4 Spectral Dual Layer CT for Choledocholithiasis: A Blinded Comparison Study

Abstract:
Introduction: Objective: To evaluate the effectiveness of Spectral Dual Layer CT (DECT) in diagnosing choledocholithiasis, specifically focusing on its accuracy in detecting small biliary stones compared to other imaging modalities. Background: DECT has shown promise in improving the detection and characterization of gallstones in the common bile duct, offering potential advantages over traditional imaging methods like standard CT and ultrasonography. Methodology: Study Design: Single-blinded retrospective study conducted at a teaching hospital. Patient Selection: Reviewed records of patients who underwent DECT for suspected choledocholithiasis and had follow-up MRCP, ERCP, or IOC within 8 weeks. 23 patients with proven choledocholithiasis and 23 controls without biliary filling defects were included. DECT Protocol: Used a Philips IQ 256-slice dual-energy CT scanner with standard protocols including 120 kVp and 40 keV mono-E images. Assessment: Four radiologists, blinded to the study question, evaluated images for the presence of choledocholithiasis. Sensitivity, specificity, PPV, and NPV were calculated based on consensus diagnoses. Results: Diagnostic Performance: DECT showed an overall sensitivity of 47.8% and specificity of 78.3% for detecting choledocholithiasis. The accuracy of the diagnosis ranged from 54% to 63% among observers. Stone Detection: Of the identified stones, 6 were calcified and 17 non-calcified. Detection of calcified stones was more accurate (83.3%) compared to non-calcified stones (35.3%). Differences in signal between stones and bile were noted in certain imaging parameters. Interobserver Agreement: The agreement among radiologists was fair, with a Fleiss Kappa coefficient of 0.30. Conclusion: This is the first study to specifically analyse the performance of spectral CT in choledocholithiasis detection using a control group and blinding of reviewers. Our modest results demonstrating lower overall sensitivity than had been reported previously at 47.8% overall while identifying 40% of non-calcified stones <9 mm. We believe further research and development on advancements in spectral CT technology or newer techniques such as photon counting CT is warranted.Keywords: dual energy CT, choledocholithiasis, gallstones, body imaging

Abstract:
Obesity is the most prevalent global problem that continues to rise at alarming rates both in the industrialized and developing countries. Adipose tissue is an endocrine tissue that secretes numerous chemical signals, hormones, lipids, cytokines and coagulation factors as well as prompting insulin resistance which is a primary contributor to Type II Diabetes- one of its most common adverse effects on health. Other hormones whose levels are linked to obesity and nutritional state are leptin, IGF-1, and adiponectin. Several studies indicate that obesity is the leading cause of high levels of cholesterol that leads to fatty liver disease, gallstones, hypertension, increased risk for cancer and degenerative joint disease that primarily affects the weight bearing joints of the lower extremities. The activation of inflammatory pathways promotes synovial pathology that results in accelerated degeneration of the joints. The study examines the prevalence of obesity in the US female population in comparison to that of the developing world and its emergence as a significant and potentially modifiable risk factor in degenerative disease of the hip and knee joints that has resulted in staggering healthcare cost. Studies have shown that as the prevalence of obesity rises, we continue to see a rise in degenerative joint disease. The percentage of arthritis cases linked directly to obesity has risen from 3 percent in 1971 to 18 percent in 2002. A person with obesity is around 60 percent more likely to develop arthritis than someone of normal body weight. In women, obesity is associated with increased mortality from breast, cervical, endometrial and ovarian cancer that may accompany debilitating joint diseases and restricted mobility.Keywords: obesity, endocrine, degenerative, mortality, joint diseases, cancer, debilitating, mobility

Abstract:
Aim of Study: The purpose of the study was to screen for diabetes through HbA1c in patients with chronic pancreatitis (CP) within the Belfast Trust. Background: Patients with chronic pancreatitis are at risk of developing diabetes, earlier diagnosis with subsequent multi-disciplinary input has the potential to improve clinical outcomes. Methods: Clinical and laboratory data of patients with chronic pancreatitis were obtained through the Northern Ireland Electronic Healthcare Record (NIECR), specialist hepatobiliary, and gastrointestinal clinics. Patients were invited to have a blood test for HbA1c. Newly diagnosed patients with diabetes were then invited to attend a dedicated Belfast City Hospital (BCH) specialist chronic pancreatitis and diabetes clinic for follow up. Results: A total of 89 chronic pancreatitis patients were identified; Male54; Female:35, mean age 52 years, range 12-90 years. Aetiology of CP included alcohol 52/89 (58%), gallstones 18/89 (20%), idiopathic 10/89 11%, 2 were genetic, 1: post ECRP, 1: IgG autoimmune, 1: medication induced, 1: lipoprotein lipase deficiency 1: mumps, 1: IVDU and 1: pancreatic divisum. No patients had pancreatic carcinoma. Mean duration of CP was nine years, range 3-30 years. 15/89 (16%) of patients underwent previous pancreatic surgery/resections. Recent mean BMI was 25.1 range 14-40 kg/m². 62/89 (70%) patients had HbA1c performed. Mean HbA1c was 42 mmol/mol, range 27-97mmol/mol, 42/62 (68%) had normal HbA1c (< 42 mmol/mol) 13/62 (21%) had pre-diabetes (42-47mmol/mol) and 7/62 (11%) had diabetes (≥ 48 mmol/mol). Conclusions: Of those that participated in the screening program around one-third of patients with CP had glycaemic control in the pre and diabetic range. Potential opportunities for improving screening rates for diabetes in this cohort could include regular yearly testing at gastrointestinal and hepatobiliary clinics.Keywords: pancreatogenic diabetes, screening, chronic pancreatitis, trust experience

Abstract:
In both developed and developing countries, obesity among women is increasing, but in different patterns and at very different speeds. It may have a negative effect on health, leading to reduced life expectancy and/or increased health problems. This research studied the age distribution among obese women, the types of overweight and obesity, and the extent of the problem of overweight/obesity and the obesity etiological factors among women in Hillah city in central Iraq. A total of 322 overweight and obese women were included in the study, those women were randomly selected. The Body Mass Index was used as indicator for overweight/ obesity. The incidence of overweight/obesity among age groups were estimated, the etiology factors included genetic, environmental, genetic/environmental and endocrine disease. The overweight and obese women were screened for incidence of infection and/or diseases. The study found that the prevalence of 322 overweight and obese women in Hillah city in central Iraq was 19.25% and 80.78%, respectively. The obese women types were recorded based on BMI and WHO classification as class-1 obesity (29.81%), class-2 obesity (24.22%) and class-3 obesity (26.70%), the result was discrepancy non-significant, P value < 0.05. The incidence of overweight in women was high among those aged 20-29 years (90.32%), 6.45% aged 30-39 years old and 3.22% among ≥ 60 years old, while the incidence of obesity was 20.38% for those in the age group 20-29 years, 17.30% were 30-39 years, 23.84% were 40-49 years, 16.92% were 50-59 years group and 21.53% were ≥ 60 years age group. These results confirm that the age can be considered as a significant factor for obesity types (P value < 0.0001). The result also showed that the both genetic factors and environmental factors were responsible for incidents of overweight or obesity (84.78%) p value < 0.0001. The results also recorded cases of different repeated infections (skin infection, recurrent UTI and influenza), cancer, gallstones, high blood pressure, type 2 diabetes, and infertility. Weight stigma and bias generally refers to negative attitudes; Obesity can affect quality of life, and the results of this study recorded depression among overweight or obese women. This can lead to sexual problems, shame and guilt, social isolation and reduced work performance. Overweight and Obesity are real problems among women of all age groups and is associated with the risk of diseases and infection and negatively affects quality of life. This result warrants further studies into the prevalence of obesity among women in Hillah City in central Iraq and the immune response of obese women.Keywords: obesity, overweight, Iraq, body mass index
