408 Performance Comparison of Space-Time Block and Trellis Codes under Rayleigh Channels
Authors: Jing Qingfeng, Wu Jiajia
Abstract:
Due to the crowded orbits and shortage of frequency resources, utilizing of MIMO technology to improve spectrum efficiency and increase the capacity has become a necessary trend of broadband satellite communication. We analyze the main influenced factors and compare the BER performance of space-time block code (STBC) scheme and space-time trellis code (STTC) scheme. This paper emphatically studies the bit error rate (BER) performance of STTC and STBC under Rayleigh channel. The main emphasis is placed on the effects of the factors, such as terminal environment and elevation angles, on the BER performance of STBC and STTC schemes. Simulation results indicate that performance of STTC under Rayleigh channel is obviously improved with the increasing of transmitting and receiving antennas numbers, but the encoder state has little impact on the performance. Under Rayleigh channel, performance of Alamouti code is better than that of STTC.Keywords: MIMO, space time block code (STBC), space time trellis code (STTC), Rayleigh channel

Authors: Naureen Akhtar
Abstract:
The stability of the protein in detergent-containing solution is the key for its successful crystallisation. Fluorescence-detection size-exclusion chromatography (FSEC) is a potential approach for screening monodispersity as well as the stability of protein in a detergent-containing-solution. In this present study, covalently linked Green Fluorescent Protein (GFP) to bacterial nitrate transporter, NarU from Escherichia coli was studied for pre-crystallisation trials by FSEC. Immobilised metal ion affinity chromatography (IMAC) and gel filtration were employed for their purification. The main objectives of this study were over-expression, detergent screening and crystallisation of nitrate transporter proteins. This study could not produce enough proteins that could realistically be taken forward to achieve the objectives set for this particular research. In future work, different combinations of variables like vectors, tags, creation of mutant proteins, host cells, position of GFP (N- or C-terminal) and/or membrane proteins would be tried to determine the best combination as the principle of technique is still promising. Keywords: transporters, detergents, over-expression, crystallography

Authors: Young-Jun You, Youn-Ju Jeong, Min-Su Park, Du-Ho Lee
Abstract:
Due to a lot of limited conditions, a large box type floating structure is inevitably constructed by connecting many pontoons. When a floating structure is made with concrete, concrete shear key with saw-teeth shape is often used to carry shear force. Match casting for the shear key and precise construction on a sea are very important for making separated two pontoons as one body but those are not easy work and may increase construction time and cost. To solve this problem, one-way shear key is studied in this paper for a connected part where there is some difference between upward and downward shear force. It has only one inclined plane and can resist shear force in one direction. Big shear force is resisted by concrete which forms an inclined plane and small shear force is resisted by steel bar. This system can reduce manufacturing cost of individual pontoon and construction time and cost for constructing a floating structure on a sea. In this paper, the feasibility study about one-way shear key system is performed by comparing with design example. Keywords: connection, floating container terminal, pontoon, pre-stressing, shear key

Authors: Syukri Himran, Rustan Taraka, Anto Duma
Abstract:
The aim of the study is to improve the understanding of latent and sensible thermal energy storage within a paraffin wax media by an array of cylindrical tubes arranged both in in-line and staggered layouts. An analytical and experimental study was carried out in a horizontal shell-and-tube type system during the melting process. Pertamina paraffin-wax was used as a phase change material (PCM), where as the tubes are embedded in the PCM. From analytical study we can obtain the useful information in designing a thermal energy storage such as : the motion of interface, amount of material melted at any time in the process, and the heat storage characteristic during melting. The use of staggered tubes is proposed as superior to in-line layout for thermal storage. The experimental study was used to verify the validity of the analytical predictions. From the comparisons, the analytical and experimental data are in a good agreement. Keywords: latent, sensible, paraffin-wax, thermal energy storage, conduction, natural convection

Authors: Jaechun No
Abstract:
As the technology of NAND flash memory rapidly grows, SSD is becoming an excellent alternative for storage solutions, because of its high random I/O throughput and low power consumption. These SSD potentials have drawn great attention from IT enterprises that seek for better I/O performance. However, high SSD cost per capacity makes it less desirable to construct a large-scale storage subsystem solely composed of SSD devices. An alternative is to build a hybrid storage subsystem where both HDD and SSD devices are incorporated in an economic manner, while employing the strengths of both devices. This paper presents a hybrid file system, called hybridFS, that attempts to utilize the advantages of HDD and SSD devices, to provide a single, virtual address space by integrating both devices. HybridFS not only proposes an efficient implementation for the file management in the hybrid storage subsystem but also suggests an experimental framework for making use of the excellent features of existing file systems. Several performance evaluations were conducted to verify the effectiveness and suitability of hybridFS.Keywords: hybrid file mapping, data layout, hybrid device integration, extent allocation
Procedia PDF Downloads 504403 Is Brain Death Reversal Possible in Near Future: Intrathecal Sodium Nitroprusside (SNP) Superfusion in Brain Death Patients=The 10,000 Fold Effect
Authors: Vinod Kumar Tewari, Mazhar Husain, Hari Kishan Das Gupta
Abstract:
Background: Primary or secondary brain death is also accompanied with vasospasm of the perforators other than tissue disruption & further exaggerates the anoxic damage, in the form of neuropraxia. In normal conditions the excitatory impulse propagates as anterograde neurotransmission (ANT) and at the level of synapse, glutamate activates NMDA receptors on postsynaptic membrane. Nitric oxide (NO) is produced by Nitric oxide Synthetase (NOS) in postsynaptic dendride or cell body and travels backwards across a chemical synapse to bind to the axon terminal of a presynaptic neuron for regulation of ANT this process is called as the retrograde neurotransmission (RNT). Thus the primary function of NO is RNT and the purpose of RNT is regulation of chemical neurotransmission at synapse. For this reason, RNT allows neural circuits to create feedback loops. The haem is the ligand binding site of NO receptor (sGC) at presynaptic membrane. The affinity of haem exhibits > 10,000-fold excess for NO than Oxygen (THE 10,000 FOLD EFFECT). In pathological conditions ANT, normal synaptic activity including RNT is absent. NO donors like sodium nitroprusside (SNP) releases NO by activating NOS at the level of postsynaptic area. NO now travels backwards across a chemical synapse to bind to the haem of NO receptor at axon terminal of a presynaptic neuron as in normal condition. NO now acts as impulse generator (at presynaptic membrane) thus bypasses the normal ANT. Also the arteriolar perforators are having Nitric Oxide Synthetase (NOS) at the adventitial side (outer border) on which sodium nitroprusside (SNP) acts; causing release of Nitric Oxide (NO) which vasodilates the perforators causing gush of blood in brain’s tissue and reversal of brain death. Objective: In brain death cases we only think for various transplantations but this study being a pilot study reverses some criteria of brain death by vasodilating the arteriolar perforators. To study the effect of intrathecal sodium nitroprusside (IT SNP) in cases of brain death in which: 1. Retrograde transmission = assessed by the hyperacute timings of reversal 2. The arteriolar perforator vasodilatation caused by NO and the maintenance of reversal of brain death reversal. Methods: 35 year old male, who became brain death after head injury and has not shown any signs of improvement after every maneuver for 6 hours, a single superfusion done by SNP via transoptic canal route for quadrigeminal cistern and cisternal puncture for IV ventricular with SNP done. Results: He showed spontaneous respiration (7 bouts) with TCD studies showing start of pulsations of various branches of common carotid arteries. Conclusions: In future we can give this SNP via transoptic canal route and in IV ventricle before declaring the body to be utilized for transplantations or dead or in broader way we can say that in near future it is possible to revert back from brain death or we have to modify our criterion. Keywords: brain death, intrathecal sodium nitroprusside, TCD studies, perforators, vasodilatations, retrograde transmission, 10, 000 fold effect
Procedia PDF Downloads 400402 Countercurrent Flow Simulation of Gas-Solid System in a Purge Column Using Computational Fluid Dynamics Techniques
Authors: T. J. Jamaleddine
Abstract:
Purge columns or degasser vessels are widely used in the polyolefin process for removing trapped hydrocarbons and in-excess catalyst residues from the polymer particles. A uniform distribution of purged gases coupled with a plug-flow characteristic inside the column system is desirable to obtain optimum desorption characteristics of trapped hydrocarbon and catalyst residues. Computational Fluid Dynamics (CFD) approach is a promising tool for design optimization of these vessels. The success of this approach is profoundly dependent on the solution strategy and the choice of geometrical layout at the vessel outlet. Filling the column with solids and initially solving for the solids flow minimized numerical diffusion substantially. Adopting a cylindrical configuration at the vessel outlet resulted in less numerical instability and resembled the hydrodynamics flow of solids in the hopper segment reasonably well. Keywords: CFD, degasser vessel, gas-solids flow, gas purging, purge column, species transport

Authors: Clive Greenstone, Kiara Lawrence
Abstract:
This conceptual paper explains certain influences of urban planning and urban form on the design layout of housing specific vulnerable members of society. It reimagines how to use vulnerable groups and spaces that are designed for them as interventions instead of using outside intervention within these vulnerable groups. Questions of what are needed to ensure that collective values, ethics and certain moral principles are taken into consideration when creating spaces for individuals and communities are challenging. This conceptual paper offers a more appropriate approach to both offer better urban settlements as well as help solve several challenges facing the most vulnerable groups in society, namely, the elderly, vulnerable children and vulnerable domestic animals into new housing settlements that create better social connections and physical and emotional well-being, labeled urban moral clusters. This conceptual paper offers two potential case studies where these new moral clusters can be implemented. Keywords: vulnerability, inclusivity, urban planning, social capital, moral clusters

Authors: Takashi Shimizu, Tomoaki Hashimoto
Abstract:
Recently, crystal growth technologies have made progress by the requirement for the high quality of crystal materials. To control the crystal growth dynamics actively by external forces is useuful for reducing composition non-uniformity. In this study, a control method based on model predictive control using thermal inputs is proposed for crystal growth dynamics of semiconductor materials. The control system of crystal growth dynamics considered here is governed by the continuity, momentum, energy, and mass transport equations. To establish the control method for such thermal fluid systems, we adopt model predictive control known as a kind of optimal feedback control in which the control performance over a finite future is optimized with a performance index that has a moving initial time and terminal time. The objective of this study is to establish a model predictive control method for crystal growth dynamics of semiconductor materials. Keywords: model predictive control, optimal control, process control, crystal growth

Authors: Pei-Shan Liang
Abstract:
Objective: This article explores the nursing experience of caring for a patient with terminal gastric cancer complicated by an abdominal aortic aneurysm. The patient experienced physical discomfort due to the disease, initially unable to accept the situation, leading to anxiety, and eventually accepting the need for surgery. Methods: The nursing period was from June 6 to June 10, 2024. Through observation, direct care, conversations, and physical assessments, and using Gordon's eleven functional health patterns for a one-on-one holistic assessment, interdisciplinary team meetings were held with the critical care team and family. Three nursing health issues were identified: pain related to the disease and invasive procedures, anxiety related to uncertainty about disease recovery, and decreased cardiac tissue perfusion related to hemodynamic instability. Results: Open communication techniques and empathetic care were employed to establish a trusting nurse-patient relationship, and patient-centered nursing interventions were developed. Pain was assessed using a 10-point pain scale, and pain medications were adjusted by a pharmacist. Initially, Fentanyl 500mcg with pump run at 1ml/hr was administered, later changed to Ultracet 37.5mg/325mg, 1 tablet every 6 hours orally, reducing the pain score to 3. Lavender aromatherapy and listening to crystal music were used as distractions to alleviate pain, allowing the patient to sleep uninterrupted for at least 7 hours. The patient was encouraged to express feelings and fears through LINE messages or drawings, and a psychologist was invited to provide support. Family members were present at least twice a day for over an hour each time, reducing psychological distress and uncertainty about the prognosis. According to the Beck Anxiety Inventory, the anxiety score dropped from 17 (moderate anxiety) to 6 (no anxiety). Focused nursing care was implemented with close monitoring of vital signs maintaining systolic blood pressure between 112-118 mmHg to ensure adequate myocardial perfusion. The patient was encouraged to get out of bed for postoperative rehabilitation and to strengthen cardiopulmonary function. A chest X-ray showed no abnormalities, and breathing was smooth with Triflow use, maintaining at least 5 seconds with 2 balls four times a day, and SpO2 >96%. Conclusion: The care process highlighted the importance of addressing psychological care in addition to maintaining life when the patient’s condition changes. The presence of family often provided the greatest source of comfort for the patient, helping to reduce anxiety and pain. Nurses must play multiple roles, including advocate, coordinator, educator, and consultant, using various communication techniques and fostering hope by listening to and accepting the patient’s emotional responses. It is hoped that this report will provide a reference for clinical nursing staff and contribute to improving the quality of care. Keywords: intensive care, gastric cancer, aortic aneurysm, quality of care

Authors: M. Moharami Gargari, S. Javdani Zamani, A. Mohammadnejad, S. Abuali
Abstract:
As maritime container transport is developing fast, the need arises for efficient operations at container terminals. One of the most important determinants of container handling efficiency is the productivity of quay cranes and internal transportation vehicles, which are responsible transporting of containers for unloading and loading operations for container vessels. For this reason, this paper presents an integrated mathematical model formulation for discrete berths with quay cranes and internal transportations vehicles. The problems have received increasing attention in the literature and the present paper deals with the integration of these interrelated problems. A new mixed integer linear formulation is developed for the Berth Allocation Problem (BAP), Quay Crane Assignment and Scheduling Problem (QCASP) and Internal Transportation Scheduling (ITS), which accounts for cranes and trucks positioning conditions. Keywords: discrete berths, container terminal, truck scheduling, dynamic vessel arrival

Authors: Yael Dubinsky, Orit Hazzan
Abstract:
There is a constant need for hi-tech innovation to be decentralized to peripheral regions. This work describes how we applied design science research (DSR) principles to define what we refer to as the Sea of Galilee (SoG) method. The goal of the SoG method is to harness existing and new technological initiatives in peripheral regions to create a socio-technological network that can initiate and maintain hi-tech activities. The SoG method consists of a set of principles, a stakeholder network, and actual hi-tech business initiatives, including their infrastructure and practices. The three cycles of DSR, the Relevance, Design, and Rigor cycles, layout a research framework to sharpen the requirements, collect data from case studies, and iteratively refine the SoG method based on the existing knowledge base. We propose that the SoG method can be deployed by regional authorities that wish to be considered smart regions (an extension of the notion of smart cities). Keywords: design science research, socio-technological initiatives, Sea of Galilee method, periphery stakeholder network, hi-tech initiatieves
Procedia PDF Downloads 129394 Development of a Serial Signal Monitoring Program for Educational Purposes
Authors: Jungho Moon, Lae-Jeong Park
Abstract:
This paper introduces a signal monitoring program developed with a view to helping electrical engineering students get familiar with sensors with digital output. Because the output of digital sensors cannot be simply monitored by a measuring instrument such as an oscilloscope, students tend to have a hard time dealing with digital sensors. The monitoring program runs on a PC and communicates with an MCU that reads the output of digital sensors via an asynchronous communication interface. Receiving the sensor data from the MCU, the monitoring program shows time and/or frequency domain plots of the data in real time. In addition, the monitoring program provides a serial terminal that enables the user to exchange text information with the MCU while the received data is plotted. The user can easily observe the output of digital sensors and configure the digital sensors in real time, which helps students who do not have enough experiences with digital sensors. Though the monitoring program was programmed in the Matlab programming language, it runs without the Matlab since it was compiled as a standalone executable. Keywords: digital sensor, MATLAB, MCU, signal monitoring program

Procedia PDF Downloads 192392 Buck Boost Inverter to Improve the Efficiency and Performance of E-Motor by Reducing the Influence of Voltage Sag of Battery on the Performance of E-Motor
Authors: Shefeen Maliyakkal, Pranav Satheesh, Steve Simon, Sharath Kuruppath
Abstract:
This paper researches the impact of battery voltage sag on the performance and efficiency of E-motor in electric cars. Terminal voltage of battery reduces with the S.o.C. This results in the downward shift of torque-speed curve of E-motor and increased copper losses in E-motor. By introducing a buck-boost inverter between the battery and E-motor, an additional degree of freedom was achieved. By boosting the AC voltage, the dependency of voltage sag on the performance of E-motor was eliminated. A strategy was also proposed for the operation of the buck-boost inverter to minimize copper and iron losses in E-motor to maximize efficiency. MATLAB-SIMULINK model of E-drive was used to obtain simulation results. The temperature rise in the E-motor was reduced by 14% for a 10% increase in AC voltage. From the results, it was observed that a 20% increase in AC voltage can result in improvement of running torque and maximum torque of E-motor by 44%. Hence it was concluded that using a buck-boost inverter for E-drive significantly improves both performance and efficiency of E-motor.Keywords: buck-boost, E-motor, battery, voltage sag

Authors: Guo-Ming Sung, Ramavath Naga Raju Naik
Abstract:
This paper presents a low power, high speed, sense-amplifier based flip-flop (SAFF). The flip-flop’s power con-sumption and delay are greatly reduced by employing a new conditionally precharge sense-amplifier stage and a single-ended latch stage. Glitch-free and contention-free latch operation is achieved by using a conditional cut-off strategy. The design uses fewer transistors, has a lower clock load, and has a simple structure, all of which contribute to a near-zero setup time. When compared to previous flip-flop structures proposed for similar input/output conditions, this design’s performance and overall PDP have improved. The post layout simulation of the circuit uses 2.91µW of power and has a delay of 65.82 ps. Overall, the power-delay product has seen some enhancements. Cadence Virtuoso Designing tool with CMOS 90nm technology are used for all designs. Keywords: high-speed, low-power, flip-flop, sense-amplifier

Authors: Fengmei Li, Li Xu, Guoliang Xia
Abstract:
Whey acidic proteins (WAP) domain-containing proteins in crustacean are involved in innate immune response against microbial invasion. In the present study, a novel double WAP domain (DWD)-containing protein gene 3 was identified from Chinese mitten crab Eriocheir sinensis (designated EsDWD3) by expressed sequence tag (EST) analysis and PCR techniques. The full-length cDNA of EsDWD3 was of 1223 bp, consisting of a 5′-terminal untranslated region (UTR) of 74 bp, a 3′ UTR of 727 bp with a polyadenylation signal sequence AATAAA and a polyA tail, and an open reading frame (ORF) of 423 bp. The ORF encoded a polypeptide of 140 amino acids with a signal peptide of 22 amino acids. The deduced protein sequence EsDWD3 showed 96.4 % amino acid similar to other reported EsDWD1 from E. sinensis, and phylogenetic tree analysis revealed that EsDWD3 had closer relationships with the reported two double WAP domain-containing proteins of E. sinensis species. Keywords: Chinese mitten crab, Eriocheir sinensis, cloning, double WAP domain-containing protein

Authors: Mohamed M. Shahin
Abstract:
This study investigated the design of the pile foundation to support heavy structures-especially bridges for highways-in the Sahara, which contains many dunes of medium dense sand in different levels, where the foundation is supposed to be piles. The base resistance of smooth model pile groups in sand under static loading is investigated experimentally in a pile soil test apparatus. Improvement were made to the sand around the piles in order to increase the shaft resistance of the single pile and the pile groups, and also base resistance especially for the central pile in pile groups. The study outlines the behaviour of a single-pile, 4-, 5-, and 9- pile groups arranged in a doubly symmetric [square] layout with different embedment lengths and pile spacing in loose dry sand [normal] and dense dry sand [compacted] around the piles. This study evaluate the variation of the magnitude and the proportion of end bearing capacity of individual piles in different pile groups. Also to investigate the magnitude of the efficiency coefficient in the case of different pile groups. Keywords: pile group, base resistance, efficiency coefficient, pile spacing, pile-soil interaction
Procedia PDF Downloads 362388 The 10,000 Fold Effect of Retrograde Neurotransmission, a New Concept for Stroke Revival: Use of Intracarotid Sodium Nitroprusside
Authors: Vinod Kumar
Abstract:
Background: Tissue Plasminogen Activator (tPA) showed a level 1 benefit in acute stroke (within 3-6 hrs). Intracarotid sodium nitroprusside (ICSNP) has been studied in this context with a wide treatment window, fast recovery and affordability. This work proposes two mechanisms for acute cases and one mechanism for chronic cases, which are interrelated, for physiological recovery. a)Retrograde Neurotransmission (acute cases): 1)Normal excitatory impulse: at the synaptic level, glutamate activates NMDA receptors, with nitric oxide synthetase (NOS) on the postsynaptic membrane, for further propagation by the calcium-calmodulin complex. Nitric oxide (NO, produced by NOS) travels backward across the chemical synapse and binds the axon-terminal NO receptor/sGC of a presynaptic neuron, regulating anterograde neurotransmission (ANT) via retrograde neurotransmission (RNT). Heme is the ligand-binding site of the NO receptor/sGC. Heme exhibits > 10,000-fold higher affinity for NO than for oxygen (the 10,000-fold effect) and is completed in 20 msec. 2)Pathological conditions: normal synaptic activity, including both ANT and RNT, is absent. A NO donor (SNP) releases NO from NOS in the postsynaptic region. NO travels backward across a chemical synapse to bind to the heme of a NO receptor in the axon terminal of a presynaptic neuron, generating an impulse, as under normal conditions. b)Vasospasm: (acute cases) Perforators show vasospastic activity. NO vasodilates the perforators via the NO-cAMP pathway. c)Long-Term Potentıatıon (LTP): (chronic cases) The NO–cGMP-pathway plays a role in LTP at many synapses throughout the CNS and at the neuromuscular junction. LTP has been reviewed both generally and with respect to brain regions specific for memory/learning. Aims/Study Des’gn: The principles of “generation of impulses from the presynaptic region to the postsynaptic region by very potent RNT (10,000-fold effect)” and “vasodilation of arteriolar perforators” are the basis of the authors’ hypothesis to treat stroke cases. Case-control prospective study. Mater’als And Methods: The experimental population included 82 stroke patients (10 patients were given control treatments without superfusion or with 5% dextrose superfusion, and 72 patients comprised the ICSNP group). The mean time for superfusion was 9.5 days post-stroke. Pre- and post-ICSNP status was monitored by NIHSS, MRI and TCD. Results: After 90 seconds in the ICSNP group, the mean change in the NIHSS score was a decrease of 1.44 points, or 6.55%; after 2 h, there was a decrease of 1.16 points; after 24 h, there was an increase of 0.66 points, 2.25%, compared to the control-group increase of 0.7 points, or 3.53%; at 7 days, there was an 8.61-point decrease, 44.58%, compared to the control-group increase of 2.55 points, or 22.37%; at 2 months in ICSNP, there was a 6.94-points decrease, 62.80%, compared to the control-group decrease of 2.77 points, or 8.78%. TCD was documented and improvements were noted. Conclusions: ICSNP is a swift-acting drug in the treatment of stroke, acting within 90 seconds on day 9.5 post-stroke with a small decrease after 24 hours. The drug recovers from this decrease quickly. Keywords: brain infarcts, intracarotid sodium nitroprusside, perforators, vasodilatıons, retrograde transmission, the 10, 000-fold effect

Authors: Chaozhi Yang, Xinzhong Chen, Guoqing Huang
Abstract:
Inerter is a two-terminal inertial element that can produce apparent mass far larger than its physical mass. A double tuned inertial mass damper (DTIMD) is developed by combining a spring with an inerter and a dashpot in series to replace the viscous damper of a tuned mass damper (TMD), and its performance is investigated. Firstly, the DTIMD is optimized numerically with H∞ and H2 methods considering the system’s damping based on the single-degree-of-freedom (SDOF)-DTIMD system, and the optimal structural parameters are obtained. Then, compared with a TMD, the control effect of the DTIMD with the optimal structural parameters on wind-induced vibration of a wind turbine in downwind direction under the shutdown condition is studied. The results demonstrate that the vibration suppression of the DTIMD is superior than that of a TMD at the same mass ratio. And at the identical vibration suppression, the tuned mass of the DTIMD can be reduced by up to 40% compared with a TMD.Keywords: wind-induced vibration, vibration control, inerter, tuned mass damper, damped system

Abstract:
Recent technological advance has prompted significant interest in developing the control theory of quantum systems. Following the increasing interest in the control of quantum dynamics, this paper examines the control problem of Schrödinger equation because quantum dynamics is basically governed by Schrödinger equation. From the practical point of view, stochastic disturbances cannot be avoided in the implementation of control method for quantum systems. Thus, we consider here the robust stabilization problem of Schrödinger equation against stochastic disturbances. In this paper, we adopt model predictive control method in which control performance over a finite future is optimized with a performance index that has a moving initial and terminal time. The objective of this study is to derive the stability criterion for model predictive control of Schrödinger equation under stochastic disturbances. Keywords: optimal control, stochastic systems, quantum systems, stabilization

Authors: V. K. Tewari, M. Hussain, H. K. D. Gupta
Abstract:
Background: Nitric Oxide Donars (NODs) (intrathecal sodium nitroprusside (ITSNP) and oral tadalafil 20mg post ITSNP) has been studied in this context in cerebral palsy patients for fast recovery. This work proposes two mechanisms for acute cases and one mechanism for chronic cases, which are interrelated, for physiological recovery. a) Retrograde Neurotransmission (acute cases): 1) Normal excitatory impulse: at the synaptic level, glutamate activates NMDA receptors, with nitric oxide synthetase (NOS) on the postsynaptic membrane, for further propagation by the calcium-calmodulin complex. 2) Pathological conditions: normal synaptic activity, including both ANT and RNT, is absent. A NO donor (SNP) releases NO from NOS in the postsynaptic region. NO travels backward across a chemical synapse to bind to the heme of a NO receptor in the axon terminal of a presynaptic neuron, generating an impulse, as under normal conditions. b) Vasopasm: (acute cases) Perforators show vasospastic activity. NO vasodilates the perforators via the NO-cAMP pathway. c) Long-Term Potentiation (LTP): (chronic cases) The NO–cGMP-pathway plays a role in LTP at many synapses throughout the CNS and at the neuromuscular junction. LTP has been reviewed both generally and with respect to brain regions specific for memory/learning. Aims/Study Design: The principles of “generation of impulses from the presynaptic region to the postsynaptic region by very potent RNT (10,000-fold effect)” and “vasodilation of arteriolar perforators” are the basis of the authors’ hypothesis to treat cerebral palsy cases. Case-control prospective study. Materials and Methods: The experimental population included 82 cerebral palsy patients (10 patients were given control treatments without NOD or with 5% dextrose superfusion, and 72 patients comprised the NOD group). The mean time for superfusion was 5 months post-cerebral palsy. Pre- and post-NOD status was monitored by Gross Motor Function Classification System for Cerebral Palsy (GMFCS), MRI, and TCD studies. Results: After 7 days in the NOD group, the mean change in the GMFCS score was an increase of 1.2 points mean; after 3 months, there was an increase of 3.4 points mean, compared to the control-group increase of 0.1 points at 3 months. MRI and TCD documented the improvements. Conclusions: NOD (ITSNP boosts up the recovery and oral tadalafil maintains the recovery to a well-desired level) acts swiftly in the treatment of CP, acting within 7 days on 5 months post-cerebral palsy either of the three mechanisms. Keywords: cerebral palsy, intrathecal sodium nitroprusside, oral tadalafil, perforators, vasodilations, retrograde transmission, the 10, 000-fold effect, long-term potantiation

Authors: Foroozan Sadri, Mohammadmehdi Moulaii, Farkhondeh Vahdati
Abstract:
Smart materials can create a great revolution in our built environment, as living systems do. In this research, the optimal structure of healthcare building envelopes is analyzed in terms of thickness according to the utility of the smart-nano materials as nontoxic substances in the region. The research method in this paper is based on library studies and simulation. Grasshopper program is employed to simulate thermal characteristics to achieve the optimum U-value in Semnan desert climate, according to Iranian national standards. The potential of healthcare envelope layouts in thermal properties development (primarily U-value) of these buildings is discussed due to the high thermal loads of healthcare buildings and also toxicity effects of conventional materials. As a result, envelope thicknesses are calculated, and the performance of the nano-PCM and gypsum wallboards are compared. A solution with comparable performance using smart-nano materials instead of conventional materials would determine a decrease in wall thickness. Keywords: energy saving, exterior envelope, smart-nano materials, thermal performance, U-value

Authors: Dalia Almaghaslah
Abstract:
Design thinking approach; empathize, define, ideate prototype, test, implement, was used to assess outpatient experiences in two hospital pharmacies in the Asir region, Saudi Arabia. Semi-structured interviews were conducted with 40 patients. The data were analyzed using thematic analysis. The findings suggested that patients were generally satisfied with pharmaceutical services provided in both pharmacies. Pharmacists were found to have enough knowledge, good attitude, and efficient communication and counselling skills. Non-pharmacy-related factors such as cultural factors (gender segregation), long waiting times, uncomfortable waiting areas, lack of electronic prescribing, number waiting system were found to have a negative impact on patients' experiences and satisfaction. Prototypes will be used to test the effects of implementing the electronic system in Al -mahal hospital and to test changing the physical layout of the waiting area in Asir hospital. Keywords: design thinking, hospital pharmacy, patient satisfaction, Saudi Arabia

Authors: Xin Jie Li
Abstract:
Due to population agglomeration, huge scale, and complex activities, megacities have become risk centers. To resist the risks brought by development uncertainty, the construction of resilient cities has become a common strategic choice for megacities. As a key link in promoting the modernization of the national governance system and governance capacity, optimizing the layout of national land space that focuses on ecology, production, and life and improving the rationality of spatial resource allocation are conducive to fundamentally promoting the resilience construction of megacities. Therefore, based on the perspective of territorial space governance, this article explores the potential risks faced by the territorial space of megacities and proposes possible paths for the resilience construction of megacities from four aspects: promoting the construction of a resilience system throughout the entire life cycle, constructing a disaster prevention and control system with ecological resilience, creating an industrial spatial pattern with production resilience, and enhancing community resilience to anchor the front line of risk response in megacities. Keywords: mega cities, potential risks, resilient city construction, territorial and spatial governance
