194 Osteitis in the Diabetic Foot and the Risk Factor on the Population

Abstract:
Foot infections are responsible for a significant number of hospitalizations and amputations in diabetic patients. The objective of our study is to analyze and evaluate the management of diabetic foot in a surgical setting. A retrospective study was conducted based on a selected case of suspected diabetic foot infections of osteitis treated at the Mohamed Boudiaf hospital in Medea.The case was reiterated as a therapeutic charge, consisting of treating first the infection of the soft tissues, then the osteitis: biopsy after at least 15 days of cessation of antibiotic therapy. Successful treatment of osteitis was defined at the end of a follow-up period of complete wound healing, lack of bone resection/amputation surgery at the initial bone site during follow-up , Instead, biopsies are prescribed in the treatment of soft tissue infection. The mean duration of treatment for soft tissue infection was 2-3 weeks, the duration of the antibiotic-free window of therapy prior to bone biopsy was 2-4 weeks. This patient received medical management without surgical resection. The success rate for treating osteitis at one year was 73%, and healing at one year was 88%.It is often limited to a sausage of the foot at the cost of repeated amputations. The best management remains prevention, which necessarily involves setting up a specialized and adapted centre.Keywords: osteitis, antibiotic, biopsy, diabetic foot

Abstract:
Cooperative spectrum sensing is a crucial challenge in cognitive radio networks. Cooperative sensing can increase the reliability of spectrum hole detection, optimize sensing time and reduce delay in cooperative networks. In this paper, an efficient central capacity optimization algorithm is proposed to minimize cooperative sensing time in a homogenous sensor network using OR decision rule subject to the detection and false alarm probabilities constraints. Abstract:
Zinc-coated steel sheets have been joined with aluminum samples in an overlapping as well as in a butt-joint configuration. A bi-metal-wire composed from aluminum and steel was used for additional welding experiments. An advantage of the laser-assisted bi-metal-wire welding is that the welding process is simplified since the primary joint between aluminium and steel exists already and laser welding occurs only between similar materials. FEM-simulations of the process were chosen to determine the ideal dimensions with respect to the formability of the bi-metal-wire. Abstract:
The main issue in designing a wireless sensor network (WSN) is the finding of a proper routing protocol that complies with the several requirements of high reliability, short latency, scalability, low power consumption, and many others. This paper proposes a novel routing algorithm that complies with these design requirements. The new routing protocol divides the WSN into several sub-networks and each sub-network is divided into several clusters. This division is designed to reduce the number of radio transmission and hence decreases the power consumption. Despite the special features of bipartite networks, they are common in many systems. Real-world bipartite networks may show community structure, similar to what one can find in one-mode networks. However, the interpretation of the community structure in bipartite networks is different as compared to one-mode networks. In this manuscript, we compare a number of available methods that are frequently used to discover community structure of bipartite networks. These networks are categorized into two broad classes. One class is the methods that, first, transfer the network into a one-mode network, and then apply community detection algorithms. The other class is the algorithms that have been developed specifically for bipartite networks. According to 1404 forecasting documentation, among the most fundamental ways of Iran’s success in competition with other regional countries are innovations, efficiency enhancements and domestic productivity. Therefore, in this study, the energy consumption efficiency of Iran and the neighbor countries has been measured in the period between 2007-2012 considering the simultaneous economic activities, CO2 emission, and consumption of energy through data envelopment analysis of undesirable output. The results of the study indicated that the energy efficiency changes in both Iran and the average neighbor countries has been on a descending trend and Iran’s energy efficiency status is not desirable compared to the other countries in the region.Keywords: energy efficiency, environmental, undesirable output, data envelopment analysis

This paper was developed to investigate two types of sands from the Kingdom of Saudi Arabia (KSA) for potential use in the global metal casting industry. Four types of sands were selected for study, two of the sand systems investigated are natural sands from the KSA. The third sand sample is a heat processed synthetic sand and the last sample is commercially available US silica sand that is used as a control in the study. The purpose of this study is to define the durability of the four sand systems selected for foundry usage. Additionally, chemical analysis of the sand systems is presented before and after elevated temperature exposure. A depropanizer column of a particular refinery was redesigned in this work. That is, minimum reflux ratio, minimum number of trays, feed tray location and the hydraulic characteristics of the tower were calculated and compared with the actual values of the existing tower. To Design review of the tower, fundamental equations were used to develop software which its results were compared with two commercial software results. In each case PR EOS was used. Abstract:
Background: Inflammatory bowel disease (IBD) consists of two specific gastrointestinal disorders: ulcerative colitis (UC) and Crohn's disease (CD). Despite their distinct natures, these two diseases share many similar etiologic, clinical and pathological features, as a result, their accurate differential diagnosis may sometimes be difficult. Correct diagnosis is important because surgical treatment and long-term prognosis differ from UC and CD. Aim: This study aims to study the characteristic clinicopathological features which help in the differential diagnosis between UC and CD, and assess the disease activity in ulcerative colitis. Materials and methods: This study was carried out on 50 selected cases. The cases included 27 cases of UC and 23 cases of CD. All the cases were examined using H& E and immunohistochemically for bcl-2 expression. Results: Characteristic features of UC include: decrease in mucous content, irregular or villous surface, crypt distortion, and cryptitis, whereas the main cardinal histopathological features seen in CD were: epitheloid granuloma, transmural chronic inflammation, absence of mucin depletion, irregular surface, or crypt distortion. 3 cases of UC were found to be associated with dysplasia. UC mucosa contains fewer Bcl-2+ cells compared with CD mucosa. Conclusion: This study using multiple parameters such clinicopathological features and Bcl-2 expression as studied by immunohistochemical stain, helped to gain an accurate differentiation between UC and CD. Furthermore, this work spotted the light on the activity and different grades of UC which could be important for the prediction of relapse.Keywords: Crohn's disease, dysplasia, inflammatory bowel disease, ulcerative colitis

A qualitative method was adopted in this study using open, in-depth interviews with 19 media entrepreneurs and three managers of media firms. Abstract:
This study dealing with optimization the conditions affecting the formation of extracellular lignin- degrading enzymes to achieve maximal decolorization activity of Direct Violet dye by one fungal strain. In this study Aspergillus fumigates fungal strain used for production extracellular ligninolytic enzymes for removing Direct Violet dye under different conditions: culture medium, incubation period, pH and temperatures. The results indicted that the removal efficiency of A. fumigatus was enhanced by addition glucose and peptone to the culture medium. The addition of peptone and glucose was found to increase the decolorization activity of the fungal isolate from 51.38% to 93.74% after 4 days of incubation. The highest production of extracellular lignin degrading enzymes also recorded in Direct Violet dye medium supplemented with peptone and glucose. It was also found the decolorization activity of A. fumigatus was decreased gradually by increasing the incubation period up to 4 days. Also it was found that the fungal strain can grow and produce extracellular ligninolytic enzymes which accompanied by efficient removal of Direct Violet dye in a wide pH range of 4-8. The results also found that the maximal biosynthesis of ligninolytic enzymes which accompanied with maximal removal of Direct Violet dye was obtained at a temperature of 28C. This indicates that the different conditions of culture medium, incubation period, pH and temperatures are effective on dye decolorization on the fungal biomass and played a role in Direct Violet dye removal along with enzymatic activity of A. fumigatus.Keywords: A. fumigates, extracellular lignin- degrading enzymes, textile dye, dye removing

In this study, AC3 software was used to measure the effect of carbon and manganese percentage, dimensions and geometry of pieces, the type of the cooling fluid, temperature, and time on hardness and the content of 1030 steel phases. Global awareness on energy security and climate change has created much interest in assessing energy efficiency performance. A number of previous studies have contributed to evaluate energy efficiency performance using different analytical techniques among which data envelopment analysis (DEA) has recently received increasing attention. Most of DEA-related energy efficiency studies do not consider undesirable outputs such as CO2 emissions in their modeling framework, which may lead to biased energy efficiency values. Within a joint production frame work of desirable and undesirable outputs, in this paper we construct energy efficiency performance index for measuring energy efficiency performance by using environmental DEA model with CO2 emissions. This paper presents an approach which detect and isolate efficiently a fault in a system. This approach avoids false alarms, non-detections and delays in detecting faults. A study case have been proposed to show the importance of taking into consideration the uncertainties in the decision-making procedure and their effect on the degradation diagnostic performance and advantage of using Bond Graph (BG) for such degradation. The use of BG in the Linear Fractional Transformation (LFT) form allows generating robust Analytical Redundancy Relations (ARR’s), where the uncertain part of ARR’s is used to generate the residuals adaptive thresholds. The study case concerns an electromechanical system composed of a motor, a reducer and an external load. Investigation of fracture of wood components can prevent from catastrophic failures. Created fracture process zone (FPZ) in crack tip vicinity has important effect on failure of cracked composite materials. In this paper, a failure criterion for fracture investigation of cracked wood specimens under mixed mode I/II loading is presented. This criterion is based on maximum strain energy release rate and material nonlinearity in the vicinity of crack tip due to presence of microcracks. Verification of results with available experimental data proves the coincidence of the proposed criterion with the nature of fracture of wood. The purpose of this paper is to exploit compressed sensing (CS) method in order to model and compress the electrocardiogram (ECG) signals at a high compression ratio. In order to obtain a sparse representation of the ECG signals, first a suitable basis matrix with Gaussian kernels, which are shown to nicely fit the ECG signals, is constructed. Then the sparse model is extracted by applying some optimization technique. Finally, the CS theory is utilized to obtain a compressed version of the sparse signal. Reconstruction of the ECG signal from the compressed version is also done to prove the reliability of the algorithm. At this stage, a greedy optimization technique is used to reconstruct the ECG signal and the Mean Square Error (MSE) is calculated to evaluate the precision of the proposed compression method.Keywords: compressed sensing, ECG compression, Gaussian kernel, sparse representation

A tri-linear slip-shear strength law is assumed according to literature and codes provisions for developing a finite element (FE) model of a case study of a composite deck. The variation of the strength and ductility of the connection is implemented in the numerical model carrying out nonlinear analyses. The objective of this paper is to explore the effect of incorporating Nano-Silica with Silica-fume in ferrocement mortar to enhancing mechanical properties of it. One type of Nano silica with average diameter size 23nm and silica fume have been used with two percentage (1%, 2% Nano silica and 5%, 10% silica fume per weight of cement) and w/c with / without superplasticizer was been calculated by flow test method. Also three sand: cement ratios have been used (1.5, 2.0 and 2.5) with max. Aggregate size 0.6mm in this study for reference and other mixtures. Results reveal adding Nano silica with silica fume to ferrocement mortar enhances its physical and mechanical properties such as compressive strength and flexural strength. The SEM pictures and density with absorption ratio demonstrate that Nano silica with silica fume contributes to enhancement of mortar through yielding denser, more compact and uniform mixtures.Keywords: nano silica, ferrocement mortar, compresion strength, flexural strength

Abstract:
Computer-aided diagnosis (CAD) systems can play crucial roles in diagnosing crucial diseases such as breast cancer at the earliest. In this paper, a CAD system for the diagnosis of breast cancer was introduced and evaluated. This CAD system was developed by using spatio-temporal analysis of data on a set of consecutive thermographic images by employing wavelet transformation. By using this analysis, a very accurate machine learning model using random forest was obtained. The final results showed a promising accuracy of 91% in terms of the F1 measure indicator among 200 patients' sample data. Initiation and propagation of cracks may cause catastrophic failures in rotating disks, and hence determination of fracture parameter in rotating disks under the different working condition is very important issue. In this paper, a comprehensive study of stress intensity factors in rotating disks containing 3D semi-elliptical cracks under the different working condition is investigated. In this regard, after verification of modeling and analytical procedure, the effects of mechanical properties, rotational velocity, and orientation of cracks on Stress Intensity Factors (SIF) in rotating disks under centrifugal loading are investigated. Also, the effects of using composite patch in reduction of SIF in rotating disks are studied. Abstract:
Synchronization of neuronal firing is a hallmark of several neurological diseases. Recently, stimulation techniques have been developed which make it possible to desynchronize oscillatory neuronal activity in a mild and effective way, without suppressing the neurons' firing. As yet, these techniques are being used to establish demand-controlled deep brain stimulation (DBS) techniques for the therapy of movement disorders like severe Parkinson's disease or essential tremor. We here present a first conceptualization suggesting that the nucleus accumbens is a promising target for the standard, that is, permanent high-frequency, DBS in patients with severe and chronic obsessive-compulsive disorder (OCD). In addition, we explain how demand-controlled DBS techniques may be applied to the therapy of OCD in those cases that are refractory to behavioral therapies and pharmacological treatment.Keywords: stereotactic neurosurgery, deep brain stimulation, obsessive-compulsive disorder, phase resetting

Abstract:
Among the natural enemies that reduce populations of the tomato leaf miner studied in experimental plots of National High school of agriculture (ENSA, Algeria, 36° 40’ à 36° 43’ N.; 3° 08’ à 3° 12’ E.), larval ectoparasites. Three larval ectoparasites are reported in this study namely Necrinmus Sp. and two species of indeterminate Chalcidae (Chalcidae Sp. 1 and 2). These species have significantly reduced the effectives of Tuta absoluta. The results for the parasitism of eggs, larval instars and pupae of Tuta absoluta on the open field tomato in the experimental plots of ENSA show high levels of parasite eggs with 25%. With 94.7%, the first larval instar (L1) is the most parasites. The second instar (L2) undergoes the action of parasitoids least 60%. Instars L3 and L4 and pupae remain unharmed.Keywords: tuta absoluta, larval ectoparasites, tomato, ensa, Algeria

Abstract:
Utilising waste materials, such as fly ash (FA) and slag (S) stockpiled in landfills, has drawn the attention of researchers and engineers in the recent years. There is a great potential for usage of these wastes in ground improvement projects, especially where deep deposits of soft compressible soils exist. This paper investigates the changes in the strength development of a high water content soft soil stabilised with alkaline activated FA and S, termed as geopolymer binder, to use in deep soil mixing technology. The strength improvement and the changes in the microstructure of the mixtures have been studied. The results show that using FA and S-based geopolymers can increases the strength significantly. Furthermore, utilising FA and S in ground improvement projects, where large amounts of binders are required, can be a solution to the disposal of these wastes.Keywords: alkaline activation, fly ash, geopolymer, slag, strength development

Abstract:
The classification of low-resolution satellite images has been a worthwhile and fertile field that attracts plenty of researchers due to its importance in monitoring geographical areas. It could be used for several purposes such as disaster management, military surveillance, agricultural monitoring. The main objective of this work is to classify efficiently and accurately low-resolution satellite images by using novel technics of deep learning and reinforcement learning. The images include roads, residential areas, industrial areas, rivers, sea lakes, and vegetation. To achieve that goal, we carried out experiments on the sentinel-2 images considering both high accuracy and efficiency classification. Our proposed model achieved a 91% accuracy on the testing dataset besides a good classification for land cover. Focus on the parameter precision; we have obtained 93% for the river, 92% for residential, 97% for residential, 96% for the forest, 87% for annual crop, 84% for herbaceous vegetation, 85% for pasture, 78% highway and 100% for Sea Lake.Keywords: classification, deep learning, reinforcement learning, satellite imagery
