6217 Cardiotrophin-1 and Leptin in Male Patients with Obstructive Sleep Apnea Syndrome
Authors: Isil Cakir, Mustafa Uluhan
Abstract:
Elevated serum Cardiotrophin-1 (CT-1) and leptin levels are important risk factors for cardiovascular diseases (CVDs). Obstructive sleep apnea syndrome (OSAS) has been reported to increase the risk of CVDs, too. The aim of this study was to evaluate the concentrations of serum CT-1 and leptin in these patients and whether their possible association with the disease severity. Fifty newly diagnosed patients with OSAS and thirty nonapneic snoring subjects were participated in this study. The mean ages of patients and control groups were 47.40±13.30 and 43.23±10.50 years, respectively (P=0.128). Fasting serum triglyseride, total cholesterol, LDL and HDL cholesterol, also CT-1 and leptin levels were evaluated. A significant difference was found in the serum CT-1 and leptin levels between the patients and the controls:serum median CT-1 levels in patients and control groups, respectively, were 19.47 and 8.23 pg/mL (P < 0.001) and leptin levels were 2.07 and 1.29 ng/mL (P < 0.001). In severe patients group (n=39), serum median CT-1 level was found statistically significantly higher than the median level in mild/moderate patients (n=11) group. Patients CT-1 concentrations were not associated with lipoprotein levels and there was no correlation between patients’ leptin and lipid profile parameters. Two risk factors for CVDs, CT-1 and leptin, have significantly elevated and they were associated with OSAS. Furthermore, CT-1 was associated with the severity of disease. We recommend the use of increased serum CT-1 and leptin concentrations as markers of the presence and severity of OSAS.They can be used as early markers in male OSAS patients without known CVDs.Keywords: obstructive sleep apnea syndrome, cardiotrophin-1, leptin, cardiovascular disease

Authors: Yuan Wan, Shumei Cui, Shaopeng Wu
Abstract:
Toroidal windings were taken advantage of to reduce of axial length of the motor, so as to match the applications that have severe restrictions on the axial length. But slotting in the out edge of the stator will decrease the heat-dissipation capacity of the water cooling of the housing. Besides, the windings in the outer slots will increase the copper loss, which will further increase the difficult for heat dissipation of the motor. At present, carbon-fiber composite retaining sleeve are increasingly used to be mounted over the magnets to ensure the rotor strength at high speeds. Due to the poor thermal conductivity of carbon-fiber sleeve, the cooling of the rotor becomes very difficult, which may result in the irreversible demagnetization of magnets for the excessively high temperature. So it is necessary to analyze the temperature rise of such motor. This paper builds a computational fluid dynamic (CFD) model of a toroidal-winding high-speed permanent magnet synchronous motor (PMSM) with water cooling of housing and forced air cooling of rotor. Thermal analysis was carried out based on the model and the factors that affects the temperature rise were investigated. Then thermal optimization for the prototype was achieved. Finally, a small-size prototype was manufactured and the thermal analysis results were verified. Keywords: thermal analysis, temperature rise, toroidal windings, high-speed PMSM, CFD

Authors: Yessica A. Aguilera
Abstract:
English is currently the only foreign language included in the national educational curriculum in Chile. The English curriculum establishes that once completed secondary education, students are expected to reach B1 level according to the Common European Reference Framework (CEFR) scale. However, the objective has not been achieved, and to the author’s best knowledge, there is still a severe lack of English language skills among students who have completed their secondary education studies. In order to deal with the fact that students do not manage English as expected, team-based learning (TBL) was introduced in English language lessons at the Universidad Federico Santa María (USM). TBL is a collaborative teaching-learning method which enhances active learning by combining individual and team work. This approach seeks to help students achieve course objectives while learning how to function in teams. The purpose of the research was to assess the implementation and effectiveness of TBL in English language classes at USM technical training education. Quantitative and qualitative data were collected from teachers and students about their experience through TBL. Research findings show that both teachers and students are satisfied with the method and that students’ engagement and participation in class is higher. Additionally, students score higher on examinations improving academic outcomes. The findings of the research have the potential to guide how TBL could be included in future English language courses. Keywords: collaborative learning, college education, English language learning, team-based learning
Procedia PDF Downloads 1926214 The Limits to Self-Defense Claims in Case of Domestic Violence Homicides
Authors: Maria Elisabete Costa Ferreira
Abstract:
Domestic violence is a serious social issue in which victims are mostly women. Domestic violence develops in cycles, starting with the building of tension, passing through the incident of abuse and ending with reconciliation, also known as honeymoon. As time goes by, the shorter these phases become, and the greater and more severe the attacks, rarely leading to the death of the victim of abuse. Sometimes, the victim stops the abuse by killing the aggressor, usually after the immediate aggression has taken place. This poses an important obstacle to the claim of self-defense by the victim of domestic violence pending trial for the homicide of her long-time abuser. The main problem with self-defense claims in such cases is that the law requires the act of aggression to be present or imminent (imminent threat or immediate danger) so that it permits the victim to take her defense into her own hands. If the episode of aggression has already taken place, this general requirement for the admissibility of self-defense is not satisfied. This paper sheds new light on the concept of the actuality of the aggression, understanding that, since domestic violence is a permanent offense, for as long as the victim stays under the domain of the aggressor, imminent threat will be present, allowing the self-defense claim of a woman who kills her abuser in such circumstances to be admissible. An actualist interpretation of the requirement of the necessity of the means used in self-defense will be satisfied when evaluated from the subjective perspective of the intimate partner victim. Necessity will be satisfied if it is reasonable for the victim to perceive the use of lethal force as the only means to release herself from the abuser. Keywords: domestic violence, homicide, self-defense, imminent threat, necessity of lethal force

Abstract:
This paper presents an incremental formal development of the Wireless Transaction Protocol (WTP) in Event-B. WTP is part of the Wireless Application Protocol (WAP) architectures and provides a reliable request-response service. To model and verify the protocol, we use the formal technique Event-B which provides an accessible and rigorous development method. This interaction between modelling and proving reduces the complexity and helps to eliminate misunderstandings, inconsistencies, and specification gaps. As result, verification of WTP allows us to find some deficiencies in the current specification. Keywords: event-B, wireless transaction protocol, proof obligation, refinement, Rodin, ProB

Authors: Mohamed Khali Zidi, Mohsen Henchiri, Walid Ben Ahmed
Abstract:
In the context of a hypogene hydrothermal travertine system, including low-porosity brittle bedrock and rock-mass permeability in Aptian dolostone of Boulaaba, Kasserine is enhanced through faulting and fracturing. This permeability enhancement related to the deformation modes along faults and fractures is likely to be in competition with permeability reduction when microcracks, fractures, and faults all become infilled with breccias and low-permeability hydrothermal precipitates. So that, fault continual or intermittent reactivation is probably necessary for them to keep their potential as structural high-permeability conduits. Dilational normal faults in strong mechanical stratigraphy associated with fault segments with dip changes are sites for porosity and permeability in groundwater infiltration and flow, hydrocarbon reservoirs, and also may be important sources of mineralization. The brecciation mechanism through dilational faulting and gravitational collapse originates according to hosting lithologies chaotic clast-supported breccia in strong lithologies such as sandstones, limestones, and dolostones, and matrix-supported cataclastic in weaker lithologies such as marls and shales. Breccias contribute to controlling fluid flow when the porosity is sealed either by low-permeability hydrothermal precipitates or by fine matrix materials. All these mechanisms of fault-related rock-mass permeability enhancement and reduction can be observed and analyzed in the region of Sidi Boulaaba, Kasserine, central Tunisia, where dilational normal faulting occurs in mechanical strong dolostone layering alternating with more weak marl and shale lithologies, has originated a variety of fault voids (fluid conduits) breccias (chaotic, crackle and mosaic breccias) and carbonate cement. Keywords: travertine, Aptian dolostone, Boulaaba, fracturing

Authors: Rupali Bhatia, Deepthi Nair, Geetika Khanna, Seema Singhal
Abstract:
Introduction: Genital tract tuberculosis is a chronic disease (caused by reactivation of organisms from systemic distribution of Mycobacterium tuberculosis) that often presents with low grade symptoms and non-specific complaints. Patients with genital tuberculosis are usually young women seeking workup and treatment for infertility. Infertility is the commonest presentation due to involvement of the fallopian tubes, endometrium and ovarian damage with poor ovarian volume and reserve. The diagnosis of genital tuberculosis is difficult because of the fact that it is a silent invader of genital tract. Since tissue cannot be obtained from fallopian tubes, the diagnosis is made by isolation of bacilli from endometrial tissue obtained by endometrial biopsy curettage and/or aspiration. Problems are associated with sampling technique as well as diagnostic modality due to lack of adequate sample volumes and the segregation of the sample for various diagnostic tests resulting in non-uniform distribution of microorganisms. Moreover, lack of an efficient sampling technique universally applicable for all specific diagnostic tests contributes to the diagnostic challenges. Endometrial sampling plays a key role in accurate diagnosis of female genital tuberculosis. It may be done by 2 methods viz. endometrial curettage and endometrial aspiration. Both endometrial curettage and aspirate have their own limitations as curettage picks up strip of the endometrium from one of the walls of the uterine cavity including tubal osteal areas whereas aspirate obtains total tissue with exfoliated cells present in the secretory fluid of the endometrial cavity. Further, sparse and uneven distribution of the bacilli remains a major factor contributing to the limitations of the techniques. The sample that is obtained by either technique is subjected to histopathological examination, AFB staining, culture and PCR. Aim: Comparison of the sampling techniques viz. endometrial biopsy curettage and endometrial aspiration using different laboratory methods of histopathology, cytology, microbiology and molecular biology. Method: In a hospital based observational study, 75 Indian females suspected of genital tuberculosis were selected on the basis of inclusion criteria. The women underwent endometrial tissue sampling using Novaks biopsy curette and Karmans cannula. One part of the specimen obtained was sent in formalin solution for histopathological testing and another part was sent in normal saline for acid fast bacilli smear, culture and polymerase chain reaction. The results so obtained were correlated using coefficient of correlation and chi square test. Result: Concordance of results showed moderate agreement between both the sampling techniques. Among HPE, AFB and PCR, maximum sensitivity was observed for PCR, though the specificity was not as high as other techniques. Conclusion: Statistically no significant difference was observed between the results obtained by the two sampling techniques. Therefore, one may use either EA or EB to obtain endometrial samples and avoid multiple sampling as both the techniques are equally efficient in diagnosing genital tuberculosis by HPE, AFB, culture or PCR.Keywords: acid fast bacilli (AFB), histopatholgy examination (HPE), polymerase chain reaction (PCR), endometrial biopsy curettage

Abstract:
Objectives. The age of the disease onset could have an impact on the effect of therapy in systemic sclerosis(SSc). Late-age onset in SSc could have a more severe course of the disease and worse clinical effects on therapy. The aim of our study was to evaluate changes in skin fibrosis on rituximab(RTX) therapy in patients with SSc and different ages of the disease onset. Methods. 151 patients with SSc were included in this study. Patients were divided into groups depending on the age of the disease onset: group 1 - younger than 30 years (40 patients(26%), group 2 - 31-59 years (90 patients(60%) and group 3 – more than 60 years (21 patients(14%). The mean follow-up period was 13±2.3month. The mean age was 48±13years, female-83% of patients, and the diffuse cutaneous subset of the disease had 52% of patients. The mean disease duration was 6.4±5years. The cumulative mean dose of RTX was 1.5±0.6grams. Patients received RTX as a therapy for interstitial lung disease. All patients received prednisone at a dose of 11.6±4.8mg/day, immunosuppressants received 48% of them. The results at baseline and at the end of the follow-up are presented in the form of mean values. Results. There was a significant decrease of modified Rodnan skin score(mRss) in all groups: in group 1 - from 10.2±8 to 7.7±6.5(p=0.01); in group 2 - from 9±7.2 to 6.2±4.7(p=0.0001); in group 3 - from 20.5±14.1 to 10.8±9.4(p=0.001). There was a significant decrease of the activity index (EScSG-AI): in group 1 from 2.5±1.8 to 1.3±1.1; in group 2 – from 3.2±1.6 to 1.5±1.2; in group 3 – from 4.2±2.1 to 1.3±1. Conclusion. There was a significant improvement in skin fibrosis in a year after initiation of RTX therapy regardless of the age of the disease onset. The improvement was more pronounced in the group with late-age onset of the disease, but these data require further investigations. Keywords: skin fibrosis, systemic sclerosis, rituximab, disease onset
Procedia PDF Downloads 376209 A New Manoeuvre for Prevention of Post-Partum Haemorrhage
Authors: Amr Hamdy
Abstract:
Background: Postpartum haemorrhage (PPH) is the leading cause of maternal mortality worldwide. Many methods have been developed to decrease its rate. The aim of this study was to evaluate the applicability of a new non-pharmacologic maneuver in decreasing its rate. Methods: This case series study was conducted in one centre in Cairo, Egypt, from January-2010 to June-2013. 400 pregnant–women aged 18 years or more and candidate for normal labour; were enrolled to this study. High-risk subjects for PPH were excluded. After placental delivery, the new maneuver was done by sustained traction of the anterior and posterior lips of the cervix by two ovum forceps for duration of 90 seconds. The amount of blood loss was estimated by standardized visual estimation after removal of the forceps. All subjects were followed up for 6 hours. Results: The rate of PPH, defined as more than 500 ml, was 8 cases (2%) with 95% CI (0.63-3.37%). The rate of PPH was not affected by parity, gestational age, episiotomy or the presence of tears. PPH is more in cases with anemia (p 0.032). It occurred in all cases with uterine atony (p <0.001). The range of estimated blood loss was 550-600ml in cases with PPH and 150-450ml in cases without PPH. Severe PPH more than 1000 ml, did not occur. Conclusion: This pilot study introduced a novel maneuver that can be helpful in decreasing the rate of PPH and reducing the amount of post partum blood loss.Despite the low rate of PPH showed in this study, the need for conducting a randomized controlled study is at its highest level before further inclusion of such manoeuvre to be a part of the current medical practice and before considering it as an evident tool to decrease the burden of PPH.Keywords: maternal mortality, new manoeuvre, post-partum haemorrhage, uterine atony

Authors: Sanjaya N. Munasinghe, R. Gnanasekeram, Dimuthu Tennakoon
Abstract:
During the last few years there has been a dramatic increase in the number of motorcyclists in Sri Lankan roads and thus an increase of motorcycle accidents (MCAs) with a heavy death and casualty toll. Extremity injuries due to MCAs cause a heavy burden on government hospitals. However, data on MCA injuries are limited. This study tries to determine the relationship between extremity injuries with protective gears and clothing motorcycle riders were wearing at the time of the accident. Data were collected from 410 motorcycle riders and passengers involved with MCAs and admitted to orthopedic and emergency observation wards in Teaching Hospital Kurunegala with extremity injuries between 1st February 2015 and 31st July 2015 using an interviewer administered questioner. Data were analyzed using SPSS version 17.0. Distal radial fracture is the most common upper extremity injury (12%), and Tibial fracture is the most common and severe lower extremity injury (23%). Very few participants were wearing safety gloves (2%) and jackets (10%). Most of the participants were wearing slippers (66%), short sleeved upper clothing (96%) and light cloth trousers (49%). According to Chi-square test associations were found between footwear and foot injuries (p-value - 0.001, Cramer's v-value - 0.203) and safety jacket and upper extremity injuries (p-value - 0.002, Cramer's v-value - 0.177). The results indicate that using safety gear can minimize the number of injuries in MCA victims. Thus it is necessary to ensure that motorcycle riders and pillion riders use proper safety gear. Keywords: extremity injuries, fractures, motorcycle accidents, safety gear
Procedia PDF Downloads 2966207 The Use of Ultrasound as a Safe and Cost-Efficient Technique to Assess Visceral Fat in Children with Obesity
Authors: Bassma A. Abdel Haleem, Ehab K. Emam, George E. Yacoub, Ashraf M. Salem
Abstract:
Background: Obesity is an increasingly common problem in childhood. Childhood obesity is considered the main risk factor for the development of metabolic syndrome (MetS) (diabetes type 2, dyslipidemia, and hypertension). Recent studies estimated that among children with obesity 30-60% will develop MetS. Visceral fat thickness is a valuable predictor of the development of MetS. Computed tomography and dual-energy X-ray absorptiometry are the main techniques to assess visceral fat. However, they carry the risk of radiation exposure and are expensive procedures. Consequently, they are seldom used in the assessment of visceral fat in children. Some studies explored the potential of ultrasound as a substitute to assess visceral fat in the elderly and found promising results. Given the vulnerability of children to radiation exposure, we sought to evaluate ultrasound as a safer and more cost-efficient alternative for measuring visceral fat in obese children. Additionally, we assessed the correlation between visceral fat and obesity indicators such as insulin resistance. Methods: A cross-sectional study was conducted on 46 children with obesity (aged 6–16 years). Their visceral fat was evaluated by ultrasound. Subcutaneous fat thickness (SFT), i.e., the measurement from the skin-fat interface to the linea alba, and visceral fat thickness (VFT), i.e., the thickness from the linea alba to the aorta, were measured and correlated with anthropometric measures, fasting lipid profile, homeostatic model assessment for insulin resistance (HOMA-IR) and liver enzymes (ALT). Results: VFT assessed via ultrasound was found to strongly correlate with the BMI, HOMA-IR with AUC for VFT as a predictor of insulin resistance of 0.858 and cut off point of >2.98. VFT also correlates positively with serum triglycerides and serum ALT. VFT correlates negatively with HDL. Conclusions: Ultrasound, a safe and cost-efficient technique, could be a useful tool for measuring the abdominal fat thickness in children with obesity. Ultrasound-measured VFT could be an appropriate prognostic factor for insulin resistance, hypertriglyceridemia, and elevated liver enzymes in obese children. Keywords: metabolic syndrome, pediatric obesity, sonography, visceral fat

Authors: Sonya Barzgar, J. Patrick, A. Hettiaratchi
Abstract:
Methane (CH4) is reputed as the second largest contributor to greenhouse effect with a global warming potential (GWP) of 34 related to carbon dioxide (CO2) over the 100-year horizon, so there is a growing interest in reducing the emissions of this gas. Methane biofiltration (MBF) is a cost effective technology for reducing low volume point source emissions of methane. In this technique, microbial oxidation of methane is carried out by methane-oxidizing bacteria (methanotrophs) which use methane as carbon and energy source. MBF uses a granular medium, such as soil or compost, to support the growth of methanotrophic bacteria responsible for converting methane to carbon dioxide (CO₂) and water (H₂O). Even though the biofiltration technique has been shown to be an efficient, practical and viable technology, the design and operational parameters, as well as the relevant microbial processes have not been investigated in depth. In particular, limited research has been done on the effects of sulphur on methane bio-oxidation. Since bacteria require a variety of nutrients for growth, to improve the performance of methane biofiltration, it is important to establish the input quantities of nutrients to be provided to the biofilter to ensure that nutrients are available to sustain the process. The study described in this paper was conducted with the aim of determining the influence of sulphur on methane elimination in a biofilter. In this study, a set of experimental measurements has been carried out to explore how the conversion of elemental sulphur could affect methane oxidation in terms of methanotrophs growth and system pH. Batch experiments with different concentrations of sulphur were performed while keeping the other parameters i.e. moisture content, methane concentration, oxygen level and also compost at their optimum level. The study revealed the tolerable limit of sulphur without any interference to the methane oxidation as well as the particular sulphur concentration leading to the greatest methane elimination capacity. Due to the sulphur oxidation, pH varies in a transient way which affects the microbial growth behavior. All methanotrophs are incapable of growth at pH values below 5.0 and thus apparently are unable to oxidize methane. Herein, the certain pH for the optimal growth of methanotrophic bacteria is obtained. Finally, monitoring methane concentration over time in the presence of sulphur is also presented for laboratory scale biofilters. Keywords: global warming, methane biofiltration (MBF), methane oxidation, methanotrophs, pH, sulphur

Authors: Kais Nasser, Ronit Levine-Schnur
Abstract:
Due to the prolonged negligence of planning institutions, a large portion of Palestinian localities in Israel lack basic infrastructure, development, and urbanism and suffer from a severe shortage of housing. In the past years, planning institutions in Israel began to promote master planning for Palestinian localities and for new neighborhoods. Land readjustment plans (PLIs) were the primary planning mechanism. According to Israel’s planning institutions, readjustment plans aimed to afford housing and to ensure that new neighborhoods enjoy developed infrastructure, modern construction, public lands and urbanism. However, a wide group of Palestinian landowners and stakeholders opposed PLIs. This article exposes the reasons behind such objections. Methodology: The research carried out an in-depth analysis of approximately 1,780 objections to PLIs that have been advanced in recent years. These objections reveal what really concerns landowners, what they defend indeed, and how planning institutions dealt with their arguments. Initial Findings: Exploring the objections submitted by landowners to readjustment plans reveals a conceptual and cultural conflict between landowners and the planning institutions. While planning institutions believe that these plans can transform landowners and Arab society in general from a rural, local, and conservative life to a modern- urban life, the landowners believe that planning institutions strive to change their way of life and force them to adopt an urban life without giving much attention and respect to their tradition, habits and cultural way of life. Keywords: land readjustment, culture, urbanization, minority

Abstract:
Object: Reversible cerebral vasospasm syndrome (RCVS) remains an underrated cause of thunderclap headache which shares similar history of the ‘worst-ever’ headache with subarachnoid hemorrhage (SAH) to the emergency physicians. This study evaluated the clinical manifestations, radiological features, and outcomes of patients with RCVS so that the physicians could raise the high index of suspicion to detect RCVS in more patients with thunderclap headache before having life-threatening complications. Methods: The electric medical records of 18 patients with diagnostic criteria of RCVS at the emergency department (ED) between January 2013 and December 2014 were retrospective reviewed. Results: The mean age was 50.7 years, and 80% were women. Patients with RCVS visit an average of 4.7 physicians before receiving an accurate diagnosis and mean duration of symptom until diagnosis is 9.3 days. All patients except one experienced severe headache, from 8 to 10 pain intensity on a numerical rating scale (NRS). 44% of patients had nausea as an associated symptom, 66% of patients experienced worsening of headache while gagging, leaning forward, defecating, urinating or having sex. The most frequently affected vessels are middle cerebral arteries demonstrating the characteristic diffuse “string of beads” appearance. Four patients had SAH as a complication. Conclusion: Patients with RCVS have a unique set of clinical and imaging features. Emergency physicians should raise the high index of suspicion to detect RCVS in more patients with thunderclap headache before life-threatening complications. Keywords: headache, thunderclap, subarachnoid haemorrhage, stroke

Abstract:
This report concerns the design, development, and analysis of the combined Darrieus and Savonius wind turbine. Vertical Axis Wind Turbines (VAWT's) are of two type's viz. Darrieus (lift type) and Savonius (drag type). The problem associated with Darrieus is the lack of self-starting while Savonius has low efficiency. There are 3 straight Darrieus blades having the cross-section of NACA(National Advisory Committee of Aeronautics) 0018 placed circumferentially and a helically twisted Savonius blade to get even torque distribution. This unique design allows the use of Savonius as a method of self-starting the wind turbine, which the Darrieus cannot achieve on its own. All the parts of the wind turbine are designed in CAD software, and simulation data were obtained via CFD(Computational Fluid Dynamics) approach. Also, the design was imported to FlashForge Finder to 3D print the wind turbine profile and finally, testing was carried out. The plastic material used for Savonius was ABS(Acrylonitrile Butadiene Styrene) and that for Darrieus was PLA(Polylactic Acid). From the data obtained experimentally, the hybrid VAWT so fabricated has been found to operate at the low cut-in speed of 3 m/s and maximum power output has been found to be 7.5537 watts at the wind speed of 6 m/s. The maximum rpm of the rotor blade is recorded to be 431 rpm(rotation per minute) at the wind velocity of 6 m/s, signifying its potentiality of wind power production. Besides, the data so obtained from both the process when analyzed through graph plots has shown the similar nature slope wise. Also, the difference between the experimental and theoretical data obtained has shown mechanical losses. The objective is to eliminate the need for external motors for self-starting purposes and study the performance of the model. The testing of the model was carried out for different wind velocities. Keywords: VAWT, Darrieus, Savonius, helical blades, CFD, flash forge finder, ABS, PLA

Abstract:
Saltwater intrusion is a common problem in estuaries around the world, as it could hinder the freshwater supply of coastal zones. This problem is likely to grow due to climate change and sea-level rise. The influence of these factors on the saltwater intrusion was investigated for the Cai River in the Khanh Hoa province in Vietnam. In addition, the Cai River has high seasonal fluctuations in discharge, leading to increased saltwater intrusion during the dry season. Sea level rise, river discharge changes, river mouth widening and a proposed saltwater intrusion prevention dam can have influences on the saltwater intrusion but have not been quantified for the Cai River estuary. This research used both an analytical and numerical model to investigate the effect of the aforementioned factors. The analytical model was based on a model proposed by Savenije and was calibrated using limited in situ data. The numerical model was a 3D hydrodynamic model made using the Delft3D4 software. The analytical model and numerical model agreed with in situ data, mostly for tidally average data. Both models indicated a roughly similar dependence on discharge, also agreeing that this parameter had the most severe influence on the modeled saltwater intrusion. Especially for discharges below 10 m/s3, the saltwater was predicted to reach further than 10 km. In the models, both sea-level rise and river widening mainly resulted in salinity increments up to 3 kg/m3 in the middle part of the river. The predicted sea-level rise in 2070 was simulated to lead to an increase of 0.5 km in saltwater intrusion length. Furthermore, the effect of the saltwater intrusion dam seemed significant in the model used, but only for the highest position of the gate. Keywords: Cai River, hydraulic models, river discharge, saltwater intrusion, tidal barriers

Abstract:
MicroRNAs (miRNAs) are short 20-23 nucleotide long non-coding RNAs; there are 2605 miRNA in humans and 1936 miRNA in mouse in total (miRBase). The nervous system expresses the most abundant miRNA and most diverse. MiRNAs play a role in many steps during neurogenesis, like cell proliferation, differentiation, neural patterning, axon pathfinding, etc. Moreover, in vitro studies suggested a role in the regulation of local translation at the synapse, thus controlling neuronal plasticity. However, due to the specific structure of miRNA molecules, an in-vivo confirmation of the general role of miRNAs in the control of neuronal plasticity is still pending. For example, their small size and their high level of sequence homology make difficult the analysis of their cellular and sub-cellular localization in-vivo by in-situ hybridization. Moreover, it was found that only 40% of the expressed miRNA molecules in a cell are included in RNA-Induced Silencing Complexes (RISC) and, therefore, involved in inhibitory interactions while the rest is silent. Definitively, the development of new tools is needed to have a better understanding of the cellular function of miRNAs, in particular their role in neuronal plasticity. Here we describe a new technique called in-vivo AGO-APP designed to investigate miRNA expression and function in-vivo. This technique is based on the expression of a small peptide derived from the human RISC-complex protein TNRC6B, called T6B, which binds all known Argonaute (Ago) proteins with high affinity allowing the efficient immunoprecipitation of AGO-bound miRNAs. We have generated two transgenic mouse lines conditionally expressing T6B either ubiquitously in the cell or targeted at the synapse. A comparison of the repertoire of miRNAs immuno-precipitated from mature neurons of both mouse lines will provide us with a list of miRNAs showing a specific activity at the synapse. The physiological role of these miRNAs will be subsequently addressed through gain and loss of function experiments. Keywords: RNA-induced silencing complexes, TNRC6B, miRNA, argonaute, synapse, neuronal plasticity, neurogenesis

Authors: Suvorov Vasyl, Filipchuk Viktor
Abstract:
Developmental Dysplasia of the Hip (DDH) is a frequent pathology in pediatric orthopedist’s practice. Neglected or residual cases of DDH in walking patients are usually treated using pelvic osteotomies. Plastic changes take place in hinge points due to acetabulum reorientation during surgery. Classically described hinge points and a traditional division of pelvic osteotomies on reshaping and reorientation are currently debated. The purpose of this article was to evaluate biomechanical changes during the most commonly used pelvic osteotomies (Salter, Dega, Pemberton) for DDH treatment in pediatric patients. Methods: virtual pelvic models of 2- and 6-years old patients were created, material properties were assigned, pelvic osteotomies were simulated and biomechanical changes were evaluated using finite element analysis (FEA). Results: it was revealed that the patient's age has an impact on pelvic bones and cartilages density (in younger patients the pelvic elements are more pliable - p<0.05). Stress distribution after each of the abovementioned pelvic osteotomy was assessed in 2- and 6-years old patients’ pelvic models; hinge points were evaluated. The new term "restriction point" was introduced, which means a place where restriction of acetabular deformity correction occurs. Pelvic ligaments attachment points were mainly these restriction points. Conclusions: it was found out that there are no purely reshaping and reorientation pelvic osteotomies as previously believed; the pelvic ring acts as a unit in carrying out the applied load. Biomechanical overload of triradiate cartilage during Salter osteotomy in 2-years old patient and in 2- and 6-years old patients during Pemberton osteotomy was revealed; overload of the posterior cortical layer in the greater sciatic notch in 2-years old patient during Dega osteotomy was revealed. Level of Evidence – Level IV, prognostic. Keywords: developmental dysplasia of the hip, pelvic osteotomy, finite element analysis, hinge point, biomechanics

Abstract:
In this paper, we introduce a mathematical algorithm which is used for estimating the states in the bilinear systems. This algorithm uses a special linearization of the second-order term by using the best available information about the state of the system. This technique makes our algorithm generalizes the well-known Kalman estimators. The system which is used here is of the bilinear class, the evolution of this model is linear-bilinear in the state of the system. Our algorithm can be used with linear and bilinear systems. We also here introduced a real application for the new algorithm to prove the feasibility and the efficiency for it.Keywords: estimation algorithm, bilinear systems, Kakman filter, second order linearization

Abstract:
Trinucleotide repeat disorders comprise ~20 severe, inherited human neuromuscular and neurodegenerative disorders, which are a result of an abnormal expansion of repetitive sequences in the DNA. The most common of these, Huntington’s disease, results from the expansion of the CAG repeat region in exon 1 of the HTT gene via an unknown mechanism. Non-coding RNAs have been implicated in the initiation and progression of many diseases; thus, we focus on one circular RNA (circRNA) molecule arising from non-canonical splicing (back splicing) of HTT pre-mRNA. This circRNA and its mouse orthologue were transgenically overexpressed in human cells (SHSY-5Y and HEK293T) and mouse cells (Mb1), respectively. High-content imaging and flow cytometry demonstrated the overexpression of this circRNA reduces cell proliferation, reduces nuclear size independent of cellular size, and alters cell cycle progression. Analysis of protein by western blot and immunofluorescence demonstrated no change to HTT protein levels but altered nuclear-cytoplasmic distribution without impacting the expansion of the HTT repeat region. As these phenotypic and genotypic changes are found in Huntington’s disease patients, these results may suggest that this circRNA may play a functional role in the progression of Huntington’s disease. Keywords: cell biology, circular RNAs, Huntington’s disease, molecular biology, neurodegenerative disorders
Procedia PDF Downloads 1026192 Dimensional Investigation of Food Addiction in Individuals Who Have Undergone Bariatric Surgery
Authors: Ligia Florio, João Mauricio Castaldelli-Maia
Abstract:
Background: Food addiction (FA) emerged in the 1990s as a possible contributor to the increasing prevalence of obesity and overweight, in conjunction with changing food environments and mental health conditions. However, FA is not yet listed as one of the disorders in the DSM-5 and/or the ICD-11. Although there are controversies and debates in the literature about the classification and construct of FA, the most common approach to access it is the use of a research tool - the Yale Food Addiction Scale (YFAS) - which approximates the concept of FA to the concept diagnosis of dependence on psychoactive substances. There is a need to explore the dimensional phenotypes accessed by YFAS in different population groups for a better understanding and scientific support of FA diagnoses. Methods: The primary objective of this project was to investigate the construct validity of the FA concept by mYFAS 2.0 in individuals who underwent bariatric surgery (n = 100) at the Hospital Estadual Mário Covas since 2011. Statistical analyzes were conducted using the STATA software. In this sense, structural or factor validity was the type of construct validity investigated using exploratory factor analysis (EFA) and item response theory (IRT) techniques. Results: EFA showed that the one-dimensional model was the most parsimonious. The IRT showed that all criteria contributed to the latent structure, presenting discrimination values greater than 0.5, with most presenting values greater than 2. Conclusion: This study reinforces a FA dimension in patients who underwent bariatric surgery. Within this dimension, we identified the most severe and discriminating criteria for the diagnosis of FA.Keywords: obesity, food addiction, bariatric surgery, regain

Authors: William Mahony, L. Jacqueline Hirth, Richard Rupp, Sandra Gonzalez, Roger Zoorob
Abstract:
Background: The impact of the COVID-19 pandemic on physicians has been considered by many researchers, but less is known about non-physician healthcare workers. The aim of this study is to examine the association of COVID-19 safety training and communication with stress. Methods: A 91-item online survey was distributed, starting January 2, 2021, to non-physician healthcare workers, including physician assistants, nurse practitioners, and medical assistants (MAs) in the United States through email and social media. A $1 donation was made to the Red Cross for each completed survey. The survey consisted of demographics, occupational questions, and perceived stress (perceived stress scale, PSS). Items on the PSS were combined for an overall score and categorized according to the severity of perceived stress. Chi-square tests were performed for bivariate analyses of categorical variables. Results: Of the 284 participants consenting to complete the survey, 197 participants completed the full survey. MAs made up most of the sample at 79%. Among all respondents, 47% had moderate PSS scores (scored between 14 and 26), and 51% had severe PSS scores (scored between 27 and 40). Unvaccinated participants reported statistically significantly lower levels of perceived stress (p = 0.002). Performing tasks outside of typical job responsibilities was not associated with PSS scores (p = .667). Discussion: Non-physician healthcare workers demonstrated a high level of perceived stress overall. The association between vaccination status and perceived stress should be examined in order to evaluate whether vaccination levels could be improved with further education about the virus and associated risks. Keywords: COVID-19, SARS-Cov-2, nursing, public health
Procedia PDF Downloads 1776188 Modelling and Simulation of Hysteresis Current Controlled Single-Phase Grid-Connected Inverter
Authors: Evren Isen
Abstract:
In grid-connected renewable energy systems, input power is controlled by AC/DC converter or/and DC/DC converter depending on output voltage of input source. The power is injected to DC-link, and DC-link voltage is regulated by inverter controlling the grid current. Inverter performance is considerable in grid-connected renewable energy systems to meet the utility standards. In this paper, modelling and simulation of hysteresis current controlled single-phase grid-connected inverter that is utilized in renewable energy systems, such as wind and solar systems, are presented. 2 kW single-phase grid-connected inverter is simulated in Simulink and modeled in Matlab-m-file. The grid current synchronization is obtained by phase locked loop (PLL) technique in dq synchronous rotating frame. Although dq-PLL can be easily implemented in three-phase systems, there is difficulty to generate β component of grid voltage in single-phase system because single-phase grid voltage exists. Inverse-Park PLL with low-pass filter is used to generate β component for grid angle determination. As grid current is controlled by constant bandwidth hysteresis current control (HCC) technique, average switching frequency and variation of switching frequency in a fundamental period are considered. 3.56% total harmonic distortion value of grid current is achieved with 0.5 A bandwidth. Average value of switching frequency and total harmonic distortion curves for different hysteresis bandwidth are obtained from model in m-file. Average switching frequency is 25.6 kHz while switching frequency varies between 14 kHz-38 kHz in a fundamental period. The average and maximum frequency difference should be considered for selection of solid state switching device, and designing driver circuit. Steady-state and dynamic response performances of the inverter depending on the input power are presented with waveforms. The control algorithm regulates the DC-link voltage by adjusting the output power. Keywords: grid-connected inverter, hysteresis current control, inverter modelling, single-phase inverter
