4 Factors Associated with Fatal and Non-Fatal Accidents of Commercial Aviation Fixed-Wing Aircraft in Indonesia (2007-2018)

Abstract:

Background: Even though safety is a priority in Commercial Aviation (CA) operations, fatal fixed-wing aircraft accidents still occur frequently in Indonesia. Objective: This research aims to determine factors associated with fatal and non-fatal CA fixed-wing aircraft accidents in Indonesia. Methods: The research used a cross-sectional design, which was carried out in July 2023. It included all final reports on fixed-wing aircraft accidents published by the Indonesian National Transportation Safety Committee (KNKT). Analysis was conducted using chi-square and Fisher’s exact test methods using IBM SPSS software version 29.0. Results: Out of 52 final reports, 25 were fatal. The study found that factors associated with a higher risk of fatal accidents are pilots in command with CPL, unpressurized aircraft, single-engine aircraft, aircraft with MTOW less than 5,700kg, accidents occurring at weekends, accidents occurring outside of airport premises, CFIT occurrences, and the cruise phase of flight. The factor associated with non-fatal accidents is the landing phase. Conclusion: Efforts such as enhancing pilot training and certification processes, implementing stricter safety regulations for small, unpressurized, single-engine aircraft, and increasing safety measures during weekends and specific phases of flight can reduce future fatal accidents.

3 Diagnosis of Intermittent High Vibration Peaks in Industrial Gas Turbine Using Advanced Vibrations Analysis

Abstract:

This paper provides a comprehensive study pertaining to diagnosis of intermittent high vibrations on an industrial gas turbine using detailed vibrations analysis, followed by its rectification. Engro Polymer & Chemicals Limited, a Chlor-Vinyl complex located in Pakistan has a captive combined cycle power plant having two 28 MW gas turbines (make Hitachi) & one 15 MW steam turbine. In 2018, the organization faced an issue of high vibrations on one of the gas turbines. These high vibration peaks appeared intermittently on both compressor’s drive end (DE) & turbine’s non-drive end (NDE) bearing. The amplitude of high vibration peaks was between 150-170% on the DE bearing & 200-300% on the NDE bearing from baseline values. In one of these episodes, the gas turbine got tripped on “High Vibrations Trip” logic actuated at 155µm. Limited instrumentation is available on the machine, which is monitored with GE Bently Nevada 3300 system having two proximity probes installed at Turbine NDE, Compressor DE &at Generator DE & NDE bearings. Machine’s transient ramp-up & steady state data was collected using ADRE SXP & DSPI 408. Since only 01 key phasor is installed at Turbine high speed shaft, a derived drive key phasor was configured in ADRE to obtain low speed shaft rpm required for data analysis. By analyzing the Bode plots, Shaft center line plot, Polar plot & orbit plots; rubbing was evident on Turbine’s NDE along with increased bearing clearance of Turbine’s NDE radial bearing. The subject bearing was then inspected & heavy deposition of carbonized coke was found on the labyrinth seals of bearing housing with clear rubbing marks on shaft & housing covering at 20-25 degrees on the inner radius of labyrinth seals. The collected coke sample was tested in laboratory & found to be the residue of lube oil in the bearing housing. After detailed inspection & cleaning of shaft journal area & bearing housing, new radial bearing was installed. Before assembling the bearing housing, cleaning of bearing cooling & sealing air lines was also carried out as inadequate flow of cooling & sealing air can accelerate coke formation in bearing housing. The machine was then taken back online & data was collected again using ADRE SXP & DSPI 408 for health analysis. The vibrations were found in acceptable zone as per ISO standard 7919-3 while all other parameters were also within vendor defined range. As a learning from subject case, revised operating & maintenance regime has also been proposed to enhance machine’s reliability.

2 Application of Cube IQ Software to Optimize Heterogeneous Packing Products in Logistics Cargo and Minimize Transportation Cost

Abstract:

XYZ company is one of the upstream chemical companies that produce chemical products such as NaOH, HCl, NaClO, VCM, EDC, and PVC for downstream companies. The products are shipped by land using trucks and sea lanes using ship mode. Especially for solid products such as flake caustic soda (F-NaOH) and PVC resin, the products are sold in loose bag packing and palletize packing (packed in pallet). The focus of this study is to increase the number of items that can be loaded in pallet packaging on the company's logistics vehicle. This is very difficult because on this packaging, the dimensions or size of the material to be loaded become larger and certainly much heavier than the loose bag packing. This factor causes the arrangement and handling of materials in the mode of transportation more difficult. In this case, it is difficult to load a different type of volume packing pallet dimension in one truck or container. By using the Cube-IQ software, it is hoped that the planning of stuffing activity material by pallet can become easier in optimizing the existing space with various possible combinations of possibilities. In addition, the output of this software can also be used as a reference for operators in the material handling include the order and orientation of materials contained in the truck or container. The more optimal contents of logistics cargo, then transportation costs can also be minimized.

1 Optimal Design of Wind Turbine Blades Equipped with Flaps

Abstract:

As a result of the significant growth of wind turbines in size, blade load control has become the main challenge for large wind turbines. Many advanced techniques have been investigated aiming at developing control devices to ease blade loading. Amongst them, trailing edge flaps have been proven as effective devices for load alleviation. The present study aims at investigating the potential benefits of flaps in enhancing the energy capture capabilities rather than blade load alleviation. A software tool is especially developed for the aerodynamic simulation of wind turbines utilising blades equipped with flaps. As part of the aerodynamic simulation of these wind turbines, the control system must be also simulated. The simulation of the control system is carried out via solving an optimisation problem which gives the best value for the controlling parameter at each wind turbine run condition. Developing a genetic algorithm optimisation tool which is especially designed for wind turbine blades and integrating it with the aerodynamic performance evaluator, a design optimisation tool for blades equipped with flaps is constructed. The design optimisation tool is employed to carry out design case studies. The results of design case studies on wind turbine AWT 27 reveal that, as expected, the location of flap is a key parameter influencing the amount of improvement in the power extraction. The best location for placing a flap is at about 70% of the blade span from the root of the blade. The size of the flap has also significant effect on the amount of enhancement in the average power. This effect, however, reduces dramatically as the size increases. For constant speed rotors, adding flaps without re-designing the topology of the blade can improve the power extraction capability as high as of about 5%. However, with re-designing the blade pretwist the overall improvement can be reached as high as 12%.
