56 Incidence and Risk Factors of Traumatic Lumbar Puncture in Newborns in a Tertiary Care Hospital

Abstract:
Background: Traumatic lumbar puncture (LP) is a common occurrence and causes substantial diagnostic ambiguity. There is paucity of data regarding its epidemiology. Objective: To assess the incidence and risk factors of traumatic LP in newborns. Design/Methods: In a prospective cohort study, all inborn neonates admitted in NICU and planned to undergo LP for a clinical indication of sepsis were included. Neonates with diagnosed intraventricular hemorrhage (IVH) of grade III and IV were excluded. The LP was done by operator - often a fellow or resident assisted by bedside nurse. The unit has policy of not routinely using any sedation/analgesia during the procedure. LP is done by 26 G and 0.5-inch-long hypodermic needle inserted in third or fourth lumbar space while the infant is in lateral position. The infants were monitored clinically and by continuous measurement of vital parameters using multipara monitor during the procedure. The occurrence of traumatic tap along with CSF parameters and other operator and assistant characteristics were recorded at the time of procedure. Traumatic tap was defined as presence of visible blood or more than 500 red blood cells on microscopic examination. Microscopic trauma was defined when CSF is not having visible blood but numerous RBCs. The institutional ethics committee approved the study protocol. A written informed consent from the parents and the health care providers involved was obtained. Neonates were followed up till discharge/death and final diagnosis was assigned along with treating team. Results: A total of 362 (21%) neonates out of 1726 born at the hospital were admitted during the study period (July 2016 to January, 2017). Among these neonates, 97 (26.7%) were suspected of sepsis. A total of 54 neonates were enrolled who met the eligibility criteria and parents consented to participate in the study. The mean (SD) birthweight was 1536 (732) grams and gestational age 32.0 (4.0) weeks. All LPs were indicated for late onset sepsis at the median (IQR) age of 12 (5-39) days. The traumatic LP occurred in 19 neonates (35.1%; 95% C.I 22.6% to 49.3%). Frank blood was observed in 7 (36.8%) and in the remaining, 12(63.1%) CSF was detected to have microscopic trauma. The preliminary risk factor analysis including birth weight, gestational age and operator/assistant and other characteristics did not demonstrate clinically relevant predictors. Conclusion: A significant number of neonates requiring lumbar puncture in our study had high incidence of traumatic tap. We were not able to identify modifiable risk factors. There is a need to understand the reasons and further reduce this issue for improving management in NICUs.Keywords: incidence, newborn, traumatic, lumbar puncture

The aim of project was to introduce the unconventional materials for statement jewellery. The materials used for statement jewellery were waste Cd’s, and scrap fabric. These materials were amalgamated with the traditional raw materials such as beads, sequins, charms and chains to form unique jewellery sets. The sets were divided into two categories based on the type of raw material used i.e. Category 1: Clef-Cd Jewellery, Category 2: Crumb-Fabric Jewellery. Reinhard et al’s algorithm was the first one to solve this problem of color transfer. In this paper, we make this algorithm efficient by introducing domain parallelism among different processors. We also comment on the factors that affect the speedup of this problem. The edges of low contrast images are not clearly distinguishable to the human eye. It is difficult to find the edges and boundaries in it. The present work encompasses a new approach for low contrast images. The Chebyshev polynomial based fractional order filter has been used for filtering operation on an image. The preprocessing has been performed by this filter on the input image. Laplacian of Gaussian method has been applied on preprocessed image for edge detection. Abstract:
Streams and rivulets are crucial in maintaining river networks and their hydrology, influencing downstream ecosystems, and connecting different watersheds of urban and rural areas. The river Assi, an urban river, once a lifeline for the locals, has degraded over time. Evidence, such as the presence of paleochannels and patterns of water bodies and settlements, suggests that the river Assi was initially an alluvial stream or rivulet that originated near Rishi Durvasha Ashram near Prayagraj, flowing approximately 120 km before joining the river Ganga at Assi ghat in Varanasi. Presently, a major challenge is that nearly 90% of its original channel has been silted and disappeared, with only the last 8 km retaining some semblance of a river. It is possible that initially, the river Assi branched off from the river Ganga and functioned as a Yazoo stream. In this study, paleochannels of the river Assi were identified using Landsat 5 imageries and SRTM DEM. The study employed the Normalized Difference Vegetation Seasonality Index (NDVSI) and Principal Component Analysis (PCA) of the Normalized Difference Vegetation Index (NDVI) to detect these paleochannels. The average elevation of the sub-basin at the Durvasha Rishi Ashram of river Assi is 96 meters, while it reduces to 80 meters near its confluence with the Ganga in Varanasi, resulting in a 16-meter elevation drop along its course. There are 81 subbasins covering an area of 83,241 square kilometers. It is possible that due to the increased resistance in the flow of river Assi near urban areas of Varanasi, a new channel, Morwa, has originated at an elevation of 87 meters, meeting river Varuna at an elevation of 79 meters. The difference in elevation is 8 meters. Furthermore, the study explored the possibility of restoring the paleochannel of the river Assi and nearby ponds and water bodies to improve the river's base flow and overall hydrological conditions.Keywords: River Assi, small river restoration, paleochannel identification, remote sensing, GIS

This paper utilizes a comparative case study design to examine a regional onion market. The particular case of onion markets is used to understand perishable product supply chains. The site for the study is Dubai, United Arab Emirates. Results from a six-month field study are outlined. In particular, the findings suggest that firms should examine adding additional destinations to their supply chain. Further, we argue that utilizing Dubai as a supply chain hub is in certain cases counterproductive. Abstract:
A discrete time fractional orderdifferentiator has been modeled for estimating the fractional order derivatives of contaminated signal. The proposed approach is based on Chebyshev’s polynomials. We use the Riemann-Liouville fractional order derivative definition for designing the fractional order SG differentiator. In first step we calculate the window weight corresponding to the required fractional order. Then signal is convoluted with this calculated window’s weight for finding the fractional order derivatives of signals. The work in this paper presents simulative comparison for different modulation formats such as NRZ, Manchester and CRZ in a 100 subscribers at 5 Gbps bit rate Gigabit Ethernet Passive Optical Network (GEPON) FTTH network. It is observed from the simulation results that the CRZ modulation format is best suited for the designed system. A link design for 1:100 splitter is used as Passive Optical Network (PON) element which creates communication between central offices to different users. OCDM-RoF hybrid network of 16 users with DPSK modulation format has been designed and performance of proposed network is analyzed for 100, 150, and 200 km fiber span length under the influence of linear and nonlinear effect. Abstract:
In this work, we have synthesized BaTiO3 via sol gel method by sintering at different temperatures (600-1000 0C) and studied their structural, optical and ferroelectric properties through X-Ray diffraction (XRD), UV-Vis spectrophotometer and PE Loop Tracer. X-Ray diffraction patterns of barium titanate samples show that the peaks of the diffractogram are successfully indexed with the tetragonal structure of BaTiO3 along with some minor impurities of BaCO3. The optical band gap calculated through UV Visible spectrophotometer varies from 4.37 to 3.80 eV for the samples sintered at 600 to 1000 0 C, respectively. The particle size calculated through transmission electron microscopy varies from 20 to 60 nm for the samples sintered at 600 to 1000 0C, respectively. This project focuses on multiple access technique proposed in 4G communication systems. It is attempted to demonstrate the IDMA (Interleave Division Multiple Access) technology. The basic principle of IDMA is that interleaver is different for each user whereas CDMA employs different signatures. IDMA inherits many advantages of CDMA such as robust against fading, easy cell planning; dynamic channel sharing and IDMA increase the spectral efficiency and reduce the receiver complexity. In the present study, we have synthesized Cr and Fe doped zinc oxide (ZnO) nano-structures (Zn1-δCraFebO; where δ= a + b=20%, a = 5, 6, 8 & 10% and b=15, 14, 12 & 10%) via sol-gel method at different doping concentrations. The synthesized samples were characterized for structural properties by X-ray diffractometer and field emission scanning electron microscope and the optical properties were carried out through photoluminescence and UV-visible spectroscopy. The particle size calculated through field emission scanning electron microscope varies from 41 to 96 nm for the samples synthesized at different doping concentrations. The optical band gaps calculated through UV-visible spectroscopy are found to be decreasing from 3.27 to 3.02 eV as the doping concentration of Cr increases and Fe decreases.Keywords: nano-structures, optical properties, sol-gel method, zinc oxide

Abstract:
In this study, the performance analyses of the twenty five coal-fired power plants (CFPPs) used for electricity generation are carried out through various data envelopment analysis (DEA) models. Three efficiency indices are defined and pursued. During the calculation of the operational performance, energy and non-energy variables are used as input, and net electricity produced is used as desired output. CO2 emitted to the environment is used as the undesired output in the computation of the pure environmental performance while in Model-3 CO2 emissions is considered as detrimental input in the calculation of operational and environmental performance. Empirical results show that most of the plants are operating in increasing returns to scale region and Mettur plant is efficient one with regards to energy use and environment. The result also indicates that the undesirable output effect is insignificant in the research sample. The primary objective of this work was to study the effect of resin chemistry, pH and molarity of binding and elution buffer on aggregate removal using Cation Exchange Chromatography and find the optimum conditions which can give efficient aggregate removal with minimum loss of yield. Four different resins were used for carrying out the experiments: Fractogel EMD SO3-(S), Fractogel EMD COO-(M), Capto SP ImpRes and S Ceramic HyperD. Runs were carried out on the AKTA Avant system. Design of Experiments (DOE) was used for analysis using the JMP software. The dependence of the yield obtained using different resins on the operating conditions was studied. Success has been achieved in obtaining yield greater than 90% using Capto SP ImpRes and Fractogel EMD COO-(M) resins. Constrained Groove Pressing (CGP) is one of the severe plastic deformation technique (SPD) by which we can process Ultra Fine Grained (UFG)/plane metallic materials. This paper discusses the effects of CGP on Cu-Zn alloy specimen at room temperature. A comprehensive study is made on the structural and mechanical properties of Brass specimen before and after Constrained grooves Pressing. Entire process is simulated in AFDEX CAE Software. It is found that most of the properties are superior with respect to brass samples such as yield strength, ultimate tensile strength, hardness, strain rate, etc., and they are found to be better for the CGP processed specimen. To meet the growing need of high data rate and bandwidth, various efforts has been made nowadays for the efficient communication systems. Optical Code Division Multiple Access over Free space optics communication system seems an effective role for providing transmission at high data rate with low bit error rate and low amount of multiple access interference. This paper demonstrates the OCDMA over FSO communication system up to the range of 7000 m at a data rate of 5 Gbps. Initially, the 8 user OCDMA-FSO system is simulated and pseudo orthogonal codes are used for encoding. Also, the simulative analysis of various performance parameters like power and core effective area that are having an effect on the Bit error rate (BER) of the system is carried out. The Kalachuri dynasty of Tripuri was a significant ruling dynasty in central India that held power over a vast region for a longer period compared to renowned dynasties like the Chandellas. Their capital, Tripuri (modern-day Tewar, a small village near Jabalpur), and its surrounding area witnessed significant developments that were later disrupted by the Royal Indian Railways' construction of railway lines. However, remnants of their achievements can still be found scattered in and around Tewar. The Kalachuris made remarkable contributions in the fields of art, architecture, and iconography. The evolution of temple architecture, particularly in Baghelkhand and the Mahakoshal range after the decline of the Gupta Empire, can be attributed to the Kalachuris. There is a notable progression from early temple styles to mature architecture, with numerous examples displaying continuity between the two. One particularly unique temple style features a ground plan resembling a complete Chaitya Hall, while the elevation showcases a circular Grabhagriha with a Mandapa and a conical Shikhara adorned with a series of Gavakshas. This distinctive temple style is among the most exceptional in central India. While several studies have been conducted on the Kalachuris' architecture, there is still a need for further research, as recent discoveries have provided valuable insights into understanding their architectural achievements. Abstract:
Cloud computing is advancing with a rapid speed. Already, it has been adopted by a huge set of users. Easy to use and anywhere access like potential of cloud computing has made it more attractive relative to other technologies. This has resulted in reduction of deployment cost on user side. It has also allowed the big companies to sell their infrastructure to recover the installation cost for the organization. Roots of cloud computing have extended from Grid computing. Along with the inherited characteristics of its predecessor technologies it has also adopted the loopholes present in those technologies. Some of the loopholes are identified and corrected recently, but still some are yet to be rectified. Two major areas where still scope of improvement exists are security and performance. The proposed work is devoted to performance enhancement for the user of the existing cloud system by improving the basic throttled mapping approach between task and resources. The improved procedure has been tested using the cloud analyst simulator. Abstract:
The reliability of the various parts of electric mining shovel has been assessed through the application of Weibull Model. The study was initiated to find reliability of components of electric mining shovel. The paper aims to optimize the reliability of components and increase the life cycle of component. A multilevel decomposition of the electric mining shovel was done and maintenance records were used to evaluate the failure data and appropriate system characterization was done to model the system in terms of reasonable number of components. The approach used develops a mathematical model to assess the reliability of the electric mining shovel components. The model can be used to predict reliability of components of the hydraulic mining shovel and system performance. Reliability is an inherent attribute to a system. When the life-cycle costs of a system are being analyzed, reliability plays an important role as a major driver of these costs and has considerable influence on system performance. It is an iterative process that begins with specification of reliability goals consistent with cost and performance objectives. The data were collected from an Indian open cast coal mine and the reliability of various components of the electric mining shovel has been assessed by following a Weibull Model.Keywords: reliability, Weibull model, electric mining shovel

Abstract:
Mumbai’s Dhobighat, 150 year old largest open laundry in the world, is the true live-work place and only source of income for some of Mumbai’s highest density ‘urban poor’ residents. The regeneration of Dhobighat, due to its ultra prime location and complex socio-political culture has been a complex issue. This once flourishing urban industrial core has been degrading for the past several decades mainly due to the decline of the open laundry business, the site’s over burdened infrastructure and conflicting socio-political and economic forces. The phenomena of ‘exaptation’ or ‘co-option’ has been observed by evolutionary biologists as a process responsible for producing highly tenacious and resilient offsprings within a species. The reddish egret uses its wings to cast shadow in shallow waters to attract small fish and hunt them. An unrelated feature used opportunistically to produce a very favorable result. How can this idea of co-option be applied to resolve the complex issue of Dhobighat’s regeneration? Our paper proposes a new methodology/approach for the regeneration of Dhobighat through the lens of evolutionary biology. Forces and systems (social, political, economic, cultural and ecological) that seem conflicting or unrelated by nature are opportunistically transformed into symbiotic and complimentary relationships that produce an inclusive, resilient and holistic solution for the regeneration of Dhobighat.Keywords: urban regeneration, exaptation, resilience, Dhobighat, Mumbai
