7699 Impact of Emergency Medicine Department Crowding on Mortality

Abstract:
Introduction: Emergency department (E.R.) crowding is a serious widespread problem in hospitals that leads to irregularities, a slower rate of delivery of services to patients, and a long-term stay. In addition, the long-term stay in the E.D. reduces the possibility of providing services with appropriate quality to other patients who are undergoing medical emergencies, which leads to dissatisfaction among patients. This study aimed to determine the relationship between ED-crowding and the mortality rate of the patients referred to the E.D. In a retrospective cohort study, all patients who expired in first 24 hours of admission were enrolled in the study. Crowding index at the moment of admission was calculated using Edwin Score. The data including history and physical examination, time of arrival in the E.D., diagnosis (using ICD 10 code), time of death, cause of death, demographic information was recoded based on triage forms on admission and patients’ medical files. Data analysis was performed by using descriptive statistics and chi square test, ANOVA tests using SPSS ver. 19. The time of arrival in E.D. to death in crowded E.D. conditions, with an average of five hours and 25 minutes, was significantly higher than the average admission Time of arrival in E.D. to death in active and crowded E.D. conditions. More physicians and nurses can be employed during crowded times to reduce staff fatigue and improve their performance during these hours.Keywords: mortality, emergency, department, crowding

Abstract:
A study was conducted during 2012 to 2013 in the selected reach of Tinau River, Nepal. The main objective of the study was to quantify employment and income generation from the extraction of construction materials from the river. A 10 km stretch of the river was selected for the study. Sample survey with a semi-structured questionnaire and field observation were the main tools used during field investigation. Extraction of riverbed materials from the banks, beds and floodplain areas of the river has provided many kinds of job opportunities for the people living in the vicinity of the river. It has also generated an adequate amount of revenues. The collected revenue has been invested for many kinds of social and infrastructures development for years. Though extraction of riverbed materials is beneficial for income and employment generation, it has also negative environmental impacts in and around the river. Furthermore, the study concluded that river bed extraction should be continued with special monitoring and evaluation in the areas where there is still room for extraction.Keywords: extraction, crusher plants, economic activities, Tinau River

Abstract:
Reduction of child mortality is an ongoing struggle and a commonly used factor in determining progress in the medical field. The under-5 mortality number is around 5 million around the world, with many of the deaths being preventable. In light of this issue, Cardiotocograms (CTGs) have emerged as a leading tool to determine fetal health. By using ultrasound pulses and reading the responses, CTGs help healthcare professionals assess the overall health of the fetus to determine the risk of child mortality. However, interpreting the results of the CTGs is time-consuming and inefficient, especially in underdeveloped areas where an expert obstetrician is hard to come by. Using a support vector machine (SVM) and oversampling, this paper proposed a model that classifies fetal health with an accuracy of 99.59%. To further explain the CTG measurements, an algorithm based on Randomized Input Sampling for Explanation ((RISE) of Black-box Models was created, called Feature Alteration for explanation of Black Box Models (FAB), and compared the findings to Shapley Additive Explanations (SHAP) and Local Interpretable Model Agnostic Explanations (LIME). Abstract:
Introduction: Considering the increase in life expectancy of world population, there is an emerging concern in health services to allocate better care and care to elderly, through promotion, prevention and treatment of health. It has been observed that frailty syndrome is prevalent in elderly people worldwide and this complex and heterogeneous clinical syndrome consist of the presence of physical frailty associated with cognitive dysfunction, though in absence of dementia. This can be characterized by exhaustion, unintentional weight loss, decreased walking speed, weakness and low level of physical activity, in addition, each of these symptoms may be a predictor of adverse outcomes such as hospitalization, falls, functional decline, institutionalization, and death. Cognitive frailty is a recent concept in literature, which is defined as the presence of physical frailty associated with mild cognitive impairment (MCI) however in absence of dementia. This new concept has been considered as a subtype of frailty, which along with aging process and its interaction with physical frailty, accelerates functional declines and can result in poor quality of life of the elderly. MCI represents a risk factor for Alzheimer's disease (AD) in view of high conversion rate for this disease. Comorbidities and physical frailty are frequently found in AD patients and are closely related to heterogeneity and clinical manifestations of the disease. The decreased platelets ADAM10 levels in AD patients, compared to cognitively healthy subjects, matched by sex, age and education. Objective: Based on these previous results, this study aims to evaluate whether ADAM10 platelet levels of could act as a biomarker of cognitive frailty. Methods: The study was approved by Ethics Committee of Federal University of São Carlos (UFSCar) and conducted in the municipality of São Carlos, headquarters of Federal University of São Carlos (UFSCar). Biological samples of subjects were collected, analyzed and then stored in a biorepository. ADAM10 platelet levels were analyzed by western blotting technique in subjects with MCI and compared to subjects without cognitive impairment, both with and without presence of frailty. Statistical tests of association, regression and diagnostic accuracy were performed. Results: The results have shown that ADAM10/β-actin ratio is decreased in elderly individuals with cognitive frailty compared to non-frail and cognitively healthy controls. Previous studies performed by this research group, already mentioned above, demonstrated that this reduction is still higher in AD patients. Therefore, the ADAM10/β-actin ratio appears to be a potential biomarker for cognitive frailty. The results bring important contributions to an accurate diagnosis of cognitive frailty from the perspective of ADAM10 as a biomarker for this condition, however, more experiments are being conducted, using a high number of subjects, and will help to understand the role of ADAM10 as biomarker of cognitive frailty and contribute to the implementation of tools that work in the diagnosis of cognitive frailty. Such tools can be used in public policies for the diagnosis of cognitive frailty in the elderly, resulting in a more adequate planning for health teams and better quality of life for the elderly.Keywords: ADAM10, biomarkers, cognitive frailty, elderly

The technologies and communication advances contributed to new tools development which allows patients to have an active role in their own health. In the light of information needs and paradigms changes about health, the patient self-manages their care. This line of care focuses on patients; specific portals come up to people with particular requirements like pregnant women. Thinking of a portal design to this sector of the population, in September 2016 a survey was made to users with the objective to knowing and understanding information’s needs at the moment to use an application for pregnant. Also, prototypes of the portal´s features were designed to try and validate with users, using the methodology of human-centered design. Investigations have made possible the identification of needs of this population and develop a tool who try to satisfy, providing timely information for each part of pregnancy and allowing the patients to make a physical check and the follow up of pregnancy seeking advice from our obstetricians.Keywords: electronic health record, health personal record, mobile applications, pregnant women

A new concept of response system is proposed for filling the gap that exists in reducing vulnerability during immediate response to natural disasters. Real Time Early Response Systems (RTERSs) incorporate real time information as feedback data for closing control loop and for generating real time situation assessment. A review of the state of the art works that fit the concept of RTERS is presented, and it is found that they are mainly focused on manmade disasters. At the same time, in response phase of natural disaster management many works are involved in creating early warning systems, but just few efforts have been put on deciding what to do once an alarm is activated. Abstract:
The goal of this study is to analyze if search queries carried out in search engines such as Google, can offer emotional information about the user that performs them. Knowing the emotional state in which the Internet user is located can be a key to achieve the maximum personalization of content and the detection of worrying behaviors. For this, two studies were carried out using tools with advanced natural language processing techniques. The first study determines if a query can be classified as positive, negative or neutral, while the second study extracts emotional content from words and applies the categorical and dimensional models for the representation of emotions. In addition, we use search queries in Spanish and English to establish similarities and differences between two languages. The results revealed that text search queries performed by users on the Internet can be classified emotionally. In Australian public hospitals, there were approximately 34,000 reported inpatient falls between 2015 to 2016. The gold standard for diagnosing intracranial injury is non-contrast enhanced brain computed tomography (CTB). Over a three-month timeframe, a total of one hundred and eighty (180) falls were documented between the hours of 4pm and 8am at a large metro hospital. Only three (3) of these scans demonstrated a positive intra-cranial finding. The rationale for scanning varied. The common indications included a fall with head strike, the presence of blood thinning medication, loss of consciousness, reduced Glasgow Coma Scale (GCS), vomiting and new neurological findings. There are several validated tools to aid in decision-making around ordering CTB scans in the acute setting, but no such accepted tool exists for the inpatient space. With further data collection, spanning a greater length of time and through involving multiple centres, work can be done towards generating such a tool that can be utilized for inpatient falls.Keywords: computed tomography, falls, inpatient, intracranial hemorrhage

Abstract:
This study presents a quasi-zero stiffness (QZS) vibration isolator using flexure-based spring mechanisms which afford both negative and positive stiffness elements, which enable self-adjustment. The QZS property of the isolator is achieved at the equilibrium position. A nonlinear mathematical model is then developed, based on the pre-compression of the flexure-based spring mechanisms. The dynamics are further analyzed using the Harmonic Balance method. The vibration attention efficiency is illustrated using displacement transmissibility, which is then compared with the corresponding linear isolator. The effects of parameters on performance are also investigated by numerical solutions. The Aluminum-Copper casting alloys are well known for their high mechanical strength, especially when compared to more commonly used Aluminum-Silicon alloys. A201 is one of the best in terms of strength vs. weight ratio among other aluminum alloys, which makes it suitable for premium quality casting applications in aerospace and automotive industries. It is reported that A201 has low castability, but it is easy to machine. However, there is a need to specifically determine the process window for feasible machining. This research investigates the machinability of A201 alloy after T7 heat treatment in terms of chip/burr formation, surface roughness, hardness, and microstructure. The samples are cast with low-pressure sand casting method and milling experiments are performed with uncoated carbide tools using different cutting speeds and feeds. Statistical analysis is used to correlate the machining parameters to surface integrity. It is found that there is a strong dependence of the cutting conditions on machinability and a process window is determined.Keywords: A201-T7, machinability, milling, surface integrity

In power industry transformer is an important component and most of us familiar by the functioning principle of a transformer electrically. There are many losses occur during the operation of a transformer that causes heat generation. This heat, if not dissipated properly will reduce the lifetime and effectiveness of the transformer. Transformer cooling helps in maintaining the temperature rise of various paths. This paper proposed to minimize the ambient temperature of the transformer room in order to lower down the temperature of the transformer. A simulation has been made using finite element methods programs called FEMM (Finite Elements Method Magnetics) to create a virtual model based on actual measurement of a transformer. The generalization of the two-dimensional (2D) FEMM results proves that by minimizing the ambient temperature, the heat of the transformer is decreased. On 5 July 2016, just days before the bloody terrorist attack on the Promenade des Anglais in Nice, the Al-Hayat media centre, one of the official propaganda branches of the Islamic State, broadcast a French nasheed which paid tribute to the Paris and Brussels attacks of November 2015 and March 2016. Entitled 'My Revenge', the terrorist anthem was of rare vehemence. It mentioned, sequentially, 'huddled bodies', in a reference to the civilian casualties of Western air strikes in the Iraqi-Syrian zone, 'explosive belts', 'sharp knives', 'large-calibre weapons' as well as 'localised targets'. France was accused of bearing the responsibility for the wave of attacks on its territory since the Charlie Hebdo massacre of January 2015 due to its 'ruthless war' against the Muslim world. Evoking an 'old aggression' and the 'crimes and spoliations' of which France has made itself guilty, the jihadist hymn depicted the rebirth of the caliphate as 'laudable revenge'. The notion of revenge has always been central to contemporary jihadism, understood both as a revolutionary ideology and a global militant movement. In recent years, the attacks carried out in Europe and elsewhere in the world have, for most, been claimed in its name. Whoever says jihad, says drama, yet few studies, if any, have looked at its dramatic and emotional elements, most notably its tragic vengefulness. This seems all the more astonishing that jihad is filled with drama; it could even be seen as a drama in its own right. The jihadists perform a script and take on roles inspired by their respective group’s culture (norms, values, beliefs, and symbols). The militants stage and perform such a script for a designated audience, either partisan, sympathising or hostile towards them and their cause. This research paper will examine the dramaturgy of jihadism and in particular, the genre that best characterises its violence: revenge tragedy. Theoretically, the research will rely on the tools of social movement theory and the sociology of emotions. Methodologically, it will draw from dramaturgical analysis and a combination of qualitative and quantitative tools to attain valuable observations of a number of developments, trends, and patterns. The choice has been made to focus mainly – however not exclusively – on the attacks which have taken place since 2001 in the European Union and more specific member states that have been significantly hit by jihadist terrorism. The research looks at a number of representative longitudinal samples identifying continuities and discontinuities, similarities, but also substantial differences. The preliminary findings tend to establish the relevance and validity of this approach in helping make better sense of sensitisation, mobilisation, and survival dynamics within jihadist groups, and motivations among individuals who have embraced violence. Besides, they illustrate their pertinence for counterterrorism policymakers and practitioners. Through drama, jihadist groups ensure the unceasing regeneration of their militant cause as well as their legitimation among their partisans. Methods for earthquakes forecasting have been intensively investigated in the last decades, but there is still no universal solution agreed by seismologists. Rock failure is most often preceded by a tiny elastic movement in the failure area and by the appearance of micro-cracks. These micro-cracks could be detected at the soil surface and represent useful earth-quakes precursors. The aim of this study was to verify whether tiny raw acceleration signals (in the 10⁻¹ to 10⁻⁴ cm/s² range) prior to the arrival of main primary-waves could be exploitable and related to earthquakes magnitude. Mathematical tools such as Fast Fourier Transform (FFT), moving average and wavelets have been applied on raw acceleration data available on the ITACA web site, and the study focused on one of the most unpredictable earth-quakes, i.e., the August 24th, 2016 at 01H36 one that occurred in the central Italy area. It appeared that these tiny acceleration signals preceding main P-waves have different patterns both on frequency and time domains for high magnitude earthquakes compared to lower ones.Keywords: earthquake, accelerometer, earthquake forecasting, seism

This paper adopts a qualitative approach based on focus group discussions and the system development life cycle model (SDLC) to describe the experience of Albert Ilemobade Library (the Federal University of Technology Akure, Nigeria (FUTA) in the development of their IR - FUTASpace. Peculiar challenges experienced in the course of the development and solutions adopted are also reported. This paper proposes a methodology for the management of noise in open mining industries based on an integral concept, which takes into consideration occupational and environmental noise as a whole. The approach relies on the characterization of sources, the combination of several measurements’ techniques and the use of acoustic prediction software. A discussion about the difference between frequently used acoustic indicators such as Leq and LAV is carried out, aiming to establish common ground for homologation. The results show that the correct integration of this data not only allows for a more robust technical analysis but also for a more strategic route of intervention as several departments of the company are working together. The purpose of this study is to determine the relationship between self-efficacy and subjective well-being among university students. We are aiming to determine whether self efficacy of university students predicts their subjective well-being and if there is a statistically significant difference among boys and girls in this context. Sample of this study consists of 245 university students from Çanakkale, ages ranging between 17 and 24. 72% (n=171) of the participants were girls and 28% (n=69) boys. Three different scales were utilized as data collection tools that Life Satisfaction Scale, General Self-Efficacy Scale, and Positive Negative Experiences Scale. Pearson correlation coefficient, independent sample t test and simple linear regression were used for data analyses. Results showed that well-being is significantly correlated with self-efficacy and self-efficacy is a statistically significant predictor of well-being too. In terms of gender differences, there is no significant difference between self-efficacy scores of boys and girls which shows the same case with well being scores, as well. Fostering university students' academic, social and emotional self-efficacy will increase their well-being which is very important for young adults especially their freshman years.Keywords: positive psychology, self-efficacy, subjective well being, university students

Passing the entrance exam to a university is a major step in one's life. University entrance exam commonly known as Kankor is the nationwide entrance exam in Afghanistan. This examination is prerequisite for all public and private higher education institutions at undergraduate level. It is usually taken by students who are graduated from high schools. In this paper, we reflect the major educational school graduates issues and propose ICT-based test preparation environment, known as ‘Online Kankor Exam Prep System’ to give students the tools to help them pass the university entrance exam on the first try. The system is based on Intelligent Tutoring System (ITS), which introduced an essential package of educational technology for learners that features: (i) exam-focused questions and content; (ii) self-assessment environment; and (iii) test preparation strategies in order to help students to acquire the necessary skills in their carrier and keep them up-to-date with instruction.Keywords: web-based test prep systems, learner-centered design, e-learning, intelligent tutoring system

Abstract:
Sweep frequency response analysis has been turning out a powerful tool for investigation of mechanical as well as electrical integration of transformers. In this paper various aspect of practical application of SFRA has been studied. Open circuit and short circuit measurement were done on different phases of high voltage and low voltage winding. A case study was presented for the transformer of rating 31.5 MVA for various frequency ranges. A clear picture was presented for sub- frequency ranges for HV as well as LV winding. The main motive of work is to investigate high voltage short circuit response. Abstract:
In the red corridor famed for the Left Wing Extremism, lies small district of Jamui in Bihar, India. The district lies at 24º20´ N latitude and 86º13´ E longitude, covering an area of 3,122.8 km2 The undulating topography, with widespread forests provides pristine environment for invigorating experience of tourists. Natural landscape in form of forests, wildlife, rivers, and cultural landscape dotted with historical and religious places is highly purposive for tourism. The study is primarily related to the identification of potential ecotourism sites, using Geographic Information System. Data preparation, analysis and finally identification of ecotourism sites is done. Secondary data used is Survey of India Topographical Sheets with R.F.1:50,000 covering the area of Jamui district. District Census Handbook, Census of India, 2011; ERDAS Imagine and Arc View is used for digitization and the creation of DEM’s (Digital Elevation Model) of the district, depicting the relief and topography and generate thematic maps. The thematic maps have been refined using the geo-processing tools. Buffer technique has been used for the accessibility analysis. Finally, all the maps, including the Buffer maps were overlaid to find out the areas which have potential for the development of ecotourism sites in the Jamui district. Spatial data - relief, slopes, settlements, transport network and forests of Jamui District were marked and identified, followed by Buffer Analysis that was used to find out the accessibility of features like roads, railway stations to the sites available for the development of ecotourism destinations. Buffer analysis is also carried out to get the spatial proximity of major river banks, lakes, and dam sites to be selected for promoting sustainable ecotourism. Overlay Analysis is conducted using the geo-processing tools. Digital Terrain Model (DEM) generated and relevant themes like roads, forest areas and settlements were draped on the DEM to make an assessment of the topography and other land uses of district to delineate potential zones of ecotourism development. Development of ecotourism in Jamui faces several challenges. The district lies in the portion of Bihar that is part of ‘red corridor’ of India. The hills and dense forests are the prominent hideouts and training ground for the extremists. It is well known that any kind of political instability, war, acts of violence directly influence the travel propensity and hinders all kind of non-essential travels to these areas. The development of ecotourism in the district can bring change and overall growth in this area with communities getting more involved in economically sustainable activities. It is a known fact that poverty and social exclusion are the main force that pushes people, resorting towards violence. All over the world tourism has been used as a tool to eradicate poverty and generate good will among people. In this paper, the case of meeting a household’s electrical energy demand with hybrid systems has been examined. The objective is to study technological feasibility and economic viability of the electrification project by a hybrid system (PV/ wind) of a residential home located in Batna-Algeria and to reduce the emissions from traditional power by using renewable energy. An autonomous hybrid wind/photovoltaic (PV)/battery power system and a PV/Wind grid connected system, has been carried out using Hybrid Optimization Model for Electric Renewable (HOMER) simulation software. In this paper with designing and proposing a compound of a heating and cooling system has been tried to show effect of this system on preventing esophagus cancer that can be caused by hot and cold drinks such as tea, coffee and ice water. This system has been simulated mechanically by fluent software and also has been validated by experimental way and a comprehensive result has been presented. Both of solution ways show that this system can reduce or increase temperature of drink to safe very dramatically and it can be a huge step toward consuming drinks safely and also it can be efficient about time issues. The system consists of a temperature sensor and an electronic controller that has a computer program to act automatically this task. Abstract:
This paper presents a software solution to enhance the content and presentation of graphic design and animation related textbooks. Using augmented and virtual reality concepts, a mobile application is developed to improve the static material found in books. This allows users to interact with animated examples and tutorials using their mobile phones and stereoscopic 3D viewers which will enhance information delivery. The application is tested on Google Cardboard with visual content in 3D space. Abstract:
Air plasma spraying system was utilized to deposit Ni3Al coatings on AISI 321 steel samples. After thermal spraying, the nickel aluminide intermetallic coatings were isothermal heat treated at various temperatures. In this regard, temperatures from 500 °C to 800 °C with 100 °C increments were selected. The coatings were soaked for 10, 30, 60 and 100 hours at the mentioned temperatures. These coatings were then tested by a pin on disk method. It was observed that the coatings exposed at comparatively higher temperature experienced lower wear rate. The decrease in wear rate is due to the formation of NiO phase. Further, the as sprayed and heat treated coatings were characterized by other tools such as Microhardness testing, optical and scanning electron microscopy (SEM) and X-Ray diffraction analysis. After isothermal heat treatment, NiO was observed the main phase by X-Ray diffraction technique. Abstract:
Autonomous conversational agents, i.e., chatbots, are becoming an increasingly common mechanism for enterprises to provide support to customers and partners. In order to rate chatbots, especially ones powered by Generative AI tools like Large Language Models (LLMs), we need to be able to accurately assess their performance. This is where chatbot benchmarking becomes important. In this paper, authors propose the use of a benchmark that they call the E2E (End to End) benchmark and show how the E2E benchmark can be used to evaluate the accuracy and usefulness of the answers provided by chatbots, especially ones powered by LLMs. The authors evaluate an example chatbot at different levels of sophistication based on both our E2E benchmark as well as other available metrics commonly used in the state of the art and observe that the proposed benchmark shows better results compared to others. In addition, while some metrics proved to be unpredictable, the metric associated with the E2E benchmark, which uses cosine similarity, performed well in evaluating chatbots. The performance of our best models shows that there are several benefits of using the cosine similarity score as a metric in the E2E benchmark.Keywords: chatbot benchmarking, end-to-end (E2E) benchmarking, large language model, user centric evaluation.

With the incessant increase of power systems capacity and voltage grade, the safety of grounding grid becomes more and more prominent. In this paper, the designing substation grounding grid is presented by means of genetic algorithm (GA). This approach purposes to control the grounding cost of the power system with the aid of controlling grounding rod number and conductor lengths under the same safety limitations. The proposed technique is used for the design of the substation grounding grid in Khalda Petroleum Company “El-Qasr” power plant and the design was simulated by using CYMGRD software for results verification.
