12 The Effects of a Hippotherapy Simulator in Children with Cerebral Palsy: A Pilot Study

Abstract:
Background: Hippotherapy considered as global techniques used in rehabilitation of children with cerebral palsy as it improved gait pattern, balance, postural control, balance and gross motor skills development but it encounters some problems (such as the excess of the cost of horses' care, nutrition, housing). Hippotherapy simulator is being developed in recent years to overcome these problems. These devices aim to create the effects of hippotherapy made with a real horse on patients by simulating the movements of a real horse. Objectives: To evaluate the efficacy of hippotherapy simulator on gross motor functions, sitting postural control and dynamic balance of children with cerebral palsy (CP). Methods: Fourteen children with CP, aged 6–15 years, seven with a diagnosis of spastic hemiplegia, five of diplegia, two of triplegia, Gross Motor Function Classification System level I-III. The Horse Riding Simulator (HRS), including four-speed program (warm-up, level 1-2-3), was used for hippotherapy simulator. Firstly, each child received Neurodevelopmental Therapy (NDT; 45min twice weekly eight weeks). Subsequently, the same children completed HRS+NDT (30min and 15min respectively, twice weekly eight weeks). Children were assessed pre-treatment, at the end of 8th and 16th week. Gross motor function, sitting postural control, dynamic sitting and standing balance were evaluated by Gross Motor Function Measure-88 (GMFM-88, Dimension B, D, E and Total Score), Trunk Impairment Scale (TIS), Pedalo® Sensamove Balance Test and Pediatric Balance Scale (PBS) respectively. Unit of Scientific Research Project of Marmara University supported our study. Results: All measured variables were a significant increase compared to baseline values after both intervention (NDT and HRS+NDT), except for dynamic sitting balance evaluated by Pedalo®. Especially HRS+NDT, increase in the measured variables was considerably higher than NDT. After NDT, the Total scores of GMFM-88 (mean baseline 62,2 ± 23,5; mean NDT: 66,6 ± 22,2; p < 0,05), TIS (10,4 ± 3,4; 12,1 ± 3; p < 0,05), PBS (37,4 ± 14,6; 39,6 ± 12,9; p < 0,05), Pedalo® sitting (91,2 ± 6,7; 92,3 ± 5,2; p > 0,05) and Pedalo® standing balance points (80,2 ± 10,8; 82,5 ± 11,5; p < 0,05) increased by 7,1%, 2%, 3,9%, 5,2% and 6 % respectively. After HRS+NDT treatment, the total scores of GMFM-88 (mean baseline: 62,2 ± 23,5; mean HRS+NDT: 71,6 ± 21,4; p < 0,05), TIS (10,4 ± 3,4; 15,6 ± 2,9; p < 0,05), PBS (37,4 ± 14,6; 42,5 ± 12; p < 0,05), Pedalo® sitting (91,2 ± 6,7; 93,8 ± 3,7; p > 0,05) and standing balance points (80,2 ± 10,8; 86,2 ± 5,6; p < 0,05) increased by 15,2%, 6%, 7,3%, 6,4%, and 11,9%, respectively, compared to the initial values. Conclusion: Neurodevelopmental therapy provided significant improvements in gross motor functions, sitting postural control, sitting and standing balance of children with CP. When the hippotherapy simulator added to the treatment program, it was observed that these functions were further developed (especially with gross motor functions and dynamic balance). As a result, this pilot study showed that the hippotherapy simulator could be a useful alternative to neurodevelopmental therapy for the improvement of gross motor function, sitting postural control and dynamic balance of children with CP.Keywords: balance, cerebral palsy, hippotherapy, rehabilitation

Abstract:
Hoşap Castle, located in Gürpınar, Van, is one of the most important symbols of the city because it hosted sacred memories of its time. Besides the location and construction features of Güzelsu, in resort city of Van, Hoşap Castle is a great place with an architecture consisting of an outer fortress and the inner fortress. It is one of the Ottoman castles and was built in the 17th century by Sarı Süleyman who was known as bey of Mahmudi. Although some parts of Hoşap Castle have been destroyed by natural disasters, it has survived until today without total collapse and most places with excavations are revealed. With the increasing competition in recent years, airline companies tend to manage their operations aiming fewer losses in a robust manner. Airline operations are complex operations and have the necessity of being performed just in time and more knock-on relevant elements in the event of a disruption. In this study a knowledge based decision support system is suggested and software is developed. The developed software includes knowledge bases which are based on expert experience and government regulations, model bases and data bases. Abstract:
Nowadays, in a furniture sector, competition of market share (portion) and production variety and changeability enforce the firm to reengineer operations on manufacturing line to increase the productivity. In this study, spring mattress manufacturing line of the furniture manufacturing firm is analyzed analytically. It’s intended to search and find the bottlenecks of production to balance the semi-finished material flow. There are four base points required to investigate in bottleneck elimination process. These are bottlenecks of Method, Material, Machine and Man (work force) resources, respectively. Mentioned bottlenecks are investigated and varied scenarios are created for recruitment of manufacturing system. Abstract:
In the recent years, urban green areas decrease dramatically as a result of increasing industrialization and population growth. At the same time, green spaces provide many ecosystem services such as controls of air pollution, noise reduction, prevents flooding and reduces the stress in the urban areas. Therefore, the plants help to these areas to get more livable and active, and also plants are one of the most significant identity elements in these open spaces. Roof gardens comes significant design comprehension as a result of global warming and also they contribute to cities with regard to ecological, economic, visual and recreational aspects. This study is mainly based on evaluation potential of green roofs and xeriscape planting design approach of Artvin (Turkey) known that generally has a remarkable floristic richness. Artvin is located on a sloping terrain, and the amount of green spaces that can be used is very limited in this city. Therefore, green roofs approach should be evaluated to supply urban green space sustainability. This study shows that it is appropriate about 20 perennial plants for green roofs and xeriscape planting design in Artvin city center. Abstract:
Background: A significant number of people with obesity that seek weight loss treatments experience binge eating episodes. Nonetheless, it is unknown whether binge eating episodes can hinder weight loss outcomes. Objective: To compare weight change in people with or without binge eating submitted to bariatric surgery, pharmacotherapy, nutritional orientation, and/or psychological therapies. Method: We conducted a systematic review with meta-analyses by searching studies in PubMed, American Psychological Association (APA), and Embase. Results: Thirty-four studies were included in our systematic review, and 17 studies were included in the meta-analyses. Overall, we found no significant difference in weight loss between people with or without binge eating submitted to any type of weight loss treatment. Additionally, we found no statistically significant differences in body weight between people with or without binge eating at short and long follow-up assessments following any type of weight loss treatment. We also examined changes in body weight in people with or without binge eating in three additional meta-analyses categorized by the type of weight loss treatment (i.e., behavioural and/or nutritional interventions; bariatric surgery; pharmacotherapy isolated or combined with behavior interventions) and found no difference in weight loss. Eleven out of the 17 studies that were assessed qualitatively (i.e., not included in meta-analyses) did not show differences in weight loss in people with or without binge eating submitted to any type of weight loss treatment. Conclusion: This systematic review with meta-analyses showed no difference in weight loss in people with or without binge eating submitted to a variety of weight loss treatments. Nonetheless, specialized therapies can be required to address eating disorder psychopathology and recurrent binge eating in people with obesity that seek weight loss.Keywords: obesity, binge eating, weight loss, systematic review, meta-analysis

Abstract:
Vertical natural gardens encountered in urban ecosystems are important elements contributing to urban ecology by raising the quality of urban life. This research covers the investigation of the semi-natural plant walls of Bartın city which is located on the western Black Sea coast of Turkey. Landscape analysis and evaluation as a result of land and office work have resulted in vertical garden ecosystems that have been processed in the urban habitat map, mostly in natural stone walls, wooden garden fences, garden entrance doors, historical buildings and building walls. Structural surfaces on old building facades, especially with abandoned or still in use with natural stone walls, have been found to have many natural vertical gardens over time. Parietaria judaica, Cymbalaria longipes and Hedera helix species were dominant, and other types of content were recorded, providing information on the current biotope potential, human activities and effects on them. It has been emphasized that the described vertical gardens together with the species they contain should be protected in terms of Bartin urban ecology and biodiversity. Abstract:
Biotopes such as forest areas rich in biodiversity, wetlands, hedgerows and woodlands play important ecological roles in agricultural landscapes. Of these semi-natural areas and features, hedgerows are the most common landscape elements. Their most significant features are that they serve as a barrier between the agricultural lands, serve as shelter, add aesthetical value to the landscape and contribute significantly to the wildlife and biodiversity. Hedgerows surrounding agricultural landscapes also provide an important habitat for pollinators which are important for agricultural production. This study looks into the identification of hedgerows in agricultural lands in the Mugada rural area within Bartın province, Turkey. From field data and-and satellite images, it is clear that in this area, especially around rural settlements, large forest areas have been cleared for settlement and agriculture. A network of hedgerows is also apparent, which might potentially play an important role in the otherwise open agricultural landscape. We found that these hedgerows serve as an ecological and biological corridor, linking forest ecosystems. Forest patches of different sizes and creating a habitat network across the landscape. Some examples of this will be presented. The overall conclusion from the study is that ecologically, biologically and aesthetically important hedge biotopes should be maintained in the long term in agricultural landscapes such as this. Some suggestions are given for how they could be managed sustainably into the future.Keywords: agricultural biotopes, Hedgerows, landscape ecology, Turkey

Abstract:
This research proposal includes the defining and evaluation of the semi-natural and cultural ecosystems at Bartın University main campus in Turkey in terms of landscape mosaics. The ecosystem mosaic of the main campus was divided into zones based on ecological classification technique. Based on the results from the study, it was found that 6 different ecosystem mosaics should be used as a base in the planning and design of the existing and future landscape planning of Kutlubeyyazıcılar campus. The first landscape zone involves the 'social areas'. These areas include yards, dining areas, recreational areas and lawn areas. The second landscape zone is 'main vehicle and pedestrian areas'. These areas include vehicle access to the campus landscape, moving in the campus with vehicles, parking and pedestrian walk ways. The third zone is 'landscape areas with high visual landscape quality'. These areas will be the places where attractive structural and plant landscape elements will be used. Fourth zone will be 'landscapes of building borders and their surroundings.' The fifth and important zone that should be survived in the future is 'Actual semi-natural forest and bush areas'. And the last zone is 'water landscape' which brings ecological value to landscape areas. While determining the most convenient areas in the planning and design of the campus, these landscape mosaics should be taken into consideration. Nature changes as a result of human necessities constantly. This change mostly feels in natural water sources which are reconstructed with an effect of dams and motorways. On the other hand, dams and motorways demolish and re-shape nature while the visual quality of landscape gets a new character. Changing and specialization new landscapes will be very important to protection-usage balance to explore sustainable usage facilities. The main cause of the selection of Artvin city is, it has very important geographical location and one of the most attraction points in the World with its biodiversity, conservation areas and natural landscape characteristics. Coruh River is one of the most significant landscape identity element of Artvin. This river begins with Erzurum and falls into the Black Sea in Batumi in Georgia, many dams, and hydroelectric station are located during this basin. Borcka, Muratli and Deriner dams have already been built. Moreover, Deriner is 6th highest dams all over the world. As a result of dams, motorways route were re-shaped and the ways which have already changed because of elevation is directly affected several of natural destruction. In contrast, many different reservoirs in Coruh Basin provide new vista point that has high visual quality. In this study, we would like to evaluate with sustainable landscape design in 76 km river corridor, which is mainly based on Deriner, Borcka and Muratli Dams and determination of their basin-lakes recreational potential and opportunities. Abstract:
The Gezi Park protests of 2013 have significantly changed the Turkish agenda and its effects have been felt historically. The protests, which rapidly spread throughout the country, were triggered by the proposal to recreate the Ottoman Army Barracks to function as a shopping mall on Gezi Park located in Istanbul’s Taksim neighbourhood despite the oppositions of several NGOs and when trees were cut in the park for this purpose. Once the news that construction vehicles entered the park on May 27 spread on social media, activists moved into the park to stop the demolition, against whom the police used disproportioned force. With this police intervention and the then prime-minister Tayyip Erdoğan's insistent statements about the construction plans, the protests turned into anti-government demonstrations, which then spread to the rest of the country, mainly in big cities like Ankara and Izmir. According to the Ministry of Internal Affairs’ June 23rd reports, 2.5 million people joined the demonstrations in 79 provinces, that is all of them, except for the provinces of Bayburt and Bingöl, while even more people shared their opinions via social networks. As a result of these events, 8 civilians and 2 security personnel lost their lives, namely police chief Mustafa Sarı, police officer Ahmet Küçükdağ, citizens Mehmet Ayvalıtaş, Abdullah Cömert, Ethem Sarısülük, Ali İsmail Korkmaz, Ahmet Atakan, Berkin Elvan, Burak Can Karamanoğlu, Mehmet İstif, and Elif Çermik, and 8163 more were injured. Besides being a turning point in Turkish history, the Gezi Park protests also had broad repercussions in both in Turkish and in global media, which focused on Turkey throughout the events. Our study conducts content analysis of three Turkish reporting newspapers with varying ideological standpoints, Hürriyet, Cumhuriyet ve Yeni Şafak, in order to reveal their basic approach to columns casting in context of the Gezi Park protests. Columns content relating to the Gezi protests were treated and analysed for this purpose. The aim of this study is to understand the social effects of the Gezi Park protests through media samples with varying political attitudes towards news casting.Keywords: Gezi Park, media, news casting, columns

Abstract:
The aim of the presented study was to develop a newly motorized spectroscopy system. Our system is composed of probe and motor parts. The probe part consists of bioimpedance and fiber optic components that include two platinum wires (each 25 micrometer in diameter) and two fiber cables (each 50 micrometers in diameter) respectively. Probe was examined on tissue phantom (polystyrene microspheres with different diameters). In the bioimpedance part of the probe current was transferred to the phantom and conductivity information was obtained. Adjacent two fiber cables were used in the fiber optic part of the system. Light was transferred to the phantom by fiber that was connected to the light source and backscattered light was collected with the other adjacent fiber for analysis. It is known that the nucleus expands and the nucleus-cytoplasm ratio increases during the cancer progression in the cell and this situation is one of the most important criteria for evaluating the tissue for pathologists. The sensitivity of the probe to particle (nucleus) size in phantom was tested during the study. Spectroscopic data obtained from our system on phantom was evaluated by multivariate statistical analysis. Thus the information about the particle size in the phantom was obtained. Bioimpedance and fiber optic experiments results which were obtained from polystyrene microspheres showed that the impedance value and the oscillation amplitude were increasing while the size of particle was enlarging. These results were compatible with the previous studies. In order to motorize the system within the motor part, three driver electronic circuits were designed primarily. In this part, supply capacitors were placed symmetrically near to the supply inputs which were used for balancing the oscillation. Female capacitors were connected to the control pin. Optic and mechanic switches were made. Drivers were structurally designed as they could command highly calibrated motors. It was considered important to keep the drivers’ dimension as small as we could (4.4x4.4x1.4 cm). Then three miniature step motors were connected to each other along with three drivers. Since spectroscopic techniques are quantitative methods, they yield more objective results than traditional ones. In the future part of this study, it is planning to get spectroscopic data that have optic and impedance information from the cell culture which is normal, low metastatic and high metastatic breast cancer. In case of getting high sensitivity in differentiated cells, it might be possible to scan large surface tissue areas in a short time with small steps. By means of motorize feature of the system, any region of the tissue will not be missed, in this manner we are going to be able to diagnose cancerous parts of the tissue meticulously.
