3 Supply Side Barriers to Maternal Health Care Utilization in District Gwadar, Balochistan

Abstract:
Pakistan has the highest rates of maternal mortality in South Asia. From the year 2000 to 2017 the global rate of maternal mortality has decreased up to 39 %. In the context of South Asia, it has decreased by 59% since 2000s. Pakistan has also reduced the rate of maternal mortality, but there is a difference on the provincial level. According to the report of the National Institute of Population Studies (NIPS) conducted in 2020, the MMR in Balochistan has crossed the ratio of most of the South Asian countries, i.e., 298 maternal deaths per 100,000 live births. In comparison, the province of Punjab has the lowest maternal mortality rate i.e. 157 deaths (per 100,000 live births). The rate of maternal mortality is much higher in Balochistan as compared to the other provinces. This research is aimed to discuss the supply side barriers and utilization of maternal healthcare services in the District Gwadar. Likert scale survey method has been used to collect data from the Healthcare Professionals from hospitals -private and government- and the maternal healthcare receiver, that is patient. Semi-structured interviews of healthcare professionals such as doctors, nurses, and Lab technicians have also been conducted. It has been found in this research study that the hospitals in Gwadar district are lagging behind in providing modern maternal healthcare to women due to the lack of staff training, medicine supply, and Laboratories. Moreover, the system of the lady health worker is also not catering to the needs of the women in District Gwadar. It has been recommended in the study that first of all the government should fulfill the supply of the medicine in the hospital. Secondly, the government should open laboratories in the hospitals. Thirdly, the government should increase the funding of the government hospital and the allocation of lady health workers in District Gwadar, Balochistan should be increased.Keywords: maternal mortality, neonatal, postnatal, supply barriers, patients, healthcare professionals, laboratory, medical supply, training

Abstract:
China’s port facility construction projects in the Indian Ocean (IO) region, Gwadar Port and Djibouti Port projects in particular, have led to a heated debate among both Chinese and Western strategists over whether the country has literally been carrying out its “string of pearls” strategy, an alleged Chinese plan to challenge America’s military predominance in South Asia. Even though the Chinese government repeatedly denied the existence of such a strategy and highlighted the civilian/commercial nature of its port projects, it has significantly enhanced its strategic cooperation with littoral countries in the IO region since the “One Belt One Road” initiative was introduced by Chinese President Xi Jinping in 2013. Whether China does have a plan to expand its sphere of military influence westward concerns the balance of power in the IO region. If the answer is positive, the security environment there will be changed drastically. This paper argues that rather than simply copying the U.S. model of developing overseas military bases along the IO periphery, Beijing has been deliberating a more sophisticated plan for its physical presence there: creating a new set of “overseas strategic pivots.” These “pivots,” semi-military and semi-commercial in nature, are designed to help Beijing sustain its anti-piracy operations in the Gulf of Aden and serve as forward stations for the transportation of China’s imported energy and merchandise. They can support the Chinese Navy’s operations overseas but are not supposed to undertake face-to-face combat missions. This upgraded Chinese scheme can be identified as “string of pearls” strategy 2.0. Moreover, it is expected to help China deepen its roots in the IO region, implying that Beijing has to a large extent scratched its old diplomatic philosophy which highlighted the merits of non-interference and nonalignment. While a full-scale maritime confrontation between China and the U.S.-India security alliance is unlikely to be witnessed in the near future, an ambitious Chinese plan to step into the global maritime domain has been evidently shown.Keywords: Chinese navy, Djibouti, Gwadar, Indian Ocean, string of pearls strategy

Abstract:
On 22nd May 2013 Chinese Premier Li Keqiang on his visit to Pakistan tabled a proposal for connecting Kashgar in China’s Xinjiang Uygur Autonomous Region with the south-western Pakistani seaport of Gwadar via the China Pakistan Economic Corridor (hereinafter referred to as CPEC). The project, popularly termed as 'One Belt One Road' will encompass within it a connectivity component including a 3000-kilometre road, railways and oil pipeline from Kashgar to Gwadar port along with an international airport and a deep sea port. Superficially, this may look like a 'game changer' for Pakistan and other countries of South Asia but this article by doctrinal method of research will unearth some serious flaws in it, which may change the entire economic system of this region heavily affecting the socio-economic conditions of South Asia, further complicating the complete geopolitical situation of the region disturbing the world economic stability. The paper besets with a logical analyzation of the socio-economic issues arising out of this project with an emphasis on its impact on the Pakistani and Indian economy due to Chinese dominance, serious tension in international relations, security issues, arms race, political and provincial concerns. The research paper further aims to study the impact of huge burden of loan given by China towards this project where Pakistan already suffers from persistent debts in the face of declining foreign currency reserves along with that the sovereignty of Pakistan will also be at stake as the entire economy of the country will be held hostage by China. The author compares this situation with the fallout from projects in Sri Lanka, Tajikistan, and several countries of Africa, all of which are now facing huge debt risks brought by Chinese investments. The entire economic balance will be muddled by the increment in Pakistan’s demand of raw materials resulting to the import of the same from China, which will lead to exorbitant price-hike and limited availability. CPEC will also create Chinese dominance over the international movement of goods that will take place between the Atlantic and the Pacific oceans and hence jeopardising the entire economic balance of South Asia along with Middle Eastern countries like Dubai. Moreover, the paper also analyses the impact of CPEC in the context of international unrest and arms race between Pakistan and India as well as India and China due to border disputes and Chinese surveillance. The paper also examines the global change in economic dynamics in international trade that CPEC will create in the light of U.S.-China relationship. The article thus reflects the grave consequences of CPEC on the international economy, security and bilateral relations, which surpasses the positive impacts of it. The author lastly suggests for more transparency and proper diplomatic planning in the execution of this mega project, which can be a cause of economic complexity in international trade in near future.Keywords: China, CPEC, international trade, Pakistan
