-classical mode of failure with separation of the concrete cover occurring in the corroded beam due to damage induced by corrosion. Another FE model used external steel stirrups around the repaired corroded beam A1CL3-R which failed with the separation of concrete cover, this model showed a change in the mode of failure form a non-classical mode of failure by the separation of concrete cover to the same mode of failure of the repaired control beam by the crushing of compressed concrete. Keywords: corrosion, repair, Reinforced Concrete, FEM, CFRP, FEMIX

Procedia PDF Downloads 1542323 Rocket Launch Simulation for a Multi-Mode Failure Prediction Analysis
Authors: Mennatallah M. Hussein, Olivier de Weck
Abstract:
The advancement of space exploration demands a robust space launch services program capable of reliably propelling payloads into orbit. Despite rigorous testing and quality assurance, launch failures still occur, leading to significant financial losses and jeopardizing mission objectives. Traditional failure prediction methods often lack the sophistication to account for multi-mode failure scenarios, as well as the predictive capability in complex dynamic systems. Traditional approaches also rely on expert judgment, leading to variability in risk prioritization and mitigation strategies. Hence, there is a pressing need for robust approaches that enhance launch vehicle reliability from lift-off until it reaches its parking orbit through comprehensive simulation techniques. In this study, the developed model proposes a multi-mode launch vehicle simulation framework for predicting failure scenarios when incorporating new technologies, such as new propulsion systems or advanced staging separation mechanisms in the launch system. To this end, the model combined a 6-DOF system dynamics with comprehensive data analysis to simulate multiple failure modes impacting launch performance. The simulator utilizes high-fidelity physics-based simulations to capture the complex interactions between different subsystems and environmental conditions. Keywords: launch vehicle, failure prediction, propulsion anomalies, rocket launch simulation, rocket dynamics

Authors: Solehuddin Shuib, Koay Boon Aik, Zainul Ahmad Rajion
Abstract:
There are many types of mechanical failure on the dental implant. In this project, the failure that needs to take into consideration is the bone resorption on the dental implant. Human bone has its ability to remodel after the implantation. As the dental implant is installed into the bone, the bone will detect and change the bone structure to achieve new biomechanical environment. This phenomenon is known as bone remodeling. The objective of the project is to improve the performance of dental implant by using different types of design. These designs are used to analyze and predict the failure of the dental implant by using finite element analysis (FEA) namely ANSYS. The bone is assumed to be fully attached to the implant or cement. Hence, results are then compared with other researchers. The results were presented in the form of Von Mises stress, normal stress, shear stress analysis, and displacement. The selected design will be analyzed further based on a theoretical calculation of bone remodeling on the dental implant. The results have shown that the design constructed passed the failure analysis. Therefore, the selected design is proven to have a stable performance at the recovery stage. Keywords: dental implant, FEA, bone remodeling, design

Authors: G. Dadashova, A. Bakhshaliyev
Abstract:
Aim: Determine gender differences in the etiology and clinical outcomes, as well as in the remodeling of the left ventricle (LV) in patients with chronic heart failure (CHF), suffering from arterial hypertension (AH) and coronary heart disease (CHD). Material and methods: The study included 112 patients of both sexes; aged 45 to 60 years with postinfarction cardiosclerosis had functional class (FC) heart failure II-IV of NYHA which were examined on the basis of Azerbaijan Scientific Research Institute of Cardiology. The patients were divided into 2 groups: 1st c. 60 males, mean age 54,8 ± 3,3 years, and 2nd gr 52 women, mean age 55,8 ± 3,1 years. To assess cardiac hemodynamic all patients underwent echocardiography (B-M-modes) using ‘Vivid 3’. Thus on the basis of indicators such as the index of the relative thickness of the left ventricle wall and the index of left ventricular mass (LVMI) was identified the architectonic model of the left ventricle. Results: According to our research leading cause of heart failure in women is 50.5% of cases of hypertension, ischemic heart disease 23.7% (with 79.5% of the cases developed in patients with chronic heart failure who did not have a history of myocardial infarction). While in men is the undisputed leader of CHD, forming 78.3% of CHF (80.3% in men with CHF occurred after myocardial infarction). According to our research in women more often than men CHF develops a type of diastolic dysfunction (DD, and left ventricular ejection fraction remained unchanged. Since DD occurs in men at 65,8% vs. 76,4% of women when p < 0,05. In the group of women was more common prognostic neblagopryatnye remodeling - eccentric hypertrophy of the left ventricle: 68% vs. 54.5% among men (p < 0,05), concentric left ventricular hypertrophy: 21% in women vs 19,1% (p > 0,05 ). Conclusions: Patients with heart failure are a number of gender-specific: the prevalence of hypertension in women, and coronary heart disease in men. While in women with heart failure often recorded diastolic dysfunction and characterized by the development of prognostically unfavorable remodeling types: eccentric and concentric LV hypertrophy. Keywords: chronic heart failure, arterial hypertension, remodeling, diastolic dysfunction, men, women, ischemic heart disease

Authors: Lei Chen, Jian Jiao, Tingdi Zhao
Abstract:
Failures of automotive electric/electronic systems, which are universally considered to be safety-critical and software-intensive, may cause catastrophic accidents. Analysis and verification of failures in these kinds of systems is a big challenge with increasing system complexity. Model-checking is often employed to allow formal verification by ensuring that the system model conforms to specified safety properties. The system-level effects of failures are established, and the effects on system behavior are observed through the formal verification. A hazard analysis technique, called Systems-Theoretic Process Analysis, is capable of identifying design flaws which may cause potential failure hazardous, including software and system design errors and unsafe interactions among multiple system components. This paper provides a concept on how to use model-checking integrated with Systems-Theoretic Process Analysis to perform failure analysis and verification of automotive electric/electronic systems. As a result, safety requirements are optimized, and failure propagation paths are found. Finally, an automotive electric/electronic system case study is used to verify the effectiveness and practicability of the method. Keywords: failure analysis and verification, model checking, system-theoretic process analysis, automotive electric/electronic system

Procedia PDF Downloads 1192316 Evaluation for Punching Shear Strength of Slab-Column Connections with Ultra High Performance Fiber-Reinforced Concrete Overlay
Authors: H. S. Youm, S. G. Hong
Abstract:
This paper presents the test results on 5 slab-column connection specimens with Ultra High Performance Fiber-Reinforced Concrete (UHPFRC) overlay including 1 control specimen to investigate retrofitting effect of UHPFRC overlay on the punching shear capacity. The test parameters were the thickness of the UHPFRC overlay and the amount of steel re-bars in it. All specimens failed in punching shear mode with abrupt failure aspect. The test results showed that by adding a thin layer of UHPFRC over the Reinforced Concrete (RC) substrates, considerable increases in global punching shear resistance up to 82% and structural rigidity were achieved. Furthermore, based on the cracking patterns the composite systems appeared to be governed by two failure modes: 1) diagonal shear failure in RC section and 2) debonding failure at the interface. Keywords: punching shear strength, retrofit, slab-column connection, UHPFRC, UHPFRC overlay

Procedia PDF Downloads 3802314 Utilization of an Object Oriented Tool to Perform Model-Based Safety Analysis According to Extended Failure System Models
Authors: Royia Soliman, Salma ElAnsary, Akram Amin Abdellatif, Florian Holzapfel
Abstract:
Model-Based Safety Analysis (MBSA) is an approach in which the system and safety engineers share a common system model created using a model-based development process. The model can also be extended by the failure modes of the system components. There are two famous approaches for the addition of fault behaviors to system models. The first one is to enclose the failure into the system design directly. The second approach is to develop a fault model separately from the system model, thus combining both independent models for safety analysis. This paper introduces a hybrid approach of MBSA. The approach tries to use informal abstracted models to investigate failure behaviors. The approach will combine various concepts such as directed graph traversal, event lists and Constraint Satisfaction Problems (CSP). The approach is implemented using an Object Oriented programming language. The components are abstracted to its failure logic and relationships of connected components. The implemented approach is tested on various flight control systems, including electrical and multi-domain examples. The various tests are analyzed, and a comparison to different approaches is represented. Keywords: flight control systems, model based safety analysis, safety assessment analysis, system modelling

We felt it pertinent to analyze 30-day readmission causes and outcomes among patients with HCF-19 using the United States using real-world big data via the National readmission database. Objective: The aim is to describe the rate and causes of readmissions and morbidity of heart failure with coinciding COVID-19 (HFC-19) in the United States, using the 2020 National Readmission Database (NRD). Methods: A descriptive, retrospective study was conducted on the 2020 NRD, a nationally representative sample of all US hospitalizations. Adult (>18 years) inpatient admissions with COVID-19 with HF and readmissions in 30 days were selected based on the International Classification of Diseases-Tenth Revision, Procedure Code. Results: In 2020, 2,60,372 adult patients were hospitalized with COVID-19 and HF. The median age was 74 (IQR: 64-83), and 47% were female. The median length of stay was 7(4-13) days, and the total cost of stay was 62,025 (31,956 – 130,670) United States dollars, respectively. Among the index hospital admissions, 61,527 (23.6%) died, and 22,794 (11.5%) were readmitted within 30 days. The median age of patients readmitted in 30 days was 73 (63-82), 45% were female, and 1,962 (16%) died. The most common principal diagnosis for readmission in these patients was COVID-19= 34.8%, Sepsis= 16.5%, HF = 7.1%, AKI = 2.2%, respiratory failure with hypoxia =1.7%, and Pneumonia = 1%. Conclusion: The rate of readmission in patients with heart failure exacerbations is increasing yearly. COVID-19 was observed to be the most common principal diagnosis in patients readmitted within 30 days. Complicated hypertension, chronic pulmonary disease, complicated diabetes, renal failure, alcohol use, drug use, and peripheral vascular disorders are risk factors associated with readmission. Familiarity with the most common causes and predictors for readmission helps guide the development of initiatives to minimize adverse outcomes and the cost of medical care. Keywords: Covid-19, heart failure, national readmission database, readmission outcomes
Procedia PDF Downloads 792310 Encoding the Design of the Memorial Park and the Family Network as the Icon of 9/11 in Amy Waldman's the Submission
Authors: Masami Usui
Abstract:
After 9/11, the American literary scene was confronted with new perspectives that enabled both writers and readers to recognize the hidden aspects of their political, economic, legal, social, and cultural phenomena. There appeared an argument over new and challenging multicultural aspects after 9/11 and this argument is presented by a tension of space related to 9/11. In Amy Waldman’s the Submission (2011), designing both the memorial park and the family network has a significant meaning in establishing the progress of understanding from multiple perspectives. The most intriguing and controversial topic of racism is reflected in the Submission, where one young architect’s blind entry to the competition for the memorial of Ground Zero is nominated, yet he is confronted with strong objections and hostility as soon as he turns out to be a Muslim named Mohammad Khan. This ‘Khan’ issue, immediately enlarged into a social controversial issue on American soil, causes repeated acts of hostility to Muslim women by ignorant citizens all over America. His idea of the park is to design a new concept of tracing the cultural background of the open space. Against his will, his name is identified as the ‘ingredient’ of the networking of the resistant community with his supporters: on the other hand, the post 9/11 hysteria and victimization is presented in such family associations as the Angry Family Members and Grieving Family Members. These rapidly expanding networks, whether political or not, constructed by the internet, embody the contemporary societal connection and representation. The contemporary quest for the significance of human relationships is recognized as a quest for global peace. Designing both the memorial park and the communication networks strengthens a process of facing the shared conflicts and healing the survivors’ trauma. The tension between the idea and networking of the Garden for the memorial site and the collapse of Ground Zero signifies the double mission of the site: to establish the space to ease the wounded and to remember the catastrophe. Reading the design of these icons of 9/11 in the Submission means that decoding the myth of globalization and its representations in this century. Keywords: American literature, cultural studies, globalization, literature of catastrophe

Abstract:
Ultimate Strength Assessment on ship cross section in accordance with Biro Klasifikasi Indonesia (BKI) Rules for Hull, follows step by step incremental iterative approach. In this approach, ship cross section is divided into plate-stiffener combinations and hard corners element. The average stress-strain relationship (σ-ε) for all structural elements will be defined, where the subscript k refers to the modes 0, 1, 2, 3 or 4. These results would be verified with a commercial software calculation in similar cases. The numerical calculations of buckling strength are in accordance with the commercial software (GL Rules ND). Then the comparison of failure behaviours of stiffened panels and hard corners are presented. Where failure modes 3 are likely to occur first follows the failure mode 4 and the last one is the failure mode 1.Keywords: ultimate strength assessment, BKI rules, incremental, plate-stiffener combination and hard corner, commercial software

Abstract:
In this paper, the seismic stability of reinforced soil slopes is studied using pseudo-dynamic analysis. Equilibrium equations that are applicable to the every kind of failure surface are written using Horizontal Slices Method. In written equations, the balance of the vertical and horizontal forces and moment equilibrium is fully satisfied. Failure surface is assumed to be log-spiral, and non-linear equilibrium equations obtained for the system are solved using Newton-Raphson Method. Earthquake effects are applied as horizontal and vertical pseudo-static coefficients to the problem. To solve this problem, a code was developed in MATLAB, and the critical failure surface is calculated using genetic algorithm. At the end, comparing the results obtained in this paper, effects of various parameters and the effect of using pseudo - dynamic analysis in seismic forces modeling is presented. Keywords: soil slopes, pseudo-dynamic, genetic algorithm, optimization, limit equilibrium method, log-spiral failure surface

Authors: Viriyavudh Sim, WooYoung Jung
Abstract:
Window system in high rise building is occasionally subjected to an excessive wind intensity, particularly during typhoon. The failure of window system did not affect overall safety of structural performance; however, it could endanger the safety of the residents. In this paper, comparison of fragility curves for window system of two residential buildings was studied. The probability of failure for individual window was determined with Monte Carlo Simulation method. Then, lognormal cumulative distribution function was used to represent the fragility. The results showed that windows located on the edge of leeward wall were more susceptible to wind load and the probability of failure for each window panel increased at higher floors. Keywords: wind fragility, window system, high rise building, wind disaster

Abstract:
Failure Mode and Effects Analysis (FMEA) has been used in the virtual Process Hazard Analysis (PHA) of the Acetone production process through the dehydrogenation of isopropyl alcohol, for which very limited process risk assessment has been published. In this study, the potential failure modes, effects, and possible causes of selected major equipment in the process were identified. During the virtual FMEA mock sessions, the risks in the process were evaluated and recommendations to reduce and/or mitigate the process risks were formulated. The risk was estimated using the calculated risk priority number (RPN) and was classified into four (4) levels according to their effects on acetone production. Results of this study were also used to rank the criticality of equipment in the process based on the calculated criticality rating (CR). Bow tie diagrams were also created for the critical hazard scenarios identified in the study. Keywords: chemical process safety, failure mode and effects analysis (FMEA), process hazard analysis (PHA), process safety management (PSM)
